Abstract

본 추골간 임플랜트는 상면(11)과 저면(12)이 개방되어 있으며 두개의 측면(13,14)과 전방벽(15) 및 후방벽(16)을 가진 프레임상의 케이지(1)를 가진다. 회전체(2)가 케이지(1)내에 회전 가능하게 지지되어 있고, 회전체(2)의 외표면은 나선구조(21)를 갖는다. 나선구조(21)는 케이지(1)로부터 상면(11) 및 저면(12)을 넘어 돌출한다. 전방벽(15)은 구동공구의 도입을 위한 개구(17)를 가지고, 이에 의해 회전체(2)는 케이지(1)내에서 회전가능하다.

Description

케이지와 회전체를 갖는 추골간 임플랜트
이러한 추골간 임플랜트는 추골체의 결합시 특히 오추영역에서의 추골체 결합시 삽입된다. 각 추골간 틈새마다 하나 내지는 두개의 임플랜트가 사용된다.
종래에는 이미 다양한 형태의 추골간 임플랜트가 공지되어 있으며, 이들은 모두 다음의 단점을 가지고 있다.
-임플랜트가 추골간 영역내에 도입될 수 있도록 하기 위하여 적절한 공구를 사용하여 관련 추골들을 분리하여야 한다.
-임플랜트가 관련 추골들의 단부판 내부로 침하할 위험이 있다.
본 발명은 특허청구의 범위 제1항의 전제부에 따른 추골간 임플랜트에 관한 것이다.

본 발명은 이러한 문제들을 시정하고자 한다. 본 발명의 목적은 비워진 추골간 틈새내에 분리 공구없이 최소의 힘을 사용하여 조절가능하게 회전삽입될 수 있는 추골간 임플랜트를 제공하는 것이다.
상기 목적의 달성을 위하여 독립청구항 1의 특징부의 특징을 갖는 서두에 언급한 장치가 제공된다.
이에 의하여, 임플랜트에 대해 최소한의 힘이 소요되면, 회전체의 외표면에 형성되어 케이지의 상면 및 저면을 넘어 돌출한 나선구조에 의해 임플랜트를 조절가능하게 삽입할 수 있게 된다는 이점이 얻어진다. 이에 따라 최소의 손상을 가져오는 공지의 수술기술을 사용할 수 있다.
큰 지지면을 갖는 케이지형상의 프레임구조에 의해 임플랜트가 단부판내로 함몰되는 것이 방지된다.
케이지를 넘어 돌출한 회전체의 나선구조는 임플랜트가 그 도입 중에 회전하여 추골간 틈새내에 체결될 수 있도록 한다.
바람직한 실시예에서는 임플랜트의 회전체가 중공으로 형성되어 있고, 그 전방벽에 차단덮개를 갖는다. 이에 의하여 뼈조각이나 뼈삽입재료가 회전체내에 용이하게 충진될 수 있으며, 임플랜트는 적은 손취급으로 확고히 장착될 수 있다. 이 경우에 케이지는 상호조합 가능한 두개의 부분으로 형성되어 조립이 가능한 것이 바람직하다. 이 응용예의 경우, 회전체는 종방향홈의 형태를 갖는 관통공을 케이지의 측벽은 장공절취부를 구비하여 뼈의 신속한 고착의 조절을 가능하게 한다. 종방향홈은 바람직하게는 절삭연부를 가지며, 이것은 단부 판조각이 중공의 회전체내로 밀고 들어올 수 있도록 한다.
케이지는 직방체형상으로 회전체는 원통체형상으로 형성될 수 있으며, 혹은 케이지는 쐐기형상으로 회전체는 원추체형상으로 형성될 수 있다.
임플랜트의 지지안정성 혹은 인접한 추골체의 회전안정성을 향상시키기 위하여, 상면과 저면은 바람직하게는 종방향돌기의 형태인 삼차원구조부(18)를 가지는 것이 바람직하다.
종래 기술에 대한 본 발명에 따른 임플랜트의 다른 장점들은 다음과 같다：

