Abstract
급성 질환 치료용 약학 조성물이 개시된다. 상기 조성물은 캐리어 입자의 표면에 부착된 적어도 하나의 약학 활성제의 마이크로입자와 상기 캐리어 입자의 표면에 부착된 생체접착 및/또는 점막접착 촉진제의 본질적으로 물을 함유하지 않는 정렬된(ordered) 혼합물을 포함하며, 상기 캐리어 입자는 활성제 또는 활성제들의 입자보다 실질적으로 크며, 본질적으로 물에 불용성이거나 물에 간헐적으로(sparingly) 용해성이다. 상기 조성물은 주로 설하 또는 비강내 투여용으로 의도된다. 본 발명은 또한 상기 조성물을 제조하는 방법 및 상기 조성물의 급성 질환 치료용으로의 용도에 관한 것이다.
Description
본 발명은 약학 제제의 설하 또는 비강내 투여를 위한 신속히 작용하는 약학 조성물, 이러한 조성물을 제조하는 방법, 및 이러한 조성물을 사용함으로써 급성 질환을 치료하는 방법에 관한 것이다.
급성 및/또는 중증 질환은 일반적으로 응급 치료 또는 입원의 원인이 된다. 이러한 타입의 가장 일반적인 질환중 하나는 브레이크스루 통증(breakthrough pain)이다. 암 환자에서, 통증은 일반적으로 비-스테로이드 항-염증 드럭(NSAIDs) 및 마취제 단독으로 혹은 조합으로 치료된다. 오피오이드가 요구되는 암 통증 환자는 일반적으로 느린-방출성 마취제(느린-방출성 모르핀 또는 케토베미돈 또는 경피성 펜타닐)이 주어진다. 암 통증의 특성적 특징은 부적절한 진통기간(브레이크스루 통증)이다. 이들은 대부분 환자의 증가된 물리적 활성에 기인한다. 그러나, 장기간 작용하는 진통제의 증가된 시간 부수적인 투여량의 투여에 의한 브레이크스루 통증의 치료는 과도한 진정, 메스꺼움, 및 변비와 같은 해로운 부작용을 야기한다.
신속-작용성 치료를 요구하는 다른 질환 및 컨디션은 예를들어, 폐 부종, 역류성 식도염, 불면증 및 신결석증이다.
현재 이용가능한 경구, 직장, 비강내 또는 설하 투여용 배합물은 급성 질환을 조절하기에 적합하지 않은 상대적으로 긴 개시 시간 또는 일정치 않은 흡수 특성을 갖는다.
급성 수술/수술후 또는 외상/외상후 통증 뿐만 아니라 중증 질환에 기인한 통증(예, 심근 인프랙션, 신결석증 등)의 컨디션은 일반적으로 진통제의 신속한 개시를 얻기위해 비경구적으로(정맥내 또는 근육내 투여) 투여된다. 이러한 경우, 신속한-개시 경구 대체법은 상당히 치료학적으로 관심있는 것이다. 다른 급성 질환의 치료에 있어서, 비경구적으로 혹은 직장으로 대신에 경구적으로 혹은 비강 내 경로에 의해 투여될 수 있는 신속-작용성 치료 조성물을 제공하는 것은 상당히 관심있는 것이다. 그러나, 경구적으로 투여되기에 유리한 다수의 약학적으로 활성적인 제제는 삼키기에는 부적합하다. 이들은 예를들어 위액에 의해 불활성화될 수 있으며 수성 매체내에서 저 용해성을 가져 느린 작용을 갖거나, 위장 효소에 의한 대사에 매우 민감하고 예를들어 펩타이드 호르몬과 같이 저조한 흡수 특성을 갖는다. 따라서 경구 또는 비경구 공동의 점액성 막을 통해 취해지는 활성 성분에 대해 배열되는 것이 보다 바람직하다. 경구 공동에 있어서, 가장 바람직한 투여방법은 설하 경로에 의한 것이다. 이러한 투여에서, 약학 조성물의 투여 유니트는 혀밑에 놓여지며 활성성분은 주위의 점액성 막을 통해 흡수된다. 그러나, 이러한 투여방법에 대해서는 환자가 타액을 삼킴으로써 약제를 삼키는 위험이 있다는 것이 잘 알려져 있다.
급성 통증의 치료시 펜타닐, N-(1-펜에틸-4-피퍼리딜)-프로피오아닐리드 또는 이의 약학적으로 허용가능한 염들중 하나가 사용될 수 있다. 이 화합물은 오피오이드 작동물질이며 모르핀 및 메페리딘과 같은 마취제의 다수의 약력학적 효과를 공유한다. 그러나, 이러한 마취제와 비교하여 펜타닐은 수면 작용을 거의 나타내지 않으며, 히스타민 방출을 거의 유도하지 않으며, 호흡 저하는 보다 단명적이다. 펜타닐은 정맥내, 협내(정제-경점막) 및 경피 투여용으로 상업적으로 이용가능하다.
