本明細書において、うつ病を診断するためのバイオマーカー(以下、「診断するためのバイオマーカー」を、「診断マーカー」とも称する)という場合、うつ病患者を高確率で特定できるバイオマーカー(罹患マーカー)の他に、健常者をうつ病患者から高確率で排除できるバイオマーカー(健常マーカー)も含むものとする。
本発明にかかる診断マーカーは、ホスホエタノールアミン、タウリン、ヒポタウリン、アスパラギン酸、メチオニン、チロシン、ホスホリルコリン、アルギニン、アスパラギン、3-アミノ酪酸、β-アラニン、フェニルアラニン、リジン、ホスホクレアチン、アラニン、尿酸、コリン、イソクエン酸、ロイシン、リンゴ酸、2-アミノアジピン酸、チラミン、バリン、グルカル酸、ヒスチジン、ADMA(非対称性ジメチルアルギニン)、イソロイシン、ヒドロキシプロリン、及びシスタチオニンである。うつ病に罹患しているかどうかは、患者から採取した血液中の診断マーカーの含有量を測定することにより診断できる。
診断マーカーの使用方法には、例えば、以下の様な態様が含まれる。
まず、ヒトまたはヒト以外の脊椎動物から採血して得られた血液を用いて、診断マーカーの含有量(本明細書では、マーカー量とも称する)を測定することにより、疾患の診断を行うことができる。ここで、血液中のバイオマーカーの含有量とは、バイオマーカーの絶対濃度が好ましいが、バイオマーカーの絶対濃度と相関して各個体の絶対濃度の比較ができる値であれば限定されず、相対濃度や、単位体積辺りの重量や、絶対濃度を知るために測定した生データ(例えば、CE-TOFMS測定によって得られるグラフのピーク面積を標準化して得られる値)などでも良い。
シスタチオニンの血漿中濃度は、CESDとの順位及びSCIDによる診断合致項目の多さと反比例しているため、うつ病患者の中で、うつ病の重症度を判定するためのバイオマーカーとして用いることができる。
まず、診断対象の動物がうつ病であるかどうか、本発明の一つであるバイオマーカーや、面接による問診やアンケートなどの従来の診断方法で診断する。その後、うつ病患者の血漿中濃度を調べ、例えば、18.36μM未満のとき、軽症うつであり、18.36μM以上のとき、重症うつであると診断できる。
一般に、疾患の治療薬は、個体によってその効果がばらつくことがある。従って、各個体における、ある治療薬の薬効を調べることは非常に有益であり、本発明の診断マーカーを用いれば、容易にその治療薬の薬効を調べることができる。例えば、うつ病の治療薬を投薬する前後で、疾患脊椎動物から血液を採血し、その採血した血液に含まれる診断マーカーの含有量を測定して、治療薬を投薬する前後の血液における診断マーカーの含有量を比較する。そして、治療薬の投与後に、その診断マーカーの含有量が健常の範囲に近づけば、治療薬は効果があると判定できる。このように、本発明の診断マーカーを用いることにより、治療薬の効果があるかどうかについて容易に判定することができる。
本発明の診断マーカーを用いることにより、うつ病のモデル動物を容易にスクリーニングすることができる。例えば、候補の動物(ヒトを除く)の血液に含まれる診断マーカーの含有量を測定して、同種の健常な動物の血液中の診断マーカーの含有量と比較することにより、その動物を診断し、うつ病のモデル動物かどうかを判断できる。
うつ病を有する実験動物を用いて、うつ病の治療に効果がある化合物を同定することができる。例えば、うつ病のモデル動物(ヒトを除く)にうつ病の治療薬の候補である化合物を投与し、投与前後で採血し、その血液に含まれる診断マーカーの含有量を測定して、その化合物を投与する前後の血液における診断マーカーの含有量を比較する。化合物投与後に、その診断マーカーの含有量が健常の範囲に近づけば、投与した化合物はうつ病の治療に効果があると判定できる。このように、本発明の診断マーカーを用いることにより、疾患の治療に効果がある化合物を容易にスクリーニングすることも可能である。
個体によって薬効が異なる薬剤の場合、うつ病を有する個体に複数の薬剤を試してみて、診断マーカーによって診断し、その個体にとって最も有効な薬剤をスクリーニングすることも可能である。
上述したように、脊椎動物個体から採取された血液に含まれる診断マーカーである化合物の含有量が測定された後、その結果が、コンピュータに送信され、上述の方法に従って、コンピュータが、その結果を利用してもよい。
