Изобретение относится к медицине, где при повреждениях и различных поражениях позвоночника и спинного мозга применяются оперативные вмешательства с вскрытием позвоночного канала. Известны оперативные доступы к спинному мозгу при сколиозах III-IV степени со сдавлением спинного мозга с параличами или парезами нижних конечностей и нарушением функций тазовых органов. При этом производится расширенная ламинэктомия с целью декомпрессии, а стабилизация достигается посредством костнопластической фиксации одним из существующих способов [1, 5, 9, 18, 19] . После травмы позвоночника с компрессией спинного мозга костными отломками, либо сместившимися элементами позвонков прибегают к ламинэктомии, убирают сдавливающие спинной мозг факторы и после декомпрессии аутотрансплантатами закрывается дефект стенки позвоночного канала [2, 4, 10, 12, 21, 28, 30] . При закрытых повреждениях позвоночника с кровоизлиянием в позвоночный канал и сдавлением спинного мозга также осуществляется удаление остистых отростков, дужек и связок на этом уровне, рассечение зубчатых связок и ревизия эпидурального пространства [4, 5, 10, 13, 25, 27-30]. При опухолях спинного мозга, его оболочек или элементов стенок позвоночного канала методом выбора является расширенная ламинэктомия на уровне патологии [6, 8, 27, 29] . У больных, страдающих спондилолистезом III-IV степени, сдавлением корешков спинного мозга и парезами нижних конечностей, производится ламинэктомия с костнопластической фиксацией данного отдела позвоночника [7, 27, 29] . Туберкулезный спондилит сопровождается разрушением тел ряда позвонков, постепенным нарастанием кифоза и сдавлением спинного мозга на высоте гиббуса с парезами или параличами нижних конечностей и нарушением функций тазовых органов. С целью хирургического устранения этих тяжелых осложнений производятся расширенная ламинэктомия, ревизия позвоночного канала и костнопластическая фиксация позвоночника по типу Генле-Уайтмена, при котором обширный дефект задней стенки канала прикрывается аутотрансплантатами, захватывающими выше- и нижележащие позвонки [13, 15-17, 20-26]. В тех случаях, когда в позвоночный канал проникают гнойно-некротические массы, секвестры из разрушенных тел позвонков, либо грануляционная ткань у ослабленных больных, ламинэктомия позволяет устранить сдавление спинного мозга и нарушение функций тазовых органов [3, 11, 14-17, 20, 22]. Образующийся дефект задней стенки позвоночного канала также закрывают аутотрансплантатами. При этом задняя поверхность спинного мозга остается открытой. У таких больных нередко производится резекция пораженных позвонков с гемиламинэктомией [3, 10-15, 19, 20]. Такая операция завершается костнопластической фиксацией аутогомотрансплантатами. Расширенная ламинэктомия считается рискованной операцией в связи с нарушением стабильности данного отдела позвоночника [19, 26], тем не менее используется с целью декомпрессии спинного мозга. При выраженном кифозе грудо-поясничного отдела позвоночника прибегают к корригирующим операциям, для чего сначала производится ламинэктомия на этом уровне, рассекают зубчатую связку, спинной мозг смещают в сторону и широким долотом иссекают клиновидный участок, либо предпринимается вертебротомия на уровне межпозвонковых дисков с последующей экстензией и костнопластической фиксацией данного отдела позвоночника аутотрансплантатами [27]. In patients suffering from grade III-IV spondylolisthesis, compression of the roots of the spinal cord and paresis of the lower extremities, a laminectomy is performed with osteoplastic fixation of this spine [7, 27, 29]. Tuberculous spondylitis is accompanied by the destruction of the bodies of a number of vertebrae, a gradual increase in kyphosis and compression of the spinal cord at the height of the hibus with paresis or paralysis of the lower extremities and a violation of the functions of the pelvic organs. In order to surgically correct these serious complications, an extended laminectomy, revision of the spinal canal and osteoplastic fixation of the spine according to the Henle-Whiteman type are performed, in which an extensive defect in the posterior wall of the canal is covered by autografts that capture the upper and lower vertebrae [13, 15-17, 20-26 ]. In cases where purulent necrotic masses, sequestra from destroyed vertebral bodies, or granulation tissue in weakened patients penetrate the spinal canal, laminectomy allows eliminating spinal cord compression and impaired pelvic organs [3, 11, 14-17, 20, 22 ]. The resulting defect in the posterior wall of the spinal canal is also closed with autografts. In this case, the posterior surface of the spinal cord remains open. In such patients, resection of the affected vertebrae with hemilaminectomy is often performed [3, 10-15, 19, 20]. Such an operation ends with osteoplastic fixation with autograft grafts. An extended laminectomy is considered a risky operation due to a violation of the stability of this spine [19, 26], but nevertheless it is used to decompress the spinal cord. With severe kyphosis of the thoracolumbar spine, corrective operations are performed, for which a laminectomy is first performed at this level, the dentate ligament is dissected, the spinal cord is displaced to the side and the wedge-shaped section is excised, or vertebrotomy is performed at the level of intervertebral discs with subsequent extension and bone extension and fixation of this spine with autografts [27].

