本發明係關於用於製備熱穩定含氧載體醫藥組成物以便有利於β-β交聯的方法。使用本發明方法可控制所得分子之氧親和力,由此可產生為特定應用定製之基於血紅素之氧載體。較低氧親和力交聯之血紅素適用於需要快速組織氧合之應用(例如出血性休克)而較高氧親和力交聯之血紅素適用於需要較慢氧合速率之應用(例如癌症輔助治
療)。
血紅素在大部分脊椎動物之血管系統與組織之間之氣體交換中起重要作用。其負責將氧經由血液循環自呼吸系統攜帶至身體細胞中且亦攜帶代謝廢物二氧化碳離開身體細胞至呼吸系統中,在呼吸系統中二氧化碳被呼出。因為血紅素具有此氧輸送特徵,所以若其可在活體外穩定且在活體內使用,則血紅素可用作有力的供氧器。
天然存在之血紅素為四聚體,當其存在於紅血球內時大體上穩定。然而,當天然存在之血紅素自紅血球中移出時,其在血漿中變得不穩定且分裂為兩個α-β二聚體。此等二聚體中之每一者之分子量大約為32 kDa。此等二聚體當經由腎過濾且排泄時會引起實質性腎損傷。該四聚體鏈結之分解亦不利地影響功能性血紅素在循環中的可維持性。
為解決該問題,近來在血紅素處理方面之發展已併入各種交聯技術以產生四聚體內之分子內鍵結及四聚體間之分子間鍵結,從而形成聚合血紅素。先前技術教示,為了延長血紅素之循環半衰期,聚合血紅素為較佳形式。然而,如本發明人所測定,聚合血紅素在血液循環中更容易轉化成變性血紅素。變性血紅素無法結合氧且因此無法氧合組織。因此,由先前技術所教示,造成聚合血紅素形成之交聯為一個問題。在此項技術中需要一種允許分子內交聯以產生穩定四聚體同時不形成聚合血紅素之技術。
在先前技術中試圖穩定血紅素之其他問題包括製造四聚血紅素,其包括不可接受高百分比之二聚體單元(或若對患者投予則快速分裂成二聚血紅素之非交聯四聚血紅素);二聚體之存在使得血紅素組成物對哺乳動物之投藥不令人滿意。二聚形式之血紅素會在哺乳動物體內造成嚴重的腎損傷;此腎損傷之嚴重程度可足以造成死亡。因此,在此項技術中需要形成在最終產物中具有不可偵測之二聚形式之穩定四聚血紅素。
先前技術血紅素產品之另一問題為投藥之後血壓突然升高。在過去,已記錄老一代基於血紅素之氧載體之血管收縮事件。因此在此項技術中需要一種在施加至哺乳動物時不引起血管收縮及高血壓之血紅素製備方法。
先前技術試圖形成穩定血紅素之其他問題包括存在可在哺乳動物中引起過敏效應之蛋白質雜質,諸如免疫球蛋白G。因此,在此項技術中需要一種可製造無蛋白質雜質之穩定四聚血紅素之方法。
除以上問題之外,在此項技術中需要無二聚體、無磷脂且能夠以工業規模製造之穩定四聚血紅素。
基於血紅素之氧載體可用於廣泛多種醫學應用中;視醫學應用而定,不同程度之氧親和力為合乎需要的。舉例而言,具有低氧親和力之血紅素分子可比具有較高氧親和力之血紅素分子更容易將氧轉移至目標組織中。因此需要控制交聯四聚血紅素之氧親和力。因此,在此項技術中需要控制交聯類型及交聯條件以製造具有精確氧結合程度的
交聯四聚血紅素。
本發明提供一種處理非聚合、熱穩定、經純化之交聯四聚血紅素的方法,該血紅素適於在哺乳動物中使用而不引起嚴重腎損傷、血管損害效應及包括死亡之嚴重不良事件。本發明移除二聚形式之血紅素、非交聯四聚血紅素、磷脂及蛋白質雜質。本發明亦提供一種控制所得交聯四聚體之氧親和力的技術,該技術藉由控制四聚體之交聯類型(例如四聚體中之β-β交聯、α-α交聯、α-β交聯之量)、四聚體之四級結構及交聯條件來實現。較低氧親和力之交聯血紅素適用於需要快速組織氧合之應用(例如出血性休克)而較高氧親和力之交聯血紅素適用於需要較慢氧合速率之應用(例如癌症輔助治療)。另外,本發明使用(1)用於精確及經控制的低滲透壓溶解之瞬時細胞溶解裝置,(2)流通管柱層析,(3)在純化程序中用於熱處理血紅素溶液之高溫短時(HTST)裝置,以移除不合需要之不穩定血紅素二聚體且移除例如免疫球蛋白G之蛋白質雜質,從而避免腎損傷、血管損害效應及其他毒性反應,及(4)氣密式輸液袋包裝,從而避免氧侵入最終產物中。
