【発明の詳細な説明】 縦に柔軟な拡張し得る改善ステント 本出願は出願Serial No.08/396,569(1995.3.1出願)の継続出願であって、そ の開示内容はこの引用によりここに包含される。技術分野 本発明は、体内器官、特に血管内における移植のための内部人工器官に関する 。更に特定すると、縦方向の拡張性が改善された管状の拡張し得るステントであ る。本発明の背景 ステントは、血管中の狭窄症(例えば瘢痕性拘縮性又は沈積性狭窄症)又は動 脈瘤を治療するために血管中に置かれ又は移植される。それらは、血管の虚脱し つつある、部分的に閉塞した、弱った又は拡張している部分を強化するために移 植される。それらは、尿管又は胆管中にも移植される。 典型的には、ステントは、設置する際には未拡張の(閉鎖した)径を持ち、器 官又は管中に設置された後拡張した(開放した)径を持つ。或る種のステントは 自己拡張性でありそして他の或る種のステントはステントの中で、バルーンの膨 脹のようにステント内から直径外方向への力により機械的に拡張される。 後者の例は、パルマズ(Palmaz)出願の米国特許第4,733,665号(19 88年3月29日公告)に記載されており、カテーテルを使用する移植のための ステント構造の数例が開示されている。そのカテーテルは、ステントを血管中に 設置した後、ステントを成形、変形して拡張するためにステント内のバルーンが 膨脹する配置を含んでいる。 自己拡張型のステントは、ドッテル(Dotter)出願の米国特許第4,503,5 69号(1985年3月12日公告)に記載されており、温度変化により移植し た構造へ拡張する形状記憶ステントが開示されている。形状記憶材料から製造さ れていない他の型の自己拡張型も知られている。 本願発明は、下記詳細に説明するように構築される際に縦方向に拡張性である 上述の全ての型に関する。柔軟性は、器官中で屈折するのに順応できるようにす るステントでは特に望ましい性能である。そのような例は、ヒルステッド(Hills tead)出願の米国特許第4,856,516号;ウォルフ(Wolff)出願の米国特許 第5,104,404号;マクグレゴル(MacGregor)出願の米国特許第4,99 4,071号、パルマズ(Palmaz)出願の米国特許5,102,417号;シャッ ツ(Schatz)出願の米国特許第5,195,984号;ヒルステッド(Hillstead) 出願の米国特許第5,135,536号;シェプペッシュ他(Shepp-Pesch)出願 の米国特許第5,354, 309号;ラウ(Lau)出願の欧州特許出願No.0 540 290 A2;シ ャッツ(Schatz)出願の欧州特許出願No.0 364 787 B1、及びPC T出願WO94/17754(ドイツ出願43 03 181としても同定)に 記載されている。 概して言うと、これらの種のステントは、一列にした、拡張性の、比較的柔軟 性でない、環状のセグメントの複数からなっていて、そのセグメントは普通は管 体を形成するために柔軟な素子により相互に連結しており、そしてその管体は可 なり分節しそして屈曲することができる。不幸なことに、そのようなステントに は、互いに向かって移動しそして接触するセグメントのために屈曲部の内側で隣 接するセグメントの間で屈折、重複又は干渉が起こり得るか、又は屈曲部位の外 側でセグメントは互いに移動して離れ、大きい間隔をおいてしまうかも知れない 問題点がある。これは、器官の不適当な支え、器官の損傷、流れ障害、捩れ、拡 張中のバルーンの破裂そして既に移植した装置を通過した後器官の未だ支えられ ていない部分へ設置される装置の再通過の困難等の原因になるかも知れない。 各々のセグメントの間の対角線結合を持つダイヤモンド構造と各々のセグメン トの全てのダイヤモンド構造も知られているがそのような密接構造は柔軟性に欠 ける。 本発明の目的は、このような問題がなくそしてセグメ ント間の柔軟関節におけるよりはむしろその構造のステント体セグメント内にお いて改善された(直径方向と縦方向の)柔軟性を発揮して、開放構造が縦方向に 柔軟であるステントを提供するものである。 本発明の概要 この目的のために、本発明は管状の拡張し得るステントを提供するものであっ て、 そのステントは、ほぼ管状のステント体を区画するために共通縦軸上に一列に 配置された複数の円筒形の開口円筒状セグメントからなり; 各々のセグメントは、拡張性のステントセグメントの周縁を区画するために、 互いに連結した実質的に平行である複数の支柱からなる波状の柔軟性のあるパタ ーンで形成された部材により区画されておりそしてそれら支柱の対は交代に互い に連結する末端部を持ち、そして上記の複数のセグメントの中で、各々のセグメ ントの中の対になった支柱の連結した末端部は、ステントが拡張される前には、 隣接するセグメント中の対である支柱の連結した末端部分に実質的に向かい合っ て位置する。