도 1 내지 도 3에 분리된 상태로 도시되어 있는 추골간 임플랜트는, 그 상면(11)과 저면(12)이 개방되어 있고, 각 하나의 장공(19)을 갖는 두개의 측면(13,14)과, 개구(17)를 갖는 전방벽(15) 및 후방벽(16)을 가진 프레임상의 케이지(1)을 가진다. 케이지(1)의 형상은 본 실시예의 경우 직방체형상을 가지지만, 도 7 및 도 8에 도시되어 있는 바와 같이 쐐기형상일 수도 있다. 케이지(1)는 도 1에 도시된 바와 같이 단일체로 형성되거나, 도 7 및 도 8에 기초하여 후술되는 바와 같이 여러 부분으로 바람직하게는 두 부분으로 만들어질 수도 있다.
상면(11) 및 저면(12)은 바람직하게는 종방향홈의 형상을 갖는 삼차원 구조부(18)를 가지며, 이에 의해 임플랜트의 도입 및 인접한 회전체의 회전안정성을 개선한다.
중공의 원통형 회전체(2)가 케이지(1)내에 회전가능하게 지지되어 있다. 이를 위하여 회전체(2)는 그 후방 단부면에 핀(25)을 가지고 있고, 이 핀(25)은 케이지(1)의 후방벽(16)에 있는 절취부(26)내에 삽입된다. 회전체(2)의 외표면은 숫나사의 형태로 된 나선구조(21)를 갖는다. 나선구조(21)는 적어도 2mm의 피치와 0.8 내지 2.2mm 바람직하게는 1.5 내지 2.0mm의 깊이를 갖는다.
회전체(2)의 길이는 케이지(1)내에 상부로부터 충분히 삽입될 수 있도록 케이지(1)의 내부칫수에 맞추어져 있으며, 케이지 내에 다른 지지수단없이 회전가능하게 지지되어 수용된다(도 4 및 도 6). 회전체(2)는 그 전면(22)에, 구동공구를 수용하는 역할을 하는 6각 개구(27)를 가진 착탈가능한 차단덮개(23)를 가지며, 이에 의해 뼈조각이나 뼈삽입재료가 도입될 수 있다. 차단덮개(23)는 적어도 두개의 상호대향 배치된 돌기(28)를 가지고, 이들은 회전체(2)의 전면(22)에 있는 대응하는 홈(29)에 맞물려, 차단덮개(22)가 전면(22)에 확고히 안탁될 수 있게 한다. 또한 회전체(2)는 관통공(24)을 가지며, 이 관통공(24)은 바람직하게는 절삭연부를 갖는 종방향 절취부로서 형성된다. 절삭연부는 바람직하게는 사후절삭된다.
케이지(1)의 전방벽(15)은, 도 4에 명백히 도시된 바와 같이, 차단덮개(23)의 6각 개구(27)내로 공구가 도입될 수 있도록 개구(17)를 가지며, 이에 의해 회전체(2)는 케이지(1)내에서 회전할 수 있다.
도 5에 명백히 도시된 바와 같이, 회전체(2)의 외표면에 숫나사의 형태로 형성된 나선구조(21)는 케이지(1)로부터 상면(11) 및 저면(12)을 넘어 각각 1.0 내지 2.0mm 돌출하여 있다.
도 7 및 도 8에는 본 발병에 따른 임플랜트의 다른 실시예가 도시되어 있다. 여기서는 케이지(1)가 두 부분으로 형성되어 있고, 회전체(2)는 원추대로서 형성되어 있다. 이 임플랜트의 구조는 케이지(1)를 제외하고는 도 1 내지 도 6에 따른 실시예와 거의 동일하다. 케이지(1)는 두 개의 단위부분인 상부부분(1a)과 하부부부(1b)으로 이루어져 있다. 상부부분(1a)은 네 개의 돌기(30)를 가지고 이들은 하부부분(1b)에 대응하도록 마련된 네개의 홈(31)에 맞물릴 수 있다. 이에 의해 원추대형상의 회전체(2)는 양 단위부분(1a,1b) 사이에 간단하고 신속하게 장착되어 회전가능하게 고정될 수 있다.
케이지(1)와 회전체(2)는 바람직하게는 티탄, 티탄합금, 세라믹 혹은 생체적응성있는 플라스틱으로 만들어진다.
이하에서는 도 7 및 도 8에 따른 임플랜트가 기초하여 임상적인 적용예를 상세히 설명한다.
도 7에 도시된 중공의 회전체(2)는 뼈조각(연골 혹은 뼈삽입재료)으로 경우에 따라서는 압축된 상태로 충전되어 차단덮개(23)에 의해 차단된다. 그런 다음 회전체(2)는 케이지(1)의 양 단위부분(1a,1b) 사이에 지지되며, 이때, 회전체(2)의 핀(25)은 단위부분(1a,1b)들의 절취부(26)내에 위치하게 되고, 양 단위부분(1a,1b)을 상하에서 가볍게 가압함으로써 돌기 케이지(1)는 폐쇄되게 된다. 이제 임플랜트는 가능한한 분리공구없이 비워져 있는 추골간 틈새내에 회전삽입된다. 구동공구 바람직하게는 6각 스크류드라이버가 케이지(1)의 전방벽(13)에 있는 개구(17)를 통하여 차단덮개(23)의 6각 개구(27)내에 삽입되어, 케이지(1)내의 회전체를 가볍게 돌리며 케이지(1)를 넘어 돌출한 나선구조(21)때문에 추골간 틈새내에서 나사의 형태로 체결될 수 있다.

