Abstract
Description
The invention relates to medicine, namely to otorhinolaryngology, and can be used when performing sanitizing operations in patients with congenital cholesteatoma located in the anterior mesotympanum.
Congenital cholesteatoma of the middle ear is considered to be a pearly-white neoplasm (epidermal cyst), located medially to the intact tympanic membrane with intact stretched and loose parts of it, in the absence of a patient's history of otorrhea, perforation of the eardrum, previous otological procedures, atresia of the external auditory canal, intramembranous and giant cholesteatoma.
In this case, a closed form of congenital cholesteatoma is more often observed - the formation has a spherical shape (“pearl”) and a well-defined capsule, represented by the cholesteatoma perimatrix. When this cholesteatoma is removed without violating the integrity of the capsule, the risk of recurrence of the disease is greatly reduced.
When diagnosed early, congenital cholesteatoma is usually small and does not cause carious changes in the ossicular chain. In this case, the violation of the auditory function is absent or minimally expressed.
The most common congenital cholesteatoma of the middle ear is localized in the anterior-upper part of the mesatympanum, adjacent to the medial surface of the tympanic membrane. This area is difficult to visualize due to the frequent anterior curvature of the external auditory canal, which makes it difficult to visualize the anterior meatotympanic angle.
The tympanic membrane consists of three layers, epidermal, fibrous and mucous. The fibrous layer is the most dense and is represented by two rows of fibers: external radial and internal circular. Radial fibers are stretched between the bony tympanic ring and the handle of the malleus. They are woven into the periosteum of the malleus in the umbo region, which causes the greatest fixation of the tympanic membrane in this place.
With complete separation of the tympanic membrane and malleus during the operation, in the postoperative period, rotation of the handle of the malleus medially (medialization) under the action of the force of the muscle straining the tympanic membrane can be observed. This leads to impaired sound conduction from the tympanic membrane to the ossicular chain, and as a result, conductive hearing loss.
Surgical intervention for the rehabilitation of congenital cholesteatoma of the anterior mesotympanum should be aimed at its complete removal, while maintaining the integrity of the capsule to reduce the risk of recurrence of the disease, and at the same time be minimally traumatic for the patient, and able to preserve the anatomical integrity of the structures of the middle ear to exclude hearing impairment.
Despite the existing possibilities of using modern surgical equipment and diagnostic research methods, the risk of developing residual and/or recurrent middle ear cholesteatoma in patients with congenital middle ear cholesteatoma, as well as hearing loss after sanitizing operations to remove it, remains relevant.
М. Tos, Manual of middle ear surgery, 2004, Vol. 1, P. 81-82).A known method of surgical access to the anterior sections of the mesotympanum, including the formation of the meatotympanic flap, opening the tympanic cavity, removing the anterior sections of the bone tympanic ring (see M. Tos, Manual of middle ear surgery, 2004, Vol. 1, P. 81-82).
When implementing this method, the following actions are performed.
The method is performed endaurally. Rosen's knife is used to make an arcuate incision of the skin of the anterior wall of the external auditory canal from 12 to 6 hours 5.0 mm lateral to the annulus fibrosus. Then the tympanomeatal flap is separated, the mucous membrane is dissected, after which the flap is folded back. To improve the visualization of the anterior parts of the tympanic cavity, the anterior parts of the bone tympanic ring are removed with a bur.
The disadvantages of this method are:
- трудность при манипуляции в передних отделах наружного слухового прохода, особенно при наличии выраженного его изгиба кпереди, который затрудняет детализацию передних отделов барабанной перепонки;- difficulty in manipulation in the anterior sections of the external auditory canal, especially in the presence of its pronounced anterior bend, which makes it difficult to detail the anterior sections of the tympanic membrane;
- ограничение интрооперационной визуализации может привести к нарушению целостности барабанной перепонки или капсулы холестеатомы, что в свою очередь увеличивает риск рецидива заболевания;- restriction of intraoperative visualization can lead to disruption of the integrity of the tympanic membrane or cholesteatoma capsule, which in turn increases the risk of recurrence of the disease;
- способ исключает возможность проведения ревизии задних отделов барабанной полости, и, следовательно, цепи слуховых косточек, что не позволяет, при необходимости, выполнение оссикулопластики.- the method eliminates the possibility of revision of the posterior parts of the tympanic cavity, and, consequently, the ossicular chain, which does not allow, if necessary, performing ossiculoplasty.
