Description
Puerta basculante para un dispositivo de bloqueo
en una grapadora cortadora curvada.
La presente invención se refiere a instrumentos
de corte y grapado quirúrgicos adaptados para uso en el diagnóstico
y la terapia de patologías tratadas por resección con grapas. Más en
concreto, la invención se refiere a un mecanismo de bloqueo para
utilización en unión con instrumentos de corte y grapado
quirúrgicos.
Los instrumentos de corte y grapado quirúrgicos
se utilizan comúnmente en el diagnóstico y tratamiento de
patologías tratadas por resección con grapas. Los instrumentos de
corte y grapado quirúrgicos proporcionan un mecanismo para ampliar
la utilización transluminal de dispositivos mecánicos de sutura
introducidos mediante el canal anal, la boca, el estómago y los
accesos de servicio. Aunque los instrumentos de corte y grapado
quirúrgicos son de uso muy común con patologías rectales, los
instrumentos de corte y grapado quirúrgicos pueden ser usados en
varios entornos.
Con el tiempo se han desarrollado instrumentos
de corte y grapado quirúrgicos. Estos instrumentos incluyen
generalmente un bastidor de soporte, un yunque unido al bastidor de
soporte y un alojamiento de cartucho que contiene una pluralidad de
grapas. Los instrumentos también incluyen un accionador dentro del
alojamiento de cartucho que expulsa simultáneamente todas las
grapas al yunque para dar a las grapas una forma general de B,
suturando el tejido conjuntamente. Además, estos instrumentos
incluyen mecanismos de aproximación que permiten que el alojamiento

Además de los componentes básicos de los

ser utilizado como una pinza cuando sea necesario durante una
emergencia. Sin embargo, el mecanismo de bloqueo está diseñado de
tal manera que el mecanismo de disparo solamente funcione con un
módulo de cartucho que no haya sido usado previamente.
EP 1 550 413 describe una grapadora quirúrgica
lineal que tiene un mecanismo de bloqueo según el preámbulo de la
reivindicación 1.
Los actuales instrumentos de grapado quirúrgico
incluyen un bloqueo de barra de disparo que es activado por el
accionador. Cuando se carga un nuevo módulo de cartucho en el
instrumento, la posición del accionador, en lo que se refiere al
módulo de cartucho en el instrumento, interfiere con el brazo de
bloqueo de forma que el instrumento pueda disparar grapas. Después
de que el instrumento dispara grapas, la posición del accionador se
mueve distalmente de forma que ya no interfiera con el brazo de
bloqueo. El brazo de bloqueo se mueve a una posición que ahora

En particular, los dispositivos de la técnica
anterior permiten que el accionador sea empujado de nuevo de manera
que pueda dar lugar a la desactivación del bloqueo puesto que
tienden a depender de la posición del accionador en relación al
mecanismo de bloqueo. Estos productos, si el accionador se empujase
hacia atrás lo suficiente para ocultar visualmente el accionador en
el cartucho, ya no proporcionaría al usuario un indicador visual de
un cartucho gastado. Además, algunos mecanismos de bloqueo de la
técnica anterior no realizan un bloqueo fuerte debido a los
componentes que interactúan en la formación del bloqueo después del
disparo. Como tal, se necesita un mecanismo de bloqueo mejorado que
proporcione una clara indicación de que el mecanismo de bloqueo ha
sido activado y supere los otros inconvenientes de los mecanismos de
bloqueo de la técnica anterior. La presente invención proporciona
dicho mecanismo de bloqueo.
Por lo tanto, un objeto de la presente invención
es proporcionar una grapadora quirúrgica lineal adaptada para
aplicar una pluralidad de sujetadores quirúrgicos a tejido corporal.
La grapadora quirúrgica incluye una estructura de yunque y un
alojamiento de cartucho conteniendo una pluralidad de sujetadores
quirúrgicos. El alojamiento de cartucho y la estructura de yunque
son relativamente móviles entre una primera posición espaciada y
una segunda posición en estrecha aproximación una con otra. Un
mecanismo de disparo está asociado con el alojamiento de cartucho
para expulsar los sujetadores quirúrgicos del alojamiento de

Otro objeto de la presente invención es
proporcionar una grapadora quirúrgica lineal donde la palanca de
bloqueo asienta dentro de un rebaje formado en el bastidor de

