Abstract

Description
본 발명은 일반적으로 조직 소작에 관한 것이다. 보다 구체적으로, 본 발명은 조직 소작을 위한 개선된 전극에 관한 것이다.
많은 수술 절차에 있어서, 다양한 목적으로 조직 및 기관을 제거할 필요가 있다. 모든 조직 제거 절차에서의 주요한 관심은 지혈, 즉, 출혈을 멈추게 하는 것이다. 한 기관이나 조직 단위에 공급되는 모든 혈관들은 봉합되어야 한다. 예를 들어, 자궁절제술에서 자궁을 제거하는 경우, 자궁의 그 측 부를 따라 혈액을 공급하는 혈관뿐만 아니라, 절제되는 경부 목에서의 출혈을 억제해야만 한다. 마찬가지로, 종양의 제거와 관련하여 또는 다른 목적으로 일부 폐를 절제할 경우 폐 내의 혈관을 개별적으로 봉합해야 한다. 폐는 매우 혈관이 발달한 기관이고 혈관의 봉합은 상당히 시간을 소비하게 된다. 개복 수술 절차와 최소 침습 절차 모두에서 지혈을 달성하는 것이 필요하다. 그러나, 이후의 경우에서, 캐뉼라를 통한 제한된 액세스와 그 밖의 작은 통로 때문에, 혈관의 봉합은 보다 더 많은 시간을 소비하며 문제가 많다.

전극 구조물의 전기적 활성 영역은 적어도 1㎠의 면적을 가지며, 보다 통상적으로는 적어도 2㎠의 면적 갖고, 대개는 5㎠ 이상의 면적을 가지며, 보다 대개는 10㎠ 이상의 면적을 갖는다. 전극은 다양한 특성을 가질 수 있으며, 일반적으로 강성, 유연성, 탄성, 융통성, 적응성 등이 있을 수도 있다. 전극의 체결을 용이하게 하도록 전극은 조직 표면에 대하여 유연하며, 또한 순응성을 갖는 것이 바람직하다. 적어도 어떤 경우에 있어서는, 조직이나 기관 표면의 외부 주변에 대하여 순응될 수 있는 유연하고 탄성을 갖는 전극을 제공하는 것이 바람직하며, 여기서 전극의 탄성은 견고한 체결과 전극 접촉을 확실하게 한다. 그러나, 그 밖의 경우에서는, 특정 조직 표면에 대한 원하는 기하학적 구조를 갖도록 전극을 구체적으로 구성할 수도 있다.
일반적으로 기관이나 조직에 인가된 고주파수 에너지는 무선 주파수로 제공되는 것이 전형적이지만, 100kHz 내지 10MHz의 범위에 한정되지 않으며, 통상적으로 200kHz 내지 750kHz가 된다. 전력 레벨은 치료되는 조직의 표면적과 체적에 따라 다르지만, 일반적으로는 10W 내지 500W 범위 내에 들어가고, 통상적으로 25W 내지 250W이고, 보다 통상적으로는 50W 내지 200W가 된다. 전력은 1W/㎠ 내지 500W/㎠의 레벨에서 인가되는 것이 통상적이며, 보다 통상적으로는 10W/㎠ 내지 100W/㎠가 된다. 전력은 치료되는 조직 덩어리에서의 조직 온도가 소작이나 괴사시키는데 필요한 임계 레벨 이상으로 상승하는데 충분한 시간 동안 인가되며, 통상적으로 적어도 60℃ 이상으로 되며, 빈번하게 70℃ 이상으로 되는 경우가 있으며, 종종 80℃이상이 되거나, 더 높게 된다. 그러나, 에너지의 응용은 제한되어야하며, 인접 조직이 심각하게 가열되거나 손상되지 않도록 한다. 대치된 양 극성 전극을 사용하는 경우, 에너지 흐름을 이 전극 사이에 집중시켜서 대치된 전극 내에 한정되지 않는 인접 조직에 미치는 영향을 제한하기 때문에, 이점에 있어서 특히 유리하다. 그 결과 괴사된 조직은 실질적으로 치료되는 전체 기관을 포함하거나, 다른 경우에는, 예를 들어, 평면 영역의 보다 좁은 영역을 포함할 수도 있다.