펜타닐의 비경구 투여후, 진통 작용은 보다 즉각적이며 모르핀 및 메페리딘보다 덜 지연된다. 정맥내 투여후 무통의 개시는 신속하다. 피크 무통은 수분내에 획득된다. 정제에 의한 협내 투여후, 정제의 소모는 일반적으로 30분내에 완료되며 피크 혈장 농도는 예를들어, Farrar 등., J. Natl. Cancer Inst., 1998, 90(8), p. 611-616에 기술된 바와 같이 약 20분후에 나타난다. 이는 위장 흡수를 위한 경구 투여에 비해 향상성을 가지며 환자에게 실질적으로 유익한 보다 빠른 무통의 시작이 이루어진다. 또한, 실질적인 양의 정제-투여된 펜타닐은 환자에 의해 삼켜진다. 이는 바람직하지 않으며 과도한 양의 약물 투여를 초래하며, 이는 부작용을 일으킬 수 있다.
본 발명의 목적
본 발명의 목적은 투여후 단 시간내에 적어도 하나의 약학적으로 활성적인 제제의 약리학적으로 효과적인 혈장 수준으로 증가시키도록 적어도 하나의 약학적으로 활성적인 제제를 경구 또는 비강 투여에 의해 급성 질환을 치료하는 것이다.
본 발명의 다른 목적은 이러한 목적에 적절한 약학 조성물을 제공하는 것이다.
본 발명의 다른 목적은 이러한 조성물을 제조하는 방법을 제공하는 것이다.
본 발명의 다른 목적은 급성 질환의 치료에 유용한 적어도 하나의 약학적으로 활성적인 화합물의 생리적으로 효과적인 투여량을 함유하는 설하 또는 비강내 투여용 의약품의 제조방법을 제공하는 것이다.
도 1은 본 발명에 따른 조성물의 생체접착성 강도의 시험 결과를 나타낸다. 이는 농도에 대한 최대 인장 강도를 나타내는 도표이다.
본 발명에 따른, 급성 질환의 경구 치료는 적어도 하나의 약학적으로 활성적인 제제의 약학적으로 효과적인 양을 포함하는 정렬된(ordered) 혼합물의 설하 투여를 포함한다. 상기 제제 또는 제제들은 생체접착 및/또는 점막접착 촉진 화합물과 함께 설하 투여된다. 동일한 방법으로, 상기 조성물은 또한 비강내 투여에 유용하다.
또한 본 발명에 따라, 적어도 하나의 약학적으로 활성적인 제제의 약리학적으로 효과적인 양을 포함하는 설하 또는 비강내 투여용 단일-투여 약학 조성물이 또한 제공된다. 상기 조성물은 또한 생체접착 및 점막접착 촉진 화합물을 함유한다. 이 화합물은 삼켜진 타액을 통해 일정치 않은 약물 흡수를 감소시키며 상기 제제 또는 제제들의 소량 투여를 가능케 한다. 따라서, 이는 실질적으로 부작용의 위험을 감소시킬 뿐만 아니라 치료학적 반응의 환자내 및 환자사이의 변화를 감소시킨다. 따라서 약물 축적의 위험이 감소되며, 이는 그 약학 제조물이 급성 질환으로부터 고통받는 환자에게 반복 투여하기에 매우 적합하도록 해 준다.
본 발명의 약학 조성물내에 함유된 활성 제제 또는 제제들의 양은 치료하는 내과의사에 의해 평가되는 다수의 요인에 따라 분명히 달라진다. 이러한 요인중 사용된 특정 제제 및 치료되는 질환의 타입, 환자의 의학적 상태 등이 언급될 수 있다.
펜타닐이 급성 또는 브레이크스루 통증의 치료에 사용되는 경우, 본 발명의 조성물은 펜타닐 또는 그 약학적으로 허용가능한 염중 하나를 0.05 내지 최고 20중량%로 함유하여야 한다. 보다 바람직하게, 상기 조성물은 0.05-5중량%의 펜타닐, 특히 0.1-1중량%의 펜타닐을 함유한다. 상기 함량은 정제와 같이 조성물의 투여 유니트로 펜타닐의 양으로 나타내어질 수 있다. 이 경우, 투여 유니트는 0.025-10mg, 바람직하게 0.05-2mg의 펜타닐을 함유해야 한다. 펜타닐이 염의 형태로 사용되는 경우, 이러한 퍼센트 및 양은 이에 따라 재계산되어야 한다.
본 발명에 따른 상기 설하 또는 비강내 조성물은 미세 입자 형태의 약학적으로 활성적인 제제 또는 제제들로 코팅된 하나 또는 그 이상의 생체접착 및/또는 점막접착성 캐리어 물질의 정렬된 혼합물을 포함한다. 본 발명에 따라 상기 캐리어 물질 또는 물질들은 물에 불용성이거나 간헐적으로 용해성이다. 용어 "정렬된 혼합물"은 보다 거친 부형제 입자와 친밀히 혼합된 활성 성분(들)의 미세한 입자질의 사용을 나타내는 것을 의미한다. 그 다음, 미세한 약물 입자는 부형제(캐리어) 입자의 표면상에 일차 입자로서 필수적으로 결합된다. 또한 "상호작용성 혼합물" 또는 "접착성 혼합물"과 같은 용어가 본 문헌에서 상호교환적으로 사용될 수 있다.
