Description
毎年のインフルエンザウイルスの流行は、世界中での罹患率および死亡率の重要な原因を示す。アメリカ合衆国において、200,000人を越える人々が、毎年インフルエンザウイルスに関連する症候群のために入院し、約40,000人が大体それに直接関連して死ぬと概算されている(Thompson et al., JAMA, 2003, 289:179-186)。これらの数だけでなく、我々は、また、仕事日数の喪失による不可避の経済的な影響を有する、たいてい長期間にわたって家で滞在する感染した対象の指数関数的に多い数の場合を考慮すべきである。最近の研究では(Molinari et al., Vaccine, 2007, 25: 5086-5096)、流行に直接関連する医療費は毎年104億USドルであり、欠勤による収入の損失のため163億USドルが加えられるべきであると概算された。計算において、他の項目、例えば、感染された対象の死に関連する経済的損失の貨幣も考慮するとき、総額は信じられない額の871億USドルに達する。ヒトに対する新型インフルエンザウイルスの出現によって近い将来、今すぐにも起こり得る非常に恐ろしい大流行と共に、毎年の流行と関連するこれらの経済的なデータは、これらのウイルスの拡大の有効な戦略の探求において、考慮すべき関心事であることを説明する。
上記されているデータに基づいて、本発明の1つの対象物は、インフルエンザA型ウイルスの異なるサブタイプに対して反応性であるモノクローナル抗体、好ましくはヒトまたはヒト化抗体を提供するものである。
驚くべきことに、本発明者は上記の望ましい特徴を有するモノクローナル抗体の提供に成功した。
1.患者の選択
試験において登録した患者は、交差反応性抗インフルエンザ抗体、すなわち異なるサブタイプに属するインフルエンザウイルス分離株を認識し、潜在的に中和することができる抗体のクローニングの機会を増やすように選択された。特に、数人の個体は、(ときどき、職業上の理由で、医師、小児科医、幼稚園および学校で働く人として)インフルエンザウイルスに連続的に暴露されているにもかかわらず、疾患に罹患しないと述べられている。これらの珍しい個体は、未知の理由による有効なヘテロサブタイプ免疫の発達によって、インフルエンザウイルス感染を引き起こしにくいと考えた。この理由のため、これらの人々がヒトモノクローナル抗体の生産のためのもっとも良い候補者であると考えた。特に、以下の試験対象患者基準に従った:
−25から55歳;
−臨床的インフルエンザ症候群に対して試験の前10年間陰性である最近の病理学的病歴;
−試験の前5年間に、毎年の流行に関与しているウイルス分離株、サブタイプH1N1およびH3N2に対して1:1000より大きい抗体価;
−試験の前5年間に、毎年の流行に関与しているウイルス分離株、サブタイプH1N1およびH3N2に対して高い中和価(IC50>=1:400);
−2つの参照サブタイプA型ウイルス分離株(A/PR/8/34サブタイプH1N1;A/PC/1/73サブタイプH3N2)に対して検出できる中和価(IC50>=1:20);
−これまでに抗インフルエンザワクチン接種なし;
−抗インフルエンザワクチン接種を受けるコンプライアンス。
ワクチン接種時およびワクチン接種約3週間後に、約20mlの血液をそれぞれの患者からヘパリン化試験チューブで抜き取った。
10%の不活性化のウシ胎児血清(FBS)(56℃で30分処理)(EuroClone)、50μg/mlのペニシリン、100μg/mlのストレプトマイシン(GIBCO)および2mMのL−グルタミン(EuroClone)を補った改変イーグル培地(MEM)(GIBCO)中で培養させたMDCK(Madin−Darbyイヌ腎臓)細胞(ATCC(登録商標)番号CCL−34TM)を細胞系として使用した。細胞を37℃で5%CO2雰囲気下でインキュベートし、1:3の比率で1週間に2回通過させた。