Description

(1.分野)

本明細書に開示される治療モダリティの1以上の物理的特性及び/又は該治療モダリティを投与する方法を改善することは、多くの場合、有益であり、時に、必要不可欠である。物理的特性の改善には、例えば、免疫原性を調節すること;溶解度、バイオアベイラビリティ、血清半減期、及び/もしくは治療的半減期を増大させる方法;並びに/又は生物学的活性を調節することが含まれる。ある修飾は、例えば、検出アッセイ(例えば、エピトープタグ)で使用するための及びタンパク質精製の簡便性を提供するための抗体を作製するのに有用でもあり得る。そのような改善は、通常、治療モダリティの生物活性に悪影響を及ぼすことなく及び/又はその免疫原性を増大させることなくもたらされなければならない。
タンパク質治療剤の臨床的有効性は、短い血漿半減期及びプロテアーゼ分解に対する感受性によって限定されることが多い。様々な治療的タンパク質(例えば、フィルグラスチム)の研究により、そのような困難が、例えば、タンパク質を種々の非タンパク質性ポリマー、例えば、ポリエチレングリコール(PEG)、ポリプロピレングリコール、又はポリオキシアルキレンのいずれかにコンジュゲートし又は連結させることにより克服され得ることが示されている。これは、多くの場合、タンパク質と非タンパク質性ポリマー、例えば、PEGの両方に共有結合した連結部分によって達成される。そのようなPEGコンジュゲート生体分子は、より優れた物理的及び熱的安定性、酵素的分解への感受性に対する保護、溶解度の増大、より長いインビボ循環半減期及びクリアランスの減少、免疫原性及び抗原性の低下、並びに毒性の低下を含む、臨床的に有用な特性を保持することが示されている。薬物動態パラメータに対するペグ化の有益な効果に加えて、ペグ化それ自体が活性を増強し得る。
本明細書で使用される場合、「グリコシル化」は、グリカンが、タンパク質、脂質、又は他の有機分子に結合させられる酵素的プロセスを広範に指すことが意図される。本明細書における「グリコシル化」という用語の使用は、通常、1以上の炭水化物部分を(基礎となるグリコシル化部位を除去することによるか、又は化学的及び/もしくは酵素的手段によってグリコシル化を欠失させることによるかのいずれかで)付加しもしくは欠失させ、並びに/或いはネイティブな配列に存在していても存在していなくてもよい1以上のグリコシル化部位を付加することを意味することが意図される。さらに、この語句は、存在する様々な炭水化物部分の性質及び比率の変化を伴うネイティブなタンパク質のグリコシル化の定性的変化を含む。
ある実施態様において、また本明細書に提供されるのは、ポリペプチドの安定性及びインビボ薬物動態を改善するための、天然の生分解性α-(2→8)結合型ポリシアル酸(「PSA」)へのポリペプチドのコンジュゲーションである、ポリシアル酸化の使用である。
直接的な融合に代わるものとして、いくつかのアルブミン結合戦略が開発されており、本明細書に記載される薬剤とともに使用することができる。例として、ある実施態様において、本明細書に提供されるのは、共役脂肪酸鎖(アシル化)並びにアルブミン結合ドメイン(ABD)ポリペプチド配列及び本明細書に記載されるポリペプチドのうちの1つ又は複数の配列を含む融合タンパク質によるアルブミン結合である。
コンジュゲーションのための追加の好適な成分及び分子としては、例えば、チログロブリン;破傷風トキソイド;ジフテリアトキソイド;ポリアミノ酸、例えば、ポリ(D-リジン:D-グルタミン酸);ロタウイルスのVP6ポリペプチド;インフルエンザウイルス血球凝集素、インフルエンザウイルス核タンパク質;キーホールリンペットヘモシアニン(KLH);並びにB型肝炎ウイルスコアタンパク質及び表面抗原;又は前述のものの任意の組合せが挙げられる。