También es otro objeto de la presente invención

lineal 20 tiene un mango 21 en un primer extremo próximo y un

describirán los componentes del mecanismo de bloqueo de dispositivo
disparado 180. El mecanismo de bloqueo de dispositivo disparado 180
según la presente invención emplea una lengüeta de puerta basculante

basculante 182 se gira a su segunda posición sustancialmente
paralela al eje longitudinal de la grapadora quirúrgica lineal 20 y
quita de la superficie superior 188 de la palanca de bloqueo 184
permitiendo que el muelle 194 empuje la palanca de bloqueo 184
hacia arriba a su posición bloqueada.
Las figuras 6 y 7 muestran el mecanismo de
bloqueo 180 al inicio de su ciclo. La lengüeta de puerta basculante
182 está en su primera posición sustancialmente transversal al eje
longitudinal del dispositivo de grapado quirúrgico 20. La lengüeta
de puerta basculante 182 asienta directamente sobre la palanca de
bloqueo 184 que sujeta la palanca de bloqueo 184 dentro del rebaje
196 de tal manera que no interfiera con el disparo de la grapadora
quirúrgica lineal 20, y en particular, la barra de disparo 43.
Cuando el mecanismo de aproximación se activa
como se representa en las figuras 8 y 9, es decir, la activación
del gatillo de cierre 26, el alojamiento de cartucho 121 avanza a la
posición completamente cerrada. Sin embargo, la lengüeta de puerta
basculante 182 permanece sustancialmente transversal al eje
longitudinal de la grapadora quirúrgica lineal 20, sujetando la
palanca de bloqueo 184 dentro del rebaje 196 en su posición
desbloqueada.
Con referencia a las figuras 10 y 11, la
activación del conjunto de transmisión de disparo avanza la barra
de disparo 43 a través de la lengüeta de puerta basculante 182. El
movimiento de la barra de disparo 43 a través de la lengüeta de
puerta basculante 182 tiene lugar independientemente de la posición
del accionador 131 o la terminación de la carrera de disparo. Este
movimiento de la barra de disparo 43 a través de la lengüeta de
puerta basculante 182 mueve la lengüeta de puerta basculante 182 de
su primera posición evitando que la palanca de bloqueo empujada por
muelle 184 se desplace hacia arriba a su segunda posición
sustancialmente paralela al eje longitudinal de la grapadora
quirúrgica lineal 20. Cuando está en su segunda posición, la
lengüeta de puerta basculante 182 ya no está colocada encima de la
superficie superior 188 de la palanca de bloqueo 184 y ya no
bloquea el movimiento hacia arriba de la palanca de bloqueo 184 en
base al empuje del muelle 194 colocado entre la superficie inferior
190 de la palanca de bloqueo 184 y el rebaje 196.
Después de que la lengüeta de puerta basculante
182 se desplaza a su segunda posición y la lengüeta de puerta
basculante 182 ya no sujeta la palanca de bloqueo 184 en su posición
desbloqueada, la barra de disparo 43 asienta momentáneamente encima
de la palanca de bloqueo 184 sujetándola dentro del rebaje 196.
Cuando se retira el conjunto de transmisión de disparo, en
particular, la barra de disparo 43, la palanca de bloqueo 184 ya no
se mantiene dentro del rebaje 196 por la lengüeta de puerta
basculante 182 o la barra de disparo 43, y la palanca de bloqueo
184 se mueve a una posición bloqueada bajo la fuerza del muelle 194.
Mientras esté en esta posición, la palanca de bloqueo 184 bloqueará
cualquier intento adicional de activar los medios de disparo
(véanse las figuras 12 y 13). Aunque la palanca de bloqueo 184
bloquee el movimiento hacia atrás del accionador 131, la barra de
disparo 43 se retira en último término a su posición próxima.
Después de liberar la palanca de bloqueo 184 a su posición superior
cuando la barra de disparo 43 se mueve a su posición distal, hay una
superficie inclinada 185 en la cara distal de la palanca de bloqueo
184 que actúa como una excéntrica que permite que la barra de
disparo 43 empuje "hacia abajo" durante la retracción hacia
atrás de la barra de disparo 43.
Con referencia a la figura 14, aunque se
describe una palanca de bloqueo lineal según una realización
preferida de la presente invención, la palanca de bloqueo 184'
también podría ser una palanca de bloqueo rotativa 184' que se
monte en un extremo 184a' para rotación pivotante alrededor del
montaje de tal manera que la palanca de bloqueo rotativa hacia
arriba 184' empujada por muelle 194' todavía funcione de la misma
forma que la descrita con referencia a la realización preferida
descrita con referencia a las figuras 6 a 13.