R. McCabe, D.J. Lee, М. Fina. The Endoscopic Management of Congenital Cholesteatoma, Otolaryngol Clin North Am., 2021, 54(1): 111-123).There is also known a method for endoscopic transcanal surgical debridement of congenital cholesteatoma of the anterior tympanic cavity, including the formation of a meatotympanic flap, opening of the tympanic cavity, resection of congenital cholesteatoma of the anterior mesotympanum, revision of the tympanic cavity with a rigid angled endoscope (see R. McCabe, D.J. Lee, M. Fina. The Endoscopic Management of Congenital Cholesteatoma, Otolaryngol Clin North Am., 2021, 54(1): 111-123).
This method is performed endourally under the control of a rigid endoscope with 0°, 30°, 45° viewing angles. With a round knife, an arcuate incision is made in the skin of the anterior wall of the external auditory canal from 12 to 6 hours 6.0 mm lateral to the annulus fibrosus. Then, the meatotympanic flap is separated with exposure of the anterior part of the bone tympanic ring and then with the transition to the handle of the malleus, the tympanic cavity is opened, the congenital cholesteatoma is removed, then a control revision of the tympanic cavity is performed.
The disadvantages of this method are: - the possibility of manipulation with only one hand
- использование достаточно больших по диаметру (3,0 мм) эндоскопов, что затрудняет работу в ограниченном пространстве, а использованию техники меньшего диаметра мешает ее быстрое запотевание;- the use of sufficiently large diameter (3.0 mm) endoscopes, which makes it difficult to work in a limited space, and the use of equipment of a smaller diameter is hindered by its rapid fogging;
- при наличии даже незначительного кровотечения использование эндоскопов становится затруднительным, а порою практически невозможным, при этом возникает необходимость к переходу на традиционную отомикроскопическую технику, что удлиняет время проведения операции.- in the presence of even minor bleeding, the use of endoscopes becomes difficult, and sometimes almost impossible, while there is a need to switch to traditional otomicroscopic technique, which lengthens the time of the operation.
Robert K. Jackler, Ear Surgery Illustrated: A Comprehensive Atlas of Otologic Microsurgical Techniques, 2019, P. 373-375).The closest in technical essence to the claimed solution is a method of surgical sanitation of congenital cholesteatoma of the anterior tympanic cavity, including the formation of a meatotympanic flap, opening of the tympanic cavity, separation of the tympanic membrane together with the periosteum from the handle of the malleus, resection of congenital cholesteatoma of the anterior mesotympanum (see Robert K. Jackler , Ear Surgery Illustrated: A Comprehensive Atlas of Otologic Microsurgical Techniques, 2019, P. 373-375).
This access is carried out endaurally, with a Rosen knife an arcuate incision is made in the skin of the posterior wall of the external auditory canal from 12 to 6 hours, the tympanic cavity is opened, the tympanomeatol flap is separated up to the handle of the malleus, a longitudinal incision of the periosteum of the malleus is made with a sickle-shaped knife. The tympanic membrane is separated from the handle of the malleus together with the periosteum and completely separated from it, the tympanomeatal flap is displaced anteriorly, congenital cholesteatoma of the anterior tympanic cavity is removed. After that, the tympanomeatal flap is lowered to its original place. To exclude medialization of the malleus with the help of an operating laser, myringotomy is performed in the umbo area, followed by fixation of the distal malleus handle in the formed perforation.