本特許出願に記載されている実験のために、以下のインフルエンザウイルス分離株を使用した:H1N1、株A/PR/8/34(ATCC(登録商標)番号VR−1469TM);H3N2、株A/PC/1/73(ATCC(登録商標)番号VR−810)および株B/Lee/40(ATCC(登録商標)番号VR−101)。ウイルスを培養させるための培養培地として、1μg/mlの無血清トリプシン(SIGMA)を補ったMEMを使用した。ウイルス貯蔵物を細胞外ウイルスとして培養上清から得た。手短に言えば、細胞を感染後、細胞変性効果の出現をモニタリングするために単層を毎日観察した。通常、感染から4日後、上清を回収し、10分間1000RCF(相対遠心力)で遠心し、細胞残屑を除去し、0.22μmフィルター(MILLIPORE)で濾過した。次に上清をアリコートし、無細胞ウイルスとして−80℃で保存した。
患者由来のモノクローナル抗体の生産を、Cole et al, 1984 Cancer Research 22:2750-2753により以前に述べられている、Bリンパ球のエプスタイン・バーウイルス(EBV)での感染による形質転換方法を使用することにより実施した。得られた異なるクローン由来の上清をELISAにより抗体の存在に関して評価した。次に、ELISAにおいて2つの参照分離株、サブタイプH1N1およびH3N2に感染した細胞溶解物に対して反応することができる、上清におけるIgG抗体を生産することができるクローンを、後の特徴付けのために選択した。特に、MDCK細胞を感染の高い多重度で前記分離株に感染させた。感染約48時間後、細胞をフラスコから分離し、PBSで2回洗浄した。次に細胞ペレットを300μlの溶菌溶液(100mMのNaCl、100mMのTris pH8および0.5%のTriton−X)に懸濁し、20分間氷上で保存した。細胞残屑を5分間10000gで遠心し、上清を−20℃でタンパク質抽出物として保存した。対照抗原の製造物として、非感染細胞を同じ方法で処理した。上清タンパク質濃度をBCATMのタンパク質アッセイキット(Pierce)を使用してデュプリケートで決定した。簡潔には、サンプルタンパク質の量を、ウシ血清アルブミン(BSA)の一連の既知の濃度希釈物により得られた標準曲線を参照することにより決定した。すべてのサンプルの吸光度を分光光度計で540nmの波長で測定した。次にそうして得られた溶解物を使用して(ウェルあたり300ng)、ELISAプレート(COSTAR)を被覆し、4℃で一晩インキュベートした。次の日、プレートを蒸留水で洗浄し、PBS/1%BSA(Sigma)で45分37℃でブロックした。次に、それぞれのクローン由来の40μlの上清をそれぞれのウェルに加え、これを1時間37℃でインキュベートした。PBS/0.5%のTween−20(Sigma)で5回洗浄後(WASHER ETI−SYSTEM、DiaSorin)、40μlのペルオキシダーゼ結合抗ヒトFc(PBS/1%BSA中で1:4000、Sigma)をそれぞれのウェルに加え、プレートを1時間37℃でインキュベートした。PBS/0.5%Tween−20で5回以上洗浄後、40μlのペルオキシダーゼ基質であるTMB(Pierce)をそれぞれのウェルに加えた。約15分後、酵素活性を40μlのH2SO4を加えることによりブロックし、シグナルを分光光度計で450nmで測定した。すなわちサブタイプH1N1に属する株に感染した細胞溶解物およびサブタイプH3N2に属する株に感染した細胞溶解物の両方を認識することができる、交差反応性抗体を生産することができる6つの推定のクローン(それぞれcINF4、cINF16、cINF28、cINF39、cINF43およびcINF47として表される)の上清に対して、特に注目した。
インフルエンザウイルスと反応することができる一価Fab鎖をコードする遺伝子を原核生物の発現ベクターにクローニングした。これは、抗体生産細胞クローンの不安定、分子的観点からコードする遺伝子をさらに特徴付けること、確実に自由に使えるモノクローナルである分子を有すること、ならびにそれぞれ個々の抗体の量の増加による問題を避けることができる。
