Description
Foreliggende oppfinnelse angår nye kinolinyl-pyrazolforbindelser og deres anvendelse som farmasøytiske midler, særlig deres anvendelse som TGF-J3 signaltransduksjons-inhibitorer.
Den transformerende vekstfaktor p ("TGF-P") polypeptid har innvirkning på vekst, differensiering og genekspresjon i mange celletyper. Det første polypeptidet i denne familie som blekarakterisert, TGF-P 1, hadde to identiske 112 aminosyresubenheter som var kovalent forbundet. TGF-pi er et høyt konservert protein med kun en enkelt amino-syredifferanse som skiller mennesker fra mus. Det finnes to ytterligere medlemmer av TGF-p genfamilien som uttrykkes i pattedyr. TGF-P2 er 71% homolog til TGF-pi (de Martin, et al. (1987) EMBO J. 6:3673-3677), mens TGF-P3 er 80% homolog til TGF-pi (Derynck, et al. (1988) EMBO J 7:3737-3743). De strukturelle karakteristika for TGF-P 1 som bestemt ved kjernemagnetisk resonans (Archer, et al. (1993) Biochemistry 32:1164-1171) stemmer overens med krystallstrukturen for TGF-P2 (Daopin, et al.
(1992) Science 257:369-374; Schlunegger og Gnitter (1992) Nature 358:430-434).
Det er minst tre forskjellige, ekstracellulære TGF-P reseptorer, Type I, II og III, som er involvert i de biologiske funksjoner av TGF-p 1, -p2 og -p3 (for en oversikt, henvises det til Derynck (1994) TIBS 19:548-553 og Massague (1990) Ann. Rev. Cell Biol. 6:597-641). Type I og Type II reseptorer er transmembranserin/treonin kinaser, som i nærvær av TGF-P danner et heteromerisk signalkompleks (Wrana, et al (1992)
Cell 71: 1003-1014).
Mekanismen ved aktivering av det heteromeriske signalkomplekset på celleoverflaten er belyst (Wrana, et al. (1994) Nature 370: 341-347). TGF-p binder først type II reseptoren som er en konstitutivt aktiv transmembranserin/treoninkinase. Type I reseptoren blir deretter rekruttert inn i komplekset, fosforylert på GS-domainet og aktivert til fosforylat nedstrøms signalkomponentene (for eksempel Smad-proteinene) for å initiere den intracellulære signalkaskade. En konstitutivt aktiv type I-reseptor (T204D mutant) er påvist effektivt å transdusere TGF-P responser, og derved bypasse kravet for TGF-P og type II reseptor (Wieser, et al. (1995) EMBO J 14: 2199-2208). Selv om ingen signalfunksjon er oppdaget for type III reseptoren, øker den affiniteten for TGF-P2 for type II reseptor og gjør den i det vesentlige ekvipotent med TGF-p 1 og TGF-P3 (Lopez-Casillas, et al. (1993) Cell 73:1435-1444).
Vaskulære endotelceller mangler Type III reseptor. I stedet uttrykker endotelieceller et strukturelt relatert protein kalt endoglin (Cheifetz, et al. (1992) J. Biol. Chem.
267:19027-19030), som kun binder TGF-pi og TGF-p3 med høy affinitet. Således reflekterer den relative potens av TGF-P'er typen reseptorer som uttrykkes i et celle- og organsystem. I tillegg til reguleringen av komponentene i den multi-faktorielle signalvei påvirker fordelingen av syntesen av TGF-P polypeptider også en fysiologisk funksjon. Fordelingen av TGF-P2 og TGF-p3 er mer begrenset (Derynck, et al. (1988) EMBO J 7:3737-3743) enn TGF-pi, for eksempel er TGF-P3 er begrenset til vev av mesenkymal opprinnelse, mens TGF-P 1 er tilstede i vev av både mesenkymal og epitelial opprinnelse.
TGF-p 1 er et multifunksjonelt cytokine som kritisk for vevreparasjon. Høye konsentra-sjoner av TGF-P 1 avgis til skadesetet av plategranuler (Assoian and Sporn (1986) J. Celle Biol. 102:1217-1223). TGF-pi initierer en serie av hendelser som fremmer heling inkludert kjemotaksis av celler som leukocytter, monocytter og fibroblaster, og regulering av vekstfaktorer og cytokiner som er involvert i angiogenese, celledeling assosiert med vevsreparasjon og inflammatoriske responser. TGF-pi stimulerer også syntesen av ekstracellulære matrikskomponenter (Roberts, et al. (1986) Proe. Nati. Acad. Sei. USA 83:4167-4171; Sporn, et al. (1983) Science 219:1329-1330; Massague
(1987) Cell 49:437-438) og mest viktig for forståelse av patofysiologien av TGF-p 1, TGF-P 1 autoregulerer denne sin egen syntese (Kim, et al. (1989) J. Biol. Chem. 264:7041-7045).
