플루피르틴은 운동계 질환에 의해 유발되는 급성 통증기의 치료에도 사용된다.
게다가, 플루피르틴은 퇴행성 또는 류마티스성 질환을 가지는 환자에게 성공적으로 사용된다.
우수한 진통제로서 뿐만 아니라 근육 이완제로서의 특성도 가지므로, 플루피르틴은 근육 경직 또는 이에 의해 유발되는 질환의 치료에도 사용할 수 있다[DE 40 22 442).
래트에서의 플루피르틴의 근육 이완 작용 연구에서, 흥분성 아미노산 N-메틸-D-아스파르테이트(NMDA)에 의해 플루피르틴의 작용이 방해받는다는 것을 추가로 밝혀냈다. 이러한 NMDA의 길항 작용으로 인해, 플루피르틴은, 예를 들면, 뇌 허혈, 신경 퇴행성 질환 및 간질 발작 같은, NMDA가 매개하는 중추 신경계(CNS) 질환의 치료에도 적합하다[DE 43 27 516].
화학 용어로는, 플루피르틴은 2-아미노-3-에톡시카보닐아미노-6-(p-플루오로벤질아미노)피리딘이다. 플루피르틴 및 이의 약학적으로 사용 가능한 염의 합성은 특허 문서[독일 특허 DE 17 95 858, DE 31 33 519 및 DE 34 16 609]에 설명되어 있다.
놀랍게도, 플루피르틴이 림프구 같은 조혈 세포계에서 어팝토시스성 세포 사멸(apoptotic cell death)을 억제할 수 있다는 사실이 본 발명에 이르러 밝혀졌다.
이러한 사실을 통해, 조혈 세포계의 손상과 관련된 질병 치료에 플루피르틴을 사용할 수 있다는 전망을 가지게 된다.
이러한 측면에 있어서, HIV 감염 환자/AIDS 환자의 치료에 특히 중요하다.
본 발명에 따른 새로운 작용을 하기의 약리 연구에서 설명한다.
약리 연구
재료
플루피리틴 말레산염 [2-아미노-3-에톡시카보닐-아미노-6-(p-플루오로벤질아미노)피리딘 말레산염], 및 HIV-1-gp120의 제조를 위해 HIV-1 바이러스 균주 HTLVIIIB가 사용된다[Popovic et al., 1984]. 수득되는 분리된 gp120 제제의 순도는, 폴리아크릴아미드 겔 전기영동에서 입증되는 바와 같이, 95% 초과이다[Muller et al., 1988].
항-레트로바이러스 분석
HIV-1 분석을 위해 사용되는 방법론은 이미 설명되어 있다[Sarin et al., 1987]. 사람의 T-림프아세포주 CEM(접종 농도: 1×105 세포/㎖)의 세포를 배지에서 현탁시키고, HIV-1 바이러스(균주 HTLVIIIB)로 감염시킨다. 이를 위하여, 50% 세포 배양 감염 투여량(CCID50)의 50배를 선택한다. CCID50은 세포 현탁액 1㎖ 당 세포의 50%가 감염되는 감염 투여량이다. 1㎖의 배양액을 5일 동안 항온처리시킨다. 감염시키기 2시간 전에 플리피르틴을 각기 다른 농도로 세포내로 부가한다. 살아 있는 세포의 수는 3-[4,5-디메틸티아졸-2-일]-2,5-디페닐테트라졸륨 브로마이드[MTT] 비색 분석으로 측정한다[Scudiero et al., 1988]. 이미 간행된 문헌[Muller et al., 1991]에서와 같이, ELISA 판독기를 사용하여 평가된다. 항온처리 후, 감염되지 않은 CEM 세포는 2.16회, 감염된 CEM 세포는 0.13회 세포 분열 주기를 수행한다.
