Description

DESCRIÇÃO "IMPLANTE PARA INSERÇÃO ENTRE CORPOS VERTEBRAIS, ASSIM COMO INSTRUMENTO CIRÚRGICO PARA MANUSEAMENTO DO IMPLANTE"
Implante para inserção entre corpos vertebrais, assim como instrumento de operação para manuseamento do implante em conformidade com o conceito genérico da reivindicação 1. A presente invenção refere-se a um implante para inserção entre corpos vertebrais da coluna vertebral como substituto para as vértebras ou partes de vértebras retiradas da coluna vertebral com uma primeira parte de implante e uma segunda parte de implante reguláveis reciprocamente no sentido do seu eixo longitudinal coaxial para modificação de comprimento do implante. Além disso, a invenção refere-se a um instrumento de operação com o qual é possível realizar a modificação de comprimento do implante.
Um implante do tipo mencionado inicialmente é conhecido, por exemplo, do documento DE 4423257 Al, que na prática deu bom resultado e com o qual foi possível alcançar sucessos de cura muito bons após a implantação. Durante a implantação numa cirurgia na coluna vertebral aberta, demonstrou-se, no entanto, como parcialmente problemático a necessidade de um campo de operação grande para aparafusar a primeira parte de implante em relação à segunda parte de implante, uma vez que é necessário realizar a rotação mútua por meio de movimentos de rotação. Contudo, em nem sempre existe espaço suficiente para tais movimentos de rotação, particularmente quando no sentido de uma 1 cirurgia minimamente invasiva são realizadas apenas acessos pequenos à coluna vertebral.
Por esta razão, o objectivo da invenção consiste em conceber um implante do tipo mencionado inicialmente de tal modo que também pode ser realizada uma modificação de comprimento do implante sem que para isso seja necessário um movimento de rotação do instrumento de operação previsto para o accionamento do implante. Objectivo da invenção é, além disso, disponibilizar um instrumento de operação que é especialmente bem adequado para interagir com o implante de acordo com a invenção. A parte do objectivo que diz respeito ao implante é solucionada num implante do tipo mencionado inicialmente por à primeira parte de implante estar atribuído um anel roscado giratório, o qual engrena com uma rosca anelar numa rosca atribuída à segunda parte de implante, e por o anel roscado estar provido de um dentado de engrenagem cónica.
Este implante oferece a grande vantagem de ser possível realizar devido ao dentado de engrenagem cónica uma rotação da segunda parte de implante através de um instrumento de operação aplicado de apenas radialmente no implante, unicamente com a sua rotação. Deste modo, um acesso substancialmente pontual ao implante é suficiente para a modificação de comprimento, facto pelo qual é evitada uma ferida que é aumentada unicamente para fins de distracção.
Segundo uma forma de realização preferida da invenção está previsto que a primeira parte de implante e a segunda parte de implante estejam concebidas como casquilhos em forma de tubo, e que a primeira parte de implante envolva exteriormente com o 2 anel roscado a segunda parte de implante. Esta configuração da invenção mediante a utilização de dois casquilhos oferece a vantagem de o espaço interior do implante estar livre e poder ser aproveitado para preencher, por exemplo, com cimento ósseo ou enxertos ósseos do próprio doente ou de terceiros para que o implante se possa incorporar de forma segura no osso. Com o intuito de evitar um aperto ou outro bloqueio da rosca durante a modificação de comprimento do implante ao aparafusar a segunda parte de implante em relação à primeira parte de implante, o implante está configurado de tal modo que na segunda parte de implante está concebida pelo menos uma fenda de guia para a recepção de um pino atribuído à primeira parte de implante. Para fins de uma guia melhorada, comprovou-se neste caso que dá bom resultado se a fenda de guia e o pino estiverem previstos duas vezes em disposição diametral.
Com o intuito de simplificar o accionamento do anel roscado por meio de um instrumento de operação, na parede da primeira parte de implante em forma de tubo está prevista pelo menos uma abertura que intercepta o rebordo atribuido ao anel roscado. Esta abertura pode ser aproveitada como um apoio para o instrumento de operação a aplicar no implante.
