Abstract
一种肠内吸收组合物,含有蛋白源、脂源、碳水化合物源、和纤维混合物。所述纤维混合物包含菊粉和低聚果糖,可溶性纤维占纤维混合物重量的45-55%,而不溶性纤维也占45%-55%。所述纤维混合物还可含豌豆内纤维和豌豆外皮纤维。
Description
本申请是申请日为1999年12月15日的中国专利申请99816081.4(国际申请号为PCT/EP99/10076的PCT国际申请进入中国国家阶段)的分案申请。
技术领域
本发明涉及肠内吸收组合物用的纤维混合物,并涉及含该纤维混合物的肠内吸收组合物。
背景技术
食用纤维应为日常摄取食物之一部分这一观点,已为人们普遍接受。例如Pilch S.M.(1987,《美国实验生物学会MD联合会志》223,84,2059)推荐健康人日常食物摄取的食用纤维量应为27-40g范围。
食用纤维可按其特征、化学和物理结构、其胃肠道内的可消化性、或其在胃肠道中的生理特征等来分类。
从化学上看,食用纤维被认为含多糖或木质素。在胃肠道中这些化合物不能由体内分泌物水解(T.Schweizer等人,1991,《实验》,44,p182-186)。所述食用纤维的构成物多糖可以是植物膜多糖。具体如纤维素、半纤维素或果胶,或者其它不能被消化酶水解的胞内多糖,例如稳定淀粉,半乳甘露聚糖或菊粉(Quemener等人,1994,Lebensm.Wiss.u.Technol.,27,p125-132)。
一般来说,根据其生物和物理化学特性,食用纤维可分为两类,即不溶性纤维和可溶性纤维。
不溶性纤维,例如纤维素、玉米纤维、或不溶性大豆纤维,在胃肠道中主要起机械性作用。一般它们只由肠内菌丛很轻微发酵,由此缩短其在肠道中停留时间(Scheppach等人,1990,JPEN,14,p202-209)。
可溶性纤维,例如果胶或稳定淀粉,是肠内菌丛的很好发酵底物。发酵结果释放出脂肪酸,特别是结肠内的短链脂肪酸。这有降低结肠内pH值的效果,对结肠内的病原菌有抑制其生长和繁殖的作用。
总的来说,工业社会中的大部分人并未消耗足够的食用纤维。但在临床处置中,这个问题会变的严重,例如对患者施用不含食用纤维的肠内吸收组合物,通常会引起肠道疾患,例如腹泻或便秘(Palacio等人,1990《临床实践中的营养》,5,p99-106)。因此,从临床营养学观点,有人建议在肠内吸收组合物中掺入食用纤维。例如EP0591267公开了肠内吸收组合物所用的纤维系统,包含以重量计5-50%阿拉伯胶、5-25%羧甲基纤维素钠、和45-80%燕麦皮纤维。此外,EP 0756828公开了维持良好肠道功能含食用纤维的肠内吸收组合物,该组合物为液态或干态,每2000kcal含15-50%可溶性食用纤维、15-45%不溶性食用纤维、和8-70%低聚糖或稳定淀粉。
但是大部分已知肠内吸收组合物并不含平衡量的可溶性及不溶性食用纤维。此外,含高比例可溶性纤维的组合物通常太粘而不能管饲。同时还出现稳定性问题。
发明内容
因此本发明提供肠内吸收组合物用的纤维混合物,该纤维混合物含有豌豆内纤维,豌豆外皮纤维、菊粉、和低聚果糖。
该纤维混合物提供具有良好机械特性、和良好营养及生物学特性的肠内吸收组合物。所述机械特性包括缩短胃肠道中的滞留时间,而所述营养和生物学特性,包括释放短链脂肪酸而保持肠粘膜中的细菌平衡。并阻止致病菌的生长繁衍。该纤维素混合物可以在整个胃肠道系统中起作用,即胃、小肠和结肠。此外,可以获得可溶性与不溶性纤维之间的平衡,而不使肠内吸收组合物变得太粘。
该纤维混合物以重量计含约20%-约50%豌豆内纤维、约20%-约50%豌豆外皮纤维、约5%-约30%菊粉、以及约10%-约40%低聚果糖。
本发明还提供含上面定义的纤维混合物的肠内吸收组合物,该组合物包含蛋白源、碳水化合物源,和脂源。
