- 【特許請求の範囲】 1 管腔組織に対する加熱及び加圧を同時に行い、かつ管腔を膨張及び拡張する よう構成された可膨拡張カテーテルと共に使用するよう適合され、かつ、圧力が 適用され加熱された組織の生理学的反応を検出するよう構成された手段である、 前記カテーテル制御のために検出された組織の行動に反応するカテーテル制御手 段を含む手段を有する、身体管腔の拡張のためのシステム。 2 生理学的反応を検出する前記手段が前記カテーテルと接触した管腔組織の降 伏行動を検出するために構成され、かつ前記カテーテル制御手段がこれに反応す る、請求項1記載のシステム。 3 生理学的反応を検出するための前記手段が、前記カテーテルと接触した組織 の熱伝導特性の変化を検出するよう構成され、かつ前記カテーテル制御手段がこ れに反応する、請求項1記載のシステム。 4 前記加熱後の前記管腔組織の冷却中における前記カテーテルの収縮を防止す る手段を含む、請求項1、2又は3記載のシステム。 5 前記カテーテル制御手段がマイクロプロセッサを含み、かつ前記カテーテル が適用した温度及び圧力を示すフィードバックを受けるよう配されており、前記 制御手段が、該フィードバック及び該マイクロプロセッサが実行したアルゴリズ ムを基準として前記カテーテルが適用する加熱及び圧力を規制するよう適合され ている、請求項1、2又は3記載のシステム。 6 前記加熱及び加圧に対する生理学的反応に基づいて拡張時間のタイミングを 提供するよう構成され、かつ記されたタイミング手段を含む請求項1、2又は3 のシステム。 7 前記管腔組織にさらされている前記カテーテルの壁を通じた熱伝導によって 該管腔組織を加熱するよう構成されたカテーテルと組み合わされる、請求項1、 2又は3記載のシステム。 8 前記カテーテルが、電気による伝導性のある液体を充填できる風船型カテー テルであり、かつ前記の熱生成のための関連する加熱手段が、前記風船内のrf 電極及びI2R損失によって該液体を加熱する方法でこれにrfエネルギーを適 用する手段からなる、請求項7記載のシステム。 9 前記カテーテルが可膨カテーテルであり、かつ前記カテーテル制御手段が、 膨張制御手段である、請求項1、2又は3記載のシステム。 10 生理学的反応検出のための前記手段が、前記カテーテル内の液圧を感知し 、かつ圧力に反応する加熱された管腔組織の降伏行動によって生ずる前記圧力の 減少を検出するよう構成され、かつ配された圧力センサーからなり、検出された 該圧力変化に反応する前記カテーテル制御手段が、前記カテーテルの膨張容積を 増加する、請求項9記載のシステム。 11 生理学的反応検出のための前記手段が、前記可膨カテーテルの膨張容積の 変化を示す容積センサーを含む、請求項10記載のシステム。 12 前記膨張制御手段がサーボ・モーターによって稼働する注射器型ポンプを 含む、請求項9、10又は11記載のシステム。 13 前記注射器型ポンプの転移を計測し、これによって前記可膨カテーテルの 容積を表示するためのポジション・トランデューサーを含む、請求項12記載の システム。 14 前記可熱後の前記管腔組織の冷却中における前記カテーテルの収縮を防止 する手段を含む、請求項9、10又は11記載のシステム。 15 収縮を防ぐための前記手段が流体チェック・バルブである、請求項14記 載のシステム。 16 前記可膨カテーテル内の流体温度測定のための手段をさらに含む、請求項 9記載のシステム。 17 前記カテーテル制御手段が、前記可膨カテーテルの圧力又は容積及び温度 を表示する信号を受けるよう構成されたコントローラからなり、コントローラが 追加処置のために前記信号に応じて前記膨張及び温度を制御するよう構成されて いる前記カテーテル制御手段からなる、請求項9記載のシステム。 18 処置下にある前記組織の生理学的反応を示す読み出しを提供するディスプ レイ手段をさらに含む、請求項1、2又は3記載のシステム。 19 前記読み出しが前記カテーテルが適用した圧力、前記カテーテルによる加 熱及び前記カテーテルの容積を示す、請求項18記載のシステム。 20 熱伝導による前記風船から管腔組織への加熱及び該組織への加圧を同時に 行うよう配された可膨拡張風船型カテーテル及び関連加熱手段、及び適用される 圧力に対する加熱された組織の生理学的反応を検出するよう構成され、前記カテ ーテル制御のために検出された組織の行動に反応するカテーテル制御手段を含む 手段、を備えた、 身体管腔拡張のためのシステム。 21 前記カテーテル制御手段が、前記カテーテルと接触する管腔組織の検出さ れる降伏行動に反応して前記風船の膨張を増すよう適合された、請求項20記載 のシステム。 22 生理学的反応検出のための前記手段が、前記管腔組織の熱伝導特性の変化 を検出し、かつ検出されたかかる変化を基準として加熱を減ずるよう構成される 、請求項20又は21記載のシステム。 23 前記カテーテル制御手段が、プログラムされたマイクロプロセッサの制御 に基づくコントローうからなる、請求項20、21又は22記載のシステム。 24 前記マイクロプロセッサの前記プログラムが、加熱された管腔組織の降伏 行動を示す前記カテーテルからのフィードバックに反応して、膨張圧力コントロ ーラの膨張設定点を増すよう適合される、請求項23記載のシステム。 