該方法包括哺乳動物全血之起始物質,其至少包括紅血球及血漿。紅血球與哺乳動物全血中之血漿分離,隨後過濾以得到經過濾之紅血球部分。洗滌經過濾之紅血球部分以移除血漿蛋白雜質。藉由在瞬時細胞溶解裝置中經控
制的低滲透壓溶解足以溶解紅血球而不溶解白血球之時間來破壞經洗滌之紅血球。進行過濾以移除至少一部分來自溶胞物之廢棄阻留物。自溶胞物萃取第一血紅素溶液。
對該血紅素溶液進行一或多種純化程序,諸如超濾及/或層析。
用雙-3,5-二溴水楊基延胡索酸酯(DBSF)交聯經純化之血紅素以形成熱穩定交聯之血紅素而不形成聚合血紅素,以使得所得非聚合交聯四聚血紅素之分子量為60 kDa至70 kDa。如本文所用之表述「非聚合」係指不與其他血紅素分子或諸如PEG之任何其他非血紅素分子分子間交聯的四聚血紅素。視血紅素來源、血紅素之四級結構及交聯條件而定,可製備具有高β-β交聯百分比之四聚產物。另外,可控制所得分子之氧親和力以便可製備為特定應用定製之基於血紅素之氧載體。
在此程序之後,熱處理該交聯血紅素以移除任何殘餘非交聯四聚血紅素及任何不穩定的血紅素(例如二聚形式之血紅素)、及任何其他蛋白質雜質。在熱處理之前,視情況以大約0.2%之濃度添加N-乙醯基半胱胺酸至交聯四聚血紅素中以防止變性血紅素之形成。在熱處理及冷卻之後立即視情況以大約0.2%至0.4%之濃度添加N-乙醯基半胱胺酸以進一步防止變性血紅素之形成。該熱處理較佳為高溫短時處理,在大約70℃至95℃下進行30秒至3小時,隨後冷卻至25℃。藉由離心或過濾移除在熱處理期間形成之任何沈澱物。
隨後將無二聚體、無磷脂、無蛋白質雜質、熱穩定、非聚合交聯之四聚血紅素添加至醫藥學上可接受之載體中。
其後,調配熱穩定之交聯四聚血紅素且包裝在常規製備且氣密式聚乙烯、乙烯-乙酸乙烯酯及乙烯-乙烯醇(PE、EVA、EVOH)輸液袋中。該包裝防止氧污染,氧污染將導致形成非活性變性血紅素。
血紅素為哺乳動物及其他動物血液之紅血球中之含鐵氧輸送蛋白。血紅素展現蛋白質之三級及四級結構特性。血紅素中之大部分胺基酸藉由非螺旋短片段連接形成α螺旋。氫鍵結使血紅素內部之螺旋部分穩定,造成分子內吸引,從而使每一多肽鏈摺疊成特定形狀。自四個球狀蛋白次單元組裝血紅素分子。各次單元由配置在一組α-螺旋結構片段中之多肽鏈構成,該α-螺旋結構片段連接在排列有植入原血紅素之「肌紅素摺疊(myoglobin fold)」中。
原血紅素由保持在稱為卟啉之雜環中的鐵原子組成。該鐵原子同等結合至該環中心之處於一個平面中的所有四個氮原子上。氧隨後能夠結合至垂直於該卟啉環平面之鐵中心。因此單個血紅素分子具有結合四個氧分子之能力。
在哺乳動物中,血紅素之最常見類型為四聚體;在人類中,稱為血紅素A且由稱為α2β2之兩個α及兩個β非共價結合次單元組成,每一次單元分別由141及146個胺基酸
殘基構成。α次單元及β次單元之尺寸及結構彼此非常類似。每一次單元具有約16 kDa之分子量,該四聚體之總分子量為約65 kDa。四條多肽鏈藉由鹽橋、氫鍵結及疏水性相互作用彼此結合。牛血紅素之結構類似於人類血紅素(α鏈中一致性為90.14%;β鏈中一致性為84.35%)。差異為牛血紅素中之兩個巰基定位在β半胱胺酸93上,而人類血紅素中之巰基分別定位在α半胱胺酸104、β半胱胺酸93及β半胱胺酸112上。當比較人類血紅素之胺基酸序列時,其與牛、犬、豬及馬血紅素共享高相似性。
在紅血球內部天然存在之血紅素中,α鏈與其相應β鏈之結合非常強烈且在生理條件下不分離。然而,在紅血球外部,一個αβ二聚體與另一αβ二聚體之結合極其薄弱。該結合具有分裂成兩個αβ二聚體之傾向,每個二聚體大約32 kDa。此等不合需要之二聚體足夠小以經腎過濾且排泄,結果為潛在的腎損傷及實質上減少的血管內滯留時間。
因此,出於療效及安全性兩者之考慮有必要穩定在紅血球外使用之任何血紅素。用於製造穩定血紅素之方法在下文中概述;本發明方法之概述呈現於圖1之流程圖中。
首先,選擇全血源作為血紅素來自紅血球之來源。選擇包括(但不限於)人類、牛、豬、馬及犬全血之哺乳動物全血。紅血球自血漿分離,過濾,且洗滌以移除血漿蛋白質雜質。
