Description

【発明の詳細な説明】
【0001】
【技術分野】本発明は、通常、体内における血管や導管
の再狭搾を防止し、経皮的に腫瘍を治療するために使用
する導管ステントに関する。
【0002】
【従来の技術】ステントのワイヤ支柱の間の組織の侵入
による血管や導管の再狭搾を防止する装置が提供される
のが望ましい。このような状況は腫瘍が胆管に侵入する
ところで時々発生する。このようなケースではワイヤス
テントは腫瘍がワイヤステントの間で成長することがあ
り、最終的に、血管を再狭搾する結果になるため一時的
な解決を提供するだけである。 米国特許第4、58
0、568号はこのような従来のワイヤステントを例示
する。米国特許第4、580、568は、動脈、導管及
び体内における同様なものの流動障害を減少させるワイ
ヤステントを開示しているが、ステントのワイヤ支柱の
間における組織の侵入の問題を解決していない。かくし
て従来のワイヤステントは悪性の組織成長に直面したと
きに納得のいく長期間の解決を必ずしも提供しておら
ず、またこのようなステントは腫瘍を経皮的に治療する
装置も提供していない。
【0003】また、米国特許出願第422、606号を
参照する。この出願によればうまく埋め込んだ後に、体
内から経皮的に容易に回収することのできるステントの
構造および方法を教示する。多くの場合、このような利
点は本発明に適用される。
【0004】ここに自ら広がりしかも再狭搾を防止し、
あるいは減少することができる経皮ステントアッセンブ
リのニーズが残る。さらに腫瘍を治療する事のできる経
皮ステントアッセンブリのニーズもある。
【0005】本発明の1つの目的は改良した経皮ステン
トアッセンブリを提供することにある。

【問題点を解決するための手段】本発明のステントアッ
センブリの1つの実施例では両端に開口を有する柔軟性
を有するスリーブを含む。柔軟性を有するスリーブに1
つまたはそれ以上の弾性的に圧縮可能なステントが取り
付けられている。各ステントは複数の支柱を含み、その
支柱はその間に一連の間隙を形成する。ステントはステ
ントによって形成された間隙をスリーブによって実質的
におおうように柔軟性を有するスリーブに取り付けられ
ている。スリーブは弾力があるため、スリーブを有する
経皮ステントアッセンブリは圧縮されたときに第1の小
さな形状を、広がることできるときに第2の大きい形状
を弾性的に採ることができる。小さい第1の形状によっ
てアッセンブリを体内の管に管腔を介して経皮的に通す
ことができる。埋め込み中、スリーブを有するステント
アッセンブリは広がることが可能になり、第2の大きな
形状を占め、柔軟性を有するスリーブがステントによっ
て血管の壁を押し付け、血管を広げた状態に維持する。

【実施例】以下、本発明を添付図面に示した実施例に基
づき詳細に説明する。
【0008】いま特に図面を参照すると、図1に本発明
の経皮ステントアッセンブリ10の好ましい実施例の側
断面図を示し。経皮ステントアッセンブリ10は柔軟性
を有するスリーブ13によって包囲された2つのステン
ト11、12をそれぞれ含む。ステント11、12は閉
鎖されたジグザグ形状に形成された長いステンレス鋼の
ワイヤから形成されている。ワイヤの両端は色々な方法
で閉じる事ができ、スリーブを使用してもよく、その場
合スリーブは連続的な無限の形状を形成するためにワイ
ヤの両端に熔接するか、かたく締め付ける。各ステント
11、12は一連の接合部15、17によって互いに連
結された複数の支柱14からなり、図1Aによく示され
ている。多くの点で、ステント11、12は米国特許第
4、580、568号で述べられているZーステントに
類似しており、それについての説明は言及によってここ
に組み入れられている。各接合部15、17は小穴部1
8を備えるが、それらは、ワイヤを折り曲げることによ
って先端部を形成し、次に、背中合わせに熔接するか、
あるいはハンダ付けすることによって作られる。各支柱
14の連続した対は、その間に間隙16を形成する。ス
テント11、12は、この場合ナイロンであるが、ステ
ントアッセンブリの両端で接合部17を縫合するかある
いは接着することによってスリーブ13に取り付けられ
る。スリーブをプラスチックで作る場合は、ステントを
プラスッチック内に埋め込むことによってスリーブに取
り付けることができる。ステントをスリーブに取り付け
る方法は、本発明の意図した観点から離れることなく、
スリーブ13の材料または他の要因によって変化するこ
とは理解できよう。
【0009】ステントアッセンブリはスレッド20にお
いてジョイント15によって小穴部18を連続的に結び
付けることによりスリーブ13によって形成された軸線
に沿って、収縮することを妨げ、あるいはステント1
1、12の重複を防止する。スレッド20は好ましくは
バイオコンパティブル材料の単繊維である。ステントア
ッセンブリは適切に埋め込まれるためにその軸線にそっ
て収縮することを妨げることができ、収縮された小さな
形状の間にステントがからまるのを防止できるものでな
ければならない。図1Bは体内への埋め込みを容易にす
る小さな第1の形状に圧縮されたときの経皮ステントア
ッセンブリ10の側面図を示す。同じような収縮を防止
する結果は、熔接、縫合を含む種々の方法で、あるいは
米国特許番号第422、606号に開示されるようなイ
ンターロッキングアイを使用することによってジョイン
ト15を直接結合することによっても達成することが出
来る。
【0010】図3には腫瘍の成長によってほとんどふさ
がれた胆管の断面図を示す。従来のワイヤステントは管
の開口部を広げるように連続的に埋め込まれるが、腫瘍
はステントの支柱の間で連続的に成長しわずか数週間の
間に血管が再狭搾する結果となり、一時的な解決にしか
ならない。本発明に含まれるスリーブはこの問題を解決
する。図4には図3のふさがった管内に埋め込み中の圧
縮されたステントアッセンブリ10を示す。米国特許第
4、580、568号(コラム3、5行から18行)に
記載されているように、ステントアッセンブリ10は、
その圧縮された第1の形状で、平坦な端部を有するプッ
シャ24によってシース23を介して体内の所望の地点
まで押される。適当な位置に到達したのち、シース23
を引きくと、ステントアッセンブリ10は第5図に示す
ように、弾性的に、その大きな第2の形状を採る。スリ
ーブ13は、腫瘍がステントの支柱の間で成長するのを
防止し、再狭搾を防ぎ、従来のワイヤステントで可能な
期間よりもさらに長い期間の解決を提供することができ
る。
【0011】動脈瘤を経皮的に治療するのに、特にふさ
わしい本発明の他の実施例を図6に示す。経皮ステント
アッセンブリ50は、弾性的に拡張可能なステント5
1、52、剛性の支持ロッド52、及びナイロンスリー
ブ54をふくむ。ステント51、52は通常は前に述べ
たステント11、12と同じである。ステント51、5
2はアッセンブリの両端に位置する接合部55を縫合す
るかあるいは接着することによってスリーブ54に取り
付けられる。

