항-죽종형성제로서 네비볼롤을 함유하는 조성물
[발명의 상세한 설명]
본 발명은 산화성 스트레스와 관련하여 나타나는 혈관계 및 신경계의 노화, 또는 퇴화성 질환을 앓고 있는 인간을 치료 또는 예방하기 위한 약제를 제조하기 위한 α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체의 용도에 관한 것이다.
α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체는 고혈압 치료 효능을 갖는 β-1 차단제로서 EP-0,145,067에 개시되어 있다. (RSSS) 및 (SRRR) α,α′-이미노비스(메틸렌)비스[6-플루오로-2-크로만메탄올]의 라세미 혼합물로 구성된 네비볼롤이 일반적으로 상기 명세서에서 개시되어 있다. 네비볼롤은 특히 EP-0,334,429에 개시되어 있으며, 여기에서 (SRRR)에난티오머는 효능있는 선택적 β-1 차단제인 것으로 나타났고, 반면에 (RSSS)에난티오머는 효능있는 β-1 차단제가 아닌 것으로 나타났으나, 아테놀롤, 프로판올롤, 프라조신, 히드랄라진 및 흥미롭게도, 그 자신의(SRRR)에난티오머와 같은 일련의 항고혈압제에 대한 상승 인자인 것으로 나타난다. 한편, 추후의 조사에 의해, 네비볼롤을 다른 β-1 차단제와 구별시키는 그의 수개의 유익한 혈행동태적 효과, 예로, 네비볼롤이 자발성 고혈합래트에서 혈압을 급격히 낮추고, 전체적인 실핏줄 저항을 감소시키며, 마취된 개에서 박동 부피를 증대시키는 효과가 또는 (RSSS)에난티오머에 크게 기인한다는 것이 밝혀졌다.
현재, 실험에 의하면 α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체가 시험관내 및 생체내에서 효능있는 항산화제 활성을 갖는 것으로 나타났다.
겉보기에는 이 유도체가 카브딜롤 및 그의 대사물이 시험관내 및 생체내에서 항산화제 성질을 나타낸 이후 β-1 차단제의 유일한 두 번째 그룹이다. 이 차이와 함께, α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체가 보다 효능있는 것으로 보인다. 이들의 항산화제 성질로 볼 때, α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체는 산화성 스트레스에 의해 일으켜지는 혈관계 및 신경계의 노화 및 퇴화성 질환의 치료에 치료적 유용성을 갖는다.
결과적으로, 본 발명은 하기 일반식(I)의 α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체, 약제학적으로 허용되는 산 부가염, 입체화학적 이성체 형태 및 상기 유도체, 염 및 입체이성체의 혼합물의 산화성 스트레스에 관련된 혈관 및 신경계의 노화 또는 퇴화성 질환을 앓고 있는 인간을 치료 또는 예방하기 위한 약제를 제조하기 위한 용도에 관한 것이다.
상기식에서, R1및 R3는 각각 독립적으로 플루오로, 하이드록시 또는 수소를 나타내고, R2및 R4는 각각 독립적으로 수소 또는 하이드록시를 나타내며, 각 별표는 스테레오지닉(stereogenic) 중심을 나타낸다.
본 발명은 또한 산화성 스트레스와 관련된 혈관 및 신경계의 노화 또는 퇴화성 질환을 앓고 있는 환자에게 상기 노화 및 퇴화성 질병의 경과 및/또는 효과를 개선하거나, 정지하거나, 지연시키거나 또는 완화시키는데에 효과적인 α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체의 일정량을 상기 환자에게 투여하여 상기 환자를 치료하는 방법에 관한 것이다.
R1및 R3이 하이드록시를 나타내고, R2및 R4가 수소를 나타내는 화합물이 R1및 R3가 플루오로를 나타내는 대응 화합물로부터 형성된 주요 대사물인 것으로 생각되었으나, 최근 이들은 그렇지 않은 것으로 나타났다. 그럼에도 불구하고 이들 유사-대사물들은 효능있는 항산화제 활성을 갖는다.
본 발명에 따른 특정 화합물에는 (RSSS) 및 (SRRR) α,α′-이미노비스(메틸렌)비스[6-플루오로-2-크로만메탄올], (일반적으로 네비볼롤이라 알려진 라세미 혼합물, 그의 개개의 에난티오머는 (RSSS) 및 (SRRR) α,α′-이미노비스(메틸렌)비스[6-하이드록시-2-크로만-메탄올이다];
[(RSSS)+(RSRR)+(SRSS)+(SRRR)]-α-[[[2-(크로만-2-일)-2-하이드록시에틸]아미노]-메틸]-6-하이드록시-2-크로만-메탄올에탄디오에이트(1:1);
[(SRSR)+(SRRS)+(RSSR)]-α,α′-이미노비스(메틸렌)비스[6-하이드록시-2-크로만-메탄올], 및
[(RRSR)+(RRRS)+(SSSR)+(SSRS)]-α-[[[2-(크로만-2-일)-2-하이드록시에틸]아미노]메틸]-6-하이드록시-2-크로만-메탄올에탄디오에이트(1:1)가 있다.
일반식(I)의 화합물은 EP-0,145,067 및 EP-0,334,429에 기재된 과정에 따라 제조할 수 있다. 이들 화합물은 염기 성질을 갖기 때문에 적절한 산으로 처리하여 그의 약제학적으로 허용되는 산 부가염으로 전환시킬 수 있다. 적적한 산은 예를 들어 할로겐수소산, 예로 염산 또는 브롬화수소산; 황산; 질산; 인산 등과같은 무기산; 또는 예로 아세트산 프로판산, 하이드록시-아세트산, 락트산, 피루브산, 옥살산, 말론산, 숙신산, 말레산, 푸마르산, 말산, 타르타르산, 시트르산, 메탄설폰산, 에탄설폰산, 벤젠설폰산, P-톨루엔설폰산, 시클람산, 살리실산, P-아미노살리실산, 팜산 등과 같은 유기산이다. 본 명세서에서 사용된 부가염은 또한 일반식(I)의 화합물 및 그의 염이 형성할 수 있는 용매화물이다. 이러한 용매화물은 예를들어 수화물, 알콜레이트 등이다. 네비볼롤의 바람직한 산 부가염은 염화수소염이다. 약제학적으로 허용되지 않는 염이 일반식(I)의 화합물 및 이러한 화합물을 함유하는 조성물의 제조에 유용할 수도 있다.