為自紅血球釋放血紅素,溶解細胞膜。儘管可使用各種技術來溶解紅血球,但本發明在低滲透壓條件下以可精
確控制體積以適用於工業規模製造之方式使用溶解。為此目的,使用如圖2中所見之瞬時細胞溶解裝置溶解紅血球。低滲透壓溶解產生包括血紅素及廢棄阻留物之溶胞物的溶液。為了實現工業規模製造,小心地控制溶解以使得僅溶解紅血球而不溶解白血球或其他細胞。在一個具體實例中,選擇瞬時細胞溶解裝置之尺寸以使得紅血球在2至30秒或另外足以溶解紅血球之時間內且較佳30秒內穿過該裝置。該瞬時細胞溶解裝置包括靜態混合器。使用去離子及蒸餾水作為低滲透壓溶液。當然,應理解,使用具有不同鹽濃度之其他低滲透壓溶液用於紅血球溶解將產生不同時間段。因為經控制的溶解程序僅溶解紅血球而非白血球或細胞物質,所以其最小化釋放來自白血球及其他細胞物質之毒性蛋白、磷脂或DNA。30秒後,亦即在含紅血球溶液已穿過瞬時細胞溶解裝置之靜態混合器部分之後,立即添加高滲透壓溶液。所得血紅素與用其他溶解技術所得之血紅素相比具有較高純度及較低含量之污染物,諸如不合需要之DNA及磷脂。分別藉由聚合酶鏈反應(偵測極限=64 pg)及高效液相層析(HPLC,偵測極限=1 μg/ml)法,在血紅素溶液中均未偵測到來自白血球之不合需要之核酸及磷脂雜質。
在該方法之此階段中,純化血紅素溶液以移除各種蛋白及其他雜質。此純化可為基於超濾、基於層析、或一或多種超濾及/或層析程序之組合。在一個例示性具體實例中,進行兩個超濾程序:一個在流通管柱層析之前移除分
子量大於血紅素之雜質,及另一個在流通管柱層析之後移除分子量小於血紅素之雜質。後一超濾程序濃縮血紅素。在一些具體實例中,100 kDa過濾器用於一級超濾,而30 kDa過濾器用於二級超濾。
使用流通管柱層析來移除經純化之血紅素溶液中之蛋白質雜質,諸如免疫球蛋白G、白蛋白及碳酸酐酶。在一些具體實例中,藉由使用諸如DEAE管柱、CM管柱、羥磷灰石管柱等市售離子交換管柱中之一者或組合進行管柱層析。用於管柱層析之pH值典型地為6至8.5。在一個具體實例中,使用流動CM管柱層析步驟在pH 8.0下移除蛋白質雜質。在自管柱層析洗提之後進行酶聯免疫吸附分析(ELISA)及HPLC法來偵測殘留在樣品中之蛋白質雜質及磷脂。此獨特流通管柱層析分離實現了用於工業規模製造之連續分離方案。ELISA結果顯示在洗提血紅素中此等雜質之量實質上較低(免疫球蛋白G:44.3 ng/ml;白蛋白:20.37 ng/ml;碳酸酐酶:81.2 μg/ml)。使用具有不同pH值之不同種類管柱進行的蛋白質雜質移除之結果顯示於以下表1中。
在管柱層析程序之後,用DBSF使血紅素交聯。選擇條件以便促進發生在β-β次單元之間之交聯且所得產物具有大於50%之β-β交聯。對於在去氧條件下交聯,所得血紅素具有較低氧親和力,與實質上類似條件下量測之相同物種之天然血紅素相比,p50值較高。舉例而言,對於牛血紅素,天然牛血紅素具有約23 mm Hg至29 mm Hg之p50值。在本發明之去氧條件下形成之交聯牛血紅素具有約38 mm Hg至50 mm Hg之p50值。較低氧親和力意謂四聚體可以比具有較高氧親和力之材料更容易「卸載」氧至目標上。對於在充氧條件下交聯牛血紅素,形成具有較高氧親和力之材料,與具有約23 mm Hg至29 mm Hg之p50值之天然牛血紅素相比,其具有小於大約23 mm Hg之較低p50值。當需要快速氧合時,如在由大量失血造成之組織缺氧之情況下(例如出血性休克),使用較低氧親和力組成物。較高氧親和力組成物適用於癌症治療中之氧合輔助治療,其中需要較慢氧傳遞速率。
對於人類血紅素,在去氧條件下交聯典型地產生大部分具有較低氧親和力之α-α交聯血紅素,亦即氧親和力比天然人類血紅素減小大約至少2倍。相比於在相同條件下之天然人類血紅素(p50值為約23 mm Hg至30 mm Hg),在充氧條件下交聯趨向於有利於製造具有較高氧親和力(亦即小於約23 mm Hg之較低p50值)之β-β交聯血紅素。
對於較佳小於0.1 ppm溶解氧含量之去氧交聯條件,其
