Abstract
본 발명은 비만 억제능을 갖는 균주 및 이를 함유하는 약학 조성물에 관한 것으로, 더욱 상세하게는 장 내용액(gut fluid content)에 녹아 있는 지방산 농도를 감소시킴으로써 비만 억제능을 갖는 균주 및 이를 함유하는 비만 또는 비만으로 야기된 대사성 질환의 예방 또는 치료를 위한 약학 조성물에 관한 것이다. 본 발명에 따른 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시킴으로써 비만 억제능을 갖는 장내 균주는 동물실험과 임상시험에서 모두 부작용이 전혀 없으면서도 널리 이용되고 있는 대표적인 비만치료제인 orlistat과 비슷한 정도의 비만치료 효능을 보였다. 따라서 본 발명에 따른 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시킴으로써 비만 억제능을 갖는 장내 균주를 이용할 경우, 현재 비만으로 고통 받고 있는 환자들이 보편적으로 활용할 수 있는 비만 치료제로 개발되어 인류의 보건에 크게 기여할 것으로 기대된다.
Description
본 발명은 비만 억제능을 갖는 균주 및 이를 함유하는 약학 조성물에 관한 것으로, 더욱 상세하게는 장 내용액(gut fluid content)에 녹아 있는 지방산 농도를 감소시킴으로써 비만 억제능을 갖는 균주 및 이를 함유하는 비만 또는 비만으로 야기된 대사성 질환의 예방 또는 치료를 위한 약학 조성물에 관한 것이다.
비만은 전 세계적으로 문제가 되고 있는 만성질환으로 효과적인 치료방법이 없고 또한 계속 증가추세에 있는 심각한 질환이다. 비만은 다른 질환과 다르게 대사성증후군, 고혈압, 당뇨, 고지혈증, 동맥경화증, 허혈성 심장질환, 지방간, 담석증 등 관련질환이 동반되는 특징이 있다.
비만은 그 자체로 심각한 질병이지만 미용상 문제도 야기하기 때문에 그동안 세계 각국에서 다양한 비만치료제들을 개발하고자 노력하여, 여러 비만치료제를 개발하였다. 현재까지 개발된 비만치료제는 크게 식욕저하제 계열의 비만치료제와 지방분해저해제 계열의 비만치료제로 나뉜다.
식욕저하제 계열의 비만치료제들은 중추신경계에 작용하여 식욕을 떨어트림으로써 음식물의 섭취량을 줄여주는 작용기전으로 fenfluramine, sibutramine, rimonabant, phentermine 등이 있다. 식욕억제제 계열의 비만치료제는 체중감소 효능이 뛰어나다는 장점이 있지만, 이들 식욕억제제 계열의 비만치료제들은 중추신경계(CNS)에 작용하는 물질이라는 특성상 장기 사용시 심각한 부작용을 유발하는 결정적인 문제가 있어 제품화 이후 수년 만에 대부분 퇴출되었다. 초기 시장을 이끌던 비만치료제 fenfluramine은 심장질환과 혈압상승 부작용으로 1997년 판매중단되었으며, sibutramine (Abbott사의 Meridia®, Reductil®) 역시 심장마비 증가 등의 부작용으로 인해 2009년 판매중단되었다. 유럽에서 제품화되었던 rimonabant (Sanofi-Aventis사의 Acomplia®)는 우울증, 자살 등 심각한 정신질환 부작용 문제로 인해 2008년 판매중단되었다. 최근 미국 FDA에서 식욕억제제 계열의 비만치료제 phentermine/topiramate (Qsymia®)과 lorcaserin(Belviq®)을 또다시 승인하였으나, 이 약물들도 그 작용기전상 부작용을 피할 수 없어 이들이 시장에서 얼마동안 판매될 수 있을지는 미지수이다 (Diabetes Metab J 2012;36:13-25).
지방분해 저해제 계열의 비만치료제 중 임상시험을 거쳐 성공적으로 시장에 진출한 약물은 orlistat(Roche사의 Xenical® 및 Alli®)이 유일하다. orlistat은 소장에서 분비되는 지방분해 효소인 pancreatic lipase의 저해제로서 지방의 분해를 억제하는 역할을 한다. 따라서 orlistat을 복용하면 지방분해가 억제되어 음식물로 섭취된 지방이 체내로 흡수되지 않고 배출되게 되어 인체로 흡수되는 지방의 양이 줄어들어 체중이 감소한다. 하지만 orlistat을 복용하면 소화되지 않은 지방이 위장관 (gastrointestinal tract, gut)을 따라 이동하면서 설사, 지방변 등을 일으키는데 이는 불편할 뿐만 아니라 정상적인 사회생활이 어려운 수준의 부작용들이다. 또한 orlistat 장기 복용시 일부에서 심각한 간 손상이 발생된다고 보고되고 있어 미국 FDA에서는 orlistat의 안전성에 대한 재검토 논의를 진행 중이다.