Para não ter de restringir a guia do instrumento de operação à espessura da parede do casquilho, na segunda parte de implante existem dois orifícios oblongos de forma alinhada com a abertura. Estes orifícios oblongos permitem também que esteja concebido um furo roscado no lado oposto à abertura na parede da primeira parte de implante, que pode então ser alcançado pela abertura. 3
No que diz respeito a uma incorporação mais rápida do implante, revelou-se, além disso, como vantajoso se na segunda parte de implante estiver concebida uma coroa na extremidade oposta à primeira parte de implante. Esta coroa aumenta a superfície de apoio da segunda parte de implante no corpo vertebral. Se a coroa estiver disposta com distância da extremidade livre da segunda parte de implante, esta pode servir como fixação para cimento para ossos a introduzir e simultaneamente proteger a rosca contra a deposição do cimento para ossos. Em alternativa é igualmente possivel que na coroa estejam concebidos orifícios em paralelo relativamente ao eixo longitudinal do implante.
Com o intuito de alcançar um comprimento de guia tão grande tanto quanto possivel do anel roscado na rosca, o implante está configurado de tal modo que o anel roscado apresenta um rebordo anelar no perímetro interior.
Para poder realizar uma fixação da posição do anel roscado em relação à primeira parte de implante, o implante de acordo com a invenção está, além disso, configurado de tal forma que a primeira parte de implante apresenta uma ranhura transversal concebida no casquilho em forma de tubo de forma perpendicular em relação ao eixo longitudinal do implante. Esta ranhura transversal encontra-se de forma adequada em paralelo relativamente ao eixo de ligação da abertura e do furo roscado. Além disso, é vantajoso se a ranhura transversal estiver concebida em ambos os lados do eixo de ligação no casquilho em forma de tubo da primeira parte de implante. A parte do objectivo que diz respeito ao instrumento de operação do objectivo é solucionada por um instrumento de 4 operação para distracção de um implante que apresenta um dentado de engrenagem cónica e que é ajustável em comprimento através de uma união roscada, para inserção entre corpos vertebrais da coluna vertebral como substituto para vértebras retiradas da coluna vertebral, em que na extremidade livre de um veio está disposto um pinhão coaxial em relação ao eixo do veio com um dentado de engrenagem cónica. Através deste instrumento de operação é possível rodar o dentado de engrenagem cónica do pinhão mediante o veio afastado radialmente do implante, que está orientado de forma perpendicular ao eixo longitudinal do implante, unicamente mediante a rotação do veio à volta do eixo longitudinal, que ao interagir com o dentado de engrenagem cónica do anel roscado realiza a rotação da primeira parte de implante e da segunda parte de implante relativamente uma à outra necessária para modificação de comprimento do implante.
Para um manuseamento mais fácil do instrumento de operação durante a operação, está previsto que o veio esteja montado de forma giratória num punho e que esteja provido de uma roda de accionamento na extremidade oposta ao pinhão para accionamento.
Uma forma de realização especialmente preferida do instrumento de operação está caracterizada por o veio estar concebido como veio oco, no qual está montada uma barra rotativa em relação ao veio oco, que atravessa o pinhão com a sua extremidade livre e no qual está concebida uma rosca da barra que corresponde ao furo roscado. Isto oferece a vantagem de ser possível fixar a posição da primeira parte de implante em relação ao instrumento de operação através da rosca da barra, de modo que não é provocada uma modificação da posição do substituto posicionado entre os corpos vertebrais da coluna vertebral ao exercer a pressão necessária para engrenar o 5 dentado de engrenagem cónica do pinhão com o dentado de engrenagem cónica do anel roscado.
Uma outra forma de realização preferida da invenção é caracterizada por o veio estar disposto num casquilho fixo ao punho, que na extremidade atribuída ao pinhão apresenta uma forquilha com dedos de forquilha originam uma fixação da posição axial mútua do anel roscado e da primeira parte de implante. Neste caso, revelou-se como vantajoso se a forquilha apresentar ao todo quatro dedos de forquilha que suportam o implante, dos quais dois estão agrupados num primeiro par de forquilha e previstos para o apoio no anel roscado e dois estão agrupados num segundo par de forquilha e previstos para inserção nas ranhuras transversais. Com um instrumento de operação deste género é possível agarrar o implante de forma muito segura e mantê-lo num local predeterminado, em que apesar da rotação necessária do veio para a modificação de comprimento, não existe nenhum perigo de o implante no campo de operação se deslocar ou o primeiro implante e o segundo implante se deslocarem reciprocamente e, assim, assumirem um prolongamento do comprimento, que não é realizado através de uma rotação do anel roscado e não segurado é protegido por este.