在環境溫度(15℃至25℃)下,在較佳約8至9之pH值下在血紅素與DBSF之莫耳比為1:2.5至1:4.0下維持3至16小時之時間段。所得交聯血紅素為具有60 kDa至70 kDa之分子量之四聚血紅素,表明不存在聚合血紅素。DBSF反應之產率較高,>99%且最終產物中之二聚體濃度較低。視情況,本發明程序在交聯之前不需要如各種先前技術程序中所用之諸如碘乙醯胺之巰基治療試劑來與血紅素反應。對於在充氧條件下交聯,使用充氧環境(諸如空氣,pO2
為約149 mm Hg;或純O2
,pO2
接近760 mm Hg)而以上其餘條件實質上相同。
對於牛血紅素,在去氧條件(溶解氧含量小於0.1 ppm)下交聯,β-β交聯大於50%,且較佳大於60%。對於在充氧條件下交聯牛血紅素,β-β交聯亦為有利的,典型地在大於40% β-β交聯之程度下。
對於人類血紅素,在充氧條件下交聯有利於β-β交聯。
交聯之後,用磷酸鹽緩衝鹽水(PBS,一種生理緩衝液)更換交聯溶液且藉由切向流過濾移除任何殘餘化學品。
交聯之後,本發明提供針對交聯四聚血紅素溶液之熱處理步驟(高溫短時,HTST)。該熱處理在去氧環境中進行。在熱處理之前,視情況添加N-乙醯基半胱胺酸以防止形成變性血紅素(非活性的血紅素)。在熱處理步驟之後,冷卻溶液且視情況添加N-乙醯基半胱胺酸以維持較低含量之變性血紅素。若在熱處理之前及之後添加N-乙醯基半胱胺酸,則在熱處理前添加之量大約為0.2%,而在熱處理後添
加之量大約為0.2%至0.4%。然而,若僅在熱處理之後添加N-乙醯基半胱胺酸,則添加量為0.4%。
在一些具體實例中,在去氧環境(溶解氧含量小於0.1 ppm)中,在50℃至95℃之範圍內的溫度下加熱交聯四聚血紅素溶液持續0.5分鐘至10小時。在一些具體實例中,在70℃至95℃之範圍內的溫度下加熱交聯四聚血紅素溶液且持續30秒至3小時。在一些較佳具體實例中,在80℃下加熱交聯四聚血紅素溶液持續30分鐘。且另外在其他較佳具體實例中,將交聯血紅素溶液加熱至90℃持續30秒至3分鐘,接著在大約15至30秒內快速冷卻至大約25℃,且如上所闡述視情況添加N-乙醯基半胱胺酸。
為分析HTST熱處理步驟之結果,使用HPLC分析法來偵測此熱處理步驟後二聚體之量。用於HPLC分析之移動相含有氯化鎂(0.75 M),其可分離二聚體(不穩定四聚體)與熱穩定交聯四聚血紅素。對於促進血紅素分解成二聚體,在相同離子強度下氯化鎂比氯化鈉有效大約30倍。熱處理步驟亦充當大量移除交聯四聚血紅素中非吾人所樂見之蛋白質雜質的變性步驟(免疫球蛋白G不可偵測;白蛋白減少96.15%;碳酸酐酶減少99.99%)。進行酶聯免疫吸附分析(ELISA)偵測樣品中之蛋白質雜質。因此,經純化之熱穩定交聯四聚血紅素溶液具有不可偵測含量之二聚體(低於偵測極限:0.043%)與免疫球蛋白G,及極低量之白蛋白(0.02 μg/ml)與碳酸酐酶(0.014 μg/ml)。圖3顯示二聚形式之血紅素在HPLC系統中不可偵測。表2顯示關於
藉由HTST熱處理步驟移除蛋白質雜質及二聚體之實驗結果。此HTST熱處理步驟實現了自不穩定四聚體(例如非交聯四聚體)及二聚體中選擇性分離熱穩定交聯四聚體。
在去氧條件下對交聯血紅素熱處理步驟之後,熱穩定交聯四聚血紅素已可用於醫藥調配及包裝。本發明描述在去氧環境中熱穩定交聯的四聚血紅素溶液之氣密式包裝步驟。本發明之熱穩定交聯的四聚血紅素當在去氧條件中維持超過兩年時為穩定的。
在本發明中,含氧載體醫藥組成物主要欲用於靜脈內注射應用。傳統地,先前產品使用習知PVC血袋或具有高透氧性之Stericon血袋,其將最終縮短該產品之使用期限,這是因為其在充氧條件下快速(在幾天之內)轉變為非活性變性血紅素。
本發明中所使用之包裝使得熱穩定交聯的四聚血紅素可穩定超過兩年。使用EVA/EVOH材料之多層包裝來使透氣性降至最低且避免形成非活性變性血紅素。由具有0.4
mm厚度之五層EVA/EVOH層壓材料製備設計用於本發明之經純化及熱穩定交聯四聚血紅素之100 ml輸液袋,該輸液袋在室溫下具有0.006至0.132 cm3