결론적으로, 현재 시판중인 모든 비만치료제는 모두 심각한 부작용이 있어, 부작용이 없으면서 비만을 효과적으로 치료할 수 있는 비만치료제에 관한 사회적 수요가 매우 높은 실정이다. 따라서 세계 각국의 연구팀에서는 부작용 없이 비만을 치료할 수 있는 비만 치료제 개발을 위해 다양한 연구들을 진행하고 있다. 신개념의 비만치료제를 개발하려는 다양한 연구들 중 최근 각광받고 있는 연구는 장내 미생물을 이용한 비만치료제 개발 연구들이다.
비만은 식품 섭취량과 에너지 소모량의 불균형에 의해 나타나므로 음식물의 과다섭취와 운동부족이 원인으로 생각되어왔다. 그러나 인간의 장내 미생물들의 상호 대사활동이 비만의 주요 원인이 된다는 사실이 최근 새롭게 규명되었다 (Nature 444:1027-31, Nature 474:327-36). 사람의 장에는 인체 세포 수의 약 10배인 1014개의 엄청난 수의 미생물들이 머무르면서 인체의 세포와 상호작용을 한다. 이 과정에서 음식물이 체내로 흡수되는 양을 조절함으로써 숙주의 체중과 비만에 영향을 미치게 된다. 즉 장내 미생물의 경우 체내로 흡수되지 않고 장 내 공간에 머무르면서 인체와 공생하는 상태로 존재하기 때문에 이론적으로 안전성이 탁월하다는 엄청난 장점을 가지고 있다. 따라서 장내 미생물을 새로운 작용기전의 항비만 치료제로 개발하려는 연구가 매우 활발히 진행되고 있다 (Current Pharmaceutical Design, 2009, 15:1546-1558).

sp. BC-Y009를 당뇨병 및 비만증 예방치료용 제제로 공지하였고, 한국등록특허 제0794701호는 다당류 물질 생성 균주 Lactobacillus
reuteri

또한, 본 출원인은 한국등록특허 제1292714호에서 자유지방산 흡수능이 있는 Lactobacillus acidophilus FARM1(KCTC 11513BP), FARM2(KCTC 11514BP) 및 FARM3(KCTC 11515BP)를 항비만 균주로 공지한 바 있다.
그러나, 상기 항비만 미생물들은 동물실험에서 항비만 또는 관련효능이 일부 입증되었을 뿐, 임상시험에서는 비만치료 및 예방 효과가 성공적으로 입증되지 못하여, 상업화되지 못하였다.
현재 시판중인 모든 종류의 비만치료제들은 화학물질에 의존한 비만치료제이지만, 이들 비만치료제들은 모두 심각한 부작용이 있다. 이러한 문제점을 해결하기 위하여 최근 세계 각국의 연구팀에서는 장내 미생물을 이용한 비만치료제들을 개발중에 있다. 그러나, 아직까지는 장내 미생물들을 이용한 비만치료제들이 부작용 없이 비만을 치료할 수 있다는 가능성만 입증했을 뿐, 효과적으로 비만을 치료할 수 있는 장내 미생물들을 이용한 비만치료제 개발은 이루지 못한 실정이다.
기존 장내 미생물을 이용한 비만치료제 연구들은 장내 미생물에 의해 인체에서 포만감을 조절하거나, 항비만 효능이 있는 특정 대사산물을 과다생산 시키거나, 일반 영양성분을 항비만 물질로 전환시키거나 (bioconversion), 당분해과정을 억제시키거나, 저당류를 다량류로 전환시키거나, 다당류 생성을 촉진시키는 방법 등과 같은 방법으로 비만을 치료하고자 하였다. 하지만 이러한 접근 방법으로는 비만을 효과적으로 치료할 수 없다는 사실이 기존에 수행된 수많은 동물 및 임상시험에서 입증되었다.
전 세계 각국 보건의료 담당 정부기관에서 비만치료 효능이 가장 우수하다고 인정한 장기 복용 비만치료제는 orlistat이 유일하다. orlistat은 장 내용액으로 분비되는 췌장 리파제(pancreatic lipase)의 활성을 저해하는 억제제 계열의 화학물질이다. 따라서 orlistat을 복용하면, orlistat이 장내 췌장 리파제(pancreatic lipase)의 활성을 떨어트려 지방 분해가 억제되므로, 장내에서 지방산이 덜 생성되어, 장 내용액 (gut fluid content)에 녹아 있는 지방산의 농도를 감소시킴으로써, 비만을 치료할 수 있다. 이러한 과학적 사실에 근거하여 본 발명자들은 장내 미생물을 이용하여 장 내용액 (gut fluid content)의 액상에 녹아 있는 지방산 농도를 낮추어주면 orlistat처럼 매우 효과적으로 비만을 치료할 수 있다는 사실을 인지하였다. 그러나, Orlistat은 화학물질이라는 본질적 한계 때문에 부작용 문제에서 자유로울 수 없지만, 장내 미생물은 인체의 장에 공생하는 물질이기 때문에 부작용 문제에서 자유롭다는 결정적인 장점이 있다. 더욱이 orlistat은 지방분해를 억제하기 때문에 소화불량이라는 문제를 유발시키지만, 장내 미생물을 이용하여 장 내용액에 녹아 있는 지방산만 선택적으로 제거하면 소화불량이라는 부작용에서도 자유롭다는 이점이 있다.
