Abstract
본 발명은 PPARγ 수용체의 활성화에 반응하는 질병, 예를 들어 타입 2 인슐린-내성 당뇨병의 치료를 위한 화학식 I의 스피로락신의 용도를 특징으로 한다. 본 발명은 또한 화학식 I의 스피로락신이 유효 주성분으로서 화학식 II의 모든 트랜스 레티노인산과 회합하여 작용하는, PPARγ 수용체의 활성화에 반응하는 질병, 예를 들어 급성 악성 혈액병의 치료를 위한 약학 조성물에 관한 것이다.

Description
본 발명은 PPARγ 수용체(과산화소체 증식인자 활성화된 수용체)의 활성화에 반응하는 질병, 예를 들어 타입 2 인슐린-내성 당뇨병의 치료에서 스피로락신의 용도에 관한 것이다.
본 발명은 또한 유효 주성분으로서 모든 트랜스 레티노인산과 회합된 스피로락신을 포함하는, PPARγ 수용체의 활성화에 반응하는 질병, 예를 들어 급성 악성 혈액병의 치료를 위한 약학 조성물에 관한 것이다.
PPARγ는 핵 수용체 상과의 일원이다. 상기는 레티노이드 X 수용체(RXR)와 이종 이량체를 형성하고 글루코스 및 지질 대사와 관련된 유전자들의 전사 조절인자로서 작용한다(Diabetes 47(4):507 1998년 4월 14일).
당뇨병은 유전 인자와 환경 인자의 상호작용으로부터 발생하는 증후군으로; 인슐린 분비 장애와 다른 대사 및 혈관 이상, 즉 상승된 혈당 농도, 비 특이적으로 가속화된 동맥경화증, 신경병, 및 신장과 망막의 퇴행으로 인한 모세혈관 기저 막의 비후를 특징으로 한다.
최신 분류에 따라 당뇨병을 크게 2 개의 범주로 나눈다:
1. 인슐린 의존성 당뇨병(또한 타입 1 당뇨병으로서도 알려짐)으로, 상기 유형의 당뇨병 환자는 문자 그대로 인슐린에 의존하여 케토산증과 사망을 예방한다. 내생적인 인슐린 분비에 관한 한, 타입 1 당뇨병 환자는 인슐린 감소증을 나타낸다.
2. 인슐린 비 의존성 당뇨병(또한 타입 2 당뇨병으로서도 알려짐)으로, 상기 유형의 당뇨병 환자는 생존에 인슐린을 필요로 하지 않으며; 이들 환자는 당뇨병 증상의 조절에 인슐린의 사용 여부를 결정할 수 있다. 내생적인 인슐린 분비에 관한 한, 타입 2 당뇨병 환자를 추가로 2 개의 그룹으로 분류할 수 있다. 첫 번째 그룹에서 인슐린 수준은 정상이거나 정상 이하이며; 두 번째 그룹에서는 인슐린 수치가 정상보다 높고 환자가 인슐린 내성을 나타낸다.

인슐린 감작성 약물 치료에서 PPARγ의 리간드(당뇨병 치료에 사용된다)는 이미 공지되어 있다.
예를 들어, 티아조-리딘디온 유도체가 타입 2 인슐린 내성 당뇨병 환자의 치료에 유용한 약제로서 개시되어 있다. 상기 화합물은 PPARγ에 대해 친화성이 높은 리간드이며; 이의 생체 내 당뇨 억제 작용은 상기 수용체와의 높은 결합 친화성 에 기인한다(Nippon Rinsho 2000년 2월; 58(2):401-4).
유사하게, PPARγ는 여러 백혈병 세포 주(그의 분화 불능으로 인해 가장 미숙한 수준의 축적이 초래된다)에서 높은 수준으로 발현된다(Jan, Exp. 4, 281-99, 1995).
급성 악성 혈액병은 혈액암으로, 선진국 국민들 사이에서 점진적으로 꾸준히 증가하고 있다.
점점 더 많은 오염 화합물들이 공기 중에 존재하여 인간 유전자 풀에 돌연변이를 일으킨다. 이러한 변이는 종종 충실성 암과 악성 혈액병 모두의 원인이기도 하다.
상기 언급한 바와 같이, 급성 악성 혈액병은 림프양 또는 골수양 세포의 분화 불능을 특징으로 하며, 이는 결과적으로 가장 미숙한 수준의 축적을 초래한다.
이러한 종양 세포를 제거하거나 또는 그의 종말 분화를 유도할 수 있는 약물 치료가 상기 질병을 치료하는데 흔히 사용된다(분화 요법).
