Description

在常规的麻醉中,给患者配给对大脑、脊髓和神经系统具有不同影响的麻醉药。例如,在麻醉过程中,配给麻醉剂或镇静剂以使得患者无意识并且使患者安静,还可能结合止痛剂来抑制疼痛。

在该模型中,例如在药代动力学-药效动力学模型中,患者的身体逻辑上被划分成在其之间发生药物的交换的多个区室。患者包括例如与患者的肺对应的肺区室、与患者的大脑对应的脑区室、与患者的血液循环对应的血浆区室、与患者的肌肉和器官对应的肌肉区室、与患者的脂肪和结缔组织对应的脂肪区室、以及还可能的其他区室。在麻醉过程中,将麻醉药如异丙酚、芬太尼、瑞芬太尼和/或肌肉松弛药静脉注射入患者的血浆区室,并且因此注射剂进入患者的血流。根据血流,药物被分布在患者的身体内并且进入患者的其他区室,例如肺区室和脑区室。在该模型中,能够例如通过表示区室之间的传送速率的所谓的传送速率常数来描述药物从一个区室至另一个区室的传送。
在药代动力学-药效动力学模型内,例如,传送速率常数表示血浆区室与肺区间之间、脑区室与其他区室之间的传送速率。通过这些传送速率常数来限定不同区室中的浓度的时间推迟。在本文中,血浆区室代表不同区室之间的连杆机构,因为药物交换例如主要发生在血浆区室与肺区室之间以及血浆区室与脑区室之间,但是并不直接在肺区室与脑区室之间。在该模型中还考虑了区室的体积。
在本领域中已知并且例如在WO 2005/084731 A2中或者通过以下描述了这样的药代动力学-药效动力学模型:M.Coppens等人,“Study of the time course of theclinical effect of propofol compared with the time course of the predictedeffect-side concentration:performance of three pharmacokinetics-dynamicmodels”,英国麻醉学杂志,104(4):452-8 (2010),以及J.-O.Hahn等人,“A directdynamic dose-response model of propofol for individualized anaesthesia care”,生物医学工程的IEEE 会报,第59卷,第2号,2012年2月,WO 2005/084731 A2的内容通过引用被合并至本文中。
借助于所提出的方法,对患者的不同区室的时间相关的药物浓度进行预测的预测模型——始于基于经验数据和关于患者的初始人口统计信息的初始模型——通过根据所测量的患者的第二区室例如肺区室的药物浓度值迭代地至少调整该模型而被个性化。对于模型的迭代个性化,可以贯穿控制过程迭代地重复步骤(d)至步骤(g),使得在不同的测量时间处迭代地测量患者的第二区室例如肺区室中的药物浓度,并且基于这样的测量的药物浓度值来迭代地修改该模型。借助于测量的药物浓度值,因此,调整该模型以反映特定患者内的关于患者的不同区室之间的药物的交换和分布的个人条件和行为。因此,通过根据在配给过程期间为患者特别测量的测量值对模型进行个性化,该模型变得更准确并且允许对关注的区室中例如脑区室中的实际药物浓度进行更精确的预测,在所述脑区室中,麻醉药如异丙酚导致对患者的麻醉效果。
