Description
19-nordeoxycorticosteron (19-norDOC) er blitt funnet
å utvise meget høyere hypertensiv aktivitet sammenlignet med deoxycorticosteron (DOC) [Funder et al., Endocrinology, 103, 1514 (1978)]. Det er like kraftig som aldosteron til å sti-mulere Na -transport over froskeblæreepitel [Perrone et al.,
Am. J. Physiol., 41, E406 (1981)] og har to til fem ganger styrken av DOC i Na-bibeholdende aktivitet [Kagawa et al.,
Soc. Exp. Biol. Med., 94, 444 (1957)].
19-norDOC er blitt isolert fra rotter med adrenal regenererende hypertensjon (ARH) [Gomez-Sanchez et al., Endocrinology, 105, 708 (1979)] og fra mennesker [Dale et al., Steroids, 37, 103 (1981)]. Forhøyet utskillelse av forbindelsen er blitt rapportert for tre hypertensive rottemodeller:
ARH, spontant hypertensive rotter (SHR) og salt-ømfintlige
Dahl innavlsrotter [Griffing et al., Endocrinology, 121, 645
(1987); Dale et al., Endocrinology, 110, 1989 (1982); Gomez-Sanchez et al., J. Steroid Biochem. , 25, 106 (1986)}. Forhøyede nivåer av urin 19-norDOC er blitt observert for flere klasser av humane hypertensive tilstander [Griffing et al., J. Clin. Endocrinol. Metab., 56, 218 (1983)].
I den biosyntetiske dannelse av 19-norsteroider,
slik som 19-norDOC, er det første trinn den adrenale 19-hydroxylering av et egnet steroid som DOC. Inhiberingen av den biosyntetiske dannelse av 19-norDOC ved inhibering av 19-hydroxylering av DOC vil således tjene til å nedsette nivået av 19-norDOC tilstedeværende i det involverte dyr, og redusere hypertensive effekter som tilskrives nærvær av dette materiale.
Det er blitt vist at 10-(2-propynyl)estr-4-en-3,17-dion (en kjent aromatase-inhibitor og en 19-hydroxylaseinhibitor) retarderer utviklingen av hypertensjon og reduserer nivåene av fritt 19-norDOC i urinen når de administreres til nettopp avvendte SHR-rotter [Melby et al. Hypertension, 10, 484
(1987) ] .
Den foreliggende oppfinnelse angår en analogifremgangsmåte for fremstilling av terapeutisk aktive forbindelser av formelen:
Analogifremgangsmåten ifølge oppfinnelsen er kjennetegnet ved at et 10-(2-propynyl)-20-methoxy-19-norpregna-5,17(20)-dien-21-oat med formelen reduseres med et hydridreduksjonsmiddel under dannelse av den tilsvarende 21-hydroxyforbindelse med formelen
som behandles med syre for å hydrolysere 20-enoletheren, og at den beskyttende gruppe i 3-stillingen fjernes.
Forbindelsen fremstilt ifølge oppfinnelsen er an-vendbar som 19-hydroxylaseinhibitor og antihypertensivt middel. Spesifikt kan den inhiberende aktivitet mot adrenal-19-hydroxylase demonstreres ved in vitro radioenzymatisk be-stemmelse. Testforbindelsen oppløses i buffer/løsnings-
middel ved konsentrasjoner varierende fra 1 nM til 50 nm, og tilsettes deretter til prøverør inneholdende en adrenalmito-chondrial suspensjon, dvs. rotte, hamster, kveg, primat eller menneske, et NADPH-utviklende system, og radiomerket deoxycorticosteron. Prøvekomponenten inkuberes i varierende tids-intervaller ved 25 - 37° C, og reaksjonen stanses. De hydroxy-lerte corticoider [dvs. 19-HO-DOC (19-hydroxydesoxycortico-steron), 18-H0-D0C(18-hydroxydeoxycorticosteron) og cortico-steron] ekstraheres rmB. organisk løsningsmiddel og isoleres ved standard kromatografiske prosedyrer. Inhiberingen av 19-hydroxylase bestemmes fra sammenligning av bufferkontrollprøve-rør med prøverør inneholdende inhibitorforbindelsene. De inhi-bitorkonsentasjoner som fremkaller 50 % inhibering (IC^g), ble bestemt. Under anvendelse av denne test var IC^Q-verdien (tids-avhengig enzyminhibering) observert for 21-hydroxy-10-(2-propynyl)-10-norpregn-4-en-3,20-dion ca. 50 nM, slik at den angitte forbindelse utviser en inhibering istedenfor den som observeres for substratet (DOC) som utviste en IC^q eller Km på 600 nM. Spesielt har 21-hydroxy-10-(2-propynyl-19-nor-pregn-4-en-3,20-dion 12 ganger større aktivitet for 19-hydroxy-lase-aktivt sete enn det naturlige substrat (DOC).
