Abstract
본 발명은, 보존 중에 환원당 및 아미노산에 의한 메일라드 반응을 일으키는 일이 없고, 보존 중에 지방 유제에 있어서의 지방 입자 사이즈가 커지는 일이 없으며, 한쪽 수액만을 투여한 경우라도 환자가 고칼륨혈증을 발증하거나 혈관통이나 정맥염을 일으키거나 할 우려가 작아, 2실 용기 제제임에도 불구하고 안정적으로 다종의 비타민을 배합할 수 있는 수액 제제를 제공하는 것을 과제로 한다.
본 발명에 의해, 연통 가능한 구획에 의해 이격된 2실을 갖는 수액 제제로서, 제1실에 당 및 지방 유제를 포함하는 제1실 수액이 수용되고, 제2실에 아미노산 및 전해질을 포함하는 제2실 수액이 수용되고, 상기 제1실 수액은 실질적으로 칼륨을 포함하지 않으며, 침투압비가 2.0∼3.0이고, 상기 제2실 수액은 칼륨 농도가 40 mEq/L 이하이며, 침투압비가 2.5∼3.5이고, 구획 연통시의 상기 제1실 수액 및 제2실 수액의 혼합액의 칼륨 농도가 16 mEq/L 이상인 수액 제제가 제공된다.

Description
본 발명은 당, 지방, 아미노산 및 전해질을 함유하는 수액 제제에 관한 것이다. 보다 상세하게는, 본 발명은 2실 용기가 달린 수액 제제로서, 제1실에 당 및 지방 유제를 수용하고, 제2실에 아미노산 및 전해질을 수용한 수액 제제(고칼로리 수액 제제)에 관한 것이다.
2실을 갖는 수액 백에 수용된, 당, 아미노산 및 전해질을 함유하는 수액 제제는 공지이며, 환자의 영양 관리에 범용되고 있다(비특허문헌 1 참조).
환원당과 아미노산을 동일 액 중에 배합하면, 메일라드 반응을 일으켜 변성된다고 하는 문제가 있었다. 따라서, 상기 수액 제제에서는, 환원당과 아미노산이 메일라드 반응을 일으키는 것을 방지하기 위해서, 수액 백의 1실에 환원당을 포함하는 수액이, 다른 실에 아미노산을 포함하는 수액이 각각 분별되어 수용된다. 사용시에는, 2실을 연통하여 각각의 액을 혼합한 후, 환자에게 투여한다. 이 혼합 조작을 간편히 행할 수 있도록 하기 위해서, 이들 2실은 용시에 연통 가능한 격벽(예컨대 이박리(易剝離) 시일(이지 필 시일))에 의해 이격되어 있는 것이 통상이다. 그러나, 이 연통 조작을 하는 것을 잊어서 한쪽의 액만을 환자에게 투여하여 버린다고 하는 실수가 종종 생긴다. 따라서, 이러한 2실 수용 타입의 수액 제제에서는, 가령 사용 전에 연통 조작을 하는 것을 잊어서 한쪽 실에 수용되는 수액만이 투여된다고 사태가 발생했다고 해도, 환자에게 해가 가지 않도록 해 두는 것이 매우 중요하다.
예컨대, 한쪽 수액의 칼륨 농도가 비교적 높은 경우, 그 수액만을 투여함으로써 환자가 고칼륨혈증을 초래할 우려가 있다. 이러한 문제를 해소하기 위해서, 칼륨을 2실로 나눠 수용하여, 각 수액 중의 칼륨 농도를 40 mEq/L 이하로 한 수액 제제가 검토되어 있다(특허문헌 1).
또한, 특히 말초정맥으로부터 점적에 의해 수액을 투여하는 경우, 침투압이 너무 높으면, 혈관통이나 정맥염을 야기할 우려가 있다. 따라서, 혼합 전의 수액이라도 적절한 침투압을 갖는 것이 바람직하다고 되어 있다(특허문헌 2).
또한, 당, 아미노산 및 전해질을 함유하는 수액 제제(고칼로리 수액 제제)만을 장기간 투여하고 있는 경우, 필수 지방산 결핍증을 발증할 우려가 있다. 고칼로리 수액 제제와 더불어 지방 유제를 투여함으로써, 필수 지방산 결핍증의 발증을 예방할 수 있다. 또한, 지방은 중량당 칼로리량이 높고, 당질과 달리 침투압 이뇨를 일으키지 않는 등의 이점도 갖는다. 그러나, 전해질 수액과 지방 유제를 혼합하여 장시간 경과하면, 지방 액적의 사이즈가 커져, 투여하면 지방 색전을 일으킬 우려가 있었다. 이 때문에, 지방 유제와 전해질을 다른 실에 수용하고, 사용시에 혼합하여 투여하는 2실 용기가 달린 수액 제제가 개발되어 있다.
더욱이, 고칼로리 수액 시행 중에 비타민 B1이 부족하면, 아시도시스가 발생할 우려가 있다는 것이 알려져 있으며, 이것을 방지하기 위해서 고칼로리 수액 투여시에는 비타민을 보급할 것이 요구되고 있다. 이러한 수고를 없애기 위해서, 당, 아미노산, 전해질에 더하여 비타민이 미리 배합된, 대실액, 중실액 및 소실액의 3액으로 이루어지는 고칼로리 수액 제제도 개발되어 있다(예컨대, 풀칼릭(Fulcaliq) 수액 제제 등). 이 고칼로리 수액 제제에서는, 비타민(특히 지용성 비타민)의 안정성 문제 때문에, 대실, 중실 및 소실의 3실에 다른 조성의 수액을 수용하는 형태가 채용되고 있지만, 3실을 갖는 수액 제제는 제조시 및 사용시에 수고스러워서, 생산 비용 및 사용시의 번거로움에 문제가 있었다. 또한, 예컨대 풀칼릭 수액 제제의 첨부 문서에는, 조제시의 주의로서 지방 유제와 혼합하지 않도록 하는 지시가 되어 있다.
상술한 것과 같이, 여러 가지 과제에 대하여 지금까지 검토되어 온 수법은 각각의 과제를 해결하기 위해서는 유용하지만, 모든 과제를 해결한 수액 제제는 아직 개발되어 있지 않다.

본 발명은 상기한 과제 모두를 해결할 수 있는 수액 제제를 개발하는 것을 목적으로 한다.
본 발명자들은, 놀랍게도, 연통 가능한 구획에 의해 이격된 2실을 갖는 수액 제제로서, 제1실에 당 및 지방 유제를 포함하는 제1실 수액이 수용되고, 제2실에 아미노산 및 전해질을 포함하는 제2실 수액이 수용되며, 상기 제1실 수액은 실질적으로 칼륨을 포함하지 않으며, 침투압비가 2.0∼3.0이고, 상기 제2실 수액은 칼륨 농도가 40 mEq/L 이하이며, 침투압비가 2.5∼3.5이고, 구획 연통시의 상기 제1실 수액 및 제2실 수액의 혼합액의 칼륨 농도가 16 mEq/L 이상인 수액 제제라면, 보존 중에 환원당 및 아미노산에 의한 메일라드 반응을 일으키는 일이 없고, 보존 중에 지방 유제에 있어서의 지방 입자 사이즈가 커지는 일이 없어, 한쪽의 수액만을 투여한 경우라도 환자가 고칼륨혈증을 발증하거나 혈관통이나 정맥염을 일으키거나 할 우려가 작아, 2실 용기 제제임에도 불구하고 안정적으로 다종의 비타민을 배합할 수 있음을 알아내고, 또한 개량을 거듭하여 본 발명을 완성시키기에 이르렀다.
