Abstract

ANTECEDENTES DE LA INVENCIÓN
CAMPO DE LA INVENCIÓN
La presente invención se refiere a instrumentos de engrapado y de corte quirúrgicos adaptados para utilizarse en el diagnóstico y terapia de patologías tratadas mediante resección engrapada. Más particularmente, la invención se refiere a un mecanismo de bloqueo para utilización en conjunto con instrumentos quirúrgicos de engrapado y corte.
DESCRIPCIÓN DE LA TÉCNICA ANTERIOR
Los instrumentos quirúrgicos de engrapado y corte se utilizan comúnmente en el diagnóstico y tratamiento de patologías tratadas por resección engrapada. Los instrumentos quirúrgicos de engrapado y corte proporcionan un mecanismo para extender la explotación transluminal de dispositivos de sutura mecánicos introducidos por medio del canal anal, boca, estómago y accesos de servicio. Aunque los instrumentos quirúrgicos de engrapado y corte se utilizan más comúnmente con patologías rectales, los instrumentos quirúrgicos de engrapado y corte pueden utilizarse en una variedad de entornos. Con el tiempo, los instrumentos quirúrgicos de engrapado y corte han sido desarrollados. Es otro objetivo de la presente invención proporcionar una engrapadora quirúrgica lineal en donde la palanca de bloqueo incluye una superficie superior expuesta desde el hueco y una superficie inferior que se asienta dentro del hueco. Otros objetivos y ventajas de la presente invención serán evidentes a partir de la siguiente descripción detallada cuando se observe en conjunto con los dibujos anexos, que establecen ciertas modalidades de la invención.
BREVE DESCRIPCIÓN DE LOS DIBUJOS

DESCRIPCIÓN DE LAS MODALIDADES PREFERIDAS

Durante el disparo de la engrapadora quirúrgica lineal 20, la lengüeta de la compuerta giratoria 182 se hace girar a su segunda posición sustancialmente paralela al eje longitudinal de la engrapadora quirúrgica lineal 20 y se remueve de la superficie superior 188 de la palanca de bloqueo 184 permitiendo que el resorte 194 fuerce la palanca de de bloque 184 hacia arriba en su posición bloqueada. Las figuras 6 y 7 muestran el mecanismo de bloqueo 180 al inicio de su ciclo. La lengüeta de la compuerta giratoria 182 se encuentra en su primera posición sustancialmente transversal al eje longitudinal del dispositivo de engrapado quirúrgico 20. La lengüeta de la compuerta giratoria 182 se asienta directamente sobre la palanca de bloqueo 184 manteniendo la palanca del bloqueo 184 dentro del hueco 196 de manera que no interfiera con el disparo de la engrapadora quirúrgica lineal 20, y particularmente, la barra de disparo 43. Cuando el mecanismo de aproximación se activa como se muestra en las figuras 8 y 9, es decir, la activación del activador de cierre 26, el alojamiento del cartucho 121 se hace avanzar a la posición totalmente cerrada. Sin embargo, la lengüeta de la compuerta giratoria 182 permanece sustancialmente transversal al eje longitudinal de la engrapadora quirúrgica lineal 20, manteniendo la palanca de bloqueo 184 dentro del hueco 196 en su posición desbloqueada. Con relación a las figuras 10 y 11 , la activación del ensamble de transmisión de disparo hace avanzar la barra de disparo 43 a través de la lengüeta de la compuerta giratoria 182. El movimiento de la barra de disparo 43 a través de la lengüeta de la compuerta giratoria 182 ocurre a pesar de la posición del accionador 131 o la terminación de la carrera de explosión. Este movimiento de la barra de disparo 43 a través de la lengüeta de la compuerta giratoria 182 mueve la lengüeta de la compuerta giratoria 182 de su primera posición evitando que la palanca de bloqueo cargada por resortes 184 se mueva hacia arriba a su segunda posición sustancialmente paralela al eje longitudinal de una engrapadora quirúrgica lineal 20. Cuando se encuentra en su segunda posición, la lengüeta de la compuerta giratoria 182 ya no se coloca más por arriba de la superficie superior 188 de la palanca del bloqueo 184 y no bloquea más el movimiento ascendente de la palanca de bloqueo 184 con base en el impulso del resorte 194 colocado entre la superficie inferior 190 de la palanca de bloqueo 