背景技术
随着乳房摄影术筛选程序的越来越流行,大多数乳腺癌被检测为小的、不易觉察的(或隐性的)病变,这些病变经得起保留乳房治疗的检验。不易觉察的乳腺癌的准确定位是允许以足够裕度对完整肿瘤的手术摘除的关键。如果肿瘤未被完全切除,则患者需要经历另一手术以摘除任何残余的癌组织。准确定位也有助于避免切除过量的乳腺组织,切除过量的乳腺组织可导致不利的美容效果。其它癌症要求准确定位,诸如结肠癌、前列腺癌和肺癌以及本领域的普通技术人员所知的其它病症。
在外科手术期间对于不易察觉的病变的定位的当前黄金标准为导丝定位(Wire-Guided Localization,WGL)。尽管广泛使用该技术,但是WGL具有多个缺点。第一,它涉及两个单独的过程,且可以在放射科室与外科室之间呈现运筹和调度困难。第二,导丝的定位对于在后续手术中实现预期的美容效果而言可能不是最佳的。第三,钩丝可以移动而远离病变的位点或在乳房摄影或移动患者期间变为被移位。第四,导丝的插入对于患者来说可能是痛苦的,以及最后,感染的风险意味着手术通常需要与导丝插入在同一天发生。
为了克服这些缺点,已开发了其它定位技术。一种这类技术为放射导向的隐性病变定位(Radioguided Occult Lesion Localization,ROLL),其使用注入到肿瘤中且由手持式伽玛探针检测的放射性示踪剂。尽管这消除了WGL的运筹复杂度,但是该技术引入使用放射性物质的缺点,放射性物质需要特殊处理和处置过程。
也使用磁标记,以及这些磁标记克服了通过使用放射性物质作为标记而出现的不便性和运筹挑战,并且这些磁标记还避免了导丝的缺点。然而,磁标记相比于导丝制造起来相对复杂。

对于磁性活检标记的另一挑战是为了实现有效的磁响应,材料的体积需要被最大化。该体积要求导致具有比其直径明显更大的长度的典型形状的标记。这类标记在1mm至12mm的区域中,其中长度与直径的比率大于5。该纵横比导致不均匀的磁响应,其中,当标记主轴线与探针共线时获得强很多的信号,以及当标记主轴线横切探针时获得较弱信号。通常期望更均匀的响应。
另外,标记通常被导向其位置且在超声波或立体定位的X射线成像下被确认就位。这意味着对于标记来说期望的是在X射线或超声波成像下(优选地在MRI下)清楚可见,这也可以用于该目的。
所需的是具有少量材料而不减小可检测信号的强度并且从相对于磁探针的任何方向提供更均匀的响应的标记。

发明内容

具体实施方式
在美国专利No.6,173,715中描述了一种用于制作诸如永磁体的磁标记的方法。永磁体植入物具有如下缺点,它们将吸引四周中的任何磁性材料且因此可以非预期地与其它植入物相互影响、或在组织中移动,例如,当由磁性材料制成的手术工具在附近时。
在美国公开文献No.2014/0314679中描述了一种用于制作由生物可吸收形式的超顺磁性氧化铁(SuperParamagnetic Iron Oxide,SPIO)纳米颗粒形成的标记的方法。SPIO粒子避免了对于永磁体标记发生的顾虑,因为这类材料不具有剩磁且自身无磁性,但仅在磁场存在时变得磁化。这些SPIO磁标记的检测和定位可以利用灵敏的磁强计(或磁化率计)来执行,该磁强计(或磁化率计)产生交变磁场以磁性地激励标记以及检测由该标记产生的磁场信号,如在美国公开文献No.2011/0133730中所公开的。
对于磁性活检标记的另一挑战是实现有效的磁响应,材料的体积需要被最大化。这些需求导致具有比直径明显更大的长度的典型标记,因为该标记必须通过针来传送且较小的针规(直径)对于患者来说不太疼痛。这类标记在1mm至12mm的区域中,其中长度与直径的比率大于5。实际上,这些尺寸约束限制磁响应且进而限制可利用磁探针定位标记的容易度。因此,期望更强的响应。
另外,标记通常被导向其位置且在超声波或立体定位的X射线成像下被确认就位。这意味着对于标记来说期望的是在X射线或超声波成像下(优选地在MRI下)清楚可见,MRI也可以用于该目的。SPIO粒子在X射线成像下具有受限的可见性,但是可以通过将这些SPIO粒子组合在回波材料(例如聚合物)的基质中而使其超声可见。尽管磁标记克服了导丝和放射性方法的缺点,但是仍需要这样的磁标记:其可通过窄针引入、提供强的磁响应、在X射线和超声波成像下可见、以及可以简单地来制造。
对于标记可磁性定位的需求要求来自植入材料的特定磁响应,以及对于该响应在远离材料的一定距离处是可测量的。该响应与磁化率参数有关。以SI为单位,磁化率为无量纲的比例常数,其表示响应于施加的磁场而在材料中感应的磁化程度且如下来定义:

其中,M为以安培/米测量的材料的磁化强度(磁偶极矩/单位体积),H为也以安培/米测量的磁场强度,以及χ为无量纲的比例常数,即磁化率。严格来讲,χ仅为用于顺磁性的或抗磁性的材料的常量,然而在磁滞效应很小的软磁或超顺磁材料中,且对于比使材料磁饱和所需的场小很多的施加场H,线性关系M=χH为充分逼近。