ある実施態様において、本明細書に提供されるポリペプチド配列のアミノ又はカルボキシル終端を免疫グロブリンFc領域と融合させて、融合コンジュゲート(又は融合分子)を形成させる。具体的な実施態様において、該免疫グロブリンFc領域は、ヒトFc領域である。融合コンジュゲートは、バイオ医薬品の全身半減期を増大させることが示されており、したがって、バイオ医薬製品は、より頻度の低い投与を必要とし得る。ある実施態様において、該半減期は、免疫グロブリンFc領域に融合されていない同じポリペプチドと比較して増大している。

ある実施態様において、また本明細書に提供されるのは、1以上の特性を改善するための、現在公知の又は将来開発される他の修飾の使用である。例としては、その様々な態様が、例えば、米国特許出願第2007/0134197号及び第2006/0258607号に記載されているヘス化、並びにSUMOを融合タグとして含む融合分子(LifeSensors社; Malvern, PA)が挙げられる。
リンカー及びその使用は上に記載されている。本明細書に提供されるポリペプチド配列を修飾するために使用される前述の成分及び分子はいずれも、リンカーを介して任意にコンジュゲートすることができる。好適なリンカーには、「可撓性リンカー」が含まれ、これは、通常、修飾されたポリペプチド配列と連結された成分及び分子の間でのある程度の動きを許容するのに十分な長さのものである。リンカー分子は、通常、約6〜50原子長である。リンカー分子は、例えば、アリールアセチレン、2〜10モノマー単位を含むエチレングリコールオリゴマー、ジアミン、二酸、アミノ酸、又はこれらの組合せであることもできる。好適なリンカーは容易に選択することができ、任意の好適な長さのもの、例えば、1アミノ酸(例えば、Gly)、2、3、4、5、6、7、8、9、10、10〜20、20〜30、30〜50アミノ酸、又は50アミノ酸を上回るものであることができる。

本明細書で使用される場合、対象の状態との関連において使用されるときの「胆汁酸に関係した障害」、「胆汁酸に関係した又は関連した障害」などの語句は、例えば、胆汁酸又は1以上の胆汁酸の急性、一過性、又は慢性の異常レベルとして現れ得る、胆汁酸ホメオスタシスの崩壊を意味する。該状態は、胆汁酸の合成、代謝、又は吸収の阻害、低下、又は遅延によって生じることがあり、その結果、対象は、正常な対象では通常は見られない胆汁酸レベルを示す。
最も一般的な胆汁鬱滞性肝疾患である原発性胆汁性肝硬変(PBC)は、胆汁酸を肝臓から外へ輸送する胆管の自己免疫性破壊に主に起因する進行性肝疾患である。疾患が進行すると、胆汁酸の持続的な中毒性の蓄積が慢性の炎症及び線維症を特徴とする進行性の肝損傷を引き起こす。PBCの患者はHCCの増大したリスクを有するので、本明細書に記載されるFGF19ペプチド配列の変異体、FGF19及び/又はFGF21ペプチド配列の融合体、並びにFGF19及び/又はFGF21ペプチド配列の融合体(キメラ)の変異体による療法が特に重要であるが、それは、そのような配列が、HCC形成もしくはHCC腫瘍形成を誘導しない、又は実質的に増大させないためである。
非アルコール性脂肪肝疾患(NAFLD)スペクトラムの一部と考えられる非アルコール性脂肪肝炎(NASH)は、肝臓における炎症並びに脂肪及び線維性組織の蓄積を引き起こす。NASHの正確な原因は不明であるが、リスク因子としては、中心性肥満、2型糖尿病、インスリン抵抗性(IR)、及び脂質異常症が挙げれられ;前記のものの組合せは、メタボリックシンドロームと記述されることが多い。さらに、タモキシフェン、アミオダロン、並びにステロイド(例えば、プレドニゾン及びヒドロコルチゾン)を含む特定の薬物がNASHと関連付けられている。非アルコール性脂肪肝疾患は、米国における慢性肝疾患の最も一般的な原因であり、NAFLDの推定有病率は20〜30%であり、NASHについては、有病率は3.5〜5%と推定される。(例えば、Abrams, G.A.,らの文献、Hepatology, 2004. 40(2):475-83; Moreira, R.K.の文献、Arch Pathol Lab Med, 2007. 131(11):1728-34を参照)。
