一部の実施形態において、ハイブリダイゼーションは、本明細書に開示の化合物と、PCSK9核酸との間で生じる。ハイブリダイゼーションの最も一般的な機序は、核酸分子の相補的核酸塩基間の水素結合(例えば、ワトソン・クリック、フーグスティーンまたは逆フーグスティーン型水素結合)を含む。

本明細書に記載の化合物は、医薬組成物または配合物の調製のための薬学的に許容可能な活性または不活性物質と混合することができる。組成物および医薬組成物を配合する方法は、多数の基準、例として、限定されるものではないが、投与経路または疾患の程度に依存的である。
種々の長さ、化学構造、およびモチーフの約1540個の新たに設計された化合物および少数の既に開示されている化合物を、ヒトPCSK9 mRNAに対するそれらの効果についてインビトロでいくつかの細胞タイプにおいて試験した(実施例1~2)。インビトロでの単一用量における効力について試験した1540個の化合物のうち、183個の選択化合物を用量依存的阻害についてHepG2細胞中で試験した(実施例2)。用量応答アッセイにより試験した183個の化合物のうち、134個のオリゴヌクレオチドをげっ歯類における単一用量インビボ忍容性のために選択した。これらの134個のオリゴヌクレオチドについて、その3’末端にTHA-C6-GalNAc3-(3R,5S)-5-(ヒドロキシメチル)ピロリジン-3-オールホスフェートエンドキャップ(以下、3’-THAと称される)をコンジュゲートさせた。
本出願に添付される配列表は、必要に応じてそれぞれの配列を「RNA」または「DNA」のいずれかと特定するが、実際には、それらの配列は、化学修飾の任意の組合せにより修飾されていてよい。当業者は、修飾オリゴヌクレオチドを説明するための「RNA」または「DNA」のそのような指定が、特定の例において、任意であることを容易に認識する。例えば、2’-OH糖部分およびチミン塩基を含むヌクレオシドを含むオリゴヌクレオチドは、修飾糖(DNAの天然2’-Hについては、2’-OH)を有するDNAまたは修飾塩基(RNAの天然ウラシルについては、チミン(メチル化ウラシル))を有するRNAと記載することができる。本明細書に提供されるとおり、mRNA阻害アッセイについての「0」の指定は、アンチセンスオリゴヌクレオチドがmRNA発現レベルを阻害しなかったことを示すにすぎない。
PCSK9核酸を標的化するアンチセンスオリゴヌクレオチドを設計し、PCSK9 mRNAに対するそのインビトロ効果について試験した。以下の表中のキメラアンチセンスオリゴヌクレオチドを3-10-3cEtギャップマーと指定した。ギャップマーは、16個のヌクレオシド長であり、中央ギャップセグメントは、10個の2’-デオキシヌクレオシドを含み、それぞれ3個のヌクレオシドの5’末端および3’末端上のウイングセグメントによりフランキングされている。5’ウイングセグメント中のそれぞれのヌクレオシドおよび3’ウイングセグメント中のそれぞれのヌクレオシドは、cEt糖修飾を有する。それぞれのギャップマー全体にわたるヌクレオシド間結合は、ホスホロチオエート(P=S)結合である。それぞれのギャップマー全体にわたる全てのシトシン残基は、5-メチルシトシンである。
1ウェル当たり20,000個の細胞の密度において培養したHepG2細胞に、エレクトロポレーションを使用して1,000nMのアンチセンスオリゴヌクレオチドを形質移入した。国際公開第2014179620号パンフレットに既に開示されているISIS431131もベンチマークオリゴヌクレオチドとして本試験に含めた。約24時間の処理期間後、RNAを細胞から単離し、PCSK9 mRNAレベルを定量的リアルタイムPCRにより計測した。ABI(ID# Hs03037355_m1)からのヒトPCSK9プライマープローブセットを使用してmRNAレベルを計測した。PCSK9 mRNAレベルは、RIBOGREEN(登録商標)により計測された総RNA含有量に従って調整した。結果を非処理対照細胞に対するPCSK9の阻害パーセントとして提示する。新たに設計したオリゴヌクレオチドのいくつかは、既に開示されているオリゴヌクレオチドISIS431131よりも強力であった。