可以通过一系列的已知方法测量磁化率,包括法拉第(Faraday)磁秤、古伊(Gouy)磁秤、磁共振法、和利用SQUID磁力计的归纳法。也可以使用基于计算机的有限元磁建模包(诸如ANSYS Maxwell(ANSYS Inc.,Canonsburg,PA))、通过在均质磁场中对标记建模以及测量由该标记引起的对应于磁化率的失真,来计算磁化率。例如参看K.Bartusek等人在2011年3月20-23日于摩洛哥马拉喀什的期刊“Progress In Electromagnetics ResearchSymposium Proceedings”上发表的“Magnetic Susceptibility Modelling Using ANSYS”中所描述的方法。
对于给定材料,可以定义质量磁化率,其是每单位质量的材料的感应磁响应。质量磁化率,χρ=k/ρ,其中,ρ为材料的密度且χρ的单位为m3/kg。这是归一化的磁化率且允许不同材料的相对磁化率进行比较。例如,316不锈钢(一种用于活检标记的标准材料)的质量磁化率具有大约3.80×10-7至1.27×10-6的范围,永久性钕磁铁具有大约6.67×10-6的值,基于超顺磁性氧化铁(SPIO)的标记根据基质中的粒子的密度而具有大约2.5×10-5至1.0×10-3的值,而NiZn-铁氧体具有大约3×10-3至1.22×10-1的范围。因此,NiZn-铁氧体需要检测比SPIO少的材料,而SPIO需要比永久性钕磁性材料或316不锈钢少的材料。
对于在线性磁化率区域中经受场H的体积为V的软磁标记,在该标记上的总感应矩将为m=MV=χVH。该力矩将引起其自身的磁场Hmarker,该磁场Hmarker在远离标记的一定距离处可以近似为由于等效点偶极矩引起的磁场,即:
其中,r为单位向量。该感应场Hmarker是可测量的且使磁标记可定位。
然而,存在可以影响可定位给定标记的容易度、或换言之在给定r下影响感应场Hmarker的强度的其它因素。这些因素之一被称为‘去磁’。这是发生在尺寸有限的物体中的现象,其中,在主体的一部分中由于磁化引起的感应场起作用以使同一主体的另一部分去磁。在除了最简单几何结构以外的其它结构中预测该影响是非常复杂的,因此,如此处,对于为回转椭圆体(例如,球体、柱体或盘状物)的物体的特殊情况,最经常描述该影响。在这类情况下,在物体内的感应磁化是均匀的,且局部磁场通常被写为:

其中,Hdemag为‘去磁场’且N为“去磁因子”,但是更准确地,应当沿着椭圆体的x、y、z主轴将Hdemag定义为:
以SI为单位,Nx+Ny+Nz=1。对于球体的情况,Nx=Ny=Nz=1/3。对于长z轴柱体的情况下,Nx=Ny=1/2且Nz=0。对于薄xy平面盘状物的情况下,Nx=Ny=0且Nz=1。在由给定质量的给定单一材料制成的标记的情况下,该标记的形状将影响可定位该标记的容易度。
例如,如果该标记被制成球体,则它将经历Hlocal=H-1/3M,无论该标记对于施加场的取向如何。感应矩因此将为:
m=χV Hlocal=χVH-1/3χm或m=χVH/(1+1/3χ)
感应场Hmarker因此也将在任何给定r下按因数(1+1/3χ)减小,但是该减小将存在,无论该标记对于磁场H的取向如何。
相比之下:如果该标记被制成柱体,则如果H被定向为垂直于该柱体的长轴,则Hmarker将按因数(1+1/2χ)减小,而如果H被定向为沿着长轴,则Hmarker将丝毫不减小。在该情况下,比率ξ=(1+1/2χ)/1表示各向异性的定位信号,柱体将根据用于其信号强度的Hmarker的幅值而将该各向异性的定位信号呈现给任何方法。
在长度有限的柱体的情况下,该比率可以近似为:
其中,N⊥=Nx=Ny且N||=Nz=1-2N⊥。例如,在表2中所列的铁氧体不锈钢样本(对此,χ为大约140)的情况下,针对纵横比为7:1的柱体测量的ξ比率10.3:1对应于大约为0.48的N⊥;而在纵横比为3.7:1的柱体中的相同材料展现大约为4.7:1的各向异性比率,其对应于为大约0.44的N⊥。
因此,对于磁标记,磁响应部分地取决于其一种或多种构成材料的质量磁化率,且部分地取决于标记的形状,以及对于给定形状,该响应可以随着标记的取向而改变。磁响应的各向异性可以通过使用如上所述的去磁因子来计算。然而,由于针对真实形状非常难以计算去磁因子,因此需要更实际的方法来定义响应的各向异性。磁响应随着针对给定标记的取向的变化可以被看作该标记的“有效磁化率”的变化。标记材料的基础磁化率不改变,但是由于材料、形状和取向的组合而造成的磁响应的变化可以定义为犹如磁化率在改变。
因此,对于任何给定标记,可以定义最大(有效)磁化率与最小(有效)磁化率的比率。该比率可以被称为磁化率各向异性且给出沿着标记的不同轴或来自不同方向的磁化率响应的均匀性的指示。
磁化率各向异性=最大磁化率/最小磁化率
由于磁化率是无量纲的,因此作为两个无量纲的量的比率的磁化率各向异性也是无量纲的。如果各向异性为1,则磁化率从任何方向均相同。如果各向异性很高,则磁化率相对于标记的取向是非常不均匀的。