め込まれる。図7は動脈瘤のある動脈に埋め込まれたス
テントアッセンブリ50を示す。ステント51、52が
動脈瘤の両側に位置する損傷をうけていない動脈壁に対
してスリーブ54を押し付けるとき、ステントアッセン
ブリは、所定の位置に固定される。次にスリーブ54
は、動脈瘤にかかる人工の動脈壁を形成する。これによ
って動脈瘤から圧力を取り除き、それを治療することが
できる。

Claims (11)

    【特許請求の範囲】
  1. 【請求項1】 内表面と外表面とを有し、両端が開口し
    た柔軟性を有するスリーブと、複数の支柱を有し、該複
    数の支柱はその間に一連の間隙を有する少なくとも1つ
    の弾性的に圧縮可能なステントと、前記各支柱によって
    形成された間隙を前記スリーブによって実質的に覆うよ
    うに、前記ステントを前記スリーブに取り付ける手段
    と、前記スリーブによって形成される軸線にそって経皮
    ステントアッセンブリが収縮するのを防止する手段と、
    からなり、圧縮されたとき小さい第1の形状を形成し、
    拡開可能になったとき大きい第2の形状を弾性的に形成
    する経皮ステントアッセンブリ。
  2. 【請求項2】 前記スリーブはナイロンで作られてい
    る請求項1に記載の経皮ステントアッセンブリ。
  3. 【請求項3】 前記ステントを前記スリーブに取り付け

    ステントと、前記ステントの間に配置され、両端を有す
    る剛性の支持ロッドと、からなり、前記ステントの各々
    は、剛性の支持ロッドの両端にそれぞれ結合されてお
    り、前記ステントの各々は、ステントと前記支持ロッド
    とが前記スリーブによって実質的に覆われるように前記
    スリーブに取り付けられており、経皮ステントアッセン
    ブリは、弾性的に圧縮されたとき小さい第1の形状を形
    成し、拡開可能になったとき大きい第2の形状を形成
    し、前記支柱及び剛性の支持ロッドは該支持ロッドによ
    って形成される軸線に沿って経皮ステントアッセンブリ
    が収縮するのを防止する経皮ステントアッセンブリ。
  7. 【請求項7】 前記スリーブは、ナイロンから作られて
    いる請求項6に記載の経皮ステントアッセンブリ。
  8. 【請求項8】 前記取り付け手段は複数の縫合からなる
    請求項7に記載の経皮ステントアッセンブリ。
  9. 【請求項9】 前記スリーブはプラスチックから作られ
    ている請求項6に記載の経皮ステントアッセンブリ。
  10. 【請求項10】 前記ステントをプラスチック内に埋め
    込むことにより前記ステントが前記スリーブに取り付け
    られる請求項9に記載の経皮ステントアッセンブリ。
  11. 【請求項11】 互いに平行に各ステントの間に配置さ
    れた複数の剛性の支持ロッドを有し、前記各支持ロッド
    の各々の端部は1つのステントに取り付けられている請
    求項8に記載の経皮ステントアッセンブリ。
