Description
Nærværende oppfinnelse angår en fremgangsmåte for fremstilling
av antitrombin fra blod eller blodprodukter ved adsorpsjon på gelmatrise som inneholder sulfaterte polysakkarider.
Blodets koagulering er en komplisert prosess med en mengde del-tagende komponenter. Dannelse av koagel kan utldses av en rekke forskjellige grunner, den vanligste er dog mekanisk skade på ett eller flere blodkar. Initieringen av koaguleringen skjer da dels ved kontaktaktivering av visse faktorer i blodet dels ved at vevsaktivatorer lekker ut fra det skadede sted.--Den aggrega-sjon av trombocyter som skjer samtidig påvirker også initieringen. En rekke reaksjoner startes, som til slutt forer frem til dannelsen av et koagel på det skadede sted. Ett av de siste og viktigste trinn er dannelsen av en fibrinklump ved at et ved koaguleringsprosessen dannet enzym, trombin, hydrolytisk spalter av to peptidstykker fra i blodet forekommende fibrinogen under dannelse av et modifisert fibrinogen, som hurtig aggregerer til et nettverk, klump.
For regulering av blodets koaguleringstilboyelighet og for å for-hindre at lokal koagulering sprer seg og gir total intravaskulær koagulering, forekommer det i blodet flere substanser som virker inhiberende på koaguleringen. En av de viktigste av disse er antitrombin, et protein som bindes til trombin og inaktiverer dette. På slik måte inaktivert trombin kan etterpå ikke lenger angripe fibrinogen, hvorved klumpdannelsen forhindres.
Patologisk senkede antitrombin-nivåer er blitt iakttatt ved en rekke tilstander forenet med oket risiko for blodproppsdannelse f.eks. etter storre operasjoner. Det kan av gode grunner antas at antitrombin-terapi kan komme til å bli av stor verdi i be-handlingen av slike tilfeller. Senkede antitrombinnivåer har også vært påvist i forbindelse med anvendelsen av visse steroid-legemidler.
Antitrombin har tidligere bare kunnet fremstilles i spormeng-
der og er folgelig lite kjent. U. Abildgaard, Scand J Clin Lab Invest, bind 19, (1967), side 190, bind 21 (1968) side 80, til-passet en kombinasjon av gelfiltrering og gradient ultrasentri-fugering komplettert med preparativ skiveelektroforese. F.C. Monkhouse, Methods Enzymology, bind 19 (1970), side 915, ut-nytter aluminiumhydroksyd for adsorpsjon, fulgt av ionebytter-kromatografi og preparativ elektroforese. Disse forfattere har anvendt konvensjonelle biokjemiske metoder. Det er imidlertid usedvanlig vanskelig med konvensjonelle metoder å skille antitrombin fra serumalbumin på grunn av overensstemmelse i iso-elektrisk punkt og molekylstorrelse.
Felles for tidligere kjente metoder er et bemerkelsesverdig
lavt utbytte, 1 - 2 %. Den nedenfor beskrevne metode tillater utvinning av antitrombin med utbytter over 90 %.
I forbindelse med utforte forsok på å isolere forskjellige koaguleringsfaktorer fra blodplasma ved adsorpsjon på gel som inneholder sulfaterte polysakkarider har det overraskende vist seg at visse koaguleringsfaktorer bindes på vedkommende geler.Videre separasjon av materialet ved gelfiltrering tillater isolering av en komponent med molekylvekt omkring 65 000 med sterkt uttalt koa-guleringsinhiberende effekt. Undersokelser av dette protein med fysikalsk-kjemisk og immunologisk teknikk viser identitet med det i litteraturen beskrevne antitrombin III. Forsok med gradienteluering av gelen i soylen viser at antitrombin direkte kan oppnås godt separert fra andre proteiner i utgangsmaterialet.
Framgangsmåten ifolge oppfinnelsen karakteriseres ved at sulfaterte polysakkarider anvendes som adsorpsjonsmiddel fulgt av en eluering med en pufferlosning med pH mellom 5,5 og 9 og en ionestyrke mellom 0,5 og 1,5 M.