磁化率各向异性的概念在本领域中被描述且可以通过多种方法来测量。例如,在A.K.Dubey于期刊Springer Geology上发表的“Understanding an Orogenic Belt”中描述了两种类型的方向磁化率计:可以将标本插入线圈中的等阻抗电桥;以及将标本放在铁氧体环内部的平衡变压器系统。在美国专利No.3,492,566中描述了称为三维磁各向异性的磁化率计的另一方法。在每种情况下,将始终如一的磁场施加到样本,以及随着样本取向改变而测量感应磁响应的变化。在图2(C)中示出了另一这类方法,其中,使用磁化率量测术探针来测量感应磁响应。使用与在美国公开文献No.2011/0133730中所描述的磁化率计系统相类似的磁化率计系统,由各种材料且以各种形式生产的标记具有在固定距离处测量的最大信号和最小信号。在表3中示出了结果。
使用磁化率探针或磁化率计测量磁化率各向异性是定义标记的磁响应的均匀性的理想方法。然而,存在确定磁化率各向异性的水平的替选方法。例如,可以测量标记在任何方向上的投影面积以及确定最大投影面积和最小投影面积。对于给定体积的标记材料,较低的投影面积指示场的较大聚焦效应,反之亦然。由于该聚焦效应与投影面积成反比,因此响应的均匀性可以通过来自所有可用视点或方向的标记的最小投影面积与最大投影面积的比值来定义。这是磁标记的投影面积各向异性比,且提供磁化率各向异性的有用近似值。球形标记将具有等于1的最小投影面积与最大投影面积的比值。直径d=0.75且长度l=7.5的杆形标记将具有d×1/(pi×d2/4)=12.7的比值。
还可以看出,投影面积的该比值近似等于标记的形状因子,该形状因子被定义为标记的最大尺寸与其最小尺寸的比,以及这也可以被用作磁化率各向异性的近似值。这两种几何方法均未考虑标记内的磁性能的变化。
在全文使用术语‘磁化率各向异性’或磁各向异性的情况下,术语‘投影面积各向异性’或‘最大尺寸与最小尺寸的比’被理解为磁响应的均匀性的替选的可互换测量值。
在部署之前(当在输送装置中时)和在部署之后,可以针对标记确定磁化率各向异性。在标记配置改变的情况下,磁化率各向异性在部署之前和部署之后可以具有不同的值。
从实践角度来看,在使用如在W02014/013235所描述的磁探针来检测标记的手术期间,高各向异性是非预期的:在恒定距离处的磁信号将根据标记相对于探针的取向而改变以及在从一些取向接近时使标记表现得更靠近和当远离另一些取向时使标记表现得更远。使植入标记的各向异性最小化通过使标记更直观改善了外科医生定位标记的能力,以及提高了外科医生去除病变周围的组织的安全裕度的能力。各向异性比为1是理想的,提供了从任何方向的均匀响应。然而,实际上,通过如本文中所描述的小针在输送的几何约束内实现,是具有挑战性的。小于7(即在1和7之间)、优选地小于5且更优选地小于3的各向异性比是预期的。因为磁响应以指数方式随着距离而减小,所以小于2的各向异性比对于实际使用来说足够接近理想态。
理想的磁标记在存在磁场时被磁化且在去除磁场时不展现永久性剩磁(保留的磁化),或换言之,理想的标记为软磁的、即由软磁材料形成或表现得就像它是软磁的。软磁在此被定义为具有在各个实施方式中预期的小于或等于1000Oe、或优选地小于或等于100Oe、或更优选地小于或等于50Oe的矫顽磁力Hc。在检测期间,标记被磁化且遵循在图1中所示的虚线,在去除或反转磁场时借助实线返回。当施加交变磁场时,磁化驱动场(H)在每个周期内将材料沿着围绕环的磁化实线推进一次。通过磁化率计探针,例如美国公开文献No.2011/0133730的探针,检测材料中的感应场(称为磁化M)。理想的标记材料是软的且具有与在图1A中的磁化曲线相类似的磁化曲线。
应当注意,永磁体是硬磁的,具有高剩磁和高矫顽磁力(图1(B))。永磁体通常不适合用作本申请中的磁标记,这是因为它们可以吸引其它铁磁性物体(诸如手术工具)或被其它铁磁性物体(诸如手术工具)吸引,且因为它们通常具有非常低的磁化率。
需要被植入以使磁标记可检测的材料的量取决于材料的磁化率(χv),更具体地取决于质量磁化率(χρ=χv/ρ),其可以被表达为:
χρ=(μr-1)/ρ
其中,μr为相对磁导率,且ρ为材料密度。
在磁标记中使用的材料应当具有大于100且优选地大于500的相对磁导率。当使用专业磁性材料(诸如高纯铁)或非晶体材料(诸如金属玻璃)时,相对磁导率大于1000且优选地大于5000。标记应当具有高的质量磁化率χρ。对于传统的磁性材料,诸如铁、钢和铁氧体,χρ应当大于或等于0.05m3kg-1,优选地大于或等于0.1m3kg-1,且更优选地大于或等于1m3kg-1。专业磁性材料(诸如高纯铁)或非晶体材料(诸如金属玻璃)的使用允许甚至更高的质量磁化率,以及χρ优选地大于5m3kg-1且更优选地大于10m3kg-1。