优选蛋白源提供约10%-约20%能量、脂源提供约30%-约50%能量,而碳水化合物源提供约35%-约55%能量。
另一方面,本发明提供含蛋白源、脂源、碳水化合物和纤维混合物源的肠内吸收组合物,所述纤维混合物包含菊粉和低聚果糖,且按混合的重量计可溶性纤维占45-55%而不溶性纤维也占45-55%。
另一方面,本发明提供含蛋白源、脂源、碳水化合物源和纤维混合物源的肠内吸收组合物,所述纤维混合物以重量计含有约20%-约40%菊粉、和约460%-约80%低聚果糖。
所述肠内吸收组合物为液态、或是在含水液体中可再构提供营养液态组合物的可溶性粉末。该肠内吸收组合物还可以是其它可肠内摄取的形式,例如甜食、谷物食品、小吃等、
具体实施方式
现通过实施例,仅对本发明优选方案加以描述。
本说明书中,所述“可溶性纤维”意指采用Prosky等人的方法(1998,《官方分析化学协会杂志》,70,5,1017)认为有溶解特征的食用纤维。所述方法系官方分析化学家协会的方法。所谓“不溶性纤维”意指采用Prosky等人的方法,认为有不溶性特征的食用纤维。
本发明提供含菊粉和低聚果糖的纤维混合的。该混合物有极佳促双岐菌效果(bifidogenic effect),并在结肠内产生短链脂肪酸。该纤维混合物含大致等量的可溶性和不溶性纤维。该纤维混合物还可以含豌豆内纤维和豌豆外皮纤维。
本发明书上下文中,所谓“豌豆内纤维”意指来自豌豆外包皮内或种皮里面的纤维,包括纤维素、半纤维素和果胶,例如以重量计含约15%纤维素、约45%半纤维素、及约40%果胶。该纤维的分配情况是其约66%重量属不溶性纤维。因此,以缩短在消化道中时间之方式,该豌豆内纤维对胃肠道施加机械作用。此外,豌豆内纤维成分被肠道菌丛发酵,释放短链脂肪酸,由此引起结肠内pH值降低,因而抑制结肠内致病菌的生长和繁衍。适宜的豌豆内纤维可商购。
脂肪酸的释放,对于服用抗生素治疗的患者尤为重要,因为抗生素治疗期间,肠内菌丛的完整性和功能均受到损害,可溶性纤维含量高的饮食可以缓解该使用。而且,短链脂肪酸的释放(例如丁酸)伴随钠离子吸收,引起对结肠水分吸收,这对腹泻有效果。同时,丁酸盐还是结肠细胞的高能量底物。
本说明书中,所谓“豌豆外皮纤维”意指来自豌豆外包皮(或种皮)的纤维,包括纤维、半纤维和木质素;例如以重量计含约68%纤维素、约25%半纤维、和约7%木质素。从该纤维的分配看,有约10%重量是不溶性纤维,而约90%重量为不溶性纤维。因此,豌豆外皮纤维通过以缩短消化道中滞留时间的方式,而对胃肠道施加机械作用,由此对保留肠中水分的能力有正面影响。适宜的豌豆外皮纤维可商购。
菊粉是很多植物中存在的可溶性纤维,例如竹笋、洋蓟、洋葱、小麦或菊苣等。菊粉在小肠中不消化,而在结肠中可发酵。菊粉纤维在消化系统中的主要作用是缩短肠道中滞留时间、降血糖、降血脂、降结肠中pH值、缓解便秘,以及促双岐菌作用。因此菊粉可由双岐杆菌发酵,由此提高该菌在肠道菌丛中的浓度,同时降低肠杆菌、特别是梭状芽胞杆菌的浓度。
菊粉可以以适宜人摄取的天然提取物的形式提供。菊苣提取物尤为适宜。该提取物优选至少含80%重量菊粉,更优选至少90%。该菊粉优选聚合度至少约8,例如约10-约25。适宜的菊粉提取物可购自比利时Tirlemont 3300的Orafti SA,商品名“Raftiline”。例如菊粉可以RaftilineST形式供给,它是白色细粉,含约90%-约94%重量菊粉,最多约4%重量葡萄糖和果糖,及约4%-9%蔗糖。菊粉的平均聚合度为约10-约12。
低聚果糖是可溶性纤维,是果糖的低聚物形式,含有1-蔗果三糖(GF2)、萘斯糖(nystose(GF3))、和1F-吡喃果糖基萘斯糖(GF4),其中果糖基单元(F)分别在蔗糖(GF)的β-2,1位键合。也可以含有一般含量的蔗糖和葡萄糖。低聚果糖可以通过菊粉水解获得,采用酶催化法或使用微生物。低聚果糖可以商购,例如从比利时Tirlemont3300的Orafti SA,以商品名“Raftilose”购得,或从日本的MeijiSeika公司购得。例如菊粉可以以RaftiloseP95之名购得。