따라서, 본 발명의 목적은 포유동물의 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시키는 기능이 있는 장내 균주를 제공하는데 있다.
본 발명의 다른 목적은 상기 균주 또는 이의 배양물을 함유한 비만 또는 비만으로 야기된 대사성 질환의 예방 또는 치료를 위한 약학 조성물, 예방 또는 개선을 위한 기능성 식품을 제공하는데 있다.
상기 목적을 달성하기 위하여 본 발명은 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시킴으로써 비만 억제능을 갖는 장내 균주를 제공한다.

본 발명에 따른 포유동물의 장관계(gastrointestinal tract)에서" 장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시킴으로써 비만 억제능을 갖는 장내 균주는 동물실험과 임상시험에서 모두 부작용이 전혀 없으면서도 널리 이용되고 있는 대표적인 비만치료제인 orlistat과 비슷한 정도의 비만치료 효능을 보였다.

현재까지 개발된 비만 치료제는 복용상의 부작용 또는 장기 복용시의 안전성 문제를 가지고 있다. 특히 오용과 남용이 심각한 비만 치료제의 특성상 안전한 비만 치료제의 개발이 매우 시급하지만, 기존에 식욕저하 물질이나 지방분해 저해물질을 이용한 시도는 모두 실패해왔다.
본 발명자는 기존 비만치료제들은 항비만 성분이 체내에 흡수되어 식욕저하 및 지방분해 저해에 따른 각종 부작용을 일으킨다는 사실을 인지하고, 체내에 공생하면서 숙주의 지방 흡수를 줄여줄 수 있는 장내 미생물을 이용하면 칼로리 섭취량을 줄여 비만을 치료할 수 있다는 사실을 확인하고자 하였다.
이에 본 발명에서는 1) 섭취시 위장관계에 서식하는 장내 미생물을 스크리닝하여 신규 항비만 균주들을 발굴하였으며, 2) 항비만 균주들은 섭취되어 숙주의 장에 군락을 형성하고, 3) 숙주의 장 내용액 (gut fluid content)의 지방산 농도를 감소시키고, 4) 숙주의 식이지방 흡수를 감소시키면서 반대로 식이지방 배출은 증가시킴으로서, 5) 숙주의 체중을 감소시킨다는 것을 확인할 수 있었다.
따라서, 본 발명은 일 관점에서, 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시켜 비만 억제능을 갖는 장내 균주에 관한 것이다.

상기 균주는 사람의 장내 세균총을 구성하는 미생물 또는 사람의 장관으로부터 유래된 것으로서, 포유류의 위장관 내에서 서식할 수 있으며, 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시켜, 숙주로 흡수되는 지방산의 양을 줄이는 것을 특징으로 한다.
상기 장내 균주는 다음과 같은 4가지 특성이 있어 기존의 비만 치료제가 가지고 있는 부작용 문제를 근본적으로 해결할 수 있는 장점이 있다.
첫째, 장내 미생물은 체내로 흡수되지 않고 장에서 군락을 형성하면서 작용하기 때문에 체내로 흡수되어 작용하는 화학물질 기반의 비만치료제들의 부작용 문제가 전무하다. 둘째, 기존 식욕저하제 계열의 비만치료제들과는 달리 중추신경계에 작용하지 않기 때문에 장기간 복용이나 남용에 따른 부작용이 없다. 셋째, 기존 지방분해저해제 계열의 비만치료제와는 달리 지방 분해에 관여하지 않기 때문에 소장관 내에서 설사, 지방변, 소화불량과 같은 부작용을 일으키지 않는다. 넷째, 포유동물에 유익한 미생물이자 GRAS 카테고리에 속하는 안전한 장내 미생물이다.
본 발명은 다른 관점에서, 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시켜 비만 억제능을 갖는 장내 균주 또는 상기 장내 균주의 배양물을 유효성분으로 함유하는 비만 또는 비만으로 야기된 대사성 질환의 예방 또는 치료를 위한 약학 조성물에 관한 것이다.
본 발명은 또 다른 관점에서, 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시켜 비만 억제능을 갖는 장내 균주 또는 상기 장내 균주의 배양물을 유효성분으로 함유하는 비만 또는 비만으로 야기된 대사성 질환의 예방 또는 개선을 위한 기능성 식품에 관한 것이다.
