Description
Foreliggende oppfinnelse vedrører fremstilling av
et terapeutisk salt av 1,3-bis(2-karboksykromon-5-yl-oksy)-
propan-2-ol med et antihistamin som har egenskaper som H^-reseptor-antagonist.
Flere anti-histaminer har lenge vært kjent for bruk ved palliativ behandling av allergiske tilstander. Anti-histaminene har imidlertid i de aller fleste tilfeller blitt administrert systemisk, f.eks. som eliksirer, tabletter, kapsler osv.
Videre har de fleste anti-histaminene uønskede bivirkninger,
f.eks. sedativ virkning, hvilket forårsaker at deres bruk i høy dosering ikke anbefales ved f.eks. bilkjøring og lignende.
Dinatriumkromoglykat har også vært foreslått for bruk ved behandling av allergiske tilstander og spesielt av astma og allergisk rhinitis. I motsetning til anti-histaminene har dinatriumkromoglykat blitt benyttet profylaktisk og har generelt ikke blitt anbefalt for bruk for helbredelse, dvs. ved allergiske angrep.
Man har nå funnet at blandinger av dinatriumkromoglykat
med visse anti-histaminer og salter av 1,3-bis(2-karboksykromon-5-yl-oksy)propan-2-ol med disse anti-histaminer, har den fordel at de både er palliative og profylaktiske, er mer effektive,
gir mindre bivirkninger, kan anvendes i lavere doseringer, kan administreres direkte til allergiområdet, har lenger virkning,
er mer stabile, eller har andre ønskede egenskaper sammenlignet med anti-histaminer når disse anvendes alene, videre sammen-
lignet med dinatriumkromoglykat anvendt alene, eller visse andre blandinger når disse testes i forbindelse med bestemte farmakologiske modeller vedrørerende de nasale eller okulare manifestasjoner av allergi.
Ifølge foreliggende oppfinnelse fremstilles et tera-
peutisk aktivt salt av 1,3-bis(2-karboksykromon-5-yloksy)-
propan-2-ol med et antihistamin valgt fra trimeprazin,
cyproheptadin, feniramin, mepyramin, klorfeniramin, brom-
feniramin, dimetinden, karbinoksamin, tripelenamin eller
triprolidin, og denne fremgangsmåte er kjennetegnet ved at man omsetter et salt av 1,3-bis(2-karboksykromon-5-yloksy)-propan-2-ol med et salt av antihistaminet eller 1,3-bis(2-karboksykromon-5-yloky)propan-2-ol med en fri base av antihistaminet.
Triprolidin er foretrukket som antihistamin i fremgangsmåten. Det er foretrukket at det fremstilles et salt som er minst 1% vekt/vol. og fortrinnsvis minst 4% vekt/vol. oppløselig i vann.
Saltet kan fremstilles ved en metateste-prosess, f.eks. ved omsetning av et passende salt, slik som natriumsaltet, av 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol med et egnet salt, f.eks. hydrokloridet, av anti-histaminet. Saltet fremstilles imidlertid fortrinnsvis ved omsetning av den frie syre, 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol med den frie anti-histamin-base, ettersom denne prosess ikke gir et uorganiske salt som biprodukt. Reaksjonen kan utføres i et oppløsningsmiddel som
er inert under reaksjonsbetingelsene. Oppløsningsmidlet er fortrinnsvis et hvori det ønskede salt er oppløselig, f.eks. vann eller en alkanol slik som etanol. Det ønskede salt kan isoleres og renses f.eks. ved krystallisering eller frysetørking.
Saltet tilveiebringes derfor når det
ikke er i oppløsning, f.eks. saltet når det er i vesentlig tørr form, eller når det er i blanding med utilstrekkelig væske, f.eks. vann, til å oppløse alt salt.
Saltet kan, om ønsket, anvendes i forbindelse med ett eller flere andre salter av 1,3-bis(2-karboksykromon-5-yloksy)-propan-2-ol, spesielt dinatriumsaltet derav.
Farmasøytiske preparater kan således f.eks. omfatte en blanding av: (a) ett eller flere av 1,3-bis(2-karboksykromon-5-yloksy)-propan-2-ol, eller et farmasøytisk akseptabelt salt derav, (betegnet kollektivt som "aktiv bestanddel") i kombinasjon med (b) ett eller flere av nevnte anti-histaminer med H^-reseptor-antagonist-egenskaper.
Dinatriumsaltet av 1,3-bis(2-karboksykromon-5-yloksy)-propan-2-ol, som vanligvis er kjent som dinatriumkromoglykat er foretrukket.
Anti-histaminet kan være en hver av de ovenfor angitte eller et farmasøytisk akseptabelt salt derav, f.eks. hydrokloridet, tartratet eller maleatet derav.
