发明内容

总的来说,使可膨胀支撑件膨胀的目的是使例如消融结构的操作元件与作为消融目标的组织部位形成治疗上有效的接触。通过这样的接触,可使用各种非刺入式电极图形将消融能量传递至胃肠道的目标邻接径向部位。本发明的多个方面涉及用于三维地控制传递至组织中的射频能量传递的电极图形及方法,所述三维包括:控制在目标区域内跨过组织的表面面积的能量传递,控制在目标区域内进入组织的深度中的传递,以使组织的某些体积部分被消融且组织的某些体积部分未被消融。适合与根据本发明的设备一同使用的电极图形的示例提供于在2008年5月2日提交的凯利(Kelly)等的美国申请第12/114,628号(代理人案号716.201)中,其为各种目的作为参考并入本文。可将此种类型的消融的实施例理解为在邻近目标或处理区域内的部分消融或局部消融,这样,组织的消融处理后的区域具有受影响的组织和大致未受影响的组织的区域的混合图形。
参考如图1至14C中所示的示例性实施例的以下详细描述将获得对本发明的特征及优点的更好理解,其中本发明的原理应用于示例性实施例中。
图1为包括有膨胀的可膨胀支撑构件105及布置于可膨胀支撑构件上的操作元件130的诊断或治疗设备100的一个实施例的立体图。可膨胀支撑构件105具有带有圆形或半球形末端的大体为圆柱形的构造,且安装于柄108上。在某些实施例中,如图1中所示,所述柄、或其一部分或一段长度穿过可膨胀构件延伸,且在远侧出现以形成使可膨胀构件的结构稳定的远侧引导结构,且在设备部署期间提供操作优势。可膨胀支撑构件105包括第一部分110和第二部分120。例如治疗或诊断仪器或结构的操作元件130由可膨胀支撑构件第一部分110支撑。示例性治疗仪器130可为被构造为将射频能量传递至身体中的目标组织的消融结构,且示例性柄108可为具有合适尺寸和构造以适于进入和被推入胃肠道中的导管。可使用例如发生器及控制系统、配线、电连接及用户界面等各种传统部件来操作操作元件130以及可能地操作柄108的元件。为清楚起见且为容许专注于本发明,图中省略了这些传统部件的细节。
图1还示出可膨胀支撑构件105的某些特定的结构细节。例如可为消融结构的操作元件130具有由长度130l及宽度130w所限定的表面面积。可理解的是,可膨胀支撑构件的实施例一般为圆柱形,其中它们的纵向轴线平行于支撑它们的柄或伸长构件的纵向轴线。相反地,布置于可膨胀支撑构件上的操作元件130通常占据围绕可膨胀构件的小于360度的弧,且因此占据围绕轴的小于360度的弧。总的来说,在设备的使用方法中,使可膨胀支撑构件膨胀以填充正在处理的体腔或中空器官的内部。因此,操作元件在可膨胀构件上占据的周长部分对应于在由操作元件所提供的任何单一处理传递期间正由操作元件接触的中空器官的周长部分。这样,这些设备及方法一般适于对占据中空器官的周长的一部分的目标部位的集中处理,而不是需要面向整个圆周区域的处理。这样的焦点目标弧例如可小于360度、大致为180度、介于大致20度至大致180度之间、小于180度、介于大致20度至大致90度之间、小于180度、大致为90度、或小于90度的任何圆周弧。
如图1中所示,可膨胀支撑构件第一部分110具有由与支撑构件的纵向轴线对准的长度110l及与支撑构件的径向轴线对准的宽度110w所限定的表面面积。在此实施例中,可膨胀支撑构件第一部分110的表面面积大于操作元件130的表面面积,因为可膨胀支撑构件第一部分的长度及宽度均大于操作元件130的长度及宽度。替代地,可膨胀支撑构件第一部分还可通过具有与仪器相同的宽度但是较大的长度或具有相同的长度但是较大的宽度而具有较大的表面面积。
图1中所示的实施例具有大于可膨胀支撑件第二部分120的壁厚度120t的可膨胀支撑件第一部分110的壁厚度110t。可膨胀支撑构件的壁厚度110t及120t沿部分110、120的长度及宽度分别为均一的。可膨胀支撑构件的部分(第一部分和第二部分)中的任一个或两个还可具有非均一的或变化的壁厚度。
在一个方面中,支撑消融仪器的可膨胀支撑构件第一部分110适于具有小于可膨胀支撑构件第二部分120的膨胀指数。指定的可膨胀支撑结构或可膨胀支撑结构的部分的膨胀指数指的是所述结构的所选择的部分的最终的表面面积(当其正经受特定膨胀力时)除以所述结构的所选择的部分的最初的表面面积。
在本文中使用时,“膨胀指数”一般指在所有其它条件相同的情况下指定物体在规定负载下膨胀的表面尺寸相对于未膨胀的表面尺寸的比率。所述尺寸包括但不限于长度、表面面积、及体积。在多个方面中,“膨胀指数”由数字表达。若物体在经受负载时膨胀,则称之为具有大于1的膨胀指数。若物体在经受负载时并未膨胀,则称之为具有等于1的膨胀指数。若物体在经受负载时收缩,则称之为具有小于1的膨胀指数。在多个实施例中,膨胀完全地或大部分地为弹性的,意味着材料大致恢复至其原始形状和/或尺寸。在多个实施例中,不可扩张的部分(例如第一部分)具有的最大膨胀指数为1.2,优选地为1.1。在多个实施例中,不可扩张的部分(例如第一部分)具有大致为1的最大膨胀指数。在多个实施例中,可膨胀部分(例如第二部分)具有为10的最大膨胀指数(即1000%膨胀)。
