図3は、シリンジホルダ10の全体及び針16を示す。図4は、シリンジホルダ10の前端部分(図3の点線の囲み部A)、特に針装着部17及びその周辺部分を拡大して示す。図5は、針16を装着した状態のシリンジホルダ10を示し、このときの針装着を検出するための検出ロッド19(検出部材又は第1検出部材の一例)の状態を示す。図6は、シリンジホルダ10を分解して示す。図7は、シリンジホルダ10の後端部分(図3の点線の囲み部B)、特にピストン挿入口15及びその周辺部分を拡大して示す。
図3に示すように、シリンジホルダ10は、その前端側に針装着部17、後端側にはピストン14(図12参照)の前端側が挿入されるピストン挿入口15を有する。シリンジホルダ10は内部に薬剤を収納した製剤シリンジ50が装着されている。なお、針装着部17は、製剤シリンジ50の一端に設けられる場合もある。
このように、90度間隔で設けられた4本の延在部19bのうち、対向する1対に突起20が設けられ、他の対向する1対に針の装着を検出するための突起22が設けられている。突起22は、シリンジホルダカバー10aに検出ロッド19が装着されたとき、シリンジホルダカバー10aの開口部23(ここでは、対向する位置に1対設けられている)からシリンジホルダ10外に突出するように形成されている。同様に、突起20は、シリンジホルダカバー10aの開口部21(ここでは、対向する位置に1対設けられている)からシリンジホルダ10外に突出するように構成されている(図5参照)。
図3及び図4に示すように、針装着部17に針16が装着されると、針16により検出ロッド19の先端部が後方(矢印64の方向)に押される。その結果、図5及び図7に示すように、突起20は開口部21の後端まで移動する(矢印66の方向に移動する)。この突起20の後端側(ピストン挿入口15側)は、後端側ほど高さが低くなる傾斜面20aを有する。この突起20が検出ロッド19の後端側への移動に従って、後方(図5において矢印66の方向)に移動する。傾斜面20aは、開口部21の後端に当接しながら、後方(図7において矢印66の方向)に移動する。これにより、上述のごとく突起20は開口部21内の後端まで移動し、シリンジホルダ10内にほぼ沈み込むようになる。
このため、図2に示すごとく、インナーケース11のシリンジホルダ装着口12から、シリンジホルダ10の後端側を挿入する際に、シリンジホルダ10の突起20及び検出ロッド19の突起22が、シリンジホルダ装着口12の開口縁に当接し、その挿入を阻害することは無い。
図8に示すように、インナーケース11は、本体ケース1の内部に固定して設けられたメインフレーム60に対して矢印67方向に移動可能に設けられている。インナーケース11は、外側に露出し長手方向に延びる検出レバー27(第2検出部材の一例)を備える。検出レバー27は、検出レバー27の前端側(シリンジホルダ装着口側)の外表面に肉厚部に形成した補強板27a(図9参照)と、を備える。
次に、図8、図9に基づいて、インナーケース11内にシリンジホルダ10が装着された状態における検出レバー27と検出ロッド19の突起22との係合について説明する。図8は、インナーケース11内にシリンジホルダ10が挿入された状態を示す。図9は、検出レバー27の周辺部(図8の点線の囲み部C)を拡大して示した図である。シリンジホルダ10は、インナーケース11に挿入後、時計方向(右回り:図9における矢印68の方向)に回動される。この時計方向の回動に伴い、シリンジホルダ10の検出ロッド19の突起22は、開口部からインナーケース11外に突出し、この突出部分が上述のごとく矢印68の方向に回動させられる。これにより、検出レバー27が後方(図9に示す矢印69の方向)に移動する。
なお、検出レバー27の後端部はバネ(図示せず)で前方(矢印69とは反対の方向)側へ付勢されている。この付勢に抗して、検出レバー27は後方側に移動することにより、検出センサ28がこの後退を検出する。
また、図9に示すように、検出レバー27の前端側(シリンジホルダ装着口側)の外表面には、補強板27aを設けている。
つまり、検出ロッド19の突起22は、開口部23(図7参照)からわずかしか突出していない。このため、このわずかな突出量でも検出レバー27に当接し、これを後方側に移動させるためには、検出レバー27の前端側がインナーケース11の外周方向に反らないようにすることが極めて重要となる。このように、前記補強板27aを設けることで、上記の作用が確保される。
なお、上記シリンジホルダ10とインナーケース11の装着の際には、図1に示すキャップ7は本体ケース1の前端側から取り外している。