また本明細書に提供されるのは、PBC以外の他の胆汁酸に関係した障害、並びに関連する疾患、障害、及び状態の治療及び/又は予防と関連する1以上の活性を有するFGF19ペプチド配列の変異体、FGF19及び/又はFGF21ペプチド配列の融合体、並びにFGF19及び/又はFGF21ペプチド配列の融合体(キメラ)の変異体の使用である。ある実施態様において、該ペプチドは、他の治療剤及び/又は治療モダリティと組み合わせて使用される。
また本明細書に提供されるのは、インビトロ、エクスビボ、及びインビボ(例えば、対象の表面又は中)での方法及び使用である。そのような方法及び使用は、本明細書に記載されるペプチド配列のいずれかを用いて実施することができる。様々な実施態様において、該方法は、本明細書に(例えば、配列表もしくは表1〜11に)開示されるFGF19もしくはFGF21変異体、融合体、もしくはキメラ、又は本明細書(例えば、配列表もしくは表1〜11)に開示されるFGF19もしくはFGF21変異体、融合体、もしくはキメラの部分配列、変異体、もしくは修飾形態などのペプチド配列を代謝障害又は関連障害を治療するのに有効な量で対象に投与することを含む。
方法及び使用の特定の態様において、本明細書に提供されるペプチド配列又はキメラペプチド配列は、対象に、該対象におけるグルコース代謝を改善するのに有効な量で投与される。より特定の態様において、対象は、投与前に、100mg/dlを超える空腹時血漿グルコースレベルを有するか、又は6%を上回るヘモグロビンA1c(HbA1c)レベルを有する。
さらなる実施態様において、使用又は対象の治療方法は、グルコースレベルの低下、インスリン感受性の増大、インスリン抵抗性の低下、グルカゴンの低下、耐糖能又はグルコース代謝もしくはホメオスタシスの改善、膵臓機能の改善、又はトリグリセリド、コレステロール、IDL、LDL、もしくはVLDLレベルの低下、又は血圧の減少、血管の内膜肥厚の減少、又はボディマスもしくは体重の増加の減少を意図しているか、或いはこれらをもたらす。
また提供されるのは、配列表に(並びにPCT公開WO 2013/006486号及び米国公開第2013/0023474号、並びにPCT公開WO 2014/085365号に)又は表1〜11に掲載されている配列の部分配列、配列変異体、及び修飾形態を含む、本明細書に提供されるペプチド配列をコードする核酸分子、並びに本明細書に記載される方法において使用されるペプチドをコードする核酸を含むベクターである。したがって、「核酸」には、本明細書に開示される例示されたペプチド配列をコードする核酸だけでなく、例示されたペプチド配列の機能的な部分配列、配列変異体、及び修飾形態をコードする核酸が、胆汁酸に関係した又は関連した障害(例えば、PBC)の治療又は予防において有用な少なくとも検出可能な又は測定可能な活性又は機能を保持する限り、前述のものも含まれる。

(5.1 実施例1)
以下は、本明細書における試験で使用される様々な方法及び材料の説明である。
db/dbマウスは、The Jackson Laboratory(Bar Harbor, ME)から購入した。マウスを管理光(12時間明及び12時間暗のサイクル、暗午後6:30〜午前6:30)、温度(22±4℃)、及び湿度(50%±20%)の条件下で、福祉ガイドラインに準拠して維持した。マウスは、水(オートクレーブ処理した蒸留水)を自由に摂取し、かつ17kcal%脂肪、23kcal%タンパク質、及び60kcal%炭水化物を含有する市販の飼料(Harlan Laboratories, Indianapolis, IN, Irradiated 2018 Teklad Global 18% Protein Rodent Diet)を自由に摂餌した。動物試験は全て、NGM施設内の動物の管理及び使用に関する委員会によって承認された。
ヒトFGF19(ホモ・サピエンス(Homo sapiens) FGF19、GenBankアクセッション番号NM_005117.2)変異体をコードするORFのcDNA。cDNA(GenBankアクセッション番号NP_005108.1)によってコードされるタンパク質配列。