Forbindelsene som beskrives heri viser også andre kinaseaktiviteter, som p38 kinaseinhibering og/eller KDR (VEGFR2) kinaseinhibering. Analyser for å bestemme slik kinaseaktivitet er velkjent innen teknikkens stand og fagmannen vil være i stand til å teste de beskrevne forbindelser for slik aktivitet.
Foreliggende oppfinnelse omfatter forbindelse, kjennetegnet ved formel II
og farmasøytisk akseptable salter derav.
Videre omfatter oppfinnelsen farmasøytisk preparat, kjennetegnet ved at det omfatter en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt, sammen med en farmasøytisk akseptabel diluent, eksipient eller bærer.
Foreliggende oppfinnelse omfatter også anvendelse av en forbindelse ifølge krav 1 eller et farmasøytisk akseptabelt salt derav for fremstilling av et preparat for behandling av cancer.
Foreliggende oppfinnelse angår den valgte forbindelse med formel II:
2-(6-metyl-pyridin-2-yl)-3-[6-amido-kinolin-4-yl)-5,6-dihydro-4H-pyrrolo[l,2-b]pyrazol
og farmasøytisk akseptable salter derav.
Forbindelsen ovenfor er generisk beskrevet og angitt i kravene i WO 02/094833A, av 13. mai 2002, som krever prioritet fra U.S.S.N. 60/293,464, av 24. mai 2001, og anses som del av beskrivelsen. Forbindelsen ovenfor er valgt for å ha en overraskende overlegen toksikologisk profil i forhold til forbindelser som spesifikt er beskrevet i de angitte søknader.

Fremstilling av 2-(6-metyl-pyridin-2-yl)-3-[6-amido-kinolin-4-yl)-5,6-dihydro-4H-pyrrolo [1,2-b] pyrazol
A. Fremstilling av 6-brom-4-metyl-kinolin
Omrør en oppløsning av 1 eq 4-brom-fenylamin (1 eq), i 1,4-dioksane og avkjølt til rundt 12°C. Tilsett langsomt 2 eq svovelsyre og oppvarm til tilbakeløp. Tilsett 1,5 eq etylvinylketon dråpevis til den tilbakeløpskokende oppløsning. Oppvarm oppløsningen i 1 time etter at tilsetningen er ferdig. Fordamp reaksjonsoppløsningen til tørr tilstand og oppløs i metylenklorid. Juster oppløsningen til pH 8 med 1 M natriumkarbonat og ekstraher tre ganger med vann. Kromatografer resten på Si02(70/30 heksan/etylacetat) for å oppnå det ønskede subtittel mellomprodukt.
MS ES+ m/e = 158,2 (M+l).
B. Fremstilling av 6-metyl-pyridin-2-karboksylsyremetylester
Suspender 6-metyl-pyridin-2-karboksylsyre (10 g, 72,9 mmol) i metylenklorid (200 ml). Avkjøl til 0°C. Tilsett metanol (10 ml), 4-dimetylaminopyridin (11,6 g, 94,8 mmol), og 1- (3-dimetylaminopropyl)-3-etylkarbodiimidhydroklorid (EDC) (18,2 g, 94,8 mmol). Omrør blandingen ved romtemperatur i 6 timer, vask med vann og brine, og tørk over natriumsulfat. Filtrer blandingen og konsentrer under vakuum, kromatografer resten på Si02(50% etylacetat/heksaner) for å oppnå det ønskede subtittel mellomprodukt, 9,66 g (92%), som en fargeløs væske.

