Description
Den foreliggende oppfinnelsen vedrører sammensetninger inneholdende medisinske planteekstrakter eller rensete eller standardiserte fraksjoner derav, nyttige i behandlingen av atopisk dermatitt, hudallergitilstander og akne.
KJENT TEKNIKK
Atopisk dermatitt erkarakterisert vedvedvarende inflammatoriske tilstander som kan forårsake vevskader i huden med mulige infeksjoner. Betennelse ledsages ofte av intens kløe; den påfølgende kløingen av pasienten kan ytterligere irritere det berørte området.
Dermatitt, som også ofte henvises til som eksem, er overfladiske hudbetennelser, med akutt eller kronisk forløp og variable kliniske og histologiske utslag, hvilket angår en betydelig prosent av befolkningen (2-3 %).
Etiologien til de ulike formene av dermatitt er fremdeles delvis uklar, selv om im-munkomponenten definitivt er én av faktorene. De patologiske utslagene er ofte like og er hovedsakeligkarakterisert vednærværet av papler, erytematøse utslett og vesikler, hvilke kan danne plakk, ødem, skorpeavskallingslesjoner og lignende. De vanligste dermatitter omfatter atopisk dermatitt, kontaktallergier, kontaktder-matitt, stasisdermatitt, seborrédermatitt, lichen simplex og acne vulgaris.
Behandlingsvalget består i å anvende kortikosteroider, hvilke involverer velkjente bieffekter (muligens sekundære infeksjoner, ineffektivitet etter langvarig bruk og lignende).
Dl, EP 0870507 A, er ansett som nærmeste kjente teknikk. Dl er et urteekstrakt som skal sprayes inn i munnen som skal brukes ved munnhygiene. gir umiddelbar, fordelaktig virkning hos pasienten og forårsaker hurtig bedring i
den patologiske tilstanden, til og med sammenlignet med kjente medikamenter så som topiske betennelsesdempere og steroider.
Mer nøyaktig inneholder sammensetningene ifølge oppfinnelsen:
a) Ginkgo biloba- terpener; b) floriglucinoler, enten rene eller i blandinger derav, ekstrahert fra Humulus
luipulus, Hypericum sp og Mirtus sp;
c) Zanthoxylum bungeanum eller Echinacea angustifola lipofilt ekstrakt.
Ifølge den foreliggende oppfinnelsen kan Ginkgo biloba-\nn holdet være fra 0,1 til
2 %; floroglucinolinnholdet kan være fra 0,1 til 1 %, og Zanthoxylum bungeanum eller Echinacea angustifolia lipofilt ekstrakt-innholdet kan være fra 0,01 til 0,5 %.
Ifølge oppfinnelsen er Gingko biloba- terpener til stede i fri form eller foretrukket i form av kompleks med naturlige eller syntetiske fosfolipider. " Ginkgo biloba-terpener" heri betyr terpenene, enten rene eller i en blanding, hvori det totale triterpeninnholdet er fra 60 til 100 %, foretrukket 90 %, biobalid-innholdet er fra 20 til 70 %, foretrukket 45 %, og ginkgolider A, B, C og J totalinnhold er fra 25 til
75 %, foretrukket 50 %.
Ginkgo biloba- terpener, i tillegg til den antiinflammatoriske effekten, utviser også antiallergisk virkning, som observert i et antall modeller både i dyr og mennesker, derved å redusere ett av symptomene involvert, hvilke motvirker irritasjon og kon-tinuerlige tilbakefall. Denne antiallergiske virkningen gjør nevnte terpener spesielt nyttige når en immun faktor er involvert i etiologien.
Floroglucinoler, enten rene eller i en blanding derav ekstrahert fra Humulus lupulus, Hypericum sp og Mirtus sp., er aktive mot anaerobe bakterier så som Propionibac- terium acnis og lignende og på Candida albicans-stammer, ved konsentrasjoner på 0,5 - 4 ug/ml.
Spesielt karakteriseres Humulus lupulus-ekstrakter ved floroglucinolinnhold på 20 til 80 %, foretrukket 60 %. Blant ekstraktene til Hypericum sp., nevnes spesielt et Hypericum perforatum-ekstrakt med et floroglucinol- (adhyperforin/hyperforin) innhold fra 20 til 80 %, foretrukket 60 %. Blant ekstraktene til Mirtus sp, er spesielt foretrukket et ekstrakt av Mirtus communis-blader fremstilt ved ekstraksjon med karbondioksid under trykk fra 235 til 260 bar, ved en temperatur fra 40 til 60 °C. Det resulterende ekstraktet inneholder vanligvis omtrent 35 % mirtocumulone.
Sammensetningene ifølge oppfinnelsen inneholdende en Zanthoxylum bungeanum eller Echinacea angustifolia lipofilt ekstrakt anriket i isobutylamider, aktiv som topiske smertestillere som inhiberer nerveforbindelsen som har vist seg spesielt effektiv i behandlingen av lokal kløe. Derfor, ifølge et foretrukket aspekt, vil sammensetningene ifølge den foreliggende oppfinnelsen inneholde som komponent c), et Zanthoxylum bungeanum eller Echinacea angustifolia lipofilt ekstrakt anriket i isobutylamider. Zanthoxylum-ekstraktet kan fremstilles f.eks. som beskrevet i WO 00/02570, mens Echinacea-ekstraktet kan fremstilles f.eks. som beskrevet i EP 0 464 298.