25 マイクロプロセッサが通常の拡張圧力を下回る圧力で管腔組織の加熱をな すようプログラムされる、請求項23記載のシステム。 26 管腔組織に対して同断に加熱及び加圧を行い、かつ管腔を膨張及び拡張す るよう構成された、可膨拡張カテーテルの組み合わせからなるシステムを使用し 、及び加熱された組織の圧力の適用に対する生理学的反応を検出するよう構成さ れた手段であって、前記カテーテル制御のために検出された組織の行動に反応す るカテーテル制御手段を合む手段を有する身体管腔拡張のための方法。 27 生理学的反応検出のための前記手段が、前記カテーテルによって構成され る管腔組織の降伏行動を検出するよう構成され、かつ前記カテーテル制御手段が これに反応する、請求項26記載の方法。 28 生理学的反応検出のための前記手段が、前記カテーテルに接触する組織の 熱伝導特性の変化を検出するよう構成され、かつ前記カテーテル制御手段がこれ に反応する、請求項25記載の方法。 29 前記拡張が管腔の変形のために使用される、請求項26、27又は27記 載の方法。 30 前記拡張が血管形成術のために使用される、請求項26、27又は28記 載の方法。 31 生理学的反応檢出時点から適度な期間の後前記工程の段階を終了すること を含む、請求項26、27又は28記載の方法。 32 前記可熱及び加圧に対する生理学的反応に基づく拡張期間のタイミングを 提供するよう構成され、かつ配されたタイミング手段を使用する、請求項26、 27又は28記載の方法。 33 加熱当初の温度が、摂氏約50度と70度の間である、請求項26、27 又は28記載の方法。 34 前記カテーテルが、液体が充填されている風船型カテーテルであり、かつ 風船の壁の厚みを通じて、該風船とかみ合わさっている壁の組織への前記液体か らの伝導による熱伝導で、前記管腔壁が液体の加熱によって加熱される、請求項 26、27又は28記載の方法。 35 前記風船内の液体が電気による伝導性を有し、かつ前記液体が、前記液体 に対する高周波電流の適用の今結果生ずるI2R損失によって加熱される、請求 項34記載の方法。 36 液体による膨張可能な拡張風船及び加熱される液体からの、風船壁を通じ た熱伝導を可能とするよう風船内に加熱された液体を制御可能な形で提供する手 段を設け、 カテーテルを血小板又は狭窄組織によって狭められた血管部分に挿入し、前記風 船を、血管壁を実質的に変位させることなしに、熱伝導関係において、狭められ た血管壁表面とかみ合わせるに十分な当初の副拡張圧力で膨張し、かみ合わさっ た血管壁の温度を風船内の加熱された液体からの熱伝導によって増加し、他方膨 張圧力、前記風船内の前記液体の温度、及び加熱された血管壁が前記熱伝導によ る壁の転化の結果前記拡張風船の前記圧力に降伏する生理学反応を生する処置期 間を制御し、 よって前記血管拡張が比較的低い圧力条件で生することを可能とすることからな る、 血管形成方法。 37 前記風船内の圧力を約2気圧以下に制御する、請求項36記載の方法。 38 前記温度を摂氏約60度から65度の範囲に維持する、請求項37記載の 方法。 39 当初、前記副拡張圧力を、血管を通じた血流が透視診断によって血管を観 察したときに肉眼で見えるほど該血管を広げずに実質的にブロックされる、よう な圧力に維持する、請求項36記載の方法。 40 当初、前記副拡張圧力を、血管が約10%を超えて広げられないような圧 力に維持する、請求項36記載の方法。 41 前記血管拡張後に、前記風船の温度を減少する間、前記風船の膨張を維持 する、請求項36記載の方法。 42 10ないし15秒間の前記副拡張圧力に対する前記膨張期間内に、風船の 温度を摂氏50皮から70度の最終温度に上昇し、かつ最高約60秒間前記最終 温度を維持し、その後前記膨張を維持しつつ、前記液体の前記可熱を終了し、か つ約15秒から30秒間前記風船を冷却することによって前記風船温度を減ずる 、請求項36記載の方法。 43 前記圧力が前記当初副拡張圧力を超えることを防ぐよう膨張圧力を制御す る、請求項36記載の手段。 44 さらに、前記血管形成術の進行監視、及び前記血管直径の変化率に応じた 温度、圧力又は処置時間の制御からなる、請求項36記載の方法。 45 さらに、前記血管壁の内容の亀裂の兆候となる前記血管直径の急激な変化 が生じた場合に、EE力、前記風船の温度又は処置期間を減することからなる請 求項44 記載の方法。 46 さらに、前記血管の直径が約0.5秒未満で約25%以上増加する場合に 、圧力、温度又は処置時間を減ずることからなる、請求項45記載の方法。 47 透視診断によって前記進行を監視する、かの方法。 請求項44ないし請求項46いずれ 48 前記風船内の圧力の変化を監視することによって前記工程を監視する、請 求項44ないし請求項46いずれかの方法。 49 血管の健康な部分と実質的に同一の直径を有する風船が設けられている、 請求項36記載の方法。 50 前記局部の軸長をわずかに上回る軸長を有する風船が設けられている、請 求項36又は請求項49記載の方法。 51 可膨風船が、前記膨張液体のI2R加熱手段と共に設けられている、請求 項36記載の方法。