クローニング完了後、それぞれのモノクローナル抗体に対する40の組換え細菌クローンを、熱ショックにより得られた細菌培養物由来の粗溶解物を使用するELISAにより分析した。特に、構築物PCb3/CAFで形質転換された細菌のクローンを、50μg/mlおよび10μg/mlでそれぞれアンピシリンおよびテトラサイクリンを含む10mlのSB培地に植菌し、O.D.600=1に達するまで37℃で撹拌下で培養した。次に、特定の誘導剤(IPTG−イソプロピルβ−D−チオガラクトピラノシド)を最終濃度1mMで加え、培養物を30℃で一晩撹拌しながら放置した。細胞を熱ショック(−80℃および37℃でそれぞれ冷凍/解凍を3回)により溶解し、次に遠心し、Fab含有上清由来の細胞残屑を分離した。得られた可溶性FabをELISAによりアッセイした。96ウェルマイクロタイタープレート(Nunc)を、上記参照ウイルス分離株に感染した細胞由来の溶解物で被覆した。感染していない細胞から得られた溶解物を陰性対照として使用した。次に300ngの記載されているとおりに得られた溶解物で被覆されたELISAプレートを4℃で一晩放置した。次の日、非結合抗原の除去後、プレートをPBSで5回洗浄し、非特異的結合部位をPBS中3%のアルブミンで1時間37℃でブロックした。ブロッキング溶液の除去後、可溶性Fabを含む、上記のとおり処理された細胞培養物の上清をそこに加え、次に37℃で2時間インキュベーションした。PBS/0.05%Tween 20で10回洗浄サイクル後、40μlのPBS/1%BSA中のラディッシュペルオキシダーゼ結合ヤギ抗ヒトFab免疫グロブリン(Sigma)のポリクローナル製剤の1:700希釈物をそれに加えた。37℃で1時間インキュベーションおよびさらなる一連の10回の洗浄後、基質(OPD−o−フェニレンジアミン)をウェルに加えた。次にプレートを30分室温で暗下でインキュベートした。反応を1Nの硫酸でクエンチし、450nmで分光光度を読むことにより光学濃度を評価した。アッセイされたすべてのクローンが、感染された細胞から得られた溶解物に対して反応性を示した。ヒト抗体の軽鎖および重鎖Fdフラグメントをコードする遺伝子対を含む発現ベクターで形質転換された1つの細菌クローンを、それぞれの交差反応性モノクローナルに対して選択した。このような細菌クローンは、分離株A/PR/8/34(H1N1)および分離株A/PC/1/73(H3N2)の両方に結合することができるヒトFabを生産することができる。これらのクローン(相対遺伝子対を有する)をINF4、INF16、INF28、INF39、INF43およびINF47と名付けた。
上記交差反応性クローンから生産されるFab(“クローン”または“Fab”としても簡単に表される)は記載されている発現ベクターで形質転換された細菌を介して生産され、次に免疫親和性を、ヒトFab(PIERCE、Illinois)に結合することができるヤギ抗体のポリクローナル製剤が共有結合する、プロテインG(〜2mg/ml)を含むセファロース樹脂で構成されるカラムで精製した。手短に言えば、それぞれのクローンの単一のコロニーを、50μg/mlおよび10μg/mlでそれぞれアンピシリンおよびテトラサイクリンを含む10mlのSB培地に植菌した。一晩37℃で培養された培養物を、前と同じ濃度の抗生物質を加えた500mlのSBを有するフラスコに再び植菌した。次に1mMのIPTGにより誘導された細胞を、一晩30℃で撹拌しながら放置した。培養物を5000rpmで25分遠心し、PBSに再懸濁されたペレットを超音波処理した。25分間18,000rpmでさらなる遠心分離を行うことが、細胞残屑を除去するために必要であった。上清を濾過し、次に上記セファロースカラムをゆっくり通過させた。その後、樹脂を10倍容量のPBSで洗浄し、最後に結合したFabを酸性溶液(溶出バッファー−H2O/HCl pH2,2)で溶離した。回収した種々の画分を適当な溶液(1MのTris pH9)で中和し、限外濾過(Centricon、Millipore)により濃縮した。12%のポリアクリルアミド/ドデシル硫酸ナトリウムゲル(SDS−PAGE)にある一定量を泳動することにより、精製されたFabの純度を評価した。最後に、精製されたFabの連続希釈物を記載されているとおりのELISAによりアッセイした。それぞれのプレートの中に、HCV E2糖タンパク質に対する精製モノクローナルFabの製造物を陰性対照として含ませた。この実験の結果は細菌溶解物で以前に得られた結果で確認した。