Forholdet mellom aktiv bestanddel og anti-histamin i preparatet vil variere med den spesielle aktive bestanddel og anti-histamin som anvendes og det spesielle formål for hvilket preparatet er ment. Når den aktive bestanddel er et salt av 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol med et anti-histamin, kan det være nødvendig å anvende intet eller bare en meget liten del "fritt" anti-histamin (dvs. det basiske anti-histamin alene eller et salt av anti-histaminet annet enn med 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol).
Det er foretrukket at preparatet inneholder bare én aktiv bestanddel og bare ett anti-histamin.
En passende daglig dose aktiv bestanddel for de fleste formål ligger i området 1-100 mg og fortrinnsvis 1-20 mg (målt som dinatriumkromoglykat).
En passende daglig dose anti-histamin vil variere med anti-histaminet og ligger i følgende områder:
En foretrukket daglig dose av anti-histaminet er 0,1-
5 mg og helst i området 0,5-2 mg.
Preparatet kan administreres som oppdelte doser fra 1 til 6,og fortrinnsvis fra 2 til 4 ganger pr. dag. Hver dose kan omfatte en eller flere enhetsdoser.
De spesielle forhold mellom de spesielle aktive be-standdeler og de spesielle anti-histaminer i et preparat ifølge oppfinnelsen, kan beregnes utfra de ovenfor angitte daglige doser. Det er foretrukket å benytte et preparat som inneholder 1000-
0,2 vektdeler, og helst 40-0,5 vektdeler aktiv bestanddel (målt som dinatriumkromoglykat) for hver vektdel anti-histamin. Det er særlig foretrukket å benytte et preparat som inneholder dinatriumkromoglykat og triprolidin i et vektforhold på omkring 20:1.
Det ovennevnte preparat anvendes for behandling av en tilstand i øyet, nesen eller huden, hvilken tilstand har en allergisk komponent, f.eks. allergisk rhinitis som omfatter administrasjon av et salt og/eller blanding som ovenfor angitt, til et enkelt pattedyr, f.eks. menneske, som befinner seg i en slik tilstand. Administrasjonen foretas fortrinnsvis gjennom nesen.
Tilstander i det ytre øyet omfatter vårkatarr (vår-keratoconjunctivitis) og marginal hornhinne-ulcerasjon eller -infiltrasjon. Andre tilstander som kan behandles, omfatter okulare effekter eller høyfeber, "allergiske øyne" hvor. aller-genet er kjent eller ukjent og vår/sommer-conjunctivitis.
Denne sistnevnte betegnelse anvendes på allergiske sykdommer
i øyet og som forekommer om våre og sommeren hvorved et utvendig allergen har innflytelse på sykdommen. Ytterligere tilstander ved øyet som kan nevnes er "irritert øye" eller "ikke-spesifikk conjunctivitis", herpes simplex keratitis og conjunctivitis, herpes zoster keratitis og conjunctivitis, adenovirus-infeksjoner, phlyctenular conjunctivitis, hornhinne-"homograft"-avvisning, trachoma, anterior uvetis og legemiddel-følsomhet.
Tilstander i nesen som kan nevnes er årstidsbestemt rhinitis, f.eks. høyfeber; perennial rhinitis, nesepolypper og allergiske manifestasjoner i nesesvelgrommet.
Dermatoser som kan behandles, omfatter de som inne-bærer hud-mastceller og/eller antilegeme/antigen-reaksjoner og omfatter eksemer og spesielt atypisk eksem, legemiddel-erupsjoner, psoriasis, dermatitis herpetiformis, pemphigus og kromiske hudsår, særlig de som påvirker mennesket i tropiske klimaer.
Det kan foretas sekvens eller samtidig-administrasjon av aktiv bestanddel og anti-histamin, og da er forholdene og doseringene for aktiv bestanddel og anti-histamin som be-skrevet ovenfor med hensyn til blandingene. Det er foretrukket at administrasjonen av aktiv bestanddel og anti-histamin foretas med et mellomrom på omkring 10 minutter.
En farmasøytisk pakning omfatter minst en dose aktiv bestanddel og minst en dose anti-histamin. Dosene er fortrinnsvis enhetsdoser og fortrinnsvis anordnet i pakningen i en spesiell rekkefølge sammen med skrevne eller trykkede ! indikasjoner eller retningslinjer, idet de sistnevnte og pakkemåten er slik at man får retningslinjer med hensyn til
og for å lette inntaket av en dose aktiv bestanddel og en
dose anti-histamin i en spesiell rekkefølge, f.eks.
en dose av det førstnevnte stoff før en dose av det sistnevnte.
Pakningen er fortrinnsvis en forseglet pakning og kan også
omfatte en tube, boks eller kort i eller på hvilket dosene er forpakket. Dosene er fortrinnsvis egnet for nasal
administrasjon, f.eks. spray-preparater for nesen, og
inneholder fortrinnsvis dosene av aktiv bestanddel og anti-hista-
min i de forhold som er angitt ovenfor for preparatene.
