Наиболее близким к заявляемому способу является способ лечения диастематомиелии (Meacham W.F. Surgical treatment of diastematomyelia // J. Neurosurg. 1967. Vol 27. P.78-85), принятый за прототип. Срединным разрезом осуществляют доступ к остистым отросткам и дугам позвонков на один или два сегмента выше и ниже уровня поражения. Паравертебральные мышцы отсепаровывают и смещают латерально с обеих сторон. Остистый отросток и дугу позвонка на уровне расположения костного шипа резецируют, визуализируют разделенную костным шипом твердую мозговую оболочку. Указанный технический результат при осуществлении изобретения достигается тем, что в известном способе лечения диастематомиелии, включающем ламинэктомию, отделение костного шипа от твердой мозговой оболочки, резекцию костного шипа, устранение интрадуральных сращений, вскрытие и восстановление целостности твердой мозговой оболочки, особенность заключается в том, что при вскрытии задней стенки позвоночного канала единым блоком спиливают фрагменты медиальных отделов дуг и остистые отростки позвонков, пересекают место соединения костного шипа и передней поверхности дуги позвонка, после чего единым блоком фрагменты дужек и остистых отростков приподнимают, а после удаления костного шипа этот фрагмент фиксируют на место, предварительно обработав высокооборотным бором переднюю поверхность дужки позвонка на уровне удаления костного шипа. Способ осуществляется следующим образом. После разреза кожи и мягких тканей, скелетирования дужек и остистых отростков на расстоянии 1-1,5 см от срединной линии, кусачками Листона и высокооборотным бором резецируют дуги на одном уровне выше и ниже диастематомиелии, рассекают надостистую, межостистые связки на одном уровне ниже диастематомиелии, на 1,5-2,0 см приподнимают каудальный край фрагмента дужек и остистых отростков, кусачками Листона и скальпелем рассекают костный шип по переднему краю дуги, таким образом отделяют костный шип от фрагмента дужек и остистых отростков. Резецированный фрагмент дуг и остистых отростков поднимают на 80-90 градусов вверх. После чего бором и кусачками удаляют костный шип диастематомиелии. Рассекают спайки, зашивают твердую мозговую оболочку спереди и сзади от спинного мозга. Таким образом восстанавливают герметичность дурального мешка. Пальпаторно ревизуют переднюю поверхность дужки позвонка на уровне устранения диастематомиелии, производят ее обработку высокооборотным бором. Таким образом устраняют возможную компрессия дурального мешка деформированными структурами заднего опорного комплекса при восстановлении целостности позвоночного канала. Фрагмент дужек и остистых отростков фиксируют к остистому отростку нижележащего позвонка и паравертебральным мышцам лигатурными швами, производят послойное ушивание операционной раны.The method is as follows. В результате применения данного способа была устранена диастематомиелия, фиксация спинного мозга, за счет коррекции и восстановления задней стенки позвоночного канала сохранена целостность позвоночного канала, предотвращена возможная нестабильность позвоночника, повторная фиксация спинного мозга. Больная П., 9 лет, ИБ №2802, находилась на лечении с диагнозом: врожденный порок развития позвоночника и спинного мозга, диастематомиелия на уровне L2 позвонка, субдуральная липома на уровне L2-L3, синдром фиксированного спинного мозга. Проведена операция: ламинэктомия на уровне Th12-L3 позвонков, устранение диастематомиелии, фиксации спинного мозга, пластика твердой мозговой оболочки. Положение лежа на животе. Разрез в поясничной области срединный на уровне остистых отростков Th12-L3 позвонков. Скелетированы дуги и остистые отростки. Медиальные отделы дужек и остистые отростки L1-L3 деформированы, рубцово изменены, имеет место резкое углубление борозды на уровне L1-L3 и правосторонний сколиоз. Единым блоком подняты медиальные отделы дужек, остистые отростки и задняя продольная связка Th12-L3 позвонков, при этом на передней поверхности дужки L2 позвонка рассечен костный шип, длиной 8-9 мм, шириной 2-3 мм. Эпидуральная клетчатка на уровне L1-L3 отсутствует, на уровне Th12 слабо выражена. На уровне L2 костно-хрящевым шипом (размеры: сагиттальный - 10-15 мм, поперечный - 2-3 мм) дуральный мешок разделен на две части, при этом левая половина несколько шире правой. Твердая оболочка вскрывалась вертикальным ближе к срединной сагиттальной плоскости разрезом. Удалена субдуральная липома на уровне L2-L3. На уровне L1-L2 спинного мозга костно-хрящевой шип длиной 15-20 мм разделяет спинной мозг на два канатика. При этом левая часть несколько шире правой. Оба канатика покрыты твердой оболочкой, отделяющей их от костного шипа. Костный шип кусачками и высокооборотным бором резецирован, имеет плотную поверхностную структуру и ячеистую внутреннюю часть. Основание шипа на задней поверхности тела L1 позвонка имеет овальную форму, размерами 22×4 мм. Расположено на 2-3 мм правее от средней линии (фиг.1). После удаления шипа твердая оболочка на передней стенке дурального мешка зашивалась узловыми швами 5-0. Передняя поверхность спинного мозга множественными грубыми рубцами припаяна к ТМО. Корешки спинного мозга имеют горизонтальный ход, истончены, ишемизированы, деформированы в грубых рубцах. Канатики спинного мозга на уровне диастематомиелии утолщены, имеют плотную консистенцию, покрыты утолщенной мягкой оболочкой, бедные сосудистой сетью. Ниже уровня диастематомиелии оба канатика соединяются на уровне середины тела L3. На этом уровне спинной мозг уплощен, бледный. Нижний край спинного мозга расположен на уровне L5-S1 позвонков. После миелорадикулолиза и устранения фиксации спинной мозг спонтанно сместился вертикально на 2-3 см, корешки приобрели нисходящий ход. Гемостаз. Пластика ТМО искусственной оболочкой. Произведена ревизия, обработка высокооборотным бором передней поверхности дуги L2 позвонка. Фрагменты дужек и остистых отростков уложены на место, фиксированы узловым швами (фиг.2). The lower edge of the spinal cord is located at the level of L5-S1 vertebrae.