FGF19 ORFを、ヒト小腸組織から調製した組換えDNA(cDNA)を用いて、ポリメラーゼ連鎖反応(PCR)で増幅した。Phusion(登録商標)高忠実度DNAポリメラーゼを含むPCR試薬キットは、New England BioLabs(F-530L, Ipswich, MA)から購入した。以下のプライマーを使用した:フォワードPCRプライマー:
初代ヒト肝細胞を、100nMデキサメタゾン(Sigma)及び0.25mg/ml MatriGel(商標)(Becton Dickinson Biosciences)を補充したWilliams E培地(Invitrogen)中で、コラーゲンコートされたプレート(Becton Dickinson Biosciences)上にプレーティングした。細胞を、FGF19又は変異体で、37℃で6時間処理した。Cyp7a1発現を、定量的RT-PCR(TaqMan(登録商標) ABI PRISM 7700, Applied Biosystems)により、3連で評価し、GAPDH発現に対し標準化した。
9週齢の雄のdb/dbマウス(Jackson Laboratories)に、組換えタンパク質FGF19又はFGF21を、0.1mg/kg、1mg/kg、及び10mg/kgで腹腔内注射した。動物を、注射の5時間後に、安楽死させた。肝臓を摘出し、TRIzol(登録商標)試薬(Invitrogen)中でホモジナイズした。全RNAを抽出し、DNアーゼ(Ambion)で処理し、その後、定量的RT-PCR解析し、GAPDH発現に対して標準化した。
AAV293細胞(Agilent Technologies, Santa Clara, CAから入手)を、10%ウシ胎仔血清及び1×抗生物質-抗真菌溶液(Mediatech社、Manassas, VA)を補充したダルベッコ改変イーグル培地(DMEM、Mediatech社、Manassas, VA)中で培養した。細胞を、1日目に、50%密度で150mm細胞培養プレートにプレーティングし、2日目に、リン酸カルシウム沈降法を用いて、以下の3種のプラスミド(各々20μg/プレート)でトランスフェクトした: AAV導入遺伝子プラスミド、pHelper(商標)プラスミド(Agilent Technologies)、及びAAV2/9プラスミド(Gaoらの文献、J. Virol. 78:6381(2004))。トランスフェクションから48時間後、細胞をプレートから擦り取り、3000×gでの遠心分離によりペレット化し、20mM Tris pH 8.5、100mM NaCl、及び1mM MgCl2を含むバッファーに再懸濁させた。この懸濁液を、アルコールドライアイス浴中で凍結させ、その後、37℃の水浴中で解凍した。この凍結及び解凍サイクルを3回繰り返し; Benzonase(登録商標)(Sigma-aldrich, St. Louis, MO)を50ユニット/mlまで添加し;デオキシコール酸を0.25%の最終濃度まで添加した。37℃で30分間のインキュベーションの後、細胞残屑を、5000×gで20分間の遠心分離によりペレット化した。上清中のウイルス粒子を、不連続なヨージキサナール(iodixanal)(Sigma-aldrich, St. Louis, MO)勾配を用いて、以前に記載されているように精製した(Zolotukhin S.らの文献、(1999) Gene Ther. 6:973)。ウイルスストックを、Vivaspin(登録商標) 20(MWカットオフ100,000ダルトン、Sartorius Stedim Biotech, Aubagne, France)を用いて濃縮し、10%グリセロールを含むリン酸緩衝生理食塩水(PBS)に再懸濁させ、-80℃で貯蔵した。ウイルスゲノムコピー数を決定するために、2μlのウイルスストックを、50ユニット/ml Benzonase(登録商標)、50mM Tris-HCl pH 7.5、10mM MgCl2、及び10mM CaCl2を含む6μlの溶液中で、37℃で30分間インキュベートした。
