背景技术
各种蛋白尿肾病的最终共同途径是发展为终末期肾衰竭。蛋白尿程度与肾病的进展速度有关。
人类蛋白尿疾病中,肾小管间质损伤能够比肾小球损害更好地预测肾功能衰退。经过滤的肾小管毒性血浆蛋白被认为导致了此关系,虽然这些蛋白质的性质仍不确定。经过滤的蛋白质部分地通过激活近端肾小管细胞(PTC)发挥其有害作用,这些细胞分泌趋化因子和细胞因子,从而导致炎症、间质成纤维细胞的肌转化(myotransformation)、纤维化和细胞凋亡。这最终将导致终末期肾衰竭。因此,肾小管上皮细胞被认为是间质损害进展过程中的关键。有人提出蛋白尿是肾小管损伤和间质纤维化的诱因。

发明概述
本发明提供了用于诊断或预测肾脏和肝脏的状态、功能和完整性的试验和装置。可将本发明的生物标志物、其他临床化学参数或其组合物转换成用于诊断肾脏或肝脏状态的化验、试验或装置,以监测不良事件如药物引起的肾毒性或肝毒性,以根据肾脏或肝脏状态调整药物治疗方案,或以开展用于诊断肾脏或肝脏疾病或其患病率的临床试验。

定义:本文使用的“肾毒性”或“肾损伤”或类似的“肾脏疾病”等表达都表示一种突然的(急性)或随时间逐渐衰退(慢性)的肾衰竭或功能紊乱,可能由多种疾病引发;或表示疾病过程,包括(但不限于)急性肾毒性:I型败血症(感染)、休克、创伤、肾结石、肾感染、药物中毒、毒药或毒物或用碘对比染料注射后(副作用);以及慢性肾毒性:长期高血压、糖尿病、充血性心力衰竭、狼疮或镰刀型细胞贫血。这两种肾衰竭形式都会导致危机性命的代谢紊乱。

肾脏损伤分子-1(Kim-1)是一种1型膜蛋白,含有细胞外6个半胱氨酸免疫球蛋白结构域。Kim-1mRNA和蛋白质在正常肾脏中的表达水平很低,几乎无法检测到,但是其量会在缺血后的肾脏中的近端肾小管中急剧增加。Kim-1与缺血后肾脏的形态完整和功能修复有关。Ichimura T等人,《生物化学杂志》(J.Biol.Chem.)273:4135-4142(1998)。Kim-1有时被称为“Havcr 1”。不同损伤类型后的早期大量肾小管表达使Kim-1成为肾小管细胞损伤的特异标志物,这种损伤可能与间质损害过程的恢复或损害机制有关。见WO 2004/005544,以引用的方式并入本文。
α-2u球蛋白相关蛋白质(α-2u),在人体中又被称为脂质运载蛋白2(LCN2)或嗜中性粒细胞明胶酶相关脂质运载蛋白(NGAL),储存于嗜中性粒细胞的颗粒中。它与小的亲脂性物质结合,被认为在炎症反应中起作用。Bundgaard JR等人,《生物化学与生物物理学研究通讯》(Biochem.Biophys.Res.Commun.)202:1468-1475(1994);Cowland JB和BorregaardN,《基因组学》(Genomics)45:17-23(1997);Zerega B等人,《欧洲细胞生物学杂志》(Eur.J.Cell Biol.)79,165-172(2000)。
骨桥蛋白(OPN),又被称为分泌型磷蛋白1(SPP1),是一种分泌型强酸性糖基化磷蛋白,含有精氨酸-甘氨酸-天冬氨酸(RGD)细胞粘附基序。已在多个肾脏损伤模型中阐述了OPN增量调节,它可能在组织重塑和修复过程中起作用。Persy VP等人,《国际肾脏期刊》(Kidney Int.)56(2):601-611(1999)。骨桥蛋白是泌尿系统草酸钙结石的一种主要成分。Kohri K等人,《生物化学与生物物理学研究通讯》(Biochem.Biophys.Res.Commun.)84:859-864(1992)。此外,OPN在大鼠远端肾小管细胞中被高表达,倾向于形成泌尿系统结石。Kohri K等人,《生物化学杂志》(J.Biol.Chem.)268:15180-15184(1993).