可以例如主要通过脂肪和肌肉块的体积来驱动用于在方法期间使模型优化的拟合变量,同时整个块应是恒定的。最敏感的区室是对血浆浓度有影响的肌肉块(新陈代谢使药物如异丙酚消失)和脂肪块。异丙酚例如是亲脂性的并且因此优选地存储在脂肪块中。因此,肌肉体积和脂肪体积可以是例如对PK/PD模型进行重新校准的、用于配给异丙酚的第一选择变量。用于肌肉和脂肪的相关的传送系数由于它们通过生理测量被预定的事实可以是用于对该模型进行重新校准的第二选择变量。脑容积和血浆区室与脑区室之间的传送系数优选地并不认为要被用作拟合变量。应当优选地保持大脑的生理值,因此呈现了使系统中的变异性减少并且使收敛速度增大的边界。
如果例如在患者的肺区室和脑区室中并行测量表示第二区室中的药物浓度的药物浓度值的情况下,肺的生理值还可以用作拟合变量。对于药物或者患者关于其已知的具体的生理行为,可以优选其他变量。
优选地,由根据该方法的步骤来操作的控制装置来执行该方法。本文中的测量值通过与控制装置连接的测量装置来获得,该测量装置被构造成测量患者的呼吸中的药物浓度或者测量表示患者的大脑中的药物浓度的信号。
可以例如通过在预定数目的呼吸周期期间连续测量患者的呼吸中的药物浓度并且通过对贯穿不同呼吸阶段的所测量的药物浓度进行求平均来获得测量值。因此,对于该测量,没有在单独的呼吸阶段之间进行区分,但是在所有呼吸阶段期间连续进行测量。
可替选地,还可以在呼吸阶段之间进行区分,并且仅在呼出期间且因此仅在呼气末的呼出气中可以测量患者的呼吸中的药物例如异丙酚的浓度。
预定呼吸周期的数目可以例如是六。然而,还可设想获得测量值的呼吸周期的其他数目,例如两个呼吸周期、四个呼吸周期或八个呼吸周期。
可替选地,可以在单次呼吸周期期间以高的时间分辨率连续进行测量以测量时变的药物浓度(其具有起伏)。
可替选地,可以例如通过患者的大脑中的EEG指标值来获得测量值。
控制装置将表示药物剂量曲线的控制信号提供给输注装置,因此该输注装置被命令给患者注射所表示的药物剂量。输注装置是例如计算机控制的输注泵如将药物剂量静脉注射入患者的血流并且因此注射入患者的血浆区室的注射泵。
在本文中,有益地,在给患者的药物剂量的配给几乎达到连续流量状态或完成并且已达到血流中的药物的均匀分布之后获得第一测量值。例如,控制装置可以命令输注装置配给特定时间窗上的药物剂量曲线。当该时间窗上的药物剂量的配给几乎达到连续流量状态或几乎完成时,获得另一个测量值,并且根据所获得的测量结果来调整该模型以获得与测量结果的更好拟合。
在患者的呼吸中有益地测量药物浓度以获得肺区室中的实际药物浓度的表示。然而,实际需要的信息不是呼吸中的药物浓度,而是肺区室中的药物浓度的值。因此,必须执行例如表示呼出气体中的药物浓度的测量信号至肺区室的药物浓度的转换。为此,使用转换函数,其用于将所测量的信号(从对患者的呼吸的测量来获得的)转换成表示肺区室中的实际药物浓度的测量值。转换函数作用于在其中进行测量以避免时间推迟的第二区室中。
假设配给的药物是唯一影响EEG指标值的药物,在测量值是会转换成脑区室中的药物浓度值的EEG指标值的情况下,还可以使用等效转换。
最好事先(即在执行实际的方法之前)在对其的前瞻性研究内生成转换函数,在患者的呼吸中生成测量值时执行下面的步骤:
在前瞻性研究中,对药物进行配给使得几乎实现稳态条件,即多区室 PK/PD模型表示以下平衡:不同区室中的药物浓度不再随时间改变(这样做是为了使不同区室之间的时间推迟效应最小化)。
在患者的呼吸中(测量患者的呼吸中的例如异丙酚)获得测量值,这样在已达到稳态条件时生成测量信息。
同时获得血液样本,并且通过实验室设备在血液样本中测量药物浓度,因此获得血浆区室中的药物浓度的精确值。
以血浆区室中所计算的药物浓度变得与血液样本中所测量的药物浓度相同的方法对PK/PD模型进行校准。