据报导菊粉和低聚果糖能促进胃肠道中双岐杆菌生长,且在一定环境中防止或抑制梭状芽胞杆菌之类的致病菌生长。此外,据报导,促进双岐杆菌生长还有其它各种有益作用。而且菊粉和低聚糖可以降低血中葡萄糖含量。
该混合物对于在结肠中提供最佳促双岐菌作用及产生短链脂肪酸特别有利。已发现低聚果糖比菊粉有较大促双岐菌作用,但菊粉有延迟发酵作用,这样使之大部分在结肠中发酵。通过对菊粉和低聚果糖混合物进行挑选,在结肠中的促双岐菌作用及短链脂肪酸的产生均可达到最大值。
如果需要,该纤维混合物还可含其它低聚糖。适宜的例子如低聚半乳糖、低聚木糖、或淀粉低聚衍生物。
对纤维混合物中各成分含量加以优化选择,使之按重量计含约45%-约55%可溶性纤维,和约45%-约55%不溶性纤维。该比例可以供该两种类型纤维各自发挥其最佳效果。优选可溶性纤维和不溶性纤维重量比大致1∶1。
此外,对纤维混合物中各成分含量进行优化选择,使之按重量计含约20%-40%菊粉和约60%-约80%低聚果糖。例如该纤维混合物中可以含约30%重量菊粉和约70%重量低聚果糖。
按重量计,优选该纤维混合物中含约20%-50%豌豆内纤维、约20%-约50%豌豆外皮纤维、约5%-约30%菊粉、和约10%-约40%低聚果糖。例如该纤维混合物可以含约30%-约40%豌豆内纤维、约30%-约40%豌豆外皮纤维、约5%-约15%菊粉、和约20%-约30%低聚果糖。
该纤维混合物可以加入肠内吸收组合物中。该肠内吸收组合物可含约1%-约5%纤维混合物(以重量计),例如约1%-约2%。
优选所述肠内吸收组合物包括蛋白源、碳水化合物源、和脂源。
所述蛋白源优选高质量蛋白源,例如肉蛋白、乳清蛋白、酪蛋白、大豆蛋白、以及这些蛋白混合物。蛋白源可以是完整蛋白型,或者也可以是水解蛋白。其它蛋白源,例如豌豆、稻米、及燕麦蛋白或其混合物也可用。此外,如果需要,所述蛋白源可以包括游离氨基酸。
优选该蛋白源提供组合物约10%-约25%的能量。例如蛋白源提供约12%-约18%组合物的能量,优选约15%组合物能量。
优选碳水化合物源可以是任何适宜的碳水化合物或碳水化合物混合物。例如可以是麦芽糖糊精、改性淀粉、直链淀粉、木薯淀粉、玉米淀粉、或果糖或它们的混合物。如果要求低渗透性,则优选麦芽糖糊精。
碳水化合物提供组合物约35%-约60%能量,优选约45%-约55%能量,例如提供约50%组合物能量。
所述脂源优选含单不饱和脂肪酸、多不饱和脂肪酸(ω-3及ω-6脂肪酸)、和/或饱和脂肪酸。优选多不饱和脂肪酸提供最多约30%重量脂源,例如可以提供约15%-约25%重量脂源。所述肠内吸收组合物中脂的特征是经优化设计,使其多不饱和脂肪酸ω-6(n-6)与ω-3(n-3)之比为约4∶1至约10∶1。饱和脂肪酸优选提供约30%-约70%重量脂源,例如约50%-约65%重量。大部分饱和脂肪酸优选是中链甘油三酯型,例如中链甘油三酯可以占脂源重量的约20%-约70%。
适宜的脂源是橄榄油、玉米油、葵花油、菜油、谷物油、榛子油、红花油等。分馏椰油是适宜的中链甘油三酯源。可以使用玉米油、菜油、中链甘油三酯和大豆的混合物。
脂源可以提供约25%-约45%组合物能量。优选约30%-约45%,例如可以提供约35%组合物能量。
对于临床应用而言,优选该肠内吸收组合物包括完全的维生素和矿物质。例如所含维生素和矿物质足以使营养组合物中每1000卡路里维生素和矿物质达推荐每日允许量的约25%-约250%。
对于临床应用而言,该肠内吸收组合物渗透性宜为约200mOsm/l-约400mOsm/l,例如约250mOsm/l-约350mOsm/l。