For å oppnå storre mengder antitrombin har det vist seg hen-siktsmessig å adsorbere dette fra Cohn (metode 6) fraksjon IV
ved tilsetning av gel til en opplosning av denne fraksjon.
Gelen kan etter adsorpsjon filtreres av, vasket og elueres.
Et helt rent antitrombin oppnås etter gelfiltrering av eluatet
pa "Sephadex R G150". (Pharmacia Fine Chemicals, Uppsala, Sweden).
Eksempler på egnede geler for anvendelse som adsorbsjonsmiddel:
1) tverrbundet dextransulfat, 2) tverrbundet dextransulfat-agaros, 3) tverrbundet heparin, 4) tverrbundet heparin-agaros,
og 5) tverrbundet kondroitinsulfat-agaros. De tverrbundne enkle geler 1) og 3) fremstilles ved tilsetning av bromcyan til en opplosning av polysakkarider. Ved å holde pH omkring 11 oppnås tverrbindinger mellom molekylene og gel-dannelse. De blandede gelene 2), 4) og 5) oppnås ved å til-
sette sulfatert polysakkarid til en agarosgel "Sepharose 4B"
(en i vann svellbar matrise som hovedsakelig består av agarose, Pharmacia Fine Chemicals, Uppsala, Sweden) samt deretter å til-sette bromcyan ved pH 11. Disse typer geler er lettere å be-handle og har bedre flyteegenskaper enn de enkle gelene.
Folgende eksempler belyser oppfinnelsen:
EKSEMPEL 1
100 ml dextransulfatopplosning (20 mg/ml) ble tilsatt 5 g BrCN, pH hevet til 11,0 med 5 M NaOH i lopet av 7 min. og blandingen fikk henstå under omrdring over natten. En hvit grynaktig gel-masse ble dannet. Massen ble pakket til en liten soyle 5 mm 0 x 8 cm og bringes i likevekt med 0,02 M "Tris" (tris(hydroksymetyl)-aminometan),0,01 M citrat, 0,15 M NaCl puffer, pH 8,5. 2 ml normalplasma fikk passere gjennom soylen. Plasmaen hadde etter passa-sje av soylen misten sin koaguleringsevne .Soylen ble vaslet med den opprin-nelige puffer og de sorbert ved trinnvis okning av saltkonsentrasjonen til 1 M NaCl. Desorbatet inneholdt materiale som etter dia-
lysa mot fosfatpuffer forlenget normalplasmaens koagulerings-tid etter rekalsifisering fra 3 min. til 45 min. og som immunologisk inneholdt antitrombin sammen med et lipoprotein.
EKSEMPEL 2
Adsorpsjon fra plasma mad tverrbundet dextransulfat-agaros. Gradienteluering.
Dextransulfatet ble tilberedt ved å blande 30 ml dextransulfatopplosning (15 mg/ml), 50 ml 4% perlepolymarisert agaros og 1 g BrCN ved pH 11. Blandingen fikk henstå under omroring i 7 minutter og pH holdes konstant ved kontinuerlig NaOH-tilsetning. Deretter ble alkalitilsetningen stoppet og pH sank i lopet av 5 minutter til 8,5. Omroringen ble fortsatt i romtemperatur over natten, hvoretter gelen ble vasket. En kolonne ble pakket med den oppnådde dextransulfatagarosen. I adsorpsjonstrinnet passerte 3 ml normal plasma fortynnet 1:1 med pufferen (0,02 M"Tris',' 0,01 M citrat, 0,15 M NaCl pH 3,5) gjennom soylen. Plasmafraksjonen ble konsentrert etter
passasjen til utgangsvolumet og ble provet med hensyn til koagulerbarhet. Plasmåen hadde ved passasjen gjennom soylen mistet sin evne til å koagulere og savnet bl.a. koagulerings-faktorene VIII og IX. Adsorbert materiale ble desorbert med 0,02 m"Tris"og 0,01 M citrat, pH 7,3 i en saltgradient fra 0,15 til 1,5 M NaCl. Desorbatet tilsvarte ca. 2% av inngangs-materialet og bestod hovedsakelig av to komponenter, en lipo-proteinfraksjon av pre-beta-type og antitrombin III. Identifi-seringen skjedde med immunologisk og fysikalsk-kjemisk teknikk.