본 발명의 방법, 시스템, 및 장비는 환자의 다양한 기관, 기관의 일부, 또는 다른 단단한 조직 영역을 치료하는데 유용하다. 기관 또는 다른 조직 덩어리는 이격된 조직 표면을 갖고, 통상적으로 조직 표면은 대치되어 있으며, 전극 구조물이 접근할 수 있고 상기 표면 사이에 라디오 주파수 전력 인가가 가능하다. 조직 표면에 쉽게 접근할 수 있거나 접근이 되도록, 예를 들어, 둔절개(blunt dissection), 종래 수술법을 이용하는 작은 조직이나 혈관의 절개 등의 사전 처리가 필요할 수도 있다. 본 발명에 의해 치료될 수 있는 기관은 자궁, 간, 전립선, 신장, 창자, 췌장, 폐, 흉부, 근육 등을 포함한다.
기관과 다른 조직은 전극 구조물의 이격 배치에 의해 규정되는 목표 조직 영역으로 향해진 양 극성 무선 주파수 전력으로 치료된다. 무선 주파수 전력은 수용가능한 주파수, 전형적으로 상기 기재의 범위에서 동작하는 종래 범용 전기수술 전원에 의해 공급될 수 있다. 전원은 종래의 사인파 또는 비사인파 형태를 채용할 수도 있고, 고정 또는 제어된 전력 레벨로 동작할 수 있으며, 여기서는 전압, 전류, 또는 이 둘 모두가 선택될 수도 있다. 밸리랩(Valleylab), 아스펜(Aspen), 및 보비에(Bovie)와 같은 시판 중인 전원 중에서 적합한 전원을 이용할 수 있다. 어떤 경우에는, 에너지 전달에 있어서의 효율을 향상시키기 위하여 전원과 전극 사이에 임피던스 정합 변환기를 이용하는 것이 바람직할 것이다.
전극은 조직 표면을 체결하는데 적합한 임의의 방법으로 구성될 수도 있다. 그러므로, 전극은 강성, 유연성, 탄성, 비탄성(비신장성), 평면, 비평면 등을 가질 수도 있고, 선택적으로는 조직 관통 소자를 채용하여 전극 면적을 증대시킬 뿐만 아니라, 전극 구조물과 조직 사이의 전기적 접촉을 향상시킬 수도 있다. 바람직한 전극 구성으로는 순응성이 있어 현저하게 다른 조직 표면(예를 들어, 미국 특허 공보 일련번호 제11/371,988(대리인 일람번호 ARGA0003) 참조, 참고용으로 본원에 포함됨)에 대하여 체결하여 순응시키거나, 특정 기관이나 조직 기하학적 구조를 체결하도록 의도된 기하학적 구조를 갖도록 구체적으로 구성된다. 두 경우에 있어서, 전극 구조물은 조직 관통 소자가 더 구비될 수도 있다. 이하에 각각의 예를 논의한다.