본 발명에 따른 조성물은 유럽 특허 EP 0 324 725에 개시된 신속 용해성 정렬-혼합 조성물을 배합하는 여러가지의 기술을 사용하여 배합하는 것이 바람직하다. 이러한 조성물에서, 미세하게 분산된 상태의 약물은 실질적으로 보다 큰 수용성 캐리어 입자의 표면을 덮는다. 이러한 조성물은 물에서 신속히 분해되어 따라서 이들의 미세한 약물 입자 내용물이 분산된다. 정렬된 혼합물로부터 미세 입자의 용해는 지금까지 가용성 캐리어의 사용과 결합되어왔다. 이러한 접근은 캐리어의 신속한 용해 및 이에 따른 미세 약물 입자의 유리로 특징지어진다. 현재 분리성 유니트로 표현되는 이러한 약물 입자는 호의적 유체 역학에 기인하여 신속히 용해될 것이다. 이러한 접근은 이전에 다량의 용해 유체의 사용으로 제한되어 왔다. 약물 입자가 보다 많은 양의 용해 액체에 유리되는 경우에만 포화 현상 또는 비호의적 유체 역학에 의해 용해가 방해되지 않는 것으로 여겨진다.
그러나, 공개된 PCT 출원 제 WO 00/16750에서, 가용성 캐리어로 정렬된 혼합물의 사용은 설하 투여에 적용된 바 있다. 경구 공동내에 용해되는 유체(타액)의 제한된 부피에도 불구하고 신속한 용해 및 후속적인 약물 흡수가 달성될 수 있는 것으로 발견되었다. 현재, 예기치않게 불용성이거나 간헐적 용해성 캐리어 또한 동일한 결과로 사용될 수 있는 것으로 실현되었다. 보다 거친 캐리어 입자의 표면상에 분리성 약물 입자(즉, 비-집적된 형태)의 최적 노출은 신속한 용해를 위한 결정 인자를 나타내는 것으로 여겨진다. 상기 약물은 주 정제 성분의 표면상에 위치하기때문에, 용해에 참여하는 큰 표면적은 이러한 약물 입자가 용해되기 전에 불용성 캐리어로부터 유리되지않음에도 불구하고 신속한 용해를 제공할 것이다. 따라서, 상기 약물이 매우 미세한 입자 형태이며 분리성, 비-집적된 유니트로 존재하는 한, 용해는 또한 캐리어에 부착된 약물 입자로부터 신속히 일어날 수 있다. 또 다른 필요조건은 상기 약물이 저 비율로 사용되는 것이다. 바람직하게 그 투여량은 10mg미만, 보다 바람직하게 2mg미만이다.
가용성 캐리어보다 불용성 캐리어가 갖는 잇점은 보다 미세한, 생체/점성-접착 성분으로 코팅된 후 점막에 대한 향상된 부착 경향이다. 가용성 캐리어는 투여 직후 용해되기 시작하고 이에 따라 점막부착성은 감소하는 것으로 발견되었다. 다른 한편, 생체접착성 입자로 코팅된 불용성 캐리어는 보다 긴 시간동안 점막에 부착되어 유지될 것이며 향상된 점막부착을 가져다줄 것이다. 이는 실시예 1에 더욱 설명된다.
생체접착 및/또는 점막접착 촉진제가 본 발명에 따라 상기 캐리어 입자에 부가적으로 첨가된다. 생체접착 및/또는 점막접착 촉진제는 활성제 또는 활성제들이 경구 혹은 비강 점막에 부착되는 경우 효과적이며 또한 물과 접촉시 팽창하고 확장되는 특성을 갖는다. 상기 생체/점막접착 촉진제는 그 다음 캐리어 입자의 표면상에 존재해야 한다.
용어 "점막접착"은 경구 공동내에서와 같이 점액으로 덮혀진 점막에 대한 접착을 가리키는 것을 의미하며, 용어 "생체접착"은 점액으로 덮히지 않은 점막을 포함하는 보다 일반적인 생물학적 표면에 대한 접착을 가리키는 것을 의미한다. 이러한 표현은 일반적으로 정의된 것과 중복되며, 상호교환적으로 보통 사용될 수 있으나, 용어 "생체접착"은 다소 보다 넓은 견지를 갖는다. 본 명세서 및 청구범위에서, 두 표현은 본 발명의 대상과 관련하여 동일한 목적을 제공하며, 이는 일반적인 용어 "생체/점막접착"의 사용으로 표현된다.
적절히 상기 캐리어 입자는 총 조성물 기준으로 생체/점막접착 촉진 화합물의 0.1 내지 최고 40중량%로 함유된다. 실제로, 1중량%미만의 함량은 불충분한 생체/점막접착 효과를 제공하는 것으로 발견되었다. 생체/점막접착 촉진제 함량의 바람직한 범위는 2-25중량%이다.
상기 생체/점막접착 촉진제는 중합체성 물질이 바람직하며, 바람직하게 5,000(중량 평균)이상의 평균 분자량을 갖는 물질이다. 점막접착 촉진제 접촉 영역의 수화 수준은 생체/점막접착력의 전개에 중요하다. 따라서, 중합체의 팽창이 빠를수록 생체/점막접착은 더욱 빨라진다. 생체접착 화합물의 수화는 또한 본 발명에 따라 흡착 증강제로서 유용하게 한다.