在测量时间处使用所校准的PK/PD模型计算肺区室中的药物浓度,因此获得与肺区室中的测量时间处的实际药物浓度对应的测量时间处的测量信号。通过相关地设置实际药物浓度和测量的信号来获得无时间推迟的转换函数,其可以用于将表示例如患者的呼吸中的药物浓度的测量信号转换成表示肺区室中的实际药物浓度的测量值。
可以针对不同的稳态条件(不同的连续剂量率)重复步骤以完成用于不同的稳态条件的范围的转换函数。
转换函数在几乎稳态的条件以前瞻性研究被生成并且存储于系统中。然后,在实际的药物配给过程期间,将转换函数用于患者,使得在知道药物剂量率曲线的情况下并且通过使用一般条件下的PK/PD模型来将例如在患者的呼吸中所测量的信号转换成肺区室中的药物浓度值。
在测量值是脑区室中的EEG指标值并且配给的药物是唯一影响 EEG指标值的药物的情况下,可以执行等效步骤。
因此,通过前瞻性研究来预定义转换函数。可以将转换函数存储在测量装置或者存储在控制装置(还被定义为输注管理器)中。转换函数将呼出气体中的例如异丙酚浓度转换成异丙酚的肺浓度,从而表示测量与 PK/PD模型之间的联系。在本文中,转换函数将测量信号(例如在呼出气体中获得的)与已执行测量的相关联的区室中的相关浓度联系起来。这避免了时间推迟效应并且使得能够在任何时间使用所测量的信号与相关联的区室(例如肺区室)中的相关联的药物浓度之间的简单的转换函数。
此外,该目的还通过控制用于给患者配给药物的输注装置的控制装置来实现,控制装置被构造成:
(a)提供对患者的多个区室中的时间相关的药物浓度进行预测的模型;
(b)设置要在患者的区室中至少之一中达到的靶浓度值;
(c)使用模型来确定要给患者的多个区室中的第一区室配给的药物剂量,使得区室中至少之一中的靶浓度值与预测的稳态药物浓度之间的差小于预定阈值;
(d)将表示药物剂量的控制信号作为输出提供给用于给患者配给该药物剂量的输注装置;
(e)在测量时间处获得表示多个区室中的至少第二区室中的测量的药物浓度的至少一个测量值;
(f)对模型进行调整使得该模型在一个或更多个测量时间处预测至少第二区室中的药物浓度,其至少近似匹配至少第二区室中的测量的药物浓度;并且
(g)使用模型来确定要给患者的第一区室配给的新的药物剂量,使得区室中至少之一中的靶浓度值与预测的稳态药物浓度之间的差小于预定阈值。
上面关于方法描述的优点和有利的实施方式还以类似的方式适用于控制装置,使得其应当参考上面的描述。
在有利的实施方式中,优选地,控制装置被构造成在将控制信号提供给输注装置之前请求用户确认作为步骤(d)之前的输入。因此,控制装置被构造成执行作为所谓的开环系统的方法,在所述的开环系统中,借助于用户交互在用户控制下进行模型的优化和用来控制输注装置的控制信号的输出。特别地,控制装置可以提出给患者配给新的药物剂量,其中,新的配给必须由用户特别是麻醉师来确认,使得不进行自动药物配给。
然而,原理上,还可设想将控制装置及其附接的外围装置作为闭环系统操作,在所述闭环系统中,自动地获得测量值,使模型优化并且给患者配给经调整的药物剂量。
除了输注装置和合适的监测装置之外,控制装置可以是独立的装置。然而,应当理解,控制装置可以不是独立的装置,而可以被集成至输注装置或监测装置或者所有的装置可以集成为一个单个装置。
有益地,上面描述的方法和控制装置适合于在麻醉过程期间中对给患者配给诸如异丙酚的麻醉药和/或诸如芬太尼、瑞芬太尼的止痛药或肌肉松弛药进行控制。然而,原理上,该方法和控制装置还可以用于给患者配给其他药物如抗生素或肿瘤药物。另外,利用提到的方法,还能够对同时配给多种药物如异丙酚和瑞芬太尼的过程进行控制,包括对其互相关性进行监测。此外,可以通过提到的方法来监测透析状态、细菌浓度或脓毒症。