肝臓標本をAAV注射後24週のdb/dbマウスから採取した。HCCスコアを、野生型FGF19が注射されたマウス由来のHCC結節の数で割った変異体が注射されたマウス由来の肝臓全体の表面のHCC結節の数として記録した。
マウス尾断片由来の全血(約50μl/マウス)をプレーンキャピラリーチューブ(BD Clay Adams SurePrep(商標)、Becton Dickenson and Co. Sparks, MD)中に収集した。このチューブをAutocrit(商標) Ultra 3(Becton Dickinson and Co. Sparks, MD)中で回転させることにより、血清及び血液細胞を分離した。血清中のFGF19、FGF21、及び変異体の暴露レベルを、製造業者の指示に従ってEIAキット(Biovendor)を用いて決定した。
固相ELISA(結合)アッセイ及びERKリン酸化アッセイは、精製された組換えタンパク質を用いて実施することができる。FGFR結合アッセイは、固相ELISAを用いて実施することができる。簡潔に説明すると、96ウェルプレートを、2μg/ml抗hFc抗体でコーティングすることができ、かつ1μg/ml FGFR1-hFc又はFGFR4-hFcとともにインキュベートすることができる。1μg/ml可溶性β-クロトー及び20μg/mlヘパリンの存在下でのFGF19変異体への結合を、ビオチン化された抗FGF19抗体(0.2μg/mL)と、その後のストレプトアビジン-HRPインキュベーション(100ng/mL)により検出することができる。FGFR4活性化アッセイについては、Hep3B細胞を、FGF19変異体で、37℃で10分間刺激することができ、その後すぐに溶解させ、Cis-Bio製の市販キットを用いて、ERKリン酸化についてアッセイすることができる。
FGF19変異体、例えば、本明細書に記載されているものが、cyp7a1発現を抑制することを確認するために、野生型FGF19によるcyp7a1発現の阻害を、様々な濃度を投与した後に決定した。FGF21の効果を同等の方法で評価した。
上記のアッセイを用いて、初代ヒト肝細胞におけるcyp7a1の抑制をいくつかのFGF19変異体について決定した。図3〜図5に示されているように、数種の変異体(例えば、M1、M2など)は、強力なcyp7a1抑制を示した。
以下は、脂質上昇活性及び腫瘍形成について解析された25種の追加の変異体ペプチドのデータのまとめである。このデータは、db/dbマウスにおけるHCC形成により決定される、脂質上昇と腫瘍形成の間の正の相関を明確に示している。
以下は、グルコース低下活性及び脂質上昇活性について解析された追加のFGF19変異体ペプチドのデータのまとめである。

この実施例は、ヒト患者におけるM70の投与が胆汁酸合成のマーカーである7a-ヒドロキシ-4-コレステン-3-オン(C4)の抑制をもたらすことを示している。
年齢18〜65歳の範囲で、正常体重(ボディマス指数、BMI 20〜35)を有する健康な成人を本試験に登録した。試験プロトコルは、オーストラリアヒト臨床試験倫理委員会(Human Research Ethics Committee in Australia)により承認され、書面によるインフォームドコンセントを各々の対象から得た。本試験への組入れのために、各々の対象は、スクリーニング時の病歴、身体検査、12誘導心電図(ECG)、臨床検査所見、及びバイタルサインから臨床的に重大な所見がないことにより決定される良好な健康状態でなければならなかった。何らかの重大な代謝、アレルギー、皮膚、肝臓、腎臓、血液、肺、心臓血管系、GI、神経、又は精神の障害の病歴又は臨床症状を有する対象は、登録から除外した。