즉, 본 발명은 예컨대 이하의 항에 기재된 수액 제제를 포함한다.
항 1.
연통 가능한 구획에 의해 이격된 2실을 갖는 수액 제제로서,
제1실에 당 및 지방 유제를 포함하는 제1실 수액이 수용되고,
제2실에 아미노산 및 전해질을 포함하는 제2실 수액이 수용되고,
상기 제1실 수액은 실질적으로 칼륨을 포함하지 않으며, 침투압비가 2.0∼3.0이고,
상기 제2실 수액은 칼륨 농도가 40 mEq/L 이하이며, 침투압비가 2.5∼3.5이고,
구획 연통시의 상기 제1실 수액 및 제2실 수액의 혼합액의 칼륨 농도가 16 mEq/L 이상인 수액 제제.

본 발명의 수액 제제는 상기한 과제를 전부 해결한 수액 제제이다. 이 수액 제제는, 보존 중에 환원당 및 아미노산에 의한 메일라드 반응을 일으키는 일이 없고, 지방 유제에 있어서의 지방 입자 사이즈가 커지는 일도 없다. 또한, 한쪽 수액만을 투여한 경우라도 환자가 고칼륨혈증을 발증하거나 혈관통이나 정맥염을 일으키거나 하는 일이 거의 없다. 또한, 비타민 B1 배합에 의해 아시도시스 발현의 억제가 기대된다. 또한, 지용성 비타민을 포함한 다른 비타민을 배합할 수 있는 2실 용기 제제로 되기 때문에, 3실 이상으로 이격된 형태를 갖는 제제(3실 용기 제제)에 비하여 제조시 및 사용시의 수고도 경감된다.
이하, 본 발명에 관해서 더욱 상세히 설명한다.
본 발명은, 제1실에 당, 지방 유제를 포함하는 제1실 수액이 수납되고, 제2실에 아미노산 및 전해질을 포함하는 제2실 수액이 수납되는, 연통 가능한 구획에 의해 이격된 2실을 갖는 수액 제제에 관한 것이다.
제1실 수액
본 발명에서 이용되는 제1실 수액은 당, 지방 유제를 포함한다.
제1실 수액에 배합되는 당으로서는, 포도당, 프럭토스, 말토스 등의 환원당, 크실리톨, 소르비톨, 글리세린 등의 비환원당 등을 들 수 있다. 이들 중에서도, 혈당치 관리 등의 점에서, 바람직하게는 환원당이며, 더욱 바람직하게는 포도당이다. 이들 당은 1종 단독으로 사용하더라도 좋고, 또한 2종 이상을 조합시켜 사용하더라도 좋다.
제1실 수액에 있어서의 당의 배합 비율은 70∼150 g/L의 범위로 설정되는 것이 바람직하다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 당의 농도는 바람직하게는 50∼100 g/L, 보다 바람직하게는 50∼75 g/L의 범위를 만족시키는 범위가 예시된다.
또한, 수액 요법 시행 중의 아시도시스 발증을 예방하기 위해서, 제1실 수액에는 비타민 B1을 배합하는 것이 바람직하다. 제1실 수액에 배합되는 비타민 B1으로서는 타아민염화물염산염, 타아민질산염, 프로설티아민, 옥토티아민 등을 사용할 수 있다.
제1실 수액에 있어서의 비타민 B1의 배합 비율은 티아민으로서 1.5∼10 mg/L, 바람직하게는 2∼8 mg/L를 충족하는 범위가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 B1의 농도는 티아민으로서 1∼6 mg/L, 바람직하게는 1.5∼4 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
또한, 제1실 수액에 배합되는 지방 유제는, 유지를, 유화제를 이용하여 물에 분산시켜 조제된 수중유형 유제이다. 상기 지방 유제의 조제는 통상의 방법에 준하여 행할 수 있다. 예컨대, 물에 유지 및 유화제를 가한 후, 교반하여 조유화액(粗乳化液)을 조제하고, 이어서 조유화액을 고압 유화법 등의 관용의 방법에 의해 유화함으로써 조제할 수 있다.
유지로서는 식용유를 바람직하게 이용할 수 있다. 예컨대 식물유(대두유, 올리브유, 면실유, 홍화유, 옥수수유, 야자유, 차조기유, 들기름 등), 어유(대구간유 등), 중쇄 지방산 트리글리세리드(탄소수 8-10의 지방산의 트리글리세리드)[시판 제품명: 「파나세이트」(니혼유시사 제조), 「ODO」(닛신세이유사 제조), 「코코나아드」(가오사 제조), 「미글리올」(미츠바보에키사) 등], 화학 합성 트리글리세리드류[2-리놀레오일-1,3-디옥타노일글리세롤(8L8), 2-리놀레오일-1,3-디데카노일글리세롤(10L10) 등] 등이 예시된다. 이들은 1종 단독으로 사용하더라도 좋고, 또한 2종 이상을 조합시켜 사용하더라도 좋다.
유화제로서는 예컨대 제제학적으로 허용되는 각종 유화제를 이용할 수 있다. 예컨대 난황 인지질(난황 레시틴), 수소 첨가 난황 인지질, 대두 인지질(대두 레시틴), 수소 첨가 대두 인지질 등, 또한, 비이온성 계면활성제 등이 예시된다. 이들은 1종 단독으로 사용하더라도 좋고, 또한 2종 이상을 조합시켜 사용하더라도 좋다.
특히 바람직한 유지로서는 대두유를, 특히 바람직한 유화제로서는 난황 인지질(난황 레시틴)을 각각 들 수 있다. 특히 난황 레시틴 등의 레시틴을 이용하면, 후술하는 것과 같이 인 공급원으로도 되어 적합하다.
지방 유제의 조제에 이용되는 유지 및 유화제의 사용 비율은 수중유형 지방 유제를 얻을 수 있는 한 특별히 한정되지 않는다. 통상, 유지는 얻어지는 지방 유제 중에 0.5∼6 w/v% 정도, 바람직하게는 1∼5 w/v% 정도가 되는 비율로 이용된다. 또한, 유화제는 얻어지는 지방 유제 중에 통상 0.01∼2 w/v% 정도, 바람직하게는 0.05∼1 w/v% 정도가 되는 범위에서 이용된다.
본 발명에 특히 적합한 지방 유제의 제법의 하나의 구체예는 다음과 같다. 즉, 물에 유지 및 유화제를 가하는 동시에 글리세린 및 포도당에서 선택되는 적어도 1종을 가하고, 그 후, 교반하여 조유화액을 조제하고, 이어서 조유화액을 고압 유화법 등의 관용의 방법에 의해 유화한다. 고압 유화법을 채용하는 경우, 이 방법은 예컨대 만톤고울린(Manton Gaulin) 호모게나이저 등의 유화기를 이용하여, 조유화액을 20∼700 Kg/㎠ 정도의 조건 하에서 2∼50회 정도, 바람직하게는 3∼20회 정도 통과시킴으로써 행할 수 있다. 한편, 이 방법에 있어서, 글리세린 및/또는 포도당은 유화할 때에 존재하면 되며, 예컨대 유지와 유화제를 이용하여 조제한 조유화액에 글리세린 및/또는 포도당을 첨가하여 유화를 하더라도 좋다. 글리세린 및/또는 포도당의 사용량은 통상 얻어지는 지방 유제가 글리세린 및/또는 포도당을 30∼70 w/v% 정도, 바람직하게는 40∼60 w/v% 정도 포함하는 것으로 하는 것이 적당하다.