바람직하게, 상기 캐리어 입자 크기는 750㎛미만, 보다 바람직하게 50-500㎛이다. 상기 범위를 벗어나는 입자 크기도 사용될 수 있으나, 이러한 범위를 갖는 입자로부터 약학 제조물을 배합하는 경우 실질적인 어려움을 겪는다. 사용되는 캐리어는 약학적으로 허용가능하며, 물에 불용적이거나 간헐적 용해성이며 그리고 생체/점막접착 촉진제를 편입시키기에 적합한 입자내로 배합될 수 있는 어느 물질을 포함할 수 있다. 다수의 이러한 물질은 당업자에게 알려져 있다. 적절한 예는 셀룰로즈(예, 마이크로-크리스탈린 셀룰로즈), 셀룰로즈 유도체, 전분, 전분 유도체, 예를들어, 전분, 셀룰로즈 및 폴리비닐피롤리돈에 기초한 가교 중합체와 같은 중합체들이 언급될 수 있다. 또한, 칼슘 포스페이트, 디칼슘 포스페이트 하이드레이트, 디칼슘 포스페이트 디하이드레이트, 트리칼슘 포스페이트, 칼슘 카보네이트, 및 바륨 설페이트와 같은 무기염이 사용될 수 있다. 상기 언급된 물질들의 혼합물 또는 동시-가공질이 또한 사용될 수 있다.
본 발명의 특히 바람직한 견지에 따라, 상기 캐리어는 또한 프레그먼팅 작용을 갖는다. 프레그먼팅 작용이란 일부를 형성하는 약학 조성물이 정제내로 압축되는 경우 쉽게 분쇄 혹은 부서지는 정도로, 캐리어가 부서지기 쉬운 물질로 만드는 것을 의미한다. 이러한 효과는 특히 생체/점막접착 촉진제가 또한 붕괴제로 제공되는 경우에 뚜렷하다. 디칼슘 포스페이트는 프레그먼트화 촉진제로서 특히 적절한 것으로 발견되었다.
상기 조성물에 약학적으로 허용가능한 계면활성제의 첨가 또한 본 발명의 바람직한 특징이다. 계면활성제의 증가된 습윤 효과는 캐리어 입자의 습윤을 증가시키며, 이는 생체/점막접착의 보다 빠른 개시를 일으킨다. 상기 계면활성제는 미세하게 분산된 형태이며 활성제 또는 활성제들과 친밀히 혼합되어야 한다. 계면활성제의 양은 조성물의 0.5-5중량%, 바람직하게 0.5-2중량%이어야 한다.
적절한 계면활성제의 예는 소디움 라우릴 설페이트, 폴리소르베이트, 담즙산염 및 이들의 혼합물일 수 있다.

사용된 생체/점막접착 촉진제의 타입 및 비율에 따라, 생체/점막접착의 속도 및 강도가 달라질 수 있다. 본 발명의 바람직한 일 견지에 따르면, 고 팽창 및 신속한 팽창능을 갖는 물질이 바람직하다.
그밖에 생체/점막접착 촉진제가 첨가되는 경우 본 발명의 약학 조성물이 적절히 작용하기위해, 그 제제는 캐리어 입자의 표면에 위치해야 한다. 그 다음 그 생체/점막접착 촉진제는 여러가지 방법으로 상기 캐리어 입자와 혼합될 수 있다. 본 발명의 바람직한 구현으로, 미세한 입자성 질의 생체/점막접착 촉진제가 정렬된 혼합물을 생성하기에 충분한 시간동안 거친 캐리어와 함께 혼합되며, 여기서 보다 미세한 입자는 캐리어 입자의 표면에 부착된 분리된 일차 입자로 존재한다. 따라서, 상기 생체/점막접착 촉진제는 유럽특허 제 0 324 725에 기술된 활성 화합물과 동일한 방법으로 혼합된다.
상기 생체/점막접착 촉진제는 1-100㎛의 입자크기를 갖는 것이 적절하다. 상기 촉진제의 입자가 캐리어 입자와 혼합되어 정렬된 혼합물을 형성하는 경우, 이들의 크기는 보다 낮은 부분의 크기폭을 가지며, 적절히 이들의 크기는 10㎛미만이다.

증강된 흡수에 유용하며 그 작용의 신속한 개시가 필요한 의학 조건에 사용될 수 있는 다른 약물은 이에 한정하는 것은 아니나 심방성 나트륨이뇨펩타이드(ANP, ANF, 오리쿨린)(이뇨), 뇌 심방성 펩타이드(이뇨), 혈소판 응집 억제제항응고), 스트렙토키나아제(항응고), 헤파린(항응고), 유로키나아제(항응고), 레닌 억제제(고혈압), 인슐린(항당뇨), 및 수면 유도 펩타이드(불면증)를 포함한다.
위산에 대한 노출이 회피되어야 하며 활성 약물을 함유하는 타액의 팽창이 본 발명의 생체/점막접착 특성의 수단에 의해 최소화될 수 있는 약물의 다른 예는 이에 한정하는 것은 아니나, 오메프라졸, 펜토프라졸, 퍼프라졸 및 란소프라졸과 같은 H+, K+ 및 ATPase 억제제(위산 감소)로서 사용된 벤즈이미다졸 유도체를 포함한다. 다른 H+, K+ 및 ATPase 억제제는 알릴 이소티오시아네이트, 트리플루오로페라지드, 놀리늄 브로마이드, RP 40749 및 페녹티민을 포함한다.