在多个实施例中,可膨胀支撑构件的第一部分在经受特定的膨胀力时最低程度地膨胀。其或者根本不膨胀(膨胀指数为1.0)或者其小程度膨胀(并且膨胀指数稍大于1)。简单地说,可理解的是,当经受特定的膨胀力时最低程度地膨胀的表面的膨胀指数大致为1。
另一方面,支撑结构的未用于支撑治疗仪器或元件的区域可具有显著大于1的或甚至为1的多倍的膨胀指数。如以下针对示例性附图所解释的,可膨胀支撑结构的多个部分被构造为对膨胀力做出不同的响应。并且,如以下实例中所解释的,各种制造技术或方法中的任一个可单独地使用或组合使用,以在可膨胀支撑件的两个部分中的每一个中产生期望的膨胀率。在多个实施例中,可膨胀支撑构件包括具有不同于第一部分和第二部分的膨胀指数的一个或多个其它部分。所述一个或多个其它部分可与第一部分和第二部分中的任一个或两个邻接。
图2A及2B示出了可膨胀支撑构件105,分为处于未膨胀状态及膨胀状态的弧形片的二维表示图。如在此示例说明性实施例中所看到的,可膨胀支撑构件第一部分具有为1的膨胀率,亦即,其在经受致使第二部分膨胀的特定力时并不膨胀。图2A示出处于最初的或未膨胀状态的可膨胀支撑构件105;可膨胀构件第一部分110具有宽度110w及长度110l。在此实施例中,操作元件130具有与可膨胀支撑构件第一部分的宽度及长度相同的宽度130w及长度130l。可膨胀支撑构件第二部分120具有宽度120w及长度120l。
图2B示出处于膨胀状态的图2A的可膨胀支撑构件105。可膨胀支撑构件完全地由于可膨胀构件第二部分膨胀至膨胀状态(120’)而实现膨胀状态,膨胀状态(120’)包括沿两个轴线的膨胀,亦即,沿长度及宽度(与此相反,第一部分未对这种膨胀做出贡献)。在图2B的膨胀状态中,第二部分的宽度120w已经增加至宽度120w’且第二部分的长度120l已经增加至长度120l’。因此,可膨胀支撑构件第二部分120的膨胀率大于1。在此实施例中,操作元件130的面积与可膨胀支撑构件第一部分110的面积相同。可膨胀支撑构件第一部分110及操作元件130的面积在未膨胀(图2A)及膨胀(图2B)构造中保持相同。因此,可膨胀支撑构件第一部分110及操作元件的膨胀率为1。
图3A及3B分别示出处于未膨胀状态及膨胀状态中的可膨胀支撑构件105。在此实施例中,可膨胀支撑构件第一部分具有大致为1的膨胀率。图3A示例说明处于最初的或未膨胀状态中的可膨胀支撑构件105。可膨胀构件第一部分110具有宽度110w及长度110l。在此实施例中,治疗或诊断仪器130具有宽度130w及长度130l。在此实施例中,治疗或诊断仪器的面积与可膨胀支撑构件第一部分的面积相同。可膨胀支撑构件第二部分120具有宽度120w及长度120l。
图3B示出处于膨胀状态中的图3A的可膨胀支撑构件105。可膨胀支撑构件由于可膨胀构件第二部分沿两个轴线(亦即,长度及宽度)膨胀至膨胀状态(120’)而实现膨胀状态。在图3A及3B中所示的简单二维可膨胀支撑构件105中,膨胀的大部分发生于可膨胀支撑构件第二部分120中且一部分膨胀发生于可膨胀支撑构件第一部分110中。尽管可膨胀支撑构件第二部分120既沿长度又沿宽度膨胀,可膨胀支撑构件第一部分110及操作元件130仅沿宽度膨胀。在此实例中,可膨胀支撑构件第二部分120的膨胀率大于1。在图3B的膨胀状态中,第二部分的宽度120w已经增加至宽度120w’且第二部分的长度120l已经增加至长度120l’。另外地,第一部分的宽度110w已经增加至宽度110w’且第一部分的长度110l已经增加至长度110l’。因此,可膨胀支撑构件第二部分120的膨胀率大于1。第二部分的膨胀大于第一部分的膨胀。还应指出的是,还有操作元件130的膨胀。操作元件的宽度130w已经增加至宽度130w’且操作元件的长度130l已经增加至长度130l’。由于可膨胀支撑构件第一部分110及操作元件130中的膨胀量,可膨胀支撑构件第一部分110及操作元件130的膨胀率大致为1。
为了在传达发生于弧形结构中的膨胀指数的概念时的视觉简单化,将图2A、2B、3A、及3B显示为二维片状结构。应理解的是,整个可膨胀支撑构件或第一和/或第二可膨胀支撑构件部分的膨胀可以更复杂的方式膨胀或以与可膨胀支撑构件、可膨胀支撑构件第一部分或可膨胀支撑构件第二部分的结构、形式、或构造有关的方式膨胀。