산화성 스트레스는 조직내의 강한 내인성 산화제의 작용에 관련된 현상, 특히 퇴화성 작용에 관한 것이다. 강한 내인성 산화제는 예를 들어 과산화물(O2 -)·, 과산화수소(H2O2) 및 하이드록실 라디칼(HO·)이다. 조직은 중앙, 외면 또는 골수일 수 있고, 특히 혈관계, 신경계, 신장, 췌장, 갑상선 및 생식선에 속한다. 조직 세포에 대한 산화적 손상은 세포막 일체성 및 기능에서의 변화에 이르는 지질 과산화에 기인한다. 산소 유도된 유리 라디칼에 의한 내피 손상은 현재 일반적으로 아테롬성 경화증 및 관련된 혈관 질병의 개시 및 진전에 주요 단계인 것으로 여겨진다.
치료는 일반식(I)의 화합물을 혈관계 및 신경계의 퇴화성 질병의 경과 및/또는 효과를 개선, 정지, 지연 또는 완화시키는데에 유효한 양으로 투여하는 것을 포함한다. 예방은 일반식(I)의 화합물을 혈관계 및 신경계의 노화, 또는 퇴화성 질병의 개시 및 진전을 방지 또는 지연시키는데에 유효한 양으로 투여하는 것을 포함한다.
α,α′-이미노비스(메틸렌)비스[2-크로만메탄올]유도체의 항산화제 활성은 유리 라디칼(하이드록실, 과산화물 및 일산화질소 라디칼)을 제거하여 라디칼-중재된 지질 과산화 및 세포독성을 방지하는 능력에 의해 시험관내에서 보여질 수 있다. 신경 세포의 배양물에서, 이들은 효과적으로 공지된 내인성 항산화제 비타민 E(α-토코페롤)를 대체할 수 있다. 또한, 이들 화합물은 과산화물(O2 -)·을 제거함으로써 과산화물 디스뮤타제(SOD)의 작용을 닮았다. 일반식(I) 화합물의 상이한 입체이성체들의 항산화제 작용은 비교할만하다. 일반식(I) 화합물의 항산화제 활성은 존재하는 하이드록시기의 수와 비례한다.
특히 흥미있는 발견은 R1-4의 적어도 하나가 하이드록실인 화합물은 시험관내 미세분자 범위에서 투여량의 영향을 받아 산화된 인간의 저밀도 지질단백질 (ox-LDL)의 형성을 억제한다는 것이다. 산화된-LDL은 죽종형성 및 산화된 막지질의 형성에 관련된다.

제2도는 제어환경 및 활성약제의 존재하에서 시험관내의 산화된 LDL의 형성을 보여주는 도면이다.
하이드록실화는 네비볼롤의 대사에서 중요한 효소반응이기 때문에 그의 하이드록시 유사체의 항산화 효과로 볼 때, 네비볼롤은 시험관내의 산화 손상에 대해 중요한 보호 효과를 가짐을 예견할 수 있다.
이는 생체내에서의 가장 예기치 않던 관찰, 즉, 네비볼롤로 만성적으로 처리된 시험 동물에서, 대조군 동물에서 보이는 바와 같은 혈관계의 정상적인 노화가 억압된다는 것, 즉, 처리된 동물에서의 노화 진전이 상당히 지연된다는 것에 의해 확인된다.
산화성 스트레스와 관련하여 나타나는 일반식(I) 화합물의 처리를 받을수 있는 혈관계 및 신경계의 질환 및 상태는 죽종 형성, 아테롬증(동맥 내막의 지방성 변성), 동맥경화증, 아테롬성 경화증, 고혈압으로 인한 혈관 비대, 고지단백혈증 및 노화를 통한 정상적 혈관의 퇴화와 같은 혈관계의 정상적 및 병리학적 퇴화; 생식선 및 췌장의 혈관 맥관병; 부갑상선 반응성 증식; 및 만성신질환; 종양 질병; 감염성 질환이다.
더욱이, 일반식(I)의 화합물은 또한 신경계로부터, 특히 말초 신경계로부터 산화성 손상 또는 상해와 결합된 신경원 세포 감소, 예로 혈전색전증뇌졸증, 뇌성 뇌졸중, 출혈성 뇌졸중, 뇌허혈, 뇌 혈관경련, 뇌노화, 뇌 또는 척추 외상, 심정지, 동맥 저혈압, 심장 또는 폐 수술, 심한 저혈당증, 산소 결핍증, 저산소증, 주산기 질환을 방지 또는 치료하는데에; 및 산소 대사 과정이 역할을 하는 신경 퇴행성 질환, 예로 헌팅톤무도 병, 알쯔하이머 병, 노인성 치매, 피크 병, 코르사코프 병, 올리브교소뇌 위축, 근위축성 측삭 경화증, 파킨슨 병, 다운 증후군, 글루타르산 혈증(glutaric acidaemia), 간질, 경련 상태, 다발 경색성 치매, 바이러스 감염 유도된 신경 변성, 예로 신경계-AIDS 감염성 치매, 인지 장애, HIV 감염이 관련된 신경 및 근질환, 광견병, 홍역 및 파상품을 경감시키는 데에 치료적 가치를 가질 수 있다.
죽종 형성은 동맥벽에 잔류하는 마크로파지중에 콜레스테롤의 축적에 의해 특징지워지는 복잡한 과정이다. 마크로파지는 정상적인-LDL-수용체와 대조적으로 세포 콜레스테롤 함량에 의해 조절되지 않는 스카빈저 수용체를 통해 산화된 LDL을 취한다. 조절 메카니즘의 결여는 세포에서의 콜레스테롤 축적 및 포움-세포(foam-cell) 형성을 가져온다. 산화된 LDL은 생체내에서 아테롬성경화 손상부에 근접한 지역에서 발견되었다(참조, Jackson, R.L. 등의 문헌 [(1993), Medicinal Research Reviews 13, 161-162]). LDL산화는 지질 과산화, 알데히드 형성, 단백질 단편화 및 LDL입자-결합된 비타민 E의 소모를 포함한다.
