Abstract
본 발명은 아졸 내성 진균 균주에 의한 진균 감염에서 스쿠알렌 옥시다제 길항제 터비나핀 및 아졸 14-알파-메틸디메틸 길항제 (플루코나졸 또는 이트라코나졸)의 배합제의 용도, 상응하는 제약 조성물, 제조 방법 및 치료 방법에 관한 것이다.
Description
본 발명은 사람의 진균 감염을 치료하는 것에 관한 것이다. 플루코나졸 및(또는) 이트라코나졸과 같은 아졸 14α-메틸디메틸라제 길항제와 터비나핀 (terbinafine)을 포함하는, 아졸 내성 효모 균주에 의한 진균 감염의 치료에 사용되는 항진균 조성물에 관한 것이다.
비록 이들은 종종 건강한 개체의 소화계 내에서 양성 공생 유기체로서 존재하지만, 특히 칸디다(Candida) 종은 기생하고 있는 숙주에서 광범위한 범위의 심각한 질병을 일으킨다. 이러한 감염은 뚜렷이 증가하는 추세이다. 구인두성 칸디다증은 인간 면역결핍 바이러스 (HIV)에 감염된 환자에서 가장 보편적인 진균 감염이다. 경구 투여로 생체 내에서 이용가능한 아졸 항진균제를 투여하여, 심각한 칸디다 감염증을 치료하는 것이 가능하다. 이러한 작용제 중 최초로 이용가능하게 된 케토코나졸 (Ketoconazole)은 만성 점액피부성 칸디다증에서 유효한 것으로 신속히 발견되었다. 그러나, 이 치료제가 이용된지 얼마되지 않아, 연장된 치료 기간 중에 케토코나졸의 최소 억제 농도 (MICs)가 상승되는 것과 관련한 임상 실패가 보고되었다. 이 문제는 이어진 플루코나졸 도입에 의해 현저해졌다. 경구 투여 후 생체 내 이용가능성이 90%를 초과하는 수용성 트리아졸인 플루코나졸은 광범위한 칸디다 감염증을 치료하기 위하여 광범위하게 사용된다. 특히, HIV 감염이 진전된 AIDS 환자의 구인두성 칸디다증 치료에 광범위하게 사용된다. 비록 구인두성 칸디다증이 통상적으로 플루오나졸에 쉽게 반응하지만, 감염을 완전히 근절하기 어렵고, 종종 치료가 종료된 후 수개월 내에 재발한다. 이러한 이유로, 많은 AIDS 환자들은 장기간 연속적으로 또는 간헐적으로 플루코나졸을 복용한다.

놀랍게도, 플루코나졸 및(또는) 이트라코나졸과 같은 아졸 14α-메틸디메틸라제 길항제와 스쿠알렌 에폭시다제 길항제 터비나핀 (라미실 (Lamisil) (등록상표))의 배합제가 아졸 내성 진균 균주에 대해 활성인 것으로 발견되었다. 이 화합물들의 배합제를 사용하여 아졸 내성 진균 균주에 기인한 인체의 진균 감염의 치료 방법을 제공한다.
적절한 아졸 14α-메틸디메틸라제 길항제는 특히 이미다졸과 트리아졸 항진균제이다.

바람직한 트리아졸 항진균제에는 테르코나졸 (J. Med. Chem. 26 (1983) 611-613), 이트라코나졸 (Antimicrobial Agents and Chemotherapy 26 (1984) 5-9), 비부나졸 (vibunazole) (Arzneim.-Forsch. 33 (1983) 546), 플루코나졸 (Antimicrobial Agents and Chemotherapy 27 (1985) 815-818), 및 유리 형태 또는, 염 또는 금속 착물 형태(영국 특허 제2161483호)가 포함된다 (이하 유리 형태를 간단히 화합물 A로 명명).
특히 바람직한 아졸은 이트라코나졸 및 플루코나졸이다.
모든 항진균제의 배합이 상승적 또는 상가적 효과를 나타내는 것은 아니고 심지어 길항 효과를 보이는 것도 문헌에 보고되었다. 따라서, 예를 들어 마틴 (E. Martin) 등의 문헌 [Antimicr. Agents and Chemother. 38 (1994) 1331-1338]에서는 플루코나졸이 암포테리신 B의 살칸디다 작용을 길항한다는 것이, 문헌 [Abstr. Ann. Meeting Am. Soc. Microbiol. 87 (1987) 392]에서는 칸디다 알비칸스 (Candida albicans)에 터비나핀 및 케토코나졸을 사용하면 항진균 활성이 증진되지 않는 다는 것이, 문헌 [Eur. J. Clin. Microbiol. Infect. Dis. 7 (1988) 732-735]에서는 시험관 내에서 터비나핀이 아졸과 길항 작용한다는 것이, 문헌 [Drugs Today 24 (1988) 705-715]에서는 배합 치료법에서 모든 항진균제의 배합제가 상승적 또는 상가 효과를 보이는 것은 아님이 다시 언급되어 있다.