Claims (12)

  1. 상면(11)과 저면(12)이 개방되어 있으며 두개의 측면(13,14)과 전방벽(15) 및 후방벽(16)을 가진 프레임상의 케이지(1)를 구비한 추골간 임플랜트에 있어서,
    A) 회전체(2)가 상기 케이지(1)내에 회전가능하게 지지되어 있고, 상기 회전체(2)의 외표면은 나선구조(21)를 가지며,
    B) 상기 나선구조(21)는 상기 케이지(1)로부터 상기 상면(11) 및 저면(12)을 넘어 돌출하며,
    C) 상기 전방벽(5)은 구동공구의 도입을 위한 개구(17)를 가지고, 이에 의해 상기 회전체(2)는 상기 케이지(1)내에서 회전가능한 것을 특징으로 하는 추골간 임플랜트.
  2. 제1항에 있어서, 상기 회전체(2)는 중공으로 형성되어 있으며, 그 전방벽(22)에 바람직하게는 차단덮개(23)를 갖는 것을 특징으로 하는 추골간 임플랜트.
  3. 제1항 또는 제2항에 있어서, 상기 케이지(1)는 직방체형상으로 상기 회전체(2)는 원통체형상으로 형성되어 있는 것을 특징으로 하는 추골간 임플랜트.
  4. 제1항 또는 제2항에 있어서, 상기 케이지(1)는 쐐기형상으로 상기 회전체(2)는 원통체형상으로 형성되어 있는 것을 특징으로 하는 추골간 임플랜트.
  5. 제1항 내지 제4항중 어느 한 항에 있어서, 상기 회전체(2)는 관통공(24)을 가지며, 상기 관통공(24)은 바람직하게는 절삭연부를 가진 종방향 절취부의 형태인 것을 특징으로 하는 추골간 임플랜트.
  6. 제1항 내지 제5항중 어느 한 항에 있어서, 상기 케이지(1)는 상호 조합 가능한 두개의 부분으로 형성되어 있는 것을 특징으로 하는 추골간 임플랜트.
  7. 제1항 내지 제6항중 어느 한 항에 있어서, 상기 상면(11)과 저면(12)은 삼차원구조부(18)를 가지며, 상기 삼차원구조부(18)는 바람직하게는 종방향돌기의 형태인 것을 특징으로 하는 추골간 임플랜트.
  8. 제1항 내지 제7항중 어느 한 항에 있어서, 상기 측벽(13,14)들은 장공절취부(19)를 갖는 것을 특징으로 하는 추골간 임플랜트.
  9. 제1항 내지 제8항중 어느 한 항에 있어서, 상기 나선구조(21)는 숫나사, 나선상의 돌기 혹은 홈인 것을 특징으로 하는 추골간 임플랜트.
  10. 제1항 내지 제9항중 어느 한 항에 있어서, 상기 나선구조(21)는 케이지(1)로부터 상면(11) 및 저면(12)을 넘어 각각 1.0 내지 2.0mm, 바람직하게는 1.35 내지 1.70mm 돌출하여 있는 것을 특징으로 하는 추골간 임플랜트.
  11. 제1항 내지 제10항중 어느 한 항에 있어서, 상기 나선구조(21)의 피치는 적어도 2mm인 것을 특징으로 하는 추골간 임플랜트.
  12. 제1항 내지 제11항중 어느 한 항에 있어서, 상기 나선구조(21)의 깊이는 0.8 내지 2.2mm 바람직하게는 1.5 내지 2.0mm인 것을 특징으로 하는 추골간 임플랜트.
