Description
再狭窄は、少なくともその一部は、血管形成の部位での血管平滑筋細胞の遊走、増殖、および新内膜形成の結果として発生する。電離放射線を冠状動脈内へ照射することによって巣状中膜腺維が生じ、それが血管形成の部位に送り出されると再狭窄の進行を妨げる。
この照射処置によって、隣接する冠状動脈の諸部分および周囲の心筋は損傷を受けない。
狭窄部位の電離放射線の照射は、輸注カテーテルを通して照射リボンを導入して行なうことができる。公知のシステムでは、輸注カテーテルが案内ワイヤを介して狭窄部位へ挿入されるが、該案内ワイヤは、血管形成処置を行なう前に挿入し、また処置後そこに残しておいてもよい。輸注カテーテルの挿入後、該案内ワイヤが取り外され、その代わりに照射リボンが挿入される。照射リボンは、通常、その遠端近くに複数のイリジウム−192のシード(小型容器)またはペレットを組み込んだものである。他の電離放射線源も同様に使用することができる。
現在公知のシステムには、いくつかの欠点がある。第一に、案内ワイヤは、照射リボンの挿入前に引き戻さなければならない。案内ワイヤの引き戻しは、処置に少なくとも一工程が追加されることになりまた余分な時間がかかるため、外科医に歓迎されない。余分な工程を行なうことは、合併症の機会を増やすことになる。血管形成では、処置のほとんどで血管中の血液の流れが少なくとも部分的に妨げられ、それが該血管が血液を供給する筋肉に有害な影響をあたえるため、時間はきわめて重要な要素である。血管組織にのぞむ線量を照射するためには放射能源を数分間所定の位置に残して置かなければならない場合が多いため、照射処置の間にこの問題は複雑になる。症例によっては放射線の照射後に案内ワイヤを再挿入する必要があるため、時間の問題はさらに複雑になる。
輸注カテーテルの回転は、狭窄内部での放射能源の心合わせに役立つ場合もあろうが、この方法は、常に有効な方法ではない。既知の放射線量を冒された組織に均等に照射することが重要であるため、血管形成によって損傷を受けた組織内部で放射能物質の心合わせが必要となる場合も考えられる。線源から放射されるガンマ線の強度は、線源からの径方向の距離の二乗に逆比例して変化する。したがって、放射線源が血管内部の中心に置かれていない場合には、血管の片側に照射される線量が反対側に照射される線量と大きく異なるおそれがある。さらに、線源が血管と同心状でなくあるいは少なくとも平行でなくて血管の中心線に対してある角度で配置される場合には、照射線量が狭窄の全長にわたって軸方向にかなりの量変化するおそれがある。症例によって、狭窄の片側を反対側より多く照射したい場合には、放射線源を血管の中心線に平行にしかしそこからずらして配置することがのぞましいこともあろう。片側での組織の増殖が反対側でのそれより多いことから再狭窄が予想される場合には、このような処置がのぞまれることが起こり得よう。
あるいは、カテーテル本体の遠端近くに複数の可撓性のワイヤ・ループを取り付け、各ループの一端をカテーテル本体に固定し、他端を自由にする構成とすることもできる。ワイヤ・ループは、解放されれば自動的に拡張する構成とするか、あるいは各ループの自由端を使用者が縦方向に動かすことのできる拡張手段に取り付け、それによってワイヤ・ループの自由端を取り付け端部のほうへ移動させる構成とすることもできる。この移動によって、ループは外側へ拡張する。ループは、カテーテル本体の周囲に間隔を置いて取り付けられ、拡張によって血管内部でカテーテルが心合わせされる。拡張手段は、近くに配置されるワイヤ・ループの自由端を押す構成の比較的固いワイヤとするか、あるいは遠くに配置されるワイヤ・ループの自由端を押す構成のワイヤとすることができる。自動的に拡張するワイヤ・ループは、保持用外装を引き戻すと、拡張手段の助けなしに拡張する。
後に説明するように、これらの要素の中には他の実施形態では本発明の精神から逸脱することなく他の形をとることができるものもある。
カテーテル本体12上には、内側内腔14から離して案内ワイヤ用溝18が形成されている。該案内ワイヤ用溝18は、図示のようにカテーテル本体12の壁に取り付けられたダクトとして形成することもでき、内側内腔14の内部に形成することもでき、あるいは厚いカテーテルの壁の内部を通る内腔として形成することもできる。図示の案内ワイヤ用溝部分18は、カテーテル本体12の遠端近くのみに形成され、所定の位置に置かれる案内ワイヤ20の近端を案内ワイヤ用溝部分18を通して挿入し、さらにカテーテル本体12を所定の位置に置かれた案内ワイヤ20を介して患者に挿入することで迅速交換式カテーテルとしてカテーテル本体12を容易に使用できる構成となっている。