The disadvantages of this method are:
- ограничение интраоперационнонной визуализации передних отделов барабанной полости при наличии изогнутого костного отдела наружного слухового прохода;- restriction of intraoperative visualization of the anterior parts of the tympanic cavity in the presence of a curved bone section of the external auditory canal;
- несмотря на уменьшение риска повреждения барабанной перепонки вследствие совместной ее отсепаровки с надкостницей молоточка, отделение ее в области umbo без формирования перфорации достаточно затруднительно, вследствие наибольшего контакта волокон фиброзного слоя в этом месте;- despite the reduction in the risk of damage to the tympanic membrane due to its joint separation with the periosteum of the malleus, its separation in the umbo area without the formation of perforation is rather difficult, due to the greatest contact of the fibers of the fibrous layer in this place;
- формирование хирургическим лазером перфорации барабанной перепонки в области umbo с последующей фиксацией в ней рукоятки молоточка может привести к дальнейшему нарушению целостности заведомо интактной барабанной перепонки с развитием хронического среднего отита и кондуктивной тугоухости;- the formation of a perforation of the tympanic membrane in the umbo region by a surgical laser, followed by fixation of the handle of the malleus in it, can lead to further disruption of the integrity of the obviously intact tympanic membrane with the development of chronic otitis media and conductive hearing loss;
- способ не позволяет визуализировать глубокие передне - верхние отделы барабанной полости, что в свою очередь увеличивает риск рецидива холестеатомы.- the method does not allow to visualize the deep anterior - upper parts of the tympanic cavity, which in turn increases the risk of cholesteatoma recurrence.
The technical result of the proposed solution is to reduce the risk of developing residual and/or recurrent middle ear cholesteatoma in patients with congenital anterior mesotympanum cholesteatoma by maintaining the anatomical integrity of the middle ear structures and the patient's hearing.
To achieve this technical result in the method of surgical sanitation of congenital cholesteatoma of the anterior mesotympanum, including access by a behind-the-ear approach, formation of a meatotympanic flap, opening of the tympanic cavity, separation of the tympanic membrane from the handle of the malleus, starting from the short process of the malleus, from top to bottom, after which the capsule of congenital cholesteatoma is separated from medial surface of the tympanic membrane, then resection of the congenital cholesteatoma is performed, maintaining the integrity of its capsule, after which the meatotympanic flap is placed on the bone tympanic ring, the external auditory meatus is plugged, the behind-the-ear wound is sutured in layers, according to the invention, when the meatotympanic flap is formed, the skin of the external auditory canal is separated from back to front from 8 to 4 hours, then cutting burs with a diameter of 4.0 mm smooth the bone of the distal posterior wall of the external auditory canal until the anterior meatotympanic angle is fully visualized, and separation of the tympanic membrane is carried out with maintaining contact in the umbo area, while after separation of the capsule of the congenital cholesteatoma, the meatotympanic flap is folded down, then the anterior malleus fold is dissected with a microneedle, the anterior-upper section of the bone tympanic ring is removed with a drill until the capsule of the congenital cholesteatoma is fully visualized.
45°.Also, according to the invention, after resection of congenital cholesteatoma, a control revision of the tympanic cavity is performed with a
Performing such actions as: smoothing the bone of the posterior wall of the external auditory canal in its distal part, dissection of the anterior malleus fold, removal of the anterior-upper part of the bone tympanic ring allows you to fully visualize the anterior parts of the mesotympanum for further sanitation of congenital cholesteatoma, which reduces the risk of preserving epidermal tissue in the tympanic cavities.
Separation of the tympanic membrane from the malleus handle together with the perichondrium while maintaining contact in the umbo area reduces the risk of violating the integrity of the tympanic membrane, since the tight fixation of the radiant fibers of the fibrous layer to the malleus handle in this area makes it technically difficult to separate them without forming a defect. It also excludes the medialization of the handle of the malleus with a subsequent decrease in auditory function in the postoperative period.
The use of rigid endoscopes with an angle view for the control revision of the tympanic cavity makes it possible to visualize hard-to-see parts of the tympanic cavity and thereby reduce the likelihood of preserving the epidermal tissue with the subsequent development of residual cholesteatoma.

The method is carried out as follows.
The method is carried out under conditions of multicomponent anesthesia. Perform behind the ear incision from the level of 1. temporalis to the top of the mastoid process. Soft tissues are separated to visualize the site of the mastoid process, spina Henle and the posterior wall of the bone section of the external auditory canal.
