发明内容
在一些方面,本发明提供了用于产生CAR的方法,并且提供了特定的CAR。在一些方面,提供了用于产生大量CAR的方法,所述CAR可以针对特定癌症或癌症亚型的活性被筛选;这样,可以产生并鉴定可以展现出改善的针对特定癌症或癌症亚型的治疗潜力的CAR。可以向受试者或人患者治疗性施用本文中提供的CAR,例如以治疗癌症。
临床数据证明了使T细胞靶向给定的肿瘤相关抗原(TAA)的特定的嵌合抗原受体(CAR)设计在不同患者中可以具有不同的治疗潜力。例如,当向具有急性的而不是慢性的B谱系白血病的患者施用自体遗传修饰的T细胞时,经由嵌合CD28/CD3ζ或CD137/CD3-ζ活化的第二代CD19特异性CAR可以展现出优越的临床响应。为了解决此问题,本文中提供了用于产生可以展现出改善的针对给定肿瘤的抗肿瘤效果的CAR种类的方法。
例如,本文中提供了可以用于产生大量CAR并且筛选它们治疗来自给定患者的癌症的能力的方法;这样,可以使用所述方法使用于患者的疗法个体化,并且选择展示针对具有特定癌症的特定患者或患者亚组的改善的治疗潜力的特定CAR。用于基因疗法的临床方法可以利用来自睡美人(Sleeping Beauty)(SB)转座子系统的DNA质粒的电转移,例如以降低制造用于小患者亚组的单独CAR设计的成本和复杂性。用于使CAR+T细胞个体化的这些方法可以利用大量CAR分子的产生,可以筛选所述CAR分子和评估其有益于给定患者的能力。
在一些方面,提供了使用三重位点特异性重组系统(又称为“EZ-CAR”平台)高通量装配CAR分子的方法。在一些实施方案中,这些方法可以允许从(i)限定特异性的单链可变片段(scFv),(ii)从细胞表面附加scFv的支架/铰链,和(iii)一个或多个胞内信号传导结构域快速组合原型CAR的3种组分。例如,如下文实施例中显示,使用EZ CAR平台连同临床级CD19RCD28mζCAR+T细胞(CG CAR),产生经由嵌合CD28/CD3-ζ活化的CD19特异性CAR。

具体实施方式
本文中提供了用于产生嵌合抗原受体(CAR)的方法。所述方法利用多个载体,每个编码抗原结合结构域(例如,scFv区)、铰链区、跨膜区、和/或膜内结构域。例如,在一些实施方案中,第一载体编码抗原结合结构域,第二载体编码铰链区,并且第三区编码膜内结构域。在一些实施方案中,跨膜区在第二载体中,在第三载体中,或在第四载体中编码。在一些优选的实施方案中,载体可以同源重组以形成编码CAR的核酸,所述CAR包含抗原结合结构域、铰链区、跨膜区、和膜内结构域。这样,可以产生许多CAR,并且筛选期望的活性,如例如,选择性识别和杀伤表达由CAR选择性结合的抗原的癌性细胞。然后,CAR可以在细胞,如T细胞或自然杀伤(NK)细胞中以整合核酸(例如,使用转座酶/转座子整合到宿主基因组中的DNA)或以非整合核酸(例如,经由病毒载体,如慢病毒或逆转录病毒递送的mRNA)表达。然后,表达CAR的T细胞或NK细胞可以在药物制剂或赋形剂中向受试者,如人患者施用以治疗或预防疾病(例如,癌症、真菌感染、细菌感染、或病毒感染)。
I.文库产生
可以通过本领域技术人员已知的方法产生编码多个scFv区、铰链/支架区、跨膜结构域、和膜内结构域(信号传导结构域)的文库。在一些实施方案中,对于scFv区、铰链/支架区、和膜内结构域(信号传导结构域)中的两个或三个,可得到多种可能性。在一些实施方案中,对于scFv区、铰链/支架区、跨膜结构域、和膜内结构域(信号传导结构域)中的两个、三个或全部,可得到多种可能性。例如,在表1中提供了scFv区、铰链/支架区、跨膜结构域、和膜内结构域(信号传导结构域)的实例。在一些实施方案中,文库可以编码多个靶向不同抗原,如多个抗癌抗原或肿瘤靶向抗原的scFv;在其它实施方案中,文库可以编码选择性结合单一靶标(例如,单一抗癌抗原,如CD19等)的多个不同scFv。这样,可以使用所述方法鉴定哪个肿瘤靶向构建体可以对给定细胞样品更有效起(例如以在个体化药物中使用)或可以使用所述方法鉴定在靶向给定抗原中更有效发挥功能的新CAR。scFv区一般包含从抗体衍生的可变轻(VL)和可变重(VH)链。在一些实施方案中,若想要的话,可以随机化VL和VH区的部分。用于产生文库的通用方法包括例如产生酵母文库、细菌、噬菌体文库、浸润性B细胞、杂交瘤(包括来自人和啮齿类),或来自羊驼(llamas)的文库、骆驼、马文库、和计算机方法(参见例如Lennard,2002)。
在一些实施方案中,融合编码scFv、铰链/支架区、跨膜结构域、和膜内结构域的不同载体以形成编码CAR的单一载体。可以经由转座子介导的同源重组发生融合。
例如,在一些实施方案中,编码scFv、铰链/支架区、跨膜结构域、和/或膜内结构域的载体可以是睡美人(SB)或piggyBac DNA质粒。睡美人(SB)和piggyBac DNA质粒描述于例如Maiti等(2013),Singh等(2008),和Huls等(2013)。在一些实施方案中,转座子由鲑鱼型Tc1样转座酶(SB)介导。在一些优选的实施方案中,经由如Singh等,2014或Huls等中描述的方法,将编码CAR的载体转染或掺入来自受试者,如患有癌症的人患者的T细胞中。例如,可以使用从睡美人(SB)系统衍生的DNA载体避免与用重组病毒载体转导T细胞有关的费用和制造困难。在电穿孔后,转座子/转座酶可以改善用于在T细胞中表达CAR和其它转基因的质粒的整合效率。与人工抗原呈递细胞(aAPC)组合的SB系统可以选择性增殖,并且产生适合于人应用的CAR(+)T细胞。在一些实施方案中,两种DNA质粒、SB转座子(编码感兴趣的CAR)和SB转座酶(例如,SB11)的同步电转移可以继之以通过在存在可溶性重组人IL-2和IL-21的情况下每7天添加(刺激循环)经γ-照射的aAPC回收稳定整合体。例如,可以进行4个循环(28天的连续培养)以产生临床上有吸引力的数目的稳定表达感兴趣CAR的T细胞。可以利用转座子/转座酶系统的使用递送表达CAR的T细胞,如例如Hackett等中描述。