如果材料质量磁化率足够高,则能够从所描述的传统针部署的球形标记将能够被磁性地定位且提供完全各向同性的信号。表1示出了大量磁性材料的质量磁化率。

这些磁标记的检测和定位可以利用灵敏的磁强计(或磁化率计)来执行,该磁强计(或磁化率计)产生交变磁场以磁性地激励标记以及检测由该标记产生的磁场信号,如在美国公开文献No.2011/0133730中所公开。也可以通过其它技术(诸如MRI、磁粒子成像、涡流测量、霍尔效应、或磁断层摄影术)来检测标记。
在本发明的一个方面中,标记包括超顺磁粒子。超顺磁粒子通常包含被可生物相容的涂层(诸如右旋糖酐、羧基右旋糖酐、其它糖、白蛋白、PEG、或可生物相容的聚合物)包围的氧化铁(磁铁矿和/或磁赤铁矿)芯。为了展现超顺磁行为,粒子的磁芯需要低于临界直径,通常根据材料和结构而在3nm-25nm的范围内。
氧化铁由于其低毒性而为用于超顺磁芯的优选材料,但是存在可形成超顺磁芯的其它材料。芯的材料应当为能够是磁有序的材料。它可以为:金属,诸如钴、铁或镍;金属合金;稀土和过渡金属合金;包含铝、钡、铋、铈、铬、钴、铜、镝、铒、铕、钆、钬、铁、镧、镥、锰、钼、钕、镍、铌、钯、铂、镨、钷、钐、锶、铽、铥、钛、钒、镱、和钇或其混合物的M型或尖晶石铁氧体。
芯也可以通过使铁(II)盐和另一金属盐的组合氧化来形成。有利的金属盐包括铝盐、钡盐、铋盐、铈盐、铬盐、钴盐、铜盐、镝盐、铒盐、铕盐、钆盐、钬盐、铁盐、镧盐、镥盐、锰盐、钼盐、钕盐、镍盐、铌盐、钯盐、铂盐、镨盐、钷盐、钐盐、锶盐、铽盐、铥盐、钛盐、钒盐、镱盐、和钇盐。
在本发明的另一方面中,标记包括固态软磁材料以在利用磁化率计探针被定位时提供显著增大的磁响应。由软磁材料产生的标记可以包括各种顺磁性的、铁磁的、和亚铁磁的材料,诸如:铁、镍、钴及其合金,电磁铁(包括FM、高温合金电磁铁),硅-铁(包括“A”、“A-FM”、“B”、“B-FM”、“C”变型),含磷的铁,镍-铁(例如,HyRa合金、HyMu合金、Hipemom、顺合金、超合金、Mu-金属),Heusler合金,Fernico合金(基于铁-镍-钴的合金),Cunife合金(基于铜-镍-铁的合金),Alcomax合金(基于铁-镍-铝-钴-铜的合金),来自300系列(例如302、304、316)、400系列(例如410、416、420、430、440、446、470)的各种不锈钢,以及专业不锈钢合金(例如,铬-铁合金,诸如铬-系列(Carpenter Technology Corp,Wyomissing PA)、马氏体不锈钢),铁氧体,诸如MnZn-铁氧体、NiZn-铁氧体、MgZn-铁氧体、Ba-铁氧体、MnMgZn-铁氧体、和MgZnCu-铁氧体。
在本发明的优选方面中,标记包括具有非常高的质量磁化率的金属玻璃,以提供显著改善的磁响应。金属玻璃也被称为非晶态金属或块体金属玻璃且包括:基于Fe或Co的材料,诸如由Metglas有限公司(Conway,SC)或Neomax Materials有限责任公司(日本大阪)生产的那些材料;和磁性碳同素异形体(例如,富勒烯、高度定向热解石墨、碳纳米泡沫、纳米多孔碳)。金属玻璃的示例包括但不限于:FINEMET、NANOPERM、HITPERM(日本东京的所有Hitachi Metals)、METGLAS#2605、METGLAS#2826、METGLAS#2615、METGLAS#2714A、METGLAS#2605。
为了确保生物相容性,这些材料可以被涂覆生物可相容材料或惰性材料或被包含在生物可相容材料或惰性材料内,该生物可相容材料或惰性材料例如生物玻璃、类金刚石(Diamond-Like-Carbon,DLC)、金、羟磷灰石、铁、镁、镍钛诺合金、聚对二甲苯、磷酰胆碱(PC)聚合物、聚丁基丙烯酸甲酯(PMBA)和聚乙烯醋酸乙烯酯(PEVA)、聚乙烯、PET、聚四氟乙烯(PTFE)、PEBAX、PEEK、PEKK、铂、硅酮、钛等。
另外,成形材料(诸如弹簧钢)或形状记忆材料合金(诸如镍钛诺合金)以及形状记忆聚合物(诸如PEO-PET嵌段共聚物和PEEK)也可以提供在部署上形成具体形状的附加功能,条件是包围软磁材料或被软磁材料包围。
磁性材料还可以被保持在可生物相容的基质内,诸如胶原、明胶和其它纤维素基材、聚乙烯醇(PVA)、聚葡糖酸酯、基于聚酯的材料(通过如下这些单体中的一者或多者的均聚或共聚形成:乙交酯、L-丙交酯及其异构体、ε-己内酯、对二氧环己酮和三亚甲基碳酸酯(TMC))。这些材料可以包括:均聚物,诸如聚(L-丙交酯)、聚(DL-丙交酯)、聚(TMC)、聚己内酯(PCL)、聚乙交酯(PGA)、聚(乙交酯-L-丙交酯)(PGL)、或聚(对二氧环己酮)(PDS);或共聚物,诸如L-丙交酯/DL-丙交酯、L-丙交酯/乙交酯、L-丙交酯/己内酯、DL-丙交酯/乙交酯、DL-丙交酯/己内酯、乙交酯/己内酯、L-丙交酯/乙交酯/己内酯、DL-丙交酯/乙交酯/己内酯、聚(二氧化酯共三亚甲基碳酸酯-共乙交酯)、Glykomer631(出售为);或这些物质与PDS、水凝胶的共聚物(来自如下项中的一种或多种单体:甲基丙烯酸羟乙酯、羟乙氧基乙基甲基丙烯酸酯、羟基二乙氧基乙基甲基丙烯酸酯、甲氧基乙基甲基丙烯酸酯、甲氧基乙氧基乙基甲基丙烯酸酯、甲氧基二乙氧基乙基甲基丙烯酸酯、乙二醇二甲基丙烯酸酯、N-乙烯基-2-吡咯烷酮、N-异丙基丙烯酰胺、醋酸乙烯酯、丙烯酸、MAA、N-(2-羟丙基)甲基丙烯酰胺、乙二醇、PEG丙烯酸盐、PEG甲基丙烯酸酯、PEG双醋酸酯、PEG二甲基丙烯酸酯)。