随后参照附图中示出的实施方式,更详细地描述构成本发明基础的思想。在其中:

图1示出了通常在例如给患者P配给麻醉药如异丙酚的麻醉过程中使用的设备的示意图。在该设备中,多个装置被布置在机架1上并且经由不同的线路连接至患者P。
特别地,输注装置31、32、33如输注泵特别是注射泵或容积泵被连接至患者P并且用于经由线路310、320、330将不同的药物如异丙酚、芬太尼和/或肌肉松弛药静脉注射入患者P以达到期望的麻醉效果。线路 310、320、330例如被连接至进入患者P的静脉系统的单孔,使得各种药物经由线路310、320、330能够注射入患者的静脉系统。
机架1还持有当患者P处于麻醉时为患者P提供人工呼吸的通风装置4。通风装置4经由线路400连接至接口管40使得其与患者P的呼吸系统相连接。
机架1还保持经由线路或线路束500与附接至患者的头部的电极50 连接而用来监测麻醉过程期间患者的大脑活动的EEG监测器5。
另外,控制装置2由机架1保持,该控制装置2包括经由线路200 与接口管40的接合点41连接的测量装置20。控制装置2用于在麻醉过程期间控制输注装置31、32、33中的一个或多个输注装置的输注操作,使得输注装置31、32、33以可控的方式将麻醉药注射入患者P以获得期望的麻醉效果。这将会在下面更详细地解释。
测量装置20用于测量患者P的呼吸中的一种或多种麻醉药的浓度。测量装置20可以例如测量患者P的呼出气中的异丙酚浓度。为此,测量装置20可以例如在预定数目的呼吸周期(吸入和呼出)例如六个呼吸周期期间连续测量,以然后对患者P的呼吸在呼吸周期期间所测量的浓度适当地进行求平均。可替选地,测量装置20还可以测量仅在呼出阶段期间的例如异丙酚的浓度,其中,可以使用用于触发测量的合适的触发机制,或者其连续测量异丙酚浓度。
可选地,EEG监测器5可替选地或并行地测量对大脑的效果(麻醉深度)进行量化的指标值。
控制装置2可以适于提供关于在患者P的呼吸中或患者P的其他区室中所测量的药物浓度的信息,或者关于在患者的脑区室中的药效的信息。这样的信息能够经由附接至机架1的监视器6来输出,使得人员如麻醉师可以监视在麻醉过程期间患者P中所达到的药物浓度及相关效果。
图2示出了用于在麻醉过程期间对输注装置31、32、33的输注操作进行控制的控制环路的功能图。本文中的控制环路可以原理上被设置为在不进行用户交互的情况下自动控制输注装置31、32、33的操作的闭环。然而,有益地,将系统设置为以下开环系统,在该开环系统中,在某些时间点下特别是在给患者配给药物剂量之前,需要进行用户交互以人工确认该操作。
还被标为“输注管理器”的控制装置2连接至机架1,该机架1用作与也附接至机架1的输注装置31、32、33的通信链路。控制装置2输出控制信号以控制输注装置31、32、33的操作,输注装置31、32、33根据接收到的控制信号给患者P注射限定剂量的药物。
借助于EEG监测器5来获得患者P的EEG读数,并且借助于测量装置20来测量患者P的呼吸中的一种或多种药物的浓度。由EEG监测器5和测量装置20获得的测量数据被反馈至控制装置2,该控制装置2 相应地调整其控制操作并且将经修改的控制信号输出至输注装置31、32、 33以达到期望的麻醉效果。
测量装置20可以例如由用于借助于所谓的离子迁移谱法来测量患者 P的呼吸中的药物浓度的所谓的IMS监测器构成。还可以使用其他传感器技术。