혈액 샘플
(i) HIV-1에 감염된 동성애 남성 12명[평균 연령: 35세; 연령 범위: 22 내지 43세] 및 (ii) 동일하게 감염 위험에 노출되었으나, 건강하며 HIV-1 혈청 음성 반응을 보이는 거의 동일 연령 그룹의 남성 8명[평균 연령: 30세; 연령 범위: 23 내지 35세]의 말초 혈액 샘플을 추출한다. 질병 통제 센터(CDC, 1992)의 분류에 따라 환자들을 분류한다. HIV-1 혈청 양성 반응을 보이는 환자에 대한 연구는 무증후(asymptomatic) 바이러스 보균자의 혈액 샘플을 사용하여 수행한다(CDC: A1, A2, 평균 CD4 세포 수 407/㎕, 범위: 209 내지 698/㎕). 어떤 환자도 종양 또는 증후성 감염 증상을 보이지 않았거나, 혈액 샘플 채취 전 10주 동안, 예를 들면, 코르티코스테로이드같은 것을 사용하여 면역 억제 치료를 받지 않았다.
세포 제제 및 이의 처리
단핵 세포(MNC)는 피콜-하이패크 밀도 구배(Ficoll-Hypaque density gradient)를 사용하는 원심 분리에 의해 헤파린 처리된 혈액으로부터 얻고, 이를 농축한다[Rossol et al., 1990]. 최종 용적 500㎕의 배양 지지에서, 0.5×106개의 세포를 배양한다; 사용되는 배지는 10mM 헤페스(HEPES) 완충액, 2.6% 중탄산나트륨, 100U/l 페니실린, 100㎍/㎖ 스트렙토마이신, 10% 태내 송아지 혈청(FCS) 및 1mM 글루타메이트로 보충된 MEM 배지다. 이곳에서, 세포는 분리 후 즉시 화학물질로 처리한다.
반응성 산소를 이용한 어팝토시스의 유도
하이포크산틴(HX) 및 크산틴 옥시다제(XOD) 시스템을 이용하여 반응성 산소 라디칼을 제조한다[Bruck et al., 1994].
MNC을 총 용적 500㎕의 MEM 배지내에서 24시간 동안 현탁시킨다(상기 설명한 바와 같음).
이러한 배지에 0 내지 80mU/㎖의 XOD 및 1mM의 HX를 부가한다. XOD의 농도가 90mU/㎖에서(Sigma, Munich, Germany), 림프구의 90% 초과가 어팝토시스로 사멸한다.
하기에서 설명할 연구에 있어서, 특별한 언급이 없는 한, XOD 10mU/㎖ 및 HX 1mM을 사용한다. 이러한 배양 조건하에서, 세포의 약 50%가 어팝토시스로 사멸한다. 이러한 수준의 어팝토시스를 유지하면, 특정 화합물이 어팝토시스성 세포 사멸의 억제제 및/또는 촉진제로서의 잠재성을 가지는 지의 여부를 시험하는 데 유리하다. 플루피르틴은 산소 라디칼 생성계보다 6시간 전에 세포에 부가한다; 1일이 지나면 배양을 종결한다.
연속 유식 세포 측정(continuous flow cytometry)에 의한 세포 분석
어팝토시스는, 문헌[Schmid et al., 1994]에 기재되어 있는 방법에 따라, 파장 488nm에서의 아르곤 레이저 여기를 사용하는 FACScan(Becton-Dickinson) 연속 유식 세포 측정기로 측정한다. 림프구는, 세포 크기 측정 수단으로써 전진 광 산란(FSC) 및 표면 구성 측정 수단으로써 직교 산란(SSC)을 병용하는 광학적 방법으로 분리 및 특징화시킬 수 있다. 어팝토시스성 MNC 세포를 염색하기 위해, PBS에 용해된 7-아미노악티노마이신 D[7-AAD](Sigma) 20㎍/㎖와 함께 4℃,어두운 곳에서 20분간 항온처리한다. 그리고 나서, 염색액에서 연속 유식 세포 측정기로 세포를 분석한다. 7-AAD에 의한 붉은 색 형광은 파장 650nm의 필터를 사용하여 검출한다.
세포막의 붕괴로 7-ADD가 어팝토시스성 세포로 침입하여, 삽입 과정에 의해 DNA를 염색한다. 데이터는 라이시스 II 프로그램(LYSIS II program)을 이용하여 분석한다.
평균 채널 수는 평균 형광 강도, 세포 크기 및 표면 구성으로 변환된다.