Podocin,又被称为PDCN、SRN1、肾病2,自发性,是一种在肾脏足细胞中表达的蛋白质,在肾小球通透性调节中起作用。它几乎只在胎儿和成熟肾的肾小球足细胞中表达。足细胞蛋白例如podocin的突变体导致先天性局灶节段性肾小球硬化。Komatsuda A等人,《肾衰竭》(Ren.Fail.)25(1):87-93(2003)),并且主要与抗类固醇的肾病综合征有关。
已知血管内皮生长因子(VEGF)能够促进血管生成,增加血管通透性,对单核细胞有趋化作用,并对糖尿病、伤口愈合、炎症反应和组织重塑起作用。Benjamin LE,《美国病理学杂志》(Am J Pathol.)158:1181-1184(2001)。
由于每种标志物都与不同的肾脏病理结果有关,因此可以鉴定这些与肾脏病理特别有关的标志物的基因表达产物。例如,钙结合蛋白-D28k水平被用作矿化作用钙干扰预测的早期标志物。Kim-1水平是一般性肾损伤的标志物。OPN水平是通常与肾毒性有关的巨噬细胞浸润的早期标志物,和肾损伤时组织重塑的标志。EGF水平是一般性肾毒性的早期标志物。簇集蛋白水平是免疫介导的肾毒性的早期标志物。
表2列出了在尿液和血液中确定的临床化学参数。表2中的这些生物标志物的测定将在后文描述。
体内研究和组织病理学结果。10例大鼠体内研究全部成功进行。可以从在第3、7、14、21和22天终止的几乎所有动物收集到足够尿液。仅分析能获得完整数据集(肾脏组织病理、尿液临床化学、尿蛋白生物标志物、血液临床化学、血浆蛋白生物标志物、肾脏mRNA生物标志物)的动物并在此报导。总共分析了来自739只动物的数据,包括447只用肾毒物处理的动物、188只用载体处理的动物和104只用肝毒物处理的动物。存在9个样本未获得尿液Kim-1测定值,存在84个样本未获得血液Kim-1测定值。这些样本被排除在Kim-1分析之外,但是被包含于其他参数和生物标志物分析之内。
图2显示了组织病理学评估的等级分布。其中大多数为1级和2级,缺乏5级损害,这表明主要诱导轻微肾毒性的目标已实现。施用载体和施用肝毒物的动物中有相当数量的1级,还有一些2级结果,这表明了在此项目中进行的组织病理学评估的灵敏度水平。用肝毒物未诱导出具有明显剂量依赖的肾毒性。图3详细列出了报道的所有局部损害,包括每个级别的发生数,并分为施用肾毒物的动物和对照动物(施用载体和施用肝毒物)。观察到七十五种不同的局部结果。
通常不能单独观察到不同类型的损害,这些损害通常是以高度关联的方式被观察到,代表不同分子过程。图4显示了顺铂试验的所有组织病理学结果。此过程不影响肾单位的单一孤立部分,但是以关联方式影响多个部分(例如近端肾小管和降直肾小管)。此外,能够观察到按剂量和时间排序动物的分子过程的逻辑顺序。观察到坏死这种损害结果,然后观察到再生过程,例如嗜碱性粒细胞增多和有丝分裂,但是平行地还观察到继发过程(例如肾小管扩大、肾小管扩张、细胞脱落等)。
对于鉴定生物标志物,存在两种结果:一,应发现适当的局部化水平,最好与生物标志物表达相对应。二,单独的组织病理学描述应被纳入分子过程,此过程与触发生物标志物表达的分子事件相对应。因此本文将局部损害并入10个不同过程,对于每个样本,相应局部损害的最高级别被分配到综合病理。特别研究了下列10个病理过程:
1.近端肾小管损伤:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为定位于任一肾小管S1、S2或S3段或不可定位的“坏死”、“细胞凋亡”或“肾小管细胞脱落”。
2.肾小球改变/损伤:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为“系膜细胞增殖”、“系膜细胞增大”、“肾小球空泡化”或“间质性鲍氏囊纤维化”。
3.肾小管损伤/再生/扩张:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为定位于任一肾小管S1、S2或S3段、亨利袢、粗升肾小管、远端肾小管、或集合管或不可定位的“坏死”、“细胞凋亡”、“肾小管细胞脱落”、“嗜碱性粒细胞增多”或“有丝分裂增加”,或位于皮层、髓质或乳突的“肾小管扩张”。