본 발명은 특히 시트레이트 또는 말레이트와 같은 물에 쉽게 용해되지않는 펜타닐 및 이의 약학적으로 허용가능한 염의 투여에 적절하다. 펜타닐 또는 이들의 염의 입자는 약 24㎛의 최대 입자 크기를 가지나 바람직하게 약 10㎛보다 크지 않을 것이다. 펜타닐은 예를들어, 충분한 기간의 시간동안 성분들을 건조 혼합하여 부착된다. 이 시간은 사용되는 혼합장치에 따라 달라질 수 있다. 당업자는 주어진 조합의 활성 물질, 생체/점막접착 촉진제, 및 캐리어에 대한 적절한 혼합 시간을 특정 혼합 장치를 이용하여 실험에 의해 쉽게 결정할 수 있을 것이다.
본 발명의 보다 바람직한 견지는 본 발명의 조성물내에 분해제의 편입을 포함한다. 이러한 제제는 캐리어 입자의 분산을 촉진할 것이다. 본 발명에 따른 분해제의 예는 가교 폴리비닐피롤리돈, 카르복시메틸 전분, 천연 전분, 마이크로크리스탈린 셀룰로즈, 셀룰로즈 검 및 이들의 혼합물을 포함한다. 분해제의 바람직한 함량은 조성물의 1-10%이다. 알 수 있는 바와 같이, 분해제 및 생체/점막접착 촉진제의 정의는 다소 중복되며 모든 작용은 동일한 물질에 의해 제공되는 것이 바람직할 수 있다. 그러나, 이러한 두 카테고리의 부형제는 동등하지 않으며, 생체/점막접착 특성을 갖지 않는 분해제가 효율적으로 작용하는 것이 있으며 그 역 또한 그러함이 주의되어야 한다.
본 발명에 따라 제조된 정렬된 혼합물은 비강내 투여용과 같이 사용될 수 있다. 일반적으로 분말 혼합물은 동일한 타입의 운반 기구의 보조에 의해 비강으로 취입된다. 정렬된 혼합물은 설하 투여용으로 의도된 여러가지 약학 제조물내로 또한 편입될 수 있다. 제조물에 주어진 형태에 상관없이, 그 생체/점막접착 촉진 특성은 물 또는 타액과 접촉되는 경우 실질적으로 즉각적인 수화를 일으키기때문에 그 제조물은 필수적으로 물을 함유하지 않아야 하는 것이 중요하다. 너무 이른 수화는 점막접착 촉진 특성을 크게 감소시키며 활성 물질의 너무 이른 용해를 일으킨다.
바람직한 설하 투여 경로용 약학 조성물은 상기 언급한 정렬된 혼합물과 통상적인 약학 첨가제 및 설하 제조물에 대해 당업계에서 사용하는 부형제를 혼합함으로써 얻어질 수 있다. 적절한 배합 방법은 당업자에게 잘 알려져 있다. 예를들어, Pharmaceutical Dosage Forms: Tablets. 따라서, 본 발명은 제조하기에 간편하고 저가이며, 신속한 활성 물질 방출이 가능하며, 경구 또는 비강 점막을 통해 활성제 또는 활성제들의 신속한 흡수를 촉진하며, 그리고 펩타이드와 같은 다른 저조한 용해성 물질의 흡수를 증가시키는 투여 형태를 제공한다. 활성제의 저투여 사용은 짧은 작용기간을 지지하며 재발하는 급성 질환의 치료가 요구되는 환자에 반복된 투여 스케쥴을 가능케한다.
본 발명은 이하 실시예를 통해 보다 상세히 설명될 것이다.
실시예

재료
저 수용해도를 갖는 디베이직 칼슘 포스페이트 디하이드레이트(DCP)(Emcompress, Edward Mendell Co, Inc, USA) 및 고 수용해도를 갖는 만니톨(과립질, Roquette, France)이 정렬된 혼합물의 제조시 비-생체접착 캐리어 물질로 사용되었다. 각 물질에 대해 180-355㎛의 크기 분획이 드라이 시이빙(Retsch, Germany)에 의해 획득되었다.
가교 카르복시메틸 셀룰로즈 소디움(Ac-Di-Sol, FMC, Cork, Ireland)은 점막접착/생체접착 특성을 갖는 물질을 나타내기위해 미세 분리된 형태로 사용되었다. Ac-Di-Sol의 미세 입자 크기 분획은 모르타르 그라인더(Retsch, Germany)에서 밀링한 다음 에어 클래시피케이션(100MZR, Alpine, Germany)에 의해 획득되었다.
시험 물질의 일차 특성화
모든 분말은 특성화 및 혼합전에 40% RH 및 실온에서 적어도 48시간동안 보관되었다. 만니톨 및 DCP의 보다 거친 크기 분획(180-355㎛)의 외부 표면적은 프리드리치 퍼미어미트리(n=3)를 사용하여 측정되었다(Eriksson 등 1990). 블레인 퍼미어미트리는 Ac-Di-Sol 분말의 외부 표면적을 측정하는데 사용되었다(Alderborn 등 1985)(표 1).
정렬된/상호작용성 혼합물의 제조
밀링된 Ac-Di-Sol(표 1)은 여러가지 비율로 만니톨 또는 DCP(모두 180-355㎛)에 첨가되어 다른 농도의 AC-Di-Sol을 획득하였다. 상기 분말은 2L Turbula 혼합기(W.A. Bachofen AG, Basel, Switzerland)내 유리단지에서 120rpm으로 24시간동안 혼합되었다. 혼합은 종래 연구(Westerberg 1992; Sundell-Bredenberg and Nystron 2001)에 따라 수행되었으며 그 혼합물 균질성은 육안으로 확인되었다.