/100平方吋/24小時/大氣壓之透氧性。此材料為VI級塑膠(如USP<88>中所定義),其滿足活體內生物反應性測試及物理-化學測試且適用於製造用於靜脈內注射目的之輸液袋。此初級袋(primary bag)特別適用於保護熱穩定交聯的四聚血紅素溶液以免長期氧曝露,該長期氧曝露造成該四聚血紅素溶液之不穩定性且最終影響其治療性質。
對於血液產品之二級保護,已知使用鋁外包裝以保護免遭潛在的漏氣且維持該等產品在去氧狀態中。然而,在鋁外包裝中存在潛在的針孔,其損害該包裝之氣密性且使得該產品不穩定。因此,本發明使用鋁外包裝袋作為二次包裝,其防止氧合且亦防止曝光。該外包裝袋之組成包括0.012 mm聚對苯二甲酸伸乙酯(PET)、0.007 mm鋁(Al)、0.015 mm耐綸(NY)及0.1 mm聚乙烯(PE)。該外包裝膜具有0.14 mm之厚度及在室溫下0.006 cm3
/100平方吋/24小時/大氣壓之透氧率。此二次包裝延長血紅素之穩定時間,延長產品保存期限。
本發明方法適用於大規模工業製造熱穩定交聯四聚血紅素。另外,與醫藥載體(例如水、生理緩衝液,以膠囊形式)組合之熱穩定交聯四聚血紅素適用於哺乳動物。
本發明之含氧載體醫藥組成物適用於改善在癌症治療、諸如出血性休克之缺氧病症治療及在低氧含量環境下
之心臟保存(例如心臟移植)中之組織氧合。在例示性具體實例中,選擇劑量以在輸液速率小於10毫升/小時/公斤體重之情況下具有大約0.2至1.3 g/kg之濃度範圍。
對於用於治療缺氧病症及心臟保存而言,本發明之具有較低氧親和力之含氧載體醫藥組成物用作血液替代物,以為目標器官提供氧。較低氧親和力之交聯血紅素適用於需要快速組織氧合之應用(例如出血性休克及活體外器官保存)。
對於癌症治療中之應用,本發明之具有較高氧親和力之含氧載體醫藥組成物用作組織氧合劑,以在腫瘤組織中改善氧合,從而提高化學及輻射靈敏度。較高氧親和力之血紅素適用於需要較慢氧合速率之應用(例如癌症輔助治療)。
藉助於描述本發明之特定具體實例來提供以下實施例,而不欲以任何方式限制本發明之範疇。
本發明方法之示意性流程圖展示於圖1中。牛全血收集在含有3.8%(w/v)檸檬酸三鈉溶液作為抗凝劑之密閉無菌容器/袋中。隨後立即將血液與檸檬酸三鈉溶液充分混合以抑制血液凝固。藉由脫落機制自血漿及其他較小血細胞分離並收集紅血球(RBC)。「細胞清洗器(cell washer)」
與γ滅菌一次性離心碗一起用於此程序。用相同體積0.9%(w/v氯化鈉)生理鹽水洗滌RBC。
溶解經洗滌之RBC以藉由操縱對RBC細胞膜之低滲透壓衝擊釋放血紅素內容物。圖2中所描繪之用於RBC溶解器件之特殊瞬時細胞溶解裝置係用於此目的。RBC溶解之後,藉由使用100 kDa膜之切向流超濾將血紅素分子自其他蛋白質中分離。收集濾液中之血紅素用於流通管柱層析且藉由30 kDa膜進一步濃縮成12至14 g/dL。進行管柱層析以移除蛋白質雜質。
經濃縮之血紅素溶液首先與DBSF反應形成熱穩定交聯的四聚血紅素分子。隨後在最終調配及包裝之前,在去氧條件下在90℃下進行熱處理步驟持續30秒至3分鐘。
收集新鮮的牛全血且在冷卻條件(2℃至10℃)下運輸。經由細胞清洗器且隨後用0.65 μm過濾器將紅血球自血漿分離。在用0.9%生理鹽水洗滌紅血球(RBC)濾液之後,藉由低滲透壓溶解破壞該濾液。藉由使用圖2中所描繪之瞬時細胞溶解裝置進行低滲透壓溶解。該瞬時細胞溶解裝置包括靜態混合器以輔助細胞溶解。具有經控制的血紅素濃度(12至14 g/dL)之RBC懸浮液與4體積純化水混合以對RBC細胞膜產生低滲透壓衝擊。控制低滲透壓衝擊之週期以避免非吾人所樂見之白血球及血小板之溶解。該低
滲透壓溶液在2至30秒或另外足以溶解紅血球之時間內且較佳30秒內通過瞬時細胞溶解裝置之靜態混合器部分。30秒後,藉由當1/10體積之高滲透壓緩衝液離開靜態混合器時混合溶胞物與該緩衝液來終止衝擊。所用之高滲透壓溶液為pH 7.4之0.1 M磷酸鹽緩衝液,NaCl 7.4%。圖2之瞬時細胞溶解裝置可以連續方式每小時處理50至1000公升溶胞物且較佳每小時至少處理300公升。