또한, 필요에 따라서, 지방 유제 중에 첨가 배합할 수 있다는 것이 알려져 있는 각종 첨가제를 더욱 첨가 배합할 수도 있다. 첨가제로서는 예컨대 pH 조정제를 들 수 있다. pH 조정제로서는, 염산 등의 산이나 수산화나트륨, 수산화칼륨 등의 알칼리 외에 유기산이나 아미노산을 이용할 수 있다. 유기산으로서는 초산, 젖산, 시트르산, 말산, 호박산 등이 예시된다. 또한, 아미노산으로서는 L-히스티딘, L-리신 등이 예시된다. 이들 중, 유용성 재료는 유화액을 구성하는 유성 성분 중에 미리 혼합하여 이용할 수 있다. 수용성 재료는 주사용수에 혼합하거나 또는 얻어지는 지방 유제의 수상 중에 첨가 배합할 수 있다. 이들 첨가 배합량은 적절하게 설정할 수 있으며, 예컨대 종래 알려져 있는 이들의 첨가 배합량과 같더라도 좋다.
제1실 수액에 있어서의 지방 유제의 배합 비율은 유지량으로 환산하여 0.5∼6 w/v%, 바람직하게는 1∼5 w/v%, 보다 바람직하게는 2∼5 w/v%이다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 지방 유제량은 유지량으로 환산하여 0.25∼6 w/v%, 바람직하게는 0.5∼3 w/v%, 보다 바람직하게는 1∼2.5 w/v%이다.
제1실 수액의 pH는 4.5∼6.5의 범위 내면 되는데, 바람직하게는 5.0∼6.5가 예시된다. 이러한 pH 범위 내라면, 제1실 수액에 있어서, 지방 유제와 비타민 B1의 안정화를 도모할 수 있다. 제1실 수액의 pH 조정에는 염산, 초산, 빙초산, 젖산, 말산, 시트르산, 수산화나트륨, 수산화칼륨 등의 pH 조정제를 사용하여 실시된다. 또한, pH 조정제로서 L-히스티딘을 이용하더라도 좋다.
또한, 제1실 수액은 비타민 B1의 안정성을 한층 더 향상시킨다고 하는 관점에서 적정(滴定) 산도를 1 이하로 하는 것이 바람직하다. 한편, 적정 산도는 용액 100 mL를 pH 7.4까지 중화하는 데에 필요한 0.1 mol/L 수산화나트륨 용액의 mL량값이다.
제1실 수액의 용매로서는 통상 주사용 증류수를 사용할 수 있다.
본 발명의 수액 제제에 있어서, 제1실 수액의 액량은 그 수액 제제의 총 액량이나 제2실 수액의 액량 등에 따라서 적절히 설정된다.
제1실 수액은 실질적으로 칼륨을 포함하지 않는다. 실질적으로 칼륨을 포함하지 않는다는 것은 칼륨을 포함하는 화합물이 배합되지 않음을 의미한다.
또한, 제1실 수액의 침투압비는 2.0∼3.0 정도로 한다. 여기서의 침투압비란, 생리식염수의 침투압에 대한 비(즉, 생리식염수의 침투압을 1로 했을 때의 상대비)이다. 한편, 수액의 침투압비는 특별히 양해를 구하지 않는 한 생리식염수의 침투압에 대한 비를 말한다.
제2실 수액
본 발명에서 이용되는 제2실 수액은 아미노산 및 전해질을 포함한다.
제2실 수액에 배합되는 아미노산은 생체에의 영양 보급을 목적으로 하는 아미노산 수액에 사용되고 있는 것이면 된다. 본 발명에 있어서, 아미노산은 통상 유리 아미노산의 상태로 이용되는데, 약학적으로 허용되는 염, 에스테르체, N-아실 유도체, 디펩티드의 형태라도 좋다. 제2실 수액에 배합되는 유리 아미노산의 구체예로서는 L-류신, L-이소류신, L-발린, L-리신, L-트레오닌, L-트립토판, L-메티오닌, L-페닐알라닌, L-시스테인, L-티로신, L-아르기닌, L-히스티딘, L-알라닌, L-프롤린, L-세린, 글리신, L-아스파라긴산, L-글루타민산 등이 예시된다. 또한, 아미노산의 염으로서는, 구체적으로는, L-아르기닌염산염, L-시스테인염산염, L-글루타민산염산염, L-히스티딘염산염, L-리신염산염 등의 무기산염; L-리신초산염, L-리신말산염 등의 유기산염 등이 예시된다. 아미노산의 에스테르체로서는, 구체적으로는, L-티로신메틸에스테르, L-메티오닌메틸에스테르, L-메티오닌에틸에스테르 등이 예시된다. 아미노산의 N-아실체로서는, 구체적으로는 N-아세틸-L-시스테인, N-아세틸-L-트립토판, N-아세틸-L-프롤린 등이 예시된다. 아미노산의 디펩티드로서는, 구체적으로는, L-티로실-L-티로신, L-알라닐-L-티로신, L-아르기닌-L-티로신, L-티로신-L-아르기닌 등이 예시된다. 특히, L-시스테인은 아세틸시스테인으로서 배합되는 것이 안정성의 점에서 적합하다. 이들 아미노산은 1종 단독으로 사용하더라도 좋지만, 영양 보급의 관점에서는 2종 이상을 조합시켜 사용하는 것이 바람직하다. 바람직하게는, 적어도 모든 필수 아미노산(즉, L-류신, L-이소류신, L-발린, L-리신, L-트레오닌, L-트립토판, L-메티오닌, L-페닐알라닌, L-히스티딘의 9종의 아미노산)을 포함하는 것이 예시된다.
제2실 수액에 있어서의 아미노산의 배합 비율은 유리 아미노산의 총량으로서 바람직하게는 40∼120 g/L, 보다 바람직하게는 50∼100 g/L의 범위가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 아미노산 농도는 유리 아미노산의 총량으로서 10∼50 g/L, 바람직하게는 20∼30 g/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
또한, 제2실 수액에 배합되는 아미노산의 조합 및 배합 비율의 적합한 일례는 다음과 같다. 즉, 유리 아미노산 환산으로, L-류신: 5∼15 g/L, L-이소류신: 3∼9 g/L, L-발린: 3∼9 g/L, L-리신: 3∼12 g/L, L-트레오닌: 1.2∼6 g/L, L-트립토판: 0.3∼3 g/L, L-메티오닌: 0.6∼4.8 g/L, L-페닐알라닌: 1.8∼9 g/L, L-시스테인: 0.1∼1.8 g/L, L-티로신: 0.06∼1.2 g/L, L-아르기닌: 3∼12 g/L, L-히스티딘: 1.2∼6 g/L, L-알라닌: 3∼9 g/L, L-프롤린: 1.2∼6 g/L, L-세린: 0.6∼4.2 g/L, 글리신: 1.2∼6 g/L, L-아스파라긴산: 0.12∼1.8 g/L 및 L-글루타민산: 0.12∼1.8 g/L.
