Description
Oppfinnelsen vedrører en fremgangsmåte til fremstilling av hirudinderivater som oppviser forsinket virkning.
Hirudiner er f.eks. kjent fra EP-A 142.860, EP-A 158.564, EP-A 158.986, EP-A 168.342, EP-A 171.024, EP-A 193.175, EP-A 200.655, EP-A 209.061, EP-A 227.938, DE 34 45 517 Al, DE 38 05 540.6 og Chang, FEBS, Bd. 164 (1983) 307. Chang viste bl.a. at C-terminalforkorting påvirker sterkt den anti-trombotiske virkning av hirudin. Også betydningen av hirudin for anti-koagulasjonsterapi blir beskrevet (f.eks. P. Walsmann og F. Markwardt; Pharmazie, 36 (1981) 653). Følgelig inhiberer den spesifikt trombin, men er ellers farmakologisk inert, dvs. uønskede bivirkninger er til nå ikke blitt fastslått.
Den relativt korte oppholdstid av hirudin i dyre- eller menneskekropper kan ansees som uheldig når det gjelder den medisinske anvendelsen som tromboseprofylaktikum (Richter et al., Haematol. 115 (1988) 64; Markwardt et al., Pharmazie 43
(1988) 202).
Til grunn for oppfinnelsen liggerdet å finne en fremgangsmåte for å fremstille et hirudinderivat som oppviser en lengre halveringstid eller som har ubetydligere eliminering.
Denne oppgave blir overraskende løst gjennom et hirudin-derivat, dannet av hirudin eller dets fysiologiske anvende-lige salt og en bærer.
Som hirudiner er eksempelvis de forbindelsene som er beskrevet i de siterte litteraturhenvisningene spesielt de i EP-A 171.024, EP-A 158.986, EP-A 209.061 og DE 38 05 540.6 beskrevne forbindelsene som f.eks.

Disse hirudinene kan fremstilles av en fagmann etter allment kjente metoder innenfor peptidkjemi, se f.eks. Houben-Weyl, Methoden der organischen Chemie, bind 15/2, fortrinnsvis ved hjelp av fastfasesyntese som beskrevet i f.eks. B. Merifield, J.Am.Chem.Soc. 85, 2149 (1963) eller R.C. Sheppard, Int. J. Peptide Protein Res. 21, 118 (1983) eller gjennom likeverdige kjente metoder. Alternativt er de nevnte hirudiner også tilgjengelige innenfor fagområdet gjennom gentekniske metoder.
Videre kommer følgende forandrede hirudiner i betraktning ved hjelp av genteknologiske metoder. Slike er fordelaktige hvorved f.eks. to eller tre naturlig i sekvensen forekommende lysin blir erstattet med en naturlig aminosyre, fortrinnsvis Asg og Asn. Videre også slike hirudiner som er målrettet forandret i henholdsvis N- (fortrinnsvis Lys) og C-terminale (fortrinnsvis Met) forlengelser. Ved en N-terminalforlengelse er i forløpet av sekvensen en forekommende lysin erstattet med en annen naturlig aminosyre, fortrinnsvis Asn og Arg.
Foreliggende oppfinnelse angår en fremgangsmåte for fremstilling av et hirudin-derivat, laget av hirudin eller dens fysiologisk anvendbare salt og en bærer, kjennetegnet ved at hirudin blir omsatt med en aktivert bærer valgt blant oppløselige bærere i form av polysakkarider f.eks. dekstran (MW 20 000-75 000 dt), fortrinnsvis dekstran (MW 70 000 dt), laevane, heparin (MW 6000-20 000 dt) eller "low molecular weight" heparin (MW <6000 dt) , polyetylenglykol (MW 1500-15 000 dt) eller gelatinpartialhydrolysat for eksempel med diisocyanat tverrbundet gelatinpartialhydrolysat eller uoppløselige bærere, som f.eks. sefaroser, agaroser, celluloser eller hydroksymetakrylat, ved en temperatur fra 0°C til 25°C, slik at bæreren blir kovalent bundet til hirudin, hvorved hydroksymetylert polystyrol-harpiks og CNBr-aktivert sefarose er unntatt som bærer.
Forholdet hirudin til bærer kan variere meget sterkt i hirudinderivatene. Det er avhengig av typen bærere og reaksjonsbetingelsene.
Fremgangsmåten til fremstilling av hirudinderivater er kjennetegnet ved at hirudin blir omsatt med en aktiv bærer ved en temperatur fra 0°C til 25°C, fortrinnsvis ved 4°C.