II.嵌合抗原受体
本发明的实施方案牵涉产生并鉴定编码抗原特异性嵌合抗原受体(CAR)多肽的核酸。在一些实施方案中,将CAR进行人源化以降低免疫原性(hCAR)。
在一些实施方案中,CAR可以识别由一种或多种抗原之间共享的空间构成的表位。模式识别受体,如Dectin-1可以用于驱动针对碳水化合物抗原的特异性。在某些实施方案中,结合区可以包含单克隆抗体的互补决定区、单克隆抗体的可变区、和/或其抗原结合片段。在一些实施方案中,结合区是scFv。在另一个实施方案中,结合受体或细胞靶标的肽(例如,细胞因子)可以作为可能性包括在内或者替换CAR的结合区中的scFv区。因此,在一些实施方案中,可以从编码多个scFv区和/或其它靶向蛋白的多个载体产生CAR。互补决定区(CDR)是在抗原受体(例如,免疫球蛋白和T细胞受体)蛋白的可变域中找到的互补抗原并且因此向受体提供其针对所述特定抗原的特异性的短氨基酸序列。抗原受体的每条多肽链含有三个CDR(CDR1、CDR2和CDR3)。由于抗原受体通常由两条多肽链构成,每个抗原受体有可以与抗原接触的6个CDR:每条重链和轻链含有3个CDR。由于通常在CDR中找到与免疫球蛋白和T细胞受体选择性有关的大多数序列变异,这些区域有时称为超变结构域。在这些之中,CDR3显示最大的变异性,因为它由VJ(在重链和TCR αβ链的情况下为VDJ)区的重组编码。
经由本发明产生的CAR编码核酸可以包含一个或多个人基因或基因片段以增强用于人患者的细胞免疫疗法。在一些实施方案中,可以经由本文中描述的方法产生全长CARcDNA或编码区。抗原结合区或结构域可以包含从特定的人单克隆抗体衍生的单链可变片段(scFv)的VH和VL链的片段,如那些描述于美国专利7,109,304(通过引用方式并入本文)的片段。在一些实施方案中,scFv包含人抗原特异性抗体的抗原结合结构域。在一些实施方案中,scFv区是由针对人密码子使用优化以在人细胞中表达的序列编码的抗原特异性scFv。
CAR的抗原结合结构域的排列可以是多聚体的,如双抗体(diabody)或多聚体。多聚体可以通过将轻链和重链的可变部分交叉配对成可以称为双抗体的事物而形成。在一些实施方案中,可以缩短或排除CAR的铰链部分(即,产生仅包含抗原结合结构域、跨膜区和胞内信号传导结构域的CAR)。大量铰链可以在本发明的情况下使用,例如如表1中显示。在一些实施方案中,可以使铰链区的第一个半胱氨酸得到维持,或者通过脯氨酸或丝氨酸取代突变,或者截短直至第一个半胱氨酸。从用作抗原结合区的scFv缺失Fc部分以产生根据本发明的CAR。在一些实施方案中,抗原结合区可以编码仅Fc结构域之一,例如来自人免疫球蛋白的CH2或CH3结构域。还可以包括已经被修饰以改善二聚化和寡聚化的人免疫球蛋白的铰链、CH2、和CH3区。在一些实施方案中,铰链部分可以包含8-14个氨基酸的肽(例如12AA肽)、CD8α的部分、或IgG4Fc或者由其组成。在一些实施方案中,可以使用促进寡聚化的结构域,如CD8α,从细胞表面附加抗原结合结构域。在一些实施方案中,可以使用被单克隆抗体(mAb)克隆2D3(描述于例如Singh等,2008中的mAb克隆2D3)识别的结构域从细胞表面附加抗原结合结构域。
CAR的膜内结构域或胞内信号传导结构域一般可以引起或促进包含CAR的免疫细胞的至少一种正常效应子功能的活化。例如,膜内结构域可以促进T细胞的效应子功能,如例如细胞裂解活性或辅助活性,包括细胞因子的分泌。初始、记忆的或记忆类型的T细胞中的效应子功能可以包括抗原依赖性增殖。术语“胞内信号传导结构域”或“膜内结构域”指可以转导效应子功能信号和/或指导细胞实施特化功能的CAR部分。虽然通常整个胞内信号传导结构域可以被包含在CAR中,但是在一些情况中可以包含膜内结构域的截短部分。一般地,膜内结构域包括截短的膜内结构域,其中截短的膜内结构域保留在细胞中转导效应子功能信号的能力。
在一些实施方案中,膜内结构域包含T细胞受体的ζ链或任何其同源物(例如,η、δ、γ、或ε)、MB1链、B29、Fc RIII、Fc RI和信号传导分子的组合,如CD3ζ和CD28,CD27、4-1BB、DAP-10、OX40及其组合,以及其它类似的分子和片段。可以使用活化蛋白家族的其它成员的胞内信号传导部分,如FcγRIII和FcεRI。这些备选的跨膜和胞内结构域的实例可以参见例如Gross等(1992),Stancovski等(1993),Moritz等(1994),Hwu等(1995),Weijtens等(1996),和Hekele等(1996),通过引用方式以其整体并入本文。在一些实施方案中,膜内结构域可以包括人CD3ζ胞内域。
优选地,通过跨膜结构域连接抗原特异性胞外结构域和胞内信号传导结构域。可以在CAR中包含的跨膜结构域包括例如人IgG4Fc铰链和Fc区、人CD4跨膜结构域、人CD28跨膜结构域、跨膜人CD3ζ结构域、或半胱氨酸突变人CD3ζ结构域、或来自人跨膜信号传导蛋白如例如CD16和CD8和红细胞生成素受体的跨膜结构域。例如,在表1中提供了跨膜结构域的实例。