次に、図11に示すモータ部40について説明する。図11は、本体ケース1内部に搭載されているモータ部40を示す。モータ部40は、スライドモータ32(可動部材駆動部の一例)及びギアードモータ34(ピストン駆動部の一例)等の2つの主要なモータ等から構成されている。スライドモータ32は、刺針・抜針駆動手段の一例である。ギアードモータ34は、エア抜き動作及び薬剤の注入動作等を行うときに使用するピストン駆動手段の一例である。
ここで、図12に示すプッシュシャフト41は、カップリング42に圧入されており、その先端部分はピストン14と係合している。これによりギアードモータ34の回転運動を確実にピストン14に直線運動として伝達している。
図10は、検出センサ28及びその周辺を示す要部拡大図である。検出センサ28は、本実施形態においては、光が遮蔽されたか否かで判別するフォトセンサを備えている。
図10において、検出センサ28は、上記のように、製剤シリンジ50と針16を装着したシリンジホルダ10をインナーケース11に装着したとき、インナーケース11に設けられた検出レバー27を利用して装着状態を検出する。詳しくは、シリンジホルダ10がインナーケース11に装着されると、検出レバー27が本体ケース1の後端側に移動し、この検出レバー27に設けられた遮蔽板27cが検出センサ28の光学素子を遮ることにより検出している。検出センサ28の光学素子は、発光素子28bと受光素子28c(図13)から構成されており、この発光素子28bと受光素子28cの間に前記遮蔽板27cが入ることで光学的な検知を行っている。
本実施形態に係る薬剤注入装置100においては、まず、図1の状態において、電源ボタン5を押し、次に開口部9から針キャップ18を取り外す。この状態で開口部9側を上方に向け、その後、エア抜きボタン3を押す。これにより、エア抜き動作を開始する。
その後、ギアードモータ34により、ピストン14を前方側に移動させれば、このピストン14の前端がシリンジホルダ10内に設けられている製剤シリンジ50の後端部の押し板(図示せず)を押す。これにより製剤シリンジ50内の薬剤の一部が針16の先端から流出し、製剤シリンジ50や針16内部のエア抜きが行われる。
上記のエア抜き動作が完了すると、次に、薬剤注入動作に移る。薬剤注入装置100のキャップ7を生体(人体の皮膚等)の注入位置に当接させ、この状態で、薬剤注入ボタン6を押せば、スライドモータ32によりスライドスクリュー33を介してインナーケース11全体が本体ケース1の前端側に移動する。これにより針16の先端がキャップ7の開口部9よりも前方へ突出し、生体に穿刺される。
図15は、本実施形態に係る薬剤注入装置100の制御回路及びその周辺構成を示したブロック図である。図15において、制御部103は、充電池101に充電を行う充電回路102と接続されている。制御部103は更に、電源ボタン5、エア抜きボタン3、完了ボタン4、薬剤注入ボタン6、表示部2(報知部の一例)、音を出力するサウンダ106(報知部の一例)、振動機能を有するバイブレータ107(報知部の一例)、及びID管理用のバーコードリーダ104等の各種入出力部に接続され、更にモータドライブ回路111にも接続されている。モータドライブ回路111は、モータ電流を監視する電流検知部112、ピストン14の位置を検出する位置検出部113、及びモータの回転を監視するエンコーダ114からの信号を入力する。モータドライブ回路111は、ピストン14駆動用のギアードモータ34、及び刺針動作/抜針動作を行うスライドモータ32の駆動制御を行う。
また、薬剤注入装置100は、本体ケース1の傾きを検出する加速度センサ109も設けている。この加速度センサ109は、3次元加速度センサであり、傾き情報を3次元的に検出する。
図16~図18は、本実施形態における動作を示すフロー図である。製剤シリンジ50を内蔵し針16を装着したシリンジホルダ10が本体ケース1のインナーケース11に装着された状態で、ユーザが薬剤注入装置100の電源ボタン5を押すことにより、起動する。
ステップS101:電源ボタン5が押されると、シリンジホルダ10及び/又は針16の装着状態(以下及び図中においては、製剤/針の装着状態と呼ぶ場合もある)を検出センサ28により検出する。製剤/針の装着がOK(検出センサ28がON状態)の場合は、ステップS103へ移行するが、もし装着が不良(検出センサ28がOFF状態)であれば、ステップS102へ移行する。
ステップS103:キャップ7の装着状態をキャップ検出センサ108により検出する。キャップ検出センサ108がON状態(キャップが装着されている)であれば、ステップS105へ移行し、もしOFF状態(キャップ未装着)であれば、ステップS104へ移行する。
