Abstract
Description
(1)試料パッド:その上に試料(唾液)を加える吸収パッド。
(2)複合体又は試薬パッド:これは、発色粒子(通常コロイド金粒子又はラテックス微粒子)に結合した標的(キヌレニン)検体に特異的な抗体を含む。
(3)反応膜:捕捉帯又は検査線として反応膜を横切る線の中に、抗標的検体抗体が固定化された(複合体抗体に特異的な抗体を含む対照帯があってもよい。)、通常は、疎水性のニトロセルロース膜又は酢酸セルロース膜。
(4)吸水体又は廃液容器:毛管現象により反応膜を横切った試料を吸い上げて集めるように設計された追加的な吸収パッド。
この形式においては、試料は、試料パッドから複合体パッドを通って移動し、そこでは、存在する標的検体は複合体に結合する。次に、試料は膜を横切って捕捉帯に到達するまで移動し続け、そこで標的/複合体コンプレックスは固定化抗体と結合し、膜上に目に見える線を形成する。試料はストリップに沿って対照帯に到達するまでさらに移動し、そこで過剰の複合体が結合し、膜上に目に見える第二の線を形成する。この対照線は、試料が目的通り膜を横切り移動したことを示す。膜上に2本のはっきりした線があれば、結果が確かであることを示す。対照帯に1本の線がある場合は、陰性の結果である。二重抗体サンドイッチアッセイは、細菌性病原体やウイルスなどの複数の抗原部位を有するより大きな検体に最適である。本発明において、抗体の適切な組み合わせは、キヌレニン上の異なるエピトープに結合するものから選ばなければならない。
競合アッセイは、主として低分子の検査に使用され、二重抗体サンドイッチ形式とは異なり、複合体パッドは標的検体又はその類似体と既に結合している抗体を含む。したがって、試料中に標的検体が存在すると、標的検体は複合体と結合せず、未標識のままである。試料が膜に沿って移動し、捕捉帯に到達すると、過剰の未標識検体は固定化された抗体に結合して複合体の捕捉を妨害し、目に見える線は生成されない。次に、未結合複合体は対照帯にある抗体と結合し、目に見える対照線が生成する。膜上に1本の対照線があれば陽性の結果である。捕捉帯と対照帯とに2本の目に見える線があれば陰性の結果である。しかしながら過剰の未標識標的検体が存在しなければ、捕捉帯に薄い線が生成され、不確定な結果であることを示す。競合アッセイはマイコトキシンなどの低分子の検査に最も適しており、2種以上の抗体と同時に結合することができない。ラテラルフロー技術には多数の変種がある。膜上の捕捉帯は、抗体よりもむしろ固定化した抗原又は酵素を含んでもよいが、これは標的検体によって決まる。多重検査を行うために多数の捕捉帯を使用することもできる。
キヌレニンを経由するトリプトファン代謝産物は、数十年来知られている発色反応により、体液中において定量的に測定することができる(例えば、Coppini et al., Clinical Chemistry, Vol. 5, No. 5, 1959, p. 391-401)。一般的に、発色反応産物の形成による検出方法は、標準方法により実施される。
本検査は、改良法として開発した。
発色試薬及びキヌレニンの標準溶液の希釈液を調製した。発色反応は、黄色を与えるいわゆるエールリッヒ試薬を用いて行った。20%塩酸に溶解した2重量%のジメチルアミノベンズアルデヒドを含む溶液をエールリッヒ試薬と呼ぶ。この発色試薬は、第1級アミノ基、ピロール誘導体及びインドール誘導体を検出する。濃度の比色定量は、単色光により行われる。キヌレニンの標準溶液は、L‐キヌレニン硫酸を用いて調製した。
予備研究において、術後経過がはっきりしている腎臓同種移植レシピエントの400例以上からの15000例以上の血清について、L‐キヌレニンレベルを測定した。キヌレニンレベルは、IDO活性化の程度を反映する。すべてのレシピエント群は、腎移植前には高いキヌレニンレベルを示した(16.5±5 nmol/mL、正常な安定移植患者群:5.3±1.2 nmol/mL、臓器ドナー群:6.5±5.5 nmol/mL、及び健常対照群:2.4±0.3 nmol/mL、群間差p<0.001)。キヌレニン値は血清を用いて測定した。速やかに機能している移植腎を持つレシピエントにおいて、キヌレニンレベルは、3〜5日以内に正常に戻った。移植片機能が回復していない全例で、キヌレニンレベルが高いことと関連しているが、このキヌレニンレベルは、移植片が機能した後に正常に戻った(透析及び低い尿排出の期間に活性化がある)。根本的に機能しない移植片を持つレシピエントでは、手術前の高いキヌレニンレベルは変化しなかった。もともと機能している移植片を持つレシピエントでは、移植片機能の機能停止は、キヌレニンレベルの顕著な上昇と直ちに関連する。これらの研究結果は、臨床腎移植におけるキヌレニン活性の重要性を証拠付ける。248例のレシピエントを登録した拡大研究により、キヌレニン活性の臨床的関連は、拒絶反応及び長期的な機能の予測要素であることを示した。
Claims (12)
- 唾液中のキヌレニンレベルを測定する、特に移植拒絶反応における潜在的炎症のインビトロ早期検出方法。
- 唾液中のキヌレニンレベルを定量的に測定する、請求項1に記載の方法。
- 潜在的炎症が、肝臓、膵臓、心臓、肺臓及び腎臓からなる群より選ばれる移植臓器の拒絶反応に起因する、請求項1又は2に記載の方法。
- 潜在的炎症が、角膜移植組織、網膜移植組織、軟骨移植組織及び皮膚移植組織からなる群より選ばれる移植組織に起因する、請求項1又は2に記載の方法。
- 潜在的炎症が、人工移植組織の拒絶反応に起因する、請求項1又は2に記載の方法。
- 人工移植組織が、骨置換物、関節置換物、嵌植歯、軟骨植込み物、乳房植込み物及び陰茎植込み物からなる群より選ばれる、請求項5に記載の方法。
- 移植後の患者の回復について観察することに使用される、請求項1〜6のいずれか1項に記載の方法。
- 唾液中のキヌレニンの測定が治療管理に使用される、請求項1〜7のいずれか1項に記載の方法。
- 唾液中又は血漿中のキヌレニンレベルを測定する、神経変性疾患のインビトロ検出及び/又は観察方法。
- 神経変性疾患が、アルツハイマー病、血管性認知症、パーキンソン病、術後認知障害及びうつ病からなる群より選ばれる、請求項9に記載の方法。
- 唾液中のキヌレニンレベルを測定するのに適した手段を含む、請求項1〜12のいずれか1項に記載の方法を実行するために適したキット。
- 手段がラテラルフローイムノクロマトグラフィーアッセイであることを特徴とする、請求項11に記載のキット。
