Definitions
- This invention relates to a blood pressure pulse wave measuring device.
- the lower extremity upper limb blood pressure ratio (Ankle Brachial Index: ABI) is an index that represents clogging of the blood vessels
- the pulse wave velocity Pulse Wave Velocity: PWV
- ABI Ankle Brachial Index
- PWV Pulse Wave Velocity
- ABI and PWV are indices that can be used alone, but when, for example, arteriosclerosis has spread throughout the body, a value in the normal range may be obtained when only ABI is measured, and the value of ABI is normal. In some cases, it has been found that grasping the progress of the entire arteriosclerosis by PWV helps a more accurate diagnosis.
- Patent Document 1 Japanese Patent Laid-Open No. 2000-316821
- four cuffs for the upper arm and an ankle are connected to the main body.
- a device is known that attaches and measures these cuffs to the extremities of a subject.
- the piping extending from substantially the same place is extended and attached to the upper arm and the ankle of the subject.
- a plurality of pipes are entangled with each other, which is very labor intensive and time consuming.
- an object of the present invention is to provide a blood pressure pulse wave measurement device that can easily attach a cuff to the upper arm and ankle of a subject without causing a plurality of pipes to be entangled with each other.
- a blood pressure pulse wave measuring device of the present invention is A blood pressure pulse wave measuring device for measuring a lower limb upper limb blood pressure ratio and a pulse wave propagation velocity, A first cuff for pressing a part of the subject's upper limb; A second cuff for pressing a part of the lower limb of the subject; First and second pipes individually connected to the first and second cuffs; A first detector connected to the first cuff via the first pipe and detecting a first pulse wave propagating through the first pipe; A second detection unit connected to the second cuff via the second pipe and detecting a second pulse wave propagating through the second pipe; A control unit for calculating the ankle brachial blood pressure ratio and the pulse wave velocity using the first and second pulse waves, The first detection unit and the control unit are arranged in a first casing, and the second detection unit is arranged in a second casing.
- the pulse wave velocity is typically the brachial-ankle pulse wave velocity baPWV (brachial-ankle Pulse Wave Velocity) and the heart-ankle artery pulse wave velocity haPWV (heart-ankle). Pulse Wave Velocity).
- the cardiac ankle vascular index (CAVI) (Cardio Ankle Vascular ⁇ Index), which is an index reflecting the stiffness of the artery from the heart to the ankle, corrects the heart-ankle arterial pulse wave velocity haPWV with a logarithmic pulse wave. It is possible to calculate by this.
- the lower limb upper limb blood pressure ratio typically indicates an ankle upper arm blood pressure ratio ABI (Ankle Brachial Pressure Index).
- the first casing to which the upper arm cuff is connected and the second casing to which the ankle cuff is connected are separated. Therefore, when the upper arm and ankle cuffs are attached to the upper arm and ankle of the subject, the second housing can be disposed in the vicinity of the ankle. Therefore, it is possible to easily attach the respective cuffs to the limbs of the subject without the pipes connected to the upper arm cuff and the pipe connected to the ankle cuff being entangled with each other.
- the first detection unit includes: A first pressure pump for sending air to the first cuff via the first pipe and pressurizing the first cuff; A first pressure sensor that detects a pulse wave propagating through the first pipe,
- the second detection unit includes A second pressure pump that feeds air into the second cuff through the second pipe and pressurizes the second cuff; And a second pressure sensor for detecting a pulse wave propagating through the second pipe.
- the first casing to which the upper arm cuff is connected and the second casing to which the ankle cuff is connected are separated. Therefore, the length of the first pipe and the length of the second pipe can be adjusted to be substantially the same. Accordingly, since the timing at which the pulse wave detected by the upper arm is detected and the timing at which the pulse wave detected by the ankle is detected can be made substantially the same, the measurement accuracy of ABI and PWV can be improved. It becomes.
- the control unit calculates a blood pressure value of the subject based on the first pulse wave.
- the first casing to which the upper arm cuff is connected and the second casing to which the ankle cuff is connected are separated. Therefore, when only the blood pressure of the subject is measured, it is possible to measure the blood pressure by attaching only the upper arm cuff to the subject's upper arm.
- the pulse wave velocity includes a brachial-ankle pulse wave velocity (baPWV) and a heart-ankle pulse wave velocity haPWV (heart-ankle pulse wave velocity),
- the control unit calculates a Cardio Ankle Vascular Index (CAVI) by correcting the heart-ankle arterial pulse wave velocity haPWV with a logarithmic pulse wave.