Según otra realización ejemplar exenta de la
presente invención, y con referencia a las figuras 15, 16 y 17, la
lengüeta de puerta basculante no asienta constantemente encima de la
palanca de bloqueo antes de disparar como se ha descrito
anteriormente con referencia a las figuras 6 a 13. El presente
mecanismo de bloqueo de dispositivo disparado 280 emplea una puerta
basculante 282 fijada pivotantemente al módulo de cartucho no usado
220. Según una realización preferida, el módulo de cartucho 220
incluye un alojamiento de cartucho 223 y la puerta basculante 282
está fijada a una pared lateral 221 del módulo de cartucho 220.
Teniendo esto en mente, y como se apreciará en base a la
descripción siguiente, la pared lateral 221 del módulo de cartucho
220 está provista de un rebaje 222 conformado y dimensionado para
recibir la puerta basculante 282 cuando gira hacia la pared lateral
221 permitiendo que la barra de disparo 243 pase.
La puerta basculante 282 está conformada y
dimensionada para rotación con el fin de bloquear el movimiento
adicional del accionador disparado 231 y en consecuencia evitar el
uso del módulo de cartucho previamente disparado 220. Teniendo esto
en mente, la puerta basculante incluye una superficie delantera 288
que tiene una sección cortada 286 junto a su extremo distal y una
superficie inclinada 289 adyacente a su extremo próximo. Se coloca
una superficie plana 291 entre la sección cortada 286 y la
superficie inclinada 289.
La configuración de pre-disparo
del presente mecanismo de bloqueo 280 se representa con referencia a
la figura 15. El módulo de cartucho 220 se carga con la puerta
basculante 282 en su primera posición como se representa. El
bloqueo de la puerta basculante 282 se mantiene en su primera
posición hasta que la barra de disparo 243 se mueva hacia delante y
gira la puerta basculante 282 a su segunda posición, o posición de
"cartucho gastado" (véanse las figuras 16, 17 y 18). La
lengüeta de bloqueo 284 asienta directamente detrás de la lengüeta
de puerta basculante 282 y asiste a mantener la puerta basculante
282 en su primera posición.
Al disparo del conjunto de transmisión de
disparo, la barra de disparo 243 es empujada hacia delante para
enganchar la puerta basculante 282, en particular, la superficie
plana 291 de la puerta basculante 282. La fuerza de la barra de
disparo 243 gira la puerta basculante 282 con fuerza suficiente para
desviar la lengüeta de bloqueo 284, permitiendo que la puerta
basculante 282 se mantenga dentro del rebaje 222 formado en la pared
lateral 221 del alojamiento de cartucho 223 entre la lengüeta de
bloqueo 284 y la pared del módulo de cartucho 220. El movimiento de
la lengüeta de bloqueo 284 se facilita más por la provisión de una
superficie biselada 285 sobre la que la puerta basculante 282 se
mueve durante su rotación, y el movimiento de la barra de disparo
243 con relación a la puerta basculante se facilita más por la
superficie inclinada 289 de la puerta basculante 282 sobre la que
la barra de disparo 243 desliza cuando gira la puerta basculante
282.
En esta posición, la puerta basculante 282 está
en su segunda posición sustancialmente paralela al eje longitudinal
de la grapadora quirúrgica lineal. Mientras está en esta segunda
posición, la sección cortada 286 a lo largo de la superficie
delantera 288 de la puerta basculante 282 se coloca para evitar el
movimiento hacia atrás del accionador 231, y en consecuencia evita
el uso adicional del módulo de cartucho 220.
Los mecanismos de bloqueo según la presente
invención superan el inconveniente de la técnica anterior
proporcionando varias ventajas. En particular, la presente
invención evita que el accionador sea empujado de nuevo evitando el
bloqueo. Además, el mecanismo de bloqueo de la presente invención es
mejor que otros mecanismos de bloqueo porque opera entre la
estructura de yunque, que se construye típicamente para carga alta,
y la barra de disparo para producir directamente un bloqueo más
fuerte. Además, el accionador se expone encima de la cubierta de
cartucho después del disparo, proporcionando un indicador visual de
un cartucho gastado. Esto ofrece la posibilidad de adaptar la
puerta basculante de bloqueo para bloquear el accionador en posición
y evitar que el usuario apague sin darse cuenta esta característica
visual de seguridad.
Aunque se han representado y descrito las
realizaciones preferidas, se entenderá que no se intenta limitar la
invención mediante dicha descripción, sino que más bien se ha