For å fremstille passende preparater opparbeides saltet,
den aktive bestanddel og anti-histaminen, enten separat eller som en blanding derav, med et organisk eller uorganisk farma-søytisk akseptabelt tilsetningsmiddel eller hjelpemiddel.
Det foretrekkes preparater som er beregnet for admini- j strering til øyet eller nesen, f.eks. vandige oppløsninger inneholdende f.eks. opptil 10 vekt-% av salter eller blandingen ifølge oppfinnelsen. Preparatene kan også inneholde ett eller flere preserveringsmidler, f.eks. benzylkoniumklorid og/eller fenyletylalkohol. Preserveringsmidlet kan være tilstede i opp-i til ca. 2,0% vekt/vol. og kan f.eks. omfatte 0,00 2-0,05% vekt/vol. benzylkoniumklorid og/eller 0,1-1,0% vekt/vol. fenyletylalkohol, eller 0,0005-0,005% fenylmercuri-nitrat, eller 0,005-0,05% tiomersal, eller 0,2-1,0% klorbutol. Preparatet kan også inneholde et sekvesteringsmiddel, f.eks. dinatriumedetat. Sekvesteringsmidlet kan være til stede i en i i i
0,005-0,2 vekt/vol. Preparatets pH-verdi kan reguleres ved anvendelse av ett eller flere buffermidler, f.eks. mono-natriumfosfat eller dinatriumfosfat. Preparatets pH-verdi kan reguleres slik at det er forenelig med fysiologiske oppløsninger og/eller for å forbedre bestanddelenes oppløselighet eller preparatets stabilitet. Preparatet kan også inneholde et middel som øker dets viskositet, f.eks. visse cellulosederivater eller en polyvinylalkohol.
Preparatene som skal administreres til øyet eller nesen er fortrinnsvis isotoniske.
Det benyttes alternativt pulverformige preparater
for administrasjon til nesen og fortrinnsvis omfattende fine partikler av anti-histamin og/eller av saltet av 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol.
Den aktive bestanddel og anti-histaminet kan, om ønsket, anvendes i en spesifikk form, f.eks. med en midlere massediameter på under 10 ym.
Oppfinnelsen illustreres av følgende eksempler.
Eksempel 1
Klorfeniraminsalt av 1, 3- bis( 2- karboksykromon- 5- yloksy) propan- 2- ol
1- (p_-klorf enyl) -1- (2-pyridyl) -3-N,N-dimetylpropylamin-maleat (3,7 deler) ble oppløst i vann, gjort basisk under anvendelse av 10% NaOH, ekstrahert i eter og tørket (Na2S04). Tørkemidlet og esteren ble fjernet og vann ble tilsatt fulgt av 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol (2,3 deler).
Denne blanding ble omrørt i 2 timer, vasket med eter og spor av eteren ble fjernet før produktet ble isolert fra den vandige opp-løsning ved frysetørking.
Analyse:
Funnet: C 60,0; H 5,9; N 4,9 Beregnet for c23Hi6°ii<*>(<C>16<H>19C1N2)*8H20: C 60'5' H 6'5' N 5'1%
Eksempel 2
Triprolidinsalt av 1, 3- bis( 2- karboksykromon- 5- yloksy) propan- 2- ol 1,2 g trans-2-[3-(1-pyrrolidinyl)-l-p_-tolypropenyl]-
pyridinhydroklorid ble oppløst i 50 ml vann og gjort basisk med 10% NaOH. Den frie basen ble ekstrahert i eter og tørket over magnesiumsulfat. Filtrering og inndampning av eteren ga den frie basen. Til denne ble det tilsatt et lite overskudd 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol (0,9 g) og vann (20 ml). Etter omrøring i \ time ble uoppløselige stoffer fjernet ved filtrering og saltet ble isolert fra filtratet ved frysetørking. Saltet krystalliserte fra aceton, smp. ubestemt - mørkfarging
og dekomponering over 200°C.

Claims (2)
1. Analogifremgangsmåte for fremstilling av et terapeutisk aktivt salt av 1,3-bis(2-karboksykromon-5-yloksy)-propan-2-ol med et antihistamin valgt fra trimeprazin, cyproheptadin, feniramin, mepyramin, klorfeniramin, bromfeniramin, dimetinden, karbinoksamin, tripelen-amin eller triprolidin, karakterisert ved at man omsetter et salt av 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol med et salt av anti-histaminet eller 1,3-bis(2-karboksykromon-5-yloksy)propan-2-ol med en fri base av anti-histaminet.
2. Analogifremgangsmåte ifølge krav 1, karakterisert ved at det fremstilles et salt som er minst 1% vekt/vol. og fortrinnsvis minst 4% vekt/vol. oppløselig i vann.