Under the control of the operating microscope, a meatotympanic flap is formed. The skin of the external auditory canal is separated from behind to the front from 8 to 4 hours.
A cutting bur with a diameter of 4.0 mm is used to smooth the bone of the distal posterior wall of the external auditory canal until the anterior meatotympanic angle is fully visualized.
Open the tympanic cavity. The
The
45°.A control revision of the tympanic cavity is performed with a
The operation is continued in the usual way: the meatotympanic flap is placed on the bone tympanic ring. The external auditory meatus is tamponed with a silk thread. The ear wound is sutured in layers. Apply an aseptic bandage.
The method is illustrated by the following example.
Patient K., 6 years old, was admitted to the pediatric surgical department of the St. Petersburg Research Institute of Ear, Throat, Nose and Speech with a diagnosis of right-sided chronic otitis media, without impaired auditory function. Congenital cholesteatoma of the tympanic cavity on the right.
On admission, the patient had no complaints. Congenital cholesteatoma was discovered during a routine examination by an otorhinolaryngologist before entering school. From the anamnesis it is known that during the life of the child there was no discharge from the ears, injuries of the eardrum, otosurgical procedures.
Tonal audiometry - no hearing impairment was detected. Computed tomography (hereinafter - CT) of the temporal bones: AD - pneumatic structure of the mastoid process. The bone bed of the sigmoid sinus, the upper face of the pyramid, the bone capsule of the labyrinth without destruction. The chain of auditory ossicles without signs of carious changes. In the anterior part of the tympanic cavity, a soft tissue formation of a rounded shape is determined, adjacent to the handle of the malleus, the medial surface of the tympanic membrane. Middle ear MRI in DWI mode "cholesteatoma protocol" - the revealed changes in the projection of the tympanic cavity on the right probably correspond to cholesteatoma masses.
The patient underwent surgery on the right ear.
Surgical intervention was performed under conditions of multicomponent anesthesia with tracheal intubation.
An incision was made in the right ear region. The skin of the posterior and upper walls of the external auditory canal was cut off to the annulus fibrosus. A cutting bur with a diameter of 4.0 mm smoothed the bone of the distal posterior wall of the external auditory canal.
A tympanotomy was performed. The eardrum was separated with a microraspator from 8 hours to 4 hours before visualization of the short process of the malleus. Further, the tympanic membrane is separated from the handle of the malleus from top to bottom to umbo with simultaneous separation of the skin of the anterior wall of the external auditory canal up to 4 hours.
A congenital cholesteatoma was visualized in a dense capsule adjacent posteriorly to the handle of the malleus and anteriorly covered by a canopy of the anterior part of the bone tympanic ring. The anterior malleus fold was dissected. Boron with a diameter of 1.0 mm removed the anterior-upper sections of the bone tympanic ring until complete visualization of congenital cholesteatoma.
The capsule of congenital cholesteatoma is separated from the handle of the malleus with a microneedle. The cholesteatoma was removed from the tympanic cavity without violating the integrity of the capsule.

During the control revision of the tympanic cavity with a
The removed materials were sent for histological examination. No complications were observed in the postoperative period. The sutures were removed on the seventh day after the operation, tampons from the external auditory canal on the 10th day.
The patient was discharged on the 14th day after the operation. Otomicroscopy at discharge AD: the external auditory canal is wide and free. The tympanic membrane is pink, moderately edematous, no defects. According to the control tone audiometry at discharge, no hearing impairment was detected. According to the results of histological examination, the picture corresponded to cholesteatoma.
Control examination after 6 months. AD: external auditory canal is wide and free. The tympanic membrane is gray, no defects. There are no signs of cholesteatoma recurrence. A year later, a CT scan of the temporal bones and an MRI of the middle ear were performed in the DWI mode "cholesteatoma protocol": no evidence of cholesteatoma recurrence was found in the patient.
Thus, this method made it possible to reduce the risk of developing residual and recurrent cholesteatoma of the middle ear during surgical debridement of anterior mesotympanum cholesteatoma and at the same time preserve the anatomical integrity of the middle ear structures and the patient's hearing.