C. Fremtilling av 2-(6-brom-kinolin-4-yl)-l-(6-metyl-pyridin-2-yl)-etanon
Oppløs 6-brom-4-metyl-kinolin (38,5 g, 153 mmol) i 600 ml tørr dry THF. Avkjøl til -70°C og tilsett dråpevis 0,5 M kaliumheksametyldisilazan (KN(SiMe3)2) (400 ml, 200 mmol) i løpet av 2 timer mens temperaturen holdes under -65 °C. Omrør den resulterende oppløsning ved -70°C i 1 time og tilsett en oppløsning av 6-metylpyridin-2- karboksylsyremetylester (27,2, 180 mmol) i 100 ml tørr THF dråpevis i løpet av 15 minutter. I løpet av tilsetningen vil blandingen gå fra mørkerød til ertegrønn og danne et presipitat. Omrør oppløsningen ved -70°C i 2 timer og tillat oppvarming til omgivelsestemperatur under omrøring i 5 timer. Avkjøl blandingen og quench med 12 N HC1 til pH = 1. Hev pH-verdien til 9 med fast kaliumkarbonat. Dekanter oppløsningen fra faststoffene og ekstraher to ganger med 200 ml etylacetat. Kombiner de organiske ekstrakter, vask med vann og tørk over kaliumkarbonat. Omrør faststoffene i 200 ml vann og 200 ml etylacetat og behandle det hele så med ytterligere kaliumkarbonat. Separer den organiske del og tørk med de tidligere etylacetatekstraktene. Konsentrer oppløsningen under vakuum til en mørk olje. Før oljen gjennom en 300 ml silikaplugg med metylenklorid og så etylacetat. Kombiner de riktige fraksjoner og konsentrer under vakuum for å oppnå en ravfarget olje. Skyll oljen ned sidene av kolben med metylenklorid og fortynn med heksan under slynging av kolben for å oppnå 38,5 g (73,8 %) av det ønskede subtittel mellomprodukt som en gul olje.
MS ES+ = 341 (M+l).
D. Fremstilling av l-[2-(6-brom-kinolin-4-yl)-l-(6-metyl-pyridin-2-yl)-etylidenamino] -pyrrolidin-2-on
Omrør en blanding av 2-(6-brom-kinolin-4-yl)-l-(6-metyl-pyridin-2-yl)-etanon (38,5 g, 113 mmol) og 1-aminopyrrolidinonhydroklorid (20 g, 147 mmol) i 115 ml pyridin ved omgivelsestemperatur i 10 timer. Tilsett rundt 50 g 4 Å ikke-aktiverte sikter. Kontinuer omrøringen ytterligere 13 timer og tilsett 10-15 g silika og filtrer blandingen gjennom en 50 g silikaplugg. Eluer silikapluggen med 3 1 etylacetat. Kombiner filtratene og konsentrer under vakuum. Samle hydrazon presipitatet ved filtrering og sug det hele tort for å gi 33,3 g (69,7%) av det ønskede subtittel-mellomproduktet som et hvitaktig faststoff.
MS ES<+>= 423 (M+l).
E. Fremstilling av 6-brom-4-[2-(6-metyl-pyridin-2-yl)-5,6-dihydro-4H-pyrrolo [l,2-b]pyrazol-3-yl] -kinolin
Sett 300 ml tørr N,N-dimetylformamid til en blanding av (1,2 eq.) cesiumkarbonat og l-[2-(6-brom-kinolin-4-yl)-l-(6-metyl-pyridin-2-yl)-etylidenamino]-pyrrolidin-2-on (33,3 g, 78,7 mmol). Omrør blandingen i 20 timer ved 100°C. Blandingen kan bli mørk under reaksjonen. Fjern N,N-dimetylformamid under vakuum. Fordel resten mellom vann og metylenklorid. Ekstraher den vandige del med ytterligere metylenklorid. Filtrer de organiske oppløsninger gjennom en 300 ml silikaplugg, eluer med 1,5 1 metylenklorid, 1,5 1 etylacetat og 1,5 1 aceton. Kombiner de egnede fraksjoner og konsentrer under vakuum. Samle det resulterende presipitat ved filtrering for å gi 22,7 g (71,2%) av det ønskede subtittel mellomprodukt som et hvitaktig fast stoff. MS ES<+>= 405 (M+l).
F. Fremstilling av 4-[2-(6-metyl-pyridin-2-yl)-5,6-dihydro-4H-pyrrolo[l,2-b]pyrazol-3-yl]-kinolin-6-karboksylsyremetylester
Sett 6-brom-4-[2-(6-metyl-pyridin-2-yl)-5,6-dihydro-4H-pyrrolo[l,2-b]pyrazol-3-yl]-kinolin (22,7 g, 45 mmol) til en blanding av natriumacetat (19 g, 230 mmol) og palladiumkatalysator [1,1 '-bis(difenylfosfino)ferrocen]diklorpalladium(II), komplekset med diklormetan (1:1) (850 mg, 1,04 mmol) i 130 ml metanol. Anbring blandingen under 50 psi karbonmonoksidatmosfære og omrør under oppvarming til 90°C over 1 time og med konstant tilførsel av ytterligere karbonmonoksid. Tillat blandingen å avkjøles over 8 timer, fyll igjen på med karbonmonoksid og oppvarm til 90°C. Trykket kan stige til rundt 75 psi. Reaksjonen er ferdig i løpet av ca. en time når trykket er stabilt og tic (1:1 toluen/aceton) ikke viser noe gjenværende bromid. Fordel blandingen mellom 600 ml metylenklorid og 11 vann. Ekstraher den vandige del med ytterligere 400 ml metylenklorid. Filtrer den organiske oppløsning gjennom en 300 ml silikaplugg og vask med 500 ml metylenklorid, 1200 ml etylacetat og 1500 ml aceton. Kast acetondelen. Kombiner riktige fraksjoner og konsentrer for å gi 18,8 g (87,4%) av det ønskede subtittel mellomprodukt som et rosafarget pulver. MS ES+ = 385 (M+l).