상기 약학 조성물 또는 기능성 식품에 함유된 균주 또는 상기 균주의 배양물은 "장 내용액 (gut fluid content)에 녹아 있는 지방산 농도"를 5% 이상 낮출 수 있으며, 상기 장은 위, 소장, 대장 등을 예시 할 수 있다.
또한, 상기 약학 조성물 또는 기능성 식품은 특별히 제한되지 않으나, 103~1012 cfu/gram의 균주를 포함하는 것이 바람직하다.
본 발명에 있어서, 상기 약학 조성물은 투여를 위해서 유효성분인 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시켜 비만 억제능을 갖는 장내 균주 또는 상기 장내 균주의 배양물 이외에 추가로 약제학적으로 허용 가능한 담체를 1종 이상 포함하여 약학적 조성물로 바람직하게 제제화 할 수 있으나, 이에 제한되는 것은 아니다.
본 발명에 있어서,“기능성 식품”은 식품 본래 목적 외에 특정한 기능성이 부가된 식품으로, 건강식품, 건강기능성식품, 식이보조제 등을 모두 포함하는 의미이다. 본 발명의 기능성 식품이 목적하는 기능성은 비만 예방 및 개선이다.
상기 기능성 식품은 유제품 (우유, 두유, 가공우유), 발효유 (액상 요구르트, 호상 요구르트), 드링크제, 음료 등을 예시할 수 있으나, 이에 한정되지 않으며, 상기 기능성 식품에는 상기 유효성분 외에도 필요에 따라 다양한 보조성분을 추가로 함유할 수 있다. 본 발명의 기능성 식품의 경우, 식물성 크리머, 탈지분유, 당, 비타민 A, 비타민 B1, 비타민 B2, 비타민 B3, 비타민 B6, 비타민 B12, 엽산 (folic acid), 비타민 C, 비타민 D3, 비타민 E 등의 비타민류 및 구리, 칼슘, 철, 마그네슘, 칼륨, 아연 등의 미네랄을 추가로 포함할 수 있다. 또한 본 발명의 기능성 식품 중, 음료는 통상의 음료와 같이 여러 가지 향미제 또는 천연 탄수화물 등을 추가 성분으로서 포함할 수 있다. 향미제로는 타우마틴, 스테비아 추출물과 같은 천연 감미제나, 사카린, 아스파르탐과 같은 합성 감미제 등을 들 수 있다. 천연 탄수화물로는 포도당, 과당 등의 단당류, 말토스, 수크로오스 등의 이당류, 덱스트린, 사이클로덱스트린 등의 다당류, 자일리톨, 소르비톨, 에리트리톨 등의 당알코올류 등을 들 수 있다.
본 발명에 따른 약학 조성물 또는 기능성 식품은 지방산 감소능이 있는 장내 균주를 함유하고 있으므로, 이를 섭취시 숙주에서의 에너지 흡수량을 줄여줄 수 있으므로, 효과적인 비만 및 비만으로 야기된 대사성 질환을 예방하거나 치료할 수 있다.
비만치료에 관한 본 발명의 기술사상은 포유류의 위장관 내에서 서식할 수 있고, "장 내용액 (gut fluid content)에 녹아 있는 지방산 농도"를 감소시켜, 숙주로의 에너지 흡수량을 줄여주는 균주라면 모두 적용될 수 있는 것으로, 이는 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자에게 자명하다.
이하 구체적인 실시예를 통해 본 발명을 보다 상세히 설명한다. 그러나 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 범위가 이들 실시예에 의해 한정되는 것은 아니다.
실시예 1. 항비만 장내미생물 스크리닝
항비만 장내 미생물을 얻기 위한 스크리닝 실험은 다음과 같이 수행되었다. 마른 체격의 자원자로부터 얻어진 대변 시료를 Man-Rogosa-Sharpe (MRS, Difco.) broth를 이용하여 희석한 후 MRS agar에 접종한 후 anaerobic jar (Gas-Pack anaerobic systems, BBL)에서 37℃ 혐기배양하였다. 48시간 배양한 후 MRS agar에서 자라난 colony들을 MRS broth가 들어있는 96-well plate로 옮긴 후 37℃에서 혐기 배양하였다. 이렇게 해서 얻어진 colony를 EnzyChrom™ FFA Assay Kit (Bio-Assay Systems)를 사용하여 high-throughput screening하였다. 이를 위해 0.5% (w/v) Brij™ 58 및 0.25 mM sodium palmitate이 들어있는 MRS broth를 준비하여 250 ㎕을 96-well plate의 각 well에 넣은 후 균주 배양액 10 ㎕씩 접종하였다. anaerobic jar에서 37℃로 15시간 혐기배양한 후 4,000 rpm으로 10분간 4℃에서 원심분리하여 상층액을 얻어냈다. 얻어진 배양액 안의 지방산 양은 EnzyChrom™ FFA Assay Kit (Bio-Assay Systems)를 이용하여 570nm의 흡광도를 측정하고, 계산하였다.