在一些实施方案中,膜内结构域包含编码共刺激受体,如例如经修饰的CD28胞内信号传导结构域、或CD28、CD27、OX-40(CD134)、DAP10、或4-1BB(CD137)共刺激受体的序列。在一些实施方案中,由CD3ζ启动的初始信号,由人共刺激受体提供的别的信号两者可以被包含在CAR中以更有效活化经转化的T细胞,这可以有助于改善过继性免疫疗法的体内持久性和治疗成功。如表1中记录,膜内结构域或胞内受体信号传导结构域可以包含单独或与FcγRIII共刺激信号传导结构域,如例如CD28、CD27、DAP10、CD137、OX40、CD2、4-1BB组合的CD3的ζ链。在一些实施方案中,膜内结构域包含TCRζ链、CD28、CD27、OX40/CD134、4-1BB/CD137、FcεRIγ、ICOS/CD278、IL-2Rbeta/CD122、IL-2Ralpha/CD132、DAP10、DAP12、和CD40中的一种或多种的部分或全部。在一些实施方案中,膜内结构域中可以包含1、2、3、4或更多个胞质结构域。例如,在一些CAR中,已经观察到融合在一起的至少两个或三个信号传导结构域可以导致叠加或协同效应。
在一些方面,可以产生包含编码CAR的DNA序列的分离的核酸区段和表达盒。可以使用多种载体。在一些优选的实施方案中,载体可以允许将编码CAR的DNA递送到免疫,如T细胞。CAR表达可以在调控的真核启动子,如例如MNDU3启动子、CMV启动子、EF1α启动子、或泛素启动子的控制下。此外,载体可以含有选择标记,若不是出于其它原因,以促进其体外操作。在一些实施方案中,可以从自DNA模板体外转录的mRNA表达CAR。
嵌合抗原受体分子是重组的,并且以其既结合抗原又经由存在于其胞质尾部中的免疫受体活化基序(ITAM)而转导活化信号的能力区分。利用抗原结合部分(例如,从单链抗体(scFv)产生)的受体构建体提供“通用”的额外优点,因为它们可以以不依赖于HLA的方式结合靶细胞上的天然抗原。例如,可以将scFv构建体与编码CD3复合物的ζ链(ζ)的胞内部分、Fc受体γ链、和sky酪氨酸激酶的序列融合(Eshhar等,1993;Fitzer-Attas等,1998)。已经在几种鼠和人抗原-scFv:ζ系统中证明重定向的T细胞效应子机制,包括CTL的肿瘤识别和裂解(Eshhar等,1997;Altenschmidt等,1997;Brocker等,1998)。
例如,抗原结合区可以来自人或非人scFv。使用非人抗原结合区,如鼠单克隆抗体的一个可能的问题是降低的人效应子功能性和降低的穿透肿瘤块的能力。此外,非人单克隆抗体可以被人宿主识别为外来蛋白质,并且因此重复注射此类外来抗体可以导致免疫应答的诱导,从而导致有害的超敏感性反应。对于基于鼠的单克隆抗体,此效应已经称为人抗小鼠抗体(HAMA)应答。在一些实施方案中,与一些鼠抗体相比,在CAR中包含人抗体或scFv序列可以导致很少或无HAMA应答。类似地,在CAR中包含人序列可以用于降低或避免免疫介导的内源T细胞的识别或消除的风险,所述内源T细胞存在于接受者中,并且可以基于HLA识别加工过的抗原。
在一些实施方案中,CAR包含:a)胞内信号传导结构域,b)跨膜结构域,c)铰链区,和d)包含抗原结合区的胞外结构域。在一些实施方案中,胞内信号传导结构域和跨膜结构域与膜内结构域一起由单一载体编码,所述单一载体可以与编码铰链区的载体和编码抗原结合区的载体融合(例如,经由转座子指导的同源重组)。在其它实施方案中,胞内信号传导区和跨膜区可以由融合(例如,经由转座子指导的同源重组)的两个不同载体编码。
在一些实施方案中,CAR的抗原特异性部分,又称为包含抗原结合区的胞外结构域,选择性靶向肿瘤相关抗原。肿瘤相关抗原可以是任何种类的,只要它在肿瘤细胞的细胞表面上表达。可以用经由本发明产生的CAR靶向的肿瘤相关抗原的实例包括例如CD19、CD20、癌胚抗原、甲胎蛋白、CA-125、MUC-1、CD56、EGFR、c-Met、AKT、Her2、Her3、上皮肿瘤抗原、黑素瘤相关抗原、突变p53、突变ras、Dectin-1等。在一些实施方案中,CAR的抗原特异性部分是scFv。表1中提供了肿瘤靶向性scFv的实例。在一些实施方案中,CAR可以与膜结合细胞因子共表达,例如以改善在存在有少量肿瘤相关抗原时的持久性。例如,CAR可以与膜结合IL-15共表达。
在一些实施方案中,可以用CAR靶向胞内肿瘤相关抗原,如例如HA-1、存活蛋白、WT1和p53。这可以通过通用T细胞上表达的CAR实现,所述通用T细胞在HLA的背景中识别从胞内肿瘤相关抗原描述的加工过的肽。另外,通用T细胞可以遗传修饰为表达T细胞受体配对,其在HLA的情形中识别胞内加工过的肿瘤相关抗原。
由CAR识别的病原体可以基本上是任何种类的病原体,但是在一些实施方案中,病原体是真菌、细菌或病毒。示例性的病毒病原体包括腺病毒科(Adenoviridae)、埃巴病毒(EBV)、巨细胞病毒(CMV)、呼吸道合胞病毒(RSV)、JC病毒、BK病毒、HSV、HHV病毒科、细小RNA病毒科(Picornaviridae)、疱疹病毒科(Herpesviridae)、嗜肝DNA病毒科(Hepadnaviridae)、黄病毒科(Flaviviridae)、逆转录病毒科(Retroviridae)、正粘病毒科(Orthomyxoviridae)、副粘液病毒科(Paramyxoviridae)、乳多空病毒科(Papovaviridae)、多瘤病毒(Polyomavirus)、弹状病毒科(Rhabdoviridae)、和披膜病毒科(Togaviridae)的病毒。示例性的病原性病毒引起天花、流感、流行性腮腺炎、麻疹、水痘、埃博拉和风疹。示例性的病原性真菌包括念珠菌属(Candida)、曲霉属(Aspergillus)、隐球菌属(Cryptococcus)、组织胞浆菌属(Histoplasma)、肺囊虫属(Pneumocystis)和葡萄状穗霉属(Stachybotrys)。示例性的病原性细菌包括链球菌属(Streptococcus)、假单胞菌属(Pseudomonas)、志贺菌属(Shigella)、弯曲杆菌属(Campylobacter)、葡萄球菌属(Staphylococcus)、螺杆菌属(Helicobacter)、大肠杆菌(E.coli)、立克次体属(Rickettsia)、芽孢杆菌属(Bacillus)、博德特菌属(Bordetella)、衣原体属(Chlamydia)、螺旋体属(Spirochetes)、和沙门氏菌属(Salmonella)。在一些实施方案中,病原体受体Dectin-1可以用于产生CAR,所述CAR识别真菌,如曲霉属的细胞壁上的碳水化合物结构。在另一个实施方案中,可以基于识别病毒决定簇(例如,来自CMV和埃博拉的糖蛋白)的抗体制备CAR以中断病毒感染和病理学。