对于临床应用而言,该肠内吸收组合物的能量密度优选约700kcal/l-约1500kcal/l,例如约1000kcal/l。
对于临床应用而言,该肠内吸收组合物优选配好待用型制剂。以该剂型,该组合物可以通过鼻饲管、空肠管喂给患者,或者患者自己饮下。这样该组合物可以配成各种形式,例如水果汁状饮料、乳状摇动型饮料等。但是,该组合物也可以是临使用时可重构的水溶性粉末。
可加入各种调味剂、甜味剂及其它添加剂。可以使用以乙酰磺胺和L-天冬氨酰为基础的人造甜味剂。
该肠内吸收组合物可按常规生产,例如,将蛋白源、碳水化合物源和脂源混合在一起。如果使用乳化剂,可在混合时加入。维生素和矿物质也可在混合时加入,但通常要稍晚些加,以避免热分解。混合之前,可将任何亲脂维生素、乳化剂等溶解在脂源中。然后将水(优选经过逆渗透的水)混合进去形成液态混合物。水温以约50℃至约80℃为宜,有助于成分的分散。可以采用市售液化剂形成液态混合物。
然后将该液体混合物热处理以杀灭细菌。例如可将溶液混合物快速加热至约80℃-约110℃约5秒钟-约5分钟,这可通过蒸汽注入,或热交换器进行,例如板式热交换器。
该液体混合物然后可冷却至约60℃-约85℃,例如用快速冷却法。然后将其匀化,例如分两步,第一步以约7MPa-约40MPa,第二步以约2MPa-约14MPa进行。然后进一步冷却该经匀化的组合物,再加入所有热敏感组份,例如维生素和矿物质。这时可将该经匀化的混合物的pH值及固体含量调节至一定标准。
为生产液体剂型,可将经匀化的混合物无菌充填入适宜容器中。容器中的无菌充填,可通过预热匀化的混合物(例如约75°-85℃),然后注入蒸汽进经匀化的混合物中升温至约140°-160℃进行。然后可将其快速冷却至约75°-85℃。并可将其进一步匀化,冷至室温再充填入容器中。进行无菌充填的适宜设备可以商购。
为生产粉末剂型,可将经匀化的混合物干燥成粉,例如喷雾干燥。可采用常规方法。
如果是临床补充营养用的液态形式时,通过管饲法(借助重力或泵抽)可容易地施用于患者。这种情况下,该肠内吸收组合物其室温下的粘度应小于12cp。
该肠内吸收组合物可作为人和动物患者的营养支持,特别是要求长期营养支持的患者。此外,该组合物也适宜于有正常消化功能的患者。
还应注意到该肠内吸收组合物还可以是临床营养物以外的其它形式,例如可以是甜点、谷类食品、酸奶、快餐食品等。如果喂食宠物,该组合物可以是干碎片、肉乳液,以及调配乳液产品等。
现以纤维混合物特定实施例进一步详细说明。
实施例1
将豌豆内纤维、豌豆外皮纤维、低聚果糖和菊粉,于约65-70℃在去离子水中混合在一起制备肠内吸收组合物。对各成分用量加以选择,使最终产品提供4g/l豌豆内纤维、4g/l豌豆外皮纤维、2.8g/l低聚果糖和1.2g/l菊粉。将该混合物搅拌5分钟,匀化,并搅拌下贮存。
将玉米油、菜油、大豆油和中链甘油三酯混合配制脂相,其量要使最终产品中的脂为39g/l。并加入约1.4g/l乳化剂、甘油硬脂酸酯。
然后将脂相与纤维混合物混合并匀化,将所得乳液冷至60℃,加入约38g/l蛋白混合物(含酪蛋白和大豆蛋白)、约125g/l碳水化合物和矿物质,然后加入维生素水溶液。再将pH值调节至7.1。
将该肠内吸收组合物于150℃处理6秒种、冷却、无菌填充入柔性容器中,于室温下贮存。

贮存1个月之后,测定该组合物粒度、粘度、质地和稳定性。此外,对其进行口味评估,其结果如下:

将实施例1肠内吸收组合物借助重力、通过肠道途径施用给患者,以有规流速将该组合物传送给患者。

1.一种肠内吸收组合物,含有蛋白源、脂源、碳水化合物源、和纤维混合物,所述纤维混合物包含菊粉和低聚果糖,其中可溶性纤维占纤维混合物重量45%-55%,而不溶性纤维占45%-55%。
2.一种肠内吸收组合物,含有蛋白源、脂源、碳水化合物源、和纤维混合物,所述纤维混合物含约20%-约40%重量菊粉和约60%-约80%重量低聚果糖。