在本发明的一个方面中,植入的标记由单一软磁材料制成且该标记被定形以减小磁响应的各向异性。该各向异性被定义为最大磁响应与最小磁响应的比。各向异性在具有细长纵横比的形状中出现,因为软磁材料使运行穿过它们的任何磁场线聚焦。聚焦效应取决于在场线的方向上的材料的量。因此,细长形状在其长轴与场对齐时比在其长轴垂直于场线时使穿过材料的多很多的场线聚焦。结果是由磁化率计测量的磁响应在长轴方向上比在短轴方向上大很多。

通过改变形状来影响聚焦效应,可以改变各向异性。例如,通过在标记中创建弯曲,可以显著减小用于给定预部署的尺寸比的各向异性比(表3)。130°夹角出人意料地将该各向异性比从6.7减小到4.5,且U弯曲将该比进一步减小到2.1。在另一示例中,90°弯曲将该各向异性比从10.7减小到2.6,且60°夹角将该比进一步减小到1.28。再次参考表2,使用与在美国公开文献No.2011/0133730中所描述的磁化率计系统相类似的磁化率计系统,由各种材料且以各种形式产生的标记具有在固定距离处测量的最大信号和最小信号。该图示出当角度在0°和90°之间且更优选地在0°和45°之间时,存在对于均匀信号的最佳角度。图18示出该信号对于具有不同夹角的标记如何随着相对于标记的主轴的感测角度而改变。

图2(A)为在标记中的弯曲的夹角减小时的各向异性比的相对变化的图。使用在图2(B)中的测试布置进行测量。该图(图2(A))示出当角度在0°和90°之间且更优选地在0°和45°之间时,存在对于均匀信号的最佳角度。图2(C)为示出对于具有不同夹角的铁标记该信号如何随着相对于标记的主轴的感测角度而改变的图。图2(D)为示出对于具有不同夹角的铁标记该信号如何随着相对于标记的主轴的感测角度而改变的图。
在一些情况下,对于标记来说可能有利的是,仅在其被部署之后呈现形状,从而可以在部署之前更高效地将该标记包装在部署针中。可以使用弹性材料或一段弹性材料来促成这点。
这类形状包括但不限于如下:
-“菱形”或“珠子”形状(图3(A)),其通过在横轴(垂直于珠子的长轴)上提供更多磁性材料来减小各向异性。这类标记可以包括成形端部,这些成形端部包括使场聚焦在远离主轴的方向上的较窄区段(图3(B)和图3(C))。例如,图3(C)的标记的有限元模型和在美国公开文献No.2011/0133730中所描述的类型的探针(其中标记处在距探针端部20mm的距离上)提供了2.5的磁化率各向异性比;
-如图4(A)至图4(D)所示的“哑铃类型”设计由相同材料的多个区段构成。该设计类似地沿着横轴提供更多磁性材料。在各个实施方式中,标记的长度为大约5mm-10mm且在减小对立涡流上很有用。例如,标记的有限元模型和在美国公开文献No.2011/0133730中所描述的类型的探针(其中标记处在距探针端部20mm的距离上)提供了为2.6的磁化率各向异性比;
-图5(A)和图5(B)示出的缆绳或多丝绞线可以被扭绞。示出了大量铰线形式,但是具有3、4、5、6、7、或更多条股的其它绞线形式(诸如1×4、1×5等)同样可行;使用绞线减小对立涡流损失且具有多个面提供优越的超声波响应。丝也可以为中空的,如所示,或在绞线之间包括空隙区;
-包括锐角和钝角的弯曲线或弯曲管,U形、X形(表3);
-划分为两个或更多个尺寸类似的小片的单一长度的柱形标记提供小于5的改善的各向异性比;
-具有较大直径的中心截面和较小直径的外截面的柱形标记也提供改善的各向异性比;
-由两个互锁的U形元件组成的标记(图18)。
在本发明的一个方面中,植入的标记主要由软磁材料制成且该标记在部署之前具有细长的纵横比但在部署之后将配置改变为具有低的磁化率各向异性的形状。在部署之前,标记可以具有大于5的长度与直径的比或形状因子和大于5或甚至大于7或9的磁化率磁各向异性比,这类值源自于标记在部署之前的延长的长度且有益于增大包含在针中的标记的体积,从而一旦被部署则使磁响应最大化。在部署之后,磁化率磁各向异性比小于5且优选地小于3,以及理想地为2或小于2,以提供更均匀的磁响应。
在一个方面中,标记为可弹性变形的或可恢复变形的,从而该标记在形状和尺寸上弹性地或有恢复力地从在针或部署装置内的包装或预部署形状和相关联的形状因子变化为部署后的形状和形状因子。弹性或恢复力例如可以源自于弹性或有恢复力的材料的使用或源自于弹性结构、有恢复力的结构或可展开的结构或及其组合。优选地,磁标记在具有较高的磁化率各向异性的包装配置与具有较低的磁化率各向异性的部署/打开配置之间是可弹性变形的或可恢复变形的。可替选地,磁标记在具有较高的投影面积各向异性比的包装配置与具有较低的投影面积各向异性比的部署/打开配置之间是可弹性变形的或可恢复变形的。