Недостатками существующего способа являются: высокая травматичность оперативного вмешательства связанная с разрушением и удалением задней стенки позвоночного канала, нарушение стабильности данного отдела позвоночника посредством удаления блока, состоящего из дужек, остистых отростков, надостистой, межостистых и желтых связок человека, которые не восстанавливаются после выполнения операции на спинном мозге, необходимость нанесения дополнительной травмы во время взятия аутотрансплантатов из ребер или большеберцовой кости, нарастание кифоза во время перестройки аутогомотрансплантатов из-за отсутствия надостистой и других связок в оперированном отделе позвоночника, наступление полного обездвиживания данного отдела позвоночника, отсутствие задней стенки позвоночного канала ухудшает морфо-функциональные особенности спинного мозга на этом уровне, разрастание грубоволокнистой ткани на месте дефекта сопровождается срастанием ее с твердой мозговой оболочкой, вызывая дополнительные раздражения, перетяжки и деформации. Отсутствие задней стенки позвоночного канала и костнопластическая фиксация отдела позвоночника инвалидизирует оперированного больного, не говоря о местах взятия аутотрансплантантов. Целью настоящего изобретения является снижение травматичности и повышение физиологичности оперативного доступа к спинному мозгу, сокращение периода нетрудоспособности и более полное восстановление морфо-функционального статуса оперированного отдела позвоночника, не прибегая к взятию аутотрансплантатов и костнопластической фиксации позвоночника человека. Указанная цель достигается тем, что после продольного рассечения кожи, фасциально-клетчаточных слоев, отсечения прикрепления мышц от вершин остистых отростков и поднадкостничного выделения остистых отростков и дужек продольно рассекают желтые связки с обеих сторон и в косо-фронтальной плоскости пересекают дужки возле суставных отростков, мобилизуя заднюю стенку позвоночного канала. По показанию, мобилизованную заднюю стенку пересекают по ходу межостистых связок внизу раны с образованием П-образного костно-мышечно-связочного лоскута с основанием вверху, либо заднюю стенку пересекают вверху с выкраиванием П-образного лоскута с основанием внизу, а в ряде случаев - пересекают по середине с образованием верхнего и нижнего лоскутов, имеющих свои основания. Они более физиологичны, чем предыдущие. По показанию заднюю стенку позвоночного канала мобилизуют С-образным разрезом, формируя единый костно-мышечно-связочный лоскут с широким мягкотканным основанием с одной стороны и отворачивают его кнаружи. При этом создаются более благоприятные условия для расширения доступа в сторону - по показанию в каждом конкретном случае. После выполнения запланированной операции на спинном мозге образовавшиеся лоскуты укладывают на место и фиксируют швами. Косо-фронтальное рассечение дужек обеспечивает возможность надежной фиксации фрагментов дужек чрескостными швами, костными гвоздями или другими способами, не прибегая к костнопластической фиксации. Благодаря использованию данного способа подхода к спинному мозгу значительно снижается травматизация задней стенки позвоночного канала, так как остаются все элементы, которые лишь рассекаются, а после выполнения вмешательства, укладываются на место и фиксируются швами. Косо-фронтальное рассечение корней дужек создает хорошие условия для последующего сопоставления фрагментов и надежного их соединения, чтобы мобилизованная стенка не провалилась в позвоночный канал. При этом способе имеется возможность расширения оперативного доступа кверху, книзу или в стороны также с сохранением всех элементов задней стенки позвоночного канала путем продолжения разрезов в нужном направлении. При данном способе создаются морфологические условия для более быстрой анатомо-функциональной реабилитации с полным восстановлением движений в оперированном отделе позвоночника. Сокращается период пребывания в гипсовых кроватках и ношения корсетов, активные позиции наступают быстрее, чем после удаления дужек и связочного аппарата задней стенки позвоночного канала. Исключается необходимость взятия аутотрансплантатов и костнопластической фиксации с обездвиживанием данного отдела позвоночника и инвалидизацией больного. The period of stay in plaster beds and wearing corsets is reduced, active positions are faster than after the removal of the arches and ligaments of the posterior wall of the spinal canal. It eliminates the need for autografts and osteoplastic fixation with immobilization of this spine and disability of the patient.