EKSEMPEL 3
Adsorpsjon fra plasma med tverrbundet dextransulfat-agaros. Trinnvis eluering. Gelfiltrering.
På samme måte som i eksempel 2 ble en adsorpsjon utfort på dextransulfatagaros gel. Desorpsjonen ble utfort i ett trinn med 0,02 M"Tris',' 0,01 M Na-citrat, 1 M NaCl, pH 7,3. Desorbatet ble konsentrert og gelfiltrert på "Sephadex ® G150 i fysiologisk fosfatpuffer. Rent antitrombin ble oppnådd i en fraksjon godt skilt fra lipoprotein-fraksjonen.
EKSEMPEL 4
Adsorpsjon fra plasma med tverrbundet heparin-agaros.
Heparin-agaros ble tilberedt som beskrevet i eksempel 2 for dextransulfat-agaros. I stedet for dextransulfat ble anvendt 30 ml heparinopplosning (5 000 IE/ml). Gelen ble pakket til en kromatografikolonne og forsaket ble utfort som i eksempel 2. Forsoksresultåtene stemte overens med de i eksempel 2 angitte.
EKSEMPEL 5
Adsorpsjon på dextransulfat-agaros av plasma-proteinmateriale fra Cohn (metode 6) fraksjon IV. Trinnvis eluering. Gelfiltrering.
Immunologisk kan påvises at antitrombin finnes i Cohn (metode 6) fraksjon IV. 135 g fraksjon IV pasta ble opplost i 4 liter adsorpsjonspuffer og 500 ml dextransulfat-agaros ble tilsatt. Blandingen ble omrort langsomt i kulde i 1 time. Deretter ble opplosningen sokt av fra gelen og denne vaskes omhyggelig med adsorpsjonspuffer. Gelen ble deretter pakket i et ionutvéksler-ror og eluert med et trinnvis okende NaCl-innhold (0,5 M, 1 M, 1,5 M) " Tris-ilpuffer, pH 7,3. Desorbatet som inneholdt lipoprotein og antitrombin, ble konsentrert ved ultrafiltrering og renset ved gelfiltrering på "Sephadesw G150". Identifise-ring og renhetskontroll ble utfort med immunologisk og fysikalsk kjemisk teknikk.
EKSEMPEL 6
Kondroitinsulfat-agaros gel ble tilberedt ved å blande 250 mg kondroitinsulfat VI, 40 ml perlepolymerisert 4% agaros og 1 g BrCM ved pH 11. Blandingen fikk henstå ved pH 11 under omroring i 7 min. pH ble holdt konstant ved kontinuerlig lut-tilsetning. Bare en mindre mengde alkali var nbdvendig og etter 7 min. ble pH forsiktig senket til 8,5 ved tilsetning av eddiksyre. Omrorxngen ved pH 8,5 ble fortsatt i romtemperatur over natten, hvoretter gelen ble vasket. En kolonne ble pakket med gelmaterialet og ved samme forsøksbetingelser som foran beskrevet passerte 3 ml normalplasma fortynnet 1:1 gjennom soylen. Antitrombin kunne deretter elueres fra gelen ved eluering med samme elueringspuffer som i eksempel 1.
Claims (3)
1 Fremgangsmåte for fremstilling av antitrombin fra Hod eller blodfraksjoner, karakterisert ved at sulfaterte polysakkarider anvendes som adsorpsjonsmiddel fulgt av en eluering med en pufferlosning med pH mellom 5,5 og 9 og en ionestyrke mellom 0,5 og 1,5 M.
2. Fremgangsmåte ifolge krav 1, karakterisert ved at sulfatert polysakkarid foreligger matrisebundet eller
-•" på annen måte er gjort uopploselig.
3. Fremgangsmåte ifolge krav 1 og 2, karakterisert ved at det som adsorpsjonsmiddel anvendes med bromcyan- tverrbundne, sulfaterte polysakkarider slik som dextransulfat, dextran-agaros eller heparin-agaros.