생체접착/점막접착 특성의 측정
재료 및 점막의 특성화
신선한 돼지장을 도살장(Swedish Meat AB, Uppsosala, Sweden)에서 수집하고 바로 사용하거나 사용할때까지 냉동하였다. 사용전, 냉동된 장은 4℃에서 밤새 버퍼 용액에서 해동되었다. 사용된 버퍼 용액은 pH 7.4를 갖는 Krebs-Ringer Bicarbonate(Sigma-Aldrich Chemie GmbH, Steinheim, Germany)이었다.
점액층의 질 및 그 점막을 취급하는 영향을 시험하기위해, 여러가지 조직 시편을 Corne 등(1974)에 의한 방법에 따라 알카인 블루로 염색하였다. 신선한 조직 및 냉동 조직 모두 알카인 블루 8 GX(Certistain, Me가, Germany)(1mg/ml)를 이용하여 TRIS(TRIZMA Hydrochloride, Sigma-Aldrich Chemie GmbH, Steinheim, Germany) 버퍼드 수크로즈 용액(Sigma-Aldrich Chemie GmbH, Steinheim, Germany)에 2시간동안 적셔졌다. 그 조직은 TRIS/수크로즈 버퍼에 린스되고 시각적으로 조사되었다. 점막층의 질 및 조직을 취급하는 영향 평가시 점막의 질에 영향을 주는 해동 공정(4℃ 버퍼 용액에서)이나 취급은 없었다. 즉, 점막층은 그대로 유지되었으며 따라서 신선한 점막 및 냉동 점막 모두 본 연구에 사용되었다.
접착 시험
5kg 로드 셀 및 관련 소프트웨어가 장착된 TA-HDi 텍스쳐 분석기(Stable Micro Systems, Haslemere, UK)가 생체접착 연구를 위해 사용되었다. 돼지장은 약 2㎠ 조각으로 잘려져 조직 홀더에 놓여졌다. [양면 테이프(Scotch, 3M Svenska AB, Sollentuna, Sweden)를 이용하여] 분말 혼합물을 상단 프로브에 부착하였다. 분말 혼합물의 적용은 분말 베드에 프로브를 담금으로써 수행되었고 그 후 입자 단일층을 달성하기위해 프로브를 부드럽게 흔들어 어떠한 초과분을 제거하였으며, 이는 육안으로 확인되었다. 점막에 30㎕의 버퍼를 스프레딩하여 수화를 표준화한 후, 조사 물질은 0.5N의 힘하에 30초동안 점막과 접촉되었다. 그 다음 프로브는 0.1mm/s의 일정 속도로 상승되고 분리힘은 대체 함수로서 기록되었다. 분리힘은 측정 사이클에 걸쳐 25치수/초의 샘플링 속도로 측정되었다. 체크된 최대힘, 즉 파쇄력은 컴퓨터 소프트웨어 Texture Expert Exceed(Stable Microsystems, Haslemere, UK)를 이용하여 검출되었다. 장력 자극(N/㎠)은 분리힘을 프로브의 면적으로 나눔으로써 얻어졌다.

생체접착/점막접착 성분의 양의 영향
점막과 비-생체접착 캐리어 입자 사이의 장력 자극은 거친 DCP 또는 만니톨이 미세한 입자크기의 Ac-Di-Sol와 혼합된 경우 향상되었다(도 1). 생체접착 특성은 Ac-Di-Sol의 농도 증가시 초기에 향상되었다(p<0.05).
두 최고 농도의 Ac-Di-Sol(28.2 및 39.3% w/w)를 함유하는 DCP의 정렬된 혼합물은 순수 Ac-Di-Sol의 분말보다 현저히 높은 장력 자극값을 나타내었다(p<0.05). 그러나 이러한 영향은 하기에 언급한 바와 같이 만니톨을 함유하는 혼합물을 이용한 경우에는 나타나지 않았다(p>0.1). 도 1에 나타낸 바와 같이, 생체접착 강도의 증가는 첨가된 Ac-Di-Sol의 특정 양까지 현저하다(p<0.01). 만일 약 20% w/w를 초과하는 경우에는 장력 자극이 현저히 증가한다(p>0.1).
캐리어
용해도의 영향
DCP 혼합물은 현저히 보다 생체접착성이었다(만니톨 혼합물보다 높은 장력 자극을 가졌다). 이는 만니톨의 보다 높은 수용해도 때문일 수 있다. 따라서, 만니톨 혼합물에 대한 분쇄는 전적으로 점막층을 통해 일어나는 것이 아니라 상호작용성 혼합물의 용해된 주변 영역을 통해 일어날 수 있다.

결론
점막과 보다 거친 만니톨 또는 DCP 분말 사이의 장력 자극은 이들이 미세 입자 Ac-Di-Sol과 혼합된 경우 향상되었다(p<0.0001). 이는 보다 높은 접착 경향을 갖는 물질의 첨가는 캐리어 물질과 같은 또 다른 덜 생체접착성 물질의 접착성을 증기시킬 것이다. 수용해성 캐리어보다는 수-불용성 캐리어와 생체접착 분말의 상호작용성 혼합물의 사용은 예기치않게 특히 단일입자성 표면 범위에 가까운 비율로 우수하다. 따라서, 간헐적 용해성 캐리어를 사용하는 이러한 상호작용성 혼합물은 설하 투여를 위한 즉각적인 방출 배합물과 같은 생체접착성 배합물의 개발에 중요한 배합 수단이다.