在本发明的典型的实施例中,例如消融结构的治疗或诊断仪器支撑于例如可膨胀气囊的壁或表面的弧形表面上或集成于所述弧形表面中。暂时不考虑气囊表面的部分的不同区域可扩张或膨胀的程度方面的差异,表面面积的膨胀可跨过表面面积均一地发生,或者其可在弧形意义上优选地沿一个或多个轴线发生。例如,膨胀可在曲形平面内沿第一轴线发生、或沿第一轴线及垂直于第一轴线的第二轴线发生。替代地,可以以相对于第一轴线成除90度之外的角度定向第二轴线。作为另一示例,当气囊的周长膨胀时可使第一轴线正交地对准至圆柱形气囊的纵向轴线。并且当气囊的表面在膨胀时延长时,可平行于气囊的纵向轴线纵向地对准第二轴线。发生膨胀所沿着的轴线可比两个垂直轴线的这些示例更复杂,且可跨过可膨胀构件的表面变化,且可受构件的成分、集成或附着至构件的壁的特征、或壁成分的厚度方面的变化的影响。
图4A及4B分别示例说明未膨胀的可膨胀支撑构件105的立体图及俯视图。在图4A及4B中所示的未膨胀状态中,操作元件130及第一部分110的表面面积大于第二部分120的表面面积。可膨胀构件第一部分具有宽度110w。
图5A及5B分别示出部分地膨胀的可膨胀支撑构件105的立体图及俯视图。在图5A及5B中所示例说明的部分地膨胀状态中,操作元件130(诊断或治疗仪器)及第一部分110的表面面积小于第二部分120的表面面积。
图6A及6B分别示出膨胀的可膨胀支撑构件105的立体图及俯视图。在图6A及6B中所示例说明的膨胀状态中,操作元件130及第一部分110的表面面积保持小于第二部分120的表面面积。当如图6A、6B中所示例说明的使可膨胀支撑构件膨胀时,治疗或诊断元件130的表面面积与可膨胀元件第二部分120的表面面积不同。对于图4A、4B中所示例说明的未膨胀状态而言,可以说存在相同的情况。在图6A及6B中的膨胀状态的情况中,第二部分120为不同的,因为其具有大于治疗或诊断仪器130的表面面积。
图4A至6B还示例说明了可膨胀支撑构件105的一个实施例,其中可膨胀构件第二部分的膨胀率大于1而可膨胀构件第一部分及操作元件130的膨胀率大致为1。因此,可膨胀支撑构件第一部分的宽度110w在以下全部的三种状态中保持相同:图4A、4B中所示例说明的未膨胀状态、图5A、5B中所示例说明的部分地膨胀状态以及图6A、6B中所示例说明的膨胀状态。
图6B还示例说明了可膨胀支撑构件的一个实施例,其中可膨胀支撑构件第一部分110的壁厚度大于可膨胀支撑构件第二部分120的壁厚度。如所示例说明的,壁厚度110t大于壁厚度120t。尽管理论未限制,一般考虑的是,若柔性片其它方面为相同的,则与较厚的片相比,较薄的片更加柔顺且具有较大的膨胀指数。因此,第一部分110的较厚的壁将抵抗应用至可膨胀支撑构件的膨胀力,且膨胀力将优选地使第二部分120的更加柔顺的较薄的侧壁膨胀。在多个方面中,“优选地”将理解为通常在本技术中所使用的且指的是期望的、预定的、或所选择的结果。“优选地”可指设计或控制流程以获得期望的结果。例如,可将可膨胀支撑构件制造为使所得到的产品具有拥有期望的预定的膨胀指数的第一部分。在多个方面中,稍微互换地使用“优选地”及“选择地”。
图7提供可膨胀支撑构件108的一个实施例的径向剖视图,可膨胀支撑构件108在圆周方面整体上具有变化的壁厚度,包括第一部分110和第二部分120。更特别地,第一部分110整体上具有与第二部分120的任何圆周区域相比更大的厚度110t。第二部分120整体上实际具有变化的厚度,例如如点120t1、120t2、及120t3处所示,其中相对而言,120t1具有最小厚度,120t2具有中间厚度,且120t3具有最大厚度。第二部分的壁厚度120t显示为从120t1至120t2且随后至120t3线性地或连续地增加,然而,第二部分的壁厚度可以以非线性或分阶方式、或适合于可膨胀支撑构件第二部分的所期望的膨胀指数的其它任何方式变化。
图8提供圆柱形可膨胀支撑构件105的一个特定实施例的剖视图且聚焦于沿可膨胀支撑构件第一部分110的纵向轴线的变化的壁厚度。尽管可膨胀支撑构件第二部分120及操作元件130均显示为具有各自的均一的厚度,但是存在本发明的替代实施例,其中可膨胀支撑构件第二部分120及治疗或诊断仪器130的厚度还可为变化的。图8示出在近端105p及远端105d处均具有厚度110t1的可膨胀支撑构件第一部分。从近端105p向远侧(或相对于整个第一部分向中心),所述厚度从110t1增加至110t2。在操作元件130的附近继续向远侧,所述厚度从110t2增加至110t3。超出操作元件130的附近继续向远侧,所述厚度从110t3减少至110t4。从操作元件130的附近进一步向更远侧,所述厚度从110t4减少恢复至厚度110t1。从110t1经110t2至110t3的过渡显示为具有阶跃式特性,为以上所提到的一种厚度变化类型。包括于本发明中的一个替代实施例为其中此类过渡为连续的或平滑的的实施例。