RBC溶解之後,藉由0.22 μm過濾器過濾紅血球之溶胞物以得到血紅素溶液。分別藉由聚合酶鏈反應(偵測極限=64 pg)及HPLC(偵測極限=1 μg/ml)法,在血紅素溶液中均未偵測到來自白血球之核酸及磷脂雜質。進行一級100 kDa超濾以移除分子量比血紅素更高之雜質。隨後進行流通管柱層析以進一步純化該血紅素溶液。隨後進行二級30 kDa超濾以移除分子量比血紅素更低之雜質且濃縮。
為了論證本發明產物之安全性,藉由病毒驗證研究論證(1)0.65 μm透濾步驟及(2)100 kDa超濾步驟之病毒移除能力。藉由用不同模型病毒(腦心肌炎病毒、偽狂犬病病毒、牛病毒性腹瀉病毒及牛細小病毒)有計畫的掺加(deliberate spiking)此兩種程序之縮減規模版本完成此研究。在此研究中,使用四種類型病毒(參見表3)。此等病毒在其生物物理及結構特徵方面不同且其顯示在耐受物理及化學試劑
或處理方面的差異。
驗證流程簡要地顯示於以下表4中。
4種病毒在(1)0.65 μm透濾及(2)100 kDa超濾中之log下降結果之概述顯示於以下表5中。所有四種病毒,即BVDV、BPV、EMCV及PRV,經0.65 μm透濾及100 kDa超濾有效移除。

使用CM管柱(購自GE healthcare)進一步移除任何蛋白質雜質。起始緩衝液為20 mM乙酸鈉(pH 8.0),且洗提緩衝液為20 mM乙酸鈉、2 M NaCl(pH 8.0)。在用起始緩衝液使該CM管柱平衡之後,將蛋白質樣品裝入該管柱中。用至少5管柱體積之起始緩衝液洗滌非結合蛋白質雜質。用8管柱體積之25%洗提緩衝液(0至0.5 M NaCl)進行洗提。洗提型態顯示在圖4中;血紅素溶液在流動洗提份中。由ELISA分析流動洗提份之純度。結果顯示於以下表6中。
因為血紅素溶液在pH 8之CM管柱層析之流動中(不在洗提液中),所以其為用於連續工業規模操作之良好方法。第一超濾裝配直接連接至流動CM管柱層析系統上,且流通管可連接至用於工業規模操作之第二超濾裝配中。示意性工業程序組態顯示在圖5中。
(5a)在去氧條件下與DBSF之交聯反應
在去氧條件下(亦即小於0.1 ppm溶解氧含量)進行交聯反應。添加DBSF至血紅素溶液中以形成交聯的四聚血紅素而不形成聚合血紅素。DBSF穩定程序使四聚形式之血紅素(65 kDa)穩定且防止分解成經腎排泄之二聚體(32 kDa)。在此具體實例中,使用1:2.5之血紅素與DBSF之莫耳比且pH值為8.6。在氮氣惰性氛圍中,在環境溫度(15℃至25℃)下進行此程序持續3至16小時之時間以防止血紅素氧化形成鐵變性血紅素,其在生理學上為非活性的(溶解氧含量維持在小於0.1 ppm)。藉由用HPLC量測殘餘DBSF監控DBSF反應之完全性。DBSF反應之產率較高,>99%。β-β交聯之產量大致為至少約40%。
(5b)HTST熱處理步驟
高溫短時(HTST)處理裝置顯示於圖6中。用HTST處理裝置對交聯四聚血紅素進行熱處理步驟。在此實施例中,用於熱處理之條件為90℃持續30秒至3分鐘,且較佳
持續45至60秒,但其他條件可如上文所論述加以選擇且相應地修改裝置。視情況添加有0.2% N-乙醯基半胱胺酸的含有交聯血紅素之溶液以1.0公升/分鐘之流動速率泵吸至HTST處理裝置(預加熱HTST熱交換器之第一部分且維持在90℃下)中,該裝置之第一部分之滯留時間在45秒與60秒之間,隨後溶液以相同流動速率穿過至維持在25℃下之熱交換器之另一部分中。冷卻所需之時間在15秒與30秒之間。冷卻至25℃之後,立即以0.2%至0.4%,較佳以0.4%之濃度添加N-乙醯基半胱胺酸。熱處理裝置之裝備易於控制用於工業操作。溫度型態及二聚體含量顯示於圖7中。若血紅素不交聯,則其對熱不穩定且在熱處理步驟之後形成沈澱物。隨後藉由離心或過濾移除沈澱物以在其之後形成澄清溶液。
