臨床研究及實務顯示在接近組織部位提供減壓可增加並且促進在組織部位上新組織的生長。該現象之應用非常多,但減壓之應用在治療傷口上尤其成功。該治療(在醫學界中經常稱之為"負壓傷口治療"、"減壓治療"或"真空治療")提供許多優點,包括更快癒合並且增加肉芽組織形成。減壓一般係透過多孔墊或其他歧管器件施用至組織。多孔墊含有泡孔或孔,其能夠分配減壓至組織並且引導自組織排出之液體。多孔墊經常係併入至具有其他促進治療之組份的敷料中。
用於提供減壓治療之分配岐管通常亦被稱為減壓敷料,或在治療傷口的情況下,稱為傷口敷料。該等敷料的特徵在於允許流體流動通過材料的結構特徵。例如,一經常用作傷口敷料之材料係網狀開孔聚胺基甲酸酯發泡體。該發泡體包含複數個允許液體流動遍及發泡體的連通孔隙。當施加減壓至發泡體之一區域時,該減壓會很快分配至發泡體的其他區域並容易地傳輸至鄰近發泡體的組織。開孔發泡體及類似材料的一個問題是組織內生長(in-growth),其 會妨礙在治療後發泡體的容易移除。對於具有100-1000微米左右孔隙大小的開孔發泡體,組織之內生長會相當快地發生。隨著新組織進入發泡體之孔或泡孔,發泡體充作格柵,且組織在孔內及圍繞形成孔周邊的壁生長。其有效地將發泡體附於組織部位,而發泡體必須藉由撕裂新組織並且破壞任何已形成在組織與發泡體之間的鍵結而用力移除。此不僅不利於癒合過程,並且撕裂該組織會導致病人不舒適。
一種防止發生組織內生長問題的方法係增加更換敷料的頻率。若以增加頻率施用新敷料,則發生較少組織內生長,並且因此在除去舊敷料時新組織較少破裂。增加敷料更換的一項不利之處係與材料(即新敷料)以及人工有關之成本增加。更換敷料係勞力密集的並且會使醫務人員的注意力從其他重要任務轉移開。增加敷料更換亦會導致病人及其傷口更加惡化。
現有減壓治療系統所存在之問題藉由此處描述之說明性具體實施例的方法及系統獲得解決。在一具體實施例中,提供一種減壓治療系統,其包括分配岐管,該分配岐管包含背襯基材及安置於背襯基材第一側面上的複數個突出物,每一突出物具有大體上圓形的橫截面形狀並且具有大約在0.1至2.0毫米之間的直徑。背襯基材具有形成於其中的複數個孔,其允許流體在第一側面與相對於第一側面之第二側面之間流通。一減壓源可流動地連接至背襯基材的 孔以通過位在突出物之間的孔傳遞減壓,並且到達組織部位。

參閱圖1,根據本發明之一具體實施例之減壓治療系統11包含減壓敷料,或可流動地連結於減壓導管19的分配岐管15。減壓導管19可流動地連接至減壓源23比如真空泵或另一吸力源。分配岐管15係緊靠病人組織部位31安置並且用以分配由減壓源23所提供的減壓。一般,藉由安置不可滲透或半滲透性覆蓋物25於分配岐管15及組織部位31上而維持在組織部位的減壓。減壓亦用以自組織部位31排出傷口分泌液及其他流體。可將金屬容器27可流動地連接到減壓導管19並且設置在傷口敷料15和減壓源23之間以收集自組織部位31排出的流體。可將分配轉接器35連接到減壓導管19並且安置於分配岐管15上以幫助分配減壓至分配岐管15。
參閱圖2及圖3,分配岐管15尤其相當適合促進在組織部位31的組織生長但仍防止新組織內生長至分配岐管15內。分配岐管15包含具有組織接觸面43之背襯基材41。組織接觸面43較佳包含複數個凸出,或從背襯基材41延伸之突出物51。如圖3更具體地顯示,突出物51的形狀可大體上為圓柱形。或者,突出物51的橫截面形狀可係正方形、矩形、三角形、多邊形、橢圓形或任何其他形狀。突出物51可成錐形或具有完全均勻的橫截面積。
更明確地參閱圖3,突出物51的高度,H,在約0.1到5.0毫米之間較佳,約2毫米更佳。各突出物的寬度,W,係 在約0.1到2.0毫米之間,約0.25到0.5毫米更佳。在圖3中例示的突出物51的寬度等於其直徑,因各突出物51的橫截面形狀係圓形。若突出物51的橫截面形狀係正方形,則突出物51的寬度係正方形的邊長。對於其他橫截面形狀,寬度係通過橫截面形心C之最長橫向距離與通過橫截面形心之最短橫向距離的平均值。在各突出物51之間的橫向中心到中心的間隔E較佳係在約0.1到1.0毫米之間,約0.5毫米更佳。突出物51的間隔構成分配通路61,通過分配通路61,減壓可傳遞至組織部位31並且自組織部位31排出分泌液。突出物51的高度大於突出物51的寬度一般係較佳。更明確而言,高度與寬度的比率,H:W,應大於約1:1,並且大於約2:1更佳。
突出物51的形狀、尺寸及間隔可依據受治療的特定組織部位31、製造突出物51及背襯基材41的材料種類以及施用於組織部位15的減壓量而變化。例如,對於高度分泌之組織部位,最好將突出物設置地較分開,以於突出物51之間維持適當的分配通路61。在本發明之一具體實施例中,突出物51的形狀、尺寸及間隔對於一特定的分配岐管15為均勻的。在其他具體實施例中,突出物51的形狀、尺寸及間隔可變化。例如,可將具有不同橫截面形狀的突出物51配置在背襯基材41上。同樣地,突出物51的尺寸和間隔可變化以提供組織部位31的選定部分更多或更少的減壓。