하나의 전극 구성으로서 유연성과 탄성을 모두 갖는 금속피복된 메쉬(mesh)가 사용된다. 이러한 메쉬는 최소 침습 시술에 유용한 신축성 전극과 같은 신축성 전극에 적합하게 이용될 수 있다. 메쉬는 더 강한 프레임 부재에 매달거나 그 사이에 위치될 수 있으며, 이 프레임 부재는 메쉬 전극을 전개하기 위해 그 자체가 신장 또는 축소될 수도 있다. 이러한 메쉬는 소크(sock)와 같이 기관이나 조직 덩어리 위에 배치될 수도 있는 탄성 튜브나 쉘을 제작하는데 유용하다. 이러한 튜브형 전극의 경우에서, 두 개 이상의 분리된 전극 표면을 단일 메쉬 위에 형성하는 것이 종종 바람직하며, 이 전극 표면은 통상적으로 메쉬 자체의 금속 특성으로 인해 절연되어 있다. 즉, 이들은 폴리머이며 비전도성이다. 탄성 메쉬는 브레이드(braid) 또는 다른 직조 구조물의 형태로 될 수 있다(예를 들어, 미국 특허 제5,431,676; 5,234,425; 및 4,018,230의 기재, 참고용으로 본원에 포함됨). 조직이 함유하는 내강의 직경이 축상 신장률(axial elongation)에 의해 제어될 수 있기 때문에 방사상 신장성을 갖는 브레이드 구조물을 사용하는 것이 바람직하다. 즉, 브레이드는 그 길이를 축소시킴으로써 신장될 수 있고, 그 길이를 신장시킴으로써 신축될 수 있다. 이러한 모든 메쉬와 브레이드 구조물은 종래 무전해 도금법에 의해 금속피복될 수 있다. 도금에 적합한 금속으로는 금, 은, 구리, 스테인리스강, 및 이들의 조합 및 합금을 포함한다. 적합한 탄성율계 메쉬 재료로는 매우 다양한 엘라스토머를 포함한다. 적합한 브레이드 메쉬 재료로는 나일론과 일반적으로 비팽창성의 다른 폴리머를 포함한다.
모든 형태의 전극 구조물은 도전 표면과 비도전 표면을 갖도록 구성될 수도 있다. 이는 전극의 다른 표면을 피복 또는 절연하면서 한 표면은 금속제 면에 노출되도록 방치함으로써 달성된다. 강성 전극의 경우에는, 절연체가 적층, 코팅되거나, 그렇지 않으면 대향 표면에 직접 도포될 수 있다. 유연한 탄성 전극의 경우에는, 손실이나 제거됨 없이 전극과 함께 신장 및 신축될 수 있도록 절연층도 유연성을 갖는 것이 필요하다. 어떤 경우에는, 전극과 함께 신장되고, 절연되기를 바라는 면을 보호하는 금속 분리 시트를 채용하는 것이 바람직하다.
도 1을 참조하면, 본 발명에 따른 시스템(10)은 합성의 제 1 전극(12), 제 2 전극(14), 및 무선 주파수 전원(16)을 포함한다. 제 1 전극은 전원(16)의 하나의 폴(pole)에 독립적으로 접속된 복수의 강성 플레이트를 포함하고, 제 2 전극은 반대 폴에 접속된 강성 플레이트이다. 다른 실시예에서, 제 2 전극은 비(非)강성일 수 있으며 뿐만 아니라 다수의 전극으로 나타낼 수도 있다. 실제로, 어떤 경우에 있어서는 다수의 회귀 전극이 바람직하다. 조직 덩어리(T)에 대하여 이것의 대치된 표면을 체결하기 위하여 전극(12, 14)이 체결될 수 있다. 그런 다음 전극(12, 14) 사이에 포획된 조직 덩어리를 완전히 소작하기 위해 전극(12)(이하 상세히 논의함)을 형성하는 복수의 강성 플레이트의 임의의 조합을 통하여 조직에 무선 주파수 전력을 선택적으로 인가한다. 조직이 소작된 후, 소작된 조직 영역 내에 선을 따라 절제될 수도 있다. 소작된 조직내에서의 절제는 출혈을 최소화하고 지혈을 쉽게한다는 이점이 있다.