Hirudinderivatene som blir fremstilt ifølge oppfinnelsen er verdifulle trombin-inhibitorer, som særlig utmerker seg gjennom en forlenget halveringstid, hvilket igjen muliggjør en omfattende anvendelse f.eks. i tromboseprofylaksen.
Således kan hirudin og hirudinderivater eksempelvis fordelaktig bli anvendt ved overflatebelegging av hjerteklaffer, (f.eks. av kunststoff), kunstige blodkar, katetere av bioforlikelige medisinske instrumenter eller av filtre, membraner og f.eks. keramiske materialer, alminnelige hemodialyseapparatur.
Ved bestemmelse og påvisning av relevante blodbestanddeler opptrer problemet at det blodet som skal undersøkes må forbehandles. Dette kan medfører problemer for resultatene av slike tester. Testrør som er belagt med hirudin åpner nye muligheter for den praktiserende lege.
Videre finner hirudiner og hirudinderivater anvendelse i implantasjoner, som blant annet også kan bli anvendt for forsinket frigivelse av virkestoffene, f.eks. osmotiske minipumper, biologisk nedbrytbare mikrokapsler, staver, liposompreparater.
Hirudinderivatene som blir fremstilt ifølge oppfinnelsen viser at hirudin lar seg omsette med høymolekylaere bærer-materialer, hvorved hirudinaktiviteten blir overraskende opprettholdt. Den farmakokinetiske opptreden av hirudin endrer seg fordelaktig. Injiserer man hirudin og dekstran-hirudin i et sammenlignende eksperiment i rotter og måler plasmakonsentrasjonen ved hjelp av en trombinhemmetest, så er en trombinhemming målbar etter en time med ca. 10-ganger lavere mengde av dekstran-hirudin. En ytterligere fordel ligger overraskende i forlengelsen av halveringstiden. Dette utgjør for dekstran-hirudin 6 til 7 timer, mens det for hirudin utgjør 1 til 2 timer (Markwardt et al. Pharmazie 43
(1988) 202).
De etterfølgende eksemplene der isohirudin ('Leu-Thr-' hirudin') som er beskrevet i den tyske patentsøknad 38 05 5406 blir anvendt, vil oppfinnelsen bli belyst nærmere. Det er kjent innenfor fagområdet at alle kjente hirudin-ekvivalenter til det anvendte hirudin kan bli omsatt med en bærer og også de hirudinene som er beskrevet på side 2. Med det kan betingelsene for den enkelte reaksjon endres.
Eksempel 1
Fremstilling av hirudin
Syntese av hirudin foregår eksempelvis i gjærceller, der hirudin blir uttrykt ifølge den tyske patentsøknad P 38 05 540.6 (HOE 88/F 043). Hirudin blir deretter anriket ved HP20-søylekromatografi (tysk patentsøknad P 37 38 541.0; HOE 87/F 337). Etter dialyse og deretter affinitetskromatografi på trombin-sefarose (Walsmann, P.: Pharmazie 36 (1981) 860-861) foregår den endelige rensingen i "reversed phase" HPLC. Syntesen kan også foregå ved hjelp av andre kjente gentekniske eller peptidkjemiske metoder.
Eksempel 2
Anvendelse av hirudin og deretter trombinhemmingstest
Hunnlige og hannlige Wistar-rotter med en kroppsvekt på ca. 300 g blir bedøvet med etyluretan (1,5 g/kg i.p.). Rottene blir i. v. applisert med 1000 AT-U/kg av en hirudinderivat henholdsvis autentisk 'Leu-Thr-hirudin', som var oppløst i fysiologisk koksaltoppløsning. Til blodprøvetaking blir dyrene implantert med et kateter i karotis. Til bestemmelse av anti-trombinaktivitet blir blod fra dyrene tatt ut og behandlet med citrat. 0,1 ml av det behandlede blod blir blandet med 0,2 ml Tris/HCl-buf f er (0,1 mol(l; pH 7,4) og forinkubert ved 37°C. Etter tilsats av 0,1 ml av en trombin-løsning (10 NIH-U/ml) blir koaguleringstiden bestemt ved hjelp av et koagulometer etter Schnitger og Gross. Parallelt med eksperimentet blir det benyttet en kalibreringskurve med referanseplasma, og fra den blir den aktuelle plasma-hirudin—, henholdsvis plasma-hirudinderivatkonsentrasjon avlest.