previsto cubrir todas las modificaciones y construcciones
alternativas que caigan dentro del alcance de las
reivindicaciones.
Claims (9)
1. Una grapadora quirúrgica lineal (20) adaptada
para aplicar una pluralidad de sujetadores quirúrgicos a tejido
corporal, incluyendo la grapadora quirúrgica:
- un bastidor de soporte (81);
- una estructura de yunque (122) montada en el extremo distal del bastidor de soporte (81);
- un alojamiento de cartucho (120) conteniendo una pluralidad de sujetadores quirúrgicos, pudiendo moverse el alojamiento de cartucho (120) con relación a la estructura de yunque (122) entre una primera posición espaciada y una segunda posición en estrecha aproximación;
- un mecanismo de disparo asociado con el alojamiento de cartucho (120) para expulsar los sujetadores quirúrgicos del alojamiento de cartucho (120) para moverlos contra la estructura de yunque (122); y
- un mecanismo de bloqueo (180) que interactúa con el alojamiento de cartucho (120) para activación y desactivación selectivas; caracterizada porque
- el mecanismo de bloqueo (180) incluye una lengüeta de puerta basculante (182) fijada al alojamiento de cartucho (120) en una posición junto a una palanca de bloqueo (184) montada en el bastidor de soporte (81) de la grapadora quirúrgica lineal (20) junto al alojamiento de cartucho, de tal manera que el disparo de la grapadora quirúrgica lineal (20) gire la lengüeta de puerta basculante (182) liberando la palanca de bloqueo (184) para evitar el disparo adicional del alojamiento de cartucho usado (120).
2. La grapadora quirúrgica lineal según la
reivindicación 1, donde la palanca de bloqueo (184) es empujada por
muelle.
3. La grapadora quirúrgica lineal según la
reivindicación 2, donde la palanca de bloqueo (184) asienta dentro
de un rebaje (196) formado en el bastidor de soporte (81) de la
grapadora quirúrgica lineal.
4. La grapadora quirúrgica lineal según la
reivindicación 3, donde la palanca de bloqueo (184) incluye una
superficie superior (188) expuesta del rebaje (196) y una superficie
inferior (190) asentada dentro del rebaje (188).
5. La grapadora quirúrgica lineal según la
reivindicación 4, donde la superficie superior (196) incluye un
elemento de lengüeta (192) conformado y dimensionado para enganchar
el mecanismo de disparo con el fin de evitar el disparo de la
grapadora quirúrgica lineal presente.
6. La grapadora quirúrgica lineal según la
reivindicación 1, donde la lengüeta de puerta basculante (182) está
fijada pivotantemente al alojamiento de cartucho (120) de tal manera
que asiente encima de la palanca de bloqueo (184) cuando se coloque
para bloquear la palanca de bloqueo (184) antes de disparar la
grapadora quirúrgica lineal.
7. La grapadora quirúrgica lineal según la
reivindicación 1, donde la lengüeta de puerta basculante (182) se
mueve entre una primera posición sustancialmente transversal a un
eje longitudinal de la grapadora quirúrgica lineal y una segunda
posición sustancialmente paralela al eje longitudinal de la
grapadora quirúrgica lineal, la lengüeta de puerta basculante (182)
asienta encima de la palanca de bloqueo (184) en su primera posición
sujetando la palanca de bloqueo (184) en una posición
desbloqueada.
8. La grapadora quirúrgica lineal según la
reivindicación 1, donde, antes del disparo de la grapadora
quirúrgica lineal, la lengüeta de puerta basculante (182) se coloca
ligeramente hacia delante de la palanca de bloqueo (184) de tal
manera que la rotación de la lengüeta de puerta basculante (184)
haga que la lengüeta de puerta basculante (182) entre en contacto
con la palanca de bloqueo (184).
9. La grapadora quirúrgica lineal según la
reivindicación 8, donde la lengüeta de puerta basculante (182) se
mueve entre una primera posición sustancialmente transversal a un
eje longitudinal de la grapadora quirúrgica lineal y una segunda
posición sustancialmente paralela al eje longitudinal de la
grapadora quirúrgica lineal, de tal manera que cuando la lengüeta
de puerta basculante (182) esté en su primera posición, asiente
hacia delante de una superficie superior (196) de la palanca de
bloqueo (184) y durante el disparo de la grapadora quirúrgica la
lengüeta de puerta basculante (182) se gire a su segunda posición
sustancialmente paralela al eje longitudinal de la grapadora
quirúrgica y quite de la superficie superior (196) de la palanca de
bloqueo (184) dejando que la palanca de bloqueo (184) se desplace
hacia arriba a una posición bloqueada.