G. Fremstilling av 2-(6-metyl-pyridin-2-yl)-3-[6-amido-kinolin-4-yl)-5,6-dihydro-4H-pyrrolo[l,2-b]pyrazol
Oppvarm en blanding av 4-[2-(6-metyl-pyridin-2-yl)-5,6-dihydro-4H-pyrrolo[l,2-b]pyrazol-3-yl]-kinolin-6-karboksylsyremetylester i 60 ml 7 N ammoniakk i metanol til 90°C i en trykkbeholder av rustfritt stål i 66 timer. Trykket vil stige til rundt 80 psi. Oppretthold trykket i løpet av reaksjonens varighet. Avkjøl beholderen og konsentrer den brune blanding under vakuum. Rens det gjenværende faststoff på to 12 g Redi-Pak søyler koblet i serie og eluer med aceton. Kombiner de riktige fraksjoner og konsentrer under vakuum. Suspender det resulterende nesten hvite faststoff i metylenklorid, fortynn med heksan og filtrer. De samlede, hvitaktige faststoffer gir 1,104 g (63,8%) av den ønskede tittelforbindelsen.
MS ES<+>= 370 (M+l).
Forbindelsene som er beskrevet her ble testet ved de følgende protokoller på TGF-P inhibering, som beskrevet nedenfor i protokollbeskrivelsen.
TGF- B RESEPTOR I RENSING og IN VITRO KINASE REAKSJONER
For TGF-p Type I (RIT204D) Reseptorer:
Det 6X-HIS taggede cytoplasmiske kinasedomainet av hver reseptor ble uttrykt og renset fra Sf9 insektcellelysater som beskrevet kort nedenfor: Cellepellets ble 48-72 timer etter infeksjon lysert i lyseringsbuffer (LB: 50 mM Tris pH 7,5,150 mM NaCl, 50 mM NaF, 0,5% NP40 med friskt tilsatt 20 mM p-merkaptoetanol, 10 mM imidazol, 1 mM PMSF, IX EDTA-fri fullstendig protease inhibitor (Boehringer Mannheim).
Cellelysatene ble klaret ved sentrifugering og 0,45 uM filtrering før rensing ved Ni/NTA affinitetskromatografi (Qiagen).
Kromatografiprotokoll:
Ekvilibrer med 10 CV LB, last inn prøven, vask med 10 CV RIPA buffer (50 mM Tris pH 7,5, 150 mM NaCl, 1% NP40, ImM EDTA, 0,25% natriumdeoksykolat, tilsatt frisk 20 mM p-merkaptoetanol, 1 mM PMSF), vask med 10 CV LB, vask med 10 CV IX KB (50 mM Tris pH 7,5,150 mM NaCl, 4 mM MgCl2,1 mM NaF, 2 mM p-merkaptoetanol), eluer med en lineær gradient av IX KB inneholdende 200 mM imidazol. Begge enzymer var rundt 90% rene og hadde autofosforyleringsaktivitet.
Reaksjoner: 170-200 nM enzym i IX KB, forbindelsesfortynningsserier i IX KB/16% DMSO (20 uM til 1 nM sluttkonsentrasjon med 4% DMSO sluttkonsentrasjon), reaksjoner startet ved tilsetting av ATP blanding (4 uM ATP/1 uCi<33>P-y-ATP slutt-konsentrasjoner) i IX KB.
Reaksjonene ble inkubert ved 30°C i 1 time. Reaksjonene ble stoppet og kvantifisert ved bruk av standard TC A/B SA presipitering på Millipore FB glassfiber filterplater og ved væskescintillasjontelling på en MicroBeta JET.
Forbindelsene som beskrevet her inhiberer TGF-P Type I (RIT204D) reseptorkinase-domenet med IC50verdier <20 uM, men viser mindre toksisitet in vivo enn strukturelt relaterte forbindelser som beskrevet i WO 02/094833 Alsom identifisert ovenfor.
