Claims (1)
- 【特許請求の範囲】 1. 案内ワイヤを介して血管内の選ばれた位置に核照射源を配置するために 用いられ、 該照射源を受け入れるための内側内腔を有する可撓性の管状本体、 該案内ワイヤを受け入れるために前記本体上に形成された案内ワイヤ用溝、お よび、 前記内腔内部に該照射源を保持するために前記内側内腔上に形成された閉じた 遠端、を有するカテーテル。 2. 前記カテーテルを迅速交換式カテーテルとして使用可能とするために、 前記案内ワイヤ用溝が前記本体の遠端近くに形成される請求の範囲1に記載のカ テーテル。 3. 前記本体周囲に案内ワイヤを取り囲む密閉面を配設するため、前記本体 の近端近くに形成される第二の案内ワイヤ用溝部分をさらに有する請求の範囲2 に記載のカテーテル。 4. 前記本体を血管内部で径方向にのぞむ通りに配置するため、照射源の所 定位置に隣接して前記本体上に取り付けられる位置決め手段を有する請求の範囲 1に記載のカテーテル。 5. 前記位置手段は、 前記本体上に取り付けられる少なくとも一つのバルーンを有し、前記バルーン は、前記バルーンの少なくとも一端を前記本体から血管の壁に対して前記本体の のぞむ位置まで径方向に伸ばすように膨張自在であり、また、 前記本体上に形成される膨張内腔を有し、前記膨張内腔は、前記バルーンに膨 張圧を加えるため前記バルーンと流通状に接続される、 請求の範囲4に記載のカテーテル。 6. 前記少なくとも一つのバルーンは、前記本体を取り巻く膨張自在のコイ ルを有する請求の範囲5に記載のカテーテル。 7. 前記膨張自在のコイルは、直径がほぼ一定の複数のループを有する請求 の範囲6に記載のカテーテル。 8. 前記少なくとも一つのバルーンは、前記本体を取り巻く少なくとも一つ のほぼ環状のバルーンを有する請求の範囲5に記載のカテーテル。 9. さらに前記本体を取り巻く複数のほぼ環状のバルーンを有し、前記複数 のほぼ環状のバルーンは、前記本体からほぼ等しい距離だけ径方向に伸びる端部 を有する請求の範囲8に記載のカテーテル。 10. 前記ほぼ環状のバルーンは、各々が前記本体からほぼ等距離に伸びる 複数の丸い突出部を有する請求の範囲9に記載のカテーテル。 11. 前記位置決め手段が拡張自在のワイヤ構造を有する請求の範囲4に記 載のカテーテル。 12. 前記拡張自在のワイヤ構造は、複数の可撓性のワイヤ・ループを有し 、前記ワイヤ・ループは、各々が前記本体に取り付けられた第一の端部を有し、 また、 前記ワイヤ・ループの各々の第二の端部に取り付けられる撓わめ手段を有し、 前記撓わめ手段は、使用者が選択的に動かすことができ、前記ワイヤ・ループの 前記第二の端部を前記第一の端部へ向けて動かして前記ワイヤ・ループを径方向 外側へ撓わませ、それによって前記本体を血管の壁に対してのぞむように配置す ることができる請求の範囲11に記載のカテーテル。 13. 前記撓わめ手段は、前記複数のワイヤ・ループの前記第二の端部に取 り付けられる複数のワイヤを有する請求の範囲12に記載のカテーテル。 14. 核照射源を血管内の選ばれた位置に配置するために用いられ、内側内 腔を有する可撓性の管状カテーテル本体、 前記内腔内部に配置される細長い照射リボン、 前記リボン上に取り付けられる少なくとも一つの放射能元素、 前記カテーテル本体上に形成される案内ワイヤ用溝、および、 前記カテーテル本体を血管内部の選ばれた部位へ案内するために前記案内ワイ ヤ用溝内部に配置される案内ワイヤ、 を有する照射カテーテル・組立体。 15. 前記案内ワイヤ用溝の一部分は前記カテーテル本体の遠端近くに形成 されて前記カテーテル本体を迅速交換式カテーテルとして使用可能とし、また、 前記カテーテル本体の近端近くに形成されて前記本体の周囲に前記案内ワイヤを 取り囲む密閉面を構成する第二の案内ワイヤ用溝部分をさらに有する請求の範囲 14に記載の照射カテーテル・組立体。 16. さらに、前記カテーテル本体上に取り付けられる少なくとも一つのバ ルーンを有し、前記バルーンは、前記バルーンの少なくとも一端を前記本体から 血管の壁に対して前記本体ののぞむ位置まで径方向に伸ばすように膨張自在であ り、また、 前記カテーテル本体上に形成される膨張内腔を有し、前記膨張内腔は、前記バ ルーンに膨張圧を加えるため前記バルーンと流通状に接続される、請求の範囲1 4に記載の照射カテーテル・組立体。 17. さらに、前記放射能元素を径方向に位置決めするために前記カテーテ ル本体上に取り付けられる拡張自在のワイヤ構造を有する請求の範囲14に記載 の照射カテーテル・組立体。 18. 