산화된 LDL에 대한 자가-항체가 인간에게서 동정되었다. 그의 적정량(titer)은 카로티드 아테롬성 경화증 진전의 독립적인 예견자이다(참조, Salonen, J.T. 등의 문헌 [(1992), The Lancet 339, No 8798, 883-887]). 또한, LDL산화에 대한 민감성은 관상 동맥의 아테롬성 경화증의 심각성과 관련된다(참조, Regnstrm, J. 등의 문헌 [(1992), The Lancet 339, No 8803, 1183-1186]).

하기의 실시예 1에서의 데이터는 네비볼롤의 하이드록시 유사체가 인간 LDL에 대해 광범위한 항산화제 효과를 가짐을 입증한다. 시험관내에서 사용된 농도는 높으나(2μM) 생체내 조건과 비료해서 관련이 없다. 정말로 비타민 E 및 공지된 항산화제 및 저지질혈증제인 프로부콜은 수주일에 걸친 시간경과에 걸쳐 LDL입자의 지질상에 혼입하는 것이 밝혀졌다(참조, Reaven, P.D. 등의 문헌 I (1992), Artheriosclerosis and Thrombosis, 12. No 3, 318-324; Kagan V.E. et al.(1992), J. Lip. Res., 33, 385-397]). 실시예 1에서의 실험의 짧은 기간은 상기 생체내 상황을 모사하지 않았다. LDL입자의 수성상과 지질상사이의 약제의 분배는 아마도 평형에 이르지 않아, 상기 지질-용해성 약제의 항산화 용량은 참고 화합물 비타민 E 에 대해서 요구되는 매우 높은 농도에 의해 나타나는 바와 같이 평가되지 않았다(제2도).
질적으로 네비볼롤 하이드록시 유사체의 효과와 비타민 E 의 효과도 유사하다. 상기 두물질은 모두 분석 시스템에 존재하는 항산화제의 소모후에만 개시하는 산화인 LDL의 산화를 지연시킨다.
본 발명에 따른 약제로서 적절한 일반식(I) 화합물의 약제학적 조성은 본 분야에 알려진 하나 이상의 부형제 또는 담체를 포함한다. 하나 이상의 상기 부형제 또는 담체를 적절히 선택하여 약제 조성물은 경구, 직장, 질, 국소, 비경구(근육내, 피하 및 정맥내) 또는 이식 투여에 적합하고, 흡입 또는 취입법에 의해 투여하기에 적절한 형태이다. 제제는 적절한 경우 분리된 투여 단위로 쉽게 존재할 수 있다.
이러한 조성물은 제조하는 방법은 본 분야에 잘 알려져 있고, 활성 성분 및 부형제와 서로 밀접히 혼합시킴을 특징으로 한다. 모든 방법은 활성 성분과 지질 담체 또는 미세하게 분리된 고형 담체 또는 둘모두와 결합시키고, 필요하면 생성물을 원하는 제제로 성형시키는 단계를 포함한다.
경구 투여를 위해 약제학적 조성물은 고형 투여 형태, 예로, 통상의 방법에 의해 약제학적으로 허용되는 부형제, 예로, 결합제(예, 예비젤라틴화된 전분, 폴리비닐 피롤리돈 또는 하이드록시프로필 메틸셀룰로즈); 충진제(예, 락토르, 미셀결정성 셀룰로즈 또는 인산 칼슘); 윤활제(예, 마그네슘 스테아레이트, 활석 또는 실리카); 붕해제(예, 감자 전분 또는 나트륨 전분 글리콜레이트); 또는 습윤제(예, 나트륨 라우릴 설페이트)와 함께 제조된 정제 또는 캡슐일 수 있다. 정제는 본 분야에 잘 알려진 방법에 의해 피복될 수 있다.

표피내의 국부적 투여의 경우, 본 발명의 화합물은 크림, 겔, 연고 또는 로션으로서 또는 경피성 패치로서 제형화될 수 있다. 이러한 조성물은 예를 들어 적절한 농후제, 겔링(gelling)제, 안정제, 분산제, 현탁화제 및/또는 착색제의 첨가와 함께 수성 또는 오일성 베이스와 함께 제형화될 수 있다.

일반식(I)의 화합물은 또한 예로 코코아 버터 또는 그밖의 글리세리드와 같은 통상의 좌약 베이스를 함유하는 좌약 또는 지연관장제와 같은 직장용 조성물로 제형화될 수 있다.
비내 투여의 경우, 일반식(I)의 화합물은 예를 들어, 액체 분무제로서, 산제로서 또는 드롭 형태로 사용될 수 있다.
흡입에 의한 투여 경우, 일반식(I)의 화합물은 적절한 추진제, 예로 디클로로디플루오로메탄, 트리클로로플루오로메탄, 디클로로테트라플루오로에탄, 1,1,1,2-테트라플루오로에탄, 이산화탄소 또는 다른 적절한 가스의 사용과 함께 가압된 팩 또는 분무기로부터의 에로졸 분무 제공 형태로 전달된다. 가압 에로졸 경우에, 투여 단위는 측정된 양을 전달하는 밸브를 제공하여 결정될 수 있다. 흡입기 또는 취입기에서 사용하기 위한 예로 젤라틴의 캡슐 및 카트리지는 본 발명의 화합물과 락토즈 또는 전분과 같은 적절한 분말 베이스의 분말 혼합물을 함유하게 제형화 될 수 있다. 상기 기술된 약제학적 조성물의 어느 것도 조절된 방출 형태와 관련된 통상의 방법으로 제공될 수 있다.
