Abstract
Description
本願は、2013年12月12日に、日本に出願された特願2013-257235号に基づき優先権を主張し、その内容をここに援用する。
バスケット型把持鉗子は、複数のワイヤによって異物を保持する構造を有しているので、大きい異物を回収しようとした場合、異物を保持したままでは管路からバスケットが抜去できなくなる事がある。たとえば、特許文献1,2,3に記載の複数のワイヤの間から異物が外れなくなってしまうと、バスケットを管路から抜去できなくなってしまう。
本発明の第1実施形態について説明する。図1は、本実施形態のバスケット型把持鉗子を示す側面図である。図2は、図1に符号Aで示された部分の拡大断面図である。
図1に示す本実施形態のバスケット型把持鉗子1は、胆管などの体内の管路に挿入され、異物を取り除くための医療器具である。
図1に示すように、バスケット型把持鉗子1は、シース2(挿入部)と、シース2に挿通された本体部3と、シース2に固定された操作部20とを備える。
バスケット部4は、複数のバスケットワイヤ5と、複数のバスケットワイヤ5を略紡錘状とするための固定部材6とを備える。
操作部20は、操作本体21と、スライダ29とを備える。
バスケット型把持鉗子1は、一般的なバスケット型医療用処置具と同様に、バスケット部4をシース2に収納した状態で内視鏡チャンネルを通じて患者体内の胆管等の管路に挿入される。スライダ29の前進操作により管路内でバスケット部4をシース2から突出させると、バスケットワイヤ5の弾性力によりバスケット部4が拡開する。拡開したバスケット部4内に結石等の異物35(図3参照)を取り込んだ後、スライダ29を引くことでバスケット部4を縮径させ、異物35をバスケット部4内にしっかりと保持することができる。その後、内視鏡とともにバスケット型把持鉗子1を患者体内から抜去し、異物35を回収する。
バスケット部4の内部においてバスケット部4の遠位部分に異物35が保持されて胆管等の管路から引き抜けない場合に、上記の使用方法においてバスケット部4を前方へと移動させることに代えて、図5に示すように、バスケット部4の近位部分の一部がシース2内に入り込む程度にバスケット部4を後退させる。このとき、バスケット部4の後退によりセンターワイヤ12も後退する。続いて、第二ポート23においてセンターワイヤ12の位置を固定し、センターワイヤ12が動かない状態で操作ワイヤ10を前方へと移動させる。図6に示すように、バスケット部4の遠位端の位置はセンターワイヤ12の遠位端の位置に留まろうとし、バスケット部4の近位端の位置は操作ワイヤ10によって前方へと移動する。これにより、バスケットワイヤ5における略紡錘状の形状が崩れ、バスケット部4の近位部分においてバスケットワイヤ5の隙間が開く。バスケット部4の近位部分において開いた隙間は、異物35が通過可能な内径の略円形の開口となる。この状態でバスケット型把持鉗子1の全体を前方へ移動させることにより、図7に示すように、バスケット部4内の異物35は、バスケットワイヤ5の変形により生じた開口を通じてバスケット部4の近位側へ向かって相対的に移動し、バスケット部4の外部へと排出される。
次に、本発明の第2実施形態について説明する。なお、以下に説明する各実施形態において、上記の第1実施形態と同様の構成要素には第1実施形態と同一の符号を付し、重複する説明を省略する。図8は、本実施形態のバスケット型把持鉗子を示す部分断面図である。図9は、図8に符号Bで示された部分の拡大図である。
規制部材40は、本体ワイヤ36が進退自在となる径の貫通孔を有し、この貫通孔に本体ワイヤ36が挿通された状態で、第一係止部37と第二係止部38との間に配されている。
図10に示すように、本実施形態において、バスケット部4がシース2内に収容された状態では、規制部材40は、第一係止部37から前方側へ離れた位置にあり、規制ワイヤ39は第二ポート23の栓26によって固定されている。
次に、本発明の第3実施形態について説明する。図18は、本実施形態のバスケット型把持鉗子を示す部分断面図である。
図18に示すように、本実施形態のバスケット型把持鉗子1Bは、第1実施形態で説明した本体部3に代えて、第1実施形態の本体部3とは構成が異なる本体部3Aを備える。
また、本実施形態では、バスケット部4を構成するバスケットワイヤ5のうちの一本(図18に符号5aで示す)は、近位固定部材8には固定されておらず、第二操作ワイヤ10Aとして操作部20まで延びている。
本実施形態のバスケット型把持鉗子1Bにおいては、第二ポート23の栓26が緩められていて第二操作ワイヤ10Aが操作部20に対して進退自在な状態では、バスケット部4における各バスケットワイヤ5は、操作ワイヤ10の進退に応じてすべて移動する(第1状態)。また、第二ポート23の栓26が閉められていて第二操作ワイヤ10Aが操作部20に対して進退不能な状態では、バスケット部4における各バスケットワイヤ5は、第二操作ワイヤ10Aに固定されていないもののみが操作ワイヤ10の進退に応じて移動する(第2状態)。
また、上述の各実施形態において示した構成要素は適宜に組み合わせて構成することが可能である。
なお、本発明は、以下のように表現することもできる。