变化的壁厚度以及厚度之间的过渡连续性的变化类型可用于调整整个可膨胀支撑构件的膨胀响应以及可膨胀或可扩张部分(例如,第二部分)与构件的大致不可膨胀或不可扩张部分(例如,第一部分)之间的界限,尽管如此,所述构件整体上仍然为可膨胀的。还可以理解,这些局部变化与不同的膨胀率或膨胀指数一同共同地确定膨胀构件的整体的膨胀性(包括组成部分1及2)。还可以理解的是,尽管本发明在图中已经大体被描述及例示为具有布置操作元件的第一部分,以及并未特别地为操作元件的支撑做出贡献的第二部分(这样的部分具有与第一部分相比更大的膨胀指数),然而可存在多于一个这样的“第二”部分,如可通过具有与第一部分相比更大的膨胀指数来限定,且这样的实施例包含于本发明的精神内。仅仅举例而言,可膨胀构件可包括具有大致为1的膨胀指数的第一部分、具有大致为1.4的膨胀指数的第二部分、以及具有大致为1.8的膨胀指数(亦即80%的膨胀)的第三部分(第二“第二”部分)。
改变整个可膨胀支撑构件、第一或第二可膨胀构件部分或操作元件的壁厚度或其它尺寸并非为改变膨胀指数的唯一方法。在可膨胀支撑结构第一部分上或内的一个或多个增强元件可用于此用途。此外,在一个方面中,所述增强元件为治疗或诊断设备的部分。
图9提供可膨胀支撑构件105的剖视图,其中增强元件160为诊断或治疗设备130的部分。在此实施例中,可膨胀支撑构件的第一部分110和第二部分120的壁厚度为不同的。在多个实施例中,所述壁厚度为相同的。在多个实施例中,一个或两个的壁厚度沿其长度或宽度为变化的。在此情况下,第一部分的平均壁厚度可大于第二部分的平均壁厚度。
在示例性实施例中,可膨胀支撑构件第一和第二部分之间的膨胀响应方面的变化起因于由与附接至第一部分的设备130相关联的尺寸稳定增强元件所造成的对第一部分膨胀的限制。增强元件160可为集成于诊断或治疗设备130的设计中的杆。增强元件160可为连续的或分段的。如本文所解释的,增强元件可圆周地延伸小于360度、小于180度、或小于90度。在图9中所示的实施例中,增强元件160与可膨胀支撑构件105及操作元件130的纵向轴线对准。根据对膨胀响应的期望的改变或所得到的膨胀指数,增强元件160的取向可不同于所示例说明的取向。增强元件160被适当地设定大小和间距,以便不干扰诊断或治疗设备130的操作。
图10A及10B分别为具有一个或多个增强元件170的可膨胀支撑构件第一部分110的立体图及径向剖视图。增强元件170沿可膨胀支撑构件105的轴向长度延伸。第一部分的增强部分(110w,如在图9中所看到的)大于仪器130的面积。增强元件170嵌入于可膨胀支撑构件第一部分110的壁内。增强元件170及可膨胀支撑构件第一部分110所占据的面积大于操作元件130的面积。在此实施例中,可膨胀支撑构件的部分的第一部分110和第二部分120的壁厚度为相同的。可膨胀支撑构件第一和第二部分之间的膨胀响应方面的变化起因于的由增强元件170所造成的对第一部分膨胀的限制。
增强元件170在此示例性实施例中为连续的矩形零件。根据第一部分的壁厚度及增强元件的尺寸而定,其它形状及大小为可行的。用于可膨胀支撑构件的合适的材料包括但不限于:聚合体或弹性体。用于增强元件的合适的材料包括但不限于:例如不锈钢、镍钛、铜、或钛等金属;网格线;以及具有低弹性的聚合体,例如PEEK、ABS、或聚酰亚胺。在图10A及10B中所示的实施例中,使增强元件170与可膨胀支撑构件105及操作元件130的纵向轴线对准。增强元件170的取向可根据膨胀响应的期望的改变或所得到的膨胀指数而不同于所示例说明的取向。增强元件170被适当地设置大小和间距,以便不干扰诊断或治疗设备130的操作。
图11示出具有网格增强元件175的可膨胀支撑构件第一部分110的剖视图。网格增强元件175可安置于可膨胀支撑结构第一部分110的表面上、或集成于或嵌入于所述表面中。替代地,网格增强元件175可更特别地与操作元件130相关联或集成于操作元件130中,如以上参考图9所描述的。在另一替代实施例中,网格增强元件175可安置于操作元件130及可膨胀支撑构件105之间。
图12A及12B分别为具有布置于可膨胀构件105的内表面或外表面上的一个或多个增强元件的可膨胀支撑构件第一部分110的径向剖视图。图12A示出所述部分110的外表面上的增强元件140,其通过其位置使表面部分110与操作元件130分离。增强部分140w的宽度大于操作元件130的宽度。另外地,增强的第一部分的面积大于操作元件130的面积。在多个实施例中,增强部分的表面面积大于操作元件。在多个实施例中,增强部分的表面面积小于操作元件。在多个实施例中,增强部分完全地或部分地覆盖操作元件。在多个实施例中,操作元件完全地或部分地覆盖增强部分。