Tilstander "karakterisert vedøkt TGF-P aktivitet" inkluderer de hvor TGF-P syntesen er stimulert slik at TGF-P er tilstede i økte nivåer eller der TGF-P latent protein aktiveres uønsket eller konverteres til aktivt TGF-P protein eller der TGF-P reseptorer oppreguleres eller der TGF-p protein viser økt binding til celler eller ekstracellulær matriks i sykdomslokasjonen. Således henviser i ethvert tilfelle "økt aktivitet" til enhver tilstand der den biologiske aktivitet for TGF-p er uønsket høy, uansett årsak.
Et antall sykdommer er assosiert med TGF-P 1 overproduksjon. Inhibitorer av TGF-B intracellulær signalvei er brukbare behandlinger for fibroproliferative sykdommer. Spesielt inkluderer fibroproliferative sykdommer nyrelidelser forbundet med ikke-regulert TGF-B aktivitet og for stor fibrose inkludert glomerulonefritt (GN), som mesangial proliferativ GN, immun GN og krescentisk GN. Andre renaltilstander inkluderer diabetisk nefropati, renal interstitial fibrose, renal fibrose i transplant-pasienter som får cyklosporin og HIV-assosiert nefropati. Kollagen vaskulære lidelser inkluderer progressiv, systemisk sklerose, polymyositt, skleroderma, dermatomyositt, eosinofil faskitt, morfea, eller de som er forbundet med opptredenen av Raynauds syndrom. Lungefibroser som er resultat av ekscessiv TGF- R aktivitet inkluderer adult respiratorisk distressyndrom, idiopatisk pulmonær fibrose og interstitial pulmonær fibrose som ofte assosieres med autoimmune lidelser, som systemisk lupus erytematose og skleroderma, kjemisk kontakt eller allergier. Andre autoimmune lidelser assosiert med fibroproliferative karakteristika er reumatoid artritt.
Øyesykdommer assosiert med en fibroproliferativ tilstand inkluderer retinal rearrangementskirurgi som ledsager proliferativ vitreoretinopati, katarktekstrahering med intraokkulær linseimplantering, og post glaucoma dreneringskirurgi er assosiert med TGF-pi overproduksjon.
Fibrotiske sykdommer assosiert med TGF-P 1 overproduksjon kan deles i kroniske tilstander som fibrose i nyre, lunge og lever og mer akutte tilstander som dermal årring og restenose (Chamberlain, J. Cardiovascular Drug Reviews, 19(4):329-344). Syntese og sekresjon av TGF-pi ved tumorceller kan også føre til immunsuppresjon slik det ses hos pasienter med aggressive hjerne- eller brysttumorer (Arteaga, et al. (1993) J. Clin. Invest. 92:2569-2576). Forløpet av Leishmanial-infeksjon i mus endres drastisk av TGF-pl (Barral-Netto, et al. (1992) Science 257:545-547). TGF-pl forverret sykdommen, mens TGF-P 1 antistoffer sinket progresjonen av sykdommen i genetisk mottakelige mus. Genetisk resistente mus ble mottakelige for Leishmanial-infesjon ved administrering av TGF-p 1.
De grunnleggende effekter av TGF-pi på ekstracellulær matriksavsetning er beskrevet (Rocco og Ziyadeh (1991) i Contemporary Issues i Nephrology v.23, Hormones, autocoids and the kidney. ed. Jay Stein, Churchill Livingston, New York pp. 391-410; Roberts, et al. (1988) Ree. Prog. Hormone Res. 44:157-197) og inkluderer stimulering av syntesen og inhibering av nedbrytningen av ekstracellulære matriks- komponenter. Fordi strukturen og filtreringsegenskapene for glomerulus i vesentlig grad bestemmes av den ekstracellulære matrikssammensetning av mesangium og den glomerulære membran, er det ikke overraskende at TGF-pi har dyptgripende virkninger på nyren. Akkumuleringen av mesangialmatriks i proliferative glomerulonefritt (Border, et al.
(1990) Kidney Int. 37:689-695) og diabetisk nefropati (Mauer, et al. (1984) J. Clin. Invest. 74:1143-1155) er klare og dominante, patologiske trekk for sykdommene. TGF-pi nivåene er forhøyet ved human diabetes glomerulosklerose (fremskreden neuropati)
(Yamamoto, et al. (1993) Proe. Nati. Acad. Sei. 90:1814-1818). TGF-pl er en viktig mediator i genesen av renal fibrose i et antall dyremodeller (Phan, et al. (1990) Kidney Int. 37:426; Okuda, et al. (1990) J. Clin. Invest. 86:453). Suppresjon av eksperimentelt indusert glomerulonefritt hos rotter er påvist med antiserum mot TGF-pi (Border, et al.
