Description
本出願は、2002年12月26日に提出された同時係属中の米国仮特許出願60/343,814号に基づくものである。
本発明は血管への到達に関する。より具体的には、本発明は、従来の同様な器具に比べてより容易、より安全でより迅速に使用できる血管到達具に関する。
血管内にカテーテル又は血管鞘を挿入する好適な非外科的方法は、患者の血管内に挿入される到達針を含むセルディンガー技術の使用を含む。案内ワイヤを針を通じて血管内に挿入する。針を取り除いた後、拡張具及び鞘の組合せをこの案内ワイヤ上に挿入する。次に、前記拡張具及び鞘の組合せを、組織を貫通させて短い距離、血管内に挿入し、その後、拡張具を取り出して廃棄する。次にカテーテルをこの鞘を通じて血管内に所望の位置まで挿入する。
本発明の目的の1つは血管への到達を提供することである。
本発明によれば、拡張具又は鞘及び拡張具を針に取り付けるが、この針は、針の遠位先端に近位に横方向の開口を有する。挿入前に、針の遠位端が拡張具の遠位端に対して遠位になるように、針を配置する。針を血管内に挿入後、この針の開口により、拡張具内、又は、針と拡張具との間の環状空間に血液が流れることができるため、針の遠位端が血管を穿孔したことを示すことができる。加えて、又は代替的に、血液が、鞘の形成した環状空間内へ流れることで、血管が穿孔されたことを示す。さらに、到達がなされたことの指標をさらに提供するために、血液を選択的な側面ポートに流れさせてもよい。
患者の血管系に達することがある。このような時は案内ワイヤを外科的に取り除くか、又は、皮膚を通して回収しなくてはならない。
本発明は、おそらく図面からより良好に理解されるであろう。図1及び2において、針部分102は、針104と、回転固定部材108を備えた近位部分106とを備え、そして拡張具部分112は、拡張具114及びハブ116を備える。本発明のある好適な実施態様では、針104は、一つ以上の開口120を有する。拡張具114は、好ましくは、血液が針104に、そして次に開口120を通じて(1)針104と拡張具114との間の環状空間122へ、又は、(2)拡張具114内の空間(図示せず)内へ、又は拡張具114内の空間を通じて流れたときに担当医がこの血液を見られるように、透明、半不透明又は半透明であるとよい。これは、針104の遠位端124が血管(図示せず)を穿孔したことを担当医に指し示すであろう。
Claims (43)
- 側面孔と、前記側面孔に対して遠位にある尖った遠位端を規定する中空の針と、
固定部分を含む近位針部分と、を含む針部と、
前記針部に予め装填された案内ワイヤと、
が、
前記針に対して同軸に実装された拡張具と、
前記針の固定部分と相互作用するように構成された近位の固定部分と、を含む拡張具部と、
前記拡張具に同軸に実装された鞘を含む鞘部分と、
を含む血管到達デバイスであって、
血液を受け取ることができる少なくとも1つの空間が、前記鞘と前記針の間に生じ、前記空間が、前記拡張具および/または鞘を通して前記空間内の血液を観察することができるように可視化されていてもよく、
前記拡張具部分と鞘部分の少なくとも1つは、血液が流入したときに空気または気体を前記空間から排出し、前記血液が保持されるようした、
1つ以上の小型開口、孔、または多孔質材料を有する、
血管到達デバイス。 - 前記鞘が剥ぎ取り鞘である、請求項1記載のデバイス。
- 前記鞘がカテーテルを含む、請求項1記載のデバイス。
- 前記カテーテルがマルチルーメンカテーテルである、請求項3記載のデバイス。
- 前記拡張具および鞘を、前記針上を摺動可能に一緒にずらすことができる、請求項1記載のデバイス。

- 前記拡張具が、少なくとも1つの長手部材によって中実の近位部分に接続された中実の遠位部分を有する不連続構造を有し、それによって前記空間が前記鞘と前記針の間に規定され、前記空間が前記針の側面孔を通して前記中空の針の穴と連絡している、請求項1記載のデバイス。

- 前記中空の針の前記尖った遠位端が、前記拡張具から遠位に突出して、患者の中枢血管に到達するようにした、請求項13記載のデバイス。
- 前記中空の針の前記尖った遠位端が、前記拡張具から遠位に突出して、患者の抹消血管に到達するようにした、請求項13記載のデバイス。
- 前記中空の針が剛性を有し、それによって前記デバイスが血管中を進む間に前記拡張具および鞘を支持するようにした、請求項1記載のデバイス。
- 前記拡張具が拡張具の側面孔を規定し、前記空間が前記拡張具と針の間の第2の空間と前記拡張具の側面孔を通して連絡している、請求項1記載のデバイス。
- 拡張具は少なくとも部分的に半不透明であり、前記鞘は少なくとも部分的に透明または半透明である、請求項1記載のデバイス。