上記の複数のセグメントは、一つのセグメント上の連結した末端部 の少数から隣接するセグメント上の連結した末端部の少数に伸長している複数の 相互連結素子により互いに連結されていて、各々のセグメントの一つの側からそ の他の側への複数の連結点の間に3個以上の脚があるよう になっている。更に上記連結素子は、一つのセグメントの連結した末端部から、 隣接するセグメント上の向かい合っている連結した末端部ではないその隣接する セグメントの連結末端部へ斜めに(angularly)伸長し、それによりステントが拡 張した結果、隣接するセグメントは互いに関してステント体の周縁部を移動して 離れて、複数のセグメントの間で柔軟性の連結手を関節化することによるよりは むしろ隣接するセグメントの間の干渉なしに対になっている支柱内でステントの 曲げを収める。その結果、セグメント間の連結素子は普通の使用条件下では屈折 するようなことはない。図面の簡単な説明 図1は、本発明の未拡張のステントの構造を示す。 図2は、管状の、未拡張ステントの図1のパターンを示す。 図3は、図1に示した構造の拡張ステントを示す。 図4は、本発明の別の未拡張ステントの平面構造を示す。 本発明の最良の形態 図に戻って、図1と図2は、未拡張ステント構造の部分的平面図と実際の(未 拡張)管状ステントを示す。即ち、ステントは判り易いように図1に平面で示さ れそして平らなパターン10(図1)から形成されてよくそし てそのパターンは、端部12と14(図1)を一緒にするようにパターンを巻く ことにより管状に変形される。複数の端部は溶接により又は同様のものにより接 続されて図2に示したような構造を形成する。 構造は、これらの図面に見られるように大体16に示した複数の隣接するセグ メントからなっており、それらの各々は実質的に平行である支柱18の波うつ柔 軟なパターンの形状になっている。支柱の対は交代する末端部19aと19bで 互いに連絡している。図1に見られるように、一つのセグメントの相互連結末端 部19bは隣接するセグメントの相互連結末端部19aに向かい合う。末端部は 図示したように概して楕円形であるが、丸型又は角形又は尖った形又は等々であ ってもよい。末端部のいずれの構造も図示したようにそれが波うつパターンを呈 する限りは受容できる。平面型10が図2に示したような未拡張管になる時は、 セグメントは円筒状であるが互いに隣接するセグメントの末端部19は互いに関 して向かい合っている。 製造のためにより好ましい方法は、薄壁管に始まり次にそれは所望の構造にす るためにレーザカットされる。その管は、適当な構造を形成するために化学的に エッチングされるか又はEDM(放電加工)されてもよい。 相互連結素子20は、一つのセグメント16の一つの末端部19から隣接する セグメント16の向かい合っている末端部19ではない隣接するセグメント16 の別の 一つの末端部に伸長する。一つのセグメント16の各々の側上の(複数の)点で あって相互連結素子20が末端部19と接続する(複数の)点の間の各々には少 なくとも3個の支柱が含まれる。これは、管状ステントの周縁上のセグメントの 間で相互連結素子20が斜め方向(angular dirtection)に伸長するという結果を 生む。相互連結素子20は、同じ長さであるのが好ましいが、一つのセグメント 間から他のセグメント間では異なっていてもよい。斜め方向(diagonal directio n)も、一つのセグメントから別の一つのセグメントへ或る場合には上方へ伸長し そして別の場合には下方へと逆転してもよいが、対になっているセグメントの間 の全ての連結素子は実質的に平行でなければならない。例えば、図1ではそれら が左から右へ下降して伸長している。この構造で左から右へ上昇して伸長しても よい。 この、隣接するセグメントとループの間の相互連結素子の斜め伸長の結果とし て、図3に見られるようにステントが拡張すると、セグメント16の間の最も近 い隣接末端部19は互いに移動して離れそして最早互いに向かい合うことはなく セグメント間の結合又は重複、換言すればピンチングを最小にするようになる。 相互連結する素子20の数は、個々の場合における環境に依存して変わってよ い。