(1990) Nature 346:371) og ved et ekstracellulært matriksprotein, dekorin, som kan binde TGF-pi (Border, et al. (1992) Nature 360:361-363).
For mye TGF-pi fører til dermal arrvevsdannelse. Nøytralisering av TGF-pi antistoffer som er injisert i kantene av helende sår hos rotter er påvist å inhibere arrdannelse uten å interferere med hastigheten for sårheling eller strekkstyrken for såret (Shah, et al. (1992) Lancet 339:213-214). Samtidig var det redusert angiogenese, redusert antall makrofager og monocytter i såret, og en redusert mengde disorganisert kollagen fiberavsetning i arrvevet.
TGF-P 1 kan være en faktor ved den progressive fortykning av den arterielle vegg som skyldes proliferering av glattmuskelceller og avsetning av ekstracellulær matriks i arterien etter ballongangioplasti. Diameteren for den restenoserte arterien kan reduseres 90% ved denne fortykkelse, og fordi mesteparten av diameterreduksjonen skyldes ekstracellulær matriks og ikke glattmuskelcellelegemer, kan det være mulig å åpne disse kar med 50%, ganske enkelt ved å redusere ekstensiv ekstracellulær matriksavsetning. I ikke-skadede svinearterier som var transfektert in vivo med et TGF-pi genet, ble TGF-pi genekspresjonen assosiert med både ekstracellulær matrikssyntese og hyperplasi (Nabel, et al. (1993) Proe. Nati. Acad. Sei. USA 90:10759-10763). Den TGF-pl induserte hyperplasi var ikke så utstrakt som den som ble indusert med PDGF-BB, men den ekstracellulære matrisk var mer ekstensiv med TGF-pi transfektanter. Ingen ekstracellulær matriksavsetning ble assosiert med FGF-1 (en sekretert form av FGF) hyperplasi i denne genoverføringssvinemodell (Nabel (1993) Nature 362:844-846). Det er flere typer canser der TGF-pi som fremstilles av tumoren kan være skadelig. MATLyLu rotteprostatacancerceller (Steiner og Barrack (1992) Mol. Endocrinol 6:15-25) og MCF-7 humanbrystcancerceller (Arteaga, et al. (1993) Cell Growth and Differ. 4:193-201) ble mer tumorgene og metastatiske etter transfeksjon med en vektor som uttrykker mus TGF-P 1. TGF-P 1 er blitt assosiert med angiogenese, metastase og dårlig prognose i human prostata- og fremskreden gastrisk cancer (Wikstrom, P., et al. (1998) Prostate 37: 19-29; Saito, H. et al. (1999) Cancer 86: 1455-1462). Ibrystcancer er dårlig prognose assosiert med forhøyede TGF-P (Dickson, et al. (1987) Proe. Nati. Acad. Sei. USA 84:837-841; Kasid, et al. (1987) Cancer Res. 47:5733-5738; Daly, et al.
(1990) J. Cell Biochem. 43:199-211; Barrett-Lee, et al. (1990) Br. J Cancer 61:612-617; King, et al. (1989) J. Steroid Biochem. 34:133-138; Welch, et al. (1990) Proe. Nati. Acad. Sei. USA 87:7678-7682; Walker, et al. (1992) Eur. J. Cancer 238:641-644) og induksjon av TGF-pi med tamoksifenbehandling (Butta, et al. (1992) Cancer Res. 52:4261- 4264) er assosiert med svikt av tamoksifenbehandling for brystcancer (Thompson, et al. (1991) Br. J. Cancer 63:609-614). Anti TGF-pl antistoffer inhiberer veksten av MDA-231 humanbrystcancerceller i atymisk mus (Arteaga, et al. (1993) J. Clin. Invest. 92:2569-2576), en behandling som er korrelert med en økning i naturlig drepercelleaktivitet i milt. CHO celler som er transfektert med latent TGF-pi viste også redusert NK aktivitet og økt tumorvekst i nakne mus (Wallick, et al. (1990) J. Exp. Med. 172:1777-1784). Således kan TGF-P som skilles ut av brysttumorer forårsake en endokrin immunsuppresjon. Høye plasmakonsentrasjoner av TGF-pi er også påvist å indikere dårlig prognose for fremskredne brystcancerpasienter (Anscher, et al. (1993) N. Engl. J. Med. 328:1592-1598). Pasienter med høy sirkulerende TGF-P før høydose kjemoterapi og autolog benmargtransplantasjon er under høy risiko for hepatisk veno-okklusiv sykdom (15-50% av alle pasienter med en mortalitetsgrad opp til 50%) og idiopatisk interstitiell pneumonitt (40-60% av alle pasienter). Implikasjonen av disse funn er 1) at forhøyede plasmanivåer av TGF-P 1 kan benyttes for å identifisere risiko-pasienter og 2) at reduksjon av TGF-pi kan redusere morbiditeten og mortaliteten ved disse vanlige behandlinger for brystcancerpasienter.