以下表7顯示在熱處理步驟之後移除諸如免疫球蛋白G、白蛋白、碳酸酐酶之蛋白質雜質及不合需要的不穩定四聚體或二聚體。用ELISA法量測免疫球蛋白G、白蛋白及碳酸酐酶之量,而用HPLC法測定二聚體之量。在HTST熱處理步驟之後,熱穩定交聯的四聚血紅素之純度極其高,在98.0%至100%之範圍內。
因為本發明產品在去氧條件下穩定,所以將透氣性降至最低對該產品之包裝很重要。對於靜脈內應用,自具有0.4 mm厚度之五層EVA/EVOH層壓材料製備常規設計之100毫升輸液袋,其在室溫下具有0.006至0.132 cm3
/100平方吋/24小時/大氣壓之透氧性。此特定材料為VI級塑膠(如USP<88>中所定義),其滿足活體內生物反應性測試及物理-化學測試且適用於製造用於靜脈內注射目的之容器(應注意,視所要之應用而定,亦可自此材料製備其他形成之包裝)。二級包裝鋁外包裝袋亦應用於一級包裝輸液袋,其提供另一障壁,最小化曝光及氧擴散。該袋之層包括:0.012 mm聚對苯二甲酸伸乙酯(PET)、0.007 mm鋁(Al)、0.015 mm耐綸(NY)及0.1 mm聚乙烯(PE)。外包裝膜具有0.14 mm之厚度及在室溫下0.006 cm3

(7a)由逆相高效液相層析(HPLC)分離血球蛋白鏈
天然牛血紅素之血球蛋白鏈及DBSF交聯的牛血紅素
之交聯血球蛋白鏈用由Shelton等人,1984(稍有修改)開發之梯度在VYDAC C4管柱上解析。
(7b)DBSF交聯之牛血紅素的十二烷基硫酸鈉聚丙烯醯胺凝膠電泳(SDS-PAGE)分析
藉由與還原性樣品緩衝液(62 mM Tris-HCl(pH 6.8)、10%(v/v)甘油、5%(v/v)巰基乙醇及2.3%(w/v)SDS)混合,且在95℃下加熱10分鐘製備天然牛血紅素及DBSF交聯的牛血紅素溶液。用具有4%積層凝膠之15%丙烯醯胺平板凝膠解析樣品混合物。在60 mA恆定電流下執行電泳。電泳後,用0.1%(w/v)考馬斯藍(Coomassie Blue)R350、20%(v/v)甲醇及10%(v/v)乙酸對SDS-PAGE凝膠染色。為了估算DBSF交聯的牛血紅素中不同類型交聯之百分比,用Lumi-Analyst 3.1軟體定量以黑光單位(BLU)表示之解析蛋白帶之強度。
(7c)胰蛋白酶消化還原性血球蛋白鏈

將自蛋白帶萃取之經胰蛋白酶消化之肽在Anchorchip板上點樣,該板用1 μl基質溶液(2 mg/ml氰基-4-羥基肉桂酸)預點樣,在50%ACN/0.1%TFA中飽和,且使其風乾。乾燥後,用10 mM單磷酸鹽緩衝液洗滌樣品點且用乙醇:丙酮:0.1%TFA(比率6:3:1)之溶液再結晶。用Bruker Autoflex III(Bruker Daltonic GmbH,Bremen,德國)進行MALDI-TOF MS分析,其在800 Da至3500 Da之m/z
範圍內以反射模式操作且參數設定如下:用於肽質量指紋(PMF)之離子源25 kV,及用於PMF之反射器26.3 kV。使用布魯克肽混合校準標準(Bruker Peptide Mix Calibration Standard)進行外部校準。S/N比率超過4之峰經Flex分析器(Bruker Daltonic GmbH,Bremen,德國)自動標記。經MASCOT 2.2.04及Biotools 2.1軟體(Bruker Daltonic GmbH,Bremen,德國)進一步分析MS資料,且在NCBI非冗餘(NCBInr)資料庫中針對哺乳動物蛋白質檢索此等資料。以下參數用於資料庫檢索:單同位素質量準確性<250 ppm,母電荷+1,丟失的裂解1,半胱胺酸之胺甲醯胺基甲基化作為固定改質,甲硫胺酸之氧化作為變數改質。
(7e)液相層析-電噴霧電離(LC-ESI)聯合質譜(MS/MS)分析
使用直接耦合至HCT超ESI離子阱質譜儀(Bruker Daltonic GmbH,Bremen,德國)之毛細管HPLC進行奈米-LC MS/MS分析來自蛋白帶之經胰蛋白酶消化之肽。將肽