일반식(I) 화합물의 생물학적 이용성을 증가시키기 위하여 이들 화합물은 적절한 사이클로덱스트린과 유리하게 제형화될 수 있다. 적절한 사이클로덱스트린은 α-, β-, γ-사이클로덱스트린, 또는 에테르 또는 그의 혼합된 에테르이고, 여기에서 사이클로덱스트린의 무수글루코즈 유니트의 하이드록시기의 하나 이상이 C1-6알킬, 특히 메틸, 에틸 또는 이소프로필; 하이드록시 C1-6알킬, 특히 하이드록시에틸, 하이드록시프로필 또는 하이드록시부틸; 카복시 C1-6알킬, 특히 카복시메틸 또는 카복시에틸; C1-6알킬카보닐, 특히 아세틸; C1-6알킬옥시카보닐-C1-6알킬 또는 카복시-C1-6알킬옥시C1-6알킬, 특히 카복시메톡시프로필 또는 카복시에톡시프로필; C1-6알킬카보닐옥시C1-6알킬, 특히 2-아세틸옥시프로필로 치환된다.
특히 복합화제 및/또는 용해제로서 가치있는 것은 β-CD, 2,6-디메틸-β-CD, 2-하이드록시에틸-β-CD, 2-하이드록시에틸-γ-CD, 2-하이드록시프로필-γ-CD 및 (2-카복시메톡시)프로필-β-CD 및 특히 2-하이드록시프로필-β-CD(2-HP-β-CD)이다. 본 발명의 조성물에서 사용하기 위한 가장 바람직한 사이클로덱스트린은 0.35 내지 0.50 범위(질량 스펙트로스코피에 의해 결정됨)의 평균 몰 치환(M.S.)을 갖고, 1.5% 미만의 비치환된 β-사이클로덱스트린을 함유하는 2-하이드록시프로필-β-사이클로덱스트린이다. NMR 또는 IR에 의해 결정된 M.S. 값은 바람직하게는 0.55 내지 0.75 범위이다.
약제학적 조성물은 오직 일반식(I) 화합물 및 사이클로덱스트린 또는 사이클로덱스트린 유도체로 구성될 수 있다. 상기 고체 형태는 수용액을 동결건조시키거나, 공-침전하여 손쉽게 제조할 수 있다. 이러한 형태는 특히 물, 염수 또는 사이클로덱스트린의 수용액과 재구성하거나, 비약제학적 고형물, 특히 식품과 혼합하기에 유용하다.
바람직하게는 본 발명에 따른 약제학적 조성물은 경우 투여에 적절하다.
조성물은 유리하게는 분리된 투여량 단위, 특히 단위 투여 형태로 제공될 수 있다. 편리한 단위 투여 제제는 0.1 내지 100mg의 양으로 활성성분을 함유한다. 처리에서 1일 투여량으로 요구되는 일반식(I) 화합물의 양은 선택된 특별한 화합물에 따라, 그러나 투여경로, 환자의 처리하려는 조건의 성질 및 연령, 체중 및 상태에 따라 변하고, 최종적으로는 담당 의사에 의해 참작된다. 그러나, 일반적으로 적절한 투여량은 1일 약 0.1 내지 약 20mg 의 범위이다. 예방에서 사용하기 위한 적절한 1일 투여량은 일반적으로 같은 범위이다.
바람직한 투여량은 편리하게는 단일 투여량 또는 적절한 간격으로 투여되는 분리된 투여량, 예를들어 1일 2,3,4 또는 그 이상의 부투여량으로 제공될 수 있다. 네비볼롤의 1일 투여량은 단일 투여량(o.d)으로 투여될 수 있다. 그 이유는 이러한 섭생이 24시간의 기간에 걸쳐 효과적인 혈장 수준을 얻기 때문이다. 증가된 항산화 효율을 갖는 활성 대사물의 형성에 기인하여, 네비볼롤의 항죽상형성 효과는 안정된 상태가 도달할 때까지 반복 또는 만성적 투여시 증가할 것이다.
일반식(I) 화합물은 또한 그밖의 혈압 강하제, 또는 그밖의 치료제, 예를들어 저지방혈증제, 저지질혈증제, 콜레스테롤 강하제, ACAT 억제제 또는 항산화제와 혼합하여 사용될 수 있다. 본 발명은 따라서 추가의 면에서 또 다른 치료활성제, 특히 또 다른 혈압 강하제 또는 저지방혈증제, 저지질혈증제, 콜레스테롤 강하제, ACAT 억제제 또는 항산화제와 함께, 본 명세서에서 정의된 조성물을 포함하는 배합물을 제공한다. 상기 배합물은 별개로, 즉, 상기 기술된 어느 경로에 의해 동시에, 동반하여 또는 연속적으로 투여되거나, 또한 하나의 약제학적 제제의 형태로 제공될 수 있다. 따라서 산화성 스트레스와 관련된 혈관계 또는 신경계의 노화 또는 퇴화성 질환을 앓고 있는 인간의 치료 또는 예방조처에서 동시에, 별개로 또는 연속적으로 사용하기 위한 조합된 제제로서 앞서 정의된 바와 같은 (a) 일반식(I)의 화합물 및 (b) 앞서 정의된 바와 같은 또 다른 치료제를 포함하는 약제학적 생성물은 본 발명의 추가의 면이다. 이러한 생성물은 일반식(I) 화합물의 약제학적 조성물을 함유하는 용기 및 제2치료제의 약제학적 조성물을 포함하는 또 다른 용기를 포함하는 키트를 포함할 수 있다. 두 활성성분의 별개의 조성물을 갖는 생성물은 각 성분의 적절한 양, 및 투여의 시기 및 순서가 환자에 따라 선택될 수 있다는 이점을 갖는다.