Mange maligne celler sekreterer transformerende vekstfaktor-P (TGF-P), en potent immunosuppressant, noe som antyder at TGF-P produksjon kan representere en signifikant tumorunnslippelsesmekanisme fra vertsimmunovervåking. Etablering av en leukocytt sub-populasjon med avbrutt TGF-P signalering i den tumorbærende vert gir et potentielt middel for immunoterapi av cancer. En transgen dyremodell med avbrutt TGF-P signalering i T celler er i stand til å utrydde en vanlig letal TGF-P over-uttrykkende lymfomtumor, EL4 (Gorelik og Flavell, (2001) Nature Medicine 7(10): 1118-1122). Nedregulering av TGF-J3 sekresjon i tumorceller resulterer i gjenopprettelse av immunogenisitet i vertscellen, mens T-celleinsensitivitet til TGF-p resulterer i akselerert differensiering og autoimmunitet, elementer som kan være nødvendige for å bekjempe selv-antigen-uttrykkende tumorer i en tolerant vert. De immunosuppressive effekter av TGF-P er også involvert i en subpopulasjon av HIV-pasienter med lavere enn prediktert immunrespons basert på deres CD4/CD8 T celle-tellinger (Garba, et al. J. Immunology (2002) 168: 2247-2254). Et TGF-P nøytrali-serende antistoff var i stand til å reversere effekten i kultur, og antyder at TGF-P signalerende inhibitorer kan være brukbare ved reversering av immunsuppresjonen som er tilstede i dette subpopulasjonen av HIV-pasienter.
Under de tidligste trinn av carcinogenesen kan TGF-(31 virke som en potent tumor-suppressor og kan mediere virkningene av visse kjemopreventive midler. På et visst punkt under utviklingen og progresjonen av malignante neoplasmer, synes imidlertid tumorceller å unnslippe fra TGF-P-avhengig vekstinhibering parallelt med opptredenen av bioaktiv TGF-P i mikroomgivelsene. De duale tumorsuppresjons/tumorpromosjons-roller for TGF-P er tydeligst belyst i et transgenisk system som overuttrykker TGF-P i keratinocytter. Mens de transgene var mer resistente mot dannelsen av benigne hud-lesjoner, ble hastigheten for metastatisk konvertering i de transgeniske økt dramatisk (Cui, et al (1996) Cell 86(4):531-42). Produksjonen av TGF-pl hos malignante celler i primærtumorer synes å øke med fremskredne trinn av tumorprogresjon. Studier i mange av de vesentlige epiteliale cancere antyder at den økte produksjonen av TGF-P ved humancancere opptrer ved et relativt sent evenement under tumorprogresjonen. Videre utstyrer denne tumor-assosierte TGF-P tumorcellene med en selektiv fordel og fremmer tumorprogresjon. Effektene av TGF-p på celle/celle og celle/stromainteraksjoner resulterer i en større propensitet for invasjon og metastase. Tumor-assosiert TGF-P kan tillate tumorceller å unnslippe fra immunovervåkingen fordi er en potent inhibitor av den klonale ekspansjon av aktiverte lymfocytter. TGF-P er også påvist å inhibere produksjonen av angiostatin. Cancerterapeutiske modaliteter som strålingsterapi og kjemoterapi induserer produksjonen av aktivert TGF-P i tumoren, og selekterer derved utvekst av malignante celler som er resistente mot TGF-P vekstinhibitoriske effekter. Således øker disse anticancerbehandlingene risikoen og fremskynder utviklingen av tumorer med økt vekst og invasivitet. I denne situasjon kan midler som sikter på TGF-P-mediert signaltransduksjon være en meget effektiv, terapeutisk strategi. Resistensen av tumorceller til TGF-P er vist å negere meget av de cytotoksiske effektene for strålingsterapi og kjemoterapi og den behandlingsavhengige aktivering av TGF-P i stroma kan sågar være skadelig, da den kan gjøre mikroomgivelsene mer tilbøyelige til tumorprogresjon og bidra til vevsskade som fører til fibrose. Forbindelsene er brukbare for behandling av cancer og andre sykdomstilstander som er påvirket av TGF-p ved å inhibere TGF-p hos en pasient som trenger dette ved administrering av forbindelsen(e) til pasienten. TGF-P vil også være brukbar mot aterosklerotiske (T.A. McCaffrey: TGF-Ps and TGF-P Receptors in Atherosclerosis: Cytokine and Growth Factor Reviews 2000,11,103-114) og Alzheimers (Masliah, E.; Ho, G.; Wyss-Coray, T.: Functional Role of TGF-p in Alzheimer's Disease Microvascular Injury: Lessons from Transgenic Mice: Neurochemistry International 2001, 39, 393-400) sykdommer.