PCSK9 mRNAのインビトロ阻害を示す実施例1から選択したギャップマーをHepG2細胞中で種々の用量において試験した。アンチセンスオリゴヌクレオチドは、同様の培養条件を有する一連の実験において試験した。それぞれの実験についての結果を下記の別個の表に提示する。
細胞を1ウェル当たり20,000個の細胞の密度においてプレーティングし、細胞にエレクトロポレーションを使用して以下の表中に規定される46.88nM、187.5nM、750nM、および3,000nMの濃度のアンチセンスオリゴヌクレオチドを形質移入した。国際公開第2008066776号パンフレットに既に開示されているISIS405879もベンチマークオリゴヌクレオチドとして本試験に含めた。約16時間の処理期間後、RNAを細胞から単離し、PCSK9 mRNAレベルを定量的リアルタイムPCRにより計測した。ABI(ID# Hs03037355_m1)からのヒトPCSK9プライマープローブセットを使用してmRNAレベルを計測した。PCSK9 mRNAレベルは、RIBOGREEN(登録商標)により計測された総RNA含有量に従って調整した。結果を非処理対照細胞に対するPCSK9の阻害パーセントとして提示する。

上記試験から選択したISISオリゴヌクレオチドを3’-THA-C6-GalNAc3-(3R,5S)-5-(ヒドロキシメチル)ピロリジン-3-オールホスフェートエンドキャップ(以下、3’-THAと称される)とコンジュゲートさせた。血漿化学マーカーのレベルの変化について評価した162個の3’-THA-コンジュゲートISISアンチセンスオリゴヌクレオチドを以下の表に提示する。「親オリゴ」は、上記試験に記載し、3’-THAとコンジュゲートさせ、本試験で試験したISISオリゴヌクレオチドを示す。

PCSK9トランスジェニックマウスを12時間の明/暗サイクルで維持し、普通Purinaマウス通常食を自由給餌した。動物を実験開始前に研究施設中で少なくとも7日間馴化させた。アンチセンスオリゴヌクレオチド(ASO)を緩衝生理食塩水(PBS)中で調製し、0.2ミクロンフィルターに通して濾過することにより滅菌した。注射のためにオリゴヌクレオチドを0.9%のPBS中で溶解させた。

上記試験から選択したISISオリゴヌクレオチドを5’-トリスヘキシルアミノ-(THA)-C6GalNAC3エンドキャップ(以下、5’-THAと称される)とコンジュゲートさせた。CD1(登録商標)マウス(Charles River,MA)は、安全性および効力試験に高頻度で利用される多目的マウスモデルである。マウスを、上記試験から選択し、5’-THAとコンジュゲートさせたISISアンチセンスオリゴヌクレオチドにより処理し、種々の血漿化学マーカーのレベルの変化について評価した。

雄Sprague-Dawleyラットを12時間の明/暗サイクルで維持し、Purina普通ラット通常食5001を自由給餌した。4つのSprague-Dawleyラットの群それぞれに15mg/kgのISISオリゴヌクレオチドを6週間にわたり週1回皮下注射した。最後の投与の48時間後にラットを屠殺し、さらなる分析のために臓器および血漿を回収した。
肝機能に対するISISオリゴヌクレオチドの効果を評価するため、自動臨床化学分析装置(Hitachi Olympus AU400e,Melville,NY)を使用してトランスアミナーゼの血漿レベルを計測した。ALT(アラニントランスアミナーゼ)およびAST(アスパラギン酸トランスアミナーゼ)の血漿レベルを計測し、結果を以下の表に提示し、IU/Lで表現する。同一の臨床化学分析装置を使用してビリルビン(TBIL)、BUN、アルブミン(ALB)、およびクレアチニン(CRE)の血漿レベルも計測し、その結果も以下の表に提示し、mg/dLで表現する。同一の臨床化学分析装置を使用してアルブミンの血漿レベルも計測し、その結果も以下の表に提示し、g/dLで表現する。
腎機能に対するISISオリゴヌクレオチドの効果を評価するため、自動臨床化学分析装置(Hitachi Olympus AU400e,Melville,NY)を使用して血中クレアチニンおよび総タンパク質の尿レベルを計測した。結果を以下の表に提示し、mg/dLで表現する。アンチセンスオリゴヌクレオチドについての予測範囲外の腎機能マーカーのいずれかのレベルの変化を引き起こしたISISオリゴヌクレオチドは、さらなる試験において排除した。「N/A」は、データがその群に利用可能でないことを示す。