ELISAにより得られたデータを確認するために、交差反応性抗インフルエンザFabも免疫蛍光アッセイにより分析した。簡潔には、感染培養物由来の細胞をトリプシン処理し、PBSで2回洗浄後、顕微鏡下で血球計数器でカウントした。次に細胞懸濁液を細胞遠心機(Cytospin4、Shandon Southern Products)中で90gで3分の遠心分離によってスライドの製造のために使用した。そうして製造されたスライドは、それぞれ計2×105細胞を含んだ。対照スライドを同様に非感染細胞で製造した。次に細胞を固定し、10分間室温でメタノール−アセトン溶液(−20℃の温度で使用される)にて透過可能にさせた。PBSで3回洗浄後、細胞を30分37℃で湿チャンバー中で異なるクローン(100μg/ml)とインキュベートし、次にPBSで3回洗浄した。次に細胞を、エバンスブルーで1:200希釈されたフルオレセインイソチオシアネート結合ヤギFab(Sigma)と、30分37℃で湿チャンバー中で暗下で、インキュベートした。スライドを蛍光顕微鏡(Olympus)下で検査した。M1インフルエンザウイルスタンパク質に特異的な市販のマウスモノクローナル(Argene)を陽性対照として使用した。C型肝炎ウイルスのE2糖タンパク質に対する抗体(e509; Burioni et al, Hepatology, 1998)を陰性対照として使用した。すべての組換えFabが、株A/PR/8/34(H1N1)に感染した細胞および株A/PC/1/73(H3N2)に感染した細胞の両方に対して特異的である反応性を、免疫蛍光により示した。代わりに、蛍光は非感染細胞、B型株−感染細胞または陰性対照抗体に感染した細胞において見られなかった。
選択されたクローンのインビトロ生物学的活性を特徴付けるため、試験において使用される3つの参照ウイルス分離株に対して中和アッセイを設計した。手短に言えば、MDCK細胞を96ウェルプレート中のMEM−10%FBSに播種した(2×104細胞/ウェル)。上記のとおりに得られたウイルス貯蔵物の連続希釈物(10−1から10−8)を、維持培地(2%のFBSを有するMEM)中で製造した。それぞれの希釈を6回繰り返した。培養細胞がコンフルエントしたとき、培養培地を除去し、100μlのそれぞれのウイルス希釈物をそれぞれのウェルに加えた。37℃で1時間後、播種物を除去し、1μg/mlのトリプシンを有する200μlのMEM培地をそれぞれのウェルに入れた。TCID50(細胞培養物の50%を感染する量)として表されるウイルス価をReed−Muenchの式:
前記のとおりに製造された株A/PR/8/34(H1N1)に感染した10μgの細胞溶解物を、10%のポリアクリルアミドゲル上を非変性条件下で泳動した。この目的のために、サンプルを適当な冷蔵タンク(BIORAD)中で1時間100Vで泳動した。その後、ゲルを電気泳動装置から取り出し、すべてのデタージェントの残りを除去するためにTransfer Buffer(トリス塩基 3g;グリシン 14.41g、dH2O 800ml、メタノール 200ml)中で10分インキュベートした。次にニトロセルロース膜(Hybond-ECL; Amersham Biosciences)への移動を30Vおよび90mAで一晩行った。次に膜を1×PBS中に溶解した5%の粉ミルクで1時間ブロックし、その後1×PBS−0.1%Tween中で3回洗浄した。それぞれの洗浄中、膜を揺れているプラットホーム上で10分振とうし続けた。