FARMASØYTISKE PREPARATER
Preparatene ifølge oppfinnelsen er terapeutisk effektive mengder av TGF-P antagonister som angitt ovenfor. Forbindelsene kan formuleres med vanlige eksipienter, diluenter og bærere, og presses til tabletter, eller formulerte eliksirer eller oppløsninger for hensiktsmessig oral administrering, eller administreres ved intramuskulær, intravenøs vei. Forbindelsene kan administreres transdermalt og kan formuleres som doserings-former for opprettholdt frigivning og lignende.
Fremgangsmåten for behandling av en human pasient inkluderer administrering av TGF-P antagonistene. TGF-P antagonistene formuleres i formuleringer som kan administreres oralt eller rektalt, topisk, parenteralt, for eksempel ved injeksjon, eller ved kontinuerlig eller diskontinuerlig intra-arteriell infusjon, i form av, for eksempel, tabletter, pastiller, sublinguale tabletter, lukteposer eller lignende, eliksirer, geler, suspensjoner, aerosoler, salver, for eksempel, inneholdende 1 til 10 vekt-% av den aktive bestanddel i en egnet base, myk- eller hardgelatinkapsler, suppositorier, injiserbare oppløsninger og suspensjoner i fysiologisk akseptable media, og sterile, pakkede pulvere adsorbert på et bærermateriale for fremstilling av injiserbare oppløsninger. Fordelaktig for dette formål kan preparatene tilveiebringes i enhetdose-former, og særlig inneholder hver enhetsdose fra rundt 5 til 500 mg (fra rundt 5 til 50 mg når det gjelder parenteral eller inhaleringsadministrering, og fra rundt 25 til 500 mg når det gjelder oral eller rektal administrering) av forbindelsene. Doser fira rundt 0,5 til rundt 300 mg/kg pr. dag, fortrinnsvis 0,5 til 20 mg/kg, aktiv bestanddel kan administreres selv om det selvfølgelig er klart at mengden av forbindelsen som virkelig administreres, bestemmes av en lege, i lys av alle relevante omstendigheter inkludert tilstanden som behandles, valget av forbindelse som skal administreres og valget av administreringsvei, og derfor er de foretrukne doseområder ovenfor ikke ment å begrense oppfinnelsen ramme på noen måte.
Formuleringer som er brukbare for separat administrering av TGF-J3 antagonistene vil vanligvis bestå av minst en forbindelse valgt fra forbindelsene som er spesifisert her blandet med en bærer, eller fortynnet med en bærer, eller innelukket eller innkapslet av en inntagbar bærer i form av en kapsel, pose eller lignende, et papir eller annen beholder, eller en éngangsbeholder som en ampulle. En bærer eller diluent kan være fast, halvfast eller flytende, og tjene som bærer, eksipient eller medium for aktivt, terapeutisk middel. Noen eksempler på diluenter eller bærer som kan benyttes i de farmasøytiske preparater ifølge oppfinnelsen er laktose, dekstrose, sukrose, sorbitol, mannitol, propylenglykol, flytende parafin, hvit myk parafin, kaolin, dampet silisium-dioksid, mikrokrystallinsk cellulose, kalsiumsilikat, silika, polyvinylpyrrolidon, cetostearyl alkohol, stivelse, modifiserte stivelser, acacia gummi, kalsiumfosfat, kakao-smør, etoksylerte estere, teobromoljer, arakisol, alginater, tragakant, gelatin, sirup, metylcellulose, polyoksyetylensorbitanmonolaurat, etyllaktat, metyl- og propyl-hydroksybenzoat, sorbitantrioleat, sorbitansesquioleat og oleylalkohol og drivmidler som triklormonofluormetan, diklordifluormetan og diklortetrafluorethan. Når det gjelder tabletter, kan et smøremiddel innarbeides for å forhindre klebing og binding av de pulverformige bestanddeler i former og stanser i tabletteringsmaskinen. For slike formål kan det benyttes for eksempel aluminium-, magnesium- eller kalsiumstearater, talkum eller mineralolje.
Foretrukne, farmasøytiske former ifølge oppfinnelsen er kapsler, tabletter, suppositorier, injiserbare oppløsninger, kremer og salver. Spesielt foretrukket er formuleringer for inhaleringsanvendelse, som aerosol, for injeksjon og for oralt inntak.
