[0002] 日本人の糖尿病患者は近年増加の一途をたどっており、 大きな社会問題と なってきている。 糖尿病の 9 0 %以上を占める 2型糖尿病は、 塍; S細胞から のインスリンの分泌低下とインスリン標的臓器である骨格筋、 肝臓、 脂肪組 織でのィンスリン感受性の低下が様々な程度合わさってィンスリン作用の低 下が起こり、 高血糖をきたした状態と言える。 インスリン分泌低下は主に遺 伝的に規定されている可能性が高く、 ィンスリン感受性には遺伝素因と共に 過食、 高脂肪食、 運動不足などの環境因子に起因する肥満が大きく関与する と考えられている。 肥満者では脂肪細胞の肥大を認められ、 肥大した脂肪細 胞からは腫瘍壊死因子 (T N Fひ) 、 レジスチンなどのサイ ト力インや遊離 脂肪酸が多量に産生される。 この T N Fひ、 レジスチン、 遊離脂肪酸は、 骨 格筋や肝臓でィンスリンの情報伝達を障害してィンスリン感受性の低下を惹 起する。 その結果、 生体はインスリンを過剰分泌することにより血糖コント ロールに対応するが、 高インスリン血症となることによってインスリンの標 的臓器のィンスリン受容体の減少を招くとともに、 塍; S細胞が疲弊してィン スリン分泌能が徐々に低下し、 糖尿病状態に進行する。 高血糖状態が持続す ると、 ブドウ糖自身が塍; S細胞のインスリン分泌、 末梢でのインスリン感受 性の低下を増大させ、 ブドウ糖毒性が発揮される。 ここに悪循環が形成され 、 障害の程度が更に悪化する。 従って、 インスリン感受性を改善する薬剤は 糖尿病の治療薬として極めて有用であると考えられる (非特許文献 1〜4 )
[0003] かかる観点から最近インスリン感受性改善薬が開発され、 上市されている 。 しかしながら、 インスリン感受性改善薬であるチアゾリジン系薬剤は、 重
篤な肝障害を生じることが問題となっている。
[0004] また、 従来、 小児の糖尿病のタイプは、 1型糖尿病のほうが 2型糖尿病に 比べて多いが近年、 肥満症の増加に伴った 2型糖尿病の増加が指摘されてい る。 2型糖尿病は、 1型糖尿病と異なり自覚症状が現れず薬物療法も必要な いことが多い。 このため統計的にみると、 血糖値がより高くなりやすい 1型 糖尿病よりも、 かえって網膜症、 腎症、 神経障害、 動脈硬化などの合併症が 起きる頻度が高いと報告されている。 小児の糖尿病は、 遺伝的要素が強く、 肥満を伴っていることが多い。 また、 発病の年齢が低いだけに、 合併症が起 きた場合には、 若いうちに重篤な障害に至ることがある。
当該肥満小児の発現原因のひとつとして、 インスリン感受性が低下してい る例が多く認められる。 かかる肥満小児に対して成人と同様の糖尿病治療薬 を投与した場合には、 成人においても生じる重篤な肝障害等の副作用が更に 強く生じる可能性が高いことから、 安全に使用できる肥満小児のィンスリン 感受性を改善する医薬はないのが現状である。
また、 緑茶、 紅茶、 烏龍茶等に含有されるカテキン類は、 コレステロール 上昇抑制 (特許文献 1 ) 、 αアミラーゼ活性阻害 (特許文献 2) 等において 、 その生理的な有益性が認められているが、 肥満小児におけるインスリン感 受性に対する作用については知られていない。

[0007] 本発明は、 対象が成長期にある児童であるので、 摂取エネルギーの変更な どに影響が少なく、 かつ、 自己管理しやすい投与方法であり、 かつ、 長期間 使用できる肥満小児のィンスリン感受性改善剤を提供することにある。