26日目、PCSK9のmRNA発現の計測のリアルタイムPCR分析のためにRNAを肝臓から抽出した。結果を、RIBOGREEN(登録商標)により正規化されたPBS対照に対するmRNAの変化パーセントとして提示する。以下の表に示すとおり、ISISアンチセンスオリゴヌクレオチドによる処理は、PBS対照と比較してPCSK9 mRNAの有意な低減をもたらした。
ヒト特異的ELISAキット(R&D Systems)によりPCSK9血漿タンパク質を計測した。結果をPBS対照に対するタンパク質レベルの変化パーセントとして提示する。以下の表に示すとおり、ISISアンチセンスオリゴヌクレオチドによる処理は、PBS対照と比較してPCSK9血漿タンパク質レベルの有意な低減をもたらした。
血漿中のLDL-コレステロールのレベルを自動臨床化学分析装置(Hitachi Olympus AU400e,Melville,NY)により計測した。肝臓中のLDL受容体(LDLr)タンパク質のレベルも計測した。結果をPBS対照に対するレベルの変化パーセントとして提示する。以下の表に示すとおり、いくつかのISISアンチセンスオリゴヌクレオチドによる処理は、PBS対照と比較して血漿中のLDL-コレステロールのレベルの有意な低減をもたらした。したがって、ISISアンチセンスオリゴヌクレオチドによる処理は、マウスにおける用量依存的な肝臓LDLrタンパク質増加をもたらしたことが観察された。
40センチポアズ(cP)超の粘度を有するアンチセンスオリゴヌクレオチドを選別除外する目的で、上記試験からの選択アンチセンスオリゴヌクレオチドの粘度を計測した。40cP超の粘度を有するオリゴヌクレオチドは、最適未満の粘度を有する。
カニクイザルを、上記実施例に記載の試験から選択したISISアンチセンスオリゴヌクレオチドにより処理した。アンチセンスオリゴヌクレオチドの効力および忍容性、ならびに肝臓および腎臓におけるそれらの薬物動態プロファイルを評価した。
試験前、サルを隔離飼育し、その間、動物を全身健康状態について毎日観察した。サルは、2~4年齢であり、2~4kgの体重であった。5つのランダムに割り当てられた雄カニクイザルの9つの群それぞれに対し、ISISオリゴヌクレオチドまたはPBSを、サル背中上の4つの異なる部位(すなわち左側、右側、前方、および後方)間に時計回りでそれぞれの投与日に皮下注射し、1回の投与で1つの部位とした。サルに最初の2週間にわたり週2回(1、5、9、および14日目)、次いで次の10週間にわたり週1回(21、28、35、42、49、56、63、70、77、および84日目)、10mg/kgのISISオリゴヌクレオチドを投与した。5匹のカニクイザルの対照群に同様の様式でPBSを注射し、それは、対照群として機能した。
RNA分析
86日目、PCSK9のmRNA発現の計測のリアルタイムPCR分析のためにRNAを肝臓から抽出した。結果を、RIBOGREEN(登録商標)により正規化されたPBS対照に対するmRNAの変化パーセントとして提示する。以下の表に示すとおり、アカゲザル遺伝子とミスマッチを有さないISISアンチセンスオリゴヌクレオチドのISIS863633、ISIS863670、およびISIS863681による処理は、PBS対照と比較してPCSK9 mRNAの有意な低減をもたらした。
-7日目(処理前)、16、30、58、および86日目(それぞれ14、28、56、および84日目の投与の約48時間後)、約1mLの血液を全ての利用可能な動物から回収し、EDTAのカリウム塩を含有するチューブ中に装入した。チューブを遠心分離(4℃における10分間にわたる3000rpm)して血漿を得た。結果を-7日目のレベル(処理前レベル)に対する増加または減少割合として以下の表に提示する。以下の表に示すとおり、アカゲザル遺伝子とミスマッチを有さないISISアンチセンスオリゴヌクレオチドのISIS863633、ISIS863670、およびISIS863681による処理は、PBS対照と比較してPCSK9タンパク質レベルの有意な低減をもたらした。
総コレステロール、LDL-コレステロール、HDL-コレステロール、およびトリグリセリドレベルに対するISISオリゴヌクレオチドの効果を評価するため、サルを採血前に一晩絶食させた。約1.7mLの血液試料を全ての試験群から回収した。血清分離のために抗凝固剤を有さないチューブ中に血液を回収した。チューブを室温において最低90分間保持し、次いで3,000rpmにおいて室温において10分間遠心分離して血清を得た。Toshiba 200FR NEO化学分析装置(株式会社東芝、日本)を使用してレベルを計測した。データは、アカゲザルゲノム配列との相同性を有する3個のアンチセンスオリゴヌクレオチドによる処理が血漿総コレステロールおよびLDL-コレステロールの有意な低減をもたらしたことを実証する。予測のとおり、ヒトPCSK9レベルの低減は血漿HDL-コレステロールレベルにもトリグリセリドレベルにも影響しなかった。
臨床化学パラメータ