상기 정의된 조합물에서 사용하기 위한 적절한 치료제는 예를 들어 혈압 강하제, 특히 예를 들어 아테놀롤, 셀리프롤롤, 프로판올롤과 같은 β-1 차단제; 저지방혈증, 즉 저밀도 지질 단백질(LDL) 또는 콜레스테롤을 갖추는데 유용한 약제, 예를 들어 HMG CoA 환원제 억제제, 예로 로바스타틴, 플루바스타틴, 프라바스타틴, 심바스타틴; 겜피브로질; 자라고즈산; 또는 항산화제, 예로 프로부콜, 비타민 E (α-토코페롤), 또는 상기 언급된 생리학적 성질의 수개를 조합한 화합물, 예로 카브딜롤, 베라파밀, 딜티아젬, 비타민 C(아스코브산 또는 그의 염) 등을 포함한다.
일반식(I)의 화합물이 제2 치료제와 조합하여 사용될 때, 각 화합물의 투여량은 화합물이 단독으로 사용했을 때로부터 변할 수 있다. 따라서, 일반식(I) 화합물이 제2치료제와 함께 사용될 때, 각 화합물의 투여량은 화합물이 단독으로 사용될 때 이용된 것과 같거나; 보다 흔하게는 그보다 낮을 수 있다. 적절한 투여량은 본 분야에 숙련된 자에 의해 쉽게 인식될 것이다.
[실시예 1]
[인간 LDL의 시험관내 tks화]
인간정맥 혈액을 EDTA(최종 농도 2.68mM)상에 수집하였다. 혈장을 원심분리에 의해 단리하였다. NaBr(37.3mg/ml)을 첨가하고 용해시켰다(최종 밀도 1.036g/ml의 NaBr용액으로 채운후 50 Ti로터(Beckmann)에서 4500rpm에서 18시간 원심분리했다. 원심분리후, 부유하는 지질 단백질을 버렸다. 나머지 용액을 1.055g/ml의 밀도로 조정했다. 원심분리 튜브의 채움 및 원심분리를 상기 기술된 바와 같이 반복하였다. 원심분리후, 부유하는 LDL을 pH7.4의 127mM NaCl, 8mM Na2HPO4·2H2O, 2.7mM KCl 및 1.47mM KH2PO4를 함유하는 완충제에 대해 투석하였다. 투석후, LDL을 미세공 필터 (0.45μM)를 통해 여과하고, 단백질 함량을 결정했다. 25㎍/ml의 희석액을 미세공 투석 완충제로 제조하였다. 산화는 파마시아 울트라스펙(Pharmacia Ultraspec)III분광 광도계로 결정하였다. 공액디엔 형성이 234nm에서 등록되었다. 약제 또는 용매(DMSO)의 2㎕를 이어서 1ml LDL(25㎍/ml)을 에펜도르프 튜브에 첨가하였고, 이들 물질을 온화하게 혼합하여 13000g에서 1분간 원심분리한다. 용액을 분광광도계에서 37℃로 온도조절된 퀘르쯔(Quartz)큐빗에 피펫팅 하였다. 평형후, 산화 반응을 CuSO4(최종농도 10μM)를 첨가하여 개시시켰다. 설험의 시간 경과 동안 매 3분 마다 흡광을 측정하였다. 234nm에서의 흡광은 3개상; 지연상, 증식상 및 분해상으로 분리하였다(제1도). 지연상은 증식상의 흡광 곡선의 경상의 접선의 베이스라인과의 교차로 정의하였다. 산화율은 경사의 접선으로 정의하였다. Tmax는 증식과 분해 상 사이에 최대 흡광을 얻기 위하여 필요한 시간으로 정의하였다. tmax에서의 흡광에서 to에서의 흡광을 뺀 것을 형성된 디엔의 총량의 지시자로서 취했다.
분석의 재생을 입증하기 위하여 참고 LDL샘플을 제조하고, 암흑 질소하에서 4℃로 유지했다. 이 참고 샘플은 모든 산화 실험에 포함시켰다.
LDL은 23명은 대상으로부터 단리했다. 혈장 콜레스테롤은 156내지 278mg/dl(평균 213mg/dl)범위이었고, 트리글리세리드는 47내지 328mg/dl(평균 191mg/dl)범위이었으며, 인지질은 178 내지 327mg/dl(평균 231mg/dl)범위이었다.
제2도는 네비볼롤의 2 하이드록시 유사체:
[(RSSS)+(RSRR)+(SRSS)]-α[[[2-(크로만-2-일)-2-하이드록시 에틸]아미노]-메틸]-6-하이드록시-2-크로만-메탄올에탄디오에이트(1:1) 및 [(SRSR)+(SRRS)+(RSS R)]-α,α′-이미노비스(메틸렌)비스[6-하이드록시-2-크로만-메탄올]의 항산화 효과를 보여준다. Tmax와 지연시간은 모두 증가되었고, 산화율은 감소한다. 시험관내에서 네비볼롤은 어떤 항산화 효과도 갖지 않았다. 참고 화합물로서의 비타민 E는 10μM보다 낮은 농도에서 어떤 항산화효과도 나타내지 않았다(결과는 나타나지 않은). 20μM 비타민 E에서의 tmax는 본 출원인의 실험조건에서 네비볼롤의 하이드록시 대사물 2μM 경우의 tmax에 근접했다.
산화율, 초기 흡광도 및 디엔의 총량은 비타민 E에 의해 영향을 받지 않았다.
결론적으로, 네비볼롤의 하이드록시 대사물은 시험관내에서 인간 LDL에 항산화 효과를 갖는다. 그러므로, 네비볼롤은 아테롬성 경화증, 염증 및 암에서 일어나는 유형의 시험관 내에서의 산화적 손상에 대해 중용한 보호 효과를 가질 수 있다.
[실시예 2]
[네비볼롤의 시험관내 항아테롬성 경화 효과]
네비볼롤이 노화-관련된 혈관 병리학에 어떤 효과를 갖는다는 것을 입증하기 위하여, 다수의 랫트에 네비볼롤을 24 개월동안 0(담체만), 2.5, 10 및 40mg/kg의 투여량으로 경구적으로 투여했다. 담체로서 네비볼롤의 생물학적 이용성을 개선하는 2-하이드록시-프로필-β-사이클로덱스트린을 사용했다. 각 투여량에서 50마리의 수컷과 암컷 그룹을 2년의 연구에 참여시켰다. 어떤 네비볼롤도 수용하지 않은 50마리의 수컷 및 암컷 대조군을 또한 연구에 참여시켰다. 시험 동물을 해부하여 조직학적 비-종양 변화에 대해 관찰했다. 혈관 퇴화 또는 노화에 관련될 수 있는 하기 조직학적 특징이 특히 관찰되었다.