TdT 방법에 의한 원위치 형광 표지화(labelling) 및 표면 표지화
특정 세포의 아집단(subpopulation)이 어팝토시스 과정을 겪고 있는지 여부를 확인하기 위해, 그 세포를 말단 트랜스퍼라제 시스템(TdT)으로 처리한다. 문헌 [Gorczyca et al.(1993)]의 방법을 약간 변형하여 사용한다. 현탁액내의 세포를 PBS(Gibco)내에서 2회 세척한 후, 4℃에서 10분간 1% 농도의 파라-포름알데히드에서 고정한다(Riedel de Haen). 그리고 나서, 세포를 5% AB 혈청 및 0.5% 소 혈청 알부민(BSA)이 첨가된 PBS내에서 2회 세척한다. 그리고 나서, 세포를 펠렛화하고, 0.5nM 비오틴-16-dUTP 및 25 U TdT가 첨가된 2.5mM 염화 코발트 용액 50㎕에서 재현탁시킨다. 이것을 200mM 카코딜산칼륨, 25mM 트리스-HCl 및 BSA 0.25㎎/㎖로 완충시킨다; 37℃에서 30분간 제조업자(Boehringer Mannheim, Mannheim, FRG)의 사용법에 따라 항온처리한다. 그리고 나서, 세포는 PBS(5% AB 혈청 및 0.5% BSA가 첨가된)내에서 2회 세척하고, 5㎍/㎖의 형광 이소티오시안산염(FITC)이 표지된 아비딘(세포 분석 시약의 품질)을 함유하는 완충된 염색액 100㎕에서 재현탁시킨다. 표면 표지를 동시에 하기 위해, 세포를 20㎍/㎖의 피코에리스린(phycoerythrine)으로 표지된 모노클로날 항-CD3-항체로 부가적으로 처리한다(Becton-Dickinson).
Halliwell(1987)에 의해 설명된 바와 같이, 연속 유식 측정법에 의해 형광의 수를 센다.
연구 결과
플루피르틴을 사용한 HIV-1에 감염된 CEM 세포의 치료
CEM 세포를 2시간 동안 플루피르틴으로 미리 처리한 후, 감염되지 않은 채로두거나 또는 병행 실험에서 HIV-1으로 감염시킨다.
5일 후, 관련된 실험에서 세포수를 확인한다. 도 1에서, 감염 배양 또는 비감염 배양에 있어서, 플루피르틴을 처리한 결과 세포 농도에 유의한 변화가 없음을 보여 준다(p>0.1).
플루피르틴을 최종 농도를 0.1 내지 30㎍/㎖의 범위로 실험에 부가한다. 감염되지 않은 실험에서는 세포 농도가 446,880± 31,020 세포/㎖인데 반하여, HIV-1 감염된 실험에서는 108,420± 6,710 세포/㎖이었다.
사용된 시험관 내의 항온처리 조건하에서, 플루피르틴은 세포에 대한 독성이 없으며, HIV-1 감염에도 영향을 끼치지 않음을 이 결과로 알 수 있다.
하이포크산틴/크산틴 옥시다제 시스템의 조정
크산틴 옥시다제(XOD)의 농도는 어팝토시스성 세포의 비율이 약 50%가 되도록 선택한다.
어팝토시스성 세포의 경우, 형광 강도가 임의 채널 카운트(arbitrary channel count) 10 이상을 보인다. 0mU의 XOD에서, 세포들 중에서 6.6± 2.1%만이 임의 채널 카운트 10 이상의 컷-오프 라인(cut-off line)을 넘었으며 즉, 이것은 6.6%가 어팝토시스성이라는 것을 말한다. XOD의 농도가 증가할수록 어팝토시스성 세포의 백분율이 증가하며, 80mU의 XOD에서 어팝토시스성 세포가 93.2± 6.8%가 되었다. 10mU XOD에서, 약 50%(정확하게는, 54.6 ±6.1%)의 세포가 어팝토시스 과정을 겪었다.
XOD가 없는 경우, 세포는 하기와 같은 FSC/SSC 분배 패턴을 보였다:
세포 크기는 349± 27(임의 채널 카운트)이고, 표면 구성은 112± 15이다. XOD의 농도가 80mU에서, 세포 크기는 감소하여 256± 22가 되는데 반하여, 표면 구성은 증가하여 180± 29가 되었다. 이러한 변화는 세포의 형태 변화에 기초하는 독특한 어팝토시스를 나타낸다. XOD가 10mU에서는, 세포의 분배 패턴이 상술한 극한치 값의 중간값을 보였다. 특별한 언급이 없는 한, XOD 10mU를 사용한다.