Com vista às relações higiénicas melhoradas na desinfecção, é conveniente se o veio com a roda de accionamento, a barra e o punho com o casquilho estiverem montados de forma a poderem ser desmontados.
Em seguida, a invenção deve ser explicada mais detalhadamente com base num exemplo de realização representado no desenho, ilustram: 6

Fig. 12 a representação detalhada XII da fig. 1. 0 implante 1 representado no desenho destina-se à inserção entre os corpos vertebrais, em si não representados no desenho, da coluna vertebral como substituto para as vértebras ou partes de vértebras retiradas da coluna vertebral. Este implante 1 apresenta uma primeira parte 2 de implante e uma segunda parte 3 de implante reguláveis reciprocamente no sentido do seu eixo longitudinal coaxial para poder realizar uma modificação de comprimento no sentido de uma distracção dos corpos vertebrais. A primeira parte 2 de implante e a segunda parte 3 de implante estão concebidas como casquilhos em forma de tubo, em que a primeira parte 2 de implante envolve exteriormente a segunda parte 3 de implante. À primeira parte 2 de implante está atribuído um anel 4 roscado giratório, que na sua superfície de perímetro interior apresenta uma rosca 5 anelar, com a qual engrena numa rosca 6 atribuída à segunda parte 3 de implante. Na superfície do perímetro exterior, o anel 5 roscado está provido de um dentado 7 de engrenagem cónica (fig. 8).
Como é possível visualizar particularmente nas figuras 3 e 6, na segunda parte 3 de implante estão concebidas duas fendas 8 de guia opostas uma em relação à outra, nas quais se prolongam até ao interior os pinos 9 atribuídos à primeira parte 2 de implante (fig. 2).
Para além destes pinos 9, a primeira parte 2 de implante apresenta ainda uma abertura 10, que intercepta o rebordo 11 atribuído ao anel 4 roscado e que se encontra em alinhamento recto com um furo 12 roscado concebido do lado oposto. De forma alinhada com a abertura 10, na segunda parte 3 de implante estão previstos dois orifícios 13 oblongos, através dos quais o furo 1 roscado é acessível em qualquer posição de altura da segunda parte 3 de implante em relação à primeira parte 1 de implante.
Na segunda parte 3 de implante está concebida uma coroa 14 com distância da sua extremidade livre sobre o lado oposto à primeira parte 2 de implante, na qual estão concebidos orifícios 15 paralelos em relação ao eixo longitudinal do implante 1.
No perímetro interior, o anel 4 roscado apresenta um rebordo 16 anelar, enquanto a primeira parte 2 de implante apresenta uma ranhura 17 transversal perpendicular em relação ao eixo longitudinal do implante 1 concebida no casquilho em forma de tubo de forma, e que se encontra paralelamente em relação ao eixo de ligação da abertura 10 e do furo 12 roscado, em que no exemplo de realização representado no desenho, a ranhura 17 transversal está prevista duas vezes em ambos os lados do eixo de ligação. O anel 16 circular concebido no anel 4 roscado, assim como as ranhuras 17 transversais destinam-se ao melhor alinhamento e à determinação da posição de um instrumento 18 de operação descrito em seguida mais detalhadamente, que se destina à manuseamento e accionamento do implante 1 no ajuste de comprimento.
Para isso, o instrumento 18 de operação apresenta na extremidade livre de um veio 19 um pinhão 20 coaxial em relação ao eixo do veio. Neste pinhão 20 está concebido um dentado 21 de engrenagem cónica (fig. 9, fig. 10) . O veio 19 em si está 9 concebido como veio oco, no qual está montada uma barra 22 rotativa em relação ao veio oco. Esta barra 22 atravessa o pinhão 20 e apresenta na sua extremidade livre uma rosca 23 de barra que corresponde ao furo 12 roscado. 0 veio oco em si está montado de forma giratória num punho 24, que para isso apresenta um casquilho 25 no qual está inserido o veio oco. Este casquilho 25 possui na extremidade atribuida ao pinhão 20 uma forquilha 26 com quatro dedos 27 de forquilha, dos quais dois estão agrupados num primeiro par 28 de forquilha e dois estão agrupados num segundo par 29 de forquilha.