I tillegg ble aktiviteten av foreliggende forbindelse som antihypertensivt middel demonstrert ved følgende test-prosedyre. Spontant hypertensive hannrotter (SHR) i en alder på 4,5 uker ble anvendt. Rottene ble anbragt i metaboliske bur, én rotte pr. bur, og opprettholdt på en diett av vanlig "Purina" rottefor og kranvann i et miljø med konstant temperatur i en 12-timers lys/mørkesyklus. En gruppe på seks rotter mottok daglig subkutane injeksjoner av testforbindelsen, 10 mg/kg kroppsvekt, fremstilt i 5 % ethanol og olivenolje, og
som var lydbehandlet. Syv kontroll SHR ble gitt injeksjoner av bærer. Rottene mottok behandling i flere uker med daglige injeksjoner av testforbindelse i test SHR og av bærer i kontroll
SHR.
Systolisk blodtrykk (SBP) av de bevisste, ustressede dyr ble nedtegnet under anvendelse av en fysiograf koblet fra en halemansjett og fotocelle-transduktor i et lydresistent miljø med konstant natur, idet man startet med 3 ukers behandling. Rottene ble tilvendt prosedyren i løpet av flere opptrenings-perioder. De første pålitelige SBP-målinger ble foretatt i en alder på 7 - 8 uker.
For å oppnå en ønsket effekt slik som en antihypertensiv effekt, kan forbindelsen administreres oralt, parenteralt, f.eks. intramuskulært og subkutant, til en pasient med behov for behandling. Uttrykket pasient er ment å omfatte et varmblodig dyr, for eksempel pattedyr slik som rotter, mus, hunder, katter, hester, griser, kveg, sauer, prima-ter og mennesker. Forbindelsen kan administreres alene eller hensiktsmessig blandet i form av et far-
masøytisk preparat til den pasient som skal behandles. Mengden av administrert forbindelse vil variere med strengheten av til-stande^ og gjentatt behandling kan være ønskelig. For oral og parenteral administrering er mengden av administrert forbindelse, dvs. den effektive antihypertensive mengde, fra 0,1 til 150 mg/kg kroppsvekt pr. dag, og fortrinnsvis fra 1 til 50 mg/ kg kroppsvekt pr. dag. Enhetsdoser for oral eller parenteral administrering kan for eksempel inneholde fra 5 til 250 mg av den aktive bestanddel.
For oral administrering kan forbindelsen formuleres
i faste eller væskeformige preparater, slik som kapsler, piller,
tabletter, pastiller, pulvere, løsninger, suspensjoner eller emulsjoner. De faste enhetsdoseringsformer kan være en kapsel som kan være av vanlig gelatintype inneholdende den aktive forbindelse og en bærer, f.eks. smøremidler og inerte fyll-stoffer slik som lactose, sucrose og maisstivelse.
I en annen utførelsesform kan den aktive forbindelse kompletteres med konvensjonelle tablettbaser slik som lactose, sucrose og maisstivelse, i kombinasjon med bindemidler slik som acacia, maisstivelse eller gelatin, oppbrytende midler slik som potetstivelse eller alginsyrer, og et smøremiddel slik som stearinsyre eller magnesiumstearat.
For parenteral administrering kan forbindelsen administreres som injiserbare doser av en løsning eller suspensjon til forbindelsen i et fysiologisk akseptabelt fortynningsmiddel med en farmasøytisk bærer som kan være en steril væske slik som vann-i-ol j.e, med eller uten tilsetning av et overflateaktivt middel og andre farmasøytisk akseptable adjuvanser. Eksempler på oljer som kan anvendes i disse preparater, er petroleumsoljer og de av animalsk, vegetabilsk eller syntetisk opprinnelse, for eksempel peanøttolje, soyabønneolje og mineralolje. Generelt er vann, saltvann, vandig dextrose og beslektede sukkerløsninger, ethanoler og glykoler slik som propylenglykol eller polyethyleh-glykol, foretrukne væskeformige bærere, i særdeleshet for injiserbare løsninger.
Forbindelsen kan administreres i form av et cutant plaster, en depotinjeksjon eller implantat-preparat som kan formuleres på en slik måte at det muliggjøres en forlenget fri-givelse av den aktive bestanddel. Den aktive bestanddel kan presses til pellets eller små sylindre og implanteres subcutant eller intramuskulært som depotinjeksjoner eller implantater. Implantater kan anvende inerte materialer slik som bionedbryt-bare polymerer og syntetiske silikoner, for eksempel "silastic", silikongummi fremstilt av Dow-Corning Corporation. Ytterligere informasjon med hensyn til egnede farmasøytiske bærere og for-muleringsteknikker finnes i standard-bøker slik som Remington's Pharmaceutical Sciences, Mack Publishing Company, Easton, Pennsylvania.