コイル状の心合わせ用バルーン22は、カテーテル本体12の両側部に添って間隔を置いて配置された複数の取り付け点32でカテーテル本体12に取り付けられる。これらの取り付け点32は、溶剤接着または超音波熔接などによる表面取り付けに用いられ、バルーンのコイル22と内側内腔14の間の連通は起こらない。心合わせ用バルーンのコイル22は、膨らませた状態で図示されている。
図3および図4は、本発明のカテーテル・組立体10’の他の実施形態を示す。この実施形態のカテーテル本体12’、案内ワイヤ用溝18、および膨張溝24’は、第一の実施形態のものとほぼ同様である。両実施形態の主たる相違点は、径方向の位置決めバルーンあるいは心合わせバルーンがほぼ環状の複数のバルーン・リング42として形成されることにある。各バルーン・リング42は、二つの端部あるいは丸い突出部44および46を有し、これらは、基本的に、第一の実施形態のバルーン・コイル22上の丸い突出部36および38と同じ機能を果たす。各バルーン・リング42は、膨張溝24’に添って間隔を置いて配置された複数の取り付け点32’で膨張内腔40’に流通するように取り付けられる。バルーン・リング42の内部と膨張内腔40’の間の流通は、取り付け点32’に配置される複数の膨張用入口48を通して行なわれる。上に示した二種類に加えて、他の形態の位置決めバルーンも、本発明の範囲から逸脱することなく考案することができよう。
位置決めバルーンまたはワイヤ・ループを使用する代わりに、カテーテル本体の遠端を径方向に位置決めする湾曲した案内ワイヤを使用する方法を用いることがのぞましい場合もあろう。外科医が、観察の結果、照射シードが配置されたカテーテル本体12の遠端がのぞむ通りに径方向に配置されていないとみた場合には、まず案内ワイヤ20を回転させて、ベンドをのぞむように案内ワイヤ20の遠端に向ける。該ベンドは、血管および処置する狭窄の物理的形状に応じて、図1または図3に示すように比較的穏やかなベンドとすることもできるし、あるいは、より顕著なものにすることもできる。さらに、一連のベンドを使用することが適当な場合もある。これらのベンドは、案内ワイヤ20を最初に挿入する前に該案内ワイヤ20内に導入することもできるし、あるいは、使用するカテーテルが全長にわたる案内ワイヤ用溝を有する場合には、案内ワイヤ20を引き戻し、適当に湾曲させ、それを再挿入することもできる。案内ワイヤ20を回転させて該ベンドをカテーテル本体12ののぞましい偏向方向に向けた後、カテーテル本体12を縦方向の所定の位置に保持しながら、その間に案内ワイヤ20の遠端を案内ワイヤ用溝18の遠端内に注意深く引き戻す。案内ワイヤ20の遠端上の該ベンドが案内ワイヤ用溝18の遠端に入るあるいは入り始めると、案内ワイヤ20は、案内ワイヤ用溝18の壁に横方向の力を加える。この横方向の力が、つぎにカテーテル本体12の遠端に伝えられ、該遠端をのぞむ方向に偏向させる。照射源を径方向に配置するために用いる方法のいかんにかかわらず、該照射源は、その後、のぞむ線量の照射が行なわれるまで所定の位置に残される。該放射線源をのぞむ時間所定の位置に残してのぞむ放射線の線量を得た後、カテーテル・組立体10を引き戻し、血管形成用カテーテルの挿入または必要な他の処置を行なうために、該案内ワイヤ20は所定の位置に残しておくことができる。
Claims (1)
- 核照射源を血管内の選ばれた位置に配置するために用いられた照射カテーテル組立体であって、
内側内腔を有する可撓性の管状カテーテル本体と、
前記内腔内部に配置される細長い照射リボンと、
前記リボンの遠端近くに配置される少なくとも一つの放射性元素と、
前記カテーテル本体の遠端に隣接して形成される案内ワイヤ用溝と、
前記カテーテル本体を血管内部の選ばれた部位へ案内するために前記案内ワイヤ用溝内部に配置される案内ワイヤと、
前記放射性元素を前記内腔内部に保持するために前記内側内腔上に形成される閉じた遠端と、
前記カテーテル本体を血管内部で径方向に選択的に心合わせするために前記放射性元素に隣接して前記カテーテル本体上に取り付けられる拡張自在の心合わせ手段と、
を有し、
前記心合わせ手段は、複数の可撓性のワイヤ・ループを有し、前記ワイヤ・ループは、各々が前記カテーテル本体に取り付けられた第一の端部を有し、また、前記ワイヤ・ループの各々の第二の端部に取り付けられる撓め手段を有し、前記撓め手段は、使用者が選択的に動かすことができ、前記ワイヤ・ループの前記第二の端部を前記第一の端部へ向けて動かして前記ワイヤ・ループを径方向外側へ撓ませ、それによって前記本体を血管の壁に対して心合わせすることができることを特徴とする照射カテーテル組立体。