사람 림프구에서 자발적 어팝토시스 비율에 미치는 플루피르틴의 영향
자발성 어팝토시스를 연구하기 위해, 건강한 사람과 HIV-1에 감염된 환자의 말초 혈액 MNC를 조사하였다. 혈액 샘플을 채취하고, 1일이 지난 후 세포를 분석한다. 도 2로부터 명백한 바와 같이, 비감염자로 구성된 대조군의 MNC 세포의 6.32± 0.82%가 어팝토시스를 나타내는 반면, HIV-1에 감염된 환자의 경우는 28.42± 7.84%의 세포가 어팝토시스를 나타낸다. 이 두 그룹의 MNC는, 농도 0.1 내지 30㎍/㎖ 범위의 플루피르틴으로 24시간 동안 처리한다. 도 2에서 요약된 바와 같이, 림프구에서 자발적 어팝토시스의 비율은 플루피르틴에 의해 유의하게 변하지 않았다(p>o.1). 이는 건강한 사람 및 HIV-1에 감염된 환자의 세포에 있어서 모두 사실이다.
플루피르틴으로 인한 단핵 세포에서의 유도성 어팝토시스의 감소
건강한 사람과 HIV-1에 감염된 환자의 MNC를 HX/XOD 시스템(산소 라디칼 형성)로 처리하여, 어팝토시스를 유도하였다. 이 유도 물질에 접촉시키기 6시간 전에, 각기 다른 농도의 플루피르틴을 세포 배양액에 부가한다. 도 3으로부터 명백한 바와 같이, 농도 0.1 내지 3.0㎍/㎖ 범위의 플루피르틴의 부가로 인해 어팝토시스성 세포 사멸이 유의하게 감소한다(p<0.001). 플루피르틴의 농도가 10㎍/㎖인 경우, 감염자의 림프구에 대한 분석에서만 그 보호 효과가 유의적이었다. 플루피르틴의 농도가 0.3 내지 3.0㎍/㎖인 경우, 이의 보호 효과가 가장 컸고, 유도성 어팝토시스의 억제도 가장 크게 나타났다. 건강한 사람의 림프구로 실험한 경우에는 유도성 어팝토시스가 약 40% 감소하였고, HIV-1에 감염된 환자의 경우에는 약 60% 감소하였다.
림프구에서의 어팝토시스는 DNA 단편화 방법을 사용하여 검출할 수도 있다. 세포를 HX/XOD 시스템과 함께 항온처리한 후, 세포에서 DNA를 추출하여 아가로스 겔에서 크기에 따라 분리한 후, 블롯(blot)으로 옮긴다.
DNA가 180bp의 단편들의 사다리형 패턴으로 분해된다. 이러한 단편화 패턴은, 어팝토시스 과정에서 DNA 파괴의 특징적인 패턴이다[Wyllie, 1980]. 반면에, 플루피르틴 1㎍/㎖으로 처리한 세포는 어떠한 DNA 단편화도 보이지 않았다.
자발성 및 유도성 어팝토시스를 겪고 있는 세포에 대한 플루피르틴의 영향을 보여 주는 대표적인 점 블롯(representative dot blot)도 이와 유사하게 얻을 수 있다.
이러한 일련의 실험들을 수행하기 위해, HX/20mU XOD 시스템을 사용하여 MNC에서 어팝토시스를 유도하였다. 플루피르틴이 없는 조건하에서, 세포의 64.2± 7.9%가 어팝토시스로 사멸하였다.
반면에, 이 세포들을 24시간 동안 플루피르틴으로 처리했을 때, 어팝토시스성 세포의 비율이 18.3± 5.8%로 급감하였다.
이러한 결과는, 플루피르틴이 예를 들면, AIDS 환자에 있어서 림프구의 유도성 어팝토시스에 의한 세포 사멸을 치료하는데에 유망한 약물이 될 수 있음을 명확히 보여준다.