[0008] そこで本発明者は、 カテキン類の肥満小児のィンスリン感受性に及ぼす影 響について検討した結果、 力テキン類を継続摂取することによって肥満小児 のィンスリン感受性が顕著に改善されることを見出した。
[0009] 本発明のィンスリン感受性改善剤を用いれば、 肥満小児のィンスリン感受 性を顕著に改善することができる。 また、 長期間摂取可能である。
[0010] 本発明で使用するカテキン類としては非重合体カテキン類が好ましい。 非 重合体カテキン類とは、 具体的には、 カテキン、 ガロカテキン、 力テキンガ レート、 ガロカテキンガレート等の非ェピ体非重合体カテキン類 (A) 及び ェピカテキン、 ェピガロカテキン、 ェピカテキンガレート、 ェピガロカテキ ンガレート等のェピ体非重合体カテキン類 (B) をあわせての総称である。
[0011] 本発明に使用するカテキン類は、 C am e l I i a属、 例えば C. s i n e n s i s及び C. a s s am i c a、 やぶきた種又はそれらの雑種から得 られる茶葉から製茶された、 煎茶、 玉露、 てん茶等の緑茶類や、 総称して烏 龍茶と呼ばれる鉄観音、 色種、 黄金桂、 武夷岩茶等の半発酵茶、 紅茶と呼ば れるダージリン、 アツサム、 スリランカ等の発酵茶の茶葉から水や熱水によ り抽出して得られる。
茶を抽出する方法については、 攪拌抽出など従来の方法により行う。 また 抽出時の水にあらかじめァスコルビン酸ナトリウム等の有機酸又は有機酸塩 類を添加してもよい。 また煮沸脱気や窒素ガス等の不活性ガスを通気して溶 存酸素を除去しつついわゆる非酸化的雰囲気下で抽出する方法も併用して茶 抽出液を得てもよい。
[0012] 更に、 抽出したものを濃縮して使用してもよい。 茶抽出物の濃縮物とは、 茶葉から熱水もしくは水溶性有機溶媒により抽出された抽出物を濃縮したも のであって、 特開昭 5 9 - 2 1 9 3 8 4号公報、 特開平 4— 2 0 5 8 9号公 報、 特開平 5— 2 6 0 9 0 7号公報、 特開平 5— 3 0 6 2 7 9号公報等に詳 細に例示されている方法で調製したものをいう。 市販の三井農林 (株) 「ポ リフエノン」 、 伊藤園 (株) 「テアフラン」 、 太陽化学 (株) 「サンフエノ ン」 、 サントリ一 (株) 「サンゥ一ロン」 等が挙げられる。 そのほか、 カテ キン類は他の原料起源のもの、 カラム精製品及び化学合成品でも使用できる 。 ここでいう茶抽出物の濃縮物の形態としては、 固体、 水溶液、 スラリー状 等種々のものが挙げられる。
[0013] 本発明においては、 茶抽出液及び茶抽出物の濃縮物を単独又は混合して使 用することができる。 また、 カテキンを摂取する形態としては、 カテキン類 が液体状態、 スラリー状態及び固体状態で使用することができる。
[0014] 力テキン類は抽出前の茶葉の発酵状態が進むにつれて重合するので、 水又 は茶の抽出液に各種茶抽出物の濃縮物を添加する場合は、 特に緑茶抽出物の 濃縮物が好ましい。
[0015] 製剤中で総ポリフエノール中のカテキン類の含有率としては、 非ェピ体非 重合体カテキン類 (A ) +ェピ体非重合体カテキン類 (B ) の値とポリフエ ノールの比率は、 好ましくは、 5 0〜1 0 0重量%、 より好ましくは、 6 0 〜 9 8重量%、 特に好ましくは、 7 0〜 9 5重量%である。 ここで、 総ポリ フエノ一ル濃度とは、 酒石酸一鉄法で測定されるポリフヱノ一ル量をいう。
[0016] 更に非ェピ体 (A ) とェピ体 (B ) の含有重量比は (B ) / ( A ) = 0 .