在一些实施方案中,可以将编码CAR的裸DNA或合适的载体引入受试者的T细胞(例如,从患有癌症或其它疾病的人患者获得的T细胞)中。通过使用裸DNA的电穿孔稳定转染T细胞的方法是本领域中已知的。参见例如美国专利No.6,410,319。裸DNA一般指以对于表达正确的方向在质粒表达载体中含有的编码本发明的嵌合受体的DNA。在一些实施方案中,裸DNA的使用可以减少产生表达经由本发明的方法产生的CAR的T细胞需要的时间。
或者,病毒载体(例如,逆转录病毒载体、腺病毒载体、腺相关病毒载体、或慢病毒载体)可以用于将嵌合构建体引入T细胞中。一般地,用于转染来自受试者的T细胞的编码CAR的载体在受试者的T细胞中一般应当是非复制的。基于病毒的大量载体是已知的,其中在细胞中维持的病毒的拷贝数足够低以维持细胞的活力。说明性的载体包括pFB-neo载体以及基于HIV、SV40、EBV、HSV或BPV的载体。
一旦确定经转染或转导的T细胞能够以期望的调节和在期望水平以表面膜蛋白表达CAR,可以确定嵌合受体在宿主细胞中是否有功能以提供期望的信号诱导。随后,可以向受试者再引入或施用经转导的T细胞以活化受试者中的抗肿瘤应答。为了促进施用,可以用合适的载体或稀释剂(其优选是药学上可接受的)将经转导的T细胞制备成药物组合物或者制备成适合于体内施用的植入物。本领域中已经描述了制备此类组合物或植入物的手段(参见例如Remington's Pharmaceutical Sciences,第16版,Mack编(1980))。在适当时,可以将表达CAR的经转导的T细胞配制成半固体或液体形式的制剂,如胶囊、溶液、注射液、吸入剂、或气雾剂,以用于其各自的施用途径的常见方式进行。可以利用本领域中已知的手段防止或最小化组合物的释放和吸收,直至它达到靶组织或器官,或者确保组合物的定时释放。一般地,优选地,采用不使表达嵌合受体的细胞失效(ineffectuate)的药学可接受形式。因此,期望地,可以将经转导的T细胞制备成含有平衡盐溶液,如汉克(Hanks)氏平衡盐溶液、或生理盐水的药物组合物。
IV.人工抗原呈递细胞
在一些情况中,aAPC可用于制备基于CAR的治疗组合物和细胞疗法产品。对于关于抗原呈递系统的制备和用途的一般指导,参见例如美国专利No.6,225,042、6,355,479、6,362,001和6,790,662;美国专利申请公开No.2009/0017000和2009/0004142;和国际公开No.WO2007/103009)。
可以使用aAPC扩增表达CAR的T细胞。在遇到肿瘤抗原期间,通过抗原呈递细胞对T细胞递送的信号可以影响T细胞编程及其随后的治疗效力。这已经刺激开发人工抗原呈递细胞的努力,所述人工抗原呈递细胞允许最佳控制对T细胞提供的信号(Turtle等,2010)。除了感兴趣的抗体或抗原外,aAPC系统还可以包含至少一种外源辅助分子。可以采用辅助分子的任何合适的数目和组合。辅助分子可以选自诸如共刺激分子和粘附分子等辅助分子。示例性的共刺激分子包括CD70和B7.1(又称作B7或CD80),其可以结合T细胞表面上的CD28和/或CTLA-4分子,从而影响例如T细胞扩增、Th1分化、短期T细胞存活、和细胞因子分泌,如白介素(IL)-2(参见Kim等,2004)。粘附分子可以包括碳水化合物结合糖蛋白,如选择蛋白、跨膜结合糖蛋白,如整联蛋白、钙依赖性蛋白,如钙粘蛋白和单次跨膜免疫球蛋白(Ig)超家族蛋白质,如胞间粘附分子(ICAM),其促进例如细胞与细胞或细胞与基质接触。示例性的粘附分子包括LFA-3和ICAM,如ICAM-1。例如,在美国专利No.6,225,042、6,355,479和6,362,001中例示了可用于选择、克隆、制备和表达示例性的辅助分子,包括共刺激分子和粘附分子的技术、方法和试剂。
优选地,为了变为aAPC而选择的细胞具有胞内抗原呈递、胞内肽运输、和/或胞内MHC I类或II类分子-肽加载的缺陷,或者是变温的(即,比哺乳动物细胞系对温度攻击的敏感性更小),或者拥有缺陷和变温特性两者。优选地,为了变为aAPC而选择的细胞还缺乏表达引入细胞中的外源MHC I类或II类分子和辅助分子组分的至少一种内源对应物(例如,如上文描述的内源MHC I类或II类分子和/或内源辅助分子)的能力。此外,aAPC优选地保留细胞在其修饰为产生aAPC前拥有的缺陷和变温特性。示例性的aAPC构成或衍生自与抗原加工(TAP)缺陷细胞系,如昆虫细胞系有关的转运蛋白。示例性的变温昆虫细胞系是果蝇细胞系,如Schneider 2细胞系(例如Schneider,J.m 1972)。美国专利No.6,225,042、6,355,479和6,362,001中提供了用于Schneider 2细胞的制备、生长和培养的说明性方法。
aAPC可以进行冷冻-融化循环。例如,可以通过将含有aAPC的合适容器与合适量的液氮、固体二氧化碳(干冰)、或类似的低温材料接触,从而快速发生冷冻来冷冻aAPC。然后,通过从低温材料取出aAPC并且暴露于周围室温条件,或者通过其中采用微温水浴或温手促进较短的融化时间的促进融化过程融化冷冻的aAPC。另外,可以冷冻aAPC,并且在融化前贮存延长的时间段。也可以融化冷冻的aAPC,然后在进一步使用前冻干。有利地,可以不利影响冷冻-融化程序的防腐剂,如二甲亚砜(DMSO)、聚乙二醇(PEG)和其它防腐剂可以从进行冷冻-融化循环的含有aAPC的培养基中缺乏,或者基本上被除去,如通过将aAPC转移到基本上缺乏此类防腐剂的培养基。