控制装置2使用药代动力学-药效动力学(PK/PD)模型来控制一个或多个输注装置31、32、33的输注操作,所述药代动力学-药效动力学 (PK/PD)模型是用于对作用于患者P的身体中的药物的过程进行建模的药理模型。这些过程包括患者P的身体中的药物的再吸收、分布、生化代谢和排泄(被定义为药代动力学)以及生物体中的药物的效果(被标示为药效动力学)。优选地,使用具有N个区室的生理PK/PD模型,对于这 N个区室而言其传送速率系数已事先(例如在先证者研究中)实验上测量并且因此已知。优选地,为了简化模型,使用不多于4至5个区室。
图3中示出了这样的模型p的设置的示意功能图。模型p将患者P 逻辑上划分成不同的区室A1至A5,例如与患者P的血流对应的血浆区室A1、与患者P的肺对应的肺区室A2、与患者P的大脑对应的脑区室 A3以及与例如肌肉组织或脂肪和结缔组织对应的其他区室A4和区室 A5。假设药物剂量D借助于输注装置33被注射入血浆区室A1并且血浆区室A1链接其他区室A2至A5,使得其他区室A2至A5之间的交换总是经由血浆区室A1来发生,模型p考虑了不同的区室A1至A5的体积V 肺、V血浆、V大脑、Vi、Vj以及表示血浆区室A1与其他区室A2至A5之间的传送速率的传送速率常数KPL、KLP、KBP、KPB、KIP;KPI、KJP、KPJ。模型p用于预测所注射的药物在不同的区室A1至A5中的浓度C肺、C血浆、C大脑、Ci、Cj作为时间的函数。
图4至图12示出了用于借助于控制装置2控制输注装置33的输注操作的过程的实施方式。本文中的图4至图12示出了控制过程的不同步骤,其中,在初始阶段之后,可以迭代地重复步骤,使得在某段时间期间实现期望的行为,有利地,特别关注的例如脑区室或血浆区室中的药物浓度的稳态条件。
由于患者个人条件将该过程设置为PkPd模型的优化(个性化)过程,其中,在一定的测量时间t1和t2处,从患者的呼出气测量药物浓度即肺区室A2中的药物浓度。可替选地或此外,还可以在时间t1和t2处在脑区室(A2)中测量EEG指标值以获得表示患者的大脑中药物浓度的测量值M1、M2。然后,使用这样的测量值M1和M2以这样的方式优化模型 p,使得其反映特定过程并且在特定患者P内生效,其中,以下述方式调整要给患者P配给的药物剂量曲线,该方式使得在第二区室中,即脑区室 A3中,可替选地血浆区室A1中达到一定期望的药物浓度。
在图4所示的第一步骤中,通过使用经验数据并且通过考虑关于患者 P的人口统计信息如患者P的体重、大小、性别或身体质量指数(BMI) 对模型p进行初始化。使用具有其经初始化的传送速率常数K0PB、K0BP、 K0Pi、K0iP的第一、初始模型p0,所述传送速率常数K0PB、K0BP、K0Pi、 K0iP表示血浆区室A1与脑区室A3以及血浆区室A1与其他区室A2、A4、 A5之间的传送速率,然后,通过假设在已注射药物剂量D1之后大脑中的靶浓度CT 大脑将会在脑区室A3中达到稳态条件,来确定要给患者P配给的第一药物剂量曲线D1。根据通过配给药物在大脑中所达到的效果能够人工设置靶浓度CT 大脑。例如,在给患者P注射异丙酚的麻醉过程中,可以要求大脑中一定的异丙酚浓度以达到期望的麻醉效果,使得相应地设置靶浓度CT 大脑。
可替选地,可以在血浆或肺区室中限定靶浓度(图4未示出)。
如图4所示,该模型对血浆区室A1、肺区室A2、区室A4、区室A5 和脑区室A3中的随时间变化的药物浓度进行预测。当按药物剂量曲线进行注射时,血浆区室A1的药物浓度从初始的相比较高的值衰减,然而,区室Ai中和脑区室A3中的浓度增长直至达到药物浓度的变化可忽略的稳态为止。选择第一初始药物剂量曲线D1,使得在稳态条件下脑区室A3 中的药物浓度位于由在期望的靶浓度CT 大脑周围的阈值L1大脑表示的距离内。