增强元件140可为连续或分段结构。例如,增强元件140可为与参考图9、10A、10B、及11所示例说明且描述的可膨胀支撑构件第一部分的增强部分类似的构造。在某些实施例中,图12A的增强部分140可为包括有聚酰亚胺的不可扩张的电极阵列背衬,如以下所进一步描述的。
图12B示例说明在可膨胀支撑构件的第一部分110的内部中具有增强元件180的可膨胀支撑构件105。在此实施例中,可膨胀支撑构件第一部分110的表面将治疗或诊断仪器130与增强元件180分离。增强部分的宽度180w大于操作元件130的宽度。另外地,增强的第一部分的面积大于治疗或诊断元件130的面积。增强元件180可为连续或分段结构。例如,增强元件180可为与参考图9、10A、10B、及11所示例说明且描述的可膨胀支撑构件第一部分的增强部分类似的构造。
在某些实施例中,图12A的增强部分140可为包括有聚合材料的柔性的但不可扩张的电极阵列背衬。例如,支撑件140可包括薄的、矩形聚合材料片,例如聚酰亚胺、聚酯、或其它柔性的热塑性或热固性高分子膜。支撑件140还可包括覆盖聚合物的材料、或其它不导电材料。另外地,背衬可包括具有沉积至表面上的导电材料(例如铜)的电绝缘聚合物,以使电极图形可以被蚀刻至材料中以产生电极的阵列。

本发明的实施例还包括制造具有厚度变化的区域以及具有成分变化的区域的可膨胀构件(例如气囊)的方法。制造气囊的典型的方法包括浸渍模塑,其中将具有气囊的期望最终形式的芯棒一次或多次浸渍于液态聚合物溶液中,所述液态聚合物溶液涂覆所述芯棒且干燥为薄膜。通过在变化的聚合物成分中的多次浸渍,可以将不同成分应用至单个气囊。通过在浸渍之前或在多次浸渍之间将增强元件应用至芯棒,可以将增强元件集成至气囊皮中。通过选择地使芯棒的特定区域暴露的多次浸渍,这样的区域可以被选择地制造为更厚,或具有变化的成分。通过在浸渍期间选择地掩盖芯棒的区域,正在制造的气囊的区域可以缺少在气囊皮内的其它地方所出现的元件或成分。
在多个实施例中,使用吹塑工艺制造可膨胀构件中的一个或多个。在多个实施例中,使用吹塑及在槽中浸渍来形成气囊。根据用于提供选择性增强作用的以上技术,用于吹气囊的管可以以壁的厚度方面的变化开始或包括嵌入于管壁中的支撑结构,所述支撑结构随后转移至膨胀的气囊。在多个实施例中,调节包括但不限于拉速、加热器停留时间、以及加热器喷嘴设计的吹塑工艺参数,以改变沿气囊长度或沿圆周的壁厚度。
可膨胀支撑构件的实施例还可用于为身体中的组织提供处理的方法中。所述方法包括将仪器安置为接近选择接受处理的身体中的组织。所述仪器由适于使第一部分和第二部分非均一膨胀的可膨胀支撑构件所支撑。第一部分和第二部分的非均一膨胀指的是可膨胀支撑构件的膨胀在可膨胀支撑构件的这些不同的部分之间的相对分布。已经将可膨胀支撑构件修改及构造为使得在经受膨胀力时,可膨胀支撑构件的响应在整个可膨胀支撑构件上为不均一的。第一部分支撑仪器的至少部分。接下来,使可膨胀支撑构件第二部分膨胀直至仪器相对于选择接受处理的身体中的组织处于处理位置中。随后,通过使用仪器为选择接受处理的组织提供处理。在诊断仪器的情况下,诊断过程是在处理位置中执行的。
处理位置将根据很多因素而定。在这些因素中有:正在处理、诊断或评估的组织的类型;正在处理、诊断或评估的组织的位置;所使用的处理仪器的设计或类型;以及所使用的诊断仪器的设计或类型。在一个方面中,处理位置为其中仪器与选择接受处理的组织的表面接触的位置。在另一方面中,处理位置为其中仪器与作为要接受处理的目标的组织间隔开的位置。
提供处理的方法还可根据所执行的特定处理而变化。例如,所述方法可包括通过消融选择接受处理的组织的一部分而使用仪器为选择接受处理的组织提供处理。替代地,所述方法可包括使用处理仪器执行其它行为之前、与使用处理仪器执行其它行为联合、或使用处理仪器执行其它行为之后的评估或信息收集步骤。在一个方面中,所述处理可包括通过获得与选择接受处理的组织的部分相关的信息而使用仪器为选择接受处理的组织提供处理。
图14A至14C示出由可膨胀支撑构件所支撑的仪器的定位及使用。在此示例说明的实施例中,仪器用于为人体食道5的一部分内的选择接受处理的目标组织部位(T)提供处理,且经由内窥镜传递。图14A示出操作元件,例如由未膨胀的可膨胀支撑构件105所支撑的消融结构130。已经使内窥镜10前进至胃7上方的食道5内的位置中。从食道5内的此位置,使未膨胀的可膨胀支撑构件105穿过内窥镜10的工作通道前进。调节未膨胀的可膨胀支撑构件105的位置,以将仪器130于相对于选择接受处理的组织(T)放置就位。