4.肾小球改变/损伤或肾小管损伤/再生/扩张:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为“系膜细胞增殖”、“系膜细胞增大”、“肾小球空泡化”或“间质性鲍氏囊纤维化”或定位于任一肾小管S1、S2或S3段、亨利袢、粗升肾小管、远端肾小管、或集合管或不可定位的“坏死”、“细胞凋亡”、“肾小管细胞脱落”、“嗜碱性粒细胞增多”或“有丝分裂增加”,或位于皮层、髓质或乳突的“肾小管扩张”。
5.粗升肾小管损伤/再生:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为定位于粗升肾小管的“坏死”、“细胞凋亡”、“嗜碱性粒细胞增多”或“有丝分裂增加”。
6.远端肾小管损伤/再生:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为定位于远端肾小管的“坏死”、“嗜碱性粒细胞增多”或“有丝分裂增加”。
7.集合管损伤/再生:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为定位于集合管的“坏死”、“细胞凋亡”、“肾小管细胞脱落”或“嗜碱性粒细胞增多”。
8.肾小管矿化:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为定位于任一肾小管S1、S2或S3段、亨利袢、粗升肾小管、远端肾小管、或集合管的“管内型-矿化”。
9.管内透明型:下列类型的1级(“最小”、“很少”、“很小”)或较高级损害,所述类型为定位于任一肾小管S1、S2或S3段、亨利袢、粗升肾小管、远端肾小管、或集合管的“管内型-玻璃样(蛋白质的、染色的)”。
10.肾小球旁器肥大:图5中列出了在10例试验的动物肝脏中观察到的病状。
分析说明:采用经验证的由RulesBasedMedicine(Texas,USA)开发的和可获得的多重蛋白质检测法测定蛋白质生物标志物。采用标准临床化学分析方法(ADVIA 1650装置)测定临床化学参数(例如,BUN、NAG、肌酸酐和LDH)。使用来自Applied Biosystems的TaqMan基因表达测试法在Applied Biosystems 7900HT实时PCR仪上于低密度阵列上测定mRNA生物标志物。已采用标准程序从半个肾脏中提取mRNA。
数据预处理说明。按照下列步骤对从分析装置得到的生物标志物浓度值进行预处理:(1)用定量上限的浓度值替代定量上限以上的浓度值;(2)用定量下限的浓度值替代定量下限以下的浓度值;(3)将每个样本用相应尿肌酸酐值除,使尿液生物标志物标准化为尿肌酸酐浓度;和(4)将每个值用时间匹配的和研究匹配的对照组(通常为6只动物)的算数平均值除,以倍数变化表示数据。
数据分析说明。数据的定量分析基础均是受试者工作特征分析(ROC)。在ROC分析中,由于二元判定问题(例如对照动物与患病动物),判定阈值随发生系统性改变,真阳性(灵敏性)与真阴性(1-特异性)会被标出。曲线下面积(AUC)是将灵敏性和特异性合并成一个值的诊断能力测定,其中随机鉴别器与0.5AUC对应,而理想鉴别器与1对应。通过梯形积分法计算AUC,如Bradley AP,Pat.Recogn.30(7):1145-1159(1997)所述。通过Wilcoxon统计的标准误差计算AUC的标准误差,如Bradley(1997)所述。
作为给定的生物标志物和病理的ROC分析的一个应用实例,生物标志物/病理的阈值按以下方式建立了预定义的最低特异性:
首先为给定的最小特异性(例如95%)确定最小特异性以上或在最小特异性处的全部可能的最低确切特异性。对于确切特异性,选择具有最高相应灵敏性的阈值。此算法不考虑通过进一步改变阈值产生针对此灵敏性的更高特异性的可能。本文报道了此阈值的相应特异性和灵敏性。