消化物溶解於0.1%甲酸/2%ACN中,隨後管柱注入。使用4%至90%之梯度(0.001%甲酸及0.001%甲酸之80%ACN溶液),用C18管柱(15 cm×75 nm,LC PACKINGS)來分離肽。在25℃下,流動速率為250 ng/min。將來自C18管柱之洗提液輸入HCT超ESI離子阱質譜儀中,在線性模型中操作用於線上分析。用137 V之噴霧電壓及160℃之加熱的毛細管溫度之奈米源(nanosource)使離子阱質譜儀最佳化。離子阱中肽離子之累積時間設定為200 ms,且選擇用於MS/MS分析之質荷比為100 Da至1800 Da,電荷狀態為1至3。
採用在VYDAC C4管柱上,在220 nm波長下監控之逆相HPLC分離在DBSF交聯的牛血紅素中發生在α鏈與β鏈之間的不同類型之交聯。使用用DBSF交聯之前及之後之牛血紅素得到的層析圖案顯示於圖9中。在圖9中,α鏈比天然牛血紅素之β鏈流動性更大(如虛線所示)。其一致性由MALDI-TOF分析確認。在與DBSF反應之後,β鏈為交聯的而大部分α鏈保持單獨的(如實線所示)。由於與DBSF交聯,形成了6個疏水性大於天然β鏈之主要血球蛋白峰。亦由15% SDS-PAGE解析DBSF交聯的牛血紅素中之交聯血球蛋白鏈,如圖10中所示。使主要交聯血球蛋白鏈(圖10中之B6)進行胰蛋白酶消化且隨後進行MALDI-TOF分析,且基於其肽質量指紋,鑑別為僅為β血球蛋白鏈,如圖11中所示。
雖然已根據各種具體實例描述前述發明,但該等具體
實例為非限制性的。許多變化及修改將為一般技術者所瞭解。該等變化及修改被視為包括在以下申請專利範圍之範疇內。

Claims (12)
- 一種用於製備經高度純化且熱穩定之含氧載體醫藥組成物的方法,該含氧載體醫藥組成物包括血紅素,該血紅素基本上由具有大於40%β -β 交聯之非聚合交聯四聚血紅素組成,該方法包含:a)提供至少包括紅血球及血漿之哺乳動物全血;b)將在該哺乳動物全血中之紅血球自血漿分離;c)過濾自血漿分離之紅血球以得到經過濾之紅血球部分;d)洗滌經過濾之紅血球部分以移除血漿蛋白雜質,產生經洗滌之紅血球;e)破壞經洗滌之紅血球以形成包含經破壞之紅血球之溶胞物的溶液;f)進行過濾以自該溶胞物移除至少一部分廢棄阻留物;g)自該溶胞物萃取第一血紅素溶液;h)進行至少一種純化程序以移除病毒、廢棄阻留物或蛋白質雜質之一或多者;i)藉由雙-3,5-二溴水楊基延胡索酸酯交聯該第一血紅素溶液以在充氧環境中形成交聯血紅素,其中該交聯血紅素為具有至少40% β-β交聯之非聚合交聯四聚血紅素;j)移除任何殘餘化學品;k)在去氧環境中熱處理該交聯血紅素以使任何殘餘不穩定/非交聯血紅素、任何二聚血紅素及任何其他蛋白質雜質變性並沈澱,以使得所得熱穩定交聯四聚血紅素具有不 可偵測之二聚體濃度且基本上由非聚合交聯四聚血紅素組成,該非聚合交聯四聚血紅素具有至少40%之β-β交聯且在實質上類似條件下量測的大於相同物種之天然血紅素之氧親和力的氧親和力;l)藉由離心或過濾移除沈澱物以形成澄清溶液;及m)添加經純化且熱穩定之交聯四聚血紅素至醫藥學上可接受之載體。
- 如申請專利範圍第1項之用於製備經高度純化且熱穩定之含氧載體醫藥組成物的方法,其中該熱處理步驟為在大約70℃至95℃下進行30秒至3小時,隨後立即冷卻且冷卻後立即以0.2%至0.4%之量添加N-乙醯基半胱胺酸的高溫短時(HTST)程序。
- 如申請專利範圍第1項之用於製備經高度純化且熱穩定之含氧載體醫藥組成物的方法,其中該全血為牛全血且該β-β交聯大於50%且p50值小於約23 mm Hg。

- 如申請專利範圍第5項之用於製備經高度純化且熱穩定之含氧載體醫藥組成物的方法,其中該層析管柱為 DEAE、CM及/或羥磷灰石管柱之一或多者。
- 如申請專利範圍第1項之用於製備經高度純化且熱穩定之含氧載體醫藥組成物的方法,其中該全血為人類血液。
- 如申請專利範圍第1項之用於製備經高度純化且熱穩定之含氧載體醫藥組成物的方法,其中該全血為豬血、馬血或犬血。
- 如申請專利範圍第1項之用於製備經高度純化且熱穩定之含氧載體醫藥組成物的方法,其進一步包含包裝該血紅素溶液於低透氧性包裝中以使得血紅素溶液具有約兩年之保存期限。