따라서, 항진균제를 사용한 배합 치료법은 예견하기 어려운 것으로 보인다.
따라서, 2개의 별개의 단계에서, 단일 생합성 경로를 억제하는 약리학적 활성 물질의 배합제는 통상적으로 1 단계로만 작용하는 것보다 활성이 더 크고, 플루코나졸 및(또는) 이트라코나졸과 같은 아졸과 터비나펜의 배합제는 적어도 상가 활성을 지녀야한다는 사실을 이론적으로 예상할 수 있지만, 알려지지 않은 이유로 이러한 배합제가 아졸에 대한 내성이 이미 진전된 경우 즉, 진균 균주가 아졸에 내성을 갖는 상황에서도 상승 효과를 유지할 수 있다는 사실은 매우 놀라운 것이다.
아졸 내성 진균 균주에 의한 인체의 진균 감염을 치료하기 위한 본 발명의 조성물은 플루코나졸 및(또는) 이트라코나졸과 같은 아졸 14α-메틸디메틸라제 길항제, 및 하기 화학식의 아릴메틸아민 스쿠알레 에폭시다제 길항제 터비나핀의 유리 염기 부가염 또는 염산 부가염 형태와 같은 산 부가염 형태를 함유한다.
가장 바람직한 아졸은 플루코나졸이다. 터비나핀은 약제학적으로 허용 가능한 형태가 바람직하고, 바람직하게는 염산염 형태이다. 바람직한 진균은 효모가 바람직하고, 칸디다 속이 바람직한 경우에는 특히 칸디다 알비칸스이다. 아졸 내성이 유도된 진균증은 표면성이거나 또는 전신성일 수 있고, 특히 구인두적이다. 이는 피부 또는 점막에 유해하다.
아졸 내성은 교차 내성 (cross-resistance)일 수 있고, 다중 아졸과 관련된다.
본 발명의 항진균 조성물은 화학식 I의 아릴메틸아민을 플루코나졸 및(또는) 이트라코나졸과 같은 아졸 14α-메틸디메틸라제 길항제인 항진균제에 혼입하여 제조한다.
따라서 본 발명은 상기 정의된 터비나핀, 및 플루코나졸 및(또는) 이트라코나졸과 같은 아졸 14α-메틸디메틸라제 길항제를 포함하고, 유해한 아졸 내성 유도 진균증을 예방 또는 제한하기 위한 아졸 내성 진균 균주에 의한 진균 감염 치료용 항진균 조성물에 관한 것이다.
또한, 유해한 아졸 내성 유도된 인체에서 이러한 유해한 아졸 내성 유도 진균증을 예방 또는 제한하기 위하여 아졸 내성 진균 균주에 의한 진균 감염의 치료용 약제에서 상기 정의된 터비나핀, 및 플루코나졸 및(또는) 이트라코나졸과 같은 아졸 14α-메틸디메틸라제 길항제를 함유하는 항진균 조성물의 용도에 관한 것이다.
또한, 상기 정의된 터비나핀을 플루코나졸 및(또는) 이트라코나졸과 같은 아졸 14α-메틸디메틸라제 길항 항진균제에 혼입하는 것으로 이루어진 상기 정의된 항진균 조성물의 제조 방법에 관한 것이다.
또한, 상기 정의된 항진균 조성물의 치료적 유효량을 치료를 요하는 환자에게 투여하는 것으로 이루어진, 유해한 아졸 내성 유도된 진균증을 예방 또는 제한하기 위하여 아졸 내성 진균 균주에 의한 아졸 내성 진균 감염의 치료 방법에 관한 것이다.
