Description

【発明の詳細な説明】 −の１ 本発明は、エストロゲン及びその誘導体の経皮投与用物
品に関する。この投与用物品は、ある所定の期間にわた
って一定の放出速度を有する医薬含有接着性物品である
。
の　　　　　　ひ　　゛　　　べき エストラジオールは、生殖可能年代の間に卵巣により分
泌される主要なニス］・ロゲン（卵胞ホルモン）である
、閉経期に近い時期における初期の卵巣機能の低下は究
極的には閉経期後の年代におけるエストロゲンの卵巣で
の産生をほとんど完全に停止させるに至る。したがって
、閉経期後の婦人は主としてエストラジオールが欠乏し
ている。
婦人のなかには、この欠乏が血管述動性症候の発現と生
殖器官内の衰退的変化とを迅速に生じさせることか広く
認められている。さらに、ある種の心理学的症候、例え
ば、不安、健忘性及び神経集中の困難（即ち、閉経期症
候群）は、更年期及び閉経期後の早い時期において脈泊
を増大させる。また、卵巣不全も長期間にわたって生じ
る症候を有する。なぜならば、閉経期後の婦人では骨多
孔症やある種の骨折の発現の恐れが増大しているからで
ある。
したがって、更年期及び閉経期後の期間におけるエスト
ロゲンの代替は、あらゆる急性の症候を和らげるのみな
らず、骨多孔症の発現と骨折の発生とを防止するように
思われる。
鼓近、外部からのエストロゲン療法は主に経口投かによ
ることが指示されている。この療法はある種の欠点を持
っている。即ち、エストロゲンが消化管壁及び肝臓によ
って迅速に代謝されかつ不活性化されるため、高い薬用
量のエストロゲンを投与しなければならない（第一通過
効果）、エストロゲンが全身に循環する以前に投与量の
３０％までが不活性化されることが証明された。
エストラジオールを経皮投与する際に考えられる利点は
、消化管壁及び肝臓による代謝が回避でき、したがって
毎日の総薬用量を少なくして使用することを可能にする
のみならず、肝臓酵素誘発症及びその続発症の恐れを減
少させることである。エストラジオールの経皮投与は、
悪い代謝変化を誘発させることなく閉経期症候群を有効
に抑止することができる。
米国特許第４３７９４５４号にはエストラジオールの経
皮投与用物品が開示されている。この物品は、皮膚浸透
増進剤としてエタノールを使用する透過＋１９制御によ
る貯蔵器型物品である。この場合には、エストラジオー
ルはエタノールに溶解されており、この両者が皮膚を通
して浸透する。拡散速度を抑制するために透過性膜が使
用されており、この透過性膜Ｅに接着剤が適用されてい
る。
この物品の安定性は短く、投与速度は一定ではない、さ
らに、増進剤のエタノールが接着剤と反応して接若力を
失なわしめ、しがたって物品が皮膚から剥れ落ちる。
また、米国特許第４３９０５２０号には、インドメタシ
ンの経皮投与用物品が開示されている。
この物品は、医薬含有接着性物品である。この物品は、
インドメタシンと皮膚浸透増進剤のポリオキシエチレン
ソルビタンモノオレエート（ホリソルベート）を適所に
含む。後者はアクリ１し酸２エチルへキシル−酢酸ビニ
ルコポリマーのような接着性コポリマー中に混合されて
いる。インドメタシンはコポリマー中に直接溶解される
。
さらに、米国特許第４４２０４７０号は、狭心症を抑屯
し又は予防するのに有用な医薬のための経皮投与用物品
を開示している。この医薬はイソソルヒドジナイトレー
ト及びペンタエリスリトールテトラナイトレートである
。この物品は医薬含有接着性物品である。この物品は、
アクリルＭ２−エチルへキシル−酢酸ビニルコポリマー
のような接着性コポリマーと混合された医薬とポリエチ
レングリコールのような皮膚浸透増進剤とを適所に含む
ものである。
また１重複する部分を有しているように思われるが米国
特許第４２９１０１４号、同４３２１２５２号及び同４
４３８１３９号は、それぞれエストロゲン型医薬″のた
めの経皮投与用物品を開示している。これらの物品は、
一般にモノリス型の物品である。即ち、接着剤が皮膚と
マトリックスとの間にはさまれる０例えば、米国特許第
４４３８１３９号は、極性の可塑剤、ポリビニルアルコ
ール、ポリビニルピロリドン及びエストロゲンよりなる