실시예
2. 생체/점막접착 촉진 특성을 갖는 신속 분해 정제의 제조
하기 조건에 따라 1000 정제 배치를 제조하였다: 약 250-450미크론의 입자 크기를 갖는 디베이직 칼슘 포스페어 디하이드레이트(DCP) 82.5g을 50시간에 걸쳐 미소화 펜타닐 500mg과 혼합하였다. 그 결과물인 혼합물을 미소화 소디움 알기네이트(생체/점막접착 촉진제) 10.0g과 5시간에 걸쳐 혼합하였다. 이후, Avicel® Ph 101(바인더로서 작용) 5.0g과 Ac-Di-Sol®(효과적인 분해제로서 작용하는 변형된 셀룰로즈검) 2.0g을 60분동안 혼합하였다. 그 결과물인 혼합물을 마그네슘 스테아레이트(윤활제) 0.5g과 2분동안 혼합하고, 그 다음 최종 정제 덩어리를 200MPa의 압축 압력으로 정제로 압축되었으며, 각 정제는 100mg의 중량을 가지며 펜타닐 0.5mg 을 함유하였다.

설하 투여시 거대분자의 흡수를 부가적으로 증가시키는 생체/점막접착 특성을 갖는 신속 분해 정제를 실시예 2에 따라 제조하였으며, 각 정제는 ANP 0.7mg을 함유하였다. 그러나, 이 조성물에서 소디움 알기네이트는 제거되고 Ac-Di-Sol®의 첨가는 5.0g으로 증가하였으며, 이는 이제 분해제 및 생체접착 성분 모두로 작용한다. 이 정제는 통상적인 경구 배합물과 비교하여 ANP의 신속한 방출 및 경구 배합물을 통한 ANP의 증가된 흡수를 나타낸다. 이 제조물은 폐부종의 치료용으로 사용될 수 있다.

오메프라졸
투여용 신속 분해 정제의 제조
설하 투여용 생체/점막접착 특성을 갖는 신속 분해 정제를 실시예 3에 따라 제조하였으며, 각 정제는 오메프라졸 10mg을 함유하였다. 상기 정제는 통상적인 경구 배합물에 비해 오메프라졸의 신속한 방출 및 경구 점막을 통한 오메프라졸의 증가된 흡수 뿐만 아니라 타액에서 오메프라졸의 감소된 목넘김을 나타낸다. 이 제조물은 위식도 역류의 치료용으로 사용될 수 있다.

분말의 제조
비강내 투여용 생체/점막접착 특성을 갖는 정렬된 혼합물을 실시예 2에 따라 제조하였으며, 각 투여량의 분말 혼합물은 ANP 0.7mg을 함유하였다. 실시예 2의 조성물과 반대로, 압축 및 후속적인 바인더(Avicel® Ph 101) 첨가가 이루어진 정제는 없었으며, 분해제(Ac-Di-Sol®)나 윤활제(마그네슘 스테아레이트)를 첨가하지 않았다. 비강내로 취입후, 상기 분말은 통상적인 경구 배합물과 비교하여 ANP의 신속한 용해 및 비점막을 통한 ANP의 증가된 흡수를 나타내었다. 이 제조물은 폐부종의 치료용으로 사용될 수 있다.
앞선 설명에서, 본 발명은 여러가지 실시예 및 바람직한 구현을 참고로 설명되었다. 그러나, 당분야의 숙련자에게는 본 발명의 견지가 이러한 실시예 및 구현으로 제한되지 않으며, 본 발명의 개념을 벗어나지 않고 다른 개조 및 변형이 가능하리라는 것이 명백하다. 따라서 본 발명의 견지는 첨부된 청구범위로 제한되는 것은 아니다.

- 제 1항 내지 제 4항중 어느 한 항에 있어서, 상기 캐리어 입자는 압축시 쉽게 프레그먼트화되는 부서지기쉬운 물질을 포함하는 것을 특징으로 하는 조성물.
- 제 1항 내지 제 4항중 어느 한 항에 있어서, 상기 캐리어 입자는 전체 조성물 기준으로 생체/점막접착 촉진제 0.1-40중량%를 함유하며, 바람직하게 2-25중량%를 함유하는 것을 특징으로 하는 조성물.
- 제 5항에 있어서, 상기 생체/점막접착 촉진제는 가교 중합체, 아크릴 중합체, 셀룰로즈 유도체, 생체/점막접착 특성을 갖는 천연 중합체, 및 이들의 혼합물로 구성되는 그룹으로부터 선택되는 것을 특징으로 하는 조성물.
