Description
本発明によるステントグラフトは、従来技術ではかつて適用されていなかったさまざまな特徴を提供するものであり、それによって、大動脈の自然な経路または疾患のある経路内で一層効率的に植込み/確認をし、血管穿刺の尤度を減らし、血液漏れのない血管接続性を高め、グラフトが動く可能性を減らす血管修復装置を提供する。
上述したように、従来技術は、プロテーゼ、特にステントグラフトを血管まで腔内送達するのに多くの異なるシステムを含む。多くのデリバリーシステムは似たような部品を有し、ほとんどが一般にデリバリーシステムの使用前に患者の鼠径部付近の大腿動脈に挿入することで挿入されるガイドワイヤに沿って案内される。大動脈につながる動脈(大動脈を含む)の穿刺を防ぐために、デリバリーシステムはガイドワイヤと同軸的に接続され、ガイドワイヤの経路を大動脈まで追跡する。したがって、ワイヤの上を追跡するデリバリーシステムの部品は、外径が患者の大腿動脈の内径よりも小さくなる大きさにされている。ガイドワイヤの上を追跡するデリバリーシステムの構成要素としては、ステントグラフトがあげられ、カテーテルおよびシースと呼ばれる一連の同軸内腔で作られている。ステントグラフトは一般に、ステントグラフトを圧縮して外側のカテーテルの内側にフィットさせるための外側のカテーテルによって抑制される。それによって、デリバリーシステムのステントグラフトを抑制する部分の剛性が非常に高くなり、その部分の可撓性が落ちてデリバリーシステムがガイドワイヤの上、特に大動脈弓などの曲がった血管に沿って追跡するのが困難になる。また、圧縮されてカテーテルの内側にフィットしなければならない量がゆえに、ステントグラフトは抑制用カテーテルに極めて高い半径方向の力を加えることになるため、抑制用カテーテルをステントグラフトから離して摺動させてステントグラフトを展開するプロセスには、一般に展開力と呼ばれる極めて大きな力が必要になる。また、カテーテルは、グラフトを抑制できるだけの十分な強さのものでなければならず、剛性の材料で作られる必要がある。大動脈弓を追跡するなどで剛性の材料が曲がると、この剛性の材料はよじれやすく、ステントグラフトを展開するのが不可能ではないにしても困難になる。
上述したように、裸ステント30は、近位端12で外方向に拡張する力を提供する。裸ステント30および近位側のステント23は、グラフトスリーブ10に取り付けられているときは圧縮状態にあり、グラフトスリーブ10に対しても外方向に拡張する力を加える。したがって、植込まれると、これらの力によってステントグラフト1の近位端が血管内で中央におかれ、グラフトスリーブ10を血管壁に当接させてグラフトスリーブ10と血管壁との間で生じ得る漏れを防止する。このような近位端12での漏れは、ステントグラフト植込みの際に回避されるべきものである。
Claims (16)
- 近位頂部を含む近位裸ステントと遠位頂部を含む遠位裸ステントとを有するプロテーゼを送達および展開するための送達システムであって、前記送達システムは、
近位端と、遠位端と、長手方向の軸に沿って延在する内側とを有する第1の中空の管腔、
近位端と、遠位端を有し、前記第1の管腔の周囲に摺動可能に設けられた第2の中空の管腔、
遠位端を有し、前記第2の管腔の周囲に摺動可能に設けられた中空の頂部解放管腔、
遠位頂部捕捉装置、ここで、前記遠位頂部捕捉装置は、
前記第1の管腔の前記遠位端に固定的に接続される第1の遠位捕捉部分、ここで、前記第1の遠位捕捉部分は、
第1の近位端を有する第1の遠位部分と、
前記長手方向の軸に対してほぼ垂直な面に設けられた遠位側を有する第1の近位ストッパ、ここで、前記第1の近位ストッパは、前記第1の遠位部分の前記近位端から近位距離で設けられ、第1の停止表面部分を画定する、
を有する、および
前記第2の管腔の前記遠位端に固定的に接続される第1の近位捕捉部分、ここで、前記第1の近位捕捉部分は、前記第1の遠位捕捉部分に向かって遠位方向に延在するフィンガーを有し、前記フィンガーは、前記第1の近位ストッパを収容するよう形成された第1の内部キャビティを画定し、前記近位捕捉部分および前記遠位捕捉部分が、その間に前記近位頂部を保持するよう形成され、一緒に、前記第1の近位ストッパが前記第1の内部キャビティに収容され、前記第1の遠位捕捉部分が前記長手方向の軸に沿って前記第1の内部キャビティをほぼ閉じる第1のステント捕捉状態を画定する、

前記頂部解放管腔の前記遠位端に固定的に接続される第2の近位捕捉部分、ここで、前記第2の近位捕捉部分は、前記第2の遠位捕捉部分に向かって遠位方向に延在するフィンガーを有し、前記フィンガーは、前記第2の近位ストッパを収容するよう形成された第2の内部キャビティを画定し、前記第2の遠位捕捉部分および前記第2の近位捕捉部分が、その間に遠位頂部を保持するよう形成され、一緒に、前記第2の近位ストッパが前記第2の内部キャビティに収容され、前記第2の近位捕捉部分が前記長手方向の軸に沿って前記第2の内部キャビティをほぼ閉じる第2のステント捕捉状態を画定する、