ステップS105:上記の準備ができた状態で、「エア抜き動作」に移行する。本体ケース1の先端側(キャップ7が装着される側)を上に向けるように促す表示を表示部2に表示する。
ステップS108:「エア抜きボタン」の押下を検出すると、ステップS109へ移行する。
ステップS110:シリンジホルダ10及び/又は針16の装着を促す表示を表示部2に表示する。
ステップS112:図16に示す処理により「エア抜き動作」の準備ができた後、図17に示すように、スライドモータ32をモータドライブ回路111により駆動し、シリンジホルダ10及びインナーケース11を刺針位置へ移動させる。
ステップS114:刺針位置までの移動が完了すれば、スライドモータ32を停止させる。
ステップS116:異常を示すエラー表示を表示部2に行い、動作を中止し、エラー処理にて強制終了する。
ステップS118:ピストン14の所定量の移動の完了を検出する。このとき、制御部103に接続されているエンコーダ114などにより、キアードモータ34の移動量を検出する。
ステップS120:モータドライブ回路111によりスライドモータ32を駆動して、シリンジホルダ10及びインナーケース11を抜針位置まで移動させる。
ステップS121:抜針位置までの移動を検出する。位置検出部113の信号をモータドライブ回路111経由で制御部103(図15参照)に入力することにより、シリンジホルダ10及びインナーケース11が抜針位置まで移動したか(戻ったか)どうかを検出する。
ステップS122:スライドモータ32を停止させる。このとき、薬剤注入の準備が完了する。
ステップS123:「薬剤注入可能」を示し、薬剤注入装置のキャップ7装着側を皮膚に当てることと「注入ボタン6」の押下を促す旨の表示を表示部2に行う。
ステップS124:制御部103により「注入ボタン6」の押下を検出する。
ステップS125:薬剤注入ボタン6が押下されれば、再度、シリンジホルダ10及び/又は針16の装着を検出する。装着がOKの場合は、ステップS128(図18)の穿刺動作へ移行する。もし装着が不良の場合は、ステップS126に移行する。
ステップS126:シリンジホルダ10及び/又は針16の装着を行うよう警告表示を行う。
ステップS128:図18に示すように、モータドライブ回路111によりスライドモータ32を駆動させて、シリンジホルダ10及びインナーケース11を刺針位置に移動させる。
ステップS130:検出後、スライドモータ32を停止する。この時、キャップ7は皮膚に当接した状態であるので、円筒形状したキャップ7の内部にある針16が、先端から突出することになり、皮膚を穿刺する。
ステップS131:シリンジホルダ10及び/又は針16の装着状態を検出する。装着がOKであれば、ステップS133に移行する。装着が不良の場合は、ステップS132に移行する。
ステップS133:ギアードモータ34を駆動してピストン14を移動させる。これにより、製剤シリンジ50内の薬剤の注入が開始される。
ステップS134:エンコーダ114により所定量の薬剤に対応した所定距離分、ピストン14が移動したかどうかを検出する。所定の距離分移動したことを検出すると、ステップS135へ移行する。
ステップS136:シリンジホルダ10及び/又は針16の装着状態を検出する。装着がOKであれば、ステップS137に移行する。装着が不良の場合は、ステップS132に戻り、エラー表示を表示部2に表示し、エラー処理にて動作を中止する。
ステップS138:位置検出部113の信号をモータドライブ回路111経由で制御部103(図15参照)に入力することにより、シリンジホルダ10及びインナーケース11が抜針位置まで移動したことを検出する。
以上のように、薬剤注入作業の各工程において、シリンジホルダ10及び/又は針16の装着状態を常時監視することにより、未装着、装着不良を早期に発見することができる。これにより、装置の異常状態を早期に発見できると共に、次の動作へ移るまでに中止することができ、その対処を迅速に行うことにより、ユーザが装置の異常状態や装着不良を早期に認識し、適切且つ安心して装置の使用をすることができる。
上記実施形態1に係る薬剤注入装置100は、本体ケース1に対し移動可能なインナーケース11と、インナーケース11に装着される針16を装着するシリンジホルダ10と、インナーケース11に装着され、針16の装着を検出するための検出ロッド19と、インナーケース11に設けられた検出センサ28とを備えることによって、シリンジホルダ10のインナーケース11への装着後は、針16、検出ロッド19、インナーケース11及び検出センサ28の位置関係は固定されるため、針/製剤の装着状態の検出の精度を高め、安全性を高めることができる。