前記拡張自在のワイヤ構造は、 複数の可撓性のワイヤ・ループを有し、前記ワイヤ・ループは、各々が前記カ テーテル本体に取り付けられた第一の端部を有し、また、 前記ワイヤ・ループの各々の第二の端部に取り付けられる撓わめ手段を有し、 前記撓わめ手段は、使用者が選択的に動かすことができ、前記ワイヤ・ループの 前記第二の端部を前記第一の端部へ向けて動かして前記ワイヤ・ループを径方向 外側へ撓わませ、それによって前記放射能元素を血管の壁に対してのぞむように 配置することができる、 請求の範囲17に記載のカテーテル。 19. 核照射源を血管内の選ばれた位置に配置するために用いられ、内側内 腔を有する可撓性の管状カテーテル本体、 前記内腔内部に配置される細長い照射リボン、 前記リボンの遠端近くに配置される少なくとも一つの放射能元素、 前記カテーテル本体の遠端に隣接して形成される案内ワイヤ用溝、 前記カテーテル本体を血管内部の選ばれた部位へ案内するために前記案内ワイ ヤ用溝内部に配置される案内ワイヤ、 前記放射能元素を前記内腔内部に保持するために前記内側内腔上に形成される 閉じた遠端、および、 前記カテーテル本体を血管内部で径方向に選択的に心合わせするために前記放 射能元素に隣接して前記カテーテル本体上に取り付けられる拡張自在の心合わせ 手段、 を有する照射カテーテル組立体。 20. 前記心合わせ手段は、 前記カテーテル本体上に取り付けられる少なくとも一つのバルーンを有し、前 記バルーンは、前記バルーンの少なくとも一端を前記カテーテル本体から径方向 に伸ばして血管の壁に対して前記カテーテル本体を径方向に心合わせするように 膨張自在であり、また、 前記カテーテル本体上に形成される膨張内腔を有し、前記膨張内腔は、前記バ ルーンに膨張圧を加えるため前記バルーンと流通状に接続される、請求の範囲1 9に記載の照射カテーテル組立体。 21. 前記心合わせ手段は、拡張自在のワイヤ構造を有する請求の範囲19 に記載の照射カテーテル組立体。 22. 前記拡張自在のワイヤ構造は、複数の可撓性のワイヤ・ループを有し 、前記ワイヤ・ループは、各々が前記カテーテル本体に取り付けられた第一の端 部を有し、また、 前記ワイヤ・ループの各々の第二の端部に取り付けられる撓わめ手段を有し、 前記撓わめ手段は、使用者が選択的に動かすことができ、前記ワイヤ・ループの 前記第二の端部を前記第一の端部へ向けて動かして前記ワイヤ・ループを径方向 外側へ撓わませ、それによって前記本体を血管の壁に対して心合わせすることが できる、 請求の範囲21に記載のカテーテル組立体。 23. 血管は内部に配置された案内ワイヤを有し、 閉じた遠端および前記遠端に隣接して形成される案内ワイヤ用溝を有する照射 カテーテルを配設する工程、 その遠端に隣接して放射能源を有する照射リボンを配設する工程、 前記案内ワイヤの近端を前記案内ワイヤ用溝を通して挿入する工程、 前記照 射カテーテルを、前記照射カテーテルの前記遠端が前記狭窄部分内部に配置され るまで前記案内ワイヤを介して前記血管内に挿入する工程、 前記照射リボンを、前記放射能源が前記狭窄部分内部に配置されるまで前記照 射カテーテル内に挿入する工程、 を有する血管の狭窄部分を照射するための方法。 24. さらに、前記照射カテーテルの前記遠端を前記血管の壁に対してのぞ む径方向の位置に選択的に配置する工程を有する請求の範囲23に記載の方法。 25. 前記照射カテーテルの前記遠端を選択的に配置する工程が、ベンドを 前記案内ワイヤの遠端近くまで前記案内ワイヤ内に導入する工程、前記案内ワイ ヤを回転させて、前記ベンドを前記照射カテーテルののぞむ偏向方向へ向ける工 程、および、 前記案内ワイヤを前記案内ワイヤ用溝から部分的に引き戻しながら、前記ベン ドが前記案内ワイヤ用溝上に十分な力を加えて前記照射カテーテルがのぞむ量だ け前記のぞむ方向に偏向するまで前記照射カテーテルを縦方向の所定位置に保持 する工程、 を有する請求の範囲24に記載の方法。 26. 前記照射カテーテルの前記遠端を選択的に配置する工程が、前記照射 カテーテル上の前記カテーテルの前記遠端近くに拡張自在の位置決め手段を配設 する工程、および、 前記位置決め手段を選択的に拡張して、前記照射カテーテルを径方向にのぞむ 通りに配置する工程、 を有する請求の範囲24に記載の方法。 27. 前記拡張自在の位置決め手段がワイヤ構造を有し、また、 前記位置決め手段を拡張する前記工程が、前記ワイヤ構造を拡張する工程を有 する、 請求の範囲26に記載の方法。 28. 前記拡張自在の位置決め手段が膨張自在な構造を有し、また、 前記 位置決め手段を拡張する前記工程が、前記膨張自在の構造を膨張させる工程を有 する、請求の範囲26に記載の方法。