在其它优选的实施方案中,可以通过交联使异体核酸和aAPC的内源核酸失活,从而在失活后基本上不发生细胞生长、核酸的复制或表达。例如,可以在表达外源MHC和辅助分子,在aAPC表面上呈递此类分子,并且给呈递的MHC分子加载一种或多种选择的肽后的点时使aAPC失活。因而,虽然此类加载有失活的且选择的肽的aAPC使得基本上不能增殖或复制,但是它们可以保留选择的肽呈递功能。交联也可以导致基本上没有污染性微生物,如细菌和病毒的aAPC,而基本上不降低aAPC的抗原呈递细胞功能。因此,交联可以用于维持aAPC的重要APC功能,同时帮助减轻关于使用aAPC开发的细胞疗法产品的安全性的忧虑。关于与交联和aAPC相关的方法,参见例如美国专利申请公开No.20090017000,其通过引用方式并入本文。
IV.实施例
包括以下实施例以证明本发明的优选的实施方案。本领域技术人员应当理解,以下实施例中公开的技术代表由发明人发现的在本发明的实施中良好发挥功能的技术,并且因此可以认为构成其优选的实施模式。然而,根据本公开内容,本领域技术人员应当理解可以在不偏离本发明的精神和范围的前提下对公开的具体的实施方案进行许多改变,而仍然获得相同或相似的结果。
实施例1
材料和方法
临床级DNA质粒的产生
SB转座子CoOpCD19RCD28ζ/pSBSO在EF-1/HTLV杂合复合启动子(InvivoGen)下表达人密码子优化的(CoOp)第二代CoOpCD19RCD28ζCAR,所述EF-1/HTLV杂合复合启动子(InvivoGen)由延长因子-1a(EF-1a[Kim等,1990]和人T细胞白血病病毒(HTLV)的59非翻译区[Singh等,2011;Davies等,2010]构成。此DNA质粒的衍生描述于图S1中。从DNA质粒pCMV-SB11以顺式表达在巨细胞病毒(CMV)启动子下的SB转座酶SB11(Singh等,2011;Singh等,2008)。对这两种质粒完整测序,并且由Waisman Clinical Biomanufacturing Facility(Madison,WI)使用用于选择细菌菌株大肠杆菌DH5a的卡那霉素制备。
产生三重位点特异性重组DNA质粒:EZ-Build-CAR
使用来自上文描述的CAR(CoOpCD19RCD28z/pSBSO)的DNA序列,各部分CD19ScFv、铰链IgG4Fc和与CD3ζ信号传导结构域偶联的域结构域CD28跨膜和胞质溶胶部分侧翼有λ重组位点,由Geneart(Life Technologies)以PCR产物合成。将这三个部分单独插入pDonors221质粒中(通过酶BP clonase(两者都来自Invitrogen)。将三个质粒与三重位点特异性重组睡美人质粒通过酶LR PLUS clonase(Invitrogen)重组,以scFv-B-支架-C-信号传导结构域形式产生EZ-Build CD19CD28ζCAR(图1)。
细胞计数
使用台盼蓝排除法(Trypan-blue exclusion)区分活细胞与死亡细胞,并且使用Cellometer(Nexecelom Bioscience)计数(Singh等,2011)。
PBMC的分离
来自两名男性志愿者健康供体的白细胞去除术(Leukapheresis)产品购自KeyBiologics LLC(Memphis,TN)。通过我们修改Biosafe Sepax系统(Eysins,Switzerland)根据cGMP工作分离外周血单个核细胞(PBMC)。简言之,在闭合CS-900试剂盒上的所有夹子后,经由60mL注射器通过鲁尔锁紧连接器(Luer-lock connector)将100mL Ficoll(GEHealthcare)无菌转移到密度梯度介质袋(“ficoll袋”),并且加热使用手持式密封器(Sebra,型号#2380)密封的管道。将试剂盒用钉连接到1,000mL含有具有20mL 25%人血清清蛋白(HSA)(Baxter)的CliniMACS缓冲液(PBS/EDTA,Miltenyi,Cat#70026)(2%v/v,清洗缓冲液)以用于清洗的袋、最终产品袋[300mL具有偶联器的转移包(Baxter/Fenwal4R2014)]和试剂/血液袋。使用基于密度梯度的分离方案(v126),将注射器活塞加载到离心机室中,并且将Sepax盖加载到aAPC(克隆#4)中以选择性增殖CAR+T细胞。使用经c-照射的aAPC在数字上扩增经遗传修饰的T细胞。在VueLife细胞培养袋中在CM中使来自WCB的融化的aAPC增殖长达60天,并且使用Biosafe Sepax II收获程序收获。简言之,经由鲁尔锁定连接将CS-490.1试剂盒连接到300mL输出袋(转移包)。在坑中安装分离室,并且将管道插入光学传感器中,并且旋塞阀对准成T位置。在连接压力传感器线后,在支持架上吊起产品袋和上清液/血浆袋。修改的方案PBSCv302选自Sepax手册,并且有待处理的输入产品的体积(初始体积)设置为#840mL。在确认和试剂盒测试后,开始程序。在完成后,除去袋,闭合夹子,并且除去试剂盒。无菌取出来自最终产品袋的细胞,用清洗介质(Plasmalyte中的10%HSA)清洗两次,并且计数。使用CIS BIO国际放射器(IBL-437C#09433)照射(100Gy)aAPC,并且使用控制速率冷冻器(Planer Kryo 750)在冷冻保存介质中冷冻保存备用。