マトリックスを開示している。極性の可塑剤はポリエチ
レングリコールであって、マトリックスを可撓性にさせ
、かつニストロ、ゲンの拡散による放出の信頼性を増大
させるために必要である。
さらに、ソルビタンのような洗浄剤（界面活性剤）及び
吸収促進剤も添加することができる。
さらに１重複する部分を有しているように思われるが米
国特許第４０５８１２２号及び同４０７７４０７号はそ
れぞれエストロゲンだめの医薬投与用物品を開示してい
る。これらの物品は、一般に貯蔵姦型物品である０例え
ば、米国特許第４０５８１２２号は、通路を有し、活性
剤を取り囲んでいる壁よりなる物品を開示している。こ
の壁は、外部流体に対して透過性であり、この流体がそ
の壁を通過して壁の内部で浸透圧を生じさせる。この浸
透圧が医薬を壁の後方から通路を通して押出す、壁を通
して流れる流驕を調節するため壁（単層）に添加される
物質としてはポリエチレングリコールが包含される。ま
た、一体的な複合物を作るため壁に添加される分散剤と
しては、５〜２０モルのエチレンオキシドを含有するポ
リオキシエチレン化ソルビタンオレエートが包含される
。
本発明は、不透過性支持層とこれに付着されたマトリッ
クスとからなっており、そして区分された透過性のポリ
マー製拡散制御膜を何ら含まない経皮投与用物品を提供
することを目的とする。
るための 本発明によれば、活性物質のための経皮投与用物品が提
供される。この活性物質とは、エストロゲン（卵胞ホル
モン）又はその誘導体であり、好ましくは１７β−エス
トラジオールである。この物品は、不透過性支持層とこ
れに付着されたマトリックスとを有する。このマトリッ
クスは、重量％基準で、接着性ポリマー約６０〜約９５
％、溶媒約５〜約２０％、皮膚浸透増進剤約０．２〜約
４％及び活性物質約０．５〜約５％からなっている。
この物品は、区分された透過性のポリマー製拡散制ｖ４
膜を何ら含まないものである。
ここで、本発明の経皮投与用物品の好ましい具体例の斜
視図を第１図に示す、また、第２図は、第１図に示した
物品の横断面図である。
第１図を参照するに、本発明の経皮投与用物品１０は、
不透過性支持層１２と、好ましくは一層以上の薄い層（
シルエットで示す）により形成されたポリマー製の拡散
マトリックス１４とを有し、そして区分されたポリマー
製の拡散膜を何ら含まないものである。このような拡散
膜の例は、米国特許第４３７９４５４号に記載されてい
る。
不透過性支持層は、斯界で周知であって、本発明に対し
て限定的なものではない。
マトリックス１４は、ｆｆｉ量％基準で、接着性ポリマ
ー約６０〜約９５％、製薬用等級の溶媒約５〜約２０％
、皮膚浸透増進剤約０．２〜約４％及び活性物質約０．
５〜約５％から構成される。
接着性ポリマーは、感圧性接着剤であって、医薬用とし
て認められているものである。この種のポリマーのうち
でも、水をベースとした材料又は溶媒をベースとした材
料のいずれも使用することができる。これらのポリマー
は、本発明において二つの４１　脂を持っている。第一
に、これらのポリマーは皮ｊＧに対して接着性であり、
マトリフクスを皮膚トにしっかりと保持するとともに皮
膚と良好な拡ｌ牧接触を維持させる。第二に、ポリマー
は活性物質を貯蔵するためのキャリアー又は担体である
。好ましくは、接着性ポリマーは酢酸ビニル−アクリル
酸エステルマルチポリマーである。このようなプルチボ
リマーは、セントルイス市のモンサンド社から登録商標
名「ＧＥＬｖＡ」として人手できる。ＧＥＬＶＡ７３７
．７８８及び２４８４を使用することができる。ＧＥＬ
ＶＡ７３７は、詳しくは、１．１％までのアクリル＃２
−エチルヘキシルを含み、残部は酢酸ビニルである。
身体の循環系統に対して治療上有効な薬量で経皮投与さ
れる活性物質は、好ましくは１７β−エストラジオール
である。しかしながら、周知であるエストロゲン又はエ
ストロゲン誘導体のいずれも使用することができる。
溶媒は、活性物質を溶解させるものである。溶媒は、好
ましくは、ポリエチレングリコール、ポリエチレングリ
コールエステル、プロピレングリコール又はこれらの組
合せである。ポリエチレングリコールとは、本明細書で