对于数据分析,采用了对照动物和患病动物的下列定义:(a)对照组:施用载体或肝毒物的动物,它们不表现出所研究的损害。(b)患病组:施用肾毒物的动物,它们在组织病理学评估中被分配了1级或较高级的目标损害。
由于在这些试验中未观察到剂量相关的肾毒性,因此将施用肝毒物的动物划分到对照组,以测试标志物与其他肝毒性变化相比较的特异性。
考虑到这些动物不“干净”,因此在ROC中不考虑给药但是未表现出任何损害或仅表现出其他损害(不是组织病理学方面的特定目标损害)的动物。这组动物代表一种未定义状态,特别是当一种分子反应比组织病理学表现更早更快时。在这种情况下,无法判定(1)一侧肾脏的一份组织病理学切片是否无法代表肾脏的全部状态(假阴性组织病理);(2)分子反应是否比组织病理更早/更灵敏,一种所谓的前兆状态;或(3)如果组织病理是唯一的真实状况(假阳性生物标志物),这些动物是否是假阳性。处理这些动物的最保守的方法是将这些动物排除在任何分析之外,因为无法将其明确分配给任何组。
目标生物标志物和病理的肾损伤监测结果。图6中显示了大多数感兴趣的肾脏病理和生物标志物的结合了组织病理学、研究信息和生物标志物值的ROC分析结果。
监测和鉴定肝损伤的蛋白质和临床化学参数。这些生物标志物来自动物,这些动物施用了载体或不同剂量的肝毒物ANIT(α-萘异硫氰酸酯)。见图7。在肝脏中观察到与剂量相关的组织病理检查结果,但是在肾脏中未观察到。图中显示了相对浓度水平(与对照物相较的倍数变化)。剂量水平用不同颜色的阴影表示,采样时间点用标签表示。生物标志物水平能很好地反映剂量,因此能很好地反映与剂量相关的肝毒性。这些图中描绘的生物标志物为当前的标准在血浆中测定的ALAT(图7A顶部)和在血浆中测定的新标志物脂质运载蛋白-2(图7A底部)、在血浆中测定的GST-mu(图7B顶部)和在尿液中测定的脂质运载蛋白-2(图7B底部)。
监测和鉴定肾损伤的蛋白质和临床化学参数。这些生物标志物来自动物,这些动物施用了载体或不同剂量的肾毒物顺铂。见图7。图中显示了相对浓度水平(与对照物相比较的倍数变化)。剂量水平用x轴上的标签表示,采样时间点用样本标签表示。样本根据在肾脏中观察到的组织病理结果的最高等级进行着色。横条表示在病例对照动物(在肾脏中无组织病理结果的施用载体的动物)中观察到的最高标志物水平。高于横条的蛋白质水平增加明显(病例对照的特异性为100%)。也可能存在其他阈值。图6中给出了95%特异性的具体阈值。用不同临床参数和蛋白质检测肾损伤,具有不同的灵敏度。在血清中测定肌酸酐,这是当前用于肾损伤和肾功能的标准外周测试,是最不灵敏的方法(图8A顶部)。在血清中测定的半胱氨酸蛋白酶抑制剂C(图8A底部)、在尿液中测定的Kim-1(图8B顶部)、在尿液中测定的脂质运载蛋白-2(图8B底部)、在尿液中测定的骨桥蛋白(图8C顶部)和在尿液中测定的簇集蛋白(图8C底部)鉴定了更多具有显著肾脏结果的动物。
监测和鉴定肾损伤的基因转录。这些生物标志物来自动物,这些动物施用了载体或肾毒物顺铂。见图9。图中显示了表达值(Ct值参照Polr2g)。剂量水平用x轴上的标签表示,采样时间点用样本标签表示。样本根据在肾脏中观察到的组织病理结果的最高等级进行着色。横条表示对照动物中观察到的最高表达(图中的最低值)。横条下的值对应显著增加的表达水平(未给药动物的特异性为100%)。也可能存在其他阈值。图6中提出了95%特异性的具体阈值。Kim-1(图9A)和Cyr61(图9B)都能以一种比组织病理学检查更灵敏的方式(在较低剂量水平)更早检测到(在较早时间点)肾损伤。
本发明的实施方案和涉及的方面。本发明利用文中所述的数学运算和数据,提供了下列实施方案:
本发明通过将本发明的一种生物标志物或生物标志物的组合物(临床化学参数、蛋白质、mRNA转录)用于生物矩阵(尿液、血液、来自肾脏和血液的mRNA),从而提供了对肾毒性和/或肝毒性进行诊断、预测和分类的方法。本发明还提供了借助在一个生物矩阵中或不同生物矩阵中对单一生物标志物或生物标志物的组合物(临床化学参数、蛋白质、mRNA转录)进行的测定,对组织病理学结果、组织病理学结果组合在肾脏、肝脏或肾脏和肝脏中的组织病理学结果表现出的机制进行诊断、预测和分类的方法。在使用本发明时,肾脏中的组织病理学结果对应一种肾毒性或肾脏疾病症状,肝脏中的组织病理学结果对应一种肝毒性或肝脏疾病症状。