Det etterfølgende eksempel illustrerer oppfinnelsen.
Eksempel
En løsning av 6,11 g 3,3,17,17-bis-ethylendioxy-10-(2-propynyl)-19-norandrost-5-en i 43 ml diklormethan og 150 ml t-butanol ble behandlet med 0,3 % perklorsyre. Blandingen ble oppvarmet til forsiktig tilbakeløpskokning i 2 timer under om-røring og ble avkjølt til romtemperatur. 2 timer senere ble reaksjonsblandingen helt over i mettet vandig natriumcarbonat-løsning og ble ekstrahert i ether. Etherekstraktet ble vasket med vann og saltvann og ble tørket over magnesiumsulfat. 3,3-ethylendioxy-10-(2-propynyl)-19-norandrost-5-en-17-on ble erholdt etter konsentrering av etherløsningen. Omkrystallisering av det urene produkt fra ethylacetat ga 3,2 g (47 %) av analyttiske rent materiale. Sm.p. 198 200° C. NMR (CDC13) 6 0,98 (s, 3H, 13-CH3); 3,95 (m, 4H, ketal); 5,59 (m, 1H, Hg).<13>C NMR; 220,93 (17-carbonyl). IR (KBr); 3385, 2110, 1735 cm<-1>.
MS: (EI) m/z 354 { lit, 1 %), 99 (100 %) ; CI (CH4) m/z 355 (M+H, 76 %), 99 (100 %). Ytterligere 1,07 g ble erholdt etter konsentrering av modervæskene hvilket bragte det totale utbytte av anvendbart materiale til 78,7 %.
En løsning av 12,4 ml methylmethoxyacetat i 40 ml tetrahydrofuran ble tilsatt i løpet av 5 minutter til en kald (-78° C) løsning av lithiumdiisopropylamid, fremstilt fra 18 ml (125 mmol) diisopropylamin og 12,4 ml (125 mmol) 2,9 M n-butyl-lithium i hexan i 150 ml av samme løsningsmiddel. Løsningen ble omrørt ved -78° C i 45 minutter. En løsning av 5,55 g 3,3-ethylendioxy-10-(2-propynyl)-19-norandrost-5-en-17-on i 50 ml tetrahydrofuran ble deretter dråpevis tilsatt i løpet av 5 - 10 minutter og løsningen ble omrørt i 3 timer ved samme temperatur. 15 ml mettet vandig ammoniumkloridløsning ble deretter dråpevis tilsatt, og blandingen ble helt over i isvann og ekstrahert med
ethylacetat. Ekstraktet ble vasket med saltvann, ble tørket over magnesiumsulfat, filtrert og konsentrert under dannelse av methyl-3 ,3-ethylendioxy-10- (2-propynyl) -17|3-hydroxy-20-methoxy-19-norpregn-5-en-21-oat. 9,31 g av det urene produkt ble filtrert gjennom en kolonne av silicagel og eluert med 1:1 ethylacetat: hexan under dannelse av 5,97 g (83 %) av produkt som en blanding av isomerer. Omkrystallisering fra ethylacetat/hexan ga en enkel isomer som den analyttiske prøve, sm.p. 158 - 159° C. NMR (CDC13); 6 0,94 (s, 3H, 13-CH3); 3,21 (s, 1H, HO); 3,35 (s, 3H, ether CH3); 3,76 (s, 1H, H2Q); 3,80 (s, 3H, ester CH3) ; 3,95 (m, 4H, ketal); 5,53 (m, 1H, Hg). IR (KBr): 3450, 2115, 1745 cm"1. MS: (EI) m/z 458 (Mt, 1,4 %), 99 (100 %); (CI/CH4) m/z 459 (32 %, M+H), 441 (100 %).
En løsning av 5,79 g methyl-3,3-ethylendioxy-10-(2-propynyl)-17|3-hydroxy-20-methoxy-19-norpregn-5-en-21-oat i 95 ml pyridin ble avkjølt til -20° C og ble dråpevis behandlet med 9,5 ml thipnylklorid i løpet av 5 - 10 minutter. Etter omrøring i 45 minutter ved samme temperatur ble løsningen helt over i isvann. Produktet ble ekstrahert i ethylacetat og ekstraktet ble vasket to ganger med saltvann, ble tørket over magensium-sulfat, filtrert og konsentrert under dannelse av 5,5 g (96 %) av urent produkt. Flashkromatografi (20 % ethylacetat/80 % hexan) ga 2,59 g (45 %) methyl-(E)-3,3-ethylendioxy-10-(2-propynyl)-20-methoxy-19-norpregna-5,17(20)-dien-21-oat. Krystallisering fra ethylacetat/hexan ga den analyttiske prøve, sm.p. 188 - 190° C. NMR (CDC13); 5 1,02 (s, 3H, 13-CH3); 3,55 (s, 3H, ether CH3); 3,77 (s, 3H, ester CH3); 3,95 (m, 4H, ketal); 5,54 (m, 1H, H.). IR (KBr); 3300 (2130, 1735 cm<-1>. MS: (EI) + 6 m/z 440 (MT, 12 %), 99 (100 %); (CI/CH4) m/z 441 (M+H, 100 %).