使用するときは、２００．３００，４００，５００．６
００及び８゜Ｏの各平均分子量を持つポリエチレングリ
コール並びにそれらの誘導体を意味する。ポリエチレン
グリコールエステルとは、本明細書で用いるとＳは、ポ
リエチレングリコールモノラウレート、ポリエチレンク
リコールモノオレエート、ポリエチレングリコールモノ
パルミテートなどを、ａ：味する。ここに記・成の溶媒
は、エメリー・インダストリー及びＢＡＳＦのような会
社から入手できる。
１＆　Ｂ浸透増進剤はポリオキシエチレンエーテルであ
って、好ましくはポリソルベート８ｏ、即ち、ソリビラ
ンモノ−９−オクタデセノエートポリ（オキシ−１，２
−エタンジイル）及びその誘導体である。しかしながら
、ポリソルベート２１．４０．６０．６１．６５．８０
，８１及び８５のようなその他のポリソルベートも使用
することができる。さらに、高級脂肪族アルコールのポ
リオキシエチレンエーテル、例えばＢＲＩ　Ｊ　５２．
５６．５８．３０．３５．９２．９６．９８．７２．７
６．７８．７００．７２１なども使用することができる
。このＢｌ（ＩＪは、プラウエア州のＩＣＩユナイテ−
７ド・ステーツ社の登録商標である。ポリソルベート８
０は、プラウエア州つイルミントン市のＩＣＩ社の製品
である。
使用するにあたって、本発明の物品は皮膚に適用され、
そこで接着性ポリマーによって該物品が皮膚に貼り付け
られる。溶媒によって溶解されている活性物質はマトリ
ックスの全体に配分されている。この活性物質は、増進
剤によってマトリックスから拡散する。皮膚角質層にお
いて増進剤がこの角質層から循環系統内への経皮拡散を
容易にさせる。活性物質の経皮１１ｉ、量は実質上一定
である０毛無しマウスの皮膚での経皮１に量の研究結果
を示すグラフを表わす第３図並びに人の死体の皮膚での
経戊流敬の研究結果を示すグラフを表わす第４図を参照
されたい。
本発明による経皮投与用物品の製造は、次のように行わ
れる。まず、活性成分が溶媒と増進剤とに溶解されて溶
液とされる。この溶液はポリマーに添加され、約２０〜
３０分間混合され、次いで気泡を除去するため約２０〜
６０分間放置される。次いで、この混合物は不透過性膜
であるポリエチレンフィルム又はスリーエム社製のアル
ミニウム被覆ポリエチレンフィルム（例えば、３Ｍ−ス
コッチパック１００６又は３Ｍ−１０１２）上に１＆延
され、約４０℃〜５０℃で約２０〜４０分間乾燥される
。被膜を乾炸させた後、周知のンリコーン剥離紙などの
ような剥離ライナー層１６がマトリックスの露出表面上
に設けられる。次いで、この物品はダイにより最適なサ
イズに切断される。多数のマトリックス層を必要とする
場合には、先の層の上に次の各層が流延され又は干ねら
れる。仕、ヒげられた物品はパウチ（平たい袋）に入れ
られ、気密封止される。
非呈上 下記の実施例は本発明を例示するためにのみ示すもので
ある。
実施例１ マトリックスを調整するため、０．５ｇのポリエチレン
グリコール２００．０．２５ｇのプロピレングリコール
及び０．０５ｇのポリソルベート８０に０．２ｇの１７
β−エストラジオールを溶解し、溶液とする。この溶液
をｌｏｇのポリマー溶液（モンサント社製のＧＥＬＶＡ
７３７）に添加し１次いで２０〜３０分間混合する。混
合した後、混合物を約２０分間放置して気泡を除去し、
支持用材料（３Ｍ−スコッチパック１００６又は１０１
２）上に流延する。流延された混合物を４５°Ｃで３０
分間乾乾燥る。三層のマトリックスを作るため、先の層
を放置し乾燥した後に第二及び第三層を先の層の上に順
次に流延させる０次いでこの成形体を４ｃｒｒｒ′の形
状に切断する。
このマトリックス成形体についての毛無しマウスの皮膚
での経皮流ｒ＾の研究結果を下記の表１に示す。
実施例２ ０．１ｇの１７β−エストラジオールを１．０ｇのポリ
エチレングリコール及び０．０１ｇのポリソルベート８
０に溶解する。この溶液を１２ｇのポリマー溶液（モン
サント社製のＧＥＬＶＡ７８８）と２５分間混合Ｔる。
２０分間放置した後、この混合物を支持用材料上に流延
させる。その後の残りの−［程は実施例１に記載のもの
と同じとする。