因此,可以确定个体中标志物的表达水平或功能水平以选择对个体合适的药剂进行治疗性治疗或预防性治疗。了解了这些,当应用于给药或药物选择时,可避免不良反应或治疗失败,因此当用标志物表达的调节剂治疗受试者时,可增强治疗或预防效果。见WO 2004/005544,以引用的方式并入本文作为参考。
本发明还提供了借助生物矩阵中的单一生物标志物和生物标志物的组合物对生物矩阵中的单一生物标志物或生物标志物的组合物进行诊断、预测和分类的方法。
本发明还提供了借助在生物矩阵中对本发明的生物标志物进行的测定来预测个体的命运(例如存活率)的方法。
一方面,本发明显示了可以用本发明的生物标志物对组织病理学结果、组织病理学结果组合和肾脏、肝脏或肾脏和肝脏中的组织病理学结果表现出的机制进行预测、分类和诊断,比起其他生物标志物,本发明的标志物具有更高的灵敏性和特异性,时间点更早,剂量水平更低。
另一方面,本发明显示了可在早于肝脏和肾脏的组织病理学评估的研究过程中在此研究时间点用本发明的生物标志物对组织病理学结果、组织病理学结果组合和肾脏、肝脏或肾脏和肝脏中的组织病理学结果表现出的机制进行预测、分类和诊断。这意味着借助生物标志物进行的组织病理学建模、诊断、分类和预测比组织病理学本身的评估更早、更灵敏、更具特异性。
本发明提供的方法不仅可以与本发明所述的生物标志物一起使用,也可以与其降解产物或所述生物标志物的组合物及其降解产物一起使用。降解产物可以为所述蛋白质的肽或肽片段或所述mRNA转录的mRNA片段及其降解产物。此外,本发明提供的方法不仅可以使用所述生物标志物,还可以使用基本上相似的生物标志物,例如不同的同工型、剪接变体或片段。在一些实施方案中,本发明提供的方法可以利用本发明所述生物标志物的基因的多态现象。
本发明还提供了在一个装置中实施这些标志物、这些生物标志物和生物标志物的模型、关联、预测、分类和诊断与组织病理和其他生物标志物的结合方法。特别是本发明提供了用于监测或诊断肾脏的状态和功能或肝脏的状态和功能的试验、装置、化验、生物标志物检测、临床检测试剂盒或诊断装置。在另一实施方案中,本发明提供了用于监测或诊断对肾脏或肝脏的副作用的试验、装置、化验、生物标志物检测、临床检测试剂盒或诊断装置。在另一实施方案中,本发明提供了用于鉴定不会引起或诱导对肾脏或肝脏的副作用的候选试剂(药物)的试验、装置、化验、生物标志物检测、临床检测试剂盒或诊断装置。在另一实施方案中,本发明提供了确定个体是否会对关于肾毒性、肝毒性或肝肾毒性的治疗有反应的试验、装置、化验、生物标志物检测、临床检测试剂盒或诊断装置。
试剂盒的组分可用于采用杂交印迹分析、逆转录PCR或实时定量PCR、分支DNA、基于核酸序列的扩增(NASBA)、转录介导的扩增、核糖核酸酶保护分析或微阵列测定基因表达。另一特定实施方案提供了一种试剂盒,其中确定基因表达水平或基因表达产物的工具包含至少一种针对一种蛋白质(由选自本发明的生物标志物的标志物基因编码)的特异性抗体。这种抗体优选地选自多克隆抗体、单克隆抗体、人源化抗体或嵌合抗体、以及生物功能性抗体片段,其中所述生物功能性抗体片段足以用于将该抗体片段结合到标志物。特别优选的是确定基因表达水平的免疫测定方法。
在本发明的另一个优选实施方案中,提供了一种试剂盒,此试剂盒包含用于获得个体的身体样本、特别是血浆或尿液的工具。一个特别优选的实施方案还包含一种容器,适合容纳用于确定基因表达水平的工具和个体的身体样本。在另一优选实施方案中,此试剂盒包含使用说明和试剂盒结果解释。
在一些实施方案中,本发明提供了用于(i)鉴定能够改善肾毒性的一种或多种症状的候选试剂(药物);(ii)鉴定能够改善肝毒性的一种或多种症状的候选试剂(药物);(iii)比较两种试剂(药物)的潜在肾毒性;(iv)比较两种试剂(药物)的潜在肝毒性的试验、装置、化验、生物标志物检测、临床检测试剂盒或诊断装置。