또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 아미노산 농도의 적합한 일례는 다음과 같다. 즉, 유리 아미노산 환산으로, L-류신: 3∼9 g/L, L-이소류신: 1.5∼4.5 g/L, L-발린: 1.5∼4.5 g/L, L-리신: 1.5∼5 g/L, L-트레오닌: 0.6∼3 g/L, L-트립토판: 0.15∼1.5 g/L, L-메티오닌: 0.3∼2.4 g/L, L-페닐알라닌: 0.85∼4.5 g/L, L-시스테인: 0.03∼0.9 g/L, L-티로신: 0.03∼0.6 g/L, L-아르기닌: 1.5∼5 g/L, L-히스티딘: 0.6∼3 g/L, L-알라닌: 1.5∼4.5 g/L, L-프롤린: 0.6∼3 g/L, L-세린: 0.3∼2.1 g/L, 글리신: 0.6∼3 g/L, L-아스파라긴산: 0.06∼0.9 g/L 및 L-글루타민산: 0.06∼0.9 g/L.
제2실 수액에 배합되는 전해질은 수액 분야에서 이용되는 의미에서의 전해질이며, 구체적으로는 체액(예컨대 혈액, 세포내액)에 포함되는 전해질(체액 전해질)이다. 생리학상 중요한 전해질이라고 하더라도 좋다. 보다 구체적으로는 칼륨, 칼슘, 나트륨, 마그네슘, 인, 아연, 염소 등이 예시된다. 한편, 본 발명의 수액 제제에서는 이들 전해질은 바람직하게는 제1실 수액에는 배합되지 않는다. 특히 칼륨은 고농도 칼륨의 투여에 의한 위험성을 피하기 위해서 2실 수액 제제에서는 통상 양쪽의 수액에 배합되는데, 본 발명의 수액 제제에서는 제2실 수액에만 배합된다.
칼륨 공급원으로서는, 염화칼륨, 초산칼륨, 시트르산칼륨, 글리세로인산칼륨, 황산칼륨, 젖산칼륨 등이 예시된다. 이들 중에서도, 글리세로인산칼륨은 인 공급원으로도 되기 때문에 적합하다. 이들 칼륨 공급원은 수화물 형태라도 좋다. 칼륨은 제2실 수액에 있어서의 농도가 40 mEq/L 이하(바람직하게는 25∼40 mEq/L)가 되도록 배합된다. 또한, 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 칼륨 농도는 16 mEq/L 이상(바람직하게는 16∼25 mEq/L, 보다 바람직하게는 16∼20 mEq/L)이다.
칼슘 공급원으로서는, 글루콘산칼슘, 염화칼슘, 글리세로인산칼슘, 젖산칼슘, 판토텐산칼슘, 초산칼슘 등의 칼슘염이 예시된다. 또한, 칼슘염은 수화물 형태(예컨대 글루콘산칼슘 수화물 등)라도 좋다. 칼슘은 제2실 수액에 있어서의 농도가 15 mEq/L 이하(바람직하게는 6∼12 mEq/L)가 되도록 배합된다. 또한, 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 칼슘 농도는 9 mEq/L 이하(바람직하게는 3∼6 mEq/L)이다.
나트륨 공급원으로서는, 염화나트륨, 젖산나트륨, 초산나트륨, 황산나트륨, 글리세로인산나트륨, 시트르산나트륨, 젖산나트륨 등의 나트륨염이 예시된다. 또한, 본 발명의 수액 제제에 인과, 칼슘 및/또는 마그네슘을 배합하는 경우, 이들이 침전을 일으키는 것을 방지하기 위해서, 나트륨 공급원(의 일부)으로서 시트르산나트륨을 이용하는 것이 바람직하다. 또한, 나트륨 공급원은 수화물 형태라도 좋다. 나트륨의 제2실 수액에의 배합 비율은 제2실 수액에 있어서 50∼100 mEq/L, 바람직하게는 40∼80 mEq/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 나트륨 농도는 25∼50 mEq/L, 바람직하게는 30∼40 mEq/L가 되는 범위를 만족하도록 설정되어 있는 것이 바람직하다.
마그네슘 공급원으로서는 황산마그네슘, 염화마그네슘, 초산마그네슘 등이 예시된다. 또한, 마그네슘 공급원은 수화물 형태라도 좋다. 마그네슘의 제2실 수액에의 배합 비율은 제2실 수액에 있어서 1∼20 mEq/L, 바람직하게는 5∼15 mEq/L가 예시된다. 또한, 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 마그네슘 농도는 0.5∼10 mEq/L, 바람직하게는 2∼6 mEq/L가 되는 범위를 만족하도록 설정되어 있는 것이 바람직하다.
인 공급원으로서는, 무기염을 이용하면 인산칼슘이나 인산마그네슘의 침전을 일으키는 경우가 있기 때문에, 예컨대 글리세로인산나트륨, 글리세로인산칼륨 등의 유기염을 이용하는 것이 바람직하다. 또한, 제1실에 유화제로서 레시틴을 사용할 때에는 그 레시틴도 인 공급원이 된다. 상기 레시틴 유래의 인만으로 필요량의 인을 공급할 수 있는 경우는, 인을 제2실에 배합할 필요는 없어, 인산칼슘 등의 침전을 일으키는 일이 없기 때문에 적합하다. 인의 제2실 수액에의 배합 비율은 제2실 수액에 있어서 0∼20 mmol/L가 예시된다. 또한, 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 인의 농도는 1∼20 mmol/L, 바람직하게는 5∼10 mmol/L가 되는 범위를 만족하도록 설정되어 있는 것이 바람직하다.
아연 공급원으로서는 황산아연, 염화아연 등이 예시된다. 또한, 아연 공급원은 수화물 형태라도 좋다. 아연의 제2실 수액에의 배합 비율은 제2실 수액에 있어서 2.5∼15 ㎛ol/L가 예시된다. 또한, 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 아연 농도는 1.5∼9 μmol/L가 되는 범위를 만족하도록 설정되어 있는 것이 바람직하다.
염소 공급원으로서는 염화나트륨, 염화칼륨, 염화마그네슘, 염화칼슘 등이 예시된다. 염소의 제2실 수액에의 배합 비율은 제2실 수액에 있어서 50∼100 mEq/L, 바람직하게는 40∼80 mEq/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 염소 농도는 25∼60 mEq/L, 바람직하게는 30∼40 mEq/L가 되는 범위를 만족하도록 설정되어 있는 것이 바람직하다.
제2실 수액은 필요에 따라서 pH 조정제에 의해 pH를 6.0∼7.4, 바람직하게는 6.5∼7.2로 조정되는 것이 바람직하다. pH 조정제로서는 제1실 수액에 관해서 기재한 것과 같은 것을 이용할 수 있다. 특히 시트르산은 인산칼슘의 석출을 억제할 수 있기 때문에 바람직하게 이용할 수 있다. 제2실 수액이 상기 pH 범위를 만족함으로써, L-시스테인, L-글루타민산 등의 화학 변화를 일으키기 쉬운 아미노산의 안정화를 도모하고, 나아가서는 제1실 수액과의 혼합 후의 혼합액의 pH를 후술하는 최적 범위로 유지시킬 수 있게 된다.