본 발명의 항진균 조성물에서, 아졸 항진균제 대 아릴메틸아민 항진균제의 중량비는 광범위하게 변화될 수 있으나, 바람직하게는 100:1 내지 1:500, 더욱 바람직하게는 25:1 내지 1:125이다. 아졸 항진균제 및 아릴메틸아민 항진균제 터비나핀을 상기 범위 내에 중량비로 혼합함으로써, 아졸 내성 진균 균주, 특히 효모 균주, 예를 들어 칸디다 알비칸스(albicans), 칸디다(토룰롭시스(torulopsis)) 글라브라타 (glabrata), 칸디다 크루사이 (C. krusei) 및 칸디다 트로피칼리스 (C. tropicalis)와 같은 칸디다 균주, 크립토코커스 네오포르만스 (Cryptococcus neoformans)와 같은 크립토코커스 균주, 또는 트리코피톤 멘타그로피테스 (Trichophyton mentagrophytes)와 같은 트리코피톤 균주, 이 중 특히 칸디다 균주, 특히 칸디다 알비칸스에 의한 진균증의 치료에서 우수한 효과를 얻을 수 있다.
본 발명의 조성물은 국소 투여될 수 있고, 광범위한 농도로 (통상적으로 총 조성물의 약 0.1 중량% 내지 약 10 중량%) 일반적인 제약학적 담체에 혼입하여 제제화할 수 있다. 본 발명의 조성물은 정제, 캡슐 또는 액제의 형태로 경구 투여될 수 있고, 또한 피하, 근육내 또는 정맥내 투여와 같은 비경구 투여용으로 사용될 수 있다. 따라서, 조성물은 통상적으로 고정되어 있는 배합제이다. 그러나, 개별적으로, 예를 들어 연속적으로 순서에 관계없이 유리 복합제의 형태의 유효 성분을 투여하는 것도 역시 효과는 나타낼 수 있다.
다양한 아졸 내성 균주를 사용한 시험관 내에서 유용한 활성이 나타날 수 있다. 분석은 바둑판 무늬의 약제 희석계를 사용하는 96개 웰의 바닥이 평평한 미세희석 플레이트에서 RPMI 1640 배지를 사용하여 수행한다. 터비나핀 (염산염 형태) 및 아졸을 100 ㎍/㎖ 내지 0.006 ㎍/㎖의 농도로 사용한다. 최소 억제 농도 (MIC)는 37℃에서 48시간 동안 인큐베이션한 후 측정한다. 최소 살진균 농도 (MFC)는 약물로 처리된 세포를 약물이 없는 배지로 옮긴 지 24시간 후 측정하였다. MIC 측정에 사용된 종말점은 100% 억제 (표 1 내지 4) 또는 (통상적으로) 80% 억제 (표 2 및 3)이다 (Terb=터비나핀; Flu=플루코나졸, Itra=이트라코나졸, 씨.=칸디다).

진균은 단독으로 사용된 플루코나졸 또는 터비나핀에 노출시킨 경우와 비교하여, 플루코나졸과 터비나핀의 배합제를 사용하는 경우 하나의 균주만을 제외하고 모든 경우에서 진균 성장이 100% 억제되었다. 4개의 균주에서, 2개의 약물 복합제는 100 ㎍/㎖ 미만의 농도에서 두 약제는 살진균 작용을 나타냈고, 이 약물을 단독으로 사용하는 경우에는 얻을 수 없는 결과였다.

Claims (14)
- 화학식 I의 터비나핀의 유리 염기 또는 산 부가염 형태, 및 아졸 14α-메틸디메틸라제 길항제를 함유하고, 유해한 아졸 내성 유도 진균증을 예방 또는 제한하기 위한 아졸 내성 진균 균주에 의한 진균 감염 치료용 항진균 조성물.
- 기생하는 사람 객체에서 아졸 내성에 노출된 인체에서 유해한 아졸 내성 유도 진균증을 예방 또는 제한하기 위한 아졸 내성 진균 균주에 의한 진균 감염 치료용 약제를 제조함에 있어서, 제1항에 정의된 터비나핀 및 아졸 14α-메틸디메틸라제 길항제를 포함하는 항진균 조성물의 용도.
- 제1항에 정의된 터비나핀을 아졸 14α-메틸디메틸라제 길항제 항진균제에 혼입하는 것을 포함하는 제1항에 정의된 항진균 조성물의 제조 방법.
- 제1항에 정의된 항진균 조성물의 치료적 유효량을 치료를 요하는 환자에게 투여하는 것으로 이루어진, 유해한 아졸 내성 유도 진균증의 예방 또는 제한을 위하여 아졸 내성 진균 균주에 의한 아졸 내성 진균 감염을 치료하는 방법.
- 제1항 내지 4항 중 어느 한 항에 있어서, 터비나핀은 염산염의 형태인 조성물, 용도, 제조 방법 또는 치료 방법.