例如,可以在接受肾病或毒性治疗的受试者的临床试验中监测一种试剂影响标志物表达的效果。在一优选实施方案中,本发明提供了一种用某种试剂(例如,激动剂、拮抗剂、拟肽类药物、蛋白质、肽、核酸、小分子或其他候选药物)监测受试者的治疗效果的方法,包括下列步骤:(i)在受试者施用试剂前才从受试者处取施用前样本;(ii)检测施用前样本中一种或多种所选的本发明所述标志物的表达水平;(iii)从受试者处取一份或多份施用后样本;(iv)检测施用后样本中标志物的表达水平;(v)比较施用前样本中的标志物表达水平与施用后样本中的标志物表达水平;和(vi)根据比较结果,改变对受试者施用的试剂。例如,经调整过的试剂施用能够将标志物的表达提高到比检测到的水平高一级的级别,即提高试剂效力。或者,希望通过增加/减少试剂的施用来分别地提高/降低试剂效力。
在一特定实施方案中,本发明提供了一种鉴定用于肾毒性治疗的候选试剂的方法,包括下列步骤:(a)用候选试剂接触中毒肾脏组织样本;(b)从肾脏组织确定所选本发明生物标志物的一种或多种基因相对应的基因表达水平或基因表达产物特别是蛋白质的水平,以获得第一组值;(c)将该第一组值和与基因表达水平或基因表达产物特别是蛋白质的水平相对应的第二组值比较,其中所述第二组值是在与(b)步骤相同的条件下对不是该候选试剂诱导的中毒肾脏组织内的相同基因评估的。
在另一特定实施方案中,本发明提供了鉴定不会引起或诱导肾毒性的候选试剂的方法,包括下列步骤:(a)用候选试剂接触未中毒肾脏组织样本;(b)从肾脏组织确定所选本发明生物标志物的一种或多种基因相对应的基因表达水平或基因表达产物特别是蛋白质的水平;(c)将该第一组值和与基因表达水平或基因表达产物特别是蛋白质的水平相对应的第二组值比较,其中所述第二组值是在与(b)步骤相同的条件下对未中毒肾脏组织内的相同基因评估的。
在另一实施方案中,本发明提供了一种比较两种候选药物的潜在肾脏细胞毒性的方法,包括下列步骤:(a)用第一种候选药物接触未中毒肾脏组织样本,从肾脏组织确定所选本发明生物标志物的一种或多种基因相对应的基因表达水平或基因表达产物特别是蛋白质的水平,以获得第一组值;(b)用第二种候选药物接触未中毒肾脏组织样本,从肾脏组织确定所选本发明生物标志物的一个或多个基因相对应的基因表达水平,以获得第二组值;和(c)比较第一组值和第二组值。
在另一实施方案中,本发明提供了一种通过施用治疗有效量的一种调节化合物治疗个体患者的肾毒性和/或肝毒性的治疗方法,其中所述化合物调节肝脏和/或肾脏中一种或多种基因、基因表达产物或表1所列的蛋白质的合成、表达或活性,从而改善至少一种肾脏和/或肝脏中毒症状。
在本发明的一个特定方面,提供了一种预防和治疗兼备的方法,治疗患有肾脏疾病或肾毒性或有患肾脏疾病或肾毒性风险的患者。可以在肾脏疾病症状特征表现出来之前施用预防试剂,从而阻止或延迟肾脏疾病的发展。

因此本发明提供了在培养的肾脏组织或细胞中测定上述生物标志物的试验,以对体内肾毒性和/或肝毒性预测、分类或诊断。可能是单个或一群肾脏或肝脏细胞,例如人肾脏上皮293T细胞、HK2细胞系、人胚胎肾脏细胞系或人胚胎肝脏细胞系。当细胞暴露于各种改变的条件下时,例如接触药物或其它活性分子时,表达谱相对于基线谱的改变可用于指示此类作用。
培养的肾脏组织或细胞可有利地基于模拟人类细胞和组织疾病优选地肾脏疾病的体内动物模型。它也可以是单个或一群肾脏细胞,例如人肾脏上皮293T细胞或人胚胎肾脏细胞系。由于肾脏的功能特性,肾脏对药物的肾毒性作用很敏感,这些功能特性包括(a)肾血流量大,因此带来大量毒素;(b)肾小球或肾小管上皮与药物的接触面积大,因此促进了毒素的交互作用或摄取;(c)肾脏转移活性物质的能力提供了介导细胞摄取的特定转移机制;(d)药物分解可能发生在肾小管内,从而导致非毒性母体物质产生有毒代谢产物;(e)肾脏的浓缩机制能够增加非吸收产物的尿液和间质浓度;正常功能所需的肾小管细胞高代谢率,这种高代谢率易受干扰。