제2실 수액의 용매도 통상 주사용 증류수를 사용할 수 있다.
본 발명의 수액 제제에 있어서, 제2실 수액의 침투압비는 2.5∼3.5 정도로 한다.
또한, 필요에 따라서 본 발명의 수액 제제에 안정화제를 배합하더라도 좋다. 본 발명의 수액 제제에 배합되는 안정화제로서는 예컨대 아황산수소나트륨 등의 아황산염이 예시된다. 아황산염은 제1실 수액에 포함되는 비타민 B1의 분해를 피하기 위해서 제2실 수액에 배합된다. 제2실 수액에 있어서의 아황산염의 배합량은 예컨대 20∼50 mg/L의 범위가 예시된다.
본 발명의 수액 제제에는 비타민 B1 이외에도 다종의 비타민을 배합할 수 있다. 3실 또는 4실 수액 제제로 하지 않더라도, 2실 수액 제제에 있어서 다종의 비타민을 안정적으로 배합할 수 있다는 점도 본 발명 수액 제제의 특징의 하나이다. 비타민에는 수용성 비타민과 지용성 비타민이 있다. 본 발명의 수액 제제에서는, 지용성 비타민은 제1실 수액에 배합된다. 또한, 수용성 비타민은 제1실 수액, 제2실 수액 중 어디에 배합되더라도 좋다. 단, 상술한 것과 같이 비타민 B1은 제1실 수액에 배합된다.
본 발명의 수액 제제에 배합되는 수용성 비타민으로서는 비타민 B군, 비타민 C를 들 수 있다. 비타민 B군으로서는 비타민 B1(티아민) 외에, 비타민 B2(리보플라빈), 비타민 B3(나이아신), 비타민 B5(판토텐산), 비타민 B6, 비타민 B7(비오틴), 비타민 B9(엽산), 비타민 B12(시아노코발라민) 등을 들 수 있다. 또한, 지용성 비타민으로서는 비타민 A, 비타민 D(특히 콜레칼시페롤), 비타민 E, 비타민 K 등을 들 수 있다.
비타민 C(아스코르빈산)를 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있지만, 제2실 수액에 배합되는 것이 바람직하다. 비타민 C가 제2실 수액에 배합되는 경우, 제2실 수액에 있어서의 비타민 C의 배합 비율은 50∼500 mg/L, 바람직하게는 100∼400 mg/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 C의 농도는 통상 25∼250 mg/L, 바람직하게는 50∼200 mg/L, 보다 바람직하게는 40∼100 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
비타민 B2로서는, 리보플라빈, 리보플라빈인산에스테르나트륨, 플라빈모노뉴클레오티드 등을 사용할 수 있다. 비타민 B2를 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있다. 단, 비타민 B2와 엽산과의 반응에 의한 엽산의 불안정화를 피하기 위해서 이들은 분별 수용되는 것이 바람직하다. 예컨대, 제1실 수액에 엽산이 배합되는 경우에는 비타민 B2는 제2실 수액에 배합되는 것이 바람직하다. 비타민 B2가 제2실 수액에 배합되는 경우, 제2실 수액에 있어서의 비타민 B2의 배합 비율은 통상 리보플라빈으로서 2.5∼15 mg/L, 바람직하게는 4∼8 mg/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 B2의 농도는 통상 리보플라빈으로서 0.5∼10 mg/L, 바람직하게는 0.5∼3 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
비타민 B6으로서는 피리독신, 피리독신염산염 등의 피리독신의 염 등을 사용할 수 있다. 비타민 B6을 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있지만, 비타민 B6은 비타민 B2와 공존함으로써 빛에 대하여 매우 불안정하게 되기 때문에 비타민 B2와는 다른 쪽에 배합하는 것이 바람직하다. 비타민 B6을 제1실 수액에 배합하는 경우, 제1실 수액에 있어서의 비타민 B6의 배합 비율은 통상 피리독신으로서 2∼10 mg/L, 바람직하게는 2.5∼5 mg/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 B6의 농도는 통상 피리독신으로서 1∼10 mg/L, 바람직하게는 1.5∼3.5 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
엽산을 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있는데, 제1실 수액에 배합하는 것이 바람직하다. 엽산을 제1실 수액에 배합하는 경우, 제1실 수액에 있어서의 엽산의 배합 비율은 통상 0.1∼0.8 mg/L, 바람직하게는 0.2∼0.5 mg/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액 및 제2실 수액의 혼합액에 있어서의 엽산의 농도는 통상 0.1∼0.7 mg/L, 바람직하게는 0.2∼0.4 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
비타민 B12로서는 시아노코발라민, 초산히드록소코발라민, 메틸코발라민 등을 사용할 수 있다. 비타민 B12를 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있는데, 제1실 수액에 배합되는 것이 바람직하다. 비타민 B12가 제1실 수액에 배합되는 경우, 제1실 수액에 있어서의 비타민 B12의 배합 비율은 2∼10 ㎍/L, 바람직하게는 2.5∼5 ㎍/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 B12의 농도는 통상 0.5∼10 mg/L, 바람직하게는 0.5∼3 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
나이아신으로서는 예컨대 니코틴산아미드를 바람직하게 이용할 수 있다. 나이아신을 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있는데, 제2실 수액에 배합되는 것이 바람직하다. 나이아신이 제2실 수액에 배합되는 경우, 제2실 수액에 있어서의 나이아신의 배합 비율은 10∼100 mg/L, 바람직하게는 20∼50 mg/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 나이아신의 농도는 통상 5∼50 mg/L, 바람직하게는 5∼20 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
판토텐산으로서는 판테놀을 바람직하게 이용할 수 있다. 판토텐산을 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있는데, 제2실 수액에 배합되는 것이 바람직하다. 판토텐산이 제2실 수액에 배합되는 경우, 제2실 수액에 있어서의 판토텐산의 배합 비율은 판테놀로서 5∼30 mg/L, 바람직하게는 10∼20 mg/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 판테놀의 농도는 통상 2.5∼15 mg/L, 바람직하게는 5∼10 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
비오틴을 배합하는 경우, 제1실 수액 및 제2실 수액 중 어느 한쪽 또는 양쪽에 배합할 수 있는데, 제2실 수액에 배합되는 것이 바람직하다. 비오틴이 제2실 수액에 배합되는 경우, 제2실 수액에 있어서의 비오틴의 배합 비율은 10∼100 ㎍/L, 바람직하게는 20∼80 ㎍/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비오틴 농도는 통상 1∼50 ㎍/L, 바람직하게는 10∼40 ㎍/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
비타민 A로서는 레티놀팔미틴산에스테르를 바람직하게 이용할 수 있다. 또한, 이것을 오일에 용해시킨 비타민 A유를 사용할 수도 있다. 비타민 A는 지용성 비타민이며, 제1실 수액에 배합된다. 제1실 수액에 있어서의 비타민 A의 배합 비율은 1000∼5000 IU/L, 바람직하게는 2000∼4000 IU/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 A의 농도는 통상 500∼2500 IU/L, 바람직하게는 1000∼2000 IU/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다. 한편, IU는 인터내셔날 유닛(국제 단위)이다. 비타민 A 단위라고도 한다.