184 y el hueco 196. Después de que se mueve la lengüeta de la compuerta giratoria 182 a su segunda posición y la lengüeta de la compuerta giratoria 182 ya no mantiene más la palanca de bloqueo 184 en su posición desbloqueada, la barra de disparo 43 momentáneamente se asienta por arriba de la palanca de bloqueo 184 manteniéndola dentro del hueco 196. A medida que el ensamble de transmisión de disparo, en particular, la barra de disparo 43, se repliega, la palanca de bloqueo 184 no se mantiene más dentro del hueco 196 por la lengüeta de la compuerta giratoria 182 o la barra de disparo 43, y la palanca de bloqueo 184 se mueve en un posición bloqueada bajo la fuerza del resorte 194. Mientras se encuentra en esta posición, la palanca de bloqueo 184 bloqueará cualquier intento adicional por activar los medios de disparo (véase figuras 12 y 13). Mientras la palanca de bloqueo 184 bloquea el movimiento hacia atrás del accionador 131 , la barra de disparo 43 se repliega por último a su posición proximal. Después de que se libera la palanca de bloqueo 184 en su posición ascendente cuando la barra de disparo 43 se mueve hacia su posición distal, existe una superficie inclinada 185 en la cara distal de la palanca de bloqueo 184 que actúa como una leva que permite que la barra de disparo 43 la empuje hacia "abajo" fuera del camino durante el repliegue posterior de la barra de disparo 43. Con relación a la figura 14, aunque se describe una palanca de bloqueo lineal de conformidad con una modalidad preferida de la presente invención, la palanca de bloqueo 184' también puede ser una palanca de bloqueo giratoria 184' que se monta en un extremo 184a' para rotación en pivote alrededor del montaje de manera que la palanca de bloqueo giratoria 184' ascendente con resorte desviado 194' aún funcione de la misma manera como se describe con referencia a la modalidad preferida descrita con referencia a las figuras 6 a 13. De acuerdo con una modalidad adicional de la presente invención, y con referencia a las figuras 15, 16 y 17, la lengüeta de la compuerta giratoria no se asienta constantemente por arriba de la palanca de bloqueo antes del disparo como se describió anteriormente con referencia a las figuras 6 a 13. El mecanismo de bloqueo del dispositivo disparado de la presente 280 emplea una compuerta giratoria 282 asegurada en pivote al módulo del cartucho sin utilizar 220. De acuerdo con una modalidad preferida, el módulo del cartucho 220 incluye un alojamiento del cartucho 223 y la compuerta giratoria 282 se asegura a una pared lateral 221 del módulo del cartucho 220. Con esto en mente, y como se apreciará con base en la siguiente descripción, la pared lateral 221 del módulo del cartucho 220 se proporciona con un hueco 222 formado y dimensionado para recibir la compuerta giratoria 282 a medida que gira hacia la pared lateral 221 permitiendo que la barra de disparo 243 pase a través de la misma. La compuerta giratoria 282 se forma y dimensiona para rotación para bloquear el movimiento adicional del accionador disparado 231 y como consecuencia evitar el uso del módulo del cartucho previamente disparado 220. Con esto en mente, la compuerta giratoria incluye una superficie que mira hacia el frente 288 que tiene una sección de corte 286 adyacente a su extremo distal y una superficie sesgada 289 adyacente a su extremo proximal. Una superficie plana 291 se coloca entre la sección de corte 286 y la superficie sesgada 289. La configuración de pre-disparo del mecanismo de bloqueo de la presente 280 se muestra con referencia a la figura 15. El módulo del cartucho 220 se carga con la compuerta giratoria 282 en su primera posición como se muestra. El cierre de la compuerta giratoria 282 se mantiene en su primera posición hasta que la barra de disparo 243 se mueve hacia adelante y hace girar la compuerta giratoria 282 a su segunda posición, o posición de "cartucho gastado" (véase figuras 16, 17 y 18). La lengüeta de bloqueo 284 se asienta directamente por detrás de la lengüeta de la compuerta giratoria 282 y ayuda a mantener la compuerta giratoria 282 en su primera posición.