図示した構造ではセグメント当たり3個の素子で充分であり、少なくとも3 個が典型的に使用されるだろう。 図4に示した別の設計は、中間セグメント16中の支柱より長い支柱18aを 二つの末端セグメント16aの中に含む。これは末端セグメント(16a)が、 中間セグメント(16)より少ない圧縮抵抗を持つようにして、生来の器官から ステントの支持構造への遷移を更に緩やかにする。さもなければ、構造は図1に 示したのと同様である。 既に示したように、本発明は自己拡張性、機械拡張性の構造そして金属とプラ スチックの両方そして拡張性ステントとして機能し得る材料を含む多様の材料に 適用し得る。例えば、ステントはタンタル、ステンレス鋼又は同様のものの金属 の針金又はリボンであってもよい。それは薄壁であってもよい。それはニチノー ル(Nitinol)又は同様のもの等々のような形状記憶合金であってもよい。 上記の実施例と開示は説明のためであって、これによる限定を意図するもので はない。これらの説明と開示は多くの変形と別法を当業者に示唆するものである 。これらの全ての別法と変法は、添付した請求項の範囲内に包含されるものであ る。当業者はここに記載された具体例に同等な他のものを認識し、それらも添付 した請求項に包含されることを意図するものである。
【手続補正書】特許法第184条の4第4項 【提出日】1996年9月5日 【補正内容】 請求の範囲 1. ほぼ管状のステント体を区画するために共通縦軸上に一列に配置された 複数の円筒形セグメントであって、各々のセグメントは、拡張し得るステント体 の周縁を区画するために、相互連結した実質的に平行である複数の支柱からなる 波状のパターンで形成された部材により区画されておりそして上記複数の円筒形 セグメントの中で一つの与えられたセグメント内の支柱の隣接する対が向かい合 う末端で相互連結し、一つのセグメントの相互連結した末端は隣接するセグメン トの相互連結した末端に対して実質的に向かい合う複数の円筒形セグメント;及 び 各々が一つのセグメント上の対になった支柱の末端から隣接するセグメント上 の対になった支柱の末端へ伸長する複数の相互連結素子であって、該素子は一つ のセグメント上の一つの末端から、隣接するセグメント上の向かい合った末端へ ではなく別の一つの末端へ斜めに伸長し、該素子の分布はセグメントの向かい合 う側上の各々の連結点の間に少なくとも3個の支柱が存在するようになっている 複数の相互連結素子; からなる管状で可撓性の拡張し得るステントであって、 それにより、該ステントが拡張した際、隣接するセグメントの対になった支柱 はステント体の周縁上で互いに関して分離させられて、ステントの縦方向の屈曲 をセグ メント内で、隣接するセグメント間の干渉なしに、吸収する 管状で可撓性の拡張し得るステント。 2. 金属である材料からなる請求項1記載のステント。 3.金属が形状記憶合金である請求項2記載のステント。 4.ステントが薄壁の管状部材である請求項2記載のステント。 5.自己拡張構造である請求項1記載のステント。 6.機械的拡張性構造である請求項1記載のステント。 7.隣接するセグメント間の相互連結素子が同じ長さである請求項1記載のス テント。 8.ステントが末端セグメントと中間セグメントを含みそしてステントの末端 セグメントがステントの中間セグメントより長い支柱を含む請求項1記載のステ ント。 9. ほぼ管状のステント体を区画するために共通縦軸上に一列に配置された 複数の円筒形セグメントであって、各々のセグメントは、拡張し得るステント体 の周縁を区画するために、相互連結した実質的に平行である複数の支柱からなる 波状のパターンで形成された部材により区画されておりそして上記複数の円筒形 セグメントの中で一つの与えられたセグメント内の支柱の隣接する対が向かい合 う末端で相互連結し、一つのセグメントの相互連結した末端は隣接するセグメン トの相互連結した末 端に対して実質的に向かい合う複数の円筒形セグメント;及び 各々が一つのセグメント上の対になった支柱の末端から隣接するセグメント上 の対になった支柱の末端へ伸長する複数の相互連結素子であって、該素子は一つ のセグメント上の一つの末端から、隣接するセグメント上の一つの末端へ斜めに 伸長する複数の相互連結素子; からなる管状で可撓性の拡張し得るステントであって、 それにより、該ステントが拡張した際、隣接するセグメントの対になった支柱 はステント体の周縁上で互いに関して分離させられて、ステントの縦方向の屈曲 をセグメント内で、隣接するセグメント間の干渉なしに、収める 管状で可撓性の拡張し得るステント。