증식 억제 활성을 갖는 PPARγ의 리간드가 문헌[the European Journal of Cell Biology 67, 379-85, 1995년 8월; 및 European Journal of Cell Biology 77, 214-19, 1998년 11월]에 개시되어 있으며, 상기는 망막과 회합 시 상이한 골수양 백혈병 세포 주에 대해 강한 전(pro)-분화 상승작용을 나타낸다.
실제로, RXT 레티노이드 수용체와 이종 이량체를 형성함으로써 상기 PPARγ는 활성화된 수용체 복합체의 활성 증가와 상기 두 수용체 모두의 동시 활성화를 결정한다(Cell Vol. 93, 241-52, 1998년 4월).
레티노인산과 PPARγ의 리간드에 의한 복합 요법은 세포 분화의 결여를 특징으로 하는 질병, 예를 들어 급성 악성 혈액병의 치료에 대해 치료 이점을 제공한다.
스피로락신은 공지 화합물이며; 문헌[Phytochemistry, 1990, Vol. 29, No. 2, pages 613-616]에 진균 스포로트리쿰 락슘의 대사산물로서 개시되어 있다. 상기 스피로락신의 항종양 활성은 일본 특허 출원 제 JP 08177033 호에 보고되어 있다. 상기 출원에 개시된 실험 모델들은 종양 주의 증식 억제에 대한 생체 외 시험을 참조한다. 상기 시험들은 상기 증식이 시험된 종양 주에 대한 직접적인 세포독성에 의해 현저하게 억제됨을 보인다.
WO 9605204에서, 스피로락신은 헬리코박터 파이로리에 의해 야기된 위십이지장 질환의 치료에 유용한 화합물로서 개시되어 있다.
일본 특허 출원 제 JP 94-82785 호에서, 스피로락신은 콜레스테롤혈증 억제 활성을 갖는 지질 강하 화합물로서 개시되어 있다.
상기 스피로락신의 제조 과정은 문헌[Phytochemistry, 1990, Vol. 29, No 2, pages 613-616]에 개시되어 있다.
레티노인산 역시 공지 화합물이다. 상기 화합물의 독성 및 기형발생 프로파일이 문헌[J.J. Kamm, J. Am. Acad. Dermatol. 6, 652, 1982]에 발표되었다. 상기 화합물의 합성은 문헌[C.D. Robertson et al., J. Am. Chem. Soc. 77, 4111, 1995]에 개시되었다.
상기 스피로락신은 단독으로 또는 모든 트랜스 레티노인산과 함께 PPARγ 수 용체의 활성화에 반응하는 질병들의 치료에 유용한 약제로서 결코 개시되지 않았다.
레티노인산은 종양 세포 클론의 전골수세포의 분화를 자극하는 그의 능력(분화 요법) 덕분에 특정한 유형의 악성 혈액병인 급성 전골수구성 백혈병(APL)의 치료에 유용한 약제이다.
다른 유형의 백혈병에 비해, 상기 APL은 덜 현저한 백혈구 증가증, 빈혈 및 혈소판 감소증뿐만 아니라, 통상적인 화학 요법에 의해 치료 시 보다 작은 완화 퍼센트 및 보다 높은 사망률을 보인다.
상기 APL은 변칙적인 전좌를 특징으로 하는데, 이는 레티노인산 수용체 알파의 유전자를 포함하여 염색체 15 번 및 17 번의 긴 팔[전좌 t(15;17)]을 포함한다(Cin. Lab. Sci. 2000년 봄; 13(2):106-16).
ATRA의 경구 투여는 대부분의 APL 환자에서 완전한 완화를 유도한다. 그러나, 일부 경우에 ATRA에 의한 치료는 소위 "레티노인산 증후군"을 야기시킬 수 있다. 상기 증후군은 처리된 환자에서 백혈구 수의 급속하고 점진적인 증가를 특징으로 하며 다른 화학요법에 의해 치료된다.
더욱 또한, ATRA에 의한 치료 도중 종양 세포는 상기 화합물에 대해 점진적으로 내성이 되기 때문에, 후-완화 요법이 필요하다.
최근 수년간 수행된 노력들에도 불구하고, PPARγ 수용체의 활성화에 반응하는 질병들의 치료에 유용할 수 있는, 단독적이거나 회합된 새로운 화합물들이 여전히 크게 요구된다.
하기 화학식 I의 스피로락신이 PPARγ 수용체의 활성화에 반응하는 질병의 치료를 위한 약제의 제조에 유용한 약제임이 밝혀졌다:
본 발명의 하나의 목적은 PPARγ 수용체의 활성화에 반응하는 질병을 치료하기 위한 약제의 제조를 위한 화학식 I의 스피로락신의 용도로, 이때 상기와 같은 수용체의 활성화에 반응하는 질병은 타입 2 인슐린 내성 당뇨병이다.