비타민 D로서는 콜레칼시페롤(비타민 D3)을 바람직하게 이용할 수 있다. 비타민 D는 지용성 비타민이며, 제1실 수액에 배합된다. 제1실 수액에 있어서의 비타민 D의 배합 비율은 2∼10 ㎍/L, 바람직하게는 2.5∼5 ㎍/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 D의 농도는 통상 0.5∼10 ㎍/L, 바람직하게는 0.5∼3 ㎍/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
비타민 E로서는 토코페롤초산에스테르를 바람직하게 이용할 수 있다. 비타민 E는 지용성 비타민이며, 제1실 수액에 배합된다. 제1실 수액에 있어서의 비타민 E의 배합 비율은 2∼50 mg/L, 바람직하게는 5∼20 mg/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 D의 농도는 통상 1∼25 mg/L, 바람직하게는 2.5∼10 mg/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
비타민 K로서는 피토나디온(비타민 K1)을 바람직하게 이용할 수 있다. 비타민 K는 지용성 비타민이며, 제1실 수액에 배합된다. 제1실 수액에 있어서의 비타민 K의 배합 비율은 50∼2500 ㎍/L, 바람직하게는 80∼2000 ㎍/L가 예시된다. 또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 혼합액에 있어서의 비타민 K의 농도는 통상 20∼1200 ㎍/L, 바람직하게는 30∼1000 ㎍/L의 범위를 만족하도록 설정되어 있는 것이 바람직하다.
한편, 제1실 수액 조성 및 제2실 수액 조성의 바람직한 일례로서 이하의 조성을 예시할 수 있다.

한편, 제1실 수액 및 제2실 수액 모두 공지된 수액 제조 방법에 준하여 제조할 수 있다. 예컨대, 상기 각 수액 성분을 주사용 증류수에 용해시켜 제조할 수 있다. 지용성 성분은 예컨대 상술한 것과 같이 유화시킨 후에 이용하는 것이 바람직하다.
제1실 수액과 제2실 수액의 혼합액
본 발명의 수액 제제는 용시에 제1실 수액과 제2실 수액을 혼합하여 사용된다. 제1실 수액과 제2실 수액의 혼합액은 혈관통이나 정맥염의 발생을 억제하여 안전성을 높이기 위해서, pH가 6∼7.4, 바람직하게는 6.5∼7.0, 적정 산도가 1∼10, 침투압비가 2∼3인 것이 바람직하다.
또한, 본 발명의 수액 제제에 있어서, 제1실 수액과 제2실 수액의 체적비는, 상술하는 제1실 수액과 제2실 수액의 액량 등에 따라서 적절히 설정되는데, 함유하는 각 성분의 안정성 및 각 실의 침투압 설정의 관점에서, 예컨대 (제1실 수액:제2실 수액)의 체적비가 (3:2∼3:5)가 되는 체적비를 들 수 있다.
또한, 상기 혼합액에 있어서의 열량은 바람직하게는 450∼750 kcal/L, 보다 바람직하게는 500∼650 kcal/L이다. 상기 열량에 있어서 지방이 차지하는 비율은 바람직하게는 40% 이하이며, 보다 바람직하게는 20∼40%이다. 또한, 상기 열량에 있어서 당, 지방 및 아미노산이 차지하는 비율은 당: 40∼60%, 지방: 20∼40%, 아미노산: 10∼30%인 것이 바람직하고, 당: 45∼55%, 지방: 25∼35%, 아미노산: 15∼25%인 것이 보다 바람직하다.
한편, 대략의 열량은 각 성분의 배합량(g)에 대하여 당은 4를, 지방은 9를, 아미노산은 4를 각각 곱해 얻을 수 있다. 즉, 당 1 g의 열량은 약 4 kcal이고, 지질 1 g의 열량은 약 9 kcal이고, 아미노산 1 g의 열량은 약 4 kcal이며, 이것을 기초로 열량을 구할 수 있다. 상기한 「상기 혼합액에 있어서의 열량」의 기재는 이 계산에 의해 산출한 값에 기초한다.
또한, 상기 혼합액에 있어서의 각 성분 조성의 바람직한 일례로서 이하의 조성을 예시할 수 있다.
수액 제제의 사용 양태
본 발명의 수액 제제는, 경구 섭취가 불충분하고 경도(輕度)의 저단백혈증 또는 경도의 저영양 상태에 있는 경우나 침습기(侵襲期)인 경우, 수술 전후의 환자의 영양 관리를 목적으로 하여 사용되며, 특히 수술 후나 소화기 질환 등에 의해 경구적으로 영양 보급이 곤란한 환자(바람직하게는, 소화기 절제 수술을 받은 환자)의 영양 관리를 목적으로 적합하게 사용된다. 본 발명의 수액 제제를 수술 후 1∼14일간, 바람직하게는 수술 후 1∼3일간 환자에게 투여함으로써, 환자의 영양 상태를 건전하게 유지시킬 수 있다. 투여량 및 투여 속도는 각 환자의 증상이나 연령 등을 고려한 뒤에 적절하게 설정할 수 있다. 특히, 본 발명의 수액 제제는 상기 투여 기간, 상기 수액 제제만으로 환자의 영양 상태를 건전하게 유지시킬 수 있다.
본 발명의 수액 제제는 바람직하게는 말초정맥으로부터 투여된다. 즉, 본 발명의 수액 제제는 바람직하게는 말초정맥 투여용 수액 제제이다. 통상, 말초정맥으로부터 수액을 투여할 때, 수액의 침투압이 너무 높으면 혈관통이나 정맥염을 야기할 우려가 있는 바, 본 발명의 수액 제제라면 이 우려가 없으므로, 말초정맥으로부터 투여하는 경우에 본 발명의 수액 제제의 상기 효과가 바람직하게 발휘된다.
수액 용기
제1실 수액과 제2실 수액을 수용하는 용기로서는, 연통 가능한 2실을 갖는 것이라면 특별히 한정되지 않지만, 예컨대 이박리 시일에 의해 격벽이 형성된 것(일본 특허 공개 평2-4671호 공보, 일본 실용 공개 평5-5138호 공보 등), 실 사이를 클립으로 끼움으로써 격벽이 형성된 것(일본 특허 공개 소63-309263호 공보 등), 격벽에 개봉 가능한 여러 가지 연통 수단을 설치한 것(일본 특허공고 소63-20550호 공보 등) 등과 같이 연통 가능한 격벽으로 이격된 2실 용기(수액 백)를 들 수 있다. 이들 중, 격벽이 이박리 시일(이지 필 시일)에 의해 형성된 수액 백이 대량 생산에 알맞고, 또한 연통 작업도 용이하기 때문에 바람직하다. 또한, 상기 용기의 재질로서는, 의료용 용기 등에 관용되고 있는 각종 가스 투과성 플라스틱, 예컨대 폴리에틸렌, 폴리프로필렌, 폴리염화비닐, 가교 에틸렌·초산비닐 공중합체, 에틸렌·α-올레핀 공중합체, 이들 각 폴리머의 블렌드나 적층체 등의 유연성 플라스틱을 들 수 있다.
