Приобретенная атрезия наружного слухового прохода может развиваться как вследствие течения воспалительного процесса, так и ранее проведенного хирургического вмешательства, затрагивающего ткани наружного слухового прохода и барабанную перепонку [1, 2, 3]. И если распространенность поствоспалительной атрезии мала (0,5‰) [4], то риск ее развития после такого рода операций высок. Хирургическое вмешательство проходит под эндотрахеальным наркозом. Разрез производится позади ушной раковины, отступя 1 см от заушной складки. Отсепаровываются мягкие ткани. Бором снимается гребешок Henle и кортикальная часть височной кости, сглаживается задняя стенка НСП для лучшего обзора. Отсепаровывается кожа наружного слухового прохода, начиная от задней костной стенки до атретических тканей и по их границе до передней костной стенки, формируя кожный «мешок». Затем отсепаровывается фиброзная ткань атрезии вглубь до фиброзного кольца барабанной перепонки, а затем и от самой барабанной перепонки, используя в качестве ориентира рукоятку молоточка, которая анатомически спаяна с тканями барабанной перепонки. Далее фиброзная ткань атрезии удаляется. Заушный разрез ушивается послойно. Сформированный наружный слуховой проход тампонируется нерассасывающимися асептическими материалами с добавлением антисептика. Накладывается асептическая повязка. Then, the burs expand the bony canal of the external auditory canal by removing the bone mass of the anterior part of the bone wall and partially the lower and upper ones in such a way as to smooth the anterior meatotympanic angle. Orientation is carried out according to computed tomography of the temporal bone in order to avoid injury to the temporomandibular joint. The separated skin “bag” is dissected with a U-shaped incision on the side facing the back wall, stepping a few millimeters up from its bottom so that the front (large) flap is formed, which is laid on the front bone wall, lowering to the eardrum, and back - respectively on the back bone wall. The skin deficiency, if any, is made up by laying down the displaced free thinned skin grafts taken from the behind-the-ear area along the edge of the incision or from the extremity (inner surface of the shoulder, thigh). The behind-the-ear incision is sutured in layers. The formed external auditory canal is plugged with non-absorbable aseptic materials with the addition of an antiseptic. An aseptic dressing is applied. For the prevention of bacterial infections, antibiotic therapy is prescribed for 7 days.

The anterior meatotympanal angle is narrow. In accordance with the data of computed tomography of the temporal bone, the canopy removed the canopy of the front wall of the external auditory meatus by about 5-6 mm in the protruding part and slightly upper and lower bone walls. After the extension of the external auditory meatus, the anterior tympanal angle approaches a straight line. The epidermal "bag" is dissected with the formation of the anterior and posterior flaps. For the prevention of bacterial infections, antibiotic therapy is prescribed for 8 days.

В результате применения заявляемого способа получены хорошие функциональные результаты, костно-воздушный интервал в послеоперационном периоде составлял в среднем от 10 до 15дБ. Повторного развития стеноза или атрезии слухового прохода не произошло.As a result of the application of the proposed method, good functional results were obtained, the bone-air interval in the postoperative period averaged from 10 to 15 dB. Нами прооперировано 6 пациентов с приобретенной атрезией наружного слухового прохода с расширением костной части наружного слухового прохода, со сглаживанием переднего меатотимпанального угла и формированием кожных лоскутов «на ножке» из собственной ткани наружного слухового прохода. В послеоперационном периоде случаев рестенозирования не наблюдалось.We operated on 6 patients with acquired atresia of the external auditory meatus with the expansion of the bony part of the external auditory meatus, with smoothing of the anterior meatotympanic angle and the formation of skin flaps “on the leg” from our own tissue of the external auditory meatus.