图14B示出如图14A中的由可膨胀支撑构件105所支撑的仪器130,其中使支撑构件105部分地膨胀。图14C示出如图14A及14B中的由可膨胀支撑构件105所支撑的操作元件130,其中使可膨胀支撑构件105完全地膨胀。在此位置中,仪器130处于为选择接受处理的目标组织部位(T)提供处理的位置中。通过可膨胀构件的如此完全或适当的膨胀程度,实现消融结构101及目标组织之间的“处理接触”或“治疗上有效接触”。这样的处理接触一般指的是组织表面上的目标部位(例如胃肠道的管腔器官的壁上的部位)的全部或一部分与消融结构130的消融表面的全部或大部分之间的完全的或大致完全的接触。
图14A至14C示出与内窥镜联合的可膨胀支撑构件的使用。然而,本发明的可膨胀支撑构件的实施例的使用并不受此限制;可将其安置为用于通过使用很多技术中的任一个来提供处理或诊断。例如,可膨胀支撑构件可安装于导管上以及直接地插入管腔中。更进一步地,基于导管的实施例可单独使用或与放置于管腔中的例如内窥镜的另一设备并行使用。基于导管的可膨胀支撑构件可被独立于内窥镜操作、可安装至内窥镜上、或者被构造为穿过内窥镜的工作通道(图14A至14C)。
本发明的可膨胀支撑构件105的实施例可用于提供体腔、中空的身体器官、或身体内的腔的处理、治疗或诊断。根据选择处理的特定的解剖部位可修改可膨胀支撑构件105的尺寸或可调节膨胀响应。特别地,治疗或诊断仪器的尺寸或操作特征可要求仪器为特定大小、由特定的成分形成、或放置于可膨胀仪器上的特定位置或相对于选择接受处理的组织的特定位置中。因此,例如消融结构的操作元件在可膨胀支撑构件上的放置可依靠可膨胀支撑构件的膨胀特征,以使仪器运动至期望的位置以提供处理。出于这些各种考虑因素考虑,可将可膨胀支撑构件第一部分与第二部分的大小的比率以及这些部分与仪器之间的大小的比率设计为根据仪器的预期用途而不同。另外地,包括可膨胀支撑构件第一与第二部分的膨胀率的膨胀特征还可根据具体应用及感兴趣的解剖部位而变化。
以下实例示例说明可如何使用可膨胀支撑构件的大小及操作方面的差异来适应解剖部位的差异。在这些实例中所考虑的解剖部位为人体食道。要被处理的食道的大小方面的差异可导致其中可膨胀支撑构件的第二部分即使在可膨胀构件膨胀时也小于可膨胀支撑构件的第一部分的情况(膨胀的绝大部分来自第二部分)。在其它环境中,典型地在较大的腔尺寸中,当未使第二部分膨胀时,可膨胀支撑构件的第一部分可大于可膨胀支撑构件的第二部分。然而,当使膨胀构件膨胀时,第二部分(主要地为整个膨胀负责)膨胀,其可变为大于第一部分。
导致此种结果的一个因素为可膨胀支撑构件第一部分以及所附带的操作元件被典型地优化以提供处理或以诊断腔表面的最佳尺寸,以容许用于大量的患者群体中。当使可膨胀支撑构件膨胀时,操作元件的表面面积与可膨胀支撑构件第二部分的表面面积不同。在一个方面中,表面面积方面的差异可能导致操作元件的表面面积大于可膨胀支撑构件第二部分。在另一方面中,表面面积方面的差异可能导致操作元件的表面面积小于可膨胀支撑构件第二部分。因此,腔的大小方面的结构变化由可膨胀支撑构件的第二部分补偿。在以下示例中示例说明使用第二部分适应腔尺寸方面的变化。
实例1
示例性体腔具有大致60mm的周长。覆盖40mm弧的圆周跨度或弧的设备(例如消融结构)安装于大小相当的可膨胀支撑构件的第一部分上。在此示例中,当40mm的设备处于用以提供处理的位置中时,将使可膨胀支撑构件第二部分膨胀以填满腔的周长中的剩余的20mm。因此,当安置可膨胀支撑构件以在腔中使用且使可膨胀支撑构件的第二部分膨胀时,第二部分具有比第一部分小的面积。
实例2
示例性体腔具有大致120mm的周长。覆盖40mm弧的圆周跨度或弧的设备例如消融结构安装于大小相当的可膨胀支撑构件第一部分上。在安置设备以供使用之前,可膨胀支撑构件第二部分具有比可膨胀支撑构件第一部分小的面积。然而,当使可膨胀支撑构件膨胀且处于用于使用的位置中时,将使可膨胀支撑构件第二部分膨胀以覆盖腔的周长中的剩余的80mm。因此,当安置可膨胀支撑构件以在腔中使用时且使第二部分膨胀时,可膨胀支撑构件第二部分具有比可膨胀支撑构件第一部分大的面积。

Claims (47)
1.一种医学设备,其包括:
具有第一部分和第二部分的可膨胀支撑构件,其中,所述第一部分被布置成占据与所述可膨胀支撑构件的纵向轴线正交的弧且没有完全环绕所述可膨胀支撑构件地延伸,并且沿着所述可膨胀支撑构件的纵向轴线延伸的长度小于可膨胀支撑构件的全长;
抗膨胀增强元件,其位于可膨胀支撑构件的第一部分的外表面上,所述第一部分的膨胀至少由增强元件限制,使得所述第一部分的膨胀指数比所述第二部分的膨胀指数小;以及
由所述可膨胀支撑构件的第一部分所支撑的操作元件。
2.根据权利要求1所述的设备,其特征在于,所述第一部分和第二部分为所述可膨胀支撑构件的弧形部分,所述第一部分部分地环绕所述可膨胀支撑件。
3.根据权利要求1所述的设备,其特征在于,所述操作元件包括治疗仪器。
4.根据权利要求3所述的设备,其特征在于,所述治疗仪器包括从包含有双极射频(RF)电极阵列、RF单极电极及其组合的组所选择的构件。