En løsning av 2,34 g methyl-(E)-3,3-ethylendioxy-10-(2-propynyl)-20-methoxy-19-norpregna-5,17(20)-dien-21-oat i 50 ml toluen ble avkjølt til -20° C og ble dråpevis behandlet med 11,7 ml av en 20 % løsning av diisobutylaluminiumhydrid i hexan. Løsningen ble omrørt ved -20° C i 30 minutter. 6 ml vann ble tilsatt og blandingen ble omrørt ved 0° C i 30 minutter, ble helt over i isvann og ekstrahert med 3:1 ether:diklormethan. Ekstraktene ble vasket med saltvann, ble tørket over magnesium-sulf at og konsentrert. 2,06 g residuum ble underkastet flash-kromatograf i og ble eluert med 1:1 ethylacetat/hexan under dannelse av (E)-3,3-ethylendioxy-21-hydroxy-20-methoxy-10-(2-propynyl)-19-norpregna-5,17(20)-dien. Omkrystallisering fra ethylacetat ga den analyttiske prøve, sm.p. 180° C. IR (KBr); 3480, 3290, 2130, 1685 cm"<1>. MS: (EI) m/z 412 (Mit, 7 %) , 99 (100 %); (CI/CH4) m/z 413 (M+H), 395 (100 %).
Til en løsning av 1,15 g (2,79 mmol) (E)-3,3-ethylendioxy-21-hydroxy-2 0-methoxy-10i2-propynyl)-19-norpregna-5,17(20)-dien i 50 ml aceton og 5 ml vann ble tilsatt 0,07 g 10 mol% pyridinium-p-toluensulfonat. Løsningen ble oppvarmet til tilbakeløpskokning i 2 timer, ved hvilket tidspunkt analyse ved tynnsjiktskromatografi indikerte at utgangsmater-±ale ikke er tilstede. Løsningen ble konsentrert og residuet ble tatt opp i ether:diklormethan (3:1). Løsningen ble vasket to ganger med vann og én gang med saltvann og ble tørket over magnesiumsulfat. 3,3-ethylendioxy-21-hydroxy-l0-(2-propynyl)-19-norpregn-5-en-20-on ble erholdt etter konsentrering, som vist ved NMR. Uten ytterligere karakterisering ble materialet oppløst i 50 ml methanol, ble behandlet med 5 ml IN saltsyre og ble omrørt i 24 timer, ved hvilket tidspunkt analyse ved tynnsjiktskromatografi indikerte at ikke noe ketal var tilbake. Løsningen ble konsentrert og residuet ble tatt opp i en blanding av ether og ethylacetat. Løsningen ble vasket suksessivt med vann, mettet vandig natriumbicarbonat og saltvann, og ble tørket over magnesiumsulfat. Kdnsentrering ga 0,98 g (99 %)
av urent materiale fra hvilket 21-hydroxy-10-(2-propynyl)-19-norpregn-4-en-3,20-dion (42 %) ble isolert ved flashkromatografi og krystallisering fra ethylacetat, sm.p. 169,5 - 171° C. NMR (CDC13); <5 0,95 (s, 3H, 13-CH3); 2,02 (t, 1H, HCC, J = 2,8); 3,25 (t, 1H, OH, J = 4,7); 4,19 (dABq, 2H, H21, JQH CH<=>4,7, JAB=19); 5,88 (br s, lH, H4). IR (KBr): 3500 - 3300, 3275, 2130, 1700, 1670, 1650 cm"<1>. MS: (EI) m/z 354 (Mt, 32 %), 323 (100 %); (CI) m/z 355 (M+H, 100 %).
Claims (1)
- Analogifremgangsmåte for fremstilling av terapeutisk aktive forbindelser av formelen:karakterisert ved at et 10-(2-propynyl)-20-methoxy-19-norpregna-5,17(20)-dien-21- oat med formelen reduseres med et hydridreduksjonsmiddel under dannelse av den tilsvarende 21-hydroxyforbindelse med formelen som behandles med syre for å hydrolysere 20-enoletheren, og at den beskyttende gruppe i 3-stillingen fjernes.