またインナーケース11の移動による経年劣化の影響によって、シリンジホルダ10及び針16の装着状態の検出の精度が悪くなることも防ぐことができる。
次に、図19~図20は、本実施形態の変形例に係る薬剤注入装置100の処理を示す。ここでは、上記図17及び図18に示すステップS112~S139(エア抜き動作から薬剤注入動作終了まで)に対応する動作中のシリンジホルダ10及び/又は針16の装着状態を検出する検出センサ28の検出タイミングを変えている。
ステップS151:まず、シリンジホルダ10及び/又は針16をインナーケース11に装着後、キャップ7を装着して準備を行い、エア抜きボタン3が押された状態において、スライドモータ32を駆動し、シリンジホルダ10及びインナーケース11の刺針位置へ移動を開始する。
ステップS153:シリンジホルダ10及び/又は針16等の装着に問題が無い場合は、位置検出部113の信号をモータドライブ回路111経由で制御部103に入力することにより、シリンジホルダ10及びインナーケース11が刺針位置に移動したかどうかを検出する。正しく刺針位置への移動が完了していればステップS154に移行する。
ステップS155:ステップS152において装着が不良であれば異常と判断し、スライドモータ32を停止する。
ステップS156:エラー表示を表示部2に行い、動作を中止し、エラー処理にて終了する。この時、サウンダ106により警告音を鳴らして通知したり、バイブレータ107を動作させて振動による通知をしたりしてもよい。
ステップS158:ステップS157におけるギアードモータ34を駆動中においても、シリンジホルダ10及び/又は針16の装着状態を検出する。このギアードモータ34の駆動中に装着不良を検出すれば、ステップS164に移行する
ステップS159:上記の装着に問題ない場合、エンコーダ114により所定量のエア抜きに対応したピストン14の所定移動量の完了を検出する。制御部103に接続されているエンコーダ114などにより、キアードモータ34の移動量を検出することになる。
ステップS161:シリンジホルダ10及びインナーケース11が抜針位置に移動させるべくスライドモータ32を駆動させる。
ステップS162:ステップS161におけるスライドモータ32の駆動中においても、シリンジホルダ10及び/又は針16の装着状態を検出する。装着不良を検出すれば、異常と判断しステップS155に戻り、スライドモータ32を停止し(ステップS155)、エラー表示(ステップS156)後、エラー処理にて動作を中止(強制終了)する。
ステップS164:ステップS158において、装着不良が検出されれば、異常と判断し、ギアードモータ34を停止し、エラー表示(ステップS156)後、動作を中止し、エラー処理にて強制終了する。
ステップS166:次に、「注射の準備完了」、「先端を皮膚に当てること」及び「薬剤注入ボタンの押下を促す」表示を表示部2に行い、薬剤注入動作へ移る。
ステップS167:制御部103によりユーザによる薬剤注入ボタン6の押下を検出する。
ステップS168:シリンジホルダ10及び/又は針16の装着を確認し、装着OKの場合、ステップS171に移行する。
ステップS169:装着不良の場合は、シリンジホルダ10及び/又は針16の装着を行うよう警告表示を行う。
ステップS170:ユーザにより製剤/針の装着が行われるとステップS101に戻る。
ステップS172:ステップS171におけるモータ動作中においても、シリンジホルダ10及び/又は針16の装着状態を検出センサ28により検出する。
ステップS173:位置検出部113の信号をモータドライブ回路111経由で制御部103に入力することにより、シリンジホルダ10及びインナーケース11が刺針位置に到達したかどうかを検出する。
ステップS175:ステップS172において、モータ稼働中にシリンジホルダ10及び/又は針16の装着不良を検出すると異常と判断し、駆動しているスライドモータ32を停止させ、エラー表示を表示部2に表示し、動作を中止後、エラー処理にて強制終了する。
ステップS176:ギアードモータ34を駆動してピストン14を移動させる。これにより、製剤シリンジ50内の薬剤の注入が開始される。
ステップS177:ステップS176におけるモータ稼働中にシリンジホルダ10及び/又は針16の装着不良を検出すると、ステップS182に移行する。
ステップS178:エンコーダ114により所定量の薬剤に対応した所定距離分、ピストン14が移動したかどうかを検出する。所定の距離分移動したことを検出すると、ステップS179へ移行する。また、到達したことを検出しない場合(つまり、到達していない場合)は、ステップS176へ戻る。