使用加载有抗CD3(OKT3)的K562衍生的aAPC克隆#4来增殖尚未进行遗传修饰的对照(CAR-)自体对照T细胞。在称作加载培养基(LM)的含有0.2%乙酰半胱氨酸(Acetadote,CumberlandPharmaceuticals)的无血清X-Vivo 15(目录号04-744Q,Lonza)中将获自培养物的aAPC温育过夜。次日,洗涤细胞,使用γCell 1000 Elite Cs-137放射器(MDS Nordion)照射(100Gy),与1mg/106细胞功能级纯化的抗人CD3(克隆-OKT3,16-0037-85,eBioscience)一起以106个细胞/mL的浓度重悬于LM中,并且在3-D旋转器(Lab-Line)上在温和搅拌的情况下于4℃温育30分钟。在用LM洗涤三次后,细胞用于实验或者在气体层中在液氮中以等分试样冷冻以备用。
CAR+T细胞的制备
将融化的PBMC重悬于(i)人T细胞试剂盒(目录号VPA-1002,Lonza;对于2x107个细胞,一个小杯中100μL)中,与(ii)编码CD19RCD28CAR转座子的DNA质粒(CoOpCD19RCD28/pSBSO)(每个小杯每2x107个PBMC的15μg超螺旋DNA),和(iii)编码SB11转座酶的DNA质粒(pCMVSB11)(每个小杯每2x107个PBMC的5μg超螺旋DNA)。将此混合物立即转移到小杯(Lonza),使用Nucleofector II(程序U-14,Amaxa/Lonza)电穿孔(限定培养第0天),在10%RPMI完全培养基中静止2至3小时,并且在半培养基更换后,在37℃,5%CO2温育过夜。次日,收获细胞,计数,通过流式细胞术测定表型,并且以比率1:2(CAR+T细胞:aAPC)与经c-照射的aAPC共培养,其标记为培养第1天和7天刺激周期的开始。分别从第1天和第7天起按星期一-星期三-星期五日程表添加IL-21(目录号AF-200-21,PeproTech)和IL-2(目录号NDC65483-116-07,Novartis)。NK细胞可以防止CAR+T细胞的数字扩增,尤其在它们的过度生长在组织培养过程中早期出现的情况下。因此,若CD3-CD56+细胞$10%,则在含有25%HAS的CliniMACS缓冲液(80mL/107个细胞)中在LS柱(目录号130-042-401,Miltenyi Biotech)上使用CD56珠粒(目录号70206,Miltenyi Biotech,20mL珠/107个细胞)实施CD56消减。
CAR-对照T细胞的产生
作为对照,在7天刺激循环中以1:1的比率将5x106个mock转染的PBMC与经照射的并且加载有抗CD3(OKT3)的K562衍生的aAPC克隆#4共培养。从培养第1天起给所有培养物补充IL-21(30ng/mL),并且在开始培养后7天开始补充IL-2(50U/mL)。随后,每隔一天添加所有细胞因子。
细胞的免疫表型
在4℃使用100mL FACS缓冲液(2%FBS,0.1%叠氮化钠)中的抗体使染色细胞30分钟。使用FACSCalibur(BD Bioscience)进行采集,并且使用FCS Express 3.00.0612分析。
铬释放测定
使用经51Cr标记的靶细胞,在标准的4小时铬释放测定中对T细胞评估其细胞毒性。一式三份在1x105、0.5x105、0.25x105、0.125x105底部板(Costar)上铺板T细胞。在温育后,将50μL上清液收获到LumaPlate(Perkin Elmer)上,在TopCount NXT(Perkin Elmer)上读数,并且如下计算特异性裂解百分比:

实施例2
CD19+CAR的产生
使用上文在实施例1中描述的方法(称为“EZ”法)产生CD19+CAR。比较这些CD19+CAR(CD19CAR)与经由先前的方法产生的临床级(“CG”)CD19CAR。
数据显示了三重位点重组系统产生与临床级CD19CAR(CG)相似的CD19CAR(EZ)。质粒中由重组位点留下的足迹不干扰CAR的表达和功能(图2A-B)。
实施例3
含有(CD8,CD28)跨膜结构域和(CD28,4-1BB)信号传导结构域的CAR的产生
测试的各个CAR已经显示相似的扩增、细胞毒性和Th1细胞毒性。CD19-BB-z已经显示了较低的Th2细胞因子产生;在体内,它在控制小鼠中的疾病方面是有效的(MolecularTherapy 17(8):1453–1464,2009)。不过,由于细胞在没有抗原性刺激的情况下在体外存活的事实而存在忧虑。
下文在表2中显示了临床中的CAR。在一些实施方案中,本发明的CAR没有如下文表2中显示的特定构建体。或者,在一些实施方案中,可以使用本发明的方法产生具有下文表2中提到的CAR特征,不过与目前在临床中使用的CAR截然不同的CAR的另一种变化。

图3中说明了具体的CAR构建体设计。如图3中显示,产生使用CD19scfv、CD8a铰链或IgG4Fc茎部、CD8跨膜(TM)或CD28TM或CD137TM和经CD28或CD137膜内结构域连同CD3ζ膜内结构域信号传导的组合的各个CAR的示意图。
然后,将图3中显示的CAR构建体克隆到含有SIM和FRA标签的睡美人质粒中以允许在使用常见的CVseq7引物扩增时在竞争再增殖研究中追踪。图4中显示了睡美人追踪质粒。
使用Amaxa Nucleofector II将图3中显示的CAR构建体电穿孔到T细胞中,并且在存在细胞因子(IL2,IL-21)的情况下与aAPC共培养28天。显示了电穿孔后1天(第1天)和与aAPC共培养28天(第28天)后的CAR表达。显示了CD3和CAR的点图,其中使用CD3和抗CD19scfv特异性Ab区分T细胞和CAR。图5中显示了CAR表达结果。
