Description
(1) 動脈瘤を閉塞するためのデバイスであって、
畳み込み状態から展開状態へと移動可能なインプラントであって、近位端と、遠位端と、前記近位端と前記遠位端との間に実質的に連続的な編組構造を形成する編組区分と、を備える、インプラント
を備え、
前記展開状態では、前記インプラントは
前記インプラントの前記近位端から延在し、動脈瘤頸部を閉塞することが可能な外側閉塞袋と、
前記インプラントの前記遠位端を形成して延在し、前記外側閉塞袋内に溝を形成する内側閉塞袋と、
前記外側閉塞袋と前記内側閉塞袋との間に位置付けられた前記編組区分内の折り畳み部と、
を備える、デバイス。
(2) 前記編組区分の第1の部分は、自己拡張して前記外側閉塞袋を形成することができ、前記編組区分の第2の部分は、自己反転して前記内側閉塞袋を形成することができる、実施態様1に記載のデバイス。
(3) 前記展開状態では、前記外側閉塞袋は、動脈瘤壁まで延在して、前記動脈瘤壁に対して力を提供することができる、実施態様1に記載のデバイス。
(4) 前記展開状態では、動脈瘤壁に対する前記外側閉塞袋の対置は、前記動脈瘤内の前記インプラントの位置を維持するのに十分である、実施態様1に記載のデバイス。
(5) 塞栓充填剤を更に備え、
前記塞栓充填剤は、前記動脈瘤の嚢内に埋め込み可能であり、
前記展開状態では、前記インプラントは、前記塞栓充填剤が前記嚢を出ることを阻害し、
前記塞栓充填剤は、前記インプラントを動脈瘤壁に並置する力を提供する、
実施態様1に記載のデバイス。
(7) 前記インプラントの前記近位端又は前記インプラントの前記遠位端のうちの1つ以上は閉鎖されている、実施態様1に記載のデバイス。
(8) 前記遠位端又は前記近位端のうちの1つ以上は、バンド、端キャップ又はヒートセットのうちの1つによって閉鎖されている、実施態様7に記載のデバイス。
(9) 前記編組区分は、形状記憶材料を含み、前記編組区分は、第1の所定の形状及び第2の変形形状を含み、
前記編組区分は、前記インプラントが前記畳み込み状態にあるときに前記第2の変形形状にあり、
前記編組区分は、前記インプラントが前記展開状態にあるときに第3の展開形状へと移動し、前記第3の展開形状は、前記所定の形状及び動脈瘤壁の曲率に少なくとも部分的に基づく、
実施態様1に記載のデバイス。
(10) 前記展開状態では、前記外側閉塞袋は、前記動脈瘤頸部を封止して、前記動脈瘤内への流れの偏向、迂回及び減速のうちの少なくとも1つを行うことができる、実施態様1に記載のデバイス。
(12) 前記展開状態では、前記インプラントの前記遠位端及び前記近位端はそれぞれ、前記動脈瘤頸部によって画定された平面に対してほぼ垂直な軸に沿って位置付けられており、前記動脈瘤頸部の中央に近接する、実施態様1に記載のデバイス。
(13) 前記インプラントは、分岐した血管枝状部に隣接する動脈瘤に埋め込み可能であり、前記インプラントは、前記分岐した血管枝状部に供給する幹枝状部を通して前記動脈瘤に送達可能である、実施態様1に記載のデバイス。
(14) 編組メッシュを含む動脈瘤を治療するためのインプラントであって、
前記編組メッシュは、埋め込み構成へと移動可能であり、前記編組メッシュは、前記埋め込み構成において実質的に囲まれたボウル形状の容積を画定する実質的に連続した表面を備える、インプラント。
(15) 前記編組メッシュは、第1の端部及び第2の端部を有する実質的に管状の構成から前記埋め込み構成へと移動可能であり、
前記埋め込み構成では、前記第1の端部及び前記第2の端部はそれぞれ、前記ボウル形状の容積の中央に近接して位置付けられており、前記編組メッシュ内の折り畳み部は、前記ボウル形状の容積の環状隆起部を画定する、
実施態様14に記載のインプラント。
拡張可能な編組をマイクロカテーテル内に位置付けることであって、前記編組は、前記マイクロカテーテル内で畳み込み状態にあり、遠位端及び近位端を備える、ことと、
前記マイクロカテーテルを通して前記編組を前記動脈瘤に向かって遠位方向に摺動させることと、
前記編組の前記遠位端を含む前記編組の第1の部分を前記マイクロカテーテルから前記動脈瘤の嚢内に排出することと、
前記編組の前記第1の部分を拡張することと、
前記編組の前記近位端を含む前記編組の第2の部分を前記マイクロカテーテルから前記動脈瘤の前記嚢内に排出することと、
前記編組の前記第2の部分を拡張させて前記動脈瘤の頸部を閉塞し、外側閉塞袋を形成することであって、前記編組の前記近位端は、前記動脈瘤の前記頸部の中央に近接して位置付けられ、前記編組の前記第2の部分は、前記近位端から半径方向に延在して前記外側閉塞袋を形成する、ことと、
前記編組の前記第1の部分を反転させて、前記外側閉塞袋内に入れ子になった内側閉塞袋を形成することと、
を含む、方法。