対象物を収容するために前記挿入部の先端側に位置するバスケット部、このバスケット部は複数のバスケットワイヤから構成され、先端側は前記バスケットワイヤが密に配置された密部と、基端側は前記バスケットワイヤが疎に配置された疎部から構成される、と、
前記バスケット部の先端側に接続された支持部材、この支持部材は前記バスケット部内を経由して前記挿入部内に挿通される、と、
前記バスケットワイヤを進退操作するためのバスケットワイヤ操作部、この操作部は、バスケットワイヤ全部を同時に進退させる第1状態と、バスケットワイヤの半数以上を他のバスケットワイヤに対して相対移動させる第2状態を含む、と、
前記バスケット部内に収容された対象物を前記バスケット部外に排出させる排出機構、この排出機構は、前記バスケットワイヤの半数以上に対して他のバスケットワイヤを固定させる固定機構を含み、前記半数以上のバスケットワイヤの密部若しくは疎部の、他の操作ワイヤに対する相対移動により、前記バスケット部内の対象物に対して前記半数以上のバスケットワイヤを相対移動させて前記対象物を排出させる、
ことを特徴とする内視鏡用処置具。」

Claims (5)
- 挿入部と、
対象物を収容するために前記挿入部の遠位部分に配され、複数のバスケットワイヤから構成され、前記複数のバスケットワイヤが疎に配置された近位部分および前記近位部分よりも密に配置された遠位部分を有するバスケット部と、
前記バスケット部の遠位端に接続され前記バスケット部の内部を経由して前記挿入部内に挿通される支持部材と、
前記バスケット部の近位端に接続され前記挿入部内に挿入された操作ワイヤと、
前記操作ワイヤを進退操作するために前記挿入部の近位端部に配された操作部と、
前記対象物が前記バスケット部の内部に収容されているときに前記バスケット部の内部に収容された前記対象物を前記バスケット部の外へ排出させる排出機構と、
を備え、
前記排出機構は、前記複数のバスケットワイヤに対して前記支持部材を進退させることが可能であり、前記支持部材の位置を固定可能であり、前記支持部材と前記バスケット部との相対移動により、前記バスケット部の内部の前記対象物を、前記バスケット部において前記バスケットワイヤの前記近位部分から前記バスケット部の外部へ排出する

前記排出機構は、前記バスケット部の内部の前記対象物を、前記バスケット部の前記遠位部分から前記近位部分へと移動させる

前記支持部材は、
前記バスケット部の遠位端に連結され前記バスケット部の近位端近傍まで延びる本体ワイヤと、
前記本体ワイヤの近位端に固定され前記本体ワイヤの外径よりも大きな外形寸法を有する第一係止部と、
前記第一係止部よりも遠位側に前記第一係止部から離れた位置で前記本体ワイヤに固定され前記本体ワイヤの外径よりも大きな外形寸法を有する第二係止部と、
を有し、
前記操作部は、
前記本体ワイヤが進退自在となる径の貫通孔を有し前記貫通孔に前記本体ワイヤが挿通された状態で前記第一係止部と前記第二係止部との間に配された規制部材と、
前記規制部材に遠位端部が固定され前記挿入部の内部を通じて前記操作部まで延びる規制ワイヤと、
を備え、
前記規制部材が前記第二係止部に当接した状態で前記規制ワイヤの進退位置を固定することによって前記挿入部に対する前記本体ワイヤの近位側への移動が規制され、前記規制部材が前記第一係止部に当接した状態で前記規制ワイヤの進退位置を固定することによって前記挿入部に対する前記本体ワイヤの遠位側への移動が規制される

前記バスケットワイヤの遠位部分を束ねる結束部と、
前記結束部の遠位側にワイヤにより連結され前記支持部材の遠位端に固定されたチップ部材と、
を備え、
前記支持部材は、前記結束部に対して前記バスケット部の中心線よりも外周方向へオフセットされた位置で前記バスケット部の内部を通過する