随时间对图3中显示的CAR构建体评估CAR表达持续28天,并且显示。在21天后,大多数培养物具有>80%CAR表达。图6中显示了CAR表达动力学。

在共培养28天后,对CAR+T细胞(表达图3中描述的CAR)评估属于记忆(CD45RA、CCR7、CD27)、活化(CD69、HLA-DR)、细胞毒性(穿孔蛋白、粒酶B)、耗竭/衰老(CD57、KLRG1、PD1)、和粘附(CD39、CD150)的标志物表达。图8A-B中显示了此延伸表型的结果。
使用蛋白质印迹对CAR+T细胞(表达图3中描述的CAR)评估CD3ζ的表达。在变性条件下运行细胞裂解物,转移,并且使用一级小鼠抗人CD3ζmAb和偶联有HRP的山羊抗小鼠IgG使用SuperSignal West Femto Maximum Sensitivity底物测量嵌合CD3ζ的表达。相对于CAR构建体的大小,观察到52、71和78kD的嵌合CD3ζ条带。图9中显示了这些蛋白质印迹结果。
在第1天和此后每7天持续28天,用K562aAPC刺激用CAR构建体(图3中描述)电穿孔的T细胞。在每个刺激循环结束时,使用台盼蓝排除方法计数细胞,并且在CD3和CAR表达方面进行表型分析。图10中显示的图描绘了随时间总共、CD3、和CAR+T细胞的推断的细胞计数。
测量细胞的扩增。通过与(电穿孔后)第1天比较计数在共培养的第14天、第21天和第28天计算总细胞(图11)和CAR+(图12)T细胞的倍数扩增。图11和图12中显示了结果。
对CAR表达T细胞(CAR+T细胞)测量细胞毒性。在标准的4小时铬释放测定法中,与CD19-EL-4相比,对CAR+T细胞(表达图3中描述的CAR)评估其针对CD19+肿瘤靶标(Daudiβ2m、NALM-6和CD19+EL-4)的细胞毒性。在图13、图14、图15和图16中显示了结果。
胞内IFN-γ产生。在存在蛋白质转运抑制剂的情况下将CAR+T细胞(表达图3中描述的构建体)与(CD19+和CD19-)刺激细胞一起温育4-6小时,固定,透化,并且用IFN-γ特异性mAb染色。PMA-离子霉素用作阳性对照。图17、图18、图19和图20中显示了胞内IFN-γ产生的结果。
针对SB11转座酶的PCR。在热循环仪中使用SB11特异性引物扩增从CAR+T细胞分离的DNA(图3)。GAPDH用作持家基因,并且线性化pCMV-SB11质粒,来自表达SB11的Jurkat细胞的基因组DNA用作阳性对照。CAR-细胞(无DNA)用作阴性对照。图21中显示了这些PCR结果。
使用定量PCR(qPCR)测量CAR拷贝数。通过使用对IgG4Fc茎部和反向/同向重复序列(IR/DR)特异性的引物和探针扩增基因组DNA来评估细胞中的CAR转基因(图3中显示的CAR构建体)的整合数目。RNA酶P基因用作内部对照,并且表达单拷贝CAR的Jurkat细胞系用于产生标准曲线。图22中显示了结果。
接着,测量CAR+T细胞的自主生长的存在或缺乏。通过在缺乏细胞因子和aAPC的情况下培养T细胞来监测并测量CAR+T细胞(表达图3中显示的CAR构建体)的异常生长。每7天计数细胞,并且计算并绘图活细胞/死亡细胞百分比(从第1天起)。如图23中显示,观察到超过80%的T细胞到第14天是死亡的,显示缺乏自主生长。
各个CAR可以进行表达(>80%),扩增(约1010),并且在相似程度上有细胞毒性(约60%,Daudi)。支架结构域(IgG4或CD8α)用于建立CAR,并且不影响表达或效力。跨膜结构域(CD8,CD28)不影响效力。4-1BB跨膜结构域(216)影响表达(抗scFv Ab),但是不影响细胞毒性和细胞因子产生。信号传导结构域、CD28和4-1BB的组合没有叠加效应。CAR+T细胞展现出记忆/效应子表型。与其它CAR相比,仅含有4-1BB结构域(212、214、217)的CAR具有更高的CCR7表达。细胞表达记忆(CD27hi、CD45RAhi、CCR7lo)、活化(CD69med、HLA-DRhi)、细胞溶解(粒酶hi,穿孔蛋白lo),和粘附(CD39hi,CD150lo)的标志物,但是观察到可忽略不计量的抑制性标志物(CD57、PD1、KLRG1)。所有CAR,包括含有4-1BB结构域域的CAR,缺乏SB11转座酶,并且不自身增殖。
实施例4
含有CD3-ζ的CAR的产生
在本实施例中提供了含有CD3ζ的CAR。图24中显示了CAR设计的总图。如图24中显示,显示了CAR设计(图24,右)与抗体分子(图24,左)的比较。
图25中显示了CD3ζ序列。图25中显示了CD3ζ及其同等型的序列。CAR设计包括CD3ζ(同等型1),其形成膜内结构域信号传导部分之一并且具有三个ITAM。
图26和图27中显示了具体的CAR构建体。图26显示了具有长(IgG4)、中等(CD8a铰链)和小(IgG 12aa)茎部,经由CD28或CD137膜内结构域进行信号传导的CD19特异性CAR的示意图。图27中显示了具有不同茎部和信号传导的CAR分子的命名。
测量CAR表达。在电穿孔后1天(第1天)和在aAPC上共培养28天(第28天)后测量CAR(如图26中描述)的表达。图28中显示了CD3和CAR(如通过CD19scfv特异性mAb测量)的点图。
对CAR测量扩增动力学。在7天刺激循环中在aAPC上共培养用CAR构建体(图26中显示)电穿孔的T细胞。对细胞计数,并且评估CD3和CAR的表达。图29和图30中显示了结果。
测量CAR+T细胞的细胞毒性。在28天共培养结束时,在铬释放测定法中评估CAR+T细胞(表达图26中显示的构建体)的针对肿瘤靶标的细胞毒性。如图31中显示,对针对CD19+和CD19-肿瘤靶标的CD19RCD28(CAR 194)和CD19RCD137(CAR 217)CAR以各个效应物与靶标比率测量细胞毒性百分比。如图32中显示,获得在20:1的E:T比率时CAR+T细胞(表达图26中显示的CAR构建体)对CD19+EL-4的裂解百分比的数据。测量CAR+T细胞(表达图26中显示的构建体)上的CD27、CD62L、CD28和CCR7的表达百分比,并且在图33中显示了结果。