- 복부 중피, 췌장 및 고환의 맥관병,

- 신장 조직학, 특히 만성 신장 질병 및 무기질 침착에 대한 증거인 호염기성 세관의 수.
조직 병리학적 관찰은 다음과 같이 0 내지 3의 점수를 받았다:

1치 배 해마 배양물을 본질적으로 앞서 기술된 바와 같이 제조하였다

(1992) Chronic treatment with sabeluzole protects cultured rat brain neurons from the neurotoxic effects of excitatory aminoacids. Synapse, 12:271-280). 배아기 17 일째 랫트의 해마 형성물을 절개해서 DMEM(Dulbecco Modified Eagle Medium) 중의 0.05%트립신, 0.1mg/ml DNase I 중에서 분리시켰다. 열-불활성화된 말 혈청(HS)을 4% 농도로 첨가하고 세포를 원심분리한 후, DMEM으로 세척하여 10% HS를 함유하는 DMEM/Ham의 F12(3:1)에서 재현탁시켰다.
세포를 폴리 L-라이신(0.001%) 예비코팅된 다중웰-24 플레이트 4×105세포/㎠의 밀도로 플레이팅시켰다. 배양하여 1일째, 배지를 화학적으로 정의된 CDM-R12-배지(0.26%소 혈청 알부민, 30 nM 나트륨 셀레니트, 3 nM 3,3′, 5-트리요도-L-티로신, 0.35μM레티놀, 0.35μM레티놀 아세테이트, 2.3μM DL-a-토코페롤, 2.1μM DL-a-토코페롤 아세테이트, 3.6μM 리놀렌산, 3.6μM 리놀레산, 0.125% 인간 트랜스페린, 20μM 프로게스테론, 57.7nM 코티코스테론, 49 U/ℓ 인슐린 0.4μM 비오틴, 10μM L-카니틴, 83μMD(+)-갈락토즈, 3.3μM 글루타치온, 10μM 에탄올아민, 0.1 mM-푸트리신을 함유하는 DMEM-HEPES/Ham′s F12(3:1))(Romijin, H. J., van Huizen, F., Wolters, P.S. 의 문헌[(1984) Towardsan improved serum-free, chemically defined medium for long-term culturing of cerbral cortexissue. Neurosei. Behav. Rev., 8:301-334)]로 변화시켰다. 배양물의 급성 및 연장된 예비처리는 본질적으로 앞서 기술된 바와 같다(Pauwels등, 1992). 약제를 10% 또는 1% 하이드록시프로필-β-사이클로덱스트린에 용해시켜 희석시켜 급성 및 만성의 노출에서의 용매의 최종 농도는 각각 0.1% 및 0.01%였다. 하기 실시예에서 사용된 화합물 A는[(RRSR)+(RRRS)+(SSSR)+(SSRS)]-2-[[[2-(크로만-2-일)-2-하이드록시에틸]아미노]메틸]-6-하이드록시-2-크로만-메탄올 에탄디오에이트(1:1)이다.
급성 처리를 위하여 약제 또는 용매를 7일째 배양물에 첨가했다. 20분후, 배지를 200㎕의 화학적으로 정의된 DMEM 배지(Ham′s F12 가 없는 CDM-R12 배지와 같은 조성)(대조군) 또는 산화성 스트레스 트리거(trigger)를 함유하는 화학적으로 한정된 DMEM 배지로 대체했다. 사용된 산화성 스트레스 트리거는 1mM 나트륨 니트로프루시드(SNP, 일산화질소 라디칼 발생 화합물), 또는 7.5-10 mM 디에틸디티오카바메이트(DDTC, 과산화물 디스뮤타제 억제제) 및 시험화합물이었다.
연장된 예비처리는 실험 경우, 약제(농도 범위 0.1 nM-10μM)를 배양의 1일 및 4일째 혈청없는 배지에 첨가하였다. 7일째, 배양물을 한 번 DMEM으로 세척하고, 0.2ml의 화학적으로 정의된 DMEM 배지와 함께 37℃에서 2 시간 동안 보온했다. 세포를 세척하고, 배지를 0.2ml의 화학적으로 정의된 DMEM 배지(대조군), 또는 산화성 스트레스 트리거를 함유하는 화학적으로 정의된 DMEM배지(상기 참조)로대체하였다.
급성 실험 및 연장된 예비처리 실험 모두의 경우, 세포외 및 세포내 LDH(락테이트 디하이드로게나제) 활성을 베르그메이어(Bergmeyer)의 방법 (Bergmayer, H.U.(1974) Biochemical reagents. In: Methods of Enzymatic Analyis. H.U. Bergmeyer, eds. Academic Press, New York, 2 nd Ed., pp. 480-242)에 따라 EPOS Analyzer 5060 으로 340 nm에서 분광 분석법으로 측정했다. 배양 배지중의 세포외 LDH 활성과 1ml H2O 중에서 세포의 분해후 측정된 세포질 LDH 활성을 총 LDH 활성 및 방출된 LDH %를 계산하기 위하여 사용하였다. 신경 보호 %는 LDH 방출의 억제 %를 나타내며, 여기에서 산화성 스트레스-트리거-유도된 LDH 방출은 0% 보호로 정의되며, LDH 방출의 기초(대조군)수준은 100% 보호로 정의된다.