5〜2 0が好ましく、 更に好ましくは 1〜1 5、 特に 3〜1 5が好ましい。
この範囲であると肥満小児のインスリン感受性改善効果が明瞭に発現される
[0017] また、 カテキン類の含有量の 30〜 98重量%、 好ましくは 40〜 90重 量0 /oが、 ェピガロカテキンガレート、 ガロカテキンガレート、 ェピガロカテ キン及びガロカテキンから選ばれたものであると、 製剤としての呈味が更に 優れ、 無理なく連用できるため、 好ましい。 ここでェピガロカテキンガレ一 ト、 ガロカテキンガレ一ト、 ェピガロカテキン、 ガロカテキンは 1種以上含 有する。
[0018] 本発明のィンスリン感受性改善剤の効果を出すための小児 1 日当り投与量 は、 非重合体カテキン類として、 300〜250 Omgである。 特に、 好ま しくは 400〜1 300mg、 更に好ましくは 450〜1 300mg、 50 0〜800mgである。
[0019] 本発明を利用する場合、 カテキン類の吸収量の点から、 1 日あたりのカテ キン類量を少ない回数で摂取する方がカテキン類の血中濃度が高くなり、 力 テキン類のィンスリン感受性改善作用を発現しやすい。
[0020] 本発明のィンスリン感受性改善剤の投与対象は肥満小児である。 ここで小 児とは、 児童 (2〜1 1才) と青少年 (1 2〜1 6才) をいう。 また、 肥満 小児における肥満とは、 肥満度 (= (実測体重一標準体重) ÷標準体重 X 1 00) 20%以上であることをいう。
本発明のインスリン感受性の対象としては、 6才以上の小児で、 肥満度 2 0%〜80%、 好ましくは、 30%〜80%、 更に好ましくは、 30%〜5 0 <½の小児を対象とするのが効果の点から好ましい。
[0021] 本発明の対象小児は、 ィンスリン感受性が低下している肥満小児であれば よいが、 ィンスリン感受性検査指数 (QU I CK I ) が 0. 357以下、 好 ましくは、 0. 31以下、 特に好ましくは、 0. 21〜0. 30の小児を対 象とするのが好ましい。 インスリン感受性の判定には、 ソマトスタチンで内 因性のィンスリン分泌を抑制し、 一定のィンスリンとグルコースを経静脈的 に持続注入して、 血糖値が上昇した後にほぼ平衡状態 (s t e a d y— s t
a t e p l a sma g l u c o s e) になった頃の血糖値をィンスリン 感受性の指標とする S S PG (s t e a d y - s t a t e p l a sma g l u c o s e) 法が使用される。 し力、し、 この方法は非常に時間がかかる うえ、 機器も必要で、 更に患者も苦痛を伴うことから、 通常は空腹時血糖値 ( g I u c ; mg/d L) とインスリン値 ( i n s ; mU/L) から 1 / ( l o g (g I u c * i n s) ) によって算出されるインスリン感受性検査指 数 (QU I CK I ) 力 最も簡便で一般的な指標として用いられている。
[0022] 本発明の対象小児は、 ィンスリン感受性が低下している肥満小児であれば よいが、 インスリン感受性の低下はインスリン抵抗性をも生じる。 従って本 発明の対象として、 インスリン抵抗性の指標である HOMA— R ( t h e h om e o s t a s i s mo d e l a s s e s sm e n t o f i n s u l i n r e s i s i t a n c e) 値が 2以上、 好ましくは 4以上、 更 に好ましくは 5以上 50以下を示す肥満児を対象とするのが好ましい。 なお 、 HOMA— Rは、 以下の式で算出される。

[0024] インスリン感受性を改善するための摂取期間としては、 8週を超えての期 間が好ましく、 1 2週以上が特に好ましい。