5.根据权利要求3所述的设备,其特征在于,所述治疗仪器为适于将能量传递至组织的消融设备,所述消融设备选自包括有RF消融设备、微波消融设备、超声消融设备、电阻加热消融设备、化学消融设备、低温消融设备、用于传递可加热流体的消融设备、以及光学消融设备的组。
6.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件第一部分的表面面积基本上与操作元件的表面面积相同。
7.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件第一部分的膨胀指数为1。
8.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件第一部分的膨胀指数小于1.1。
9.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件第二部分的膨胀指数大于1。
10.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件第二部分的膨胀指数介于2至10之间。
11.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件的第二部分的膨胀指数涉及沿第一轴线的膨胀。
12.根据权利要求11所述的设备,其特征在于,所述可膨胀支撑构件的第二部分的膨胀指数还涉及沿不同于第一轴线的第二轴线的膨胀。
13.根据权利要求1所述的设备,其特征在于,当使可膨胀支撑构件膨胀时,操作元件的表面面积与可膨胀支撑构件第二部分的表面面积不同。
14.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件第一部分的壁厚度大于可膨胀支撑构件第二部分的壁厚度。
15.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件的壁厚度围绕包括支撑操作元件的轮廓的径向横截面而变化。
16.根据权利要求15所述的设备,其特征在于,所述可膨胀支撑构件的第一部分的壁厚度围绕包括支撑操作元件的轮廓的径向横截面而变化。
17.根据权利要求15所述的设备,其特征在于,所述可膨胀支撑构件的第二部分的壁厚度围绕包括支撑操作元件的轮廓的径向横截面而变化。
18.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件第二部分的壁厚度沿包括支撑操作元件的轮廓的纵向截面而变化。
19.根据权利要求16所述的设备,其特征在于,所述可膨胀支撑构件第二部分的第一部分的壁厚度沿包括支撑操作元件的轮廓的纵向截面而变化。
20.根据权利要求18所述的设备,其特征在于,所述可膨胀支撑构件第二部分的第二部分的壁厚度沿包括支撑操作元件的轮廓的纵向截面而变化。
21.根据权利要求1所述的设备,其特征在于,所述抗膨胀增强元件包括柔性的、不可扩张的聚合物支撑件。
22.根据权利要求21所述的设备,其特征在于,所述柔性的、不可扩张的聚合物支撑件包括聚酰亚胺。
23.根据权利要求1所述的设备,其特征在于,所述抗膨胀增强元件包括集成至操作元件的结构。
24.根据权利要求1所述的设备,其特征在于,第一部分包括第一成分且第二部分包括第二成分,所述第一部分和第二部分相连以便形成一体相连的可膨胀构件,所述第一成分的膨胀指数低于第二成分的膨胀指数。
25.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件包括气囊。
26.根据权利要求1所述的设备,其特征在于,所述可膨胀支撑构件基本上为圆柱形并包括纵向轴线,且所述第一部分被布置成使得与所述纵向轴线正交的弧没有完全环绕所述可膨胀支撑构件。
27.根据权利要求1所述的设备,还包括伸长构件,所述伸长构件适于将可膨胀支撑构件支撑于伸长构件的远侧部分处。
28.根据权利要求27所述的设备,其特征在于,所述伸长构件具有中心纵向轴线,且所述可膨胀支撑构件基本上为圆柱形并包括纵向轴线,伸长构件的中心纵向轴线及可膨胀支撑构件的纵向轴线平行。
29.根据权利要求27所述的设备,其特征在于,所述可膨胀支撑构件围绕伸长构件的远侧部分环绕地布置。
30.根据权利要求27所述的设备,其特征在于,所述伸长构件的至少一部分延伸通过可膨胀构件的全部长度。
31.根据权利要求30所述的设备,其特征在于,所述伸长构件的至少一部分向远侧延伸超过可膨胀构件。
32.一种医学设备,其包括:
适于联接至伸长构件的远侧部分的可膨胀支撑构件,所述可膨胀支撑构件具有第一弧形部分和第二弧形部分,其中,所述第一弧形部分被布置成占据与所述可膨胀支撑构件的纵向轴线正交的弧且没有完全环绕所述可膨胀支撑构件地延伸,并且沿着所述可膨胀支撑构件的纵向轴线延伸的长度小于可膨胀支撑构件的全长;