优选地,标记的部署或输送配置具有小于5、更优选地小于3且理想地小于2的磁化率各向异性,从而提供最均匀的检测信号;而在部署在输送装置内之前,该标记在其包装配置中具有大于5且更优选地大于7的磁化率各向异性,从而在给定输送装置的受约束直径下使标记中的材料的体积最大化。通过类比,该标记在其包装配置中具有大于5且更优选地大于7的投影面积各向异性比;但是在部署/打开配置中具有小于5、更优选地小于3且理想地小于2的投影面积各向异性比。为了实现该配置变化,标记需要一定程度的恢复力,由此该标记的一个尺寸(例如垂直于主轴的直径)按至少1.5倍的系数从其包装值变化为其部署值。优选地,部署尺寸大于包装尺寸的2倍,且更优选地大于包装尺寸的3倍,从而在部署上提供接近1或2的形状因子(最大尺寸与最小尺寸的比),同时仍具有足够的待检测材料。
处于其包装配置的磁标记在使用之前被包装在输送装置内。输送装置需要能够使标记输送穿过皮肤以标记感兴趣的组织区域,例如癌性病变。合适的输送装置包括任何基于针或套管的输送系统,其通常包括针和用于推动标记穿过针的端部或穿出针的侧面的部件,诸如活塞或通管丝。该针优选地为14至18个标准尺寸。这意味着,针具有通常0.8mm至1.5mm的内直径,但是对于特定针设计有可能达1.8mm那么大。优选地,该针的直径在1.0mm和1.5mm之间。如果使用真空辅助针,针尺寸通常为11个标准尺寸,其中内直径为2.3mm至2.5mm。
实现磁化率各向异性在包装配置和打开配置之间的预期变化的标记包括但不限于如下项:
“纱线球”类型形式(图6(A)和图6(B)),其通过在标记的任何给定轴上提供基本一致的磁性材料量而减小各向异性。多个小平面也提供优越的超声波响应。纱线球优选地由具有大磁响应的材料形成,该材料诸如来自上述列表的金属玻璃或软磁材料。该球可以由直径例如在10μm和250μm之间且长度例如在5mm和150mm之间以及弯曲成球形状的细丝形成。该标记可以为可弹性变形的或可恢复变形的。该标记被压缩以安放在部署针或输送装置的内部,其中,在输送之前,该标记可以被约束为采用更加柱形的形状。在部署时,该球扩张到接近其原始尺寸且采用基本上球形的形状。表3示出了该标记可以实现接近理想值1的各向异性比。
“中国灯笼”形式(图7)。在部署时,该标记被布置使得其从柱体有恢复力地重配置为更压缩的灯笼配置,因此增大磁响应的均匀性(减小各向异性),因为横轴上的材料的量比柱体的材料的量增多。多个小平面也提供优越的超声波响应。
如图8(A)至图8(G)所示的包括用铰链接合的多个软磁元件的标记,该多个软磁元件变形为在部署时的形状(可塑性地被预施压或通过材料中的形状记忆的行为)。在本实施方式中,标记的更大长度是可能的,因为在部署针中的标记的形状是线状的但是在注入位点中之后折叠为另一配置。这通过在标记的任何给定轴上提供基本一致的磁性材料量而减小各向异性。
有利地,多个小平面也提供优越的超声波响应。可预期的是具有至少三个元件以获得均匀响应,且可以添加更多元件,但是针对复杂度而言,数量优选地小于20且更优选地小于10。铰接的和其它柔性的或可恢复变形的形式可以包括通过非磁性柔性的或可恢复变形的链接件(诸如塑性或形状记忆材料)接合的多个较小的磁性单元或部件。也包括这些形式的组合,例如由多股缆绳制成的弯曲丝。如表3所示,具有多个球状元件且预部署几何长度与直径的比为8的这种标记可以具有小于2的部署后的磁化率各向异性。
包括通过它们之间的可折叠链接件而接合的多个元件的标记,在部署时,这些元件折叠在一起以形成具有低的磁化率各向异性比的非结晶区域。这些链接件可以由绳索状材料(诸如缝合线或其它聚合物)形成。优选地,软磁元件为可以在部署之后自身折叠的链中的链接件。
包括在部署之前的有弹力的或可恢复变形的丝或柱体形状的标记,在部署时该丝或柱体形状形成结构或线框。这类结构可以包括柱形线圈、螺旋状物、锥形线圈、球形线圈、随机的“纱线球”、或多面体(诸如四面体)或其一部分。
包括可恢复变形的环状的或成形的螺旋的弹簧的标记,该弹簧在部署之前被压缩且在部署时扩张为具有低的磁化率各向异性比的形状。例如,成形为圆环的螺旋弹簧提供惊人的低的磁化率各向异性比(表3)。优选地,圆环直径与弹簧线圈直径的比小于5,从而保持更均匀的磁响应。