Tras disparar el ensamble de transmisión de disparo, la barra de disparo 243 se mueve con fuerza hacia adelante acoplando la compuerta giratoria 282, particularmente, la superficie plana 291 de la compuerta giratoria 282. La fuerza de la barra de disparo 243 hace girar la compuerta giratoria 282 con suficiente fuerza para desviar la lengüeta de bloqueo 284, permitiendo que la compuerta giratoria 282 se mantenga dentro del hueco 222 formado en la pared lateral 221 del alojamiento del cartucho 223 entre la lengüeta de bloqueo 284 y la pared del módulo del cartucho 220. El movimiento de la lengüeta de bloqueo 284 además se facilita mediante la provisión de una superficie biselada 285 sobre la cual la compuerta giratoria 282 se mueve durante la rotación de la misma y el movimiento de la barra de disparo 243 con relación a la compuerta giratoria se facilita además mediante la superficie sesgada 289 de la compuerta giratoria 282 sobre la cual se desliza la barra de disparo 243 a medida que hace girar la compuerta giratoria 282. En esta posición, la compuerta giratoria 282 se encuentra en su segunda posición sustancialmente paralela al eje longitudinal de la engrapadora quirúrgica lineal. Mientras se encuentra en esta segunda posición, la sección de corte 286 junto con la superficie que mira hacia el frente 288 de la compuerta giratoria 282 se colocan para evitar el movimiento hacia atrás del accionador 231 , y como consecuencia evitar el uso adicional del módulo del cartucho 220.
Los mecanismos de bloqueo de conformidad con la presente invención superan el inconveniente de la técnica anterior al proporcionar una variedad de ventajas. En particular, la presente invención evita que el accionador retroceda en una manera que desactive el bloqueo. Además, el mecanismo de bloqueo de la presente es mejor que otros mecanismos de bloqueo ya que trabaja entre la estructura de yunque, que típicamente se construye para cargas elevadas, y la barra de disparo directamente para producir un bloqueo más fuerte. Además, el accionador se expone por arriba de la plataforma del cartucho después del disparo, proporcionando un indicador visual de un cartucho gastado. Esto proporciona la posibilidad de adaptar la compuerta giratoria de bloqueo para bloquear el accionador en posición y evitar que el usuario desactive inadvertidamente esta característica de seguridad visual. Aunque las modalidades preferidas se han mostrado y descrito, se entenderá que no se pretende limitar la invención mediante dicha descripción, sino que preferiblemente, se pretende cubrir todas las modificaciones y construcciones alternativas que estén dentro del espíritu y alcance de la invención.
Claims (15)
1.- Una engrapadora quirúrgica lineal adaptada para aplicar una pluralidad de sujetadores quirúrgicos a tejido corporal, la engrapadora quirúrgica comprende: una estructura de yunque; un alojamiento del cartucho que contiene una pluralidad de sujetadores quirúrgicos, el alojamiento del cartucho y la estructura de yunque relativamente móviles entre una primera posición separada y una segunda posición en aproximación cercana entre sí; un mecanismo de disparo relacionado con el alojamiento del cartucho para expulsar los sujetadores quirúrgicos del alojamiento del cartucho a ser accionados contra la estructura de yunque; y un mecanismo de bloqueo que interactúa con el alojamiento del cartucho para activación y desactivación selectiva; el mecanismo de bloqueo incluye una lengüeta de la compuerta giratoria asegurada al alojamiento del cartucho en una posición adyacente a una palanca de bloqueo, de manera que el disparo de la engrapadora quirúrgica lineal haga girar la lengüeta de la compuerta giratoria liberando la palanca de bloqueo para evitar disparo adicional del alojamiento del cartucho utilizado.
2.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 1 , caracterizada además porque la palanca de bloqueo se desvía por resorte.
3.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 2, caracterizada además porque la palanca de bloqueo se monta en una estructura de soporte de la engrapadora quirúrgica lineal adyacente al alojamiento del cartucho.
4.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 3, caracterizada además porque la palanca de bloqueo se asienta dentro un hueco formado en la estructura de soporte de una engrapadora quirúrgica lineal.
5.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 4, caracterizada además porque la palanca de bloqueo incluye una superficie superior expuesta desde el hueco y una superficie inferior que se asienta dentro del hueco.
6.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 5, caracterizada además porque la superficie superior incluye un elemento de lengüeta formado y dimensionado para acoplar el mecanismo de disparo en una manera para evitar el disparo de la engrapadora quirúrgica lineal de la presente.