抗膨胀增强元件,其位于可膨胀支撑构件的第一弧形部分的外表面上,所述第一弧形部分的膨胀至少由增强元件限制,使得所述第一弧形部分的膨胀指数比所述第二弧形部分的膨胀指数小;以及
支撑于所述可膨胀支撑构件的第一部分上的操作元件。
33.根据权利要求32所述的设备,其特征在于,所述支撑构件的第一弧形部分具有与所述操作元件的表面面积大致相同的表面面积。
34.根据权利要求32所述的设备,其特征在于,所述第一和第二弧形部分适于使膨胀的大部分发生于第二弧形部分中。
35.根据权利要求32所述的设备,其特征在于,所述可膨胀支撑构件的第一弧形部分包括膨胀性基本上受限制的区域。
36.根据权利要求35所述的设备,其特征在于,所述膨胀性基本上受限制的区域包括所述抗膨胀增强元件。
37.一种医学设备,其包括:
适于联接至伸长构件的远侧部分的可膨胀支撑构件,所述可膨胀支撑构件具有基本上不可膨胀的局部圆周的弧形部分,所述弧形部分被布置成与所述可膨胀支撑构件的纵向轴线正交的弧且没有完全环绕所述可膨胀支撑构件地延伸,并且沿着所述可膨胀支撑构件的纵向轴线延伸的长度小于可膨胀支撑构件的全长;
抗膨胀增强元件,其位于基本上不可膨胀的部分的表面上,所述基本上不可膨胀的部分的膨胀至少由增强元件限制;以及
支撑于所述可膨胀支撑构件的基本上不可膨胀的部分上的操作元件。
38.根据权利要求37所述的设备,其特征在于,所述可膨胀支撑构件还包括可膨胀的弧形部分,所述操作元件仅支撑于可膨胀支撑构件的基本上不可膨胀的部分上。
39.根据权利要求38所述的设备,其特征在于,所述基本上不可膨胀的部分和可膨胀部分相互邻近。
40.根据权利要求37所述的设备,其特征在于,所述操作元件延伸少于设备的整个周长。
41.根据权利要求37所述的设备,其特征在于,所述设备在膨胀状态中大致为圆形且操作元件围绕设备延伸少于360度。
42.根据权利要求37所述的设备,其特征在于,所述操作元件被安装至可膨胀支撑构件上。
43.一种用于食道中的目标部位处的组织的消融的系统,其包括:
可膨胀构件,所述可膨胀构件适于联接至伸长构件的远侧部分且大小被设置为能置于食道中并且能在食道中膨胀,所述可膨胀构件具有第一部分和第二部分,其中,所述第一部分被布置成占据与所述可膨胀构件的纵向轴线正交的弧且没有完全环绕所述可膨胀构件地延伸,并且沿着所述可膨胀构件的纵向轴线延伸的长度小于可膨胀构件的全长;
抗膨胀增强元件,其位于可膨胀构件的第一部分的外表面上,所述第一部分的膨胀至少由增强元件限制,使得所述第一部分的膨胀指数比所述第二部分的膨胀指数小;以及
不可扩张地布置于可膨胀构件的第一部分上的RF消融元件,所述RF消融元件包括多个RF电极对,每个RF电极的宽度及邻近的RF电极之间的间距被选择为容许进行目标部位处的深度受控的消融。
44.根据权利要求43所述的系统,其特征在于,所述RF消融元件直接地布置于可膨胀构件的第一部分的局部圆周外表面上,所述第一部分基本上为不可扩张的,并且所述第二部分是可扩张的。
45.根据权利要求43所述的系统,还包括在第一部分的局部圆周外表面上成层的不可扩张的RF消融元件支撑件,所述RF消融元件布置于所述不可扩张的RF消融元件支撑件上。
46.一种用于食道中的目标部位处的组织的消融的医学设备,其包括:
可膨胀构件,所述可膨胀构件适于联接至伸长构件的远侧部分且大小被设置为能置于食道中并且能在食道中膨胀;
抗膨胀增强元件,抗膨胀增强元件位于可膨胀构件的第一部分的外表面上,使得所述第一部分的膨胀至少由抗膨胀增强元件抵抗,其中所述第一部分占据与所述可膨胀构件的纵向轴线正交的弧且没有完全环绕所述可膨胀构件地延伸,并且沿着所述可膨胀构件的纵向轴线延伸的长度小于可膨胀构件的全长;以及
在一电极支撑件上的多个RF电极对,每个RF电极的宽度及邻近的RF电极之间的间距被选择为容许进行目标部位处的深度受控的消融。
47.一种用于食道中的目标部位处的组织的消融的医学设备,其包括:
可膨胀构件,所述可膨胀构件适于联接至伸长构件的远侧部分且大小被设置为能置于食道中并且能在食道中膨胀,所述可膨胀构件包括:
局部圆周的第一弧形部分,其包括电极支撑件,并且被布置成占据与所述可膨胀构件的纵向轴线正交的弧且没有完全环绕所述可膨胀构件地延伸,并且沿着所述可膨胀构件的纵向轴线延伸的长度小于可膨胀构件的全长;
局部圆周的第二弧形部分;以及
抗膨胀增强元件,其位于可膨胀构件的第一弧形部分的外表面上,所述第一弧形部分的膨胀至少由增强元件限制,使得所述第一弧形部分的膨胀指数比第二弧形部分的膨胀指数小;以及
在所述电极支撑件上的多个RF电极,每个RF电极的宽度及邻近的RF电极之间的间距被选择为容许进行目标部位处的深度受控的消融。