Sammensetningene ifølge oppfinnelsen inneholder ytterligere naturlige forbindelser med østrogen og/eller antiandrogen virkning som er vist spesielt effektiv i behandlingen av seboréisk eksem og akne. Derfor, ifølge et foretrukket aspekt, vil sammensetningene ifølge den foreliggende oppfinnelsen inneholde, i tillegg til kompo-nenter a), b) og c) over, også naturlige forbindelser med østrogenvirkning, så som ferutinin og/eller ekstrakter av ulike Ferula-arter, og/eller naturlige forbindelser med antiandrogen virkning, så som laurinsyre.
Ifølge et ytterligere foretrukket aspekt, vil sammensetningene ifølge den foreliggende oppfinnelsen inneholde Oenothera biennis-olje som lipofil eksipiens.
Den foreliggende oppfinnelsen vedrører derfor topiske sammensetninger for behandlingen av atopisk dermatitt, hudallergitilstander og akne, inneholdende kombi-nasjonene beskrevet over. Nevnte sammensetninger vil fremstilles ifølge konven-sjonelle fremgangsmåter velkjente i farmasøytisk teknikk, så som de beskrevet i "Remington's Pharmaceutical Handbook", Mack Publishing Co., N.Y., USA, sammen med passende eksipienser vanligvis anvendt i fagfeltet.
Eksemplene gitt under illustrerer oppfinnelsen ytterligere.
Claims (22)
1. Topiske sammensetninger for behandlingen av atopisk dermatitt, hudallergitilstander og akne, inneholdende: a) Ginkgo biloba-terpener; b) floroglucinoler, enten rene eller i blandinger derav, ekstrahert fra Humulus lupulus, Hypericum sp og Mirtus sp; c) Zanthoxylum bungeanum eller Echinacea angustifolia lipofilt ekstrakt.
2. Sammensetninger ifølge krav 1, inneholdende: a) 0,1 til 2 % Ginkgo biloba; b) 0,1 til 1 % floroglucinoler, enten rene eller i en blanding derav, ekstrahert fra Humulus lupulus, Hypericum sp og Mirtus sp: c) 0,01 til 0,5 % Zanthoxylum bungeanum eller Echinacea angustifolia lipofilt ekstrakt.
3. Sammensetninger ifølge kravene over, hvori Ginkgo biloba-terpener er til stede i den frie formen.
4. Sammensetninger ifølge kravene over, hvori Ginkgo biloba-terpener er til stede i formen av kompleks med naturlige eller syntetiske fosfolipider.
5. Sammensetninger ifølge krav 4, hvori det totale triterpen-innholdet er fra 60 til 100 %.
6. Sammensetninger ifølge krav 5, hvori det totale triterpen-innholdet er 90 %.
7. Sammensetninger ifølge krav 4, hvori bilobalid-innholdet er fra 20 til 70 %.
8. Sammensetninger ifølge krav 7, hvori bilobalid-innholdet er 45 %.
9. Sammensetninger ifølge krav 4, hvori det totale A, B, C og J ginkgolideinn-holdet er fra 25 til 75 %.
10. Sammensetninger ifølge krav 9, hvori det totale A, B, C og J ginkgolideinn-holdet er 50 %.
11. Sammensetninger i følge krav 1-2, hvori flourglukinolinnholdet av Humulus lupulus ekstrakter er mellom 20 % og 80 %.
12. Sammensetninger ifølge kravene 1-2, hvori floroglucinol-innholdet av Humulus lupulus-ekstrakter er 60 %.
13. Sammensetninger ifølge kravene 1-2, hvori Hypericum sp.-ekstraktet er et Hypericum perforatum-ekstrakt hvori floroglucinolinnholdet er fra 20 til 80 %.
14. Sammensetninger ifølge krav 13, hvori floroglucinolinnholdet i Hypericum perforatum-ekstraktet er 60 %.
15. Sammensetninger ifølge kravene 1-2, hvori ekstraktet av Mirtus communis-blader fremstilles av ekstraksjon med karbondioksid under trykkbetingelser fra 235 til 260 bar og temperaturer fra 40 til 60 °C.
16. Sammensetninger ifølge krav 15, hvori Mirtus sp.-ekstaktet er et ekstrakt av Mirtus communis-blader med et mirtocumulone-innhold på 35 %.
17. Sammensetninger ifølge kravene 1-2, hvori Zanthoxylum bungeanum eller Echinacea angustifolia-lipofilekstrakt er anriket i isobutylamider.
18. Sammensetninger ifølge kravene over, ytterligere inneholdende ferutinin.
19. Sammensetninger ifølge kravene over, ytterligere inneholdende ekstrakter av ulike Ferula-arter.
20. Sammensetninger ifølge kravene over, ytterligere inneholdende laurinsyre.
21. Sammensetninger ifølge kravene over, ytterligere inneholdende Oenothera biennis-olje som lipofil eksipiens.
22. Anvendelse av en kombinasjon av: a) Ginkgo biloba-terpener; b) floroglucinoler rene eller i blanding derav, ekstrahert fra Humulus lupulus, Hypericum sp og Mirtus sp; c) Zanthoxylum bungeanum eller Echinacea angustifolia lipofilt ekstrakt;
for fremstillingen av et medikament for behandlingen av atopisk dermatitt, hudallergitilstander og akne.
