背景技术
伤科跌打片活血散瘀,消肿止痛。用于跌打损伤,伤筋动骨,瘀血肿痛,闪腰岔气。市售伤科跌打片由于组方和工艺原因,疗效不甚理想,制剂采用传统工艺制备,存在崩解迟缓、疗效低等不足。
发明内容
本发明为克服上述不足,提供一种崩解速度快、疗效高的伤科跌打片及其制备方法。
发明实施方案如下:
取海风藤26g,广藿香26g,小茴香78g,罗布麻叶78g,旋覆花7.8g,莪术(制)30g,青皮15g,香附(醋制)32g,当归32g,三七26g,续断30g,牡丹皮30g,蒲黄30g,防风40g,延胡索(醋制)7.5g,枫香脂(制)42g,红花30g,郁金30g,木香18g,乌药26g,柴胡22.5g,粉碎成60目粗粉,采用二氧化碳超临界萃取法提取,萃取压力21~42Mpa,萃取温度22~38℃,分离器压力11~19Mpa,分离器温度41~58℃,分离时间2.4~3.3小时,二氧化碳流量每小时23~39L,得提取液;取提取液64~76℃减压干燥,得干膏;取干膏加入枸橼酸80~120g,采用高能纳米冲击磨粉碎成粒径200~300nm的混合干膏粉;取混合干膏粉,微晶纤维素35~45g,羟丙纤维素35~45g,交联羧甲基纤维素钠35~45g,硅酸镁铝15~25g,氯化钠5~15g,甘露醇4~6g,麦芽糊精4~6g,混合均匀,用50~70%乙醇湿法制粒,60℃~80℃干燥,外加海藻酸钠7~9g,硼酸1~3g,整粒,压片,制得伤科跌打片1000片。

广藿香:中国药典2005年版一部标准。本品为唇形科植物广蓄香Pogostemoncablin(Blanco)Benth.的干燥地上部分。枝叶茂盛时采割,日晒夜闷,反复至干。
小茴香:中国药典2005年版一部标准。本品为伞形科植物茴香FoeniculumvulgareMill.的干燥成熟果实。秋季果实初熟时采割植株,晒干,打下果实,除去杂质。
罗布麻叶:中国药典2005年版一部标准。本品为夹竹桃科植物罗布麻ApocynumvenetumL.的干燥叶。夏季采收,除去杂质,干燥。
旋覆花:中国药典2005年版一部标准。本品为菊科植物旋覆花InulajaponicaThunb.或欧亚旅覆花InulabritannicaL.的干燥头状花序。夏、秋二季花开放时采收,除去杂质,阴干或晒干。
莪术:中国药典2005年版一部标准。本品为姜科植物莪术CurcumaphaeocaulisVal.、广西莪术CurcumakwangsiensisS.G.LeeetC.F.Liang或温郁金CurcumawenyujinY.H.ChenetC.Ling的干燥根茎。后者习称“温莪术”。冬季茎叶枯萎后采挖,洗净.蒸或煮至透心.晒干或低温干燥后除去须根及杂质。
青皮:中国药典2005年版一部标准。本品为芸香科植物橘CitrusreticulataBlanco及其栽培变种的干燥幼果或未成熟果实的果皮。5-6月收集自落的幼果,晒干,习称“个青皮”;7-8月采收未成熟的果实,在果皮上纵剖成四瓣至基部,除尽瓤瓣,晒干,习称“四花青皮”。
香附:中国药典2005年版一部标准。本品为莎草科植物莎草CyperusrotundusL.的干燥根茎。秋季采挖,燎去毛须,置沸水中略煮或蒸透后晒干或燎后直接晒干。
当归:中国药典2005年版一部标准。本品为伞形科植物当归Angelicasinensis(Oliv.)Diels的干燥根。秋末采挖,除去须根及泥沙,待水分稍蒸发后,捆成小把,上棚,用烟火慢慢熏干。
三七:中国药典2005年版一部标准。本品为五加科植物三七Punaxnotoginseng(Burk.)F.H.Chen的干燥根及根茎。秋季花开前采挖,洗净,分开主根、支根及根茎,干燥。支根习称“筋条”,根茎习称“剪口”。
续断:中国药典2005年版一部标准。本品为川续断科植物川续断DipsacusasperoidesC.Y.ChengetT.M.Ai的干燥根。秋季采挖,除去根头及须根,用微火烘至半干,堆置“发汗”至内部变绿色时,再烘干。
牡丹皮:中国药典2005年版一部标准。本品为毛茛科植物牡丹PaeoniasuffruticosaAndr.的干燥根皮。秋季采挖根部,除去细根和泥沙,剥取根皮,晒干。
蒲黄:中国药典2005年版一部标准。本品为香蒲科植物水烛香蒲TyphaangustifoliaL.、东方香蒲TyphaorientalisPresl或同属植物的干燥花粉。夏季采收蒲棒上部的黄色雄花序,晒干后碾轧,筛取花粉。剪取雄花后,晒干,成为带有雄花的花粉,即为草蒲黄。
防风:中国药典2005年版一部标准。本品为伞形科植物防风Saposhnikoviadivaricata(Turcz.)Schischk.的干燥根。春、秋二季采挖未抽花茎植株的根.除去须根及泥沙,晒干。
延胡索
枫香脂:中国药典2005年版一部标准。本品为金缕梅科植物枫香树LiquidambarformosanaHance的干燥树脂。7、8月间割裂树干,使树脂流出.10月至次年4月采收,阴干。
红花:中国药典2005年版一部标准。本品为菊科植物红花CarthamustinctoriusL.的干燥花。夏季花由黄变红时采摘,阴干或晒干。
郁金:中国药典2005年版一部标准。本品为姜科植物温郁金CurcumawenyujinY.H.ChenetC.Ling、姜黄CurcumalongaL.、广西莪术CurcumakwaugsienisS.G.LeeetC.F.Liang或蓬莪术Curcumapha-eocaulisVal.的干燥块根。前两者分别习称“温郁金”和“黄丝郁金”,其余按性状不同习称“桂郁金”或“绿丝郁金”。冬季茎叶枯萎后采挖,除去泥沙及细根,蒸或煮至透心,干燥。
木香:中国药典2005年版一部标准。本品为菊科植物木香AucklandialappaDecne.的干燥根。秋、冬二季采挖,除去泥沙及须根,切段。大的再纵剖成瓣,干燥后撞去粗皮。
乌药:中国药典2005年版一部标准。本品为樟科植物乌药Linderaaggregata(Sims)Kosterm.的干燥块根。全年均可采挖,除去细根,洗净,趁鲜切片.晒干,或直接晒干。
柴胡:中国药典2005年版一部标准。本品为伞形科植物柴胡BupleurumchinenseDC.或狭叶柴胡BupleurumscocrzonerifoliumWilld.的干燥根。按性状不同,分别习称“北柴胡”及“南柴胡”。春、秋二季采挖,除去茎叶及泥沙,干燥。