この後、5%の粉ミルクを有するPBSで希釈した異なるFabを、5μg/mlの濃度で加えた。Fab28のほかに、以下の対照を加えた:陰性対照としてe509;市販のマウス抗HA全IgG1(COVANCE);市販のマウス抗M1全IgG1(ARGENE);マウス抗M2全IgG1(ABCAM);1:200に希釈されたヒト血清。それぞれの抗体を室温で1時間撹拌し続けた。その後、膜を前記のとおりにPBSで再び洗浄した。次に、検出される抗体の源に依存して、ELISAアッセイに対して記載されているものと同じマウス(1:1000)またはヒト(1:2000)二次抗体を加えた。シグナルの検出のために、光源に暴露しないように特に注意深く、1:1比で2つの基質(SuperSignal(登録商標) West Pico Chemiluminescent Substrate Pierce)を混合することにより、希釈標準溶液を製造した。ニトロセルロース膜を希釈標準溶液と5分インキュベートし、次に取り出し、HyperCassette(AMERSHAM)にのせた。必要な暴露時間後に、これをKodak X線フィルムに暗室下で現像した。記載されているアッセイを2つの異なる時間に行い、それぞれにおいてFab28とインキュベートした膜部分は、ウイルス血球凝集素(HA0)の未熟型の重量と一致する80KDaより少し小さい重量のバンドの存在を示した。これは抗血球凝集素対照抗体とインキュベートした細長い一片で表されている同じバンドによっても確認された。他のものよりも非常に強度の類似のバンドも、ヒト血清とインキュベートした膜部分において検出された。この実験の結果は、血球凝集素が免疫中和抗体応答の標的であることが知られているため、中和データと完全に一致して、抗体がインフルエンザウイルス血球凝集素に対することを示した。
Fab28の中和活性をより正確に評価するために、中和アッセイをプラークアッセイ技術を使用することにより実施した。最初に、ウイルス分離株、サブタイプH1N1およびH3N2の製造は、以下のプロトコールにてプラークアッセイにより定量した。MDCK細胞を、6ウェルプレート(Costar)にてペニシリンおよびストレプトマイシン(pen/strep)を補い、10%のウシ胎児血清(FBS)を含むMEM培地中で培養した。細胞単層が100%のコンフルエントに達した後、ウェルをPBSで洗浄し、同じ抗生物質(pen/strep)およびトリプシン(1μg/ml)を補った新鮮なMEM培養培地をそれに加えた。ウイルス貯蔵物の連続希釈物を同じウェル中で作り、ウイルスを34℃で5%CO2雰囲気下で1時間吸着させるために放置した。次に培地を吸引し、PBSで2回洗浄した。さらに、抗生物質、トリプシン(1μg/ml)および0.8%アガロースを補ったMEMを、42℃を越えない温度で穏やかに加えた。感染後、細胞単層の健康状態を位相差光学顕微鏡下でチュックし、プレートを34℃で5%CO2雰囲気下でインキュベートした。感染48時間後、細胞単層に損傷を与えないように非常に注意深く、アガロース層を取り出す。その後、クリスタル・バイオレット(1%w/v)を加えられた70%のメタノール水溶液を、ウェルに加えた。プレートを透過性物質/色素と室温で5分インキュベートした。インキュベーション後、プレートを室温にて蒸留水で洗浄し、層流下で5分乾燥させるために放置した。最後に、PFU(プラーク形成単位)数を位相差顕微鏡下で4×大きさで評価した。ウイルス力価をPFUとして計算し、対応するTCID50を計算し、同じ力価を終点限界希釈技術により類似のウイルス貯蔵物の力価と比較した。

いくつかの手段を実施し、立体構造的エピトープであるが、エピトープを認識する能力が以前の実験によりすでに示されている、Fab28により認識される血球凝集素領域を同定した。確かに、Fab28は、準自然条件下(0.1%のSDS)で実施したウエスタンブロットアッセイにおいて、該タンパク質のみを認識することができた。同じ実験も、個々のサブユニット(HA1およびHA2)ではなくタンパク質(HA0)の未熟型のみを認識するFabの能力を示した。血球凝集阻害アッセイ(HAI)を、ニワトリ赤血球およびヒト赤血球の両方で同時に実施した。顕著な中和活性にもかかわらず、Fab28はHAI活性が無いことを証明し、血球凝集素およびシアル酸間の結合に関与する残基に結合しないことを示唆した。