예를 들어, 자궁의 조직을 소작하는데 이용하기 위해서는, 순응성 전극이 자궁의 대치된 외부 표면 위에 배치될 수 있다. 상술한 바와 같이, 반대 극성의 고 주파수 에너지를 인가하는 것보다, 전원에 의해 공통 극성으로 전극에 급전할 수도 있다. 프로브가 자궁 내강 내로 삽입되어 반대 극성으로 급전될 수도 있다. 이와 같이, 대치된 조직 표면은 한편으로는 자궁 내강의 내부 표면(lining)과 다른 한편으로는 자궁의 외부 표면을 포함한다. 자궁의 경우, 가능한 자궁경관의 목(cervical neck) 없이, 전체 조직 덩어리를 실질적으로 소작하는 것이 일반적으로 바람직하다. 그러나, 다른 신체 기관과 조직 덩어리의 경우에는, 조직의 일부만을 소작하는 것이 바람직할 수도 있다. 고주파수 에너지는 전극의 구성을 선택함으로써 조직의 제한된 부위로 향하게 할 수 있다.
바람직하게는, 전극은 복수의 다른 도전성 영역을 포함하고, 이 영역은 서로 절연되거나 서로 전기적으로 연결될 수도 있다. 단일 전극 구조물은 3, 4, 5 및 10개 이상의 분산된 도전 영역이 그 위에 포함될 수도 있다. 이러한 도전 영역은 통상 절연 영역 또는 그 사이의 구조물에 의해 규정된다. 도전 영역의 2개 이상이 전기적으로 연결되는 것이 바람직할 경우, 영역 사이의 절연을 중개하기 위해 작은 전기적 접속이 제공될 수 있다. 통상적으로, 전극 구조물 상에서 적어도 절연된 도전 영역들 중 몇몇은 반대 극성으로 급전되고, 어떤 경우에는 본 발명의 방법은 다수의 도전 영역을 갖는 단일 전극 구조물만을 이용하여 실행될 수 있다. 또한, 단일 전극 구조물 상에 절연된 도전 영역들은 동일한 극성으로 급전될 수도 있으며, 다른 영역들의 주요 목적은 조직 덩어리 내로 전달되는 높은 전기 에너지 플럭스를 제어하거나 구성하도록 하는 것이다. 예를 들어, 소작될 영역의 사이 또는 인접한 다른 면적을 소작하지 않고 조직 덩어리의 이격된 영역 내로 고주파수 전기 에너지를 전달하는 것이 바람직할 수도 있다. 이러한 경우에, 전극 구조물은 도전 영역의 적합한 배치에 의해 구성될 수 있다.

도 4는 본 발명에 따른 전력 변조를 위한 알고리즘을 나타내는 플로차트이다. 프로세스의 개시에서, 사용자는 특정 시술에 적용될 프로파일을 선택한다(단계 400). 예를 들어, 어떤 조직 또는 시술은 높은 초기 전력 레벨이 필요하고 그런 다음 소작 프로세스 동안에 전력 레벨의 감소가 제공될 수도 있다.

그런 다음 시스템은 종점이 언제 접근할지를 판단하는 임계값을 설정한다(단계 420). 이들 임계값은 임피던스, 전류, 전압, 에너지, 전력, 및 온도와 같은 측 정값에 의해 판정될 수도 있다. 임계값은 시간 요소와 연계하여 동작할 수도 있다. 그러므로, 임계값에 도달한 경우, 적합한 종점에 도달한 것을 명확히 하여, 소작을 완료하도록, 시스템은 어떤 시간 동안 계속 동작할 수도 있다. 몇몇 시스템에서, 종점에 도달한 조직의 일부에 대해 전극이 할당된 전력 발생기 채널은 아직 조직을 소작하고 있는 전극에 재할당되어 프로세스에 추가 전력을 제공하므로 그 종결을 앞당길 수도 있다.

본 발명의 이 실시예의 다른 장점은 죠 또는 프로브를 무겁고 강한 물질로 제작할 필요가 없다는 것이다. 본 발명은 경량의 물질로 만들어진 프로브를 제조할 수 있게 한다. 이러한 물질은 본 장치가 보다 저렴하도록 한다.