스크리닝을 통해 얻어진 상위 후보 균주들의 항비만 효능을 평가하기 위해 동물실험을 수행하였다. 동물실험을 위한 균체 분말을 준비하기 위해 후보 균주들의 MRS 배양액을 원심분리하여 균체를 회수한 후 여기에 탈지분유와 설탕을 10%씩 첨가하여 동결건조시켜 준비하였다.
모든 동물실험은 동물관리 및 이용위원회(Institutional Animal Care and Use Committee)의 규정에 따라 수행하였다. 6주령의 C57BL/6 mice 암컷(Joongang Experimental Animal Co., Seoul, Korea) 열 마리씩 우리(cage)에 넣고 처음 1주일 동안 사료 (10% fat; D12450B; Research Diets Inc., New Brunswick, NJ, USA)와 물을 자유 급이하였다. 실험기간 동안 12시간 간격으로 밤낮 조명을 바꾸었으며 온도는 22℃, 습도는 55 ± 5%로 일정하게 유지하였다. 일주일 후 쥐들을 무작위로 분리하여 대조군 또는 실험군으로 나누었다. 대조군에는 고지방 사료 (HFD containing 45% kcal as fat, D12451, Research Diets Inc.)를 급이 하였고, 실험군은 고지방사료에 장내 미생물 균체 분말을 각각 1×107 CFU/day을 추가하여 급이하였다. 이후 쥐들은 각자 해당하는 식이를 4주간 자유롭게 섭취하였으며, 주별로 체중을 측정하였다.
실험 종료 후 각 군의 동물들은 마취하여 sacrifice 시키고, 장을 분리하여 무게를 잰 후 멸균 식염수로 장내 내용물을 회수하였다. 장내 내용물은 10,000g로 30분간 원심분리한 후 상층액을 얻어내어 지방산 양을 EnzyChrom™ FFA Assay Kit (Bio-Assay Systems)로 측정한 후 장내 지방산 농도를 계산하였다.
도 1의 (A)는 발굴된 균주들을 섭취시킨 쥐에서의 장 내용액 (gut fluid content)에 녹아있는 지방산 농도(mM)를 보여주고 있다. 대조군인 G1에 비해 실험군의 경우 뚜렷이 감소되는 경향을 보였으며, 특히 G5-1의 경우 대조군의 66.8%로 감소하였다.
도 1의 (B)는 발굴된 균주들의 섭취시 숙주의 체중 변화량을 측정한 결과이다. 대조군인 G1에 비해 실험군의 경우 뚜렷이 체중증가가 억제되는 경향을 보였으며 특히 G5-1가 가장 뚜렷하였다. 즉, 장 내용액 (gut fluid content)에 녹아있는 지방산 농도(mM) 감소능과 숙주에서의 체중 감소능이 비례하는 것으로 나타났다.
상기 기재한 스크리닝 방법을 통해 20,000개 이상의 균주들을 high-throughput screening하여, 지방산 농도를 감소시키는 효능이 우수한 장내 균주들을 추가적으로 발굴한 다음, 발굴된 균주들을 12주간 섭취시킨 쥐에서의 장 내용액 (gut fluid content)에 녹아있는 지방산 농도(mM) 및 실험동물의 체중증가량을 측정하고, 그 결과를 도 2에 나타내었다.
실험예 1. 비만 억제능을 갖는 장내 균주 발굴
실시예 1의 스크리닝 방법을 통해 지방산 농도를 감소시키는 효능이 우수한 장내 미생물인 G5-1(JBD301), JBD302~JBD309, JBD311~JBD315 균주들을 형태학적으로 관찰하고, 16S rRNA 유전자 분석을 실시한 결과를 바탕으로 계통분류 하였다.
형태학적 관찰은 그람 염색 후 광학현미경 및 투과전자현미경(TEM, H7650)을 이용하여 실시하였고, 그중 G5-1(JBD301)에 대한 결과를 대표적으로 도 3에 나타내었다. 도 3에 나타난 바와 같이, G5-1 균주는 형태학적으로 그람 양성 간균임을 확인할 수 있었다.
16S rRNA 유전자 분석은 MRS 배지에서 혐기 배양한 G5-1 균주 배양액에서 DNA를 추출한 후, 16s rDNA 증폭을 위하여 PCR에서 95℃ 5분 동안 denaturation시킨 다음, 95℃ 30초, 52℃ 45초, 72℃ 2분 조건으로 30회 증폭시킨 후 72℃에서 10분 처리한 후, 시퀀싱하여 확인하였다. PCR 프라이머로는 universal primers(서열번호 1 및 2)를 사용하였다.

시퀀싱으로 확인된 G5-1(JBD301), JBD302~JBD309, JBD311~JBD315 균주들의 16s rDNA sequence(서열번호 3~16)로 NCBI GenBank, DNA data bank of Japan, European Nucleotide Archive를 검색한 결과, 상기 균주들은 모두 신규한 종임을 확인하고 그중 G5-1(JBD301)를 NCBI GenBank에 Accession number KJ957189로 등록하였다.