本試験は、無作為化二重盲検プラセボ対照設計であった。対象の予備スクリーニングは、登録の7〜30日前に実施し、ベースライン評価は、処置前に実施した。各々の対象に、3mg/日の用量のM70の皮下注射を、単回ボーラス投与で、7日間、毎日投与した。血液試料を、留置カテーテルを通して、ヘパリン処理チューブに収集した。1日目及び7日目にM70又はプラセボの投与後4.5時間又は24時間で採取した血液試料を解析した。7a-ヒドロキシ-4-コレステン-3-オン(C4)の血清レベルを用いて、CYP7A1酵素活性(胆汁酸合成)をモニタリングした。これらを、以前に記載されているように(Galmanらの文献、(2003) J Lipid Res. 2003;44(4):859-66)、試料抽出と、それに続く、高圧液体クラマトグラフィー(HPLC)の後に、個々の血清試料から解析した。
図6に示されているデータは、1日目及び7日目に、投与後4.5時間と24時間の両方で、C4の血清レベルが、プラセボを投与された患者と比較して、患者で有意に抑制されたことを示している。
この実施例は、ラット筋芽細胞細胞株における、FGF19、M3、及びM70による、マウスFGFR4-β-クロトーシグナル伝達の活性化を示している。
ELKルシフェラーゼアッセイを、マウスFGFR4、b-クロトー、並びにELK1に融合された5×UASルシフェラーゼ及びGAL4-DNA-結合ドメイン(DBD)を含むレポーターコンストラクトで一過性にトランスフェクトされたL6細胞で実施した。このシステムにおいて、ルシフェラーゼ活性は、内在性のリン酸化された細胞外シグナル調節キナーゼ(ERK)によって調節される。細胞をリガンドととも6時間インキュベートし、その後、ルシフェラーゼ活性測定のために溶解させた。
L6細胞におけるFGFR4及びβ-クロトーの共発現は、M3、M70とFGF19の両方において、細胞内シグナル伝達経路の活性化を増強することが分かった(それぞれ、EC50=20、38、及び53pM)(表12及び図7を参照)。

この実施例は、M70が、腫瘍形成能がある2つの異なるモデルで示されるように、マウスでHCCを有利に引き起こさないように、KLB/FGFR4受容体複合体を介するシグナル伝達を選択的に活性化することを示している。
rasH2トランスジェニックモデルとして知られるFDA承認腫瘍形成促進モデル、及びdb/db動物モデル。
ヒト血液中のFGF19の正常レベルよりも約1,000倍大きい暴露で発現されたM70は、1年間の暴露の後、HCCを引き起こさなかった。対照的に、マウス実験で陽性対照として利用されたヒトFGF19はHCCを引き起こした。
この実施例は、健康な成人参加者におけるM70の安全性、忍容性、及び薬物動態を評価するための第1相無作為化二重盲検プラセボ対照単回漸増用量(SAD)試験及び複数回漸増用量(MAD)試験の結果を考察している。本試験の概要は、表13に示されている。

この盲検プラセボ対照第1相試験において、過体重又は肥満であるが、それ以外は健康な成人を無作為に割り付けて、M70又はプラセボを、毎日の皮下注射として、漸増用量で投与した。
図8に示されているように、血清C4濃度の急速かつ用量比例的な低下は、M70が、胆汁酸合成に対して、0.3mg、1mg、及び3mgの用量で統計的に有意な効果を有することを示した。血清C4濃度の約94%の平均低下は、投与前レベルと比較したとき、3mgでの6回目の投与の後に顕著であった。この急速なC4の低下は、CYP7a1を介する胆汁酸合成の阻害剤としてのM70の潜在的な生物学的活性を支持する。2つの外れ値データ点は、図8に示されていないが、統計解析に含まれた(プラセボ、7日目: 45.1ng/ml; 0.3mg NGM282、ベースライン時: 62.1ng/ml)。
この実施例は、NASHの治療に対するM70の役割を支持する前臨床試験の結果を考察している。
STAM(商標)として知られるNASHのマウスモデルを用いて、NASHの治療におけるM70の有益な効果を調べた。このモデルは、ヒトにおけるNASH病理と一致する脂肪症、小葉炎症、及び肝細胞膨化を特徴とする。M70が連続発現されたマウスは、総肝脂肪含有量の減少を反映する、総体重、肝臓重量、及び肝臓対体重比の統計的に有意な減少を有していた(対照と比べて、p<0.001)。
