Description
本発明は、一般に、IL−5および過剰な好酸球産生によって媒介される症状の処置および診断のための方法、より詳しくは、抗IL−5抗体を用いた好酸球性気管支炎の処置に関する。
好酸球は、肺組織における過敏性反応に関連するアレルギー性障害を含む多様な炎症性病態の病因に関連づけられている(Butterfield et al., In: Immunopharmacology of Eosinophils, H. Smith and R. Cook, Eds., p.151-192, Academic Press, London(1993))。
顕著な例は喘息、すなわち、非特異的な気管支過敏症に至る気道の可逆的閉塞を特徴とする疾患である。そして、これは気管支粘膜のレベルでの慢性の炎症性反応およびマクロファージ、リンパ球および好酸球による特徴的な浸潤の発生に依存する。好酸球は、この疾患に典型的な粘膜損傷の開始に中枢的役割を果たすようである(Corrigan et al., Immunol. Today, 13:501-507(1992))。慢性喘息患者の循環、気管支分泌物および肺実質において活性化された好酸球の数の増加が報告され、様々な肺機能検査により測定される疾患の重篤度は血中の好酸球数と相関する(Griffen et al., J. Aller. Clin. 本発明の一実施態様において、好酸球性気管支炎を患う前記ヒトに少なくとも1種類の抗IL−5抗体を含んでなる組成物を投与することを含んでなる、好酸球性気管支炎の処置方法が提供される。
I.定義

1.例えば長時間作用型βアゴニスト、テオフィリンまたはロイコトリエンアンタゴニストなどの吸入コルチコステロイドに加え、対照投薬による毎日の処置が必要。
2.毎日、またはほぼ毎日の短時間作用型βアゴニストの使用が必要な喘息症状。
3.持続性気道閉塞(FEV1<80% 予測値;日内PEF変動>20%)4.喘息の救急受診が1年に1回以上
5.経口ステロイドの「バースト」が1年に3回以上
6.経口または吸入コルチコステロイド用量の25%以下の減量で急速に悪化
7.過去に致死に近い喘息事象
*他の状態は除外されていること、増悪因子が処置されていること、患者は一般に服薬順守することが必要。

もう1つの態様では、少なくとも1種類の抗IL−5抗体を含んでなる組成物は、約10分から約4時間以上の範囲、またはより具体的には、約20分から約60分間の範囲にわたって静脈内に投与されてよい。本発明のもう1つの態様では、少なくとも1種類の抗IL−5抗体を含んでなる組成物は皮下投与され、それは250mgの用量であってよい。
皮下用量は、ヒトに対して1〜3回またはそれ以上投与可能である。
実施例1
・外来患者として追跡調査されており、たびたび起こる誘導痰中好酸球増加症を伴う増悪を予防するために(高用量のステロイド吸入処置に加えて)最小用量のプレドニゾン処置が必要であることが分っている、年齢18〜70歳の成人患者。
・スクリーニングおよびベースライン調査の来診時に、痰中好酸球増加症および症状を示した患者。症状は、活動および睡眠に影響を与える程度でもよいが、処置する医師の意見では、憂慮するほど重度のものであってはならない。
・気管支拡張薬を適切に差し控えた後に、サルブタモール(200mg)、およびメタコリンPC20吸入前後のFEV1が測定されるが、これらが異常である必要はない。なぜならプレドニゾンは好酸球性気管支炎の管理およびその臨床的帰結のために必要であり、気管支炎は、喘息のこれらの特徴なしに起こりうるからである。
・同じ用量のコルチコステロイドを、少なくとも1ヶ月間服用している。
期間1:症候性好酸球性気管支炎(喘息ありまたは無し)プレドニゾンが同用量で6週間以上投与される。
期間2:臨床的および好酸球性増悪、または厄介なステロイド離脱の影響がみられるまで、4週間間隔で、標準化されたプレドニゾン減量(および吸入ステロイド(プレドニゾンが試験処置中に中断された場合))を行う。
期間3:ウォッシュアウト
20人の患者のうち、19人が試験を終了した。作用性薬物を投与された1人の被験体は、心不全のため、3回目の注入後に離脱したが、解析には含められた。従って、主要解析には20人の患者を含め、評価可能例のみの解析には、ベースライン評価時に気道好酸球増加症がみられた18人の被験体のみが含められた。
増悪