実施例３ ０．４ｇの１７β−エストラジオールを６ｇのポリエチ
レングリコール４００．２ｇのプロピレングリコール及
び０．５ｇのジエチレングリコールオレイルエーテルに
溶解する。この溶液を３０ｇのポリマー溶液（モンサン
ト社製のＧＥＬＶＡ２４８４）と２０分間混合する。そ
の後の残りの工程は実施例１に記載のものと同じである
。
実施例４ ０．５ｇの１７β−エストラジオールを４ｇのポリエチ
レングリコール４００．３ｇのプロピレングリコール及
び０．５ｇのポリソルベート２０に溶解する。この溶液
を２０ｇのポリマー溶液と３０分間混合する。２０分間
放置した後、実施例１に記載のその後の工程を行う。
この処方物についての人の死体の皮膚での経皮流量４の
研究結果を以下の表２にて記載する。
実施例５ ０．７ｇの１７β−エストラジオールを１５ｇのポリエ
チレングリコール４００．０．５ｇのポリソルベート６
０及び３ｇのプロピレングリコールに溶解する。この溶
液を２５ｇのポリマー溶液（モンサント社製のＧＥＬＶ
Ａ７３７）と２５分間混合する。その後の残りの工程は
実施例１に記載のものと同じである。
実施例６ ０．３ｇの１７β−エストラジオールを６ｇのポリエチ
レングリコール４００モノラウレート及び０．５ｇのＢ
ＲＩＪ９２に溶解する。この溶液を３０ｇのポリマー溶
液と３０分間混合する。２０分間放置した後、その後の
残りの工程は実施例１に記載のものと同じとする。

Claims (1)

  1. 【特許請求の範囲】 １）不透過性支持層（１２）と、この支持層（１２）に
    付着されていて、重量％基準で表わして、接着性ポリマ
    ー約６０〜約９５％、溶媒約５〜約２０％、皮膚浸透増
    進剤約０．２〜約４％及び活性物質約０．５〜約５％含
    有するマトリックス（１４）とを有し、そして区分され
    た透過性のポリマー製拡散制御膜を何ら含まない経皮投
    与用物品（１０）。 ２）活性物質がエストロゲン、エストロゲン誘導体及び
    それらの組合せよりなる群から選択される請求項１記載
    の物品。 ３）活性物質が１７β−エストラジオールである請求項
    １記載の物品。 ４）接着性ポリマーが酢酸ビニル−アクリル酸エステル
    マルチポリマーである請求項１記載の物品。 ５）溶媒がプロピレングリコール、プロピレングリコー
    ル誘導体及びそれらの組合せよりなる群から選択される
    請求項１記載の物品。 ６）溶媒が約２００、３００、４００、５００、６００
    及び８００の分子量を有するポリエチレングリコール；
    ポリエチレングリコールモノラウレート、ポリエチレン
    グリコールモノオレエート及びポリエチレングリコール
    モノラウレートのようなポリエチレングリコールエステ
    ル；及びこれらの組合せよりなる群から選択される請求
    項１記載の物品。 ７）皮膚浸透増進剤がポリオキシエチレンエステルであ
    る請求項１記載の物品。 ８）皮膚浸透増進剤がソルビタンモノ−９−オクタデセ
    ノエートポリ（オキシ−１，２−エタンジイル）及びそ
    の誘導体である請求項１記載の物品。 ９）さらに剥離層（１６）を有し、使用前にマトリック
    ス（１４）が支持層（１２）と剥離層（１６）との間に
    サンドイッチされていることを特徴とする請求項１記載
    の物品。 １０）不透過性支持層（１２）と、この支持層（１２）
    に付着されていて、重量％基準で表わして、酢酸ビニル
    −アクリル酸エステルマルチポリマーのような接着性ポ
    リマー約６０〜約９５％；プロピレングリコール、プロ
    ピレングリコール誘導体及びそれらの組合せよりなる群
    から選ばれる溶媒約５〜約２０％；ポリオキシエチレン
    エステルのような皮膚浸透増進剤約０．２〜約４％並び
    にエストロゲン、エストロゲン誘導体及びそれらの組合
    せよりなる群から選択される活性物質約０．５〜約５％
    を含有するマトリックス（１４）とを有し、そして区分
    された透過性のポリマー製拡散制御膜を何ら含まない経
    皮投与用物品（１０）。