前記第2の近位捕捉部分と前記第2の遠位捕捉部分が、前記第2のステント頂部キャビティに遠位頂部を保持するよう形成される、請求項1に記載の送達システム。 - 前記第1および第2の管腔の前記遠位端が互いに隣接して移動する際に前記第1のステント捕捉状態が存在し、
前記第2の管腔および前記頂部解放管腔が互いに隣接して移動する際に前記第2のステント捕捉状態が存在する、請求項1に記載の送達システム。 - 前記第1の遠位部分の前記近位端と前記第1の近位ストッパが、第1のステント頂部キャビティを画定し、
前記第1の近位捕捉部分と前記第1の遠位捕捉部分が、前記第1のステント頂部キャビティに、近位裸ステントの近位頂部を保持するよう形成され、
前記第1および第2の管腔の相対移動によって、前記第1の近位捕捉部分と前記第1の遠位捕捉部分が互いに対して移動して、前記第1のステント頂部キャビティを実質的に閉じ、これによって前記第1の近位捕捉部分および前記第1の遠位捕捉部分を前記第1のステント捕捉状態におく、請求項1に記載の送達システム。 - 前記第2の遠位部分の前記近位端と前記第2の近位ストッパが、第2のステント頂部キャビティを画定し、
前記第2の近位捕捉部分と前記第2の遠位捕捉部分が、前記第2のステント頂部キャビティに、遠位裸ステントの遠位頂部を保持するよう形成され、
前記第2の管腔および前記頂部解放管腔の相対移動によって、前記第2の近位捕捉部分と前記第2の遠位捕捉部分が互いに対して移動して、前記第2のステント頂部キャビティを実質的に閉じ、これによって前記第2の近位捕捉部分および前記第2の遠位捕捉部分を前記第2のステント捕捉状態におく、請求項1に記載の送達システム。 - 前記第1の管腔の前記近位端を前記第2の管腔の前記近位端に作動可能に接続する頂部解放アセンブリをさらに含む、請求項1に記載の送達システム。
- 前記頂部解放アセンブリが、
前記第1の管腔の前記近位端と接続された近位解放部と、
前記第2の管腔の前記近位端と接続された遠位解放部と、
前記遠位解放部に対して前記近位解放部を選択的に係止することで、前記第1の管腔を前記長手方向の軸に沿って前記第2の管腔に選択的に固定する係止部と、
を有する、請求項7に記載の送達システム。 - 頂部解放アセンブリをさらに含み、ここで、前記頂部解放アセンブリは、
前記第1の管腔の前記近位端と接続された近位解放部と、
前記第2の管腔の前記近位端と接続された遠位解放部と、
前記遠位解放部と前記近位解放部の間の中間円板と、
2つの着脱可能な中間部、ここで、1つの着脱可能な中間部は、前記遠位解放部と前記中間円板の間に着脱可能に挿入され、もう1つの着脱可能な中間部は、前記中間円板と前記近位解放部の間に着脱可能に挿入され、前記遠位解放部および前記近位解放部が互いに対して移動すると、前記第1の遠位捕捉部分および前記第1の近位捕捉部分の互いに対する対応する移動が発生し、これによって近位頂部を前記第1の内部キャビティで解放または捕捉する、
を有する、請求項1に記載の送達システム。 - 前記遠位解放部と前記近位解放部を分離する移動によって、前記第1の遠位捕捉部分および前記第1の近位捕捉部分の互いに対しての対応する移動が発生し、これによって前記第1の近位捕捉部分の前記フィンガーの前記第1の内部キャビティを実質的に閉じ、
前記遠位解放部と前記近位解放部を隣接させる移動によって、前記第1の遠位捕捉部分および前記第1の近位捕捉部分の互いに対しての対応する移動が発生し、これによって前記第1の近位捕捉部分の前記フィンガーの前記第1の内部キャビティが開放される、請求項9に記載の送達システム。 - 前記遠位解放部と前記近位解放部を分離する移動によって、前記第1の遠位捕捉部分および前記第1の近位捕捉部分の互いに対しての対応する移動が発生し、これによって近位頂部を捕捉し、
前記遠位解放部と前記近位解放部を隣接させる移動によって、前記第1の遠位捕捉部分および前記第1の近位捕捉部分の互いに対しての対応する移動が発生し、これによって近位頂部を解放する、請求項9に記載の送達システム。 - 前記係止部が、レバーおよびクリップのうちの1つである、請求項8に記載の送達システム。
- 展開対象となるステントが、近位裸ステントの所定数の近位頂部と遠位裸ステントの所定数の遠位頂部とを有し、
前記近位頂部捕捉装置の前記フィンガーの数が、遠位裸ステントの遠位頂部の数と等しく、
前記遠位頂部捕捉装置の前記フィンガーの数が、近位裸ステントの近位頂部の数と等しい、請求項1に記載の送達システム。 - 前記第2の近位ストッパが、前記長手方向の軸から離れて半径方向に延在する伸長部を有し、
前記第2の近位ストッパの前記伸長部の数が、前記第2の停止表面部分の数と等しく、
前記伸長部が各々、前記第2の停止表面部分の1つを含む、請求項1に記載の送達システム。 - 前記第2の遠位捕捉部分が、前記第2の遠位部分と前記第2の近位ストッパとを、前記第2の遠位部分の前記近位端から近位距離で接続する中間部分を有する、請求項1に記載の送達システム。
- 前記第1の遠位捕捉部分が、前記第1の遠位部分を前記第1の近位ストッパに接続する中間部分を有する、請求項1に記載の送達システム。