プラセボ群では、12例の喘息増悪がみられた。9例は痰中好酸球増加症を、3例は痰中好中球増加症を伴っており、そのうちの2例は、痰中好酸球増加症を伴う増悪のために最終的に処置が施された患者であった。このように、プラセボ群では11人の患者のうち10人にプレドニゾンまたは抗生物質による処置につながる増悪がみられた。対照的に、メポリズマブ治療群では2例の事象がみられた(1例の好中球の増悪、および1例の副作用による離脱)(p=0.008)。増悪は、7人の患者におけるFEV1およびさらなる判定基準における減少(1人は積極的処置群、および6人はプラセボ群)、β2−アゴニスト救済の増加(プラセボ群3人の患者)、ならびに夜間症状(プラセボ群1人の患者)により特定された。Likertスケールのみにおける変化、または医師の裁量を、患者における増悪の特定またはプレドニゾン用量の変更に用いることはなかった。増悪までの時間の中央値は(気管支炎のタイプに関係なく)、プラセボ群では12週間、およびメポリズマブ群では20週間であった(p=0.003)(図2)。
メポリズマブ群において、患者のプレドニゾンの平均(±標準偏差)減少量は、プロトコールに合致する最大可能減少量の83.8%±33.4%であったのに対し、プラセボ群では47.4%±40.5%(p<0.04)であった。絶対的には、プレドニゾンの平均用量は、メポリズマブ群では11.9mgから3.9mgに減少し、プラセボ群では10.7mgから6.4mgに減少した(2群における減少の中央値、10mgから5mg)(P=0.11)。
プラセボ群の10人の患者のうち、痰中好酸球増加症を伴う増悪がみられた8人のデータはその時に検閲したが、一方で、メポリズマブを投与された患者では、痰中好酸球増加症を伴う増悪はみられなかった(P=0.02)。プラセボ群において、痰中好中球増加症を伴う3例の増悪がみられた。メポリズマブ群では、定期的な来診の間に痰中好中球数の増加が5例発現したが(4人の患者において)、その発現は、増悪と定義される評価基準の変化を伴っていなかったため、処置されなかった。
事前設定二次転帰
メポリズマブの単回注入は、痰中および血液中の好酸球を正常範囲内へ減少させ、その値はプレドニゾン用量の減量後(p<0.05)、およびメポリズマブの最終注入後8週間続いた。対照的に、プラセボ治療群においては、プレドニゾン減量により痰中および血液中の好酸球は有意に増加した(表2B)。メポリズマブ処置は、表2Aに示されるように、FEV1、喘息症状および喘息管理質問票スコアを改善させた。
メポリズマブ群の1人の患者において、薬物注入3回後に進行性の息切れが起こり、試験から除外された。試験群の割付を知らない治験責任医師は、この患者は冠動脈疾患の既往症があると判断した;息切れは、虚血性心筋症による心不全に起因するものであった。
患者は、好酸球性心筋症を除外するための心内膜生検または心臓の磁気共鳴診断は受けなかった。プラセボ群の1人の患者は、全試験が完了した6ヶ月後に自宅にて急死した。剖検では、死因は、おそらく心室の頻脈性不整脈による突然の心停止と特定され、喘息の悪化が原因ではないとされた。メポリズマブ群の1人の患者では、プレドニゾンの用量が2.5mgに減量された時に、痛みと疲労感があると報告した。プラセボ群の1人の患者は、プレドニゾン12.5mg/日を5mg/日に減量する期間中に副腎機能低下症を示した(短期間コルチコトロピン刺激試験に対するコルチゾール反応の鈍麻により示される)。他に重篤な副作用はなかった。メポリズマブに起因する血液化学的検査値の有意な異常はなかった。
これらのデータは、経口のプレドニゾンおよびコルチコステロイド高用量吸入で処置された場合でさえ、痰中好酸球増加症が継続する、稀な喘息患者において、IL−5に対するヒト化モノクローナル抗体を用いる処置により、喘息の増悪の発生なしに、プレドニゾン用量の減量ができるようになることを示している。これらの結果は、類似抗体(Leckie, Qt al. Lancet 2000; 356: 2144-8; Kips, et al. Am J Respir Crit Care Med 2003; 167: 1655-9; and Flood-Page, et al. Am J Respir Crit Care Med 2007; 176: 1062-71)の、より一般的な型の喘息患者、および過好酸球増加症症候群(HES)と一致する患者(Klion, et al. Blood 2004; 103: 2939-2941; Garrett, et al. J Allergy Clin Immunol 2004; 113: 115-119; and Rothenberg, et al. N EnglJ Med 2008; 358: 1215-28)、ならびに痰中好酸球増加症を伴う喘息の2例の症例報告概要(Korn, et al. Am J Respir Crit Care Med 2007; 175: A486)、および喘息を伴うHES(Hargreave, et al. Clin Exp Allergy (abstract) 2004; 34: 1807)における以前の試験の否定的な結果とは対照的である。