문헌[DE-A-43 27 516]은 예를 들면, 뇌 허혈, 알츠하이머 병, 파킨슨 병, 및 AIDS 뇌병증 또는 크로츠펠트-야곱 병 같은 감염에 의해 유도되는 신경 퇴행성 질환에 있어서, 플루피르틴이 신경 보호 작용을 설명하고 있다.
진행성 치매와 함께 AIDS 뇌병증은 AIDS의 신경학적 합병증이다. 감염된 대식 세포(macrophage)로부터 퀴놀린산 같은 NMDA 효능성 신경 독성 물질이 HIV-1(gp120)에 의해 유도되어 방출(최종적으로는, 신경 세포가 사멸함)되는 것을 AIDS 뇌병증의 원인 중의 하나로 생각하고 있다.
AIDS 뇌병증 및 AIDS(후천성 면역 결핍증) 그 자체는 각각 서로 다른 기관계에 영향을 미치는 질병이다. 전자는, 이미 설명한 바와 같이, AIDS의 신경학적 합병증(신경 세포의 어팝토시스)인 데 반하여, 후자는 T 세포수의 급격한 감소를 수반하는 세포 면역계 질환(조혈 세포계 성분의 어팝토시스)에 기초한 것이다.
조혈 세포계 세포(예를 들면, 림프구)의 유도성 어팝토시스를 억제하는 본 발명에 따른 플루피르티느이 효과는 중추 신경계 이외에 NMDA 수용체가 존재한다는 것이 이제까지 알려진 적이 없기 때문에 아주 놀라운 사실이다. 따라서, 당해 기술 분야의 숙련자라도 신경 세포에 대한 플루피르틴의 보호 효과에 기초하여, 조혈 세포계의 세포에 대해서도 플루피르틴이 보호 효과를 가진다고 예측할 수는 없었다.
본 발명에 따른 플루피르틴의 효과에 기초하여, 하기 예와 같은 다른 질병들을 예방하거나 치료할 수 있다:
- 약품(예를 들면, 세포 성장 억제제 및 코르티코스테로이드), 독성 물질, 방사능 물질 또는 다른 질병에 의해 유도되는 호중구 감소증/림프구 감소증(neutropenia/lymphocytopenia),
- 약품 또는 감염(예를 들면, HIV)에 의해 유도되는 혈소판 감소증.
플리피르틴은 공지된 방식으로, 하기 형태로 예방 및 치료를 위해 투여할 수 있다:
정제, 제피 정제, 경질 젤라틴 캡슐, 연질 젤라틴 캡슐, 펠렛, 과립, 당의정, 좌약, 마이크로캡슐, 물 또는 오일 기재 현탄액, 오일 기재 용제, 근육내 투여를 위한 주사액, 정맥내 투여를 위한 주사액.
약물 제조에 적합한 염은 플루피르틴의 모든 생리학적으로 허용되는 염이다. 이러한 예들로는, 플루피르틴의 하이드로클로라이드, 말레이트, 설페이트 및 글루코네이트가 있다.
본 발명에 따르는 약물에서의 플루피르틴의 함량은 0.1 내지 3000㎎이며, 바람직하게는 10 내지 500㎎이다. 상술한 약물의 단일 투여량은 매일 1 내지 5회 투여할 수 있고, 바람직하게는 매일 1 내지 3회 투여하는 것이다.
항상 언급되는 투여량은 염기로서의 플루피르틴을 말한다. 플루피르틴의 염을 사용하는 경우에는 분자량을 고려하여 전환해야 한다.
본 발명에 따른 화합물은 통상의 표준 방법에 따라 약학적으로 가공된다. 예를 들면, 플루피르틴과 부형제 및/또는 보조제를 일반적으로, 20 내지 80℃에서, 바람직하게는 20 내지 50℃에서 교반 또는 균질화에 의해 완전히 혼합한다.
Claims (2)
- 활성 화합물인 플루피르틴 또는 이의 약제학적으로 사용 가능한 염을 포함하는, HIV-감염 환자/AIDS 환자의 조혈 세포계내 유도된 어팝토시스를 치료하기 위한 약제학적 조성물.
- 제1항에 있어서, 플루피르틴 또는 이의 염이 투여하기에 적합한 형태를 형성시키기 위한 약제학적으로 적합한 부형제, 보조제 또는 이들 모두와 혼합됨을 특징으로 하는 약제학적 조성물.