VitE 결여 실험 경우, 초기 배양물을 상기와 같이 설정하나, 배양 1일째 배지를 시험 화합물의 존재 또는 부재하에서 DL-a-토코페롤(VitE) 및 DL-a-토코페롤 아세테이트가 있는 (대조군) 또는 없는 화학적으로 정의된 CDM-R12 배지로 바꿨다. VitE를 배양 배지로부터 뺐을 때, 심각한 세포 치사가 시험관내에서 4일째 관찰되었다. 화합물을 첨가함으로써 배양물을 구조할 수 있었다. 배양물 생존을 세포질 LDH 활성에 의해 측정하였다. VitE 보상%는 세포질 LDH%를 나타내며, 여기에서, VitE를 함유하는 배양물에서 나타난 바와 같이 높은 LDH 수준은 100%라 정의하고, VitE 가 결여된 배양물에서 나타난 바와 같은 낮은 LDH 수준은 0%라 정의하였다. VitE의 보상에 대한 EC50은 배양물 생존을 VitE 보충된 배지에서 증식된 배양물에 대해 나타난 생존의 50% 까지 회복하기 위하여 요구되는 화합물의 농도이다. 각 화합물의 7개 농도를 3중으로 시험하였고, 독립된 실험의 수는 표에 나타내었으며, 평균 EC50-값±SD를 계산하였다.

초기 신경 세포 배양물은 시험관 내에서의 생존에 대해 배양 배지중의 항산화제 VitE의 존재에 영향을 받는다. VitE가 결여된 배지를 배양물을 배양하기 위하여 사용할 때, 생존은 대조군의 ±20%로 떨어진다. 항산화제 성질을 갖는 화합물은 VitE의 결여를 보상할 수 있고, 이들은 배양 배지에 첨가하였을 때 VitE결여 배양물을 구조할 수 있다. 수개의 네비볼롤 유도체를 초기 신경 세포 배양물에 대한 VitE 결여 시험에서 10-7M 및 10-6M에서 시험하였다. 화합물 A, 네비볼롤, ℓ-네비볼롤 및 d-네비볼롤은 모두 VitE 결여된 배양물의 생존을 특정 정도까지[표 2] 구조했다.
화합물 A는 가장 효능있는 화합물이었다: VitE 결여된 배지에서 증식된 1차 신경 세포 배양물의 완전한 구조가 10-6M에서 나타났다[표 2]. 배얄물을 대조군(대조군 4.4μM VitE를 함유하는 배지에서 증식된 배양물이다)의 50% 까지 구조시킨 화합물 A의 농도는 126±88 nM 이었다[표 3].
배양물을 과산화물 디스뮤타제 억제제 DDTC, 및 일산화질소 도너 SNP로 배양물로 처리하는 것을 라디칼 (각각 O2 -및 NO) 개재된 신경 독성을 가져온다.
항산화 성질을 갖는 화합물은 트리거 바로 직전 및 그동안 첨가하거나, 트리거전 6일동안 첨가한 경우에(연장된 예비처리) 배양물을 라디칼 개재된 신경 독성으로부터 보호할 수 있다. 표 3으로부터, 네비볼롤과 화합물 A가 하나 또는 나머지 처리 모범에서 DDTC 또는 SNP에 대해 배양물에 신경보호를 제공할 수 있다. 처리-의존 신경보호에 대한 기초가 되는 메카니즘은 지금가지 알려져 있지 않다. 그러나, SNP 및 DDTC 에 대한 네비볼롤 및 화합물 A 경우 나타난 부분 보호가 카브딜올 경우 관찰되지 않으며, 이는 상기면에서 네비볼롤과 화합물 A를 우수한 약제로 만든다.

시험 화합물을 배양의 1일 및 4일째(연장된 예비처리) 또는 배양 7일째에 (급성 처리) 투여했다. 산화성 스트레스 트리거를 7일째 투여하고, 신경보호를 LDH 방출의 억제에 기초하여 계산했다. NA는 활성이 아님, 즉 득점 <25%의 신경보호를 의미한다.
[실시예 4]
특정 세포에서 시스틴 흡수의 글루타메이트에 의한 캄피티티브(Commpetitive)억제는 글루타치온 결여 및 산화성 스트레스에 이른다. 산화성 스트레스 모델은 신경교의 C6 글리오마((glioma) 세포(Kato 등의 문헌 [1992, A. mechanism for glutamate toxicity in the C6 glioma cells involving inhibition of cystine uptake leading to glutathion depletion. Neurosci. 48: 903-914)]. 및 신경 세포계 N18RE105(Murphy 등의 문헌 [1989, Glutamate toxicity in a neuronal cell line involve inhibition of cystine transport leading to oxidative stress, Neuron 2: 1547-1558])에 대해 기술되어 있다.
[세포 배양]
C6 글리오마 세포계(American Type Culture Collection, CCL 107)를 2 또는 4mM 글루타치온, 1mM 피루베이트 및 5 또는 10% 열-불활 성화된 송아지 태아 혈청이 보충된 DMEM에서 배양하였다. 배양물을 공기/5-10% CO2.물포화 대기하에 37℃에서 유지했다.
[글루타치온 결여 및 항산화제로서의 약제의 평가]
24-웰 배양물 플레이트에 30,000-50,000 세포/㎠ 로 플레이팅된 배양물(독성 및 과산화 지질 측정을 위하여) 또는 96-웰 배양 플레이트에 136,000 세포/㎠로 플레이팅된 배양물(GSH 결정)로 실험을 하였다. 8-24 시간후 배양물을 최종 농도 10mM 의 시스틴 흡수 억제제 글루타메이트의 부재하 또는 존재하의 배양 배지로 옮겼다. 산화성 스트레스의 억제에 대해 약제를 시험하기 위하여 약제를 글루타메이트와 함께 첨가했다(용매의 최종농도는 0.01% 하이드록시프로필-β-사이클로덱스트린, 0.1% DMSO). 세포내 글루타치온 수준을 6-8 시간후 측정하고, 과산화 지질을 14-20 시간후 측정하며, 독성 및 보호를 베르그메이어 및 베른트(Bernt)의 방법(UV-assay with pyruvate and NADH. In: Methods of Enzymatidc Analysis, 1974. H.U. Bergmeyer, ed. Acad. Press, New York, 2nd ED., pp 574-579)에 따라 LDH 분석을 이용하여 48 시간후 분석했다.