Eksempel 3
Dekstran- hirudin
'Koblingen' av 'Leu-Thr-hirudin' til dekstran (MW 70 000 dt) foregår etter metoden til Parikh, J. et al. Meth. Enzymol 34
(1974) 77. Til den blir dekstranet aktivert gjennom behand-ling med meta-perjodat ved 4°C og deretter dialysert og lyofilisert. 2 g av det aktiverte dekstran blir deretter oppløst i 285 ml 0,1 mol/l natriumfosfatbuffer (pH 8,8) ved romtemperatur, løsningen blir avkjølt til 4°C, 20 mg hirudin blir tilsatt og inkubert i 14 timer ved 4°C. Deretter blir 75 ml fraskilt og lyofilisert. Etter at det lavmolekylaere buffersaltet blir fjernet ved gelfiltrering (Sephadex G-25, 1 cm x 100 cm) blir løsningen på nytt lyofilisert. Reaksjonen finner sted mellom minst en av de tre lysinene og den aktiverte dekstranen.
Til reduksjon av den dannede Schif f -basen blir 75 mg NaBH4 tilsatt den 75 ml inkuberte løsningen, omrørt i 40 minutter ved 4°C og deretter dialysert mot vann i 24 timer ved 4°C (Membran; Servapor Dialysis Lubing; diameter 10 mm, Serva Feinbiochemica GmbH Heidelberg). Så en gelfiltrering med Sephadex G-25 og en lyofilisering.
Dekstran-hirudin blir renset ved hjelp av trombin-sefarose-affinitetskromatografi tilsvarende eksempel 1. Bestemmelsen av dekstran-hirudin-aktivitet foregår ifølge fremgangsmåten til Griesbach et al. (Thromb. Res. 37 (1985) 347-350). Med den blir hemming av trombin-katalysert spalting av kromozym TH målt. Avhengig av den molare basis tilsvarer de målte aktivitetene den av hirudin. Dekstran-hirudin renset på denne måten blir i.v. applisert i Wistar-rotter ifølge eksempel 2. Etter 1 time er det funnet en målbar konsentrasjon i plasma til rottene. Dette kan i sammenligningseksperimentene bare bli beskrevet med den ca. 10-ganger høyere dose av hirudin. Følger man hirudinkonsentrasjonen i plasma videre, finner man en halveringstid av eliminering til substansen på 6-7 timer. Dette er en tydelig forlengelse i forhold til hirudiner (1-3 timer).
De anvendte forsøksdyrene viste ingen påvirkning i deres tilstand.
Eksempel 4
Sefarose- hirudin
8 mg hirudin blir løst i 2 ml 0,1 M NaHC03 og 0,5 M NaCl med pH = 8,0. 400 mg aktivert CH-sefarose 4 B (Pharmacia) blir svellet etter forskriftene fra fabrikanten. Deretter blir proteinløsningen tilsatt til bæreren og får stå i 1,5 timer ved 23°C. Deretter blir det suget av og immobilisatet får stå 1 time til inaktivering av de gjenværende bindingsgrupper i 0,1 M TRIS, pH = 8,0. Deretter blir bæreren vasket etter forskriftene fra fabrikanten.
Protein-koblingsutbytte utgjør 5$. Den biologiske aktiviteten blir bestemt in vitro etter eksempel 2. Fra det behandlede plasma blir sefarose-hirudin isolert, vasket med en 1 til 1,5 M NaCl-løsning og på nytt anvendt i hemmingstesten. De målte aktivitetene ligger innenfor feilgrensen til første måling.
Claims (5)
1.
Fremgangsmåte til fremstilling av et hirudin-derivat, laget av hirudin eller dens fysiologisk anvendbare salt og en bærer, karakterisert ved at hirudin blir omsatt med en aktivert bærer valgt blant oppløselige bærere i form av polysakkarider f.eks. dekstran (MW 20 000-75 000 dt), fortrinnsvis dekstran (MW 70 000 dt), laevane, heparin (MW 6000-20 000 dt) eller "low molecular weight" heparin (MW <6000 dt), polyetylenglykol (MW 1500-15 000 dt) eller gelatinpartialhydrolysat for eksempel med diisocyanat tverrbundet gelatinpartialhydrolysat eller uoppløselige bærere, som f.eks. sefaroser, agaroser, celluloser eller hydroksymetakrylat, ved en temperatur fra 0°C til 25"C, slik at bæreren blir kovalent bundet til hirudin, hvorved hydroksymetylert polystyrol-harpiks og CNBr-aktivert sefarose er unntatt som bærer.