[0025] 本発明の製剤としては、 例えば散剤、 顆粒剤、 カプセル剤、 丸剤、 錠剤等 の固形製剤、 水剤、 懸濁剤、 乳剤等の液剤等の経口投与剤が挙げられる。 こ の経口投与剤は、 経口投与剤の形態に応じて一般に用いられる賦形剤、 崩壊 剤、 結合剤、 滑沢剤、 界面活性剤、 アルコール類、 水、 水溶性高分子、 甘味 料、 矯味剤、 酸味料等を添加して製造することができる。 カテキン類は、 経 口投与用医薬品中に、 医薬品の用途及び形態によっても異なるが、 一般に 0 . 1〜1 00重量%、 特に 1〜80重量%含有するのが好ましい。
[0026] 本発明を食品として用いる場合は、 食品の一部として種々の食品に用いる
ことができる。 当該食品の形態には、 飲料、 醤油、 牛乳、 ヨーグルト、 味噌 等の液状又は乳状又はペースト状食品;ゼリー、 グミ等の半固形状食品; ガ ム、 豆腐、 サプリメント等の固形状食品; あるいは粉末状食品等いかなる形 態でもよい。
[0027] 本発明のィンスリン感受性改善剤としては、 飲料の形態、 特に容器詰飲料 の形態とするのが、 力テキン類を容易に摂取でき、 かつ、 連用のしゃすさの 点から好ましい。
飲料中のカテキン類の含有量は 0 . 0 6〜0 . 5重量%、 好ましくは 0 . 8〜0 . 2 6重量0 /o、 更に好ましくは 0 . 1 0〜0 . 2 6重量%、 特に0 . 1 2〜0 . 1 6重量%が好ましい。 この量であるとカテキン類の多量の摂取 が容易でありながら、 強烈な苦味、 渋味、 強い収斂性も生じなく好ましい。 また 0 . 0 6重量%以下であると、 一日あたり飲料の摂取量が増えるため、 好ましくない。
[0028] 飲料とする場合は、 非ェピ体力テキン類が多くなれば、 色調の長期安定性 が図られるので好ましい。 カテキン類中の非ェピ体力テキン類とェピ体力テ キン類の含有重量比率は 1 0 : 9 0以上、 好ましくは 2 0 : 8 0以上、 更に 好ましくは 3 0 : 7 0以上、 最も好ましくは 4 0 : 6 0以上が飲料としての 保存性の面で好ましい。
[0029] また飲料中のカテキン類は、 溶解状態で存在する方が、 味わいや喉越しの 面で飲み易く、 連用性が高く好ましい。
[0030] カテキン量の調節の点から、 茶抽出液と茶抽出物の濃縮物とを併用するこ とができる。 非重合体カテキン類合計含有量のうち茶抽出物の濃縮物由来の カテキン類含有量の占める比率が 5〜 1 0 0 %であり、 更に好ましくは 1 0 〜 1 0 0 %、 特に 2 0〜 1 0 0 %含有することが風味の観点から、 好ましい
[0031 ] 飲料の p Hは、 2 5 °Cで 3〜7、 好ましくは 4〜7、 特に好ましくは 5〜
7とするのが、 味及び非重合体カテキン類の化学的安定性の点で好ましい。
[0032] 飲料としては、 果汁■果実飲料、 コーヒー飲料、 烏龍茶飲料、 緑茶飲料、
紅茶飲料、 麦茶飲料、 野菜飲料等の他の飲料と組み合わせることで、 幅広い 範囲の非重合体カテキン含有飲料を提供することが可能である。 例えばソフ トドリンクである炭酸飲料、 果実エキス入り飲料、 野菜エキス入りジュース や、 ニァウォーター、 スポーツ飲料、 ダイエット飲料等に適宜添加すること もできる。
[0033] これらの中でも特に緑茶、 烏龍茶、 紅茶などの本来甘味料を必要としない 飲料形態での処方が好ましい。 また紅茶などに甘味料を使用する場合につい ては低カロリーの人工甘味料を使用するほうが好ましい。
[0034] 飲料には、 非重合体カテキン類に併せて、 酸化防止剤、 香料、 各種エステ ル類、 有機酸類、 有機酸塩類、 無機酸類、 無機酸塩類、 無機塩類、 色素類、 乳化剤、 保存料、 調味料、 甘味料、 酸味料、 果汁エキス類、 野菜エキス類、 花蜜エキス類、 p H調整剤、 品質安定剤等の添加剤を単独、 又は併用して配 合しても良い。
[0035] 飲料を容器詰飲料にする場合、 使用される容器は、 一般の飲料と同様にポ リエチレンテレフタレートを主成分とする成形容器 (いわゆる P E Tポトル ) 、 金属缶、 金属箔ゃプラスチックフィルムと複合された紙容器、 瓶等の通 常の形態で提供することができる。