[C6글리오마 배양물내의 GSH 함량의 결정]
GSH 수준을 세척 및 균질화 과정을 변형하여 본질적으로 반디푸트(Vandiputte) 등에 의해 기술된 바와 같이 (Amicrotiterplate assay for total glutathione and glutathione disuolfide contents in cultured/isolated cells: performance study of a new miniaturised protocol. 1994. Cell Biol. Toxicol. 10: 415-421) 미세방법에 의해 분석했다. 세포(96-웰 플레이트 중에)를 PBS 로 세척하고, 1.3% 5-설포살리실산을 함유하는 50㎕의 10mM HCl 중에서 균질화시키며, 균질화물을 4℃에서 1200 ×g로 10분간 원심분리했다. 상등액 40㎕를 96 플레이트의 한 웰에 옮기고, 143 mM 인산염 완충제 pH 7.4 중의 200㎕ 시약(1 mM 5,5′-디티오비스-(2-니트로벤조산), 0.34 mM NADPH 및 6.3 mM EDTA를 첨가하여 실온으로 5분간 평형화시킨후, 40㎕ 글루타치온 리덕타제 (8.5 IU/ml 143 mM 인산염 완충제, 6.3 mM EDTA pH 7.4)를 첨가하여 반응을 개시시켰다. NADPH 산화가 멀티스칸(Multiskan) MCC/340(Labsystems)으로 414 nm에서 5분간 이어지고 1분당 흡광(DA) 변화를 계산했다. 글루타치온 함량을 시험마다 0.2 내지 2 나노몰 시판 GSH 범위의 표준 곡선(농도 대 플로팅된 DA/분)으로부터 추론했다.
[세포내 과산화물의 형광 측정]
세포내 과산화물의 형성은 6-카복시-2,7-디클로로하이드로플루오레세인 디아세테이트, 디(아세톡시메틸 에스테르 (C-DCDHF, 분자 플로브)를 사용하여 검출하였다. C-DCDHF를 10mM 의 농도로 DMSO에 용해시키고 질소하에서 -70℃에서 저장했다. 배양물을 글루타치온 결여제에 노출시킨후, 세포에 10μM C-DCHF를 37℃에서 1시간 적재했다. 배지를 아스피레이션하고, PBS를 세포에 첨가하여, 플레이트를 시토플루어 II마이크로플레이트 판독기(Cytofluor II microplate reader)(PerSeptive Biosystems)에서 판독했다. 여기 및 방출 파장은 485/530nm 필터쌍으로 선택했다. 결과를 상대 형광 유니트/㎍ 단백질로 표현했다.
[결과]
C6 글리오마 세포 배양물을 10mM 글루타메이트로 6시간 이상 동안 처리하면 세포내 글루타치온 수준이 급격히 감소한다[표 5]. 글루타치온의 감소는 독성 세포내 과산화물에서의 ±6-배 증가에 의해 나타낸 바와 같이[표 5], 산화-스트레스를 가져왔다. 최종적으로 세포 치사는 16 내지 48 시간후 일어난다. 여기-독성은 세포질 LDH 방출 (세포 독성의 지수)이 NMDA 길항제 MK801에 의해 방해되지 않기 때문에 (데이타는 나오지 않음), 상기 독성에 의해 포함되지 않는다. 배양 조건하에서, C6 글리오마 배양물중의 기초적 LDH 방출은 총 LDH(평균치±SD, n=12)의 2.7±0.3%이었고, 10mM 글루타메이트로 48시간 처리후 LDH 방출은 총방출의 93.2±1.3%이었다. 네비볼롤과 화합물 A 모두는 이들 배양물을 세포치사로부터 고효능적으로 보호하였다. 보호에 대한 IC50은 대략 9nM 임이 밝혀졌다. 이 보호는 글루타치온 수준[표 5]의 회복에 기인하지 않고, 이 글루타치온 수준은 건강한 대조군 세포에 나타난 수준의 약 반으로 남았으며, 이는 두 화합물이 GSH-결여-중재된 산화성 스트레스를 방해하는 결과임을 나타낸다.
세포내 과산화물의 평가는 네비볼롤과 화합물 A가 과산화에서 글루타메이트 중재된 증가를 억제함을 보였다[표 5].
[결론]
이는 두 화합물이 효능있는 산화제로서 세포 배양물에 작용하는 명백한 시험관내의 증거이다; 이 두 화합물은 세포내 독성 과산화물의 산화성 스트레스-중재된 증가를 억제하기 때문에 세포를 산화성 스트레스-중재된 세포 치사로부터 보호한다.

- 제3항에 있어서, 네비볼롤의 1일 용량이 0.1 내지 20mg 범위인 조성물.
- 제4항에 있어서, 1일 용량을 단일 투여형으로 제공하는 조성물.
- 제1항에 있어서, 약제학적으로 허용되는 담체 및 활성성분으로서 유효량의 제1항에서 정의된 일반식(I)의 화합물을 함유하고, 유효량의 아테놀롤, 셀리프롤롤, 프로판올롤, 로바스타틴, 플루바스타틴, 프라바스타틴, 심바스타틴, 겜피브로질, 자라고즈산, 프로부콜, 비타민 E, 카브딜롤, 베라파밀, 딜티아젬, 또는 비타민 C를 추가로 함유하는 조성물.
- 제6항에 있어서, 일반식(I) 화합물이 네비볼롤인 조성물.
- 제7항에 있어서, 경구 투여에 적합한 조성물.
- 제1항에 따르는 일반식(I)의 화합물, 및 아테놀롤, 셀리프롤롤, 프로판올롤, 로바스타틴, 플루바스타틴, 프라바스타틴, 심바스타틴, 겜피브로질, 자라고즈산, 프로부콜, 비타민 E, 카브딜롤, 베라파밀, 딜티아젬, 또는 비타민 C로 구성된 그룹중에서 선택된 또 다른 치료제를 함유하는, 노화로 인한 정상적 혈관 퇴화를 앓고 있는 인간을 치료 또는 예방하는데 있어서 동시에, 별개로 또는 연속적으로 사용하기 위한 배합 제제.