微晶纤维素:中国药典2010年版二部标准。
羟丙纤维素:中国药典2010年版二部标准。
交联羧甲基纤维素钠:中国药典2010年版二部标准。
硅酸镁铝:中国药典2010年版二部标准。

以上伤科跌打片所用到的原材料均可从医药公司购买得到,只要满足国家标准均可用来实施本发明方案。
上述发明方案中所用术语为药学专用术语,如“减压”、“制”、“醋制”等皆遵从中国药典规定和相关药学规范。
本发明中的单位g也可以是其它重量份,不影响本发明方案的实施。
本发明方案中所述的设备市场均有销售,并不限于典型生产厂家,只要技术指标能够达到要求,均可用来实现本发明。

取海风藤26g,广藿香26g,小茴香78g,罗布麻叶78g,旋覆花7.8g,莪术(制)30g,青皮15g,香附(醋制)32g,当归32g,三七26g,续断30g,牡丹皮30g,蒲黄30g,防风40g,延胡索(醋制)7.5g,枫香脂(制)42g,红花30g,郁金30g,木香18g,乌药26g,柴胡22.5g,粉碎成60目粗粉,采用二氧化碳超临界萃取法提取,萃取压力21Mpa,萃取温度22℃,分离器压力11Mpa,分离器温度41℃,分离时间2.4小时,二氧化碳流量每小时23L,得提取液;取提取液64℃减压干燥,得干膏;取干膏加入枸橼酸80g,采用高能纳米冲击磨粉碎成粒径200~300nm的混合干膏粉;取混合干膏粉,微晶纤维素35g,羟丙纤维素35g,交联羧甲基纤维素钠35g,硅酸镁铝15g,氯化钠5g,甘露醇4g,麦芽糊精4g,混合均匀,用50%乙醇湿法制粒,60℃干燥,外加海藻酸钠7g,硼酸1g,整粒,压片,制得伤科跌打片1000片。

以上实施例说明,采用本发明实施方案的极端条件和优化条件均能制成伤科跌打片。
下面以实施例3制得的伤科跌打片考察本发明的实际效果:

上述结果表明,本发明制备的伤科跌打片相对于市售伤科跌打片具有崩解速度快、生物利用度高等显著优点。
(二)实施例3伤科跌打片和市售伤科跌打片治疗跌打损伤,伤筋动骨,瘀血肿痛,闪腰岔气临床疗效观察
1病例情况
统计门诊和住院病例,共观察跌打损伤,伤筋动骨,瘀血肿痛,闪腰岔气病例153例,平均年龄46岁。将患者分为两组,试验组服用实施例3伤科跌打片,对照组服用市售伤科跌打片。
2疗效评定标准
依据中药新药治疗临床研究指导原则中医证候疗效判定标准:
临床痊愈:中医临床症状、体征消失或基本消失,证候积分减少≥95%。
显效:中医临床症状、体征明显改善,证候积分减少≥70%。
有效:中医临床症状、体征均有好转,证候积分减少≥30%。
无效:中医临床症状、体征均均无明显改善或者加重,证候积分减少<30%。
计算公式:[(治疗前积分-治疗后积分)÷治疗前积分]×100%。

上述临床疗效观察结果表明,本发明制备的伤科跌打片在治疗跌打损伤,伤筋动骨,瘀血肿痛,闪腰岔气时,疗效显著高于市售伤科跌打片,p<0.05。