[0036] また、 容器詰飲料は、 例えば、 金属缶のように容器内を完全に液で満たす 力、、 脱気、 窒素置換又はその両方を行って後、 加熱殺菌できる場合にあって は食品衛生法に定められた殺菌条件で製造される。 P E Tボトル、 紙容器の ようにレトルト殺菌できないものについては、 あらかじめ上記と同等の殺菌 条件、 例えばプレート式熱交換器等で高温短時間殺菌後、 一定の温度迄冷却 して容器に充填する等の方法が採用される。 また無菌下で、 充填された容器 に別の成分を配合して充填してもよい。 更に、 酸性下で加熱殺菌後、 無菌下 で p Hを中性に戻すことや、 中性下で加熱殺菌後、 無菌下で p Hを酸性に戻 す等の操作も可能である。
実施例

メンブランフィルタ一 (0. 8 m) でろ過した飲料を、 島津製作所製、 高速液体クロマトグラフ (型式 S C L— 1 O AV P) を用い、 ォクタデシル 基導入液体クロマトグラフ用パック ドカラム L—カラム TM O D S (4 . 6 mm0 X 2 5 Omm:財団法人 化学物質評価研究機構製) を装着し、 カラム温度 3 5°Cでグラジェント法により行った。 移動相 A液は酢酸を 0. 1 m o I /L含有の蒸留水溶液、 B液は酢酸を 0. 1 m o I /L含有のァセ トニトリル溶液とし、 試料注入量は 20 L、 U V検出器波長は 2 80 n m の条件で行った。

外来小児肥満者を対象とし、 茶カテキン高含有飲料 (1 日茶カテキン 57 6 m g摂取、 カテキン群) 及び対照飲料 (茶カテキン 7 5 m g摂取、 コント ロール群) との間でダブルブラインド並行群間比較を行った。 試験前観察期 間を 4週間、 試験飲料摂取期間 24週間、 試験後観察期間を 1 2週間とした
[0039] 空腹時血糖値 (g I u c ; m g/d L) とインスリン値 ( i n s ; mU/
L) から 1 / ( l o g ( g l u c * i n S) ) によって算出されるインスリ ン感受性検査指数 (Q U I C K I ) により、 試験飲料 24週間摂取前後のィ ンスリン感受性改善効果を評価した。

[0045] カテキン群では Q U I C K Iの初期値と 2 4週時の変化量に有意な負の相 関性が認められた。 これはカテキン飲料継続摂取によってインスリン感受性 が悪い肥満小児には改善を、 ィンスリン感受性が良い肥満小児には影響がな いことを示す。
—方、 コントロール群では Q U I C K Iの初期値と 2 4週時の変化量に有 意な関係性は認められず、 コントロール飲料 (一般的なお茶相当) を継続摂 取しても Q U I C K Iに対して何ら影響を及ぼさない事を示す。
[0046] 表 1及び図 1から明らかなように、 カテキン群では初期値と 2 4週間後の 変化量に有意な負の相関性が得られ、 カテキン群ではィンスリン感受性が悪 い小児ほど良く改善していることが判明した。 一方、 コントロール群では有 意な相関性は認められなかった。 また、 表 2から明らかなように、 カテキン 類の効果は、 H O M A _ Rが 5以上の肥満小児で顕著であった。

[3] 肥満小児が、 ィンスリン感受性検査指数 (QU I CK I ) 0. 357以下 を示す肥満小児である請求項 1又は 2記載のィンスリン感受性改善剤。
[4] 肥満小児のインスリン抵抗性の指標 (HOMA— R) が 5以上である請求 項 1〜 3のいずれか 1項記載のィンスリン感受性改善剤。
[5] 肥満小児の年齢が 6歳〜 1 6歳である請求項 1〜 4のいずれか 1項記載の インスリン感受性改善剤。
[6] 飲料の形態である請求項 1〜 5のいずれか 1項記載のィンスリン感受性改 善剤。
[7] カテキン類の、 肥満小児のインスリン感受性改善剤製造のための使用。
[8] カテキン類が非重合体カテキン類である請求項 7記載の使用。
[9] 肥満小児が、 ィンスリン感受性検査指数 (QU I CK I ) 0. 357以下 を示す肥満小児である請求項 7又は 8記載の使用。