メポリズマブに応答する患者は、コルチコステロイド処置にもかかわらず痰中の好酸球数がより多い患者であると考えられる。第二に、本試験では有意なプレドニゾン節約効果が示されたにもかかわらず、2つの処置群において、臨床的により重要な転帰である最終プレドニゾン用量に統計学的に有意な差がなかったことが挙げられる。第三に、本試験は、変わりやすい気流制限により示される、喘息の過去の客観的エビデンスに依存していることが挙げられる。大部分の患者は、過去において頻繁な増悪があり、長時間作用型気管支拡張薬の維持量を服用しているか、またはベースライン時に中等度の気流閉塞があり、本試験では、ベースライン時にすべての患者においてアルブテロールの回復度、もしくはメタコリン気道過敏性を再テストしなかった。第四に、試験した患者は喘息および持続性の痰中好酸球増加症患者のほんの一部に過ぎず、本発明者らの得た結果は喘息の大部分の患者にあてはまるものではないであろう。第五に、努力にもかかわらず、治験責任医師はある程度痰中細胞数を知っていたために、彼らが処置の割り付けに対して完全にブラインド化されていなかった可能性がある。第六に、本試験は非常に小規模なものであり、臨床的方向性を示すものとみなすことはできないことが挙げられる。
Claims (15)
- 少なくとも1種の中和型ヒト化抗ヒトIL−5抗体を含んでなる、ステロイドと併用投与して喘息を患う被験体における好酸球性気管支炎の処置に用いるための医薬組成物であって、ステロイドが、プレドニゾンであり、プレドニゾンの開始用量が5.0〜7.5mgであり、プレドニゾンの用量が処置後少なくとも約90%減量され、被験体が、プレドニゾンによる処置にもかかわらず好酸球性気管支炎を患う、医薬組成物。
- 前記少なくとも1種の中和型ヒト化抗ヒトIL−5抗体が、配列番号19を含んでなる重鎖を含んでなる、請求項1に記載の医薬組成物。
- 前記少なくとも1種の中和型ヒト化抗ヒトIL−5抗体が、配列番号21を含んでなる軽鎖を含んでなる、請求項1に記載の医薬組成物。
- 前記少なくとも1種の中和型ヒト化抗ヒトIL−5抗体が、配列番号19を含んでなる重鎖と配列番号21を含んでなる軽鎖とを含んでなる、請求項1に記載の医薬組成物。
- 好酸球性気管支炎を患う被験体が、アトピー性皮膚炎、アレルギー性鼻炎、非アレルギー性鼻炎、慢性好酸球性肺炎、アレルギー性気管支肺アスペルギルス症、セリアック病、チャーグ・シュトラウス症候群、好酸球性筋肉痛症候群、過好酸球性症候群、偶発性血管性浮腫を含む浮腫反応、蠕虫感染症、寄生虫感染および/または寄生虫感染の処置に関連する好酸球増加症、オンコセルカ皮膚炎、好酸球性食道炎、好酸球性胃炎、好酸球性胃腸炎、好酸球性腸炎、好酸球性大腸炎、鼻マイクロポリープ症、鼻ポリープ症、閉塞性睡眠時無呼吸、クローン病、硬皮症および心内膜心筋線維症からなる群から選択される過剰な好酸球産生に関連する少なくとも1つの付加的障害を患っている、請求項1に記載の医薬組成物。
- 静脈内に投与するための、請求項1に記載の医薬組成物。
- 750mgの用量で投与するための、請求項6に記載の医薬組成物。
- 前記静脈用量を、30分以上かけて静脈内に投与するための、請求項6に記載の医薬組成物。
- 少なくとも1回用量を受容した後、好酸球性気管支炎の増悪を統計学的に有意に軽減するための、請求項1に記載の医薬組成物。
- 少なくとも1回投与した後に好酸球レベルを正常範囲内まで減少させるための、請求項1に記載の医薬組成物。
- 最終投与後前記好酸球レベルを少なくとも8週間、正常範囲内にとどめるための、請求項10に記載の医薬組成物。
- 第1の中和型ヒト化抗ヒトIL−5抗体および第2の中和型ヒト化抗ヒトIL−5抗体を含んでなる、請求項1に記載の医薬組成物。
- 前記好酸球レベルを、血液および/または痰において減少させるための、請求項10に記載の医薬組成物。
- 少なくとも1種の中和型ヒト化抗ヒトIL−5抗体とプレドニゾンを含んでなる、喘息を患う被験体における好酸球性気管支炎の処置に用いるための組合せ医薬であって、プレドニゾンの開始用量が5.0〜7.5mgであり、プレドニゾンの用量が処置後少なくとも約90%減量され、被験体が、プレドニゾンによる処置にもかかわらず好酸球性気管支炎を患う、組合せ医薬。
- プレドニゾンの開始用量が5.0〜7.5mgであり、プレドニゾンの開始用量が処置後少なくとも約90%減量される、請求項14に記載の組合せ医薬。