对CAR+T细胞测量胞内细胞因子产生。在存在蛋白质转运抑制剂的情况下将刺激细胞(CD19+和CD19-)与CAR+T细胞(表达图26中显示的CAR)一起温育4小时,并且用IFN-γ和IL-2mAb染色。PMA-离子霉素充当阳性对照,并且单独的T细胞充当阴性对照。图34显示了刺激后的IFN-γ生成细胞的百分比。图35显示了与细胞刺激混合物(PMA-离子霉素)一起温育后IFN-γ和或IL-2生成细胞的分解。
对CAR+T细胞测量自主生长的存在或缺乏。对CAR+T细胞(表达图26中描述的CAR)评估其在缺乏外部刺激(细胞因子和aAPC)的情况下缺乏异常生长持续18天。在18天结束时,超过80%的细胞是死亡的,显示了缺乏不想要的生长。如图36中显示,观察到自主生长的缺乏。
在CAR+T细胞中测量CAR拷贝数。通过qPCR使用对IgG4-Fc和IR/DR区特异性的引物/探针评估整合的CAR分子的拷贝数。如图37中显示,使用IR/DR探针观察到整合的CAR拷贝数(图26中显示的CAR)。如图38和图39中显示,在CAR构建体(图3中显示的两种CAR构建体和图26中显示的CAR构建体)的表和图形形式中提供了CAR拷贝数数据的汇编,如在两个不同实验中测试(P491;C714和GCR357861)。
这些数据显示了可以在如本文中描述的培养系统中体外表达并培养具有各种间隔区的CAR。观察到所有CAR具有相似的CAR表达。在具有CD8铰链区的CAR中观察到CD19+EL-4s的最大细胞毒性。对测试的所有CAR观察到相似的CD62L和CD28表达。除了含有IgG4-Fc茎部的CAR外,在所有CAR中观察到如通过CAR拷贝数测量的高整合频率。通过PCR观察到自主生长和SB11的缺乏。与先前的报告形成对比,在CAR中包含12aa间隔区在这些研究中没有赋予改善的功能性。
实施例5
从主要组分快速装配CAR
与临床级CD19RCD28mζCAR+T细胞(CG CAR)平行,发明人使用EZ CAR平台产生经由嵌合CD28/CD3-ζ活化的CD19特异性CAR。将临床级CD28/CD3-ζ和EZ CAR CD19RCD28mζCAR序列两者插入睡美人转座子载体中,并且电穿孔到T细胞中。在电穿孔后,在存在CD19+人工抗原呈递细胞(又称作活化和增殖细胞,或AaPC)的情况下培养T细胞,用于T细胞的抗原特异性扩增。通过流式细胞术(Fc+表达)每周测量T细胞表面中的CAR表达,显示了临床级CD19CAR T细胞和EZ CD19CAR T细胞中的相似CAR表达。还实施铬释放测定(CRA)以评估通过EZ CAR平台产生的T细胞CD19CAR+针对肿瘤细胞的杀伤功能。在温育4小时后,通过EZCAR T细胞观察到特异性细胞裂解的百分比为52%,并且通过CG CAR T细胞为49%。
这些结果证明了使用这些方法产生功能性CAR+T细胞。然后,发明人使用与含有CAR分子的以下三种组分的质粒文库组合的如上文描述的方法进行CAR的快速产生:(i)抗CD19scFv,(ii)具有不同尺寸的5种铰链(长-IgG4a和IgG4ΔEQ,中等-CD8α,短-t-20AA和t-12AA)和(iii)具有CD3ζ结构域的7种信号传导结构域(CD27、CD28、CD28ΔY173→F173、CD134、CD137、CD278)的不同组合。用含有CAR转基因的质粒转染HEK 293细胞用于筛选27种不同CAR构建体以确保细胞表面中的CAR蛋白的表达。使用iQueTM筛选器(Intellicyt,Albuquerque,NM)(一种高通量流式细胞仪)进行个别CAR分子的高通量测试,其中使用表达荧光粒酶B报告物或GFP的工程化靶细胞进行细胞毒性测定。图40A-E中显示了结果。
使用IntelliCyt’s iQueTM,进行额外的实验以筛选CAR分子。iQueTM使用高通量流式细胞术,一种通过使用复用(multiplexing)性能和逐个细胞分析(cell-by-cellanalysis)研究大群体产生信息的补充技术。发明人采用此技术来告知用CAR组修饰的T细胞的治疗潜力。可以针对活力,及活化信号(例如,CD25的上调)、细胞因子释放、和杀伤方面对来自孔的T细胞染色。因此,发明人调适iQue筛选器,并且利用其进行复用的基于珠粒的细胞因子检测和基于细胞的测定的能力。获得的结果指示此技术可以用于测试通过EZ CAR平台产生的大量不同CAR T细胞。使用IntelliCyt’s iQueTM产生数据,其中对CAR T细胞的2个群体评估其杀伤靶细胞的能力。图41A-B和图42中显示了结果。这些结果证明了CAR分子有活性,并且iQueTM方法可以有效用于评估CAR活性。

Claims (8)

2.一种经转化的T细胞,其表达根据权利要求1所述的多肽。
3.根据权利要求2所述的经转化的T细胞,其中所述细胞是永生化细胞。
4.根据权利要求2所述的T细胞,其中所述T细胞是αβT细胞、γδT细胞或NKT细胞。
5.一种药物制剂,所述药物制剂包含根据权利要求2-4中任一项所述的经转化的T细胞。
6.一种编码如下所述多肽的核酸,所述多肽特异性结合CD19并引起或促进包含所述多肽的免疫细胞的至少一种正常效应子功能的活化并且是由SEQ ID NO:5所示DNA序列、SEQID NO:5第1-1485所示核酸序列或SEQ ID NO:5第67-1485所示核酸序列编码的。
7.根据权利要求6所述的核酸,其中所述核酸被包含在T细胞中。