包括通过由弹性的或可恢复变形的或有弹力的材料形成的链接件而连接的两个或更多个元件的标记,从而这些元件一旦被部署则弹性地变为提供低的磁化率各向异性的新配置。表4示出了一个这类实施方式,其包括通过弹性细丝接合的两个由软磁材料形成的短柱形元件。在部署针中,这两个元件被约束成通过细丝轴向地对齐接合。该细丝被偏置,使得在部署时,这两个元件跨越彼此而弹回以接近具有小于2的低的磁化率各向异性比的四面体形状。弹性细丝可以由弹簧钢、形状记忆材料、或其它弹性或可恢复变形的材料形成。优选地,柱形元件的长度为其直径的2倍到5倍,以提供响应的良好均匀性以及紧凑的形状。在该概念的另一实施方式中,由针部署弹性材料上的多个元件且该弹性材料被偏置使得这些元件在部署时形成为一个结构。示例性结构可以包括柱形线圈、螺旋状物、锥形线圈、球形线圈、随机的“纱线球”、或多面体或其一部分。在这些设计中,标记的长度在处于其包装配置时可以为其直径的2倍到5倍,但是根据所使用的材料的结构的弹性程度而可以甚至更大,例如高达直径的10倍或更多倍,因此允许更多磁性材料被部署以使标记的可检测性最大化。
“纱线球”(图6)成股的或多小平面的形式的另一益处是优于反射性成像技术(诸如超声、IR、或超宽带雷达)的可见性。这些益处也可以通过提及的其它形式的外表面的修正来实现,包括铰接形式(图7)(从圆柱体形式到带小平面或带凹槽的形式)的无铰接截面,诸如但不限于三角形、五边形、十二边形、齿轮状横截面(图9)。从诸如铁氧体的烧结材料,在超声波中可以观察到类似效果。具有任何所提及形式的软材料的正确选择可以提供对X射线成像的可见性。互锁的U采用与U类似的空间,但具有增大的信号和减小的各向异性(图18)。
可以组合上述特征以提供各向异性减小且成像得到改善的标记。
在本发明的另一方面中,通过改变形成标记的软磁材料沿着标记的长度的磁性能来修改植入标记的磁化率各向异性比。
在本发明的另一方面中,可以通过使用复合材料来实现磁化率各向异性的减小,该复合材料诸如借助烧结用两种或更多种材料形成的铁氧体,其中,至少一种材料为分布以提供较小的各向异性响应的软材料。可以形成类似的形式,如上所述,在标记内使用多种材料。一个这类实施方式为具有恒定横截面的单一区段的标记。
本发明的这个方面的附加益处为软分布或形状可以不依赖于外部形式和横截面,外部形式和横截面改善了在超声波或X射线下的成像可见性。各向异性减小的复合磁标记的其它示例包括分布在可折叠的支架状结构上的软标记,具体地,那些自扩展的软标记。
另外,在缆绳或多丝绞线(尤其在被扭绞的情况下)实施方式中包括中空形式,其中,各个丝由不同材料形成或由复合材料形成,例如,芯和覆盖或护套材料,其中,这些材料中的至少一者或多者为软磁材料。具体示例为,芯或覆盖材料由镍钛诺合金或用于形成部署后的形状的其它形状记忆材料(包括形状记忆聚合物)形成(图10)。同样可能的是形成丝,其中,由多种材料产生的、被护套材料覆盖的芯被制作且直接被用作磁标记或后续被用在缆绳或多股形式中(图11)。丝或多芯丝的区段之一也可以用于提供改善的X射线可见性。
图12(A)和图12(B)为分段标记的示例(注意:更多或更少区段是可能的)。分段增大了轴向上的截面之间的损失且管理对立涡流效应,减小各向异性。
图13(A)和图13(B)示出软材料(深灰色)的示例,该软材料可以被模制为不依赖于标记的外形的形状(注意:在构造内可以使用多于两种的材料)。通过在横轴上提供更多磁性材料且通过所选材料的组合减小对立涡流来改善各向异性。在图13(C)中所示的一个实施方式中,软磁芯由具有非常高的磁响应的材料(诸如金属玻璃)形成,从而可以从该材料的仅非常小的球面获得足够的磁响应。芯被包在保护层中,该保护层包括可生物相容的涂层或囊,例如钛壳或可生物相容的聚合物涂层。由于芯为球形,则磁化率各向异性比接近1。
在图14(A)至图14(E)中示出了采用具有不同磁性能的多种材料的、具有塑性/预应力/形状记忆的铰链的成形标记。这些配置改善了响应的定制,从而通过在标记的任何给定轴上提供基本均匀的响应而减小各向异性。多个小平面也提供优越的超声波响应。
在本发明的另一方面中,细长标记被划分为多个更小标记。在部署之前包装在一起的多个标记具有相同的整体尺寸和材料,且可以用于减小相对于材料的整体尺寸的各向异性。例如,3×2mm长或2×3mm长或6×1mm长的标记片具有比外直径相同的1×6mm长的标记减小的各向异性。出人意料地,各向异性的该减小甚至发生在多个区段以与组合尺寸的个体标记相同的形状一个接一个对齐时。(表5)例如,采用铁氧体不锈钢的尺寸为5mm的单一标记提供6.7的各向异性比。采用相同材料的3mm标记的两个标记根据相对取向提供在3.2和4.6之间的各向异性比。

在这样的另一具体示例中,由软磁材料生产的一组标记或磁性粒子可以保留少量磁性。一旦部署到患者中,则这些磁性粒子可以自组装成相关联的磁标记,该磁标记拥有较低的磁化率各向异性。具有小剩磁的多个软磁标记将自组装成相关联的磁标记,该磁标记在每个方向上具有基本上一致的材料量,从而使各向异性最小化(图15)。该小剩磁可以通过如上所述的磁力计(或磁化率计)的激励场的磁激励来克服。表6示出了用两种类型的磁组成组装的粒子的参数。