Claims (15)
- インフルエンザA型ウイルスの複数のサブタイプに結合し、中和することができることを特徴とする、インフルエンザA型ウイルスの血球凝集素抗原に対するヒトモノクローナル抗体であって、該インフルエンザA型ウイルスの複数のサブタイプが、少なくとも、血球凝集素H1を含む1つのインフルエンザA型ウイルスのサブタイプおよび血球凝集素H3を含む1つのインフルエンザA型ウイルスのサブタイプを含み、該モノクローナル抗体は、少なくとも1つの重鎖可変ドメインおよび1つの軽鎖可変ドメインを含み、重鎖可変ドメインは配列番号:1のアミノ酸配列を有し、そして軽鎖可変ドメインは配列番号:2のアミノ酸配列を有する、モノクローナル抗体。
- 少なくとも1つの重鎖可変ドメインおよび1つの軽鎖可変ドメインを含み、重鎖可変ドメインは配列番号:3の核酸配列によってコードされており、そして軽鎖可変ドメインは配列番号:4の核酸配列によってコードされている、請求項1に記載のモノクローナル抗体。
- 請求項1に記載のモノクローナル抗体により特異的に認識される血球凝集素の立体構造的エピトープに結合することができる、モノクローナル抗体。
- 少なくとも1つの重鎖可変ドメインおよび1つの軽鎖可変ドメインを含む全免疫グロブリンおよび免疫グロブリンフラグメントからなる群から選択される、請求項1から3のいずれかに記載のモノクローナル抗体。
- 該免疫グロブリンフラグメントがFabフラグメント、Fab’フラグメント、F(ab’)2フラグメント、Fvフラグメント、一本鎖抗体(scFv)の群から選択される、請求項4に記載のモノクローナル抗体。
- 請求項1から5のいずれかに記載のモノクローナル抗体のイディオタイプに対して特異的である抗イディオタイプ抗体。
- 配列番号:3および配列番号:4の核酸配列を含む発現ベクター。
- 請求項7に記載の発現ベクターにより形質転換された宿主細胞。
- 請求項1から6のいずれかに記載の有効量の少なくとも1つの抗体および薬学的に許容される担体および/または希釈剤を含む医薬組成物。
- インフルエンザA型ウイルス感染またはインフルエンザA型ウイルス感染によって直接的もしくは間接的に引き起こされる病状の予防用または治療用医薬としての請求項1から6のいずれかに記載の抗体。
- インフルエンザA型ウイルス感染またはインフルエンザA型ウイルス感染によって直接的もしくは間接的に引き起こされる病状の予防用または治療用医薬の製造のための、請求項1から6のいずれかに記載の抗体の使用。
- インフルエンザA型ウイルス感染によって引き起こされる病状がインフルエンザ症候群である、請求項11に記載の使用。
- 患者由来の生物学的サンプルにおいて、ヘテロサブタイプの交差中和特性を有する抗インフルエンザウイルス抗体の存在を検出するためのアッセイ方法であって、該生物学的サンプルと特定のアッセイ試薬として請求項1から5のいずれかに記載のモノクローナル抗体を接触させることを含む方法。
- 特定の試薬として請求項1から5のいずれかに記載のモノクローナル抗体を含む診断キットであって、患者から得られた生物学的サンプルにおいて、ヘテロサブタイプの交差中和特性を有する抗インフルエンザA型ウイルス抗体を検出または定量するための方法において使用するために特に設計されたキット。
- 免疫原性またはワクチン組成物において、投与される対象において、インフルエンザA型ウイルスに対するヘテロサブタイプの交差中和特性を有する抗インフルエンザA型ウイルス抗体を誘発することができるインフルエンザA型ウイルスのエピトープの存在を検出するためのアッセイ方法であって、該組成物と特定のアッセイ試薬として請求項1から5のいずれかに記載のモノクローナル抗体をインビトロで接触させることを含む方法。