ステップS180:スライドモータ32を駆動して、シリンジホルダ10及びインナーケース11を抜針位置まで移動させる。つまり、皮膚から針16を抜くことになる。
ステップS181:ステップS180におけるスライドモータ32の駆動中においても、シリンジホルダ10及び/又は針16の装着状態を検出する。装着不良を検出すれば、ステップS183に移行する。
ステップS183:ステップS181においてシリンジホルダ10及び/又は針16の装着不良を検出すると、駆動しているスライドモータ32を停止させ、エラー表示(ステップS175)後、エラー処理にて動作を中止(強制終了)する。
ステップS185:スライドモータ32を停止し、薬剤の注入動作を終了する。これにより、皮膚からの針16を抜く抜針動作が完了する。
<2.1 薬剤注入装置200の構成>
図21~図25を参照しながら、本発明の実施形態2に係る薬剤注入装置200について説明する。本実施形態においては、インナーケースにRF-ID(無線タグの一例)を有する検出部を設けて、装置本体側との無線通信を可能にした点で、上記実施形態1と異なる。なお、実施形態1と同様の構成要素について同じ符号を付し、その説明は省略する。
図23は、本実施形態2に係る薬剤注入装置200の制御回路220及びその周辺構成を示したブロック図である。同図に示すように、検出部270は、検出センサ228とRF-ID271とを有する。また、装置本体(固定側)には、RF-ID271の検出情報を読み取るRF-IDリーダ272(受信部の一例)が設けられている。制御回路のその他の構成は、実施形態1と同様である。
図24及び図25は、本実施形態における動作であって、エア抜き動作を示すフロー図である。なお、エア抜き動作後の処理(図17のステップS112以降の処理)については、上記実施形態1と同様である。本実施形態2においては、製剤シリンジ50を内蔵し、針16を装着したシリンジホルダ10が本体ケース1のインナーケース211に装着された状態で、ユーザが薬剤注入装置100の電源ボタン5を押すことにより、起動する。
ステップS201:電源ボタン5が押されると、まず針検出処理(詳細は図25)を行い、制御部103は、針16(及び製剤シリンジ50)が装着されたかどうかを検出する。つまり、実施形態1のステップS101(図16参照)の代わりに、本実施形態においては、図25に示す針検出処理を行う。
ステップS203:「製剤/針」の装着を促す旨の表示を本体ケース1の表示部2に表示し、シリンジホルダ10及び/又は針16の装着を待つ(ステップS201に戻る)。
ステップS205:「キャップ」の装着を促す旨の表示を表示部2に表示し、キャップの装着を待つ(ステップS204に戻る)。
ステップS207:加速度センサ109により本体ケース1の傾きを検出する。加速度センサ109により、地面と垂直方向に対して所定角度の範囲内に入れば、ステップS208へ移行する。ここで、エア抜き動作を行うには、本体ケース1の先端側が少しでも上方に向いていればよいのであるが、好ましくは、所定角度は、地面と垂直上方向に対して-45度~+45度の範囲等が想定される。より好ましくは、-30度~+30度の範囲である方が効果的である。
ステップS209:「エア抜きボタン」の押下を検出すると、ステップS210へ移行する。
ステップS211:ステップS210において、制御部103は、RF-ID271からの検出信号に基づいて、シリンジホルダ10及び/又は針16の装着が検出されたかどうかを判定する。装着がOKの場合は、図17のステップS112へ移行するが、もし装着が不良であれば、ステップS212へ移行する。
ステップS213:ユーザにより、シリンジホルダ10及び/又は針16の装着が行われ、再度装着状態を検出する(ステップS210に戻る)。
次に、図24のステップS201及びS210における針検出処理について説明する。ここで、針検出処理とは、シリンジホルダ10及び/又は針16の装着状態を検出することである。
ステップS2011:検出センサ28により検出ロッド19の突起22の状態(位置)を検出する。検出ロッド19の突起22が発光素子の光を遮断していたら、「ON状態」、遮断していない場合は、「OFF状態」とする。
ステップS2012:検出センサ28のON/OFFに基づく検出情報をRF-ID271のメモリに記録する。
本実施形態係る薬剤注入装置200においては、上記実施形態1の特徴に加え、以下の特徴を有する。
更に、RF-ID271は無線であり、RF-IDリーダ272は装置本体の任意の場所に取り付けることができるため、商品設計上、制約が少なく、余分な部材も不要になるため、コストダウンや装置の小型化に寄与できる。