- 제 6항에 있어서, 상기 생체/점막접착 촉진제는 히드록시프로필메틸 셀룰로즈, 히드록시에틸 셀룰로즈, 히드록시프로필 셀룰로즈, 소디움 카복시메틸 셀룰로즈, 메틸 셀룰로즈, 에틸 히드록시에틸 셀룰로즈, 카복시메틸 셀룰로즈 및 변형 셀룰로즈검을 포함하는 셀룰로즈 유도체; 크로스카라멜로즈; 변형 전분; 카보머 및 그 유도체를 포함하는 아크릴 중합체; 폴리에틸렌 옥사이드; 키토산; 젤라틴; 소디움 알기네이트; 펙틴; 스클레로글루칸; 잔탄검; 구아검; 폴리-코-(메틸 비닐 에테르-말레익 안하이드라이드); 및 이들의 혼합물로 구성되는 그룹으로부터 선택되는 것을 특징으로 하는 조성물.
- 제 1항 내지 제 7항중 어느 한 항에 있어서, 미세하게 분산된 형태이며 상기 활성제 또는 활성제들과 친밀히 혼합된 약학적으로 허용가능한 계면활성제를 더욱 포함하는 것을 특징으로 하는 조성물.
- 제 8항에 있어서, 상기 계면활성제는 조성물의 약 0.5-5중량%의 양으로 존재하며, 바람직하게 0.5-3중량%의 양으로 존재하는 것을 특징으로 하는 조성물.
- 제 8항 또는 제 9항에 있어서, 상기 계면활성제는 소디움 라우릴 설페이트, 폴리소르베이트, 담즙산염 및 이들의 혼합물로 구성되는 그룹으로부터 선택되는 것을 특징으로 하는 조성물.
- 제 1항 내지 제 10항중 어느 한 항에 있어서, 상기 캐리어 입자는 약학적으로 허용가능한 중합체, 약학적으로 허용가능한 무기염 및 이러한 물질의 혼합물 또는 동시-가공질(co-processed qualities)로 구성되는 그룹으로부터 선택된 적어도 하나의 물질을 포함하는 것을 특징으로 하는 조성물.
- 제 11항에 있어서, 상기 캐리어 입자는 셀룰로즈, 셀룰로즈 유도체, 전분, 전분 유도체, 전분 또는 셀룰로즈에 기초한 가교 중합체, 또는 폴리비닐피롤리돈으로 구성되는 그룹으로부터 선택된 적어도 하나의 물질을 포함하는 것을 특징으로 하는 조성물.
- 제 11항에 있어서, 상기 무기염은 칼슘 포스페이트, 디칼슘 포스페이트 하이드레이트, 디칼슘 포스페이트 디하이드레이트, 트리칼슘 포스페이트, 칼슘 카보네이트 및 바륨 설페이트로 구성되는 그룹으로부터 선택되는 것을 특징으로 하는 조성물.
- 제 1항 내지 제 13항중 어느 한 항에 있어서, 상기 조성물은 혼합된 활성제 또는 활성제들과 캐리어 입자의 분산을 설하 점막에 걸쳐 촉진하는 적어도 하나의 약학적으로 허용가능한 분해제를 포함하는 것을 특징으로 하는 설하 조성물.
- 제 14항에 있어서, 상기 분해제는 가교 폴리비닐피롤리돈, 카르복시메틸 전분, 천연 전분, 마이크로크리스탈린 셀룰로즈, 셀룰로즈검, 및 이들의 혼합물로 구성되는 그룹으로부터 선택되는 것을 특징으로 하는 설하 조성물.
- 제 14항 또는 제 15항에 있어서, 상기 분해제는 조성물의 1-10중량%의 양으로 존재하는 것을 특징으로 하는 설하 조성물.
- 제 1항 내지 제 16항중 어느 한 항에 있어서, 약학 활성제는 펜타닐 또는 이들의 약학적으로 허용가능한 염인 것을 특징으로 하는 조성물.
- 제 1항 내지 제 17항중 어느 한 항에 있어서, 설하 투여에 의한 급성 질환의 치료용으로 사용되는 것을 특징으로 하는 조성물.
- 제 18항에 있어서, 펜타닐 또는 이들의 약학적으로 허용가능한 염의 설하 투여에 의한 급성 또는 브레이크스루 통증(breakthrough pain)의 치료용으로 사용되는 것을 특징으로 하는 조성물.
- 제 1항 내지 제 13항 또는 제 17항중 어느 한 항에 있어서, 비강내 투여에 의한 급성 질환의 치료용으로 사용되는 것을 특징으로 하는 조성물.
- 제 20항에 있어서, 펜타닐 또는 이들의 약학적으로 허용가능한 염의 비강내 투여에 의한 급성 또는 브레이크스루 통증의 치료용으로 사용되는 것을 특징으로 하는 조성물.
- 급성 질환으로 고통받는 개체에게 마이크로입자보다 실질적으로 크며 물에 간헐적으로 용해성인 캐리어 입자의 표면에 부착된 마이크로입자 형태의 유효량의 적어도 하나의 약학 활성제, 및 상기 캐리어 입자의 표면에 부착된 생체접착 및/또는 점막접착 촉진제를 포함하는 본질적으로 물을 함유하지 않는 약학 조성물을 적어도 1 투여 유니트로 설하 또는 비강내 투여하는 급성 질환의 치료 방법.
- 제 22항에 있어서, 상기 약학 활성제는 펜타닐 또는 이들의 약학적으로 허용가능한 염인 것을 특징으로 하는 방법.
- 제 23항에 있어서, 상기 펜타닐은 투여 유니트당 0.025-10mg의 양으로, 바람직하게 0.05-2mg의 양으로 투여되는 것을 특징으로 하는 방법.