为了使输注装置33给患者P注射药物剂量曲线D1,控制信号S1被馈送至输注装置33,其中,S1可以表示具有可变的剂量率R(t)和步长L1t 的阶跃函数。因此,输注装置33给患者P注射药物剂量曲线D1。
在已经开始根据剂量曲线D1给患者P的药物配给之后的短时间内 (即使还未达到稳态条件),借助于测量装置20在患者的呼吸中测量药物浓度并且由装置5测量可选的EEG指标值(图5)。通过转换函数TF (M1-7肺浓度CM1 肺(t1))将获得的测量信号转换成时间t1处的肺浓度(测量值M1),并且将肺区室CM1 肺(t1)中的实际浓度馈送至控制装置2。可替选地,可以将测量的信号馈送至控制装置2,并且可以在控制装置2内使用转换函数以将控制装置2中所测量的信号转换成表示肺区室中的实际浓度的测量值M1。
在下一步骤(图6)中,通过使用例如最小二乘法拟合来调整模型p,使得由模型p预测的肺区室A2中的浓度至少近似匹配实际测量的药物浓度CM1 肺(t1)(测量值M1)。为此,例如在模型p的最小二乘法拟合常规参数中,特别地,调整传送速率常数KPB、KBP、KPi、KiP和/或体积Vi、 Vj的值(参见图3),使得模型p对至少近似匹配测量值M1的、肺区室 A2中的药物浓度曲线进行预测。因此,实现了经调整的模型p’1,其对在特定患者P内已发生(回顾性视图)的过程和效果进行更准确的建模。
在下一步骤(图7)中,使用具有其经调整的参数的经调整的模型来确定新的药物剂量曲线,其中,以下述方式确定新的药物剂量曲线,该方式使得通过使用经调整的模型处于稳态的脑区室A3中的药物浓度C大脑达到透视图中期望的靶浓度CT 大脑。(可替选地,还可以为血浆区室A1 或肺区室A2设置靶浓度CT 血浆、靶浓度CT 肺(图7中未示出)。
在下一步骤(图8)中,借助于控制信号S2将新的药物剂量曲线D2 (被标示为TCI1)传送给输注装置33,其中,D2可以是具有可变的剂量率R(t)和步长L1t的阶跃函数,于是输注装置33给患者P注射药物剂量曲线D2。
然后,在第二测量时间t2处获得下一测量值M2(图9)。测量值 M2也表示在肺区室A2中实际获得的呼出气体中的药物浓度,于是使用最小二乘法拟合来再次调整模型p,使得由模型p为肺区室A2预测的药物浓度曲线至少近似预测所有实际测量的测量值M1和测量值M2(图 10)。如图10所示,由于模型p及其参数的调整,在其他的区室A1、区室A3至A5(包括脑区室A3)中所预测的药物浓度曲线也改变。
在下一步骤(图11)中,然后,以下述方式使用最新调整的模型p 来确定另一药物剂量曲线:处于稳态的脑区室A3中的药物浓度落入由阈值L1大脑表示的期望的药物浓度目标CT 大脑附近的范围内。(相应地,处于稳态的血浆区室A1的药物浓度达到靶浓度CT 血浆。)然后,最新确定的药物剂量曲线D3借助于控制信号S3被传送给输注装置33(图12),并且由输注装置33注射给患者P,其中,D3可以是具有可变的剂量率 R(t)和步长L1t的阶跃函数。
可以迭代地重复图9至图12的步骤以获得与靶浓度CT 大脑匹配的脑区室A3中的稳态药物浓度,其中,通过对借助于输注装置33而给患者P 注射的药物剂量D1、D2、D3进行迭代地调整,可以将脑区室A3中(相应地,血浆区室A1或肺区室A2中)的药物浓度维持在期望的靶浓度CT 大脑(相应地,CT 血浆或CT 肺)直至麻醉将要结束为止。
在测量值是脑区室中的EEG指标值并且配给的药物是唯一影响 EEG指标值的药物的情况下,可以执行等效步骤。在这种情况下,由装置5测量EEG指标值(图5),并且通过转换函数TF(M1-7大脑浓度 CM 大脑(t1))将在时间t1处所测量的指标值转换成大脑中的药物浓度C大脑的测量值M1。