例えば、医療従事者が、脊椎動物個体から血液を採取し、適切に処理をして、サンプルをバイオマーカー測定装置にセットする。コンピュータは、バイオマーカー測定装置に、サンプル中のバイオマーカーの含有量を測定させ、測定結果を取得する。そして、コンピュータは、取得した血漿中マーカー含有量に基づいて、診断対象の被験者が健常者であるか、健常者でないか、うつ病患者であるか、うつ病患者でないか、のいずれか一つ以上を判定する。また、コンピュータは、上述した方法と同様にして、うつ病の重症度や、抗うつ剤の薬効を判定することもできる。コンピュータは、そのようにして得られた判定結果を出力し、医療従事者は、診断対象の被験者についての情報を得ることが可能になる。
なお、本発明のプログラムは、コンピュータに、取得した血漿中マーカー含有量を利用する方法を実行させるものである。また、コンピュータに、バイオマーカー測定装置にサンプル中のバイオマーカーの含有量を測定させる工程を実行させてもよい。
そして、このようなプログラムを、コンピュータ読み取り可能な記録媒体に記録することもできる。記録媒体は、ハードディスク、CD、DVD、USBメモリ、フロッピーディスクなど、特に限定されない。
[1]うつ病患者の診断
うつ病患者は、国立精神・神経センター病院の外来で診察し、主に初診、及び、再診2-3回目までに、SCID(Structured Clinical Interview for Diagnosis)面接法にて、うつ病であると診断された患者から選択し、倫理委員会の承認のもと、所定のインフォームドコンセント手続きを経て、研究協力を依頼した患者である。除外規定は、I軸およびII軸の診断との併発のある大うつ病(MD)である。SCID面接法とは、精神障害の鑑別診断法の一つであって、DSM-IVの診断基準に準拠して、再現性のある診断が可能なように組み立てられた構成的面接法である。初診時には、喫煙習慣などの生活習慣、薬物の摂取状況、主訴、月経、睡眠状態などの、一般的な問診情報と、既往歴、家族歴、身体疾患の有無などの検査、B型肝炎、C型肝炎の有無、頭部CT、血液生化学検査、心電図、レントゲン、などの医学情報を得た。インフォームドコンセント取得後に、アンケート調査により、CESD(抑うつに関する得点)、STAI(不安に関する得点)についても調査した。その際、研究コーディネーター(臨床心理士)が直接面談することによって、回答が欠落している箇所を確認した。また、各患者から、本発明のマーカーの血漿中濃度を測定するために、14mLの血を採取し、2時間以内に血漿を分離し、測定まで液体窒素中で保管した。いずれかの不安障害の併発するうつ病、適応障害との鑑別が難しい軽症うつ病、境界性人格障害やなんらかの人格障害を持つうつ病、気分変調性障害の経過の中で起こった大うつ病エピソードの患者などを除外し、このようなI軸の患者およびII軸の診断との併発のない大うつ病(MD)の患者を35名選択し、解析を行った。
血液中に含まれる各診断マーカーである化合物の量を、キャピラリー電気泳動-飛行時間型質量分析計(CE-TOFMS)で測定するべく、以下の方法でCE-TOFMS測定用血漿サンプルを調製した。
CE-TOFMSの測定、および、引き続くデータ解析は、Agilent CE-TOF-MSD system(Agilent Technologies社)において、キャピラリーにFused silica capillaryを用いて行った。

うつ病患者及び、健常者から採血し調製された血漿由来のCE-TOFMS測定用サンプルについて、カチオンモード及びアニオンモードでのCE-TOFMS測定を行った。この測定により、各サンプル内に含まれる、各化合物の含有量のデータをピーク面積として得た。
本実施例では、上記の化合物について、バイオマーカーの感度の閾値を94.3%と設定して健常マーカーとして用いる場合に、閾値以上、または閾値未満の範囲に含まれる、うつ病患者数または健常者数を表7に示し、表8に特異度を計算した結果を示す。
実際には、感度が94.3%とした場合、特異度が5.7(=100-94.3)%より大きければ、集団中のうつ病患者と健常者の割合にかかわらず、この方法によって、うつ病患者の割合を増やすことができる。表8に示されたバイオマーカーについては、全て、特異度が5.7%以上であり、健常マーカーとして有用である。
上記の化合物のうち、複数のバイオマーカーを用いることにより、診断精度を上げることができる。特に、罹患マーカーと健常マーカーの組み合わせ、両方に有効なバイオマーカーと罹患マーカーの組み合わせ、両方に有効なバイオマーカーと健常マーカーの組み合わせ、両方に有効なバイオマーカー同士の組み合わせが有効であると考えられる。以下に、具体例を示す。