突出物51的存在及尺寸允許突出物51分配減壓至組織部位31,但可防止在組織部位31生長的新組織附著於分配岐 管15的突出物51。藉由免除一般用以傳遞減壓至組織部位的孔或泡孔,新組織不會捲繞形成孔或泡孔的壁。雖然新組織生長將生長進入突出物51的領域中並且甚至可能捲繞一些突出物51,但因各突出物的底部係固定在背襯基材41上,故新組織無法將其本身固定至突出物51。
除分配減壓至組織部位31之外,分配岐管15亦可賦予組織部位31應力及應變,類似於該等可見於傳統上用於減壓系統中之多孔狀發泡體者。有時作為分配岐管用於減壓系統之其他材料,諸如紗布,對組織並不具有該作用。咸信由分配岐管15所產生之應力及應變會導致存在組織的微變形,且其在組織部位的新組織產生中扮演重要角色。賦予組織部位的應力及應變量係由供給至組織部位的減壓量以及接觸組織部位之歧管的表面形態所決定。隨著施加減壓,部分組織部位被拉引靠近分配岐管15,並且尤其靠近突出物51,其導致於組織內產生應力及應變。咸信規模類似於多孔狀發泡體之孔之突出物51的尺寸係產生與利用發泡體所見者類似之應力及應變的一個理由。
在一具體實施例中,背襯基材41係由與突出物51相同的材料形成。該材料較佳係聚矽氧或另一對於流體流動相對不能滲透的醫療級材料。或者,該材料可係允許選定流體或流體量通過的半透性材料。背襯基材41可包含複數個孔71,其允許從與突出物51相背對之背襯基材41的表面分配至突出物51自其中延伸的組織接觸面43。因為孔71之存在可對組織內生長具有與孔相同之作用,因此在任何新組織 前進入孔71內之前自組織部位31除去背襯基材41及突出物51很重要。實際上,其可藉由知曉組織生長的大致速率、突出物51的高度以及測定新組織生長達到孔71大致需要的時間來完成。
雖然分配岐管15主要經描述為包含背襯基材41及複數個突出物51,但分配岐管15可進一步包含多孔狀發泡體或另一安置於接近或附著於與突出物51相背對之背襯基材41之表面的材料。使用多孔狀發泡體或其他材料增加減壓導管19或分配轉接器35傳遞並分配減壓至背襯基材41的能力。突出物51及背襯基材41作為障礙物阻止新組織生長進入多孔狀發泡體或其他材料的孔中。
參閱圖4及圖5,其說明根據本發明之另一具體實施例的分配岐管115。替代複數個諸如分配岐管15之突出物的突出物,分配岐管115包含複數個柱形空隙151,其係經形成或以其他方式安置於背襯基材141內。術語柱形並不意指任何特定的橫截面形狀,因為空隙的形狀可係如先前關於突出物51所描述之任何形狀。更確切地,術語柱形係指長度通常大於寬度的空隙。空隙151自身構成複數個分配通路161,其可由位在與組織部位相背對之分配通路161之一端的主通路結合。或者,分配通路161可簡單地係完全穿過背襯基材141的孔。
空隙151的形狀及大小可類似於歧管15之突出物51。如之前描述,可將多孔狀發泡體、分配轉接器或其他歧管器件設置成與分配通路161流體相通以傳遞減壓至組織部 位。
具有突出物之示範性分配岐管
參閱圖6及圖7,已經測試且展現與多孔狀發泡體類似之生長誘導速率的一特定分配岐管215利用2英寸直徑之背襯基材241。背襯基材包含複數個突出物251,其之橫截面形狀大致係圓形並且自該等突出物與背襯基材241接觸的底部向內成錐形。於背襯基材241中提供複數個孔271以允許與位在突出物251之間的分配通路261流體連通。孔271係以配置於突出物251之行列之間的行列配置。該類型的孔271的設置導致一個孔271正中位在每四個以方形圖案設置的相鄰突出物251之間(參看圖6)。
突出物215的尺寸係使得在2英寸直徑之背襯基材241上,大約存在7500個突出物。各突出物底部的寬度係約0.5 mm,各突出物的高度係約1.5 mm,並且突出物之間的橫向中心到中心的間隔係約0.75 mm。突出物的高度與寬度的比係約3:1,並且間隔與寬度的比係約1.5:1。各突出物51的錐度係距突出物51縱軸大約5度的拔模角,以有助於分配岐管215之成型。
分配岐管215係依據預期組織生長速率以及在更換分配岐管215之間的期望使用期間定尺寸。對於大約125 mm Hg的減壓,在48小時期間內可預期1至2毫米之組織生長。因為希望每隔48小時更換分配岐管215,大約1.5 mm高度的突出物允許大多數突出物251之間的間隔在更換敷料之間經填充新組織生長,但防止組織附著於分配岐管251。
更明確地參閱圖7,其提供藉由顯微鏡獲得的分配岐管215的相片的例示複製。該相片例示分配岐管215的俯視圖,其顯示突出物251從背襯基材241延伸出。亦例示配置在突出物251之間的孔271。

Claims (20)