7.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 1 , caracterizada además porque la lengüeta de la compuerta giratoria se asegura en pivote al alojamiento del cartucho de manera que se asiente por arriba de la palanca de bloqueo cuando se coloca para bloquear la palanca de bloqueo antes del disparo de la engrapadora quirúrgica lineal.
8.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 1 , caracterizada además porque la lengüeta de la compuerta giratoria se mueve entre una primera posición sustancialmente transversal al eje longitudinal de la engrapadora quirúrgica lineal y una segunda posición sustancialmente paralela al eje longitudinal de la engrapadora quirúrgica lineal, la lengüeta de la compuerta giratoria se asienta por arriba de la palanca de bloqueo en su primera posición reteniendo la palanca de bloqueo en una posición desbloqueada.
9.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 1 , caracterizada además porque antes de disparar la engrapadora quirúrgica lineal la lengüeta de la compuerta giratoria se coloca ligeramente hacia adelante de la palanca de bloqueo de manera que la rotación de la lengüeta de la compuerta giratoria haga que la lengüeta de la compuerta giratoria entre en contacto con la palanca de bloqueo.
10.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 9, caracterizada además porque la lengüeta de la compuerta giratoria se mueve entre una primera posición sustancialmente transversal a un eje longitudinal de la engrapadora quirúrgica lineal y una segunda posición sustancialmente paralela al eje longitudinal de la engrapadora quirúrgica lineal, de manera que cuando la lengüeta de la compuerta giratoria se encuentra en una primera posición se asiente hacia adelante de una superficie superior de la palanca de bloqueo y durante el disparo del dispositivo de engrapadora quirúrgica la lengüeta de la compuerta giratoria se haga girar a su segunda posición sustancialmente paralela al eje longitudinal del dispositivo de engrapadora quirúrgica y se remueva de la superficie superior de la palanca de bloqueo permitiendo que la palanca de bloqueo se mueva hacia arriba en una posición bloqueada.
11.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 9, caracterizada además porque la fuerza del mecanismo de disparo hace girar la lengüeta de la compuerta giratoria con suficiente fuerza para desviar la palanca de bloqueo hacia abajo en el hueco y fuera del camino de la barra de disparo y el movimiento de la lengüeta de la compuerta giratoria continúa a su segunda posición sustancialmente paralela al eje longitudinal del dispositivo de engrapado quirúrgico en donde, y una vez que el mecanismo de disparo se repliega, la palanca de bloqueo regresa a una posición de bloqueo evitando cualquier rotación adicional de la compuerta giratoria y disparando la engrapadora quirúrgica lineal.
12.- Un engrapadora quirúrgica lineal adaptada para aplicar una pluralidad de sujetadores quirúrgicos a tejido corporal, la engrapadora quirúrgica comprende: una estructura de yunque; un alojamiento del cartucho que contiene una pluralidad de sujetadores quirúrgicos, el alojamiento del cartucho y la estructura de yunque móviles relativamente entre una primera posición separada y una segunda posición en aproximación cercana entre sí; un mecanismo de disparo relacionado con el alojamiento del cartucho para expulsar los sujetadores quirúrgicos del alojamiento del cartucho a ser accionado contre la estructura de yunque; y un mecanismo de bloqueo que interactúa con el alojamiento del cartucho para activación y desactivación selectiva; un mecanismo de bloqueo incluye una compuerta giratoria asegurada en pivote al alojamiento del cartucho de manera que el disparo de la engrapadora quirúrgica lineal haga girar la compuerta giratoria para evitar disparo adicional del alojamiento del cartucho utilizado.
13.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 12, caracterizada además porque una superficie que mira hacia el frente de la compuerta giratoria incluye una sección de corte colocada para acoplar un accionador de la engrapadora y evitar movimiento adicional de la misma.
14.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 12, caracterizada además porque incluye una lengüeta de bloqueo que evita el movimiento de la compuerta giratoria después de la rotación de la compuerta giratoria durante el disparo.
15.- La engrapadora quirúrgica lineal de conformidad con la reivindicación 12, caracterizada además porque la compuerta giratoria se muestra a lo largo de una pared lateral del alojamiento del cartucho.