본 발명의 추가의 목적은 유효 주성분으로서 하기 화학식 I의 스피로락신을 하기 화학식 II의 모든 트랜스 레티노인산과 함께 포함하는, PPARγ 수용체의 활성화에 반응하는 질병의 치료를 위한 약학 조성물이다:
화학식 I
본 발명의 추가의 목적은 화학식 I의 스피로락신과 화학식 II의 모든 트랜스 레티노인산과의 회합이다.
본 발명의 추가의 목적은 유효 주성분으로서 화학식 I의 스피로락신을 화학식 II의 모든 트랜스 레티노인산 및 하나 이상의 부형제 및/또는 비히클과 함께 포함하는 약학 조성물이다.
본 발명의 추가의 목적은 PPARγ 수용체의 활성화에 반응하는 질병을 치료하기 위한 약제의 제조를 위한 화학식 II의 모든 트랜스 레티노인산과 회합된 화학식 I의 스피로락신의 용도로, 이때 상기 PPARγ의 활성화에 반응하는 질병은 림프양 백혈병, 골수양 백혈병, 단구성 백혈병 및 급성 거대 모구성 백혈병으로 이루어진 그룹 중에 포함된 급성 악성 혈액병이다.
본 발명의 추가의 목적은 급성 전골수구성 백혈병을 치료하기 위한 약제의 제조를 위한 화학식 II의 모든 트랜스 레티노인산과 회합된 화학식 I의 스피로락신의 용도이다.
더욱 또한, 보다 많은 항종양 약제들을 동시에 또는 순차적으로 투여하는 치료 프로토콜의 사용이 약물 분야에 공지되어 있다.
치료 프로토콜 내에서 보다 많은 항종양 약제를 투여하는 불가피성은 상기 약제들이 상이한 대사 수준에서 작용함으로써 일부의 경우 암의 완전한 완화에 기여할 수 있는 반면, 다른 경우에는 치료된 환자들을 도와 보다 오래 살게하고/하거나 또는 그들의 삶의 질을 개선시킬 수 있다는 사실에 의해 정당화된다. 본 발명에 따른 회합은 급성 악성 혈액병의 치료를 위해 하나 이상의 공지된 항종양 약제들과 함께 사용될 수 있다.
따라서, 본 발명의 추가의 목적은 또한 하나 이상의 공지된 항종양 약제들이 겸비된, 화학식 II의 모든 트랜스 레티노인산과 회합된 화학식 I의 스피로락신을 포함하는 급성 악성 혈액병의 치료를 위한 약학 조성물이다. 상기 언급한 공지된 항종양 약제에는 알킬화제; 국소이성화효소 억제제; 튜불린 억제 약물; 삽입물(intercalant); 대사길항물질; 천연 산물, 예를 들어 빈카 알칼로이드, 에피포도필로톡신, 항생제, 효소 및 탁산을 포함하는 그룹이 포함된다.
본 발명을 예시하기 위해서 실험 데이터를 하기에 보고한다.
실시예 1
스피로락신(ST 1397)에 의한 PPARγ의 활성화
PPARγ 수용체에 결합하고 PPAR-γ(PPAR-γ RE) 반응 서열을 갖는 유전자의 활성화를 결정하는 스피로락신의 능력은 PPAR-γ를 발현하는 플라스미드와 PPAR-γ RE 통제 하에 있는 루시페라제 유전자를 암호화하는 정보제공 벡터를 사용하는 몇몇 세포 형질감염 실험을 통해 입증되었다(Cell 68; 879-887; 1992; J. Biol. Chem. 272; 25252-25259; 1997).
상기 루시페라제 발현의 활성화는 NH-3T3 쥐 섬유아세포를 PPAR-γ 플라스미드와 TK-PPAR-Reluc 정보제공 벡터로 형질감염시킴으로써 입증되었으며; 상기 루시페라제 활성은 40 nM 농도의 스피로락신으로 24 시간 처리한 후에 측정되었다.
상기 스피로락신의 활성을 5 μM의 농도로 시험된, 타입 2 인슐린 내성 당뇨병의 치료에 사용되는 공지 화합물, 즉 트로글리타존(TZD)의 활성과 비교하였다.