在这样的另一示例中,包在疏水性表面(借助荷叶效应的纳米纹理、二氧化硅纳米涂层、EnBio CoBlast PTFE、羟基磷灰石、碳纳米管涂层、沉淀碳化钙和具有复合物乳胶的脂肪酸涂层、锰氧化物聚苯乙烯或锌氧化物聚苯乙烯纳米合成物)或球体内的软磁标记或粒子将在部署时拉在一起(自组装)进入患者内。为了使表面能量最小化,该实施方式将形成紧密包装的形状,诸如球体或椭球体系统,该系统将具有相对于输送系统内的粒子的改善的磁化率各向异性,该输送系统将约束在将被拉长的细长形状中。图16示出疏水性涂覆的标记或粒子,为了使表面能量最小化,这些标记或粒子自组装成相关联的磁标记,该磁标记在每个方向上具有基本上一致的材料量,使各向异性最小化。
在本发明的另一方面中,软材料的小型微标记或微粒子可以被悬浮在可生物相容的基质内(图17(A)和图17(B))。以与相对于图10至图12所讨论的方式类似的方式对磁性粒子分布的控制允许减小的磁化率各向异性以及不依赖于外在形式。也可以用于确保磁标记之间的设定位置和取向。
在对于该情况的另一示例中,通过使用悬浮有软磁粒子的凝胶基质且通过适当地调整这些粒子的尺寸,可以通过部署针部署凝胶和粒子(图17(B))。部署的凝胶和粒子的形状将仅受注入位点(即病变的组织)约束,该注入位点相比于针更小。也可以在注入时设置该凝胶,使得不太可能从其部署位置迁移且如果该组织正被去除的话,更易于手术去除该凝胶。如果需要,则可以执行其它粒子(诸如金)的添加,以提供附加的对于X射线可视化的不透射线响应。为了标记定位,各向异性应当小于或等于9、优选地小于或等于6、更优选地小于或等于3。
本发明提供制作具有可接受的各向异性的磁标记、同时不影响粒子的输送容易度的各种方式。
在本发明的另一方面中,提供一种用于标记感兴趣的软组织位点的方法,该位点诸如例如在胸部、肺、肠/结肠、直肠、前列腺或其它癌症影响的器官内的肿瘤或良性病变的位点,或者在腋窝或体内其它结节凹陷处内的淋巴结。该方法包括如下步骤:(i)将这类磁标记插入目标病变或位点附近的组织中,以及(ii)使用磁化率计检测这类标记,以及可选地(iii)通过手术切割标记周围的目标组织。优选地,该磁化率计通过在标记的区域中提供磁场且测量该标记中的感应磁化来检测该标记。
除非另有指示,否则,在说明书和权利要求中所使用的表达长度、宽度、深度或其它尺寸等的所有数字将在所有情况下被理解为指示所示的准确值以及通过术语“大约”来修饰。相应地,除非相反指示,否则,在说明书和所附权利要求中提出的数值参数为可根据寻求获得的预期性能而改变的近似值。至少地且不企图将等效物的教义的应用限制到权利要求的范围,至少应当鉴于上报的有效位的数目且通过应用普通舍入技术来解释每个数值参数。任何具体值可以改变20%。
在描述本发明的上下文中(尤其在所附权利要求的上下文中)所使用的术语“一”、“一个”、“该”和类似的指示对象将被视为覆盖单数和复数,除非在本文中另有指示或清楚地与上下文相矛盾。本文中所描述的所有方法可以按任何合适次序来执行,除非在本文中另有指示或清楚地与上下文相矛盾。本文中所提供的任何示例和所有示例或示例性语言(例如“诸如”)的使用仅仅意图更好地说明本发明且不对任何权利要求的范围造成限制。在说明书中的语言都不应当被视为指示对本发明的实践来说必不可少的任何非请求保护的元素。

Claims (15)
1.一种用于标记组织中的位点的部署的标记,所述标记包括:
具有一个或多个区段的分段标记,每个区段由矫顽磁力小于1000Oe以及磁化率大于0.05m3kg-1的软磁材料制成;
其中,所述标记的磁化率各向异性比小于5;以及
其中,每个磁性区段定义一种或多种形状,或者,一种或多种形状的组合,所述一种或多种形状选自:环、纱线球、中国灯笼、U形、链和线圈。
2.根据权利要求1所述的标记,其中,所述标记在部署之前具有大于2.7的尺寸比。
3.根据权利要求1所述的标记,其中,所述标记的磁化率各向异性比小于3。
4.根据权利要求1所述的标记,其中,所述磁性区段包括磁性非晶体材料。
5.根据权利要求1所述的标记,其中,第一磁性区段通过柔性的非磁性部件链接到第二磁性区段。
6.根据权利要求1所述的标记,还包括基质,所述基质包含生物可相容材料。
7.根据权利要求1所述的标记,其中,所述标记的区段被涂覆生物可相容材料或设置在生物可相容材料内。
8.根据权利要求7所述的标记,其中,所述生物可相容材料选自:生物玻璃、类金刚石(DLC)、金、羟磷灰石、铁、镁、镍钛诺合金、聚对二甲苯、磷酰胆碱(PC)聚合物、聚丁基丙烯酸甲酯(PMBA)和聚乙烯醋酸乙烯酯(PEVA)、聚乙烯、PET、聚四氟乙烯(PTFE)、PEBAX、PEEK、PEKK、铂、硅酮、钛。
9.根据权利要求1所述的标记,其中,所述分段标记具有恒定横截面。
10.根据权利要求1所述的标记,其中,每个区段的长度基本上等于所述标记的其他区段的长度。
11.根据权利要求1所述的标记,其中,所述标记的区段的横截面不同于所述标记的外部形状。
12.根据权利要求1所述的标记,其中,所述标记是具有较大直径的中心区段和较小直径的外区段的柱形标记。