상기 용기에의 제1실 수액 및 제2실 수액의 충전, 수용은 통상의 방법에 따라서 행할 수 있으며, 예컨대 각 수액을 각 실에 불활성 가스 분위기 하에서 충전한 후, 뚜껑을 닫아, 가열 멸균하는 방법을 들 수 있다. 여기서, 가열 멸균은 고압 증기 멸균, 열수 샤워 멸균 등의 공지된 방법을 채용할 수 있으며, 필요에 따라서 이산화탄소, 질소 등의 불활성 가스 분위기 중에서 행할 수 있다.
더욱이, 상기 용기에 수용된 제1실 수액 및 제2실 수액은, 변질, 산화 등을 확실하게 방지하기 위해서, 용기를 탈산소제와 함께 산소 배리어성 외장 주머니로 포장하는 것이 바람직하다. 특히, 용기로서 격벽이 이박리 시일에 의해 형성된 수액 백을 채용하는 경우에는, 그 수액 백은, 외압에 의해 격벽이 연통되지 않도록 이박리 시일 부분에서 접어 포개진 상태, 예컨대 이박리 시일 부분에서 둘로 접힌 상태로 포장되어 있는 것이 바람직하다. 또한, 필요에 따라서 불활성 가스 충전 포장 등을 행할 수도 있다.
상기 포장에 알맞은 산소 배리어성 외장 주머니의 재질로서는 일반적으로 범용되고 있는 각종 재질의 필름, 시트 등을 사용할 수 있다. 그 구체예로서는, 예컨대 에틸렌·비닐알코올 공중합체, 폴리염화비닐리덴, 폴리아크릴로니트릴, 폴리비닐알코올, 폴리아미드, 폴리에스테르 등, 또는 이들의 적어도 1종을 포함하는 재질로 이루어지는 필름, 시트 등을 들 수 있다.
또한, 탈산소제로서는 공지된 각종의 것, 예컨대 수산화철, 산화철, 탄화철 등의 철 화합물을 유효 성분으로 하는 것이나, 저분자 페놀과 활성탄을 이용한 것을 사용할 수 있다. 그 대표적인 시판 제품의 상품명으로서는 「에이지레스」(미쓰비시가스가가쿠사 제조), 「모듀란」(니혼가야쿠사 제조), 「세큘」(니혼소다사 제조), 「타모츠」(오지가코사 제조), 「키피트」(도렌시사 제조) 등을 들 수 있다.

표 2에 나타내는 조성에 따라서, 정제 대두유, 정제 난황 레시틴 및 포도당을 물에 가하여 호모믹서에 의해 조유화한 후, 고압 유화기(만톤고울린)로 정유화(精乳化)하고, 또한 물을 가하여 전량을 250 ml로 조정했다. 또한, pH 조정제(L-히스티딘 및 염산)를 이용하여 pH를 약 6.0으로 조정했다. 이와 같이 하여 얻어진 제1실 수액의 침투압비는 3.0, 적정 산도는 0.5였다.
2. 제2실 수액의 조제
표 3에 나타내는 조성에 따라서, 각 아미노산, 전해질 및 안정화제(아황산수소나트륨)를 주사용 증류수에 용해하여, 아미노 전해질액을 조제했다. 또한, 상기 액의 pH를 빙초산으로 6.7로 조정하여, 전량 250 mL로 하여 제2실 수액을 조제했다. 이와 같이 하여 얻어진 제2실 수액의 침투압비는 3.0, 칼륨 농도는 40 mEq/L였다.
3. 충전·포장
상기에서 얻어진 제1실 수액 250 mL 및 제2실 수액 250 mL를, 각 실이 이박리 시일로 구획된 폴리에틸렌제 2실 용기의 각 실에 각각 충전하고, 각 실 공간부의 질소 치환을 하여, 밀봉한 후, 통상의 방법에 따라서 고압 증기 멸균을 했다. 그 후, 용기를 이박리 시일부에서 접어 포개, 탈산소제(상품명 「에이지레스」; 미쓰비시가스가가쿠사 제조)와 함께, 다층 배리어 필름(상품명 「보브론」; NSR사 제조)의 외장 주머니(산소 배리어성 외장 주머니)에 봉입하여, 수액 제제를 얻었다. 한편, 이 수액 제제의 제1실 수액과 제2실 수액을 혼합한 후의 혼합액(조성을 표 4에 나타냄)은 pH가 6.7, 적정 산도가 7, 칼륨 농도는 20 mEq/L였다. 또한, 혼합 후 3일간의 보존에 있어서 입자경 0.5 ㎛ 이상의 조대 입자의 체적은 USP(미국 약전)의 규격인 0.05% 이하로, 지방 입자는 안정적이었다. 입자경은 아큐사이저 780(PERTICLE SIZING SYSTEM사 제조)를 이용하여 측정했다.
실시예 2 수액 제제의 조제
1. 제1실 수액의 조제
표 2의 성분에 더하여, 티아민 염화물 염산염 0.96 mg을 더욱 배합한 액제 250 ml를 실시예 1과 같은 식으로 조제하여, 제1실 수액을 제작했다. 한편, 상기 제1실 수액의 pH는 약 6.0으로 조정했다. 또한, 상기 제1실 수액의 침투압비는 3, 적정 산도는 0.5였다.
2. 제2실 수액의 조제
실시예 1과 같은 식으로 하여 제2실 수액을 제작했다.
3. 충전·포장
상기에서 얻어진 제1실 수액 250 mL 및 제2실 수액 250 mL를 각 실이 이박리 시일로 구획된 폴리에틸렌제 2실 용기의 각 실에 각각 충전하고, 각 실 공간부의 질소 치환을 하여, 밀봉한 후, 통상의 방법에 따라서 고압 증기 멸균을 했다. 그 후, 용기를 이박리 시일부에서 접어 포개어, 탈산소제(상품명 「에이지레스」; 미쓰비시가스가가쿠사 제조)와 함께, 다층 배리어 필름(상품명 「보브론」; NSR사 제조)의 외장 주머니(산소 배리어성 외장 주머니)에 봉입하여, 수액 제제를 얻었다. 한편, 이 수액 제제의 제1실 수액과 제2실 수액을 혼합한 후의 혼합액(조성을 표 5에 나타냄)은 pH가 6.7, 적정 산도가 7, 칼륨 농도는 20 mEq/L였다. 또한, 혼합 후 3일간의 보존에 있어서 입자경 0.5 ㎛ 이상의 조대 입자의 체적은 0.05% 이하로, 지방 입자는 안정적이었다.
실시예 3 수액 제제의 조제
1. 제1실 수액의 조제
표 2의 성분에 더하여, 비타민 A유 2.75 mg(825 비타민 A 단위), 콜레칼시페롤 1.25 ㎍, 토코페롤초산에스테르 2.5 mg, 피토나디온 1 mg, 티아민 염화물 염산염 0.96 mg, 피리독신염산염 1.23 mg, 시아노코발라민 1.25 ㎍ 및 엽산 0.1 mg을 더욱 배합한 액제 250 ml를 실시예 1과 같은 식으로 조제하여, 제1실 수액을 제작했다. 한편, 비타민 A유, 콜레칼시페롤, 토코페롤초산에스테르, 피토나디온은 미리 정제 대두유에 용해시키고나서 이용했다. 상기 제1실 수액의 pH는 약 6.0으로 조정했다. 또한, 상기 제1실 수액의 침투압비는 3.0, 적정 산도는 0.5였다.