도 1에 예시된 결과는 스피로락신이 상기 언급한 당뇨병 억제 화합물보다 더 활성이 있음을 나타낸다. 실제로, 비교 화합물에 의해 유도되고 TK-PPAR-Reluc 정보제공 벡터에 의해 매개된 루시페라제 활성(상기 수용체의 활성화 지수로서)은 대조군보다 5 배 이상 높은 반면, 본 발명에 따른 스피로락신에 의해 유도된 루시페라제 활성은 대조군보다 7 배 이상 높았다.
실시예 2
인간 전골수구성 백혈병(NB4) 세포 주의 분화에 대한 본 발명에 따른 회합 효과
스피로락신(ST 1397) 및 ATRA(단독으로 및 회합하여)의 전 분화 활성을 본 실험 모델에서 평가하였다.
모든 트랜스 레티노인산은 0.1 내지 1 μM 범위의 농도에서 활성으로 되고; 분화 절정 효과는 통상적으로, 성장이 현저하게 중단된 경우 치료 3 일/4 일 이내에 관찰되는 것으로 널리 알려져 있다.
NB4 세포를 10% 송아지 태아 혈청(FCS)이 있는 RPMI 1640 배양액 5 ㎖ 중의 약 100.000 세포/㎖의 농도로 25 ㎠ 플라스크에서 증식시켰다. 하루 후에, 상기 세포들을 10-7 M 농도의 ATRA, 또는 0.1, 0.5 및 1 μM 용량의 ST 1397, 또는 동 부피의 상기 2 개의 화합물로 함께 처리하였다. 이어서, 상기 세포들을 배양 배지의 교체 없이 배양기에 2 내지 3 일간 두었다.
처리 2 일 또는 3 일 후의 끝에서, 상기 세포들의 과립성 백혈구로의 분화를 NBT 염색 감소 및 샘플의 분광 광도 측정 분석에 의해 측정하였다.
레티노인산을 DMSO 1 mM의 용액으로 배양 배지에 용해시켰다. 대조용 배양액을, 상기 화합물(DMSO)이 특정 실험 조건 하에서 분화될 수 있으므로 동 부피의 DMSO로 처리하였다.
분화 효과를 측정하기 위해서, 500.000 개의 세포를 각 샘플로부터 모아 원심분리시키고 10% 혈청, 1 ㎎/㎖의 니트로블루 테트라졸륨(NBT) 및 100 ng의 PMA가 있는 1 ㎖의 RPMI 1640 배지에 재 현탁시켰다. 상기 재 현탁시킨 세포들을 37 ℃에서 20 내지 60 분간 배양시켰다. 배양의 끝에서, 세포들을 원심분리시키고 상기와 같이 수득된 펠릿을 10% 트리톤 X 100을 함유하는 1 ㎖의 PBS에 재 현탁시켰 다.
상기 샘플들을 초음파 처리하여 완전히 용해시키고 이어서 540 ㎚의 파장에서 분광 광도 측정기로 판독하였다.
도 2에 예시된 결과는 ST 1397이 단독으로 투여될 때 NB4 세포에서 분화를 유도하지 않음을 나타낸다. ATRA 분화 효과는 이미 널리 공지되어 있으나, 상기 효과는 상기 언급한 바와 같이, 단독으로 사용될 때 불활성인 ST 1397의 동시 투여에 의해 향상되는 것으로 밝혀졌다.
Claims (10)
- 제 1 항에 있어서, 상기 질병은 타입 2 인슐린 내성 당뇨병인 것을 특징으로 하는 약제학적 조성물.
- 제 3 항에 있어서, 적어도 하나 이상의 부형제 또는 비히클을 추가로 포함하는 것을 특징으로 하는 약제학적 조성물.
- 삭제
- 제 3 항 또는 제 4 항에 있어서, 상기 질병은 급성 악성 혈액병인 것을 특징으로 하는 약제학적 조성물.
- 제 6 항에 있어서, 상기 급성 악성 혈액병은 림프양 백혈병, 골수양 백혈병, 단구성 백혈병 및 급성 거대 모구성 백혈병으로 이루어진 그룹 중에서 선택되는 것을 특징으로 하는 약제학적 조성물.
- 제 6 항에 있어서, 상기 급성 악성 혈액병은 급성 전골수구성 백혈병인 것을 특징으로 하는 약제학적 조성물.
- 제 3 항에 있어서, 하나 이상의 공지된 항종양 약제가 추가로 포함되는 것을 특징으로 하는 약제학적 조성물.
- 제 9 항에 있어서, 상기 항종양 약제는 알킬화제; 국소이성화효소 억제제; 튜불린 억제 약물; 삽입물(intercalant); 대사길항물질; 및 천연 산물로 이루어진 그룹 중에서 선택되는 것을 특징으로 하는 약제학적 조성물.