(1)アスパラギン酸が3.95μM未満かつ、アルギニンが89.27μM未満の患者をうつ病と定義し、それ以外を健常と定義すると、感度94%、特異度88%
(2)アスパラギン酸が3.95μM未満かつチロシンが67.19μM未満の患者をうつ病と定義し、それ以外を健常と定義すると、感度94%、特異度81%
(3)チロシンが67.19μM未満かつ、グルカル酸が0.42μM以上の患者をうつ病と定義し、それ以外を健常と定義すると、感度94%、特異度81%
(4)チロシンが67.19μM未満かつ、3-アミノ酪酸が11.50μM未満の患者をうつ病と定義し、それ以外を健常と定義すると、感度91%、特異度85%
となって、単独でバイオマーカーを使用するより、診断精度が向上する。
実施例4と同様に、うつ病患者群の各サンプル内に含まれる、シスタチオニンの含有量のデータをピーク面積として得て、CESDとの順位及びSCIDによる診断合致項目の多さに関し、Spearmanの順位相関係数で検定したところ、CESD とは、r=-0.460 p=0.039で、SCIDとは、r=-0.339、p=0.049となり、逆相関した。
そこで、シスタチオニンの含有量について、軽症うつ病患者と重症うつ病患者の閾値を求めたところ、表9に示すように、閾値0.00265を以って、軽症うつ病患者と重症うつ病患者が識別できた。なお、本実施例では、CESD30点以下を軽症うつ病、31点以上を重症うつ病とし、SCIDの診断基準9項目のうち、5-6項目該当が軽症うつ病、7項目以上該当を重症うつ病と定義した。
[1]うつ病患者の診断
本実施例では、適応障害以外の不安障害を併発するうつ病患者3名、境界性人格障害を併発しているうつ病患者2名を含めた、I軸の患者34名の患者群、及び適応障害との鑑別が難しい軽症うつ病患者7名及び健常者31名を含めた対照群に対し、解析を行った。患者群及び対照群に含まれる検査対象者は、実施例1と同様に選択した。
両方の群で、年齢、身長、体重、BMI、性別に有意差はなかった。また、結婚状況、就労状況、検査一ヶ月前の体重の変動、タバコの摂取量についても、有意差は認められなかった。
上述した検査対象者からの血漿サンプルの調製、及びそれらのサンプルにおける各マーカーのCE-TOFMSによる測定方法は実施例1と同じである。
患者群及び対照群に含まれる検査対象者の血漿における診断マーカーの濃度測定、閾値決定、及び結果の解析は実施例1と同様に行った。
表10に、各診断マーカーの解析結果を記載する。
ここでは、健常マーカーによって、健常者を効率よく排除できることを示す。
各バイオマーカーの閾値を、感度を94.1%になるように設定し、うつ病患者の少ない範囲の集団を除外することで、健常者を効率よく除外した。その閾値における特異度を計算し、その結果を表11に示す。
実際には、感度が94.1%とした場合、特異度が5.9(=100-94.1)%より大きければ、集団中のうつ病患者と健常者の割合にかかわらず、この方法によって、うつ病患者の割合を増やすことができる。表11に示されたバイオマーカーについては、全て、特異度が5.9%以上であり、健常マーカーとして有用である。
[実施例3]
[1]標準添加法を用いた血漿中マーカー濃度の算出
実施例1及び2では、市販の標品を標準溶液として検量線を作成することにより、得られた閾値を、血漿中の濃度に基づく絶対値(閾値(μM))に変換した(標準溶液法)。本実施例では、所定の系列の濃度の標準溶液を作製し、サンプルごとに、各標準溶液を添加した溶液の系列を作製し、検量線を作成することによって、実施例2の各サンプルの血漿中マーカー濃度を決定した(標準添加法)。その濃度を用いて、実施例2と同様に閾値を決定したところ、ホスホエタノールアミン及びタウリンの閾値は、それぞれ、2.41及び50.54(μM)となった。
実施例1及び2の集団には入っていないうつ病患者2名について、治療前と、約6ヶ月間SSRIなどの薬物で治療し、うつ病が寛解したと診断された後(治療後)において、ホスホエタノールアミン及びタウリンの血漿中マーカー濃度(μM)を測定した。なお、濃度は、標準添加法によって決定した。
実施例1及び2で用いたサンプルと独立な11名(6名は健常者、5名はうつ病患者と確定診断されている)の血漿中マーカー濃度を、[1]と同様に測定し、[1]で決定した閾値を用いて診断した。
実施例1~3で用いたサンプルと独立な14名(全員がパニック障害を有する。9名は非うつ病者[表では健常者と記載]、5名は非定型うつ病患者と確定診断されている。)の血漿中マーカー濃度を、[1]と同様に測定し、[1]で決定した閾値を用いて診断した。