Após a descrição anterior da estrutura do implante 1 e do instrumento 18 de operação, em seguida deve estar demonstrada em maior detalhe a sua interacção.
Para posicionar o implante 1 de acordo com a invenção como substituto entre dois corpos vertebrais, o implante 1 é encaixado primeiro na forquilha 26, em que o segundo par 29 de forquilha é inserido nas duas ranhuras 17 transversais e o primeiro par 28 de forquilha assenta no anel 4 roscado e interiormente de forma radial no anel 16 circular, de modo a bloquear uma deslocação axial do anel 4 roscado no eixo longitudinal do implante 1 em relação à primeira parte 2 de implante. Em seguida, a barra 22 com a rosca 23 de barra é conduzida através v abertura 10 da primeira parte 2 de implante, assim como através dos orifícios 13 oblongos da segunda parte 3 de implante e aparafusada no furo 12 roscado da primeira parte 2 de implante, de modo a não ser possível retirar mais o instrumento 18 de operação do implante lno sentido radial, fixando, deste modo, a posição do instrumento 18 de operação ao todo em relação ao implante 1. Ao aparafusar a rosca 23 de barra no orifício 12 roscado é possível engrenar simultaneamente o 10 dentado 21 de engrenagem cónica do pinhão 20 com o dentado 7 de engrenagem cónica do anel 4 roscado, de modo gue na rotação subsequente do veio 19 à volta do seu eixo do veio através de uma roda 30 de accionamento, se realiza uma rotação do anel 4 roscado à volta do eixo longitudinal do implante 1 rodado em 90° em relação ao eixo do veio.
Após o posicionamento e distracção correctos do implante 1, o instrumento 18 de operação pode ser retirado novamente do implante 1 na inversão adequada das etapas descritas anteriormente, que pelas fendas 8 de guia, orificios 13 oblongos e furos 12 roscados então livremente acessiveis, assim como a abertura 10 oferece possibilidades de acesso suficientes ao interior das primeiras partes 2 de implante e segundas partes 3 de implante em forma de casquilho para inserir cimento para ossos ou material ósseo autólogo ou homólogo no implante. O acessório indicado com o sinal de referência 31 representado na figura 1 serve de acoplamento do instrumento 18 de operação com sensores de posição para navegação no computador.

Claims (20)

  1. REIVINDICAÇÕES 1. Implante para inserção entre corpos vertebrais da coluna vertebral como substituto para as vértebras ou partes de vértebras retiradas da coluna vertebral, com uma primeira parte (2) de implante e uma segunda parte de implante (3) reguláveis reciprocamente no sentido do seu eixo longitudinal coaxial para a modificação de comprimento do implante (1), caracterizado por à primeira parte (2) de implante estar atribuído um anel (4) roscado giratório que engrena com uma rosca (5) anelar numa rosca (6) atribuída à segunda parte (3) de implante, e por o anel (4) roscado estar provido de um dentado (7) de engrenagem cónica.
  2. 2. Implante de acordo com a reivindicação 1, caracterizado por a primeira parte (2) de implante e a segunda parte (3) de implante estarem configuradas como casquilhos em forma de tubo, e por a primeira parte (2) de implante envolver exteriormente com o anel (4) roscado a segunda parte (3) de implante.
  3. 3. Implante de acordo com a reivindicação 1 ou 2, caracterizado por na segunda parte (3) de implante estar concebido pelo menos uma fenda (8) de guia para a recepção de um pino (9) atribuído à primeira parte (2) de implante.
  4. 4. Implante de acordo com a reivindicação 3, caracterizado por a fenda (8) de guia e o pino (9) estarem previstos duas vezes em disposição diametral. 1
  5. 5. Implante de acordo com qualquer uma das reivindicações 2 a 4, caracterizado por na parede da primeira parte (2) de implante em forma de casquilho existir pelo menos uma abertura (10) que intercepta o rebordo (11) atribuído ao anel (4) roscado.