(17) 塞栓インプラントを送達するための第2のカテーテルの遠位端を、前記遠位端が前記動脈瘤嚢内に位置付けられるように位置付けること
を更に含み、
前記編組の前記第2の部分の前記拡張工程は、前記編組の前記第2の部分と前記動脈瘤の壁の第1の部分との間に前記第2のカテーテルを閉じ込めることを更に含む、
実施態様16に記載の方法。
(18) 前記塞栓インプラントを前記第2のカテーテルを通して前記動脈瘤に送達することと、
前記塞栓インプラントを前記動脈瘤嚢内に埋め込むことと、
前記編組の少なくとも一部分を前記動脈瘤の前記壁の第2の部分に並置するために、前記塞栓インプラントから力を提供することと、
を更に含む、実施態様17に記載の方法。
(19) 前記編組は、自己拡張性である、実施態様16に記載の方法。
(20) 前記動脈瘤内の前記編組の埋め込み位置を維持するのに十分な力を、前記外側閉塞袋と動脈瘤壁との間に提供することを更に含む、実施態様16に記載の方法。
Claims (11)
- 動脈瘤を閉塞するためのシステムであって、
畳み込み状態から展開状態へと移動可能なインプラントと、前記インプラントを前記動脈瘤内に送達ためのインプラント送達カテーテルと、塞栓充填剤と、前記塞栓充填剤を前記動脈瘤内に送達ための塞栓充填剤送達カテーテルと、を備え、
前記インプラントは、近位端と、遠位端と、前記近位端と前記遠位端との間に実質的に連続的な編組構造を形成する編組区分と、を備え、
前記展開状態では、前記インプラントは
前記インプラントの前記近位端から延在し、動脈瘤頸部を閉塞することが可能な外側閉塞袋と、
前記インプラントの前記遠位端で収束する溝を形成する内側閉塞袋と、
前記外側閉塞袋と前記内側閉塞袋との間に位置付けられた前記編組区分内の折り畳み部と、
を備え、
前記インプラント送達カテーテルの遠位端開口が、前記動脈瘤頸部の開口部付近に位置付けられており、前記塞栓充填剤送達カテーテルの遠位端開口が、前記動脈瘤頸部の前記開口部よりも前記動脈瘤内に進入した状態で前記動脈瘤内に位置付けられている際に、前記インプラント送達カテーテルの前記遠位端開口から前記インプラントが排出されて、前記インプラントが前記展開状態となって、前記インプラントの前記外側閉塞袋が、前記動脈瘤頸部の前記開口部を閉塞しつつ、前記塞栓充填剤送達カテーテルにおける前記遠位端開口よりも近位側の部分を動脈瘤壁に押し付け、この状態で、前記塞栓充填剤が、前記塞栓充填剤送達カテーテルの前記遠位端開口から排出されて、前記動脈瘤内に進入し、前記インプラントの前記溝内に入り、前記インプラントの前記内側閉塞袋に当接し、前記インプラントを前記動脈瘤壁に押し付けることができるようになっている、システム。 - 前記編組区分の第1の部分は、自己拡張して前記外側閉塞袋を形成することができ、前記編組区分の第2の部分は、自己反転して前記内側閉塞袋を形成することができる、請求項1に記載のシステム。
- 前記展開状態では、前記外側閉塞袋は、前記動脈瘤壁まで延在して、前記動脈瘤壁に対して力を提供することができる、請求項1に記載のシステム。
- 前記展開状態では、前記動脈瘤壁に対する前記外側閉塞袋の対置は、前記動脈瘤内の前記インプラントの位置を維持するのに十分である、請求項1に記載のシステム。
- 前記インプラントの前記近位端又は前記インプラントの前記遠位端のうちの1つ以上は閉鎖されている、請求項1に記載のシステム。
- 前記遠位端又は前記近位端のうちの1つ以上は、バンド、端キャップ又はヒートセットのうちの1つによって閉鎖されている、請求項5に記載のシステム。
- 前記編組区分は、形状記憶材料を含み、前記編組区分は、第1の所定の形状及び第2の変形形状を含み、
前記編組区分は、前記インプラントが前記畳み込み状態にあるときに前記第2の変形形状にあり、
前記編組区分は、前記インプラントが前記展開状態にあるときに第3の展開形状へと移動し、前記第3の展開形状は、前記第1の所定の形状及び前記動脈瘤壁の曲率に少なくとも部分的に基づく、
請求項1に記載のシステム。 - 前記展開状態では、前記外側閉塞袋は、前記動脈瘤頸部を封止して、前記動脈瘤内への流れの偏向、迂回及び減速のうちの少なくとも1つを行うことができる、請求項1に記載のシステム。
- 前記展開状態では、前記インプラントは、実質的に囲まれた容積を画定する、請求項1に記載のシステム。
- 前記展開状態では、前記インプラントの前記遠位端及び前記近位端はそれぞれ、前記動脈瘤頸部によって画定された平面に対してほぼ垂直な軸に沿って位置付けられており、前記動脈瘤頸部の中央に近接する、請求項1に記載のシステム。
- 前記インプラントは、分岐した血管枝状部に隣接する動脈瘤に埋め込み可能であり、前記インプラントは、前記分岐した血管枝状部に供給する幹枝状部を通して前記動脈瘤に送達可能である、請求項1に記載のシステム。