- CAVI Cardio Ankle Vascular Index
- the blood pressure pulse wave measuring device it is possible to calculate the cardiac ankle vascular index based on the heart-ankle arterial pulse wave velocity haPWV.
- the blood pressure pulse wave measuring device of the present invention it is possible to easily attach the cuff to the upper arm and ankle of the subject.

Accordingly, since the timing at which the pulse wave detected by the upper arm is detected and the timing at which the pulse wave detected by the ankle is detected can be made substantially the same, the measurement accuracy of ABI and PWV can be improved. It becomes.
- the blood pressure pulse wave measuring apparatus 100 performs blood pressure value measurement by a known oscillometric method and pulse wave detection as shown in a processing flow of FIG. 4 to be described later under the control of the control unit 2 (particularly the CPU 10).
- the upper arm-ankle pulse wave propagation velocity baPWV (brachial-ankle Pulse Wave Velocity) and the heart-ankle arterial pulse wave propagation velocity haPWV (heart-ankle Pulse Wave Velocity) are obtained as the pulse wave propagation velocity, and the lower limb upper limb
- the ankle upper arm blood pressure ratio ABI (Ankle Brachial Pressure Index) is obtained as the blood pressure ratio.
- the control unit 2 calculates the ankle-brachial blood pressure ratio (ABI) and the pulse wave velocity (PWV) using the pulse waves detected by the detection units 20br, 20bl, 20al, and 20ar. Moreover, the control part 2 calculates a test subject's blood-pressure value using the pulse wave detected by 20br, 20bl. Further, the control unit 2 also calculates an index such as CAVI (Cardio Ankle Vascular Index) as a heart-ankle blood vessel index calculated based on the heart-ankle arterial pulse wave velocity haPWV. Here, the control unit 2 calculates CAVI by correcting the heart-ankle arterial pulse wave velocity haPWV with a logarithmic pulse wave. As is known, the brachial-ankle pulse wave velocity baPWV and the heart-ankle arterial pulse wave velocity haPWV are indicators of blood vessel stiffness, and the ankle brachial blood pressure ratio ABI is It is an index indicating clogging.
- FIG. 4 is a flowchart showing a lower limb upper limb blood pressure ratio and pulse wave velocity measurement process executed by the blood pressure pulse wave measuring apparatus 100 of FIG. Specifically, when the measurement is started, as shown in step S1 of FIG. 4, the pump 25 in each detection unit 20 is driven to start the pressure increase of each cuff 24.
- step S2 while monitoring the cuff pressure by the pressure sensor 28, the cuff pressure is increased to a predetermined pressure (pressure higher than the maximum blood pressure of the subject 200) and the pump 25 is stopped (cuff pressure increase is completed).
- step S3 the regulating valve 26 is controlled to start pressure reduction of each cuff 24, and the cuff pressure is gradually reduced. In this decompression process, a change in arterial volume occurring in the artery at the measurement site is detected as a pulse wave signal by the pressure sensor 28 via each cuff 24.
- step S4 based on the amplitude of the pulse wave signal, a predetermined algorithm based on a known oscillometric method is applied to apply systolic blood pressure (systolic blood pressure: systolic blood pressure) and diastolic blood pressure (diastolic blood pressure). : Diastolic Blood Pressure) (blood pressure measurement).
- the pulse unit: beats / minute
- the calculation of blood pressure is not limited to the decompression process, and may be performed in the pressurization process.
- step S5 the regulating valve 26 is closed, and the cuff pressure is maintained at a specified pressure (for example, about 50 mmHg).
- a specified pressure for example, about 50 mmHg.
- the CPU 10 works as a pulse wave propagation velocity acquisition unit, and based on the pulse wave measured by the pressure sensor 28, the pulse wave propagation that is an index representing the hardness of the blood vessel of the subject. Get speed
- step S7 in FIG. 4 the adjustment valve 26 is fully opened to release the cuff pressure.
- step S8 CPU10 works as a display process part and displays a measurement result on the display screen 40 (refer FIG. 2) provided in the upper surface of the main unit 101.
- the configuration for detecting the pulse wave using the pressure sensor 28 has been described.
- the configuration may be such that the pulse wave is detected using an arterial volume sensor (not shown).
- the arterial volume sensor may include, for example, a light emitting element that emits light to the artery and a light receiving element that receives transmitted light or reflected light of the artery irradiated by the light emitting element.
- a plurality of electrodes may be included, a small constant current may be passed through the measurement site of the subject 200, and a voltage change caused by a change in impedance (biological impedance) generated according to the propagation of the pulse wave may be detected.