  6. 6. Implante de acordo com a reivindicação 5, caracterizado por existirem dois orifícios (13) oblongos de forma alinhada com a abertura (10) na segunda parte (3) de implante.
  7. 7. Implante de acordo com a reivindicação 6, caracterizado por estar concebido um furo (12) roscado no lado oposto à abertura (10) na parede da primeira parte (2) de implante.
  8. 8. Implante de acordo com qualquer uma das reivindicações 1 a 7, caracterizado por na segunda parte (3) de implante estar concebida uma coroa (14) na extremidade oposta à primeira parte (2) de implante.
  9. 9. Implante de acordo com a reivindicação 8, caracterizado por a coroa (14) estar disposta com distância da extremidade livre da segunda parte (3) de implante.
  10. 10. Implante de acordo com a reivindicação 8 ou 9, caracterizado por na coroa (14) estarem concebidos orifícios (15) paralelos em relação ao eixo longitudinal do implante (1).
  11. 11. Implante de acordo com qualquer uma das reivindicações 1 a 10, caracterizado por o anel (4) roscado apresentar um anel (16) circular no perímetro interior. 2
  12. 12. Implante de acordo com qualquer uma das reivindicações 2 a 11, caracterizado por a primeira parte (2) de implante apresentar uma ranhura (17) transversal, perpendicular em relação ao eixo longitudinal do implante (1) concebida no casquilho em forma de tubo de forma.
  13. 13. Implante de acordo com a reivindicação 12, caracterizado por a ranhura (17) transversal se encontrar paralelamente em relação ao eixo de ligação da abertura (10) e do furo (12) roscado.
  14. 14. Implante de acordo com a reivindicação 13, caracterizado por a ranhura (17) transversal estar concebida em ambos os lados do eixo de ligação no casquilho em forma de tubo da primeira parte (2) de implante.
  15. 15. Instrumento de operação para distracção de um implante (1) que apresenta um dentado (7) de engrenagem cónica ajustável em comprimento através de uma união roscada, para inserção entre corpos vertebrais da coluna vertebral como substituto para vértebras ou partes de vértebras retiradas da coluna vertebral, de acordo com qualquer uma das reivindicações 1 a 14, caracterizado por na extremidade livre de um veio (19) estar disposto um pinhão (20) coaxial em relação ao eixo do veio com um dentado (21) de engrenagem cónica.
  16. 16. Instrumento de operação de acordo com a reivindicação 15, caracterizado por o veio (19) estar montado de forma giratória num punho (24) e estar provido para accionamento de uma roda (30) de accionamento na extremidade oposta ao pinhão (20). 3

Instrumento de operação de acordo com a reivindicação 15 ou 16, caracterizado por o veio (19) estar configurado como veio oco, no qual está montada uma barra (22) rotativa em relação ao veio oco que atravessa o pinhão (20) com a sua extremidade livre e no qual está concebida uma rosca (23) de barra que corresponde ao furo (12) roscado.
  18. 18. Instrumento de operação de acordo com a reivindicação 16 ou 17, caracterizado por o veio (19) estar disposto num casquilho (25) fixo no punho (24), que na extremidade atribuída ao pinhão (20) apresenta uma forquilha (26) com dedos (27) de forquilha que originam uma fixação da posição axial mútua do anel (4) roscado e da primeira parte (2) de implante.
  19. 19. Instrumento de operação de acordo com a reivindicação 18, caracterizado por a forquilha (26) apresentar ao todo quatro dedos (27) de forquilha que suportam o implante (1), dos quais dois estão agrupados num primeiro par (28) de forquilha e previstos para o apoio no anel (4) roscado e dois estão agrupados num segundo par (29) de forquilha e previstos para inserção nas ranhuras (17) transversais.
  20. 20. Instrumento de operação de acordo com qualquer uma das reivindicações 15 a 19, caracterizado por o veio (19) com a roda (30) de accionamento, a barra (22) e o punho (24) com o casquilho (25) estarem montados de forma a poderem ser desmontados. Lisboa, 8 de Novembro de 2006 4