M70発現は、NAFLD活性スコア(NAS)の全ての成分において統計的に有意な改善を示し、下のチャートに示されるように、対照の5.33と比較して、1.5という総NASスコアをもたらした。NASは、脂肪肝疾患の活性を等級分けするために広く使用される組織学的特徴スコアリングシステムであり、総スコアは、脂肪症、小葉炎症、及び膨化のスコアの合計を表す。通常、5以上のスコアは、NASHの症状を示しているとみなされる。これらの結果を下にまとめる。

この実施例は、胆汁鬱滞性肝疾患及び他のBARDの治療におけるM70の役割を示している。
この実施例は、M70が前臨床研究において肝機能を改善したことを示している。
第一のモデルの胆管結紮(BDL)は、外科的方法を用いて、総胆管を分断し、肝臓からの胆汁の流れを妨げ、胆汁鬱滞の状態を誘導する。BDLを受け、M70を投与されたマウスは、BDL手術後、血清胆汁酸の統計的に有意な低下(p<0.001)並びに肝損傷の生化学的マーカー、例えば、アルカリホスファターゼ(ALP)、アルカリアミノトランスフェラーゼ(ALT)、アスパラギン酸アミノトランスフェラーゼ(AST)、及びγ-グルタミルトランスフェラーゼ(GGT)の改善を示した。
この実施例は、PBCの患者で28日間投与されるウルソデオキシコール酸(UDCA)と組み合わせたM70の安全性、忍容性、及び薬力学的活性を評価するための第2相無作為化二重盲検プラセボ対照並行群多施設試験を記載し、ヒト患者の治療におけるM70の役割を示している。
第2a相PBC試験は、対照と比較した、処置から28日後の、UDCAと組み合わせたM70の効果を調べるように設計した。適格対象を、対照又は高用量(3.0mg)もしくは低用量(0.3mg)のM70を含む、2つの治療群のうちの1つに無作為に割り付けた。

この実施例では、原発性硬化性胆管炎(PSC)の治療のためのM70の役割が論じられた。PSCは、胆汁鬱滞、並びにほとんどの場合、肝不全及び肝臓癌のリスクの増加を引き起こす胆管の進行性の炎症、線維症、及び閉塞を特徴とする、慢性胆汁鬱滞性肝疾患である。最終的に、胆汁鬱滞症状が現れることになるが、患者は、数年間、無症状かつ診断未確定のままであることがある。PSCと診断された後の平均余命中央値は、肝移植なしで12〜18年であり、肝移植を受けた患者の場合でも、PSCは、10年以内に、患者の30%〜50%で再発する。PSCは潰瘍性大腸炎を伴うことが多く、自己免疫性肝炎及び自己免疫性膵炎を含む、他の形態の自己免疫疾患と重複するようにも見える。患者集団は、米国及び欧州で50,000〜132,000名と推定され、男女で2:1の発生率であり、特に、北欧での発生率が高い。PSCの治療に対して承認された治療薬はないが、肝移植は末期PSCで最もよく見られる治療アプローチであり、これは、米国で5番目に多い肝移植の適応となっている。多くのPSC患者は、現在薬物治療が利用可能でないPBCの同じ掻痒症の症状に苦しんでいる。
PSC対象におけるM70の試験では、12週間の無作為化プラセボ対照二重盲検多施設試験において約60人の対象で該化合物の活性が探索される。該対象は、ALPの上昇及び胆管造影法又は肝生検によって評価したとき、PSC患者と確認され、肝硬変又は進行した肝疾患の所見がない。主要エンドポイントは、12週間の治療でのベースラインからのALPの変化である。試験は、12週間にわたって毎日投与した後の、PSCと関連する他の生化学的マーカー、例えば、ALT、GGT、及びビリルビン、血清胆汁酸、C4、掻痒症、並びに炎症性腸疾患症状のベースラインからの変化に対するM70の効果を調べるように設計される。
この実施例は、2型糖尿病の参加者に28日間投与されるM70の安全性、忍容性、及び活性を評価するための第2相無作為化二重盲検プラセボ対照並行群多施設試験を記載している。
2型糖尿病の対象でM70を評価するために、4週間の無作為化二重盲検多施設試験を実施した。両方の状態に対する肥満及びインスリン抵抗性の寄与の結果として、2型糖尿病患者及びNASH患者の罹患率は実質的に重複している。