도 4는 본 발명의 실시예 1에 따라 스크리닝하여 발굴된 비만 억제능을 갖는 락토바실러스 루테리(Lactobacillus
reuteri) JBD301 균주의 계통수(phylogenic tree)로서, 락토바실러스 루테리(Lactobacillus
reuteri) JBD301 균주는 신규한 종으로 확인되어, 이를 2014년 7월 3일자로 한국생명공학연구원 생물자원센터에 수탁번호 KCTC 12606BP로 기탁하였다.
이와 마찬가지로 JBD302~JBD309, JBD311~JBD315 균주에 대해서도 각각 락토바실러스 플란타럼(Lactobacillus

실험예 2. 비만 억제능을 갖는 장내 균주의 유산능 평가
발굴된 균주들중 대표적으로 JBD301 균주를 무균상태의 glucose (2%)를 함유한 탈지우유 (10%) 100㎖에 접종하고 37℃에서 발효시켰다. 요구르트로 발효되는 과정에서 유산능을 측정하기 위해 24시간, 48시간, 72시간이 될 때 pH 변화를 측정하였다. 10개 시료의 측정값은 평균값 ± 표준편차로 표시하였고, 표 1에 그 결과를 나타내었다.

감소능
in vitro 평가
JBD301 균주를 MRS agar에 접종한 후 anaerobic jar (Gas-Pack anaerobic systems, BBL)에서 37℃ 혐기배양하였다. 48시간 배양한 후 MRS agar에서 자라난 colony들을 MRS broth가 들어있는 96-well plate로 옮긴 후 37℃에서 혐기 배양하였다. 얻어진 배양액 안의 지방산 양은 실시예 1의 방법에 따라 EnzyChrom™ Free Fatty Acid Assay Kit (Bio-Assay Systems)를 이용하여 570nm의 흡광도를 측정하고, 계산하였다. 즉, 배양액 안의 지방산 양으로부터 JBD301의 지방산 농도 감소능을 계산하고, 도 5에 나타내었다.
도 5에 나타난 바와 같이, JBD301 실험군은 배지의 지방산 농도가 대조군의 55%로서, 통계적으로 유의한 차이로 감소하였음을 알 수 있었고(p=0.01), 나머지 JBD302~JBD309, JBD311~JBD315 균주가 배양된 배지의 지방산 농도 또한 대조군보다 감소하였음을 확인할 수 있었다.

갖는 장내 균주에 의한 장 내용액 지방산 농도
감
소능 in vivo 평가
유산균은 소장에서 균총을 형성하는데, 소장은 체내에서 지방 분해물인 지방산이 주로 흡수되는 곳이다. 대부분의 지방산이 흡수되는 소장에서 지방산의 양이 감소하면 이는 인체로 흡수 가능한 지방산을 줄여주게 되므로 숙주의 칼로리 섭취량을 줄여주게 된다. 이를 확인하기 위해 장(소장 및 대장) 내용액 (gut fluid content)에서의 지방산 농도 및 양을 측정하였다.
6주령의 C57BL/6 mice 암컷을 구입하여 (Joongang Experimental Animal Co., Seoul, Korea) 7 마리씩 우리(cage)에 넣고, 처음 1주일 동안 사료 (10% fat; D12450B; Research Diets Inc., New Brunswick, NJ, USA)와 물을 자유 급이하였다. 실험기간 동안 12시간 간격으로 밤낮 조명을 바꾸었으며, 온도는 22℃, 습도는 55%로 일정하게 유지하였다. 일주일 이상이 지난 후 실험군으로 나누어 각 실험을 시작하였다.
대조군에는 고지방 사료 (HFD containing 45% kcal as fat, D12451, Research Diets Inc.)를 급이 하였고, JBD301 첨가군은 고지방사료에 JBD301 분말을 매일 1×107 CFU 씩 추가하여 급이 하였다. 이후 쥐들은 각자 해당하는 식이를 3주간 자유롭게 섭취하였으며 실험 종료 후 마취하여 sacrifice 시켰다. 각 군의 쥐들로부터 소장과 대장을 분리하여 무게를 잰 후 멸균 식염수로 장내 내용물을 회수하였다. 장내 내용물은 10,000g로 30분간 원심분리한 후 상층액을 얻어내어 지방산의 농도 및 양을 EnzyChrom™ Free Fatty Acid Assay Kit (Bio-Assay Systems)로 측정하고, 그 결과를 도 7 및 도 8에 각각 나타내었다.
참고로, 도 6는 쥐의 장 내용액 (gut fluid content)의 지방산 양을 측정하기 위해 사용된 소장 (a, b)과 대장(c, d)의 사진이다.