原理上,可以将该过程设置为不需要进行任何用户交互的闭环过程。
然而,有益地,将该系统设置为在某些实例下需要进行用户交互的开环系统。例如,在图4的步骤中,由用户输入关于患者P的人口统计信息如患者P的大小、性别、体重或身体质量指数作为输入I0。在图5的步骤中,可以将例如重新校准请求作为输入I1输入至控制装置2。在图6 的步骤中,可以将用于对经调整的模型p的接受的确认输入为输入I2。在图7的步骤中,可以要求对最新计算的药物剂量进行确认的确认作为输入I3。在图9的步骤中,可以要求另一次重新校准请求作为输入I4。在图10的步骤中,可以要求用于对经调整的模型p的接受的确认作为输入 I5。在图11的步骤中,可以要求对最新计算的药物剂量的确认作为输入 I6。
还可以发生用户在过程期间改变靶浓度。在这种情况下,该模型重新计算新的药物剂量使得计算的浓度落入最新设置的靶浓度周围的可接受的范围内。
还可以发生:当过程接近于终止时用户想要关断用于药物配给的泵。在这种情况下,用户想要知道患者将何时再次醒来,这可以由关于例如患者的大脑中的药物浓度的透视图来预测。

另外,应当注意,可以通过测量患者的呼吸中的药物浓度或者通过获得EEG读数来获得用于监测实际药物浓度的测量值。对p模型进行校准的这样的过程(呼吸监测和EEG监测)在特定过程内配给仅一种药物的前提下是等效的。这是由于以下事实:如果配给多种药物,则EEG信号受多种药物例如异丙酚和芬太尼影响。因此,如果同时配给多种药物,则 EEG监测不是特定的,并且不能再使用EEG指标值对个别药物的动力学模型进行校准。
根据本发明的又一个实施方式,在第一时间间隔中,在一个或更多个测量时间处获得表示在一个区室优选地脑区室A3和另一个区室优选地肺区室A2中所测量的药物浓度的测量值。基于该测量值,通过使用相应的转换函数并且当获得不同区室的测量值时通过使用上面描述的模型来计算一个区室优选地肺区室A2中的药物的浓度。如果通过这两种途径计算的药物浓度存在差别,可以修改模型和/或优选地转换函数,使得存在两个结果之间的匹配或者使得两个结果至少在限定范围内。
优选地,基于在一个或更多个测量时间处获得的表示在脑区室A3中所测量的药物浓度的测量值(例如通过使用用于测量EEG信号的测量装置)并且使用模型p,确定肺区室A2中的药物的浓度。另外,基于在一个或更多个测量时间处获得的表示在肺区室A2中所测量的药物浓度的测量值(例如通过使用用于测量患者的呼吸中的药物浓度的测量装置)并且通过使用相应的转换函数TF,来确定肺区室中的药物的浓度,所述转换函数TF将表示患者的呼吸中的药物浓度的所测量的信号转换成表示肺区室中的实际药物浓度的测量值。如果针对肺区室中的药物浓度确定的这两个值之间存在差别,则修改将通过测量患者的呼吸中的药物浓度而获得的测量信号与肺区室中的药物的浓度联系起来的转换函数TF,使得肺区室中的两个值匹配或者至少在限定范围内。该方法使得能够使转换函数TF 适应患者。在另外的过程中可以使用适应患者的转换函数,提高该方法的整体性能,特别是当在后来的阶段处仅通过测量患者的呼吸中的药物浓度而获得的测量信号被用来修改模型和/或确定要配给的新的药物剂量。
当然,该方法还适用于其他区室和其他转换函数。
执行本文中描述的基于模型的优化算法的控制装置不一定必须是独立的装置。它可以是监测装置的一部分或者它可以是泵的一部分、或泵,监测装置和控制装置可以被集成为单个装置。

Claims (14)
1.一种控制用于给患者配给药物的输注装置(33)的控制装置(2),所述控制装置(2)被构造成:
(a)提供对患者的多个区室中的时间相关的药物浓度进行预测的模型;
(b)设置要在所述患者的所述多个区室的至少之一中实现的靶浓度值;
(c)使用所述模型来确定要给所述患者的多个区室中的第一区室配给的药物剂量,使得所述多个区室的至少之一中的所述靶浓度值与预测的稳态药物浓度之间的差小于预定阈值;
(d)将表示所述药物剂量的控制信号作为输出提供至用于给所述患者配给所述药物剂量的输注装置(33);
(e)在测量时间处获得表示在所述多个区室中的至少第二区室中的测量的药物浓度的至少一个测量值作为输入;
(f)对所述模型进行调整使得所述模型在一个或更多个测量时间处预测至少所述第二区室中的药物浓度,所述药物浓度至少近似匹配至少所述第二区室中的测量的药物浓度;并且
(g)使用所述模型来确定要给所述患者的第一区室配给的新的药物剂量,使得所述多个区室的至少之一中的所述靶浓度值与预测的稳态药物浓度之间的差小于所述预定阈值。
2.根据权利要求1所述的控制装置,其中,所述模型是在患者的多个区室中作为时间的函数预测药物浓度药代动力学-药效动力学模型。
3.根据权利要求1或2所述的控制装置,其中,所述第一区室是血浆区室,并且所述第二区室是大脑或肺区室。
4.根据权利要求1或2所述的控制装置,其中,所述第一区室是血浆区室,所述第二区室是肺区室,并且多个区室中的第三区室是大脑区室。
5.根据权利要求1或2所述的控制装置,其中,迭代地重复步骤(d)至步骤(g)。
6.根据权利要求1或2所述的控制装置,其中,步骤(f)包括:对所述模型的表示不同区室之间的传送速率或区室的体积的至少一个传送速率常数或体积进行调整。
7.根据权利要求1或2所述的控制装置,其中,通过被构造成测量所述患者的呼吸中的药物浓度的测量装置(20)来获得所述至少一个测量值。
8.根据权利要求1或2所述的控制装置,其中,通过在预定数目的呼吸周期期间连续测量所述患者的呼吸中的所述药物浓度来获得至少一个测量值。
9.根据权利要求1或2所述的控制装置,其中,通过被构造成测量产生EEG指标值的EEG信号的测量装置(5)来获得至少一个测量值。
10.根据权利要求1或2所述的控制装置,其中,在由所述输注装置(33)进行的由步骤(d)的所述控制信号表示的所述药物剂量的配给在至少所述第一区室中达到稳态状态或完成之后,步骤(e)的测量时间位于某个范围内。
11.根据权利要求1或2所述的控制装置,其中,在至少所述第二区室中的所述药物浓度达到稳态状态或完成之前,获得根据步骤(e)在一个或更多个测量时间处的所述至少一个测量值。
12.根据权利要求1或2所述的控制装置,包括:在测量时间处使用转换函数将所述测量值转换成所述第二区室中的测量的药物浓度。
13.根据权利要求1或2所述的控制装置,其中,在另外的步骤(h)中,在一个或更多个测量时间处获得表示所述多个区室中的第三区室中的测量的药物浓度的至少一个测量值,以及在一个或更多个测量时间处获得表示所述多个区室中的所述第二区室中的测量的药物浓度的至少一个测量值;并且通过使用所述模型基于表示所述第三区室中的测量的药物浓度的所述测量值来确定表示所述第二区室中的药物浓度的第一值,以及使用转换函数基于表示所述第二区室中的测量的药物浓度的所述测量值来确定表示所述第二区室中的药物浓度的第二值,其中,在另外的步骤中,如果所述第一值与所述第二值之间存在差,则修改所述转换函数,使得所述第一值和所述第二值匹配或者在限定范围内。
14.根据权利要求1或2所述的控制装置,其中,所述控制装置(2)被构造成在将所述控制信号提供至所述输注装置(33)之前请求用户信息作为步骤(d)之前的输入。