腎機能に対するISISオリゴヌクレオチドの効果を評価するため、サルを採血前に一晩絶食させた。約1.7mLの血液試料を全ての試験群から回収した。血清分離のために抗凝固剤を有さないチューブ中に血液を回収した。チューブを室温において最低90分間保持し、次いで3,000rpmにおいて室温において10分間遠心分離して血清を得た。Toshiba 200FR NEO化学分析装置(株式会社東芝、日本)を使用してBUNおよびクレアチニンのレベルを計測した。結果を以下の表にmg/dLで提示する。
血液パラメータに対するカニクイザルにおけるISISオリゴヌクレオチドの任意の効果を評価するため、約0.5mLの血液を利用可能な試験動物のそれぞれから、K2-EDTAを含有するチューブ中に回収した。ADVIA120血液分析装置(Bayer,USA)を使用して試料を赤血球(RBC)数、白血球(WBC)数、個々の白血球数、例えば単球、好中球、リンパ球のもの、ならびに血小板数、ヘモグロビン含有量およびヘマトクリットについて分析した。データを以下の表に提示する。
サル試験において試験したアンチセンスオリゴヌクレオチドの非コンジュゲート親オリゴヌクレオチドを、既に設計された化合物と比較した。ISIS848542、ISIS848593、ISIS848597、ISIS848598、ISIS848630、ISIS848833、ISIS849040、ISIS849171、およびISIS849236を、インビトロで最も強力なアンチセンス化合物の一部であることが既に決定されているISIS405879およびISIS405995(例えば、米国特許第8,084,437号明細書を参照されたい)、ならびに米国特許第9,127,276号明細書に既に記載されているISIS431131およびISIS480604による効力について試験した。
Claims (6)
- 修飾オリゴヌクレオチドおよびコンジュゲート基を含む化合物またはその薬学的に許容可能な塩であって、修飾オリゴヌクレオチドが16個の結合ヌクレオシド長であり、かつ配列番号1016の配列からなり、修飾オリゴヌクレオチドが、
10個の結合デオキシヌクレオシドからなるギャップセグメント、
3個の結合ヌクレオシドからなる5’ウイングセグメント、および
3個の結合ヌクレオシドからなる3’ウイングセグメント
を有し、
ギャップセグメントが5’ウイングセグメントと3’ウイングセグメントとの間に位置し、それぞれのウイングセグメントのそれぞれのヌクレオシドがcEt糖を含み、それぞれのヌクレオシド間結合がホスホロチオエート結合であり、それぞれのシトシンが5-メチルシトシンである、化合物またはその薬学的に許容可能な塩。 - 修飾オリゴヌクレオチドからなる化合物であって、修飾オリゴヌクレオチドが16個の結合ヌクレオシド長であり、かつ配列番号1016の配列からなり、修飾オリゴヌクレオチドが、