도 7은 고지방 사료만 섭취한 대조군과 JBD301을 섭취시킨 실험군의 장 내용액 (gut fluid content)의 지방산 농도를 측정한 결과로서, JBD301 실험군의 경우 소장의 장 내용액 (gut fluid content) 지방산 농도는 대조군의 69%였고, 통계적으로 유의한 차이로 감소하였으나 (p=0.022), 대장의 경우는 대조군에 비해 통계적으로 유의한 차이를 보이지 않았다 (p=0.158).
도 8은 고지방 사료만 섭취한 대조군과 JBD301을 섭취시킨 실험군의 장 내용액 (gut fluid content)의 지방산의 양을 측정한 결과로서, JBD301 실험군의 경우 소장의 장 내용액 (gut fluid content) 지방산 함량은 대조군의 59%였고, 통계적으로 유의한 차이로 감소하였으나 (p=0.01), 대장의 경우는 대조군에 비해 통계적으로 유의한 차이를 보이지 않았다 (p=0.153).

갖는 장내 균주에 의한
식이지방
흡수감소 및 배출증가 in vivo 평가
JBD301을 섭취한 동물에서 식이지방 흡수가 줄어드는지를 평가하기 위해서 실험예 4와 같이 고지방식이 또는 JBD301 첨가 고지방식이를 쥐에게 3주간 섭취시킨 후 14C 방사능표지 된 지방 triolein을 경구투여하였다. 시간별(2시간~96시간)로 채혈된 혈액을 리퀴드 신틸레이션 칵테일(liquid scintillation cocktail)과 섞은 후, 14C 방사능을 Microplate Scintillation Counter (PerkinElmer)로 측정하고, 그 결과를 도 9에 나타내었다.
도 9에 나타난 바와 같이, JBD301 실험군의 경우 고지방식이 대조군에 비해 혈액의 방사능이 모든 시간대에서 감소하였는데 특히 경구투여 후, 4시간째에서는 대조군의 62% 수준으로 감소하여 통계적으로 유의한 차이로 감소한 것을 확인할 수 있었다(p=0.048).
또한 JBD301을 섭취한 동물에서 식이지방의 배출량이 줄어드는지를 평가하기 위해서 시간별로 얻어진 대변을 Solvable™ (PerkinElmer)에 첨가하고 60℃에서 1시간동안 섞은 후 bead homogenizer로 잘 갈아서 13,000 rpm으로 5분 원심분리하였다. 다음으로 얻어진 상층액에 H2O2를 1/10 (v/v) 첨가하고 50℃에서 다시 10분간 섞어준 후 13,000rpm으로 5분간 원심분리하였다. 다음으로 얻어진 상층액에 리퀴드 신틸레이션 칵테일(liquid scintillation cocktail)을 섞은 후, Microplate Scintillation Counter (PerkinElmer)로 14C 방사능을 측정하였다.
도 10은 JBD301의 섭취시 식이지방의 체외 배출이 늘어나는지 여부를 확인하기 위해서 시간별로 대변의 방사능 양을 측정한 결과이다. JBD301 실험군의 경우 고지방식이 대조군에 비해 3일간 대변의 방사능이 증가하였는데 특히 day 1의 경우에서는 대조군의 177% 수준으로 증가하여 통계적으로 유의한 차이로 증가한 것을 확인할 수 있었다(p=0.037).

평가
모든 동물실험은 동물관리 및 이용위원회(Institutional Animal Care and Use Committee)의 규정에 따라 수행되었다. 6주령의 C57BL/6 mice 암컷을 구입하여 (Joongang Experimental Animal Co., Seoul, Korea) 열 마리씩 우리에 넣고 처음 1주일 동안 사료 (10% fat; D12450B; Research Diets Inc., New Brunswick, NJ, USA)와 물을 자유 급이하였다. 실험기간 동안 12시간 간격으로 밤낮 조명을 바꾸었으며 온도는 22℃, 습도는 55 ± 5%로 일정하게 유지하였다. 일주일 이상이 지난 후 실험군으로 나누어 각 실험을 시작하였다.
고지방 사료 (HFD containing 45% kcal as fat, D12451, Research Diets Inc.)를 12주 급이하여 대사증후군을 유발한 후 쥐들을 무작위로 분리하여 무첨가 대조군, JBD301 첨가군 (1×107 CFU per day), orlistat 첨가군 (50 mg/kg diet, Xenical®, Roche)으로 나누었다. 이후 4주 동안 쥐들은 각자 해당하는 식이를 자유롭게 섭취시키면서 체중변화를 모니터링하고, 그 결과를 도 11에 나타내었다.
도 11에 나타난 바와 같이, JBD301을 비만 유도 쥐에게 1×107 CFU씩 매일 섭취시켰을 때 뚜렷한 체중 감소 효과를 확인하였으며, 이는 비만 치료용 의약품인 오리스탓을 섭취시켰을 때만큼 우수한 항비만 효과이다.