2. 제2실 수액의 조제
표 3의 성분에 더하여, 아스코르빈산 25 mg, 비오틴 15 ㎍, 니코틴산아미드 10 mg, 펜테놀 3.5 mg 및 리보플라빈인산에스테르나트륨 1.15 mg을 배합한 액제 250 ml를 실시예 1과 같은 식으로 조제하여, 제2실 수액을 제작했다. 침투압비는 3, 칼륨 농도는 40 mEq/L였다. 또한 pH는 6.7로 조정했다.
3. 충전·포장

한편, 상기 수액 제제(제1실 수액 및 제2실 수액은 미혼합)를 실온에서 6개월간 보존한 후, 제1실 수액 또는 제2실 수액에 포함되는 각 비타민 성분량을 HPLC를 이용하여 측정한 바, 다음 표 7에 나타내는 결과를 얻을 수 있었다. 상기 결과로부터, 상기 수액 제제라면, 각 비타민 성분은 6개월 후에도 안정적으로 존재하는 것을 확인할 수 있었다.
실시예 4 수액 제제의 조제
1. 제1실 수액의 조제
표 2의 성분에 더하여, 비타민 A유 2.75 mg(825 비타민 A 단위), 콜레칼시페롤 1.25 ㎍, 토코페롤초산에스테르 2.5 mg, 피토나디온 37.55 ㎍, 티아민 염화물 염산염 1.92 mg, 피리독신염산염 1.82 mg 및 시아노코발라민 1.25 ㎍, 엽산 0.15 mg을 배합한 액제 300 ml를 실시예 3과 같은 식으로 조제하여, 제1실 수액을 제작했다. 이 때, pH는 약 6.0으로 조정했다. 또한, 침투압비는 2.5, 적정 산도는 0.5였다.
2. 제2실 수액의 조제
표 3의 성분에 더하여, 비타민 C(아스코르빈산) 50 mg, 비오틴 15 ㎍, 니코틴산아미드 10 mg, 판테놀 3.5 mg 및 비타민 B2(인산리보플라빈나트륨) 1.15 mg을 배합한 액제 250 ml를 실시예 3과 같은 식으로 조제하여, 제2실 수액을 제작했다. 이 때, pH는 약 6.7로 조정했다. 침투압비는 3.0, 칼륨 농도는 40 mEq/L였다.
3. 충전·포장
상기에서 얻어진 제1실 수액 300 mL 및 제2실 수액 250 mL를 각 실이 이박리 시일로 구획된 폴리에틸렌제 2실 용기의 각 실에 각각 충전하고, 각 실 공간부의 질소 치환을 하여, 밀봉한 후, 통상의 방법에 따라서 고압 증기 멸균을 했다. 그 후, 용기를 이박리 시일부에서 접어 포개어, 탈산소제(상품명 「에이지레스」; 미쓰비시가스가가쿠사 제조)와 함께, 다층 배리어 필름(상품명 「보브론」; NSR사 제조)의 외장 주머니(산소 배리어성 외장 주머니)에 봉입하여, 수액 제제를 얻었다. 한편, 이 수액 제제의 제1실 수액과 제2실 수액을 혼합한 후의 혼합액(조성을 표 8에 나타냄)은 pH가 6.7, 적정 산도가 6, 칼륨 농도는 18.2 mEq/L였다. 또한, 혼합 후 3일간의 보존에 있어서 입자경 0.5 ㎛ 이상의 조대 입자의 체적은 0.05% 이하로, 지방 입자는 안정적이었다.
비교예 1 수액 제제의 조제
1. 제1실 수액의 조제
표 2에 나타내는 조성에 따라서, 정제 대두유, 정제 난황 레시틴 및 포도당을 물에 가하여 호모믹서에 의해 조유화한 후, 고압 유화기(만톤고울린)로 정유화하고, 물을 더욱 가하여 전량을 350 ml로 조정했다. 또한, pH 조정제(L-히스티딘 및 염산)를 이용하여 pH를 약 6.0으로 조정했다. 이와 같이 하여 얻어진 제1실 수액의 침투압비는 2.1, 적정 산도는 0.5였다.
2. 제2실 수액의 조제
표 3의 성분을 배합한 액제 150 ml를 실시예 1과 같은 식으로 조제하여, 제2실 수액을 제작했다. 이 때, pH는 약 6.7로 조정했다. 침투압비는 5, 칼륨 농도는 67 mEq/L였다.
3. 충전·포장
상기에서 얻어진 제1실 수액 350 mL 및 제2실 수액 150 mL를 각 실이 이박리 시일로 구획된 폴리에틸렌제 2실 용기의 각 실에 각각 충전하고, 각 실 공간부의 질소 치환을 하여, 밀봉한 후, 통상의 방법에 따라서 고압 증기 멸균을 했다. 그 후, 용기를 이박리 시일부에서 접어 포개어, 탈산소제(상품명 「에이지레스」; 미쓰비시가스가가쿠사 제조)와 함께, 다층 배리어 필름(상품명 「보브론」; NSR사 제조)의 외장 주머니(산소 배리어성 외장 주머니)에 봉입하여, 수액 제제를 얻었다. 또한, 이 수액 제제의 제1실 수액과 제2실 수액을 혼합한 후의 혼합액은 실시예 1과 완전히 동일한 조성이 되지만, 비교예 1의 처방에서는 제2실의 칼륨 농도가 67 mEq/L로 고농도가 되기 때문에, 격벽 미개통으로 사용된 경우에는, 고농도의 칼륨이 체내에 투여되게 되어 매우 위험하다. 또한, 제2실의 침투압비가 5로 매우 높아 정맥염 등의 악영향도 간과할 수 없다.
Claims (7)
- 연통 가능한 구획에 의해 이격된 2실을 갖는 수액 제제로서,
제1실에 당 및 지방 유제를 포함하는 제1실 수액이 수용되고,
제2실에 아미노산 및 전해질을 포함하는 제2실 수액이 수용되고,
상기 제1실 수액은 칼륨을 포함하지 않으며, 침투압비가 2.0∼3.0이고,
상기 제2실 수액은 칼륨 농도가 40 mEq/L 이하이며, 침투압비가 2.5∼3.5이고,
구획 연통시의 상기 제1실 수액 및 제2실 수액의 혼합액의 칼륨 농도가 16 mEq/L 이상인 수액 제제. - 제1항에 있어서, 제1실 수액의 pH가 4.5∼6.5, 제2실 수액의 pH가 6.0∼7.4인 수액 제제.
- 제1항 또는 제2항에 있어서, 제1실 수액과 제2실 수액의 체적비가 3:2∼3:5인 수액 제제.
- 제1항 또는 제2항에 있어서, 비타민 B1을 제1실에 더 함유하는 수액 제제.
- 제4항에 있어서, 비타민 A, 비타민 B6, 비타민 B12, 비타민 D, 비타민 E 및 비타민 K를 제1실 수액에, 비타민 C 및 비타민 B2를 제2실 수액에 더 함유하는 수액 제제.
- 제3항에 있어서, 비타민 B1을 제1실에 더 함유하는 수액 제제.
- 제6항에 있어서, 비타민 A, 비타민 B6, 비타민 B12, 비타민 D, 비타민 E 및 비타민 K를 제1실 수액에, 비타민 C 및 비타민 B2를 제2실 수액에 더 함유하는 수액 제제.