3. 비만
억제능을
갖는 장내 균주에 의한 체중 조절 효능 평가 임상 시험
비만 억제능을 갖는 JBD301의 효능을 인체적용시험을 통해 검증하기 위해 임상시험 2상을 수행하였다(Clinical Research Information Service of Korea as KCT0000452). 이는 과체중 또는 비만인을 대상으로 JBD301의 섭취가 체중 또는 체지방 감소에 효능이 있는지 여부를 판단하기 위한 것으로서, JBD301의 임상시험 디자인을 도 12에 나타내었다.
임상시험심사위원회(IRB) 승인을 거친 임상시험 프로토콜에 따라 참가 피험자를 모집하였으며, 모집된 피험자가 선정 기준, 즉 서면 동의서 제출, 25세 이상 65세 이하의 남녀, 체중지수 25~35 kg/m2을 모두 만족하는 경우 피험자로 등록하였다. 또한 피험자 제외 기준으로 최근 12주 간 항생제를 사용한 자, 만성적인 변비 또는 설사가 있는 자, 체중에 영향을 미치는 비만치료제(흡수저해제, 항우울제, 식욕억제제 등), 이뇨제, 피임약, 스테로이드제제, 여성호르몬제를 복용하고 있는 경우, 최근 12주 이내에 비만 프로그램에 참여하거나 또는 다이어트 식품을 지속적으로 섭취/복용한 경우, 조절되지 않는 당뇨나 고혈압 환자, 심장, 신장, 간, 갑상선, 뇌혈관 질환 등이 있는 경우, 우울증, 정신분열증, 알코올중독, 약물중독 등의 질환이 있거나 병력이 있는 자, 최근 5년 이내 암의 진단 및 치료를 받은 적이 있는 경우, 시험식품이나 시험식품에 함유된 성분에 대한 과민증이 있는 자, 시험기간동안 음식물과 운동으로 체중을 줄이려는 계획이 있는 자, 임산부 및 수유부를 적용하여 이에 해당시 피험자 등록에서 제외시켰다.
등록된 피험자는 평소 식단을 유지하도록 교육시킨 후 무작위로 그룹핑하여 450mg의 위약 캡슐 또는 JBD301 분말 캡슐 (109 CFU)을 한 알씩 12주간 복용시켰다. 분말 캡슐 450 mg에는 JBD301 분말 또는 식물성 크리머가 12%, 탈지분유 40%, sucrose 40%, ascorbic acid 8% 중량으로 함유되었다.
도 13은 본 발명의 실시예 3에 따라 수행된 JBD301의 임상시험에서 모집된 피험자와 이의 대조군 및 실험군 배정 그림이다. 최종 등록된 피험자는 40명이었고, 이중 임상이 완료된 피험자는 37명이었으며, 이들의 baseline characteristics는 다음 표 2와 같다.

유효성 평가변수로는 체중을 측정하였고, 체지방 역시 computerized tomography와 Dual-Emission X-ray Absorptiometry로 측정하였다. 임상 데이터는 SAS (Version 9.2) 및 MedCalc (Version 11.6.0)를 이용하여 Mann-Whitney U test와 Wilcoxon Signed Rank test에 의해 통계분석되었다.
유효성 평가변수의 percentage change를 비교하기 위하여 Mann-Whitney U test를 사용하였는데, 12주간의 percentage change는 (12주에서의 값-0주에서의 값)을 0주에서의 값으로 나눈 후 백분율로 계산하였다. 식이 조절 및 운동을 하지 않은 상태에서 위약을 복용한 대조군 (n=19)의 체중 변화는 1.77% (1.45kg)이었으나 JBD301을 복용한 시험군 (n=18)의 체중 변화는 0.31% (0.21kg)이었다. 즉 시작점과 종료점 간 체중은 1.46% (1.24kg)의 그룹간 차이 (between-group difference)를 보였다. Mann-Whitney U test 통계처리 분석방법에 의하면 위약 대조군과 JBD301 시험군 간의 percentage change는 체중 (P = 0.026)과 BMI (P = 0.036) 모두 유의적인 차이를 보였다.

도 14는 본 발명의 실시예 3에 따라 수행된 JBD301의 임상시험 결과이다. 도 14로부터, 임상시험을 통해 JBD301을 섭취한 경우, 대조군과 통계적으로 유의한 체중의 차이가 있음을 확인하였다. 체중 또는 체지방은 음식물 섭취량이나 운동량 생활습관 등 외부 환경요인에 의해 수시로 변동하는 요인들에 의해 크게 영향 받는다는 점을 감안할 때 대조군과 실험군 간의 차이는 매우 중요한 결과이다.
본 발명에 따른 포유동물의 장관계(gastrointestinal tract)에서 "장 내용액(gut fluid content)에 녹아 있는 지방산 농도"를 감소시킴으로써 비만 억제능을 갖는 장내 균주를 이용할 경우, 현재 비만으로 고통 받고 있는 환자들이 보편적으로 활용할 수 있는 비만치료제로 개발하여 인류의 보건에 크게 기여할 것으로 